learned last time Hypertension 00:00:45.08\00:00:47.41 is part of the Insulin Resistance Syndrome. 00:00:47.42\00:00:50.68 We want to find out what are some of the underlying 00:00:50.69\00:00:53.86 causes of Essential Hypertension. 00:00:53.87\00:00:56.64 Also I mentioned last time, 00:00:57.38\00:00:58.86 in Insulin Resistance there are couple of problems. 00:01:00.81\00:01:05.71 The way it's supposed to work as you take a sugar in, 00:01:05.72\00:01:08.29 blood sugar increases in your blood. 00:01:08.30\00:01:11.46 The pancreases senses that, releases insulin 00:01:11.47\00:01:15.05 that is a little key which opens the doors to your cells, 00:01:15.06\00:01:18.17 allows sugar to leave the blood go into cells 00:01:18.18\00:01:20.17 and get burned and that brings your blood sugars down 00:01:20.18\00:01:22.30 and so is supposed to work. 00:01:22.31\00:01:23.78 Animal products we found increase amount of fats 00:01:23.79\00:01:28.58 in your diet and when those go into the system 00:01:28.59\00:01:30.84 they plug up the little keyholes 00:01:30.85\00:01:32.48 and they cause an increase in Insulin Resistance. 00:01:32.49\00:01:35.78 Now when you take a sugar, the blood sugar raises, 00:01:35.79\00:01:39.53 pancreases releases insulin, can't get in those keyholes. 00:01:39.54\00:01:42.93 The doors they shut, blood sugars rise, 00:01:42.94\00:01:44.79 that's Insulin Resistance leading to diabetes. 00:01:44.80\00:01:47.99 I want to look today how that affects your blood pressure also 00:01:48.00\00:01:52.81 What we find is that insulin's job is to get sugar 00:01:52.82\00:01:56.79 from the intestines into the cells. 00:01:56.80\00:02:00.02 And so part of what it does is, 00:02:00.03\00:02:01.66 opens the insulin doors to allow sugar 00:02:01.67\00:02:04.24 to enter the cells to be burned. 00:02:04.25\00:02:08.09 The other thing it does is it dilates arterioles 00:02:08.10\00:02:11.87 in a dose-dependent way to help blood flow from intestines 00:02:11.88\00:02:16.86 to the peripheral organs to help the delivery of sugar. 00:02:16.87\00:02:21.35 Insulin has been found to increase something 00:02:22.96\00:02:26.97 called nitric oxide in the blood arterioles. 00:02:26.98\00:02:31.64 Nitric oxide is what actually does the vessel dilating. 00:02:31.65\00:02:35.28 What they find is that in arterioles, 00:02:38.41\00:02:40.44 the inside lining of the walls of arterioles 00:02:41.17\00:02:44.18 are called Endothelial Cells, 00:02:44.19\00:02:45.78 and when insulin stimulates endothelial cells 00:02:45.79\00:02:49.22 they release something called Nitric Oxide Synthase 00:02:49.23\00:02:53.41 which changes arginine to nitric oxide 00:02:53.42\00:02:56.82 and that nitric oxide acts like 00:02:56.83\00:02:58.57 nitroglycerin under your tongue. 00:02:58.58\00:03:00.04 It's what dilates those arterioles. 00:03:00.05\00:03:03.32 It relaxes the arterial muscles 00:03:03.33\00:03:05.31 and allows it to become bigger and enlarged. 00:03:05.32\00:03:08.24 They are found that when they infuse insulin into arterioles, 00:03:11.56\00:03:15.16 the arterioles dilate and the nitric oxide levels 00:03:15.17\00:03:19.54 increase in a dose-dependent fashion. 00:03:19.55\00:03:22.17 In other words, the higher the insulin levels 00:03:22.18\00:03:23.90 the higher the nitric oxide levels. 00:03:23.91\00:03:26.49 This was true in both arterioles and also in venules. 00:03:26.50\00:03:30.60 Now this Vasodilatory Effect can be blocked 00:03:32.31\00:03:37.15 by insulin resistance just like fat can block 00:03:37.16\00:03:42.75 the effect of insulin in opening 00:03:42.76\00:03:45.68 the cell doors to allow sugar to enter a cell. 00:03:45.69\00:03:48.87 It also can block the effect of insulin 00:03:48.88\00:03:51.25 on dilating those arterioles. 00:03:51.26\00:03:53.01 This can occur in diabetics but it also can occur 00:03:53.55\00:03:57.10 in people who are obese and who have Hypertension. 00:03:57.11\00:04:00.00 It's been demonstrated that these people 00:04:00.01\00:04:02.30 have a resistance to the Vasodilatory Effect 00:04:02.31\00:04:06.60 of insulin on their arterioles. 00:04:06.61\00:04:09.24 So high sugar levels have also been shown to block 00:04:14.00\00:04:18.65 this Vasodilatory Effect of insulin on the arterioles. 00:04:18.66\00:04:22.57 So when people are having problems 00:04:22.58\00:04:27.20 with circulation in the heart, 00:04:27.21\00:04:29.12 they find that if you give insulin the perfusion 00:04:29.13\00:04:32.33 can actually increase but that this effect is blocked 00:04:32.34\00:04:35.55 if the sugar levels are too high. 00:04:35.56\00:04:37.96 So two things can block insulin's Vasodilatory Effect, 00:04:38.36\00:04:41.32 that's insulin resistance from the animal products 00:04:41.33\00:04:45.21 that we've talked about, 00:04:45.22\00:04:47.07 but also a high sugar levels can also block 00:04:47.08\00:04:49.84 that Vasodilatory Effect. 00:04:49.85\00:04:51.60 Well, the question then is what is this insulin resistance 00:04:52.36\00:04:57.99 on the Vasodilatory Effect of insulin 00:04:58.00\00:05:00.45 have to do with Hypertension. 00:05:00.46\00:05:02.48 In other words if you block insulin's ability 00:05:02.49\00:05:04.90 to dilate the arterioles how does that cause 00:05:04.91\00:05:08.26 or effect Hypertension. 00:05:08.27\00:05:10.17 Well, the first place we would look 00:05:10.18\00:05:13.15 is how does it affect the kidneys? 00:05:13.16\00:05:15.46 Kidneys get about a fifth of your blood supply 00:05:16.59\00:05:18.94 and there the organs that regulate your blood pressure. 00:05:18.95\00:05:22.77 They adjust the blood pressure thermostat 00:05:22.78\00:05:24.69 up and down based on their need. 00:05:24.70\00:05:27.39 Dr. Goldblatt did an experiment to demonstrate 00:05:29.31\00:05:33.09 how kidneys affect and adjust the blood pressure. 00:05:33.10\00:05:37.39 He took a dog and he took the artery 00:05:38.23\00:05:42.24 that would supply the dog's kidney 00:05:42.25\00:05:44.87 and he tied a string around that, 00:05:44.88\00:05:48.42 after removing the other kidney and he narrowed the artery. 00:05:48.43\00:05:52.59 So now the kidney thought it was not getting 00:05:52.60\00:05:55.41 the blood supply when kidney wasn't that it was before. 00:05:55.42\00:05:58.59 The pressure it saw was much lower 00:05:58.60\00:06:02.27 than the pressure to the rest of the body 00:06:02.28\00:06:03.76 because that constriction in the artery. 00:06:03.77\00:06:06.02 The kidney responded. how does the kidney respond? 00:06:08.03\00:06:11.35 Well, the kidney thought, you know, 00:06:11.36\00:06:13.48 I am not getting the blood pressure or the perfusion. 00:06:13.49\00:06:17.02 I was getting before I need to fix that. 00:06:17.03\00:06:19.87 What I need to do is turn up 00:06:20.30\00:06:22.28 the blood pressure thermostat and it did this. 00:06:22.29\00:06:25.97 It did this through a complicated system, 00:06:25.98\00:06:28.09 renin-angiotensin system in the kidney, turned on. 00:06:28.69\00:06:33.17 There was an initial hanging on to salt and water. 00:06:33.60\00:06:36.49 This increased the blood volume, 00:06:36.50\00:06:38.48 cardiac output increased transiently, 00:06:38.49\00:06:40.89 when that happened the other organs 00:06:40.90\00:06:43.75 tried to protect themselves from this increase 00:06:43.76\00:06:45.70 blood pressure by narrowing the arterioles 00:06:45.71\00:06:48.17 that were supplying each of those peripheral organs 00:06:48.18\00:06:51.88 that increase the pressure even more. 00:06:51.89\00:06:54.15 Once the pressure was increased the kidney said, 00:06:54.16\00:06:59.48 okay blood pressure is high. 00:06:59.49\00:07:00.87 Now we can get rid of that excess fluid. 00:07:00.88\00:07:03.75 The renal level came back to normal. 00:07:03.76\00:07:05.59 The excess fluid was released to the kidneys 00:07:05.60\00:07:09.79 and what you were left with was a higher blood pressure 00:07:09.80\00:07:13.53 and you had arterial constriction 00:07:13.54\00:07:15.82 to the peripheral organs. 00:07:15.83\00:07:17.46 Now you don't have to really understand all those steps 00:07:17.47\00:07:20.07 that led to this but the final picture was important. 00:07:20.08\00:07:23.34 It showed increased blood pressure 00:07:23.35\00:07:26.11 and arterial constriction to all the little blood vessels 00:07:26.12\00:07:31.07 supplying the peripheral organs. 00:07:31.08\00:07:32.73 Well, what is the picture that we see in Hypertension? 00:07:34.28\00:07:39.34 Is there any evidence that this is actually 00:07:39.35\00:07:41.40 what's happening in Hypertension the essential Hypertension? 00:07:41.41\00:07:45.06 Well, when they look at the physiologic picture 00:07:47.11\00:07:50.20 of essential Hypertension and this dog 00:07:50.80\00:07:54.13 with the artery that's narrowed, 00:07:54.14\00:07:55.82 we find the same identical picture between the dog 00:07:55.83\00:07:59.45 and people with essential Hypertension. 00:07:59.46\00:08:01.42 What they find is that the resistance flow through 00:08:02.68\00:08:05.76 the kidneys has increased 2-4 times in both the dog 00:08:05.77\00:08:08.59 and people with essential Hypertension. 00:08:08.60\00:08:10.91 Renal perfusion that flow of blood through 00:08:12.14\00:08:14.94 the kidneys is decreased in both the dog 00:08:14.95\00:08:17.36 and the people with essential Hypertension. 00:08:17.37\00:08:20.87 Sometime half normal. 00:08:20.88\00:08:22.70 Blood pressure is increased by 40% to 60%. 00:08:23.81\00:08:26.77 Cardiac output is maintained as the same, 00:08:29.01\00:08:32.00 you know, ability to make urines, 00:08:32.01\00:08:33.64 the same in both pictures. 00:08:33.65\00:08:35.58 Renin levels have come back down to normal. 00:08:36.01\00:08:38.13 Essentially what you have is the same picture 00:08:39.01\00:08:41.48 between essential Hypertension patients 00:08:41.49\00:08:44.26 and the dog with the narrowed arterial 00:08:44.27\00:08:47.39 or narrowed artery going to the kidney. 00:08:47.40\00:08:50.67 Dr. Guyton is a famous physiologist. 00:08:52.80\00:08:57.87 He's written the text book on "Medical Physiology" 00:08:57.88\00:09:01.11 for many years, in fact, he is the standing 00:09:01.12\00:09:04.13 feet of written the whole text book. 00:09:04.14\00:09:05.94 Most people write text book today will write a chapter 00:09:05.95\00:09:08.87 or two and sign the other chapters to other people. 00:09:08.88\00:09:12.58 He was in such caliber that he was able 00:09:12.59\00:09:14.63 to actually write the whole book. 00:09:14.64\00:09:16.55 He did and studies in Cardiac Physiology. 00:09:16.56\00:09:22.92 He gave us the concept of cardiac output, 00:09:22.93\00:09:26.20 peripheral resistance. All these things 00:09:26.21\00:09:28.66 were developed by Dr. Guyton. 00:09:28.67\00:09:30.65 He studied essential Hypertension 00:09:31.98\00:09:33.85 and question is, what did he believe, 00:09:34.32\00:09:36.93 was the cause of essential Hypertension. 00:09:36.94\00:09:40.05 He actually said that, according to him, 00:09:41.45\00:09:43.39 He said, the significant vascular changes in the kidney 00:09:43.40\00:09:46.73 that is the Increased Vascular Resistance through the kidneys, 00:09:46.74\00:09:52.20 suggest the basic kidney abnormality 00:09:53.71\00:09:55.81 is in essential Hypertension is these vascular changes 00:09:55.82\00:09:59.83 or the increased resistance to blood flow through 00:09:59.84\00:10:02.91 the kidney that is found in essential Hypertensions. 00:10:02.92\00:10:05.59 Well, what can cause increased vascular resistance 00:10:07.14\00:10:12.44 through the kidneys? 00:10:12.45\00:10:13.51 Well, as we've seen insulin resistance 00:10:14.19\00:10:16.19 can't do that very thing. 00:10:16.20\00:10:18.08 And so in essential, in insulin resistance 00:10:20.87\00:10:25.61 we would expect to see as part 00:10:25.62\00:10:28.41 of the insulin resistance syndrome, 00:10:28.42\00:10:29.91 essential Hypertension and in fact, 00:10:29.92\00:10:32.12 when we look at the list of things 00:10:32.13\00:10:33.63 that is included in the Insulin Resistance Syndrome 00:10:33.64\00:10:36.90 we have blood sugars above 100 00:10:36.91\00:10:40.35 which we've talked about diabetes. 00:10:40.36\00:10:42.32 We also have abdominal obesity but essential Hypertension 00:10:44.02\00:10:48.15 is part of the Insulin Resistance Syndrome 00:10:48.16\00:10:50.47 and it's through this mechanism 00:10:50.48\00:10:51.86 that the blood pressures are elevated. 00:10:51.87\00:10:54.80 Well, It would then follow that if we can do something 00:10:56.80\00:11:00.06 to decrease insulin resistance we should able 00:11:00.07\00:11:02.89 to increase insulin's ability to dilate those arterioles, 00:11:02.90\00:11:09.03 increase flow through the kidneys 00:11:09.04\00:11:10.74 and then allow the kidneys to say, 00:11:11.29\00:11:13.56 I don't need this high blood pressure anymore. 00:11:13.57\00:11:15.62 I can adjust the blood pressure down. 00:11:15.63\00:11:17.85 A plant-based diet we've found as we've talked 00:11:18.72\00:11:22.31 about earlier can help reverse insulin resistance 00:11:22.32\00:11:25.28 and in fact, bring blood pressures down. 00:11:25.29\00:11:27.72 And this is what we have found in patients 00:11:27.73\00:11:29.81 who chosen to try the plant based diet 00:11:29.82\00:11:33.21 that with essential Hypertension, 00:11:33.22\00:11:35.12 usually in a week or two we see blood pressures 00:11:35.13\00:11:37.21 coming down and we've been able to adjust 00:11:37.22\00:11:40.20 their medications in the downward direction. 00:11:40.21\00:11:43.15 Is there any evidence that vegetarians 00:11:45.07\00:11:46.69 actually have lower blood pressures? 00:11:46.70\00:11:48.40 Yes, there are. We find that male college students 00:11:49.03\00:11:51.26 that were Lacto-ovo vegetarians had lower blood pressures 00:11:51.27\00:11:54.48 than their carnivorous counterparts. 00:11:54.49\00:11:59.19 They found that Lacto-ovo vegetarian 00:12:01.41\00:12:03.11 German monks had lower blood pressures at all ages 00:12:03.12\00:12:06.21 compared to meat eating monks. 00:12:06.22\00:12:07.94 It's also found in Czechoslovakia areas 00:12:08.31\00:12:11.21 that had greater animal consumption 00:12:11.22\00:12:13.32 had higher blood pressures than those 00:12:13.33\00:12:15.36 that had lower animal consumption. 00:12:15.37\00:12:18.35 In Japan, greater consumption of animal products 00:12:18.36\00:12:22.13 which was also associated with higher blood pressures. 00:12:22.14\00:12:24.76 They also found that the differences 00:12:27.51\00:12:29.18 were not attributed to changes in salt intakes, 00:12:29.19\00:12:31.85 but the salt intakes were very similar between the two groups. 00:12:31.86\00:12:35.22 Railway clerks in South India had sixfold greater meat intake 00:12:35.23\00:12:39.89 than the Northern workers 00:12:39.90\00:12:42.55 and also had much higher blood pressures. 00:12:42.56\00:12:45.11 Studies in the Polynesians showed that areas 00:12:47.27\00:12:51.98 that had higher meat consumption 00:12:51.99\00:12:53.41 also had again higher blood pressures. 00:12:53.42\00:12:55.72 They've also found that in New Guinea, 00:12:56.68\00:12:58.64 the natives that had a higher meat rash 00:12:59.54\00:13:02.89 than the others again had higher blood pressures. 00:13:02.90\00:13:05.44 And numerous studies have been showing these. 00:13:05.45\00:13:08.65 Well, the question then is what about salt. 00:13:09.54\00:13:12.26 Haven't I have been told 00:13:12.69\00:13:13.82 that high salt intake causes Hypertension? 00:13:13.83\00:13:18.25 Actually what we want to look at, 00:13:18.26\00:13:19.75 what we were finding is that hanging on 00:13:19.76\00:13:22.01 to salt is the kidney's response to this narrowed arterioles 00:13:22.02\00:13:26.68 and we are going to look at that at this point. 00:13:26.69\00:13:28.50 What they found is that, yes, if we take 00:13:29.94\00:13:31.80 a general population give them a high salt intake. 00:13:31.81\00:13:36.13 Blood pressures can rise about 2.5 mmHg systolic. 00:13:36.14\00:13:42.01 If we take hypertenses 00:13:42.52\00:13:44.41 and give them a high salt intake, 00:13:44.42\00:13:46.67 their blood pressure rises about 4.5 points. 00:13:47.18\00:13:50.50 On the other hand, if we take normal tenses of people. 00:13:50.51\00:13:53.53 People who don't have high blood pressure 00:13:53.54\00:13:55.52 and give them a high salt intake, 00:13:55.53\00:13:57.53 blood pressure only rises 1.9 points. 00:13:57.54\00:14:00.88 So they have a lower response to salt 00:14:00.89\00:14:04.70 if they are not hypertensive to begin with. 00:14:04.71\00:14:07.23 And these figures can occur with Dr. Guyton's findings 00:14:08.41\00:14:12.89 that an average blood pressure drop 4 to 6 mm of Hg 00:14:12.90\00:14:17.52 occurs when you restrict salt. 00:14:17.53\00:14:20.14 Well, they found that vegetarians 00:14:21.27\00:14:25.49 again have a lower blood pressure 00:14:25.50\00:14:28.05 than non-vegetarians and one of the arguments 00:14:28.06\00:14:32.13 or hypothesis for this was that well maybe vegetarians 00:14:32.14\00:14:35.57 have a lower salt intake, 00:14:35.58\00:14:37.54 maybe that's why their blood pressure is lower. 00:14:37.55\00:14:39.71 So they want and they tested this. 00:14:39.72\00:14:41.72 They looked at a group of vegetarians and found that, 00:14:42.29\00:14:46.80 yes, the vegetarians blood pressure was significantly 00:14:46.81\00:14:49.91 lower than the meat eaters, 00:14:49.92\00:14:52.43 but when they measure the salt intake 00:14:53.16\00:14:55.55 between two groups they found that the vegetarians 00:14:55.56\00:14:59.02 were actually eating more salt than the meat eaters 00:14:59.03\00:15:02.49 but their blood pressure was less. 00:15:02.50\00:15:04.82 So he said, well, it can't be the salt 00:15:04.83\00:15:07.52 which is the advantage for the vegetarians 00:15:07.53\00:15:09.65 because they are eating more salt. 00:15:09.66\00:15:10.99 There must be something else going on 00:15:11.00\00:15:13.10 and of course, we understand 00:15:13.11\00:15:14.39 that it is the Insulin Resistance Syndrome 00:15:14.40\00:15:16.15 was not affecting the vegetarians 00:15:16.83\00:15:20.11 as it was the meat eaters. 00:15:20.12\00:15:22.35 The next question is what causes salt sensitivity? 00:15:24.02\00:15:27.96 What they found is that for some people 00:15:27.97\00:15:30.80 you give them a high salt intake 00:15:30.81\00:15:32.52 and the blood pressures go up, other groups of people 00:15:32.53\00:15:35.73 that blood pressures do not go up significantly, 00:15:35.74\00:15:38.09 what is the difference? 00:15:38.62\00:15:39.90 Well, they separated a large group of people 00:15:39.91\00:15:42.02 into those whose blood pressure went up with salt. 00:15:43.33\00:15:46.77 They were the salt sensitives. 00:15:46.78\00:15:48.47 Another group whose blood pressure 00:15:48.48\00:15:50.31 didn't go up significantly they were the salt insensitives. 00:15:50.32\00:15:54.55 And then they tested them for insulin resistance. 00:15:54.56\00:15:57.10 What they found was the people that were sensitive 00:15:57.58\00:16:00.22 to salt had insulin resistance. 00:16:00.23\00:16:02.45 They were resistance to the action of insulin. 00:16:02.46\00:16:04.61 Those that did not have sensitivity to salt, 00:16:05.03\00:16:08.62 the blood pressure stayed fairly normal 00:16:08.63\00:16:10.13 when they gave them large dose of salt, 00:16:10.14\00:16:11.58 were not insulin resistance. So what's going on? 00:16:11.59\00:16:15.60 Why does insulin resistance have anything 00:16:15.61\00:16:18.72 to do with salt sensitivity? 00:16:18.73\00:16:20.68 What happens is when there is insulin resistance 00:16:21.12\00:16:24.53 the arterials again to the kidneys 00:16:24.54\00:16:26.40 are narrowed and the kidney, 00:16:26.41\00:16:31.29 a normal kidney who has no insulin resistance 00:16:31.30\00:16:34.61 let say you know out of blood pressure 120/80, 00:16:34.62\00:16:36.92 I am getting all the perfusion I need. 00:16:37.40\00:16:39.33 I am going to leave the blood pressure 120/80. 00:16:39.34\00:16:41.78 But if they become insulin resistance 00:16:42.42\00:16:44.73 the arterioles narrow. 00:16:44.74\00:16:46.35 The kindney says, I am not getting 00:16:46.36\00:16:47.97 the perfusion I was before. 00:16:47.98\00:16:50.04 I need more perfusion. 00:16:50.05\00:16:51.57 I am going to turn up the thermostat. 00:16:51.58\00:16:54.09 And we found how the kidney does that, 00:16:54.10\00:16:56.68 hangs on the salt water and goes through process 00:16:56.69\00:16:59.08 to eventually come to a higher 00:16:59.09\00:17:00.60 equilibrium point for Hypertension. 00:17:00.61\00:17:03.40 The blood pressure is now says 160/100. 00:17:03.41\00:17:07.57 The kidney says, well, this is better perfusion. 00:17:07.58\00:17:11.75 This is better than it was. It's not ideal. 00:17:11.76\00:17:14.12 I wish I could bring it maybe upto 180, 00:17:14.13\00:17:16.66 but this is better. 00:17:16.67\00:17:17.69 If that person takes salt, a high dose of salt, 00:17:18.15\00:17:21.56 the kidney says, oh, salt. 00:17:21.57\00:17:23.02 This is just what I am looking for. 00:17:23.03\00:17:24.67 It hangs on to the salt. 00:17:24.68\00:17:26.01 Blood pressure goes up even higher. 00:17:26.02\00:17:28.22 And now kidneys say, okay 180/100 00:17:29.99\00:17:32.55 that's getting better perfusion 00:17:32.56\00:17:34.26 and he will leave it at a higher level 00:17:34.72\00:17:36.90 with addition of that salt. 00:17:36.91\00:17:39.79 If this person on the other hand would go on a say 00:17:39.80\00:17:42.10 plant based diet, the kidney's vascular system can now dilate. 00:17:42.11\00:17:47.94 It gets better perfusion and then what we find 00:17:48.58\00:17:51.57 is the kidneys can now get rid of that extra fluid 00:17:51.58\00:17:57.01 and that extra salt and bring the blood pressures 00:17:57.02\00:18:00.69 back down to say 120/80. 00:18:00.70\00:18:03.59 And it says now with these dilated arterioles, 00:18:03.60\00:18:06.17 since we don't have insulin resistance, 00:18:06.18\00:18:07.60 I am getting all the perfusion 00:18:08.08\00:18:09.19 I need with a blood pressure 120/80. 00:18:09.20\00:18:11.86 If this person gets a high dose of salt, 00:18:11.87\00:18:14.72 the kidneys say, thanks for the salt 00:18:14.73\00:18:17.15 but I really don't need it 00:18:17.16\00:18:18.46 and just dumps it out into the urine. 00:18:18.47\00:18:21.34 And so these people without insulin resistance 00:18:21.35\00:18:23.86 are not salt sensitive and are not as sensitive 00:18:23.87\00:18:28.37 to the effects of salt in their diet, 00:18:28.38\00:18:29.94 because the blood pressure can stay normal. 00:18:29.95\00:18:32.28 Now I shared this concept at a Lifestyle Center once 00:18:33.18\00:18:37.61 and I got in trouble for it. 00:18:37.62\00:18:39.42 One of the patients was furious after I shared this concept 00:18:39.43\00:18:44.23 about salt being not the core issue 00:18:44.24\00:18:46.65 but that was insulin resistance. 00:18:46.66\00:18:48.20 And she wrote an awful evaluation 00:18:48.21\00:18:51.23 and stormed out of the room and one of the patients 00:18:51.24\00:18:53.81 who had been listening said, see that lady. 00:18:53.82\00:18:55.50 She was very upset. I said, yes, what happened? 00:18:55.51\00:18:57.59 Why was she so upset? 00:18:57.60\00:18:59.34 And he explained it, well, she had been following 00:18:59.35\00:19:02.32 another patient around who had Hypertension 00:19:02.33\00:19:04.46 and every time he put salt on his food 00:19:04.47\00:19:06.32 she was behind him saying I saw you do that, 00:19:06.33\00:19:08.49 don't you put salt on that food. 00:19:08.50\00:19:09.99 I am going to watch you 00:19:10.00\00:19:11.49 and when I said that salt wasn't the core problem, 00:19:11.50\00:19:14.11 he turned around and looked at her 00:19:14.12\00:19:15.87 and said, nah, nah, nah, nah. 00:19:15.88\00:19:18.16 So she was not happy but nevertheless 00:19:18.17\00:19:23.28 salt wasn't the core issue. 00:19:24.07\00:19:26.60 Now if you have insulin resistance 00:19:26.61\00:19:29.42 it would be prudent towards salt 00:19:29.43\00:19:31.14 until you are not insulin resistant anymore. 00:19:31.15\00:19:33.95 Once you are out of the insulin resistance face 00:19:33.96\00:19:36.55 then you should be able to handle salt 00:19:36.56\00:19:39.01 much more easily then before. 00:19:39.02\00:19:41.55 So question then is, how can I lower my blood pressure? 00:19:42.19\00:19:46.47 Well, it's helpful do things that can avoid 00:19:47.30\00:19:53.35 the short term elevations of blood pressure. 00:19:53.36\00:19:56.08 Things that will cause a transient elevations 00:19:57.96\00:20:00.46 of blood pressure include caffeine, smoking, alcohol 00:20:00.47\00:20:05.90 these can cause transient elevations of blood pressure. 00:20:05.91\00:20:08.98 Nicotine, for instance cigarette can elevate 00:20:08.99\00:20:12.34 your blood pressure for about 30 minutes. 00:20:12.35\00:20:15.15 You might think well, this is in lung. 00:20:15.16\00:20:17.34 How can that really be of significance? 00:20:17.35\00:20:20.97 Well, a pack-a-day smoker will smoke about 20 cigarettes a day 00:20:20.98\00:20:24.53 and that's about 10 hours worth of blood pressure elevation. 00:20:24.54\00:20:28.02 That can be almost your whole waking time during the day. 00:20:28.03\00:20:33.38 Caffeine can also cause 00:20:35.97\00:20:37.56 elevations of blood pressures again transiently. 00:20:37.57\00:20:40.21 They are both vessel constrictors. 00:20:40.22\00:20:43.28 They cause vasoconstriction and actually this is part 00:20:43.29\00:20:48.86 of the problem with withdrawal headaches 00:20:48.87\00:20:51.95 because they cause vasoconstriction 00:20:52.71\00:20:54.34 when you come off of the caffeine 00:20:54.35\00:20:56.05 or the nicotine you actually get a vasodilation. 00:20:56.06\00:20:59.84 Now this is not a problem in the arms 00:21:00.36\00:21:02.15 or the legs where there is plenty of room 00:21:02.16\00:21:03.73 for the arteries to dilate. 00:21:03.74\00:21:05.07 But what happens in the head you can get that vasodilation 00:21:05.08\00:21:08.40 and it causes pounding headache. 00:21:08.41\00:21:10.39 The people that make aspirins realize 00:21:13.24\00:21:15.94 that many of the headaches they are treating 00:21:15.95\00:21:17.67 are caffeine withdrawal headaches 00:21:17.68\00:21:19.38 or nicotine withdrawal headache. 00:21:19.39\00:21:21.80 So what do they add to the aspirin to treat that? 00:21:22.20\00:21:25.65 Many times they will add caffeine. 00:21:26.10\00:21:27.87 This will then cause a vasoconstriction 00:21:28.74\00:21:30.89 and help relieve the vascular dilation in their head 00:21:30.90\00:21:34.45 causing that pounding headache. 00:21:34.46\00:21:35.88 The problem is that this prolongs the nicotine 00:21:36.43\00:21:39.66 and the caffeine addiction. 00:21:40.35\00:21:42.65 So what you need to do is do something 00:21:42.66\00:21:45.91 which can cause vasoconstriction in the brain 00:21:45.92\00:21:49.23 without prolonging the caffeine or nicotine addiction. 00:21:49.75\00:21:53.46 One of the best ways is to use a Hot Foot Bath, 00:21:53.84\00:21:56.58 put your feet in hot water that helps 00:21:56.59\00:21:58.62 give you some vasodilation in the feet, 00:21:59.25\00:22:00.95 helps pull blood away from the heads, 00:22:00.96\00:22:03.34 eyes to the forehead can also cause some vasoconstriction 00:22:03.87\00:22:06.89 and help relive that pounding headache. 00:22:06.90\00:22:08.82 The benefits' of this approach is that if you do it 00:22:09.44\00:22:12.41 for three or four days, by the fifth day 00:22:12.42\00:22:14.47 you are over the nicotine withdrawal syndrome 00:22:14.48\00:22:17.69 or the caffeine withdrawal symptoms. 00:22:17.70\00:22:19.20 And you don't have to worry about having to repeat 00:22:19.21\00:22:23.89 the nicotine or caffeine to cause vasoconstriction. 00:22:23.90\00:22:26.24 If on the other hand, you continue to treat 00:22:26.25\00:22:28.32 with nicotine or caffeine you gonna continue 00:22:28.33\00:22:30.29 the addiction and every time you go into withdrawal, 00:22:30.30\00:22:33.24 you can have the headaches again. 00:22:33.93\00:22:35.40 People who use high levels of caffeine and nicotine 00:22:35.91\00:22:40.56 when they go at night and go to sleep, 00:22:41.01\00:22:43.31 they go through withdrawal and what happens 00:22:43.91\00:22:47.48 is their Arterioles dilate and what do they wake up 00:22:47.49\00:22:50.24 with but many times a pounding splitting headache 00:22:50.25\00:22:53.36 and the first thing they need to do 00:22:53.37\00:22:54.96 to get rid of that is to get a cup of coffee or the nicotine 00:22:54.97\00:23:00.20 again to help relief that. 00:23:00.21\00:23:02.45 If you can, however, break the cycle 00:23:02.46\00:23:04.91 and get over these addictions, you can do much better. 00:23:05.81\00:23:09.47 Things that can be done to help get over these addictions, 00:23:09.95\00:23:14.11 drinking plenty of water can flush the nicotine and caffeine 00:23:16.43\00:23:20.40 out of your system more quickly. 00:23:20.41\00:23:21.97 You can do warm showers. 00:23:22.76\00:23:26.40 It actually flushes it out through your system. 00:23:26.93\00:23:29.85 There are people have gone through 00:23:31.26\00:23:33.42 nicotine and caffeine withdrawal programs, 00:23:36.15\00:23:39.52 where they actually put them in the sauna. 00:23:39.53\00:23:41.34 They come out and give them a massage 00:23:41.35\00:23:43.30 and the towel comes up yellow. 00:23:43.31\00:23:45.38 Because the nicotine is actually coming out their sweat pores. 00:23:45.84\00:23:48.19 So if you take warm showers or saunas, 00:23:48.20\00:23:50.83 you can actually get rid of that nicotine, 00:23:50.84\00:23:53.14 caffeine much more quickly than otherwise. 00:23:53.15\00:23:56.71 You also want to avoid triggers that will cause 00:23:58.07\00:23:59.86 you to want to smoke or use caffeine 00:23:59.87\00:24:01.92 and the biggest trigger for smokers is caffeine. 00:24:03.57\00:24:06.85 It triggers a desire to smoke. 00:24:06.86\00:24:08.70 So it's best if you can stop them both together. 00:24:08.71\00:24:12.52 They have done a study at one lifestyle center 00:24:12.53\00:24:14.46 where people that stopped nicotine and caffeine 00:24:14.47\00:24:18.64 at the same time, they were followed 00:24:18.65\00:24:20.88 after they left the program. 00:24:20.89\00:24:22.92 Those that stayed off of caffeine, 00:24:22.93\00:24:25.92 90 percent of those were able to stay off the cigarettes. 00:24:25.93\00:24:29.05 Those that went back to caffeine, 00:24:29.48\00:24:31.41 90 percent of those went back to smoking. 00:24:31.42\00:24:34.51 So there is high correlation between 00:24:34.52\00:24:36.52 smoking and cigarettes and caffeine. 00:24:36.53\00:24:40.63 You also want to avoid other triggers 00:24:41.22\00:24:43.23 that will cause you to want to smoke 00:24:43.24\00:24:44.73 or and those include alcohol, lack of sleep 00:24:44.74\00:24:49.82 and the smell of smoke. 00:24:49.83\00:24:51.14 You can avoid those. 00:24:51.53\00:24:52.57 You can decrease your risk of going back to those habits. 00:24:52.58\00:24:57.10 Also, encourage patients to pray 00:25:00.19\00:25:02.37 because giving up smoking or tobacco 00:25:02.83\00:25:05.52 can be a spiritual battle. 00:25:06.41\00:25:08.44 A friend of mine, we helped him in Guam. 00:25:09.29\00:25:11.70 He was able to stop his hard drugs 00:25:12.27\00:25:15.89 and he stopped his alcohol 00:25:15.90\00:25:18.12 and then when he stopped cigarettes, 00:25:18.13\00:25:20.59 he was coming out of the store 00:25:20.60\00:25:22.14 and somebody in the parking lot, 00:25:22.15\00:25:23.71 said hey, you wanna cigarette. 00:25:23.72\00:25:26.73 In fact, he handed the whole pack. 00:25:26.74\00:25:27.82 In fact, you can have the whole carton. 00:25:27.83\00:25:29.83 And I asked Mr. John, you are 30 years old. 00:25:30.28\00:25:32.86 Does everyone ever offer you 00:25:32.87\00:25:34.42 a free pack of cigarettes before? 00:25:34.43\00:25:35.96 And He said, no and certainly never a whole carton. 00:25:35.97\00:25:37.92 And I said, do you think it's a coincidence 00:25:38.41\00:25:40.61 that on the day you stop someone 00:25:40.62\00:25:41.88 offers you a whole carton. 00:25:41.89\00:25:43.12 I said God's got His angels, Satan's got his angels, 00:25:43.57\00:25:45.78 and they are fighting over territory. 00:25:45.79\00:25:47.17 You are the territory. 00:25:47.18\00:25:48.51 I shared this with a nurse, friend of mine. 00:25:49.04\00:25:50.78 She was a Christian. She said, oh yes, I know that. 00:25:50.79\00:25:53.02 I told the friend of mine the same thing 00:25:54.22\00:25:56.19 and he didn't believe me but the day after 00:25:56.20\00:25:58.11 he stopped smoking, there was a knock on his door. 00:25:58.12\00:26:00.96 He answered the door. 00:26:00.97\00:26:01.94 It was friend of his and his friend said, dear George 00:26:01.95\00:26:03.68 You remember that cigarette I borrowed 00:26:03.69\00:26:06.46 from you a year ago? 00:26:06.47\00:26:07.63 I never gave it back. Here it is. 00:26:08.33\00:26:10.87 So I consult patients. You gonna stop cigarettes. 00:26:10.88\00:26:14.00 Remember it is a spiritual battle 00:26:14.01\00:26:15.70 and prayer is really needed 00:26:16.19\00:26:18.96 because they are bigger than we are. 00:26:18.97\00:26:20.92 So, it's important to remember that if for any reason 00:26:20.93\00:26:25.25 your blood pressure does not response 00:26:25.26\00:26:26.92 to simple lifestyle changes, you may have a secondary 00:26:26.93\00:26:29.88 cause of Hypertension. Endocrine tumor can do this. 00:26:29.89\00:26:34.29 Renal artery stenosis can do this. 00:26:36.50\00:26:38.23 There are several things that you should be checked for. 00:26:38.24\00:26:40.63 And so for any reason it does not respond, 00:26:40.64\00:26:43.83 check with your doctor. 00:26:43.84\00:26:45.22 You may need a further evaluation, 00:26:45.23\00:26:46.42 further work up. But 95 percent of hypertensive 00:26:46.43\00:26:50.05 is essential Hypertension and does respond 00:26:50.77\00:26:53.76 very well to lifestyle programs. 00:26:53.77\00:26:56.33 So again the things to remember, 00:26:56.34\00:26:59.80 plant based diet, eliminate the animal products, 00:26:59.81\00:27:03.03 eliminate the oils, caffeine, nicotine, the alcohol, 00:27:03.04\00:27:08.89 and try to get on a walking program. 00:27:08.90\00:27:10.55 Do things proactively to avoid stress in your life 00:27:11.26\00:27:15.77 that could be very healthful. 00:27:15.78\00:27:17.46 And you will find that if you actually do these for lifestyle, 00:27:17.47\00:27:20.36 things that many times, 00:27:21.96\00:27:23.92 the stress in your life will go away. 00:27:23.93\00:27:26.36 If you are struggling remember God's grace can give us 00:27:26.81\00:27:30.28 the power to follow through on the choices 00:27:30.29\00:27:32.72 that we know are going to give us the best help, 00:27:32.73\00:27:35.15 encourage you to seek God and prayer. 00:27:36.22\00:27:38.44 He can give you the strength and guide you 00:27:38.45\00:27:41.04 in the choices that will help you to obtain the good health 00:27:41.05\00:27:46.28 that He does want you to have. 00:27:46.29\00:27:48.36 He does encourage us that He does want us 00:27:49.49\00:27:51.49 to have good health, that we might reflect His image 00:27:51.50\00:27:56.09 and be of better service to Him 00:27:56.56\00:27:57.89 and to our brothers and sisters.
