Right 00:01:46.02\00:01:47.92 Well that's great. 00:01:47.95\00:01:49.22 Our topic today is we're going to talk about a heart attack 00:01:49.25\00:01:52.70 and I understand we've collected questions from 00:01:52.73\00:01:54.82 all over the world regarding this tough topic. 00:01:54.85\00:01:58.57 We have, but before we get into that... 00:01:58.60\00:02:00.07 I've got kind of a snippet... 00:02:00.10\00:02:02.09 I've read some information and got some statistics 00:02:02.12\00:02:05.45 that are kind of interesting and I thought you might be 00:02:05.48\00:02:08.43 interested in it. 00:02:08.46\00:02:10.02 In the United States, every year, we spend 00:02:10.05\00:02:15.87 as Americans, 2.2 trillion dollars on healthcare per year 00:02:15.90\00:02:20.22 2.2 Wow! That's an amazing amount to me 00:02:20.25\00:02:23.09 In 2006 alone, we had an estimated 900,000 heart attacks 00:02:23.12\00:02:30.12 That's almost a million heart attacks a year 00:02:30.13\00:02:32.56 and we only have 300 million people here. Right... 00:02:32.59\00:02:34.86 So, you know, you've got 900,000 total 00:02:34.89\00:02:39.36 ...break it down, you've got 600,000 new heart attacks 00:02:39.39\00:02:44.11 300,000 of those are either 2nd, 3rd or 4th heart attack 00:02:44.14\00:02:49.48 So it's a situation where once you have the 1st heart attack, 00:02:49.51\00:02:53.36 it seems like it's still being repeated past that point 00:02:53.39\00:02:59.94 Out of this number, 42% died. 42%? 00:02:59.95\00:03:06.83 That's within about the first year, right? Right 00:03:06.84\00:03:09.51 Wow, that's an amazing number. 00:03:09.54\00:03:10.97 I mean, it really is staggering when you think of it... 00:03:11.00\00:03:13.41 just think... how big is your town? 00:03:13.44\00:03:18.00 Around the world, and this is another little bit of 00:03:18.03\00:03:19.93 information.. Around the world, 13 million people die 00:03:19.96\00:03:23.30 of cardiac disease very year. 13 million... wow 00:03:23.33\00:03:27.49 That's several large towns rolled into one... 00:03:27.52\00:03:29.67 Yeah, it's just an amazing number. 00:03:29.70\00:03:31.33 During the first half of the 21st century, 00:03:31.36\00:03:34.38 we're going to be looking at 1 billion deaths from 00:03:34.41\00:03:38.70 heart-related conditions. 00:03:38.73\00:03:39.85 I've heard that, and actually the prevalence every year 00:03:39.88\00:03:42.75 you know, is going up. Right 00:03:42.78\00:03:44.57 So this is a really important topic, 00:03:44.60\00:03:46.95 and I'm hoping that everyone out there that's listening 00:03:46.98\00:03:49.88 is going to tune-in because we don't want you to have 00:03:49.91\00:03:52.60 a heart attack, and we want you to 00:03:52.63\00:03:54.49 know a lot more about this subject. 00:03:54.52\00:03:56.37 It seems like it's an epidemic. 00:03:56.41\00:03:57.96 And I don't hear as much about heart disease, 00:03:57.99\00:04:00.37 as some other more trendy disease... Right 00:04:00.40\00:04:02.65 And just some of these statistics are why 00:04:02.68\00:04:06.40 Heartwise Ministries has been created. 00:04:06.43\00:04:08.19 Wow, well that's great... It's kind of interesting. 00:04:08.23\00:04:11.60 What we generally do when we get together 00:04:11.64\00:04:13.61 is take questions from people around the world basically. Okay 00:04:13.65\00:04:17.65 And we answer their questions. Okay... 00:04:17.68\00:04:21.55 And we try to give them an unbiased answer... Correct 00:04:21.58\00:04:25.15 Okay, well good... Let's go to some of these questions. 00:04:25.18\00:04:27.95 Okay, #1, "I'm a 50-year-old female with a family history 00:04:27.98\00:04:32.36 of both heart disease, and breast cancer" 00:04:32.39\00:04:34.62 Both parents suffered heart attacks in their 50s 00:04:34.65\00:04:37.53 "I worry about my health all the time" 00:04:37.56\00:04:40.91 "Someone told me my risk of having heart disease was 00:04:40.94\00:04:43.90 MUCH greater than breast cancer" 00:04:43.93\00:04:46.13 "Which is the greater risk?" 00:04:46.16\00:04:49.02 Okay, and that was from Chris, in Rome, Georgia 00:04:49.05\00:04:54.43 Well Chris, through the years, cancer has scared people 00:04:54.46\00:05:00.22 dramatically, and as it should... 00:05:00.25\00:05:03.10 But the prevalence's of heart disease, 00:05:03.13\00:05:05.76 cardiovascular disease, which includes heart attacks, 00:05:05.79\00:05:09.17 strokes, heart failure, 00:05:09.20\00:05:11.46 the funny heartbeats called "arrhythmias" 00:05:11.49\00:05:13.80 these have been increasing at far greater rate... 00:05:13.83\00:05:16.60 In fact, in women, the chance of having 00:05:16.63\00:05:19.57 heart-related disease is 00:05:19.60\00:05:21.60 GREATER than ALL of the diseases COMBINED! 00:05:21.63\00:05:24.23 Any disease that a female might have... 00:05:24.27\00:05:26.84 HEART DISEASE is far greater than all of them... 00:05:26.87\00:05:28.98 And if you look at the prevalences, 00:05:29.01\00:05:30.64 this is a major problem in America today... 00:05:30.67\00:05:32.71 we just NOT talking enough about this. 00:05:32.74\00:05:34.72 Are you telling me that that's a myth... 00:05:34.75\00:05:36.66 Why is it that the general public believes other than that? 00:05:36.69\00:05:41.06 Well, I think that, for females, breast cancer is a scary thing 00:05:41.09\00:05:46.36 ...it's very personal 00:05:46.39\00:05:47.54 It affects their whole, you know their figure... 00:05:47.57\00:05:50.78 It affects the way they look. 00:05:50.81\00:05:52.09 We heard a lot about this through the years... 00:05:52.12\00:05:54.30 and, so breast cancer awareness is very high. 00:05:54.33\00:05:56.90 And what we're trying to do is, 00:05:56.93\00:05:58.54 based on the numbers and statistics, let people know 00:05:58.57\00:06:00.98 that, no... we need to ALSO... 00:06:01.15\00:06:02.82 Now, you know, I'm not saying we should not focus on 00:06:02.85\00:06:05.51 breast cancer, but we also need to focus on the treatment, 00:06:05.54\00:06:09.05 prevention and awareness of cardiovascular disease... 00:06:09.08\00:06:12.23 Does it help Chris, to worry about the situation? 00:06:12.26\00:06:14.68 No it does not help to worry about this, 00:06:14.71\00:06:16.77 but it does help to be aware... 00:06:16.80\00:06:18.44 Because when you start being aware that there's a problem... 00:06:18.47\00:06:20.17 a lot of people out there think that they're not 00:06:20.93\00:06:22.42 even at risk of having a heart attack. 00:06:22.45\00:06:24.57 Those are the people we want to REACH the most. 00:06:24.60\00:06:27.27 And in this show and in the next show, 00:06:27.30\00:06:29.67 we're really going to talk about people... 00:06:29.68\00:06:31.07 What can I do right now in my life that may prevent 00:06:31.10\00:06:35.17 ME from having heart attack, 00:06:35.20\00:06:36.85 and being another one of these statistics. 00:06:36.88\00:06:38.70 You know, every day I hear about someone that 00:06:38.73\00:06:41.31 falls over suddenly from a heart attack... 00:06:41.34\00:06:43.15 whether it be an athlete, or a personal friend, 00:06:43.16\00:06:45.55 and we can stop this if we... there are ways to correct that 00:06:45.58\00:06:50.07 ...90% of heart attacks are preventable... 90% 00:06:50.10\00:06:54.05 and only 10% are purely genetics. 00:06:54.08\00:06:56.25 Well I have heard that 90% of most cardio-disease 00:06:56.28\00:07:00.57 is acquired... which is basically putting it back in our 00:07:00.60\00:07:04.65 court as far as our producing that problem. 00:07:04.68\00:07:09.18 So, Chris, I want to get back to your question... 00:07:09.21\00:07:11.75 As far as which is the greater risk, 00:07:11.78\00:07:13.79 ...Your risk from dying of heart disease is greater 00:07:13.82\00:07:17.02 than you dying of breast cancer... 00:07:17.05\00:07:18.93 in general, if there's no family history... Right 00:07:18.96\00:07:22.08 That's a good question. 00:07:22.11\00:07:23.59 All right, our 2nd question comes from 00:07:23.62\00:07:25.96 Ray in Flint, Michigan. 00:07:25.99\00:07:28.52 "The other day, while at work, 00:07:28.55\00:07:29.77 I had a funny feeling in my chest; this occurred after 00:07:29.80\00:07:33.75 I ate lunch but lasted for several hours. " 00:07:33.78\00:07:37.00 "Could I have had a heart attack?" 00:07:37.03\00:07:39.33 You know, Jim, my first thought that goes through my brain is 00:07:39.36\00:07:44.23 ...Where did you eat lunch, Ray? 00:07:44.26\00:07:45.42 ...Was it, you know, like at Taco Bell? 00:07:45.45\00:07:47.72 Oh yeah, Ray, I mean you got some bad food... 00:07:47.75\00:07:50.03 Ray could have gotten... You never know... too many chips 00:07:50.06\00:07:54.70 I hate to digress, but 00:07:54.74\00:07:56.07 right now, Ray, we're going to digress a second... 00:07:56.11\00:07:58.34 Did you know that 80 million people eat fast food every day. 00:07:58.38\00:08:02.59 Ray, you could have had some indigestion, no doubt about that 00:08:02.62\00:08:06.61 But you ALSO could have had a heart attack... 00:08:06.64\00:08:08.63 And that brings up... A lot of people don't even know 00:08:08.66\00:08:11.52 what symptoms of a heart attack are... 00:08:11.55\00:08:13.25 Dan, do you know what the symptoms of a 00:08:13.28\00:08:14.37 heart attack would be? 00:08:14.41\00:08:15.42 I would assume tightness of chest... Yes 00:08:15.45\00:08:18.87 Pain radiating down your left arm. Um hm 00:08:18.90\00:08:25.15 Shortness of breath. That's right 00:08:25.18\00:08:26.95 Not able to take a deep breath 00:08:26.98\00:08:31.86 I would assume would be one of the symptoms. Yeah 00:08:31.89\00:08:34.25 Pain in the back... 00:08:34.28\00:08:35.76 Well, you're doing pretty good with this, Dan 00:08:35.79\00:08:37.42 You must have been coached a little bit.. Taking a shot at it 00:08:37.45\00:08:39.96 Well anyway, Ray, any abnormal symptom that goes on 00:08:39.99\00:08:44.51 for a prolonged period of time... 00:08:44.54\00:08:45.94 And I've seen just about everything at practicing as a 00:08:45.97\00:08:49.00 cardiologist... from chest pain, to chest comfort, 00:08:49.03\00:08:52.25 to numbness in either arm, to an indigestion-like feeling, 00:08:52.28\00:08:55.93 to sudden shortness of breath, 00:08:55.96\00:08:57.43 to becoming overly fatigued, and tired... 00:08:57.44\00:08:59.68 But any prolonged symptom basically above the waist 00:08:59.71\00:09:03.61 COULD be a heart attack. 00:09:03.64\00:09:05.28 And you know, I almost always... 00:09:05.31\00:09:07.10 People will tell me when they're having a heart attack 00:09:07.13\00:09:09.57 most definitely... they think something is not right. 00:09:09.60\00:09:12.39 Now the older people, and diabetics have very 00:09:12.42\00:09:15.59 atypical symptoms... 00:09:15.62\00:09:16.84 Now when I say "atypical," everyone thinks... 00:09:16.87\00:09:18.56 "Oh, if I'm not having chest pain, 00:09:18.59\00:09:20.73 I can't be having a heart attack. " 00:09:20.76\00:09:22.26 But this indigestion, Ray, you know, 00:09:22.29\00:09:23.75 it might have been true indigestion, 00:09:23.78\00:09:25.61 or you might have been having a heart attack. 00:09:25.64\00:09:27.72 So any prolonged symptom that goes on for several hours 00:09:27.75\00:09:31.63 ...that's unexplained, if I were you, Ray, 00:09:31.66\00:09:34.62 I would get it checked out. 00:09:34.65\00:09:36.65 Now after you ate lunch, that could have been... acid reflux 00:09:36.68\00:09:41.04 ...yeah, it could have been indigestion 00:09:41.07\00:09:42.25 You know, I have some people that just pass gas... 00:09:42.29\00:09:44.59 and then it just sort of gets caught in there 00:09:44.62\00:09:47.03 for a long time, and they don't know why... 00:09:47.07\00:09:48.75 But I would still, with these statistics, 00:09:48.79\00:09:50.43 I would want to make sure I'm not having a heart attack. 00:09:50.44\00:09:55.58 Maybe Ray should pack his lunch next time... 00:09:55.61\00:09:58.11 just pack a lunch and not worry about it. 00:09:58.14\00:10:01.48 Let's go to question #3... from Dave in Idaho 00:10:01.51\00:10:04.68 Dave writes in and he says, "Is it possible to have a 00:10:04.71\00:10:08.67 heart attack without any cholesterol blockages?" 00:10:08.70\00:10:11.69 "My buddy and I were talking about basketball players 00:10:11.72\00:10:14.29 who have died suddenly. " 00:10:14.32\00:10:15.61 "Also, is there a test to tell WHO will suffer a heart attack?" 00:10:15.64\00:10:19.98 You know, this kind of goes on all the time... 00:10:19.99\00:10:23.80 We hear of football, basketball, a lot of young people 00:10:23.83\00:10:27.91 having problems like this... Right 00:10:27.94\00:10:29.78 Dying on the field... Is there anything that... 00:10:29.81\00:10:34.78 Yeah, let's sort of break Dave's question into 3 parts 00:10:34.81\00:10:37.99 Dave, the first thing is... 00:10:38.02\00:10:39.34 Is it possible to have a heart attack without any cholesterol 00:10:39.35\00:10:43.48 blockages? 00:10:43.51\00:10:44.61 The answer to that question is "Yes" 00:10:44.62\00:10:46.66 What would be some of the reasons that would lead to that? 00:10:46.69\00:10:49.41 The most common reason that we have... 00:10:49.44\00:10:51.69 when there are no blockages, is a spasm. 00:10:51.72\00:10:54.87 Let me just back up and for our audience, 00:10:54.90\00:10:56.73 the most common way we have a heart attack is 00:10:56.76\00:10:59.70 we have the small cholesterol plaque 00:10:59.73\00:11:02.09 usually less than 50% in diameter, 00:11:02.12\00:11:04.35 that ruptures. Right 00:11:04.36\00:11:06.32 And, Dan, if I cut you here, what would you do with your skin 00:11:06.35\00:11:10.64 Start to bleed. Right, you'd start to bleed, 00:11:10.67\00:11:12.20 but then sooner or later, you'd start to clot off... Right 00:11:12.23\00:11:15.81 Well, when a cholesterol plaque ruptures, 00:11:15.84\00:11:17.59 that process is going on in the artery of the heart. 00:11:17.62\00:11:20.36 Okay, so it's interior. Right 00:11:20.39\00:11:21.93 And sooner or later, you've got that clot... 00:11:21.96\00:11:23.46 no blood goes out... that has a heart attack. 00:11:23.49\00:11:25.56 Most heart attacks are like that 00:11:25.57\00:11:27.15 Even in young people now, they're starting to 00:11:27.18\00:11:29.69 get these plaques. 00:11:29.72\00:11:30.97 Now, what would be the reason for that? 00:11:31.00\00:11:34.11 Is it because they don't eat properly? Exercise? Yes 00:11:34.14\00:11:37.21 It's usually they don't eat well... 00:11:37.24\00:11:39.00 their diets... hypertension, diabetes, all these things 00:11:39.03\00:11:42.34 that can contribute to that. Right 00:11:42.37\00:11:44.52 So that's the most common cause. 00:11:44.55\00:11:46.14 Another type of heart attack is when the arteries just 00:11:46.17\00:11:48.60 gradually builds up with cholesterol until they're just 00:11:48.63\00:11:51.27 all clogged up. 00:11:51.30\00:11:52.99 But in the young people, sometimes we have this 00:11:53.02\00:11:55.46 phenomenon called "spasm" 00:11:55.49\00:11:57.32 That's where the artery shuts down, no blood goes through... 00:11:57.35\00:12:00.70 you have a heart attack! 00:12:00.73\00:12:01.86 Which people are at higher risk for that? 00:12:01.89\00:12:04.88 If you remember, several years ago, 00:12:04.91\00:12:07.22 the basketball player, Lenny Bias 00:12:07.25\00:12:09.32 He was from the University of Maryland, I think. 00:12:09.35\00:12:11.84 He had drugs in his system that caused the artery to spasm down 00:12:11.87\00:12:17.00 Drugs can cause this to happen... 00:12:17.03\00:12:18.74 Methamphetamines... 00:12:18.77\00:12:20.11 Some people that have cigarettes can have spasm. 00:12:20.14\00:12:22.86 It's been reported that people that have low oxygen levels 00:12:22.89\00:12:26.14 ...you get up really high, you know, high altitudes 00:12:26.17\00:12:29.16 The have a chance to have those arteries... 00:12:29.19\00:12:30.62 Maybe exercising at high altitudes? 00:12:30.65\00:12:32.16 That can cause spasm. 00:12:32.19\00:12:33.75 Now another type of heart attack that we RARELY see is 00:12:33.76\00:12:36.34 when a blood clot from somewhere else in the body 00:12:36.35\00:12:39.27 floats into an artery... so that's a rare one. 00:12:39.30\00:12:41.93 So, Dave, you can have a heart attack without cholesterol 00:12:41.96\00:12:46.05 blockages even though it's not the most common way to have it. 00:12:46.08\00:12:49.27 Now what was his next question? 00:12:49.30\00:12:51.33 His other one was... "Is there a test to tell 00:12:51.36\00:12:54.03 who will suffer a heart attack? 00:12:54.06\00:12:55.43 That's kind of an interesting question because 00:12:55.46\00:12:57.05 we ALL would like to take that test... 00:12:57.08\00:12:59.10 No one can tell who is going to have a heart attack, 00:12:59.13\00:13:02.03 and who doesn't. 00:13:02.06\00:13:03.05 You can be sitting here fine one second, 00:13:03.08\00:13:05.48 and if you had a plaque that ruptured, 00:13:05.51\00:13:06.92 you could have a heart attack. 00:13:06.93\00:13:08.01 However, there are things that put a person at a higher risk 00:13:08.04\00:13:11.75 of having a heart attack. 00:13:11.78\00:13:13.37 We're going to talk about those a little bit more later. 00:13:13.40\00:13:15.99 Now, getting back to the basketball players... 00:13:16.02\00:13:18.39 Most of them that die suddenly don't necessarily 00:13:18.42\00:13:21.15 die of a heart ATTACK. 00:13:21.18\00:13:23.25 They frequently have these rhythm problems... 00:13:23.28\00:13:25.86 They're called "arrhythmias" 00:13:25.89\00:13:27.13 and it's usually because they have an abnormal heart. 00:13:27.16\00:13:29.22 Either the heart has had an infection, 00:13:29.25\00:13:31.00 like a virus that hurts it... 00:13:31.03\00:13:32.51 they have a congenital abnormality... 00:13:32.54\00:13:34.54 they have some electrical problems... 00:13:34.57\00:13:36.53 Most of the young people have these electrical problems 00:13:36.56\00:13:38.87 that predispose them to dying suddenly. 00:13:38.90\00:13:41.17 What is an electrical problem? 00:13:41.20\00:13:42.73 Well, an electrical problem is the wires 00:13:42.76\00:13:46.30 that make your heart pump. 00:13:46.33\00:13:47.90 For some reason, you know, the signals aren't doing well 00:13:47.93\00:13:51.17 and the heart either pumps too fast, usually... 00:13:51.18\00:13:53.69 doesn't fill up with blood, 00:13:53.72\00:13:55.09 doesn't pump blood to your body 00:13:55.12\00:13:56.56 because the electrical system is like a series of wires 00:13:56.59\00:13:59.20 doesn't work well... Okay 00:13:59.23\00:14:00.71 Then, you're put out of rhythm... That's right 00:14:00.74\00:14:03.19 And that's how most of the young, young people 00:14:03.22\00:14:06.11 pass away suddenly from heart disease... 00:14:06.14\00:14:08.04 And now, they're even talking about making athletes 00:14:08.07\00:14:10.54 have some screening EKGs and maybe even... 00:14:10.57\00:14:13.25 physical, before they... get into the program 00:14:13.28\00:14:15.63 get into the program... And it's a big deal... 00:14:15.66\00:14:17.54 you always hear about a young athlete that dies 00:14:17.58\00:14:19.42 I mean, it always makes the media attention... Right 00:14:19.45\00:14:22.20 But heart attacks are FAR more common than that 00:14:22.21\00:14:24.97 phenomena happening... 00:14:25.00\00:14:26.70 So I hope that answered your question, Dave 00:14:26.73\00:14:29.02 No test to tell for heart attack 00:14:29.03\00:14:30.48 I think that's the key point for Dave... Okay 00:14:43.22\00:14:44.72 I'll just ask you the first to start out with... Okay 00:14:44.75\00:14:47.29 "How do cardiologists determine if you have had a heart attack?" 00:14:47.32\00:14:51.14 And I'm assuming, John, that you mean an old heart attack" 00:14:51.17\00:14:55.77 If you're having a new heart attack, 00:14:55.80\00:14:59.19 the way we tell, is your symptoms... 00:14:59.22\00:15:00.88 are you having symptoms? 00:15:00.91\00:15:02.03 We check your EKG. 00:15:02.06\00:15:03.90 If we're still not sure, there's some blood work we can do 00:15:03.93\00:15:06.97 to see if there's been heart damage. 00:15:07.00\00:15:09.05 There's also a sound-wave test that we can use to see 00:15:09.08\00:15:12.24 if the heart is moving or not. 00:15:12.27\00:15:13.51 Can you visually see that in one of these tests?
