Tell us about that hope. 00:01:27.27\00:01:28.68 Tell me, help me understand what diabetes really is. 00:01:28.71\00:01:31.55 Diabetes is a condition where you have too 00:01:32.00\00:01:34.34 much sugar in the blood. We need sugar in our 00:01:34.37\00:01:38.32 blood without it we die, but if you get too much 00:01:38.35\00:01:42.15 sugar in the blood you have 00:01:42.18\00:01:43.94 problems, lots of problems. 00:01:44.41\00:01:46.34 What makes the blood sugar go up? 00:01:46.37\00:01:48.11 There are two types of diabetes, 00:01:49.07\00:01:51.08 type I and type II. And the blood sugar 00:01:51.11\00:01:54.39 goes up for different reasons in each 00:01:54.42\00:01:57.06 type of diabetes. First of all, I have to 00:01:57.09\00:02:00.52 introduce another concept with diabetes. 00:02:00.55\00:02:03.21 There is another word and that word is insulin. 00:02:03.24\00:02:05.76 Because to understand diabetes, you have to 00:02:06.82\00:02:09.20 understand insulin. Insulin is like a key 00:02:09.23\00:02:13.79 in the door, insulin opens the door and allow 00:02:14.42\00:02:19.20 sugar to go from the blood stream into the cell. 00:02:19.23\00:02:23.83 Without insulin, you can't get sugar out of 00:02:24.84\00:02:28.52 the blood and into the cell. So you eat, 00:02:28.55\00:02:31.38 the food goes down and is digested, enters 00:02:32.25\00:02:35.15 the blood stream and then you got problems 00:02:35.18\00:02:38.93 because if you have no insulin you can't get 00:02:38.96\00:02:41.76 the sugar out of the blood stream and you die 00:02:41.79\00:02:44.66 of starvation because you don't get that sugar 00:02:44.69\00:02:47.61 in where you wanted. That's type I diabetes, 00:02:47.64\00:02:50.54 we used to say that's childhood onset diabetes, 00:02:51.25\00:02:54.38 but we don't call it childhood onset diabetes 00:02:54.85\00:02:57.34 anymore, we call it type I. Type II is a different 00:02:57.37\00:03:02.24 disease and yet it's really the same. 00:03:02.27\00:03:04.68 The difference is you have plenty of insulin 00:03:05.30\00:03:08.58 in fact in early type I diabetes or type II 00:03:08.82\00:03:13.27 diabetes, you have more insulin then normal 00:03:13.30\00:03:17.25 and you say well how can this be type I no insulin 00:03:18.01\00:03:20.94 and your sugar goes up. Type II more than normal 00:03:21.26\00:03:24.70 and your sugar still goes up. 00:03:25.03\00:03:26.51 Well, in type II diabetes the problem is you have 00:03:26.96\00:03:30.09 got the key but it doesn't fit in the lock. 00:03:30.12\00:03:33.26 It fits in but you have to turn in, turn in, turn in 00:03:34.69\00:03:37.37 and we call it insulin resistance, but it just 00:03:37.94\00:03:41.59 doesn't work and so you eat and the sugar is, 00:03:41.62\00:03:45.71 the food is digested and then the sugar is 00:03:46.19\00:03:49.65 absorbed into the blood stream but there is a 00:03:49.91\00:03:52.57 delay in getting it out of the blood stream 00:03:52.60\00:03:55.26 and as a result of that delay the sugar builds up 00:03:55.67\00:03:58.93 in the blood stream very similarly to type I 00:03:58.96\00:04:02.57 and you get all the same problems that you do 00:04:03.03\00:04:05.39 in type I and type II equally even though there 00:04:06.02\00:04:09.11 mechanisms are slightly different. 00:04:09.14\00:04:11.01 So, when you eat the food that you eat turns into 00:04:11.31\00:04:13.87 sugar and then insulin helps 00:04:13.90\00:04:16.06 this food to get into the blood cell. 00:04:16.09\00:04:17.83 That's correct, of course, the foods that we eat 00:04:18.25\00:04:21.47 are all different in how they are absorbed. 00:04:22.13\00:04:25.81 If you took a soda that had fructose and sucrose 00:04:26.25\00:04:31.82 or corn syrup that's a usual sweetener plus you 00:04:31.85\00:04:35.40 say you took sugar and it was in water, add some 00:04:35.43\00:04:38.93 coloring in it, we call it soda a little 00:04:38.96\00:04:41.22 fizz and you drank it down, like that okay, 00:04:41.25\00:04:44.76 that sugar is gonna hit your blood stream very, 00:04:45.05\00:04:47.28 very rapidly, where is if you had a potato and 00:04:47.31\00:04:53.35 you baked it and you ate it with the skin on 00:04:53.63\00:04:56.17 the sugar still going to enter into your blood 00:04:56.69\00:04:59.45 stream quite rapidly but not as rapidly as 00:04:59.48\00:05:03.41 if you had a soda. If you took that potato and 00:05:03.44\00:05:06.50 you mixed it with some broccoli and some whole 00:05:06.53\00:05:08.70 wheat bread and a few other things then the 00:05:08.73\00:05:11.60 sugar absorption into the blood is going 00:05:11.63\00:05:13.88 to be even slower and it allows the insulin 00:05:13.91\00:05:17.34 more time down stream to get the sugar out of the 00:05:17.37\00:05:20.82 blood and into the cell where it belongs. 00:05:20.85\00:05:23.35 You have mentioned insulin, so what exactly 00:05:23.89\00:05:26.51 is that, where does it come from? 00:05:26.54\00:05:28.19 Insulin comes from the pancreas, it's made in 00:05:28.55\00:05:32.70 the islet cells of the pancreas and it is made in 00:05:33.01\00:05:37.31 response to sugar in the blood. 00:05:37.34\00:05:40.86 The body senses the blood sugar level 00:05:41.98\00:05:44.70 and it makes insulin accordingly. 00:05:45.03\00:05:47.24 Okay interesting, you mentioned that if we 00:05:47.62\00:05:50.82 eat certain kinds of foods that it will slow down 00:05:50.85\00:05:53.82 the rate at which sugar enters the blood tell me 00:05:54.01\00:05:56.37 what it is about those foods, the broccoli and 00:05:56.59\00:05:58.77 the potato with the skin on that helps that process? 00:05:59.07\00:06:02.25 If you think of our foods as I mentioned if 00:06:03.70\00:06:06.86 you eat just pure sugar that sugar is absorbed 00:06:06.89\00:06:11.07 in your mouth, it's absorbed in your 00:06:11.10\00:06:13.51 stomach, it's absorbed in the small intestine 00:06:13.54\00:06:16.34 and it enters very quickly. 00:06:16.37\00:06:18.14 If you are a type I diabetic and you have 00:06:18.80\00:06:20.98 a very low blood sugar and you get very 00:06:21.01\00:06:24.43 lethargic and or having an insulin reaction, 00:06:24.46\00:06:27.47 you can put just sugar in the mouth and it will 00:06:28.95\00:06:31.97 bring them out of the slow blood sugar and it's 00:06:32.00\00:06:35.44 absorbed very rapidly, but if you eat broccoli 00:06:35.47\00:06:39.15 or if you took broccoli and put it in your mouth 00:06:39.75\00:06:41.74 it would take a very long time for them to get the 00:06:41.77\00:06:44.10 sugar out of it because you have to break the 00:06:44.13\00:06:46.35 broccoli down, you have to break down the fiber, 00:06:46.95\00:06:49.65 you have to separate it, you have to break down 00:06:50.09\00:06:52.44 the carbohydrate from complex carbohydrate 00:06:52.47\00:06:56.09 which is really it's sugar hooked together, 00:06:56.12\00:06:59.16 you have one sugar molecule and then 00:06:59.19\00:07:00.90 another and another and another in a 00:07:00.93\00:07:02.36 long chain that's called the starch and your body 00:07:02.39\00:07:06.03 has to break those off one at a time, it requires 00:07:06.06\00:07:09.01 a lot of work and then gradually it feeds that 00:07:09.04\00:07:12.34 sugar into your system and helps 00:07:12.37\00:07:15.46 keep the blood sugar normal. 00:07:15.49\00:07:17.08 So, whatever you are saying is we should 00:07:17.93\00:07:19.65 eat a lot of fiber because fiber 00:07:19.68\00:07:21.67 helps to keep our blood sugar down? 00:07:21.70\00:07:24.78 Fiber helps slow the absorption of sugar 00:07:25.50\00:07:28.49 from our foods into our bodies, yes. 00:07:29.05\00:07:32.20 Okay interesting. Most of the people in my 00:07:32.23\00:07:34.58 family who have they all have type II diabetes, 00:07:34.61\00:07:37.77 most of them are overweight. 00:07:37.80\00:07:39.27 What role does weight have in this process? 00:07:39.30\00:07:42.36 Well it's interesting, let me tell you a story, okay. 00:07:43.81\00:07:47.54 Let's pretend that you and your husband are on 00:07:48.80\00:07:52.73 a vacation and you decide this vacation we really 00:07:52.76\00:07:57.94 wanna live it up. We are gonna go to a 00:07:57.97\00:07:59.75 really fine hotel and so you go to a city 00:07:59.78\00:08:02.52 and you think well I wonder which hotel 00:08:03.51\00:08:05.71 will be the best hotel and you say well I think 00:08:06.15\00:08:09.92 probably a busy one will be the best hotel. 00:08:09.95\00:08:12.37 So, you go to the hotel and you look in there 00:08:12.95\00:08:16.53 people just going in and going in and you say 00:08:16.56\00:08:19.54 this must be a really good one. 00:08:19.57\00:08:21.01 So, you get out of your car and you enter in the 00:08:21.45\00:08:24.24 lobby and it's very busy. There are people 00:08:24.27\00:08:27.10 everywhere and as soon as you come in 00:08:27.13\00:08:29.54 you start looking around for one of the bellhops 00:08:29.57\00:08:31.95 to take your luggage and to go with you 00:08:31.98\00:08:34.62 and you check in and it's so busy, it take so 00:08:34.65\00:08:38.72 while to find a bellhop but then he gets hold 00:08:38.75\00:08:41.48 of you and takes your arm and takes your luggage 00:08:41.51\00:08:43.90 and the way you go you through the lobby, 00:08:44.23\00:08:46.42 it's a beautiful lobby, you can tell well this is 00:08:46.45\00:08:49.08 wonderful hotel, although it's very busy 00:08:49.11\00:08:51.35 and you go through the door and into the hallway 00:08:52.03\00:08:55.57 and in the hallway it's just jammed there, 00:08:55.87\00:08:58.37 bellhops everywhere and they are holding 00:08:58.40\00:09:00.69 on to peoples arms and you go down and 00:09:00.72\00:09:03.16 he say let me see how your room is, oh yes 00:09:03.19\00:09:05.89 here it is, and so you knock on the door and 00:09:05.92\00:09:09.18 he knocks there and he say why you are knocking 00:09:10.64\00:09:12.84 and he said well, I just want to make sure that 00:09:12.87\00:09:15.37 no one is in the room. You say, well isn't this 00:09:15.40\00:09:17.28 our room and he say but we have to make sure 00:09:17.31\00:09:19.66 in this hotel, we are very busy. 00:09:19.69\00:09:21.11 So he knocks on the door and he starts to open it 00:09:21.45\00:09:24.07 and he can't get it open and he said sometimes 00:09:24.44\00:09:27.36 I really have to push hard on these doors, 00:09:27.39\00:09:29.27 so he puts his shoulder there and he gives a 00:09:29.30\00:09:31.55 mighty push and lone behold there is someone 00:09:31.58\00:09:34.73 behind the door, trying to hold it shut and the 00:09:34.76\00:09:38.15 person in the room says look you have already 00:09:38.18\00:09:41.29 interrupted my sleep twice tonight, 00:09:41.32\00:09:43.13 we only have two beds, yes they are double beds 00:09:43.43\00:09:45.72 but we already have four people and we are 00:09:45.75\00:09:47.42 not gonna have anymore. I just won't have it, okay. 00:09:47.45\00:09:52.08 And you say my, this is a strange hotel. 00:09:52.54\00:09:54.94 You know, that's really what 00:09:56.09\00:09:57.06 the problem is in type II diabetes. 00:09:57.09\00:09:59.33 You coming in are sugar. 00:10:01.01\00:10:02.56 The hallway is your blood vessel. 00:10:04.18\00:10:06.33 The room is the cell and in type-II diabetes 00:10:07.64\00:10:11.54 the problem is not with insulin. 00:10:11.57\00:10:14.36 He is doing his job the best he can. 00:10:14.39\00:10:16.53 The problem is with the management, 00:10:16.96\00:10:19.04 they have double booked every room, 00:10:19.29\00:10:20.98 and you go down to the cell and pour insulin, 00:10:22.01\00:10:24.62 he's got your stuff and he is trying his best to 00:10:24.65\00:10:27.28 get the door open and shut it back but there is 00:10:27.31\00:10:30.02 already too much sugar in that cell or too much fat 00:10:30.05\00:10:33.07 or too much calories in the cell and so he is 00:10:33.10\00:10:36.95 having a hard time opening the door and 00:10:36.98\00:10:40.07 there is the bottom line problem 00:10:40.10\00:10:41.96 with type-II diabetes. We often portray it as 00:10:41.99\00:10:45.70 insulin resistance. Well, it is true that 00:10:45.73\00:10:49.44 insulin, the Bellhop, who took you down the 00:10:49.47\00:10:52.31 hall to the room has a hard time getting the 00:10:52.34\00:10:56.15 door open, but it's not because he's not trying 00:10:56.18\00:10:59.76 to do his job, it's because down in the 00:10:59.79\00:11:03.78 lobby they've double booked everything, 00:11:04.14\00:11:07.03 that is they have eaten too much for too long 00:11:07.69\00:11:10.95 and you can, you can reverse type-II diabetes 00:11:11.87\00:11:14.65 if you get it early enough with careful diet, 00:11:14.68\00:11:18.55 restricting the amount of a gassed sugar calories 00:11:19.54\00:11:25.53 that you take, also restricting the times 00:11:25.56\00:11:29.58 when you take it, those poor bellhops you know 00:11:29.61\00:11:32.16 there is check in and a check out time at most 00:11:32.19\00:11:34.46 hotels, well at this hotel that you went to they 00:11:34.49\00:11:37.37 are accepting people around the clock instead 00:11:37.40\00:11:40.06 of having regular times for guest to 00:11:40.09\00:11:42.19 check in and check out. Like eating we should eat 00:11:42.22\00:11:45.46 regularly and we should eat the right amount so 00:11:45.49\00:11:49.00 that we can fill the rooms. Now let's take this 00:11:49.03\00:11:51.55 illustration a little longer, little further, 00:11:51.58\00:11:54.30 you know what the problem is with the 00:11:54.82\00:11:55.95 diabetes really? The problem is the sugar 00:11:55.98\00:11:59.86 in the blood, where it builds up, it is really 00:12:00.29\00:12:04.69 hard on the blood vessels, you know let's go 00:12:04.72\00:12:09.22 go back to the illustration. There you are with the 00:12:09.25\00:12:11.26 Bellhop and you're starting down the 00:12:11.29\00:12:14.92 hallway, and as you enter that hallway 00:12:14.95\00:12:19.19 there is sugar everywhere, and bellhops everywhere, 00:12:19.69\00:12:23.41 what's the hallway gonna look like? 00:12:23.44\00:12:25.10 It's gonna be crowded. It's gonna to be crowded. 00:12:25.67\00:12:27.74 It's gonna to be a mess and furthermore, 00:12:28.15\00:12:30.85 you know what insulin doesn't just 00:12:30.88\00:12:32.73 put sugar into cells. Insulin does a lot of 00:12:32.76\00:12:36.07 other things just like a Bellhop would. 00:12:36.10\00:12:38.18 Insulin says to the management, look we've 00:12:38.90\00:12:42.21 got so many people in this hotel, we need to 00:12:42.24\00:12:46.41 build some more rooms, insulin is a growth factor 00:12:46.44\00:12:50.21 and it signals back to management, 00:12:50.57\00:12:52.70 hey we need to build some more rooms. 00:12:52.95\00:12:55.21 We need to increase this, so the Bellhop takes 00:12:55.24\00:12:59.12 sugar down to the cell but he also says alright 00:12:59.15\00:13:01.85 let's start construction, so not only are you in 00:13:02.04\00:13:04.91 that hallway with all your sugar friends, 00:13:04.94\00:13:07.16 and not so good friends in this case, 00:13:07.84\00:13:10.00 you're having wheelbarrows going 00:13:10.50\00:13:12.06 down the hallway full of cholesterol, 00:13:12.09\00:13:14.40 because cholesterol is a basic building 00:13:14.43\00:13:16.75 block of all cell walls, all hotel rooms okay. 00:13:16.78\00:13:20.87 So let's going down there, and you've got 00:13:21.10\00:13:23.51 that whole hallway jammed up and pretty 00:13:23.54\00:13:25.94 soon the cockpits warren and sugar 00:13:25.97\00:13:28.65 it doesn't always get into the cell where it belongs, 00:13:28.68\00:13:31.45 it gets changed and folded into closets and 00:13:31.48\00:13:34.37 everywhere else and we got a mess and what we 00:13:34.40\00:13:36.98 really need to do to this poor hotel is we need to 00:13:37.01\00:13:40.49 stop, just stop for a minute, stop accepting 00:13:42.20\00:13:45.76 guests for a while and allow repair to take place. 00:13:45.92\00:13:49.68 Often times if we just take people and we say to 00:13:50.33\00:13:53.34 them alright cut back on the amount of food you're 00:13:53.37\00:13:55.39 eating or try two meals a day at regular times, 00:13:55.42\00:14:00.38 so if it's your time to clean up in between 00:14:00.74\00:14:03.18 that will take care of the problem. 00:14:03.66\00:14:05.00 Another thing that's helpful if you've ever 00:14:05.72\00:14:07.88 been to a hotel where there is a demanding 00:14:07.91\00:14:09.84 person, I want my room right now, 00:14:09.87\00:14:12.15 oh yes, get rid of those. You know you want 00:14:12.18\00:14:15.03 people to come in. They say no problem. 00:14:15.06\00:14:17.76 I've got time. That's like our high fiber foods, 00:14:17.79\00:14:20.77 no problem there is time. You drink a soda, 00:14:21.54\00:14:24.36 it's, I want to be in there right now and in they 00:14:24.55\00:14:27.45 go and poor bellhop has to work to get him to the 00:14:27.48\00:14:30.56 room right away and we got problems, and that's 00:14:30.59\00:14:33.94 really what happens with our body is we feed at 00:14:33.97\00:14:37.35 the wrong kind of food, quick sugar boom, 00:14:37.38\00:14:40.59 and then the insulin has to work really hard, 00:14:41.14\00:14:43.28 really fast to get into the cell or we overeat 00:14:43.31\00:14:47.62 even if good foods at a wrong times, you know 00:14:47.75\00:14:51.23 you might have heard of the stuff called grazing. 00:14:51.99\00:14:54.91 They say oh we need to graze. 00:14:55.92\00:14:57.43 We need to graze to lose weight, 00:14:57.95\00:14:59.18 and eat a little bit all the time. 00:14:59.33\00:15:00.84 Like the little sheep, 00:15:00.87\00:15:01.84 chewing all day long. That's right. It's really 00:15:01.85\00:15:03.40 good to do that if you are a cow. 00:15:03.43\00:15:05.08 If you got three stomachs, but you know what cows do? 00:15:05.92\00:15:08.36 They regurgitate and chew their cud and you 00:15:10.30\00:15:13.05 know, I don't know they gonna do that, you know 00:15:13.08\00:15:14.59 Sheryl if you eat all day long you are gonna 00:15:14.62\00:15:18.31 regurgitate too. You have that acid reflux 00:15:18.34\00:15:21.49 and acid reflux is epidemic for that same reason. 00:15:21.52\00:15:24.50 It's all from this problem eating too much, 00:15:25.12\00:15:27.68 eating demanding food, I want results right now 00:15:28.84\00:15:33.97 that's it quick sugar and it's also from eating at 00:15:34.00\00:15:39.07 irregular times instead of eating you know just 00:15:39.10\00:15:41.83 on time three meals a day or for type-II diabetics, 00:15:42.73\00:15:46.85 not type I, type II diabetics especially 00:15:46.88\00:15:50.21 if they are not already on medicine for it, 00:15:50.24\00:15:52.40 if they are on medicine, you gonna be careful. 00:15:52.57\00:15:54.25 But if you are not on medicine, or if they 00:15:54.63\00:15:56.07 have early type II diabetes two meals a day, 00:15:56.10\00:15:59.04 gives time for repair, gives pancreas time to 00:15:59.52\00:16:02.74 build up insulin for the next on-slot of the gas. 00:16:03.03\00:16:06.46 Interesting, so insulin is a growth 00:16:07.93\00:16:10.58 hormone and it doesn't necessarily 00:16:10.61\00:16:12.47 cause us to grow up, but rather out. 00:16:12.50\00:16:15.20 Oh, you know, insulin does when we are small, 00:16:15.49\00:16:18.89 it helps us grow up, but it causes a lot of problems 00:16:18.92\00:16:22.46 when we are big. You know, I say to my 00:16:22.49\00:16:24.87 male patients, I say you know you can tell 00:16:24.90\00:16:27.34 whether your insulin levels are too high or not. 00:16:27.37\00:16:29.47 All you have to do is look down and see if you can 00:16:29.79\00:16:33.10 see, oh there it is, see if you can see 00:16:33.13\00:16:35.36 your belt buckle. If you can't see your 00:16:35.39\00:16:38.00 belt buckle your insulin level is too high because 00:16:38.03\00:16:40.97 insulin puts that weight right around your middle. 00:16:41.00\00:16:43.73 Now with woman that's not always, you know, 00:16:44.30\00:16:46.39 helpful because they have other things between 00:16:47.07\00:16:49.31 their belt and their chin and we men do, they get 00:16:49.34\00:16:53.01 in the way, but the way women can do it is, 00:16:53.04\00:16:55.42 if they want, you want to have your hips bigger 00:16:55.89\00:16:59.59 than your waist and if your waist is bigger 00:16:59.62\00:17:01.58 than your hips you are in trouble. 00:17:01.61\00:17:03.28 You have got too high in insulin level. 00:17:03.69\00:17:05.24 Now some of that maybe genetic, but most of 00:17:05.27\00:17:08.31 it can be controlled by how 00:17:08.34\00:17:10.62 we eat and what we eat. 00:17:10.65\00:17:11.91 What role does physical activity play? 00:17:12.84\00:17:14.78 Physical activity is an interesting thing. 00:17:15.65\00:17:17.90 If you think of what we were really doing with 00:17:17.97\00:17:21.18 sugar, when you put it into the cell the only way 00:17:21.55\00:17:24.47 really to get it out is to burn it. 00:17:24.50\00:17:27.33 Now Unfortunately, when you go to the hotel they 00:17:27.79\00:17:29.76 don't burn you up to make room for the next guest, 00:17:29.79\00:17:32.10 but in your body when you put energy whether 00:17:32.13\00:17:35.14 it's fat or sugar into the cell, the only way to get 00:17:35.17\00:17:39.23 rid of it is to use it and so either you got to eat less 00:17:39.26\00:17:43.62 than you want to or you have got to 00:17:43.65\00:17:45.33 get out and exercise. It's interesting. 00:17:45.36\00:17:47.98 Insulin works better after exercise. 00:17:48.57\00:17:51.17 Best thing that you can do after you eat is to 00:17:51.69\00:17:54.56 have a little light exercise. 00:17:54.60\00:17:56.42 If you want to eat more fine burn more, 00:17:57.31\00:17:59.98 you can take that too far, I mean you can be 00:18:01.35\00:18:03.48 running 24 hours a day, and you know, 00:18:03.51\00:18:06.01 and push it too far, but most of us don't have 00:18:07.05\00:18:09.38 to worry about over exercise. 00:18:09.41\00:18:11.36 We need to exercise on average about 30-60 00:18:11.98\00:18:16.57 minutes everyday or at least six days a week 00:18:16.60\00:18:19.63 and if we do that our insulin works better why? 00:18:20.09\00:18:23.57 Because when it goes knocking on the cells 00:18:23.99\00:18:25.97 doors the cell says, yeah come right in and 00:18:26.00\00:18:29.27 bring that sugar, I want to burn it rather than 00:18:29.30\00:18:32.19 not here, I don't want anymore you forget 00:18:32.72\00:18:35.39 it and that's what happens in type II diabetes. 00:18:35.42\00:18:38.16 Insulin resistance is really a management problem. 00:18:38.55\00:18:42.09 It's right here and right here, it's not down 00:18:42.39\00:18:44.55 in our pancreas. The poor old pancreas 00:18:44.58\00:18:47.33 in type II initially type II 00:18:47.36\00:18:49.25 A we call it makes plenty of insulin, 00:18:49.28\00:18:51.80 but after a while it gets tired and it says 00:18:52.91\00:18:56.65 oh, I'm making all these insulin molecules 00:18:56.68\00:19:01.04 and they keep calling for more and it gets 00:19:01.51\00:19:04.86 tired and it stops making enough insulin 00:19:04.89\00:19:07.61 and then often even in type II diabetes 00:19:08.05\00:19:11.53 you need to add insulin to keep the sugar normal. 00:19:11.96\00:19:14.64 Now as I tell my patients, I say you don't 00:19:15.52\00:19:17.66 have to do what I want you to. 00:19:17.69\00:19:19.34 If you want to stay fat, you want to eat the 00:19:20.41\00:19:22.40 wrong food that's fine, I've got enough pills 00:19:22.43\00:19:24.80 and enough insulin to keep your sugar 00:19:25.04\00:19:26.68 normal if you will just do it. 00:19:26.71\00:19:28.11 But that isn't the best way, the best way 00:19:29.84\00:19:33.18 is to give your pancreas a break poor thing so 00:19:33.55\00:19:36.66 overworked, working it day and night, 00:19:36.69\00:19:39.03 you know all the time. So, what would a 00:19:39.76\00:19:42.92 person like myself with a strong family 00:19:42.95\00:19:45.55 history of diabetes, what would I need to do in 00:19:45.58\00:19:48.46 order to avoid developing that particular disease? 00:19:48.49\00:19:51.78 Well unfortunately as you have said, diabetes 00:19:51.81\00:19:55.51 is genetic to some extent and I always 00:19:55.54\00:19:58.94 tell my patients. I say the most important 00:19:58.97\00:20:01.11 thing for you to do is choose your 00:20:01.14\00:20:02.57 parents carefully. That's number one. You know. 00:20:02.60\00:20:06.24 I will work on that, that's the point. 00:20:06.51\00:20:07.75 But you know its interesting I say that 00:20:08.56\00:20:11.23 but if you look back at the last 20 years in America, 00:20:11.42\00:20:16.12 the last 30 years, obesity rates have just been sky 00:20:16.99\00:20:20.91 rocketing, they just been going way up. 00:20:20.94\00:20:23.24 And right behind it its diabetes. 00:20:23.88\00:20:26.76 Now, you can't tell me that the genetics have 00:20:27.30\00:20:29.54 changed in this country markedly 00:20:29.57\00:20:31.55 in the last 30 years. 00:20:31.58\00:20:33.10 It doesn't happen that rapidly. 00:20:33.92\00:20:35.62 What's happening we all started gaining weight, 00:20:36.11\00:20:38.62 we are eating differently then we ever have, 00:20:39.44\00:20:41.51 we are eating more sugar, we are eating more 00:20:41.70\00:20:44.18 saturated fat and saturated fat also is hard on the 00:20:44.21\00:20:48.50 pancreas and makes it difficult for the islet 00:20:48.53\00:20:51.32 cells to make insulin, so a more saturated fat 00:20:51.35\00:20:55.89 were grazing all the time. We don't sit down and 00:20:55.92\00:20:58.46 have a meal like we should and we are not 00:20:58.49\00:21:01.55 following that good information that the Lord 00:21:01.58\00:21:03.95 gave us many years ago, two meals a day for 00:21:03.98\00:21:07.12 almost to everyone, not everyone but certainly 00:21:07.15\00:21:09.91 if you have a family history of diabetes 00:21:09.94\00:21:11.78 two meals a day, your body is best ready for 00:21:12.32\00:21:16.33 sugar, for calories in the morning don't skip 00:21:16.68\00:21:20.22 breakfast whatever you do if you have that 00:21:20.46\00:21:22.52 history of diabetes get up and eat your breakfast 00:21:22.55\00:21:25.53 and then a good meal in noon or at early afternoon 00:21:26.12\00:21:29.88 and then let give that whole body time to clean 00:21:30.31\00:21:33.74 the hotel and get things all set up because if you 00:21:33.77\00:21:36.70 don't, you gonna corrupt the arteries. 00:21:36.73\00:21:39.00 Now here is the problem with diabetes. 00:21:39.03\00:21:41.23 I can tell you all the big complications 00:21:42.36\00:21:44.97 and here they are. Diabetes is the number 00:21:45.00\00:21:47.69 one cause in this country today of amputations, 00:21:47.72\00:21:51.26 cutting off your legs. It's the number one 00:21:51.85\00:21:54.57 cause of blindness. It's the number one 00:21:54.60\00:21:57.50 cause of kidney failure. It's the, you have a 00:21:57.53\00:22:00.92 markedly increased risk for a heart attack 00:22:00.95\00:22:03.78 with diabetes and you have a markedly 00:22:03.81\00:22:06.06 increased risk of stroke. Those are all blood vessel 00:22:06.09\00:22:09.33 diseases; you think back to the poor old hotel with 00:22:09.36\00:22:13.69 the arteries, the hallways all jammed up. 00:22:13.72\00:22:16.60 You want to have nice clean arteries and the 00:22:17.47\00:22:20.77 best way to do that is to normalize your blood 00:22:20.80\00:22:23.23 sugar through proper eating. 00:22:23.26\00:22:24.77 Now if you not gonna do that I have got all 00:22:26.29\00:22:29.08 kinds of pills I can give you. 00:22:29.11\00:22:30.50 You won't feel well long but I have got 00:22:30.96\00:22:32.81 you know I, the drug companies are out 00:22:32.84\00:22:35.23 there making them faster then I keep up with them. 00:22:35.26\00:22:37.58 We've just had about three or four new ones 00:22:37.96\00:22:40.20 for type II diabetes this year. 00:22:40.23\00:22:42.13 And there are wonderful medicines if you can't 00:22:42.91\00:22:45.24 get the management to obey you. 00:22:45.27\00:22:46.82 You know, but talk to the management, 00:22:47.30\00:22:49.08 slow down that sugar absorption 00:22:49.11\00:22:50.81 you do really well. So, for an individual 00:22:50.84\00:22:54.49 who already has diabetes what I hear you are 00:22:54.52\00:22:57.04 saying is that it is possible to, through 00:22:57.07\00:23:01.16 lifestyle through what we choose to eat, 00:23:01.19\00:23:03.92 how you chose to move in terms of physical 00:23:04.39\00:23:06.90 activity to manage our blood sugar. 00:23:06.93\00:23:09.44 Let's take a look at type I diabetes first. 00:23:09.86\00:23:12.63 Type I diabetes, I can't make the 00:23:12.66\00:23:15.23 pancreas come back. Now there is some 00:23:15.26\00:23:17.44 evidence in type I. If you have it in your 00:23:17.47\00:23:19.59 family, and you don't want to pass it on to 00:23:19.62\00:23:22.54 your children, don't have them drink any 00:23:22.57\00:23:27.25 milk or milk products for the first two years 00:23:27.39\00:23:30.57 of life because its there is an autoimmune 00:23:30.60\00:23:32.55 process there. Is this cow's milk or 00:23:32.58\00:23:34.68 kind of milk? Cow's milk, cow's milk. 00:23:34.71\00:23:36.73 Its very interesting but that's number one if 00:23:36.99\00:23:40.84 you have type I in your family and you don't 00:23:40.87\00:23:42.84 wanted to go on, momma's milk only for 00:23:42.87\00:23:46.98 the first year and don't introduce cows milk for 00:23:47.01\00:23:50.48 at least two years or any you know yogurt 00:23:50.51\00:23:52.65 or cheese or anything else, just leave it out 00:23:52.68\00:23:55.80 and that will markedly reduce the risk of 00:23:56.18\00:23:58.75 developing type I, type II if you have it think 00:23:58.78\00:24:03.85 about the illustration. Give time for your 00:24:03.88\00:24:07.46 body to assimilate that food and don't eat 00:24:07.49\00:24:10.55 a calorie you are not planning on burning, 00:24:10.58\00:24:12.47 because you know if you pack on that 00:24:13.66\00:24:15.59 weight for tomorrow and the next day and 00:24:15.62\00:24:18.31 a next day think of insulin and what he 00:24:18.34\00:24:20.49 is doing and what he has to do to get that 00:24:20.52\00:24:23.48 sugar out of the blood and into the cell and the 00:24:23.51\00:24:26.20 cell in need it. And if you eat food that's 00:24:26.23\00:24:30.58 high in fiber, and you stay away from refine 00:24:30.61\00:24:34.08 carbohydrates and you have two meals a day 00:24:34.11\00:24:37.35 and you exercise regularly, 00:24:38.06\00:24:40.12 much of the time you can reverse diabetes 00:24:42.61\00:24:44.75 and I can tell you even its late, even if you've 00:24:44.78\00:24:48.28 had diabetes a long time you can reduce the 00:24:48.31\00:24:51.76 amount of medicine that you are taking 00:24:51.79\00:24:53.54 in every case you can. If you are on insulin 00:24:53.91\00:24:57.45 you can reduce the amount of insulin you are taking, 00:24:57.94\00:25:00.31 but you know the important thing to 00:25:01.62\00:25:03.33 remember is if you don't, you don't have 00:25:03.36\00:25:05.31 any another change, don't worry because if 00:25:05.34\00:25:08.28 you are willing to check your sugar regularly 00:25:08.31\00:25:10.57 and follow your sugar and keep it as 00:25:11.82\00:25:14.25 normal as possible. If you keep your 00:25:14.28\00:25:17.93 blood pressure normal and you keep your 00:25:17.96\00:25:23.00 cholesterol down, normal and even 00:25:23.03\00:25:26.68 below if you have diabetes, you can live 00:25:26.71\00:25:30.54 with diabetes for a very long time and live well. 00:25:30.57\00:25:33.33 Diabetes is not a death now, it isn't. 00:25:33.88\00:25:37.69 It is reversible, it is preventable, 00:25:38.31\00:25:40.46 but if you not gonna go that way it's still every 00:25:40.79\00:25:44.51 treatable and it should be treated and it should 00:25:44.54\00:25:48.49 be treated well, you know. So, we can live well 00:25:48.52\00:25:51.95 with diabetes by, you can, by eating well. 00:25:51.98\00:25:54.44 Yes, you mentioned eating foods that are 00:25:54.47\00:25:56.27 reach in fiber to help release the blood sugar, 00:25:56.30\00:26:00.92 the sugar into the blood more slowly. 00:26:01.23\00:26:02.93 You mentioned not grazing, not eating 00:26:03.40\00:26:06.16 lots of meals and there is fair bit of 00:26:06.44\00:26:08.02 controversy out there. There are those who 00:26:08.05\00:26:09.82 promote eating many small meals but what 00:26:09.85\00:26:11.84 I hear you saying is two meals, three meals at 00:26:11.87\00:26:14.36 the most, through physical activity 00:26:14.39\00:26:17.47 and through managing, making sure that our 00:26:18.14\00:26:21.24 blood sugar is within normal ranges and 00:26:21.27\00:26:23.66 our blood pressure and that's sort of thing, 00:26:23.69\00:26:25.65 what other words of wisdom would you 00:26:26.00\00:26:27.56 share for those who either want to control 00:26:27.59\00:26:30.46 their diabetes or who want to prevent diabetes. 00:26:30.49\00:26:32.97 Well, let's just go over the A, B, C's of 00:26:33.52\00:26:35.81 diabetes, and I really touched on. 00:26:35.84\00:26:37.60 The first one we say A1C, that's hemoglobin A1C. 00:26:37.63\00:26:41.40 That is the measure of how your sugar has 00:26:42.29\00:26:44.50 been on average for the last three months, 00:26:44.53\00:26:47.14 and every diabetic should know their 00:26:48.07\00:26:50.55 hemoglobin A1C. No question about it. 00:26:50.58\00:26:53.31 If you don't know it, call your doctor, 00:26:53.34\00:26:55.93 if he doesn't know it go in and say I want one. 00:26:55.96\00:26:58.56 Your hemoglobin A1C should be six and half 00:26:59.20\00:27:03.36 or less, why? Because for every one point, 00:27:03.39\00:27:08.04 it's over four your risk of having heart attack 00:27:08.63\00:27:11.56 goes up, and it goes up 20 percent. 00:27:11.59\00:27:15.47 So number one, you want you're A1C under 00:27:15.64\00:27:18.39 six and half. Number two you want your 00:27:18.42\00:27:20.31 blood pressure at 120/80 or less. 00:27:20.34\00:27:23.29 That's what you want it, 120/80 or less, 00:27:24.42\00:27:27.43 and number three you want your LDL 00:27:28.24\00:27:30.44 cholesterol, that LDL cholesterol, 00:27:30.47\00:27:34.00 you want that under 100, probably down 00:27:34.45\00:27:38.91 in the 80's. If you do those things 00:27:39.34\00:27:41.66 your diabetes is gonna be in good control 00:27:42.45\00:27:44.71 even if you don't want to do what I tell you 00:27:45.17\00:27:46.74 to do with diet and exercise. 00:27:46.77\00:27:48.48 So it's oblivious to me that we really are 00:27:48.96\00:27:51.40 fearfully and wonderfully made. 00:27:51.57\00:27:53.43 What I learned today was that if you have 00:27:54.29\00:27:57.07 diabetes or like me, in case of, you know 00:27:57.10\00:28:00.25 you want to prevent it that there are things 00:28:00.28\00:28:01.69 that we can do through what we chose to eat, 00:28:01.72\00:28:03.73 how you chose to live.
