Welcome to "Wonderfully Made" 00:00:34.05\00:00:35.42 I'm Amy Wellard with the Wildwood Lifestyle Center 00:00:35.45\00:00:38.44 and today we're talking about 00:00:38.47\00:00:40.14 labor and delivery of your precious baby. 00:00:40.17\00:00:43.62 With me in the studio is Dr. Cherie Lou Fernandez 00:00:43.65\00:00:46.29 Welcome Dr. Cherie Lou Thank you 00:00:46.32\00:00:48.50 It's an exciting time, isn't it? 00:00:48.53\00:00:50.58 The baby is about to come... 00:00:50.61\00:00:52.13 You've got mixed feelings of perhaps a little fear 00:00:52.16\00:00:55.01 and trepidation about what could happen 00:00:55.04\00:00:57.24 because the unpredictable is possible. 00:00:57.27\00:01:00.05 So what are the BEST ways to prepare yourself for the birth? 00:01:00.08\00:01:03.86 Always preparation comes BEFORE, not just on the event, per se 00:01:03.89\00:01:10.11 So it is very important for you to think who you are going to 00:01:10.14\00:01:14.73 trust to deliver your baby, and not only on time, 00:01:14.76\00:01:18.16 but you would have thought about it even previous 00:01:18.19\00:01:20.48 when you were going to have your prenatal checkups 00:01:20.51\00:01:23.47 and everything and follow-ups. 00:01:23.50\00:01:25.03 You would have already then have chosen who is 00:01:25.06\00:01:27.75 supposed to deliver your baby... 00:01:27.78\00:01:29.42 Whether you would want a doctor to deliver your baby 00:01:29.45\00:01:35.22 or a nurse midwife or a midwife or a doula or whatever, 00:01:35.25\00:01:39.43 you would be thinking of this already in advance. 00:01:39.46\00:01:42.47 It's very important because you need to have a BOND 00:01:42.50\00:01:45.60 with the person who is going to deliver your baby. 00:01:45.63\00:01:48.05 You have to TRUST, learn to trust... 00:01:48.08\00:01:50.20 Trust doesn't just come like split second, you know, 00:01:50.23\00:01:53.80 it has to be developed. 00:01:53.83\00:01:55.33 So you have to choose the person 00:01:55.36\00:01:57.41 you are going to put your trust in. 00:01:57.44\00:01:59.67 And it's important, as well, that that health care provider 00:01:59.70\00:02:03.06 knows your history so they know what 00:02:03.09\00:02:04.96 possible complications could arise... Yes 00:02:04.99\00:02:07.78 And what are the different options in front of the 00:02:07.81\00:02:09.81 pregnant lady in terms of hospital, birthing center... 00:02:09.84\00:02:12.87 What's the difference between those? 00:02:12.90\00:02:14.51 Oh, there is a lot of things going around like 00:02:14.54\00:02:17.38 home births versus birthing center births, 00:02:17.41\00:02:21.75 versus hospital births. 00:02:21.78\00:02:24.16 Well in the olden days, when the hospitals weren't there yet, 00:02:24.19\00:02:29.36 of course we can say that there were a lot of mortalities 00:02:29.39\00:02:34.03 from delivering at home. 00:02:34.06\00:02:35.46 It's just probably because people didn't know 00:02:35.49\00:02:37.51 how to do delivery well and they didn't have enough 00:02:37.54\00:02:41.85 clean things to use... 00:02:41.88\00:02:43.56 So women were dying from delivery infections, whatever, 00:02:43.59\00:02:47.58 because we just didn't know... 00:02:47.61\00:02:49.38 They just didn't know yet. 00:02:49.41\00:02:50.86 But, midwives doing home deliveries, 00:02:50.89\00:02:53.48 or nurse midwives doing home deliveries now at home, 00:02:53.51\00:02:58.36 they are fairly doing very, very well. 00:02:58.39\00:03:02.65 Delivering at home would really be the best 00:03:02.68\00:03:04.52 because you're delivering under circumstances where 00:03:04.55\00:03:07.07 you are comfortable. 00:03:07.10\00:03:10.33 Your relatives are there, your husband, your mom, 00:03:10.36\00:03:13.05 your dad, your grandfather or grandmother to help you. 00:03:13.08\00:03:16.43 The home setting is the most physiologic, if you would say... 00:03:16.46\00:03:21.12 And I'm sure the mother would have more endorphins as well 00:03:21.15\00:03:23.52 being around her loving husband. Yes, she's comfortable! 00:03:23.55\00:03:25.55 Yeah but then they want to make it a little bit more hybrid 00:03:25.58\00:03:29.04 between the hospital, which is more sterile, 00:03:29.07\00:03:31.50 and everybody is masked and you wear this gown, 00:03:31.53\00:03:36.09 and sometimes the husband is not there or nobody is there at all; 00:03:36.12\00:03:40.71 you are in a labor room or delivery room, 00:03:40.74\00:03:43.12 a very sterile environment; 00:03:43.15\00:03:44.60 then they make a half bid between that... 00:03:44.63\00:03:46.60 they have the birthing centers. 00:03:46.63\00:03:48.95 So you have to see where you would want to be, 00:03:48.98\00:03:53.81 and know the pros and the cons of each of these... 00:03:53.84\00:03:58.55 And if I'm high-risk... I'm probably going to take 00:03:58.58\00:04:01.29 the advice of my doctor to deliver in the hospital? 00:04:01.32\00:04:05.22 The high-risk pregnancies which 00:04:05.25\00:04:07.44 are those 16-year-old and below pregnancies, 00:04:07.47\00:04:12.64 and 36 and above - because the 00:04:12.67\00:04:18.07 15 below does not know what to do. 00:04:18.10\00:04:20.40 The 36 and above usually is sick, 00:04:20.43\00:04:23.17 and all the other medical conditions that might be 00:04:23.20\00:04:26.67 accompanying the pregnancy 00:04:26.70\00:04:28.37 because pregnancies can complicate already 00:04:28.40\00:04:31.62 existing diseases and existing diseases can complicate 00:04:31.65\00:04:36.25 the pregnancy - making it high-risk. 00:04:36.28\00:04:38.72 So definitely - you would choose to stay in an area where 00:04:38.75\00:04:45.34 you're easily rushed to the hospital 00:04:45.37\00:04:47.95 or already in the hospital for high-risk pregnancies. 00:04:47.98\00:04:51.11 It's like you already know you have a problem, 00:04:51.14\00:04:53.74 and you choose to stay in an area where you live 00:04:53.77\00:04:57.76 7 miles or 10 miles away from the hospital - 00:04:57.79\00:05:00.69 that's going to be hard. 00:05:00.72\00:05:02.47 And just a word to our viewers, 00:05:02.50\00:05:04.23 Dr. Cherie Lou is not suggesting that just because you are 00:05:04.26\00:05:07.33 over 36- means you HAVE to have a hospital delivery. 00:05:07.36\00:05:10.24 Of course, there are MANY women that do deliver 00:05:10.27\00:05:12.45 over that age - myself is one of them... 00:05:12.48\00:05:15.60 And you can deliver at home, 00:05:15.63\00:05:17.00 you don't HAVE to go to a hospital... That's right 00:05:17.03\00:05:18.92 So with prayerful consideration, the Lord will guide 00:05:18.95\00:05:21.92 every decision which is very 00:05:21.95\00:05:23.59 exciting and comforting, isn't it? Um hm 00:05:23.62\00:05:25.35 So what are the physical preparations 00:05:25.38\00:05:27.61 that we should make... some of them 00:05:27.64\00:05:29.59 Many women... it comes by in cultures. 00:05:29.62\00:05:34.36 Like for example, MY culture, what I grew up... 00:05:34.39\00:05:37.96 Women are SO sheltered... 00:05:37.99\00:05:41.78 and don't carry this, don't do that... 00:05:41.81\00:05:44.76 You can't bend over, you can't sit this way, 00:05:44.79\00:05:47.49 you can't walk so far and the woman is so restricted 00:05:47.52\00:05:52.56 and she doesn't get enough physical activity 00:05:52.59\00:05:55.92 which is REALLY important. 00:05:55.95\00:05:57.61 Again I say, we go back and say, "pregnancy is not a disease" 00:05:57.64\00:06:02.00 ...therefore, it's NOT supposed to limit your activity. 00:06:02.03\00:06:07.38 If you're a sprinter, if you can still sprint if you're pregnant, 00:06:07.41\00:06:11.79 as long as the pregnancy doesn't get in the way... 00:06:11.82\00:06:14.52 That's just an exaggeration but you know where I'm getting at, 00:06:14.55\00:06:18.51 but exercise is very much important, 00:06:18.54\00:06:23.28 as the pregnancy becomes bigger, as the baby becomes bigger, 00:06:23.31\00:06:27.18 the angle or the curve in your back changes, 00:06:27.21\00:06:29.92 and this causes the muscles in your lower back to go into spasm 00:06:29.95\00:06:34.32 And so, many times this pregnant woman would be complaining of 00:06:34.35\00:06:37.33 lower back pain, so then she could be exercising 00:06:37.36\00:06:40.72 There are exercises... you can go to a lot of books 00:06:40.75\00:06:43.99 that they have for exercising the back 00:06:44.02\00:06:48.20 and the other muscles of your body; 00:06:48.23\00:06:50.05 also exercising to enhance endurance. 00:06:50.08\00:06:55.12 You're going to breathe the biggest breath in your life. 00:06:55.15\00:06:59.24 You are going to PUSH the strongest PUSH in your life. 00:06:59.27\00:07:02.61 You have to learn how to make your chest... 00:07:02.64\00:07:06.28 against this BIG tummy here pressing on your chest... 00:07:06.31\00:07:11.03 You have to learn how to take deep breaths and psych yourself 00:07:11.06\00:07:15.07 I always teach my patients to pretend that the pain is coming, 00:07:15.10\00:07:19.98 the pain is there. 00:07:20.01\00:07:21.42 So as she is exercising, maybe walking, 00:07:21.45\00:07:25.03 and then she imagines in her mind... the pain is coming 00:07:25.06\00:07:29.07 The pain comes every 3 minutes, or every 2-3 minutes, 00:07:29.10\00:07:32.15 then it becomes ooOHH and it build up and it builds up. 00:07:32.18\00:07:35.70 When it starts building up like this, 00:07:35.73\00:07:37.51 you're starting to take deep breaths... okay DEEP breaths, 00:07:37.54\00:07:40.81 and then when it's painful, you take your BIGGEST breath, 00:07:40.84\00:07:44.31 and then hold it. 00:07:44.34\00:07:45.71 Of course, while you're not yet in labor, you don't push. 00:07:45.74\00:07:48.63 But you're fixing your head, it's just like the Lamaze 00:07:48.66\00:07:53.27 or the Bradley thing... 00:07:53.30\00:07:54.67 They TEACH you to... it's like a conditioning. 00:07:54.70\00:07:57.23 If you want to, if you have the means to go to a Lamaze class, 00:07:57.26\00:08:01.43 or a Bradley, it's up to you, but if you don't have the 00:08:01.46\00:08:05.63 means to do it and you would want to do it at home, 00:08:05.67\00:08:09.81 you could always teach yourself. 00:08:09.84\00:08:11.21 Again, always refer back to the times when, 00:08:11.24\00:08:15.20 even at present when we have those people in the tribes. 00:08:15.23\00:08:19.80 I think the important think is 00:08:19.83\00:08:21.41 that you're psychologically prepared in every way, 00:08:21.44\00:08:25.07 and let's talk about fear because 00:08:25.10\00:08:26.72 fear can be such a giant as you go especially 00:08:26.75\00:08:30.03 for the first-time mom. 00:08:30.06\00:08:31.65 You're going into labor, you really don't know 00:08:31.68\00:08:33.38 what to expect because you realize anything could happen 00:08:33.41\00:08:35.90 because the books tell you, really, anything CAN happen. 00:08:35.93\00:08:38.21 So how can we prepare to face that one? 00:08:38.24\00:08:41.07 The most... I mean at least for me, I wasn't afraid 00:08:41.10\00:08:45.65 WHAT my baby would become. 00:08:45.68\00:08:49.27 Maybe some people would say - 00:08:49.30\00:08:50.68 "What if my baby is abnormal," whatever, etcetera... 00:08:50.71\00:08:53.49 I didn't think about those things. 00:08:53.52\00:08:55.12 ...You know, I didn't think, but what was really 00:08:55.15\00:08:58.25 in my heart was... 00:08:58.28\00:09:00.04 "It's going to be painful, it's really going to be painful, 00:09:00.07\00:09:03.46 it's going to be painful" because you see 00:09:03.49\00:09:04.96 I deliver all the time and I see these screaming 00:09:04.99\00:09:08.08 and writhing women, you know, because they're not prepared... 00:09:08.11\00:09:11.78 But then so, you have to understand that this pain is 00:09:11.81\00:09:16.71 necessary. 00:09:16.74\00:09:18.11 It is NOT an enemy; it is supposed to be your friend 00:09:18.14\00:09:23.57 When it comes - because without the pain, you won't deliver. 00:09:23.60\00:09:29.19 It's a Bible verse - "In pain you will bring forth 00:09:29.22\00:09:34.10 this offspring you have. " 00:09:34.13\00:09:36.17 So there is a pain, but it is helpful. 00:09:36.20\00:09:39.16 Without the pain, you won't deliver. 00:09:39.19\00:09:40.72 So, it's something that you have to fix your mind and accept 00:09:40.75\00:09:45.14 because when you are ready to accept something, 00:09:45.17\00:09:47.80 it comes faster, easier. 00:09:47.83\00:09:50.54 Now you're not saying that it would be wrong to take any 00:09:50.57\00:09:53.33 intervention so we don't feel the pain. 00:09:53.36\00:09:55.16 You're just saying the pain there is an indicator that 00:09:55.19\00:09:57.38 this is a natural part of the process, 00:09:57.41\00:09:59.60 and working with the pain enables you to 00:09:59.63\00:10:01.97 work with your body rather than fighting it... 00:10:02.00\00:10:04.02 Because if we actually go into fear over the pain, 00:10:04.05\00:10:06.49 the contractions can actually stop because of fear, 00:10:06.52\00:10:10.43 and that has been known to happen. 00:10:10.46\00:10:11.88 Many women just go ahead and have their, 00:10:11.91\00:10:14.74 what-you-call "the epidural" 00:10:14.77\00:10:16.37 They put the catheter into your back and then they 00:10:16.40\00:10:19.17 inject pain anesthesia so that you don't feel the pain 00:10:19.20\00:10:22.66 or you sort of are numbed, you can't feel the pain. 00:10:22.69\00:10:27.16 The thing is... there are statistics that will tell you 00:10:27.19\00:10:30.69 that people with epidurals have problems 00:10:30.72\00:10:33.30 in delivering their babies. 00:10:33.33\00:10:35.23 Many times it becomes a C-section, 00:10:35.26\00:10:37.57 an operative procedure that happens because 00:10:37.60\00:10:40.74 the woman can't push enough... 00:10:40.77\00:10:42.65 Because, you see, the pain is the one that makes you PUSH! 00:10:42.68\00:10:48.71 You know, when you have this 00:10:48.74\00:10:50.46 thing when you're moving your bowels, 00:10:50.49\00:10:52.70 and there is this thing there, you HAVE to push, 00:10:52.73\00:10:55.70 but this is MUCH BIGGER than your bowels. 00:10:55.73\00:10:58.41 And so, you need EXTRA triggering thing to 00:10:58.44\00:11:03.12 MOTIVATE you to push. Right 00:11:03.15\00:11:05.75 It's the pain that makes you push; 00:11:05.78\00:11:07.15 without the pain, you won't push. 00:11:07.18\00:11:09.44 So that would mean that with an epidural, 00:11:09.47\00:11:11.20 you might need some kind of inducing drug to make you... 00:11:11.23\00:11:14.56 Yes and somebody has to help you really PUSH it 00:11:14.59\00:11:17.71 with more than you would regularly think you should push. 00:11:17.74\00:11:22.20 Okay, so let's go back to the signs of labor. 00:11:22.23\00:11:25.73 I used to hear stories of women walking around the supermarket 00:11:25.76\00:11:29.10 and their waters break, suddenly they are 00:11:29.13\00:11:30.86 standing in a pool of water in an embarrassing public setting. 00:11:30.89\00:11:33.71 Is that really the first sign of labor? 00:11:33.74\00:11:35.95 That's funny because we have been so much affected 00:11:35.98\00:11:39.58 by the media... not that the media is wrong, 00:11:39.61\00:11:43.32 but we go and watch movies and you see a woman 00:11:43.35\00:11:46.80 ..."Hey honey, my BACK" and she's rushed to the hospital 00:11:46.83\00:11:51.29 and in minutes she delivers! 00:11:51.32\00:11:52.76 It's not regulated that way. 00:11:52.79\00:11:55.51 There is a process... 00:11:55.54\00:11:57.43 Of course there are some women 00:11:57.46\00:11:58.89 who just deliver... they're blessed! 00:11:58.92\00:12:02.12 But that is the exception to the rule. 00:12:02.15\00:12:05.56 I have seen patients and I have had patients who 00:12:05.59\00:12:08.49 just when they think about it, it's there. 00:12:08.52\00:12:11.26 I had a close friend who would only know she was in labor 00:12:11.29\00:12:14.28 because her baby was there already. 00:12:14.31\00:12:16.88 She didn't have any pain! 00:12:16.91\00:12:20.27 Most of us would want it that way! 00:12:20.30\00:12:22.20 Not with her.. but it doesn't happen that way. 00:12:22.23\00:12:25.31 There are steps that will prompt you to know 00:12:25.34\00:12:29.75 that you're going to deliver and it's about time. 00:12:29.78\00:12:34.22 Okay, so let's talk about that... 00:12:34.25\00:12:35.67 What would be something I can look for... 00:12:35.70\00:12:37.46 I'm 40 weeks, nothing has happened and then boom... 00:12:37.49\00:12:40.40 Okay, oh - WHAT? 00:12:40.43\00:12:43.09 You're delivering right away now! 00:12:43.12\00:12:44.54 Again, I will say it varies the process. 00:12:44.57\00:12:48.56 But you won't regularly... if we'll say, this is a 00:12:48.59\00:12:51.74 normal delivery, of course you can deliver before term... 00:12:51.77\00:12:55.03 Term means the 37th to the 42nd week, 00:12:55.06\00:13:03.28 and the average is 40 weeks. 00:13:03.31\00:13:05.11 So from 37 weeks of your pregnancy 00:13:05.14\00:13:07.60 to 42 weeks of your pregnancy - that you call "term" 00:13:07.63\00:13:11.80 If you deliver before that, that's premature, 00:13:11.83\00:13:14.80 and if you deliver after 42, that is post-mature, okay? 00:13:14.83\00:13:20.14 You would want to deliver... 00:13:20.17\00:13:21.54 Let's say - we're not talking about abnormalities, 00:13:21.57\00:13:23.31 so let's say from 37 to 42... 00:13:23.34\00:13:26.08 You will know what... You see the saying in the Bible 00:13:26.11\00:13:29.73 "... Like a thief, it comes in the night" 00:13:29.76\00:13:32.98 Your delivery of the baby is ... will come like 00:13:33.01\00:13:36.17 a woman in labor... like a thief in the night. 00:13:36.20\00:13:38.70 You don't know when it's going to come. 00:13:38.73\00:13:41.03 But again it says that you don't need to be ignorant 00:13:41.06\00:13:45.26 about when I'm going to come 00:13:45.29\00:13:46.79 because I have signs and symptoms around. 00:13:46.82\00:13:49.88 We will see the signs. 00:13:49.91\00:13:51.28 Well there are signs, the first one is 00:13:51.31\00:13:53.10 not necessarily the first one but one of them is 00:13:53.13\00:13:57.14 "lightening" 00:13:57.17\00:13:59.78 Many women would say, 00:13:59.81\00:14:01.18 "I felt my uterus go down" 00:14:01.21\00:14:03.70 It's like the head of the baby now has finally entered 00:14:03.73\00:14:08.03 the pelvic cavity. 00:14:08.06\00:14:10.12 The head is usually is floating above your pelvic brim. 00:14:10.15\00:14:14.89 Lightening means the head enters the brim... 00:14:14.92\00:14:18.27 so that when your uterus was high this way, now it goes down. 00:14:18.30\00:14:22.53 So when you have that, you can be sure that labor 00:14:22.56\00:14:25.29 is coming soon because the head is now PINCHING on your 00:14:25.32\00:14:28.45 lower uterine segment and your cervix, 00:14:28.48\00:14:31.69 and then now it's going to start 00:14:31.72\00:14:33.14 to mature and cause labor to come. 00:14:33.17\00:14:37.69 So actually, that now you mention that, 00:14:37.72\00:14:40.23 I don't remember that happening with me, 00:14:40.26\00:14:42.03 so that can happen without you 00:14:42.06\00:14:43.43 being aware of it... is that right? 00:14:43.46\00:14:44.83 Yes, many are aware but if you didn't know about it, 00:14:44.86\00:14:50.25 maybe you wouldn't notice it 00:14:50.28\00:14:52.05 because it is just something that - it goes down. 00:14:52.08\00:14:56.30 It has to before it can come out. Yes 00:14:56.33\00:14:58.50 Because the head has to go into the pelvic cavity; 00:14:58.53\00:15:01.63 however, this happens the majority of the time 00:15:01.66\00:15:04.66 ONLY in primigravid, first pregnancy. 00:15:04.69\00:15:09.58 First pregnancy, the head of the 00:15:09.61\00:15:11.03 baby will go into the pelvic cavity before you go into labor. 00:15:11.06\00:15:15.89 For multigravid or women who have delivered before.. already, 00:15:15.92\00:15:20.13 the head is usually floating until the cervix is opened 00:15:20.16\00:15:24.42 fully before the head comes down. 00:15:24.45\00:15:26.51 Anyway, primigravid - the head comes down. 00:15:26.54\00:15:29.23 Okay, so what other signs? 00:15:29.26\00:15:32.41 We have also bloody show, you would call that. 00:15:32.44\00:15:36.29 There is the show - it will show I'm coming! 00:15:36.32\00:15:39.30 The show is the mucous plug of the cervix. 00:15:39.33\00:15:43.96 You see the Lord has created the cervix to be closed 00:15:43.99\00:15:47.65 so that the vagina is not sterile... 00:15:47.68\00:15:51.99 There is bacteria in the vagina and the bacteria in the vagina 00:15:52.02\00:15:56.59 can get to the baby... 00:15:56.62\00:15:57.99 But because of this mucous plug, this is a tenacious guard; 00:15:58.02\00:16:03.99 bacteria can go to the baby but when you come into labor, 00:16:04.02\00:16:08.57 this mucous plug comes out. 00:16:08.60\00:16:11.31 So it's like the cervix is now ready - ripening. 00:16:11.34\00:16:15.72 So this mucous plug comes out, 00:16:15.75\00:16:17.86 and this mucous plug, of course, is mucous you know, 00:16:17.89\00:16:20.14 and it's blood-tinged; sometimes it's brownish, 00:16:20.17\00:16:23.33 but the mucous plus will come out and you know... 00:16:23.36\00:16:26.22 Oh okay, I'm almost there. 00:16:26.25\00:16:28.16 So should I call my doctor as soon as that happens 00:16:28.19\00:16:31.51 or should I go straight to the hospital? 00:16:31.54\00:16:33.07 Well because you don't know really, you could... 00:16:33.10\00:16:36.91 You could call your doctor but there are other things that 00:16:36.94\00:16:40.43 can come together with it like, for example, 00:16:40.46\00:16:43.37 ...some women have diarrhea. 00:16:43.40\00:16:45.69 It's because when labor starts to come, your hormones change 00:16:45.72\00:16:50.83 and your gastrointestinal tract "sympathizes" with you, 00:16:50.86\00:16:57.22 and so it starts... This became a sensation because she was a... 00:25:24.03\00:25:29.09 I'm not tall, I'm only 5'1" but this patient of mine 00:25:29.12\00:25:32.71 was 4'10" and she had a really, really BIG, BIG baby; 00:25:32.74\00:25:38.53 the estimated fetal weight was something like a 9-pounder. 00:25:38.56\00:25:42.05 So she was very small and she 00:25:42.08\00:25:44.16 had an estimated fetal weight of 9 pounds. 00:25:44.19\00:25:47.21 We did an x-ray pelvimetry 00:25:47.24\00:25:50.85 which is an x-ray to see the pelvis and compare it to 00:25:50.88\00:25:54.65 the head of the baby. 00:25:54.68\00:25:56.58 And the x-ray pelvimetry said there is a marked disproportion. 00:25:56.61\00:26:04.00 The pelvis is too small for the head of the baby, 00:26:04.03\00:26:06.68 and this wasn't even taken at term. 00:26:06.71\00:26:09.85 But she said, "Doctor, just watch me, I KNOW I'm 00:26:09.88\00:26:13.89 going to deliver normally.
