James Marcum and I'm a cardiologist 00:00:57.61\00:01:01.27 with the Chattanooga Heart Institute. 00:01:01.30\00:01:02.98 Now for those of you who might not be familiar 00:01:03.02\00:01:06.33 with medical jargon, a cardiologist is a doctor 00:01:06.37\00:01:09.61 who specializes in internal medicine and specifically 00:01:09.65\00:01:12.62 focuses on treatment and prevention 00:01:12.65\00:01:15.04 of diseases of the heart and blood vessels. 00:01:15.07\00:01:17.35 This morning I have chosen the topic of myocardial infarction 00:01:17.39\00:01:21.57 to talk about, because it's so prevalent in our society. 00:01:21.60\00:01:25.20 In fact,42% of all heart attacks and cardiovascular disease 00:01:25.24\00:01:28.81 are the source of death in America. 00:01:28.84\00:01:31.30 In talking about this important topic 00:01:32.11\00:01:34.78 we have Shelley Quinn joining us this morning. 00:01:34.82\00:01:37.27 And I'm so happy that she's with us to help us 00:01:37.30\00:01:39.68 talk about this topic that I think everyone is involved 00:01:39.72\00:01:43.33 with either loved ones or they know someone 00:01:43.36\00:01:46.52 that's had a heart attack. 00:01:46.55\00:01:47.52 So Shelley, I wanna thank you for joining us this morning. 00:01:47.62\00:01:50.01 Thank you. I'm just so excited to be here. 00:01:50.04\00:01:52.81 And you know, every one of us has a heart. 00:01:52.85\00:01:55.73 So we all know someone or we have experienced 00:01:55.77\00:01:58.76 problems of ourselves or we want to avoid 00:01:58.79\00:02:01.34 experiencing these problems. That's right. 00:02:01.38\00:02:03.90 So, I say that again... 00:02:03.93\00:02:07.79 A myocardial infarction or a heart attack. 00:02:07.83\00:02:10.00 That's what it means, it's a heart attack. 00:02:10.03\00:02:12.13 That's what we're talking about. So that's what 00:02:12.17\00:02:13.49 we're gonna be talking about. 00:02:13.60\00:02:14.76 In fact I've gathered questions from allover the world 00:02:14.79\00:02:18.45 pertaining to hearts. And we're gonna talk about 00:02:18.48\00:02:21.72 myocardial people's own questions. 00:02:21.75\00:02:23.90 These are people that write in with serious concerns, they want 00:02:23.94\00:02:26.60 their questions answered. And in the course 00:02:26.63\00:02:28.58 of the conversation today, I'm hoping we can learn 00:02:28.62\00:02:30.99 a lot of good things that are practical about heart attacks. 00:02:31.03\00:02:34.04 I'm looking forward to it. 00:02:34.07\00:02:35.50 And because this is such an important topic, 00:02:35.54\00:02:37.80 Jim, why don't we pray before we begin. 00:02:37.83\00:02:40.66 Ok,would you like to. Amen. 00:02:56.36\00:02:59.56 Well, Jim has a lot of questions here, 00:02:59.59\00:03:02.11 that have come from around the world. 00:03:02.15\00:03:05.17 And he has a ministry called "Heart Wise" 00:03:05.20\00:03:08.19 in Chattanooga, Tennessee. 00:03:08.22\00:03:10.03 So you're out there teaching people 00:03:10.07\00:03:11.97 how to be healthy and to avoid these problems. 00:03:12.01\00:03:15.46 And our first question comes from Carol in Michigan. 00:03:15.50\00:03:18.85 Carol says: I am a 44 year old healthy male. 00:03:18.88\00:03:22.32 A recent physical at work turned out well. 00:03:22.35\00:03:25.74 However, I have a strong family history of heart attack. 00:03:25.78\00:03:29.56 My dad died in his early 50s of a heart attack 00:03:29.59\00:03:33.06 and my older brother recently had a stent placed. 00:03:33.09\00:03:36.53 Could I also have a heart attack? 00:03:36.56\00:03:39.52 First, could you explain to us what a stent is? 00:03:39.56\00:03:42.59 Yeah. A stent is a device. 00:03:42.62\00:03:45.19 When a person has an acute myocardial infarction 00:03:45.23\00:03:47.99 or a heart attack, what that is, we don't have 00:03:48.04\00:03:50.42 enough blood reaching the muscles of the heart. 00:03:50.46\00:03:52.81 When that happens, the muscle dies. 00:03:52.84\00:03:55.45 And when the muscle dies, lots of bad things happen. 00:03:55.49\00:03:58.25 Funny heart beats can develop 00:03:58.28\00:04:00.09 You see the defibrillators that shock people. 00:04:00.12\00:04:02.77 That's cause of these funny heart beats that develop. 00:04:02.81\00:04:04.98 The heart can go into failure and you can't breathe 00:04:05.01\00:04:07.15 because of fluid in the lungs. 00:04:07.18\00:04:09.10 Lots of bad things can happen quickly. 00:04:09.14\00:04:11.31 So in an acute heart attack you need to get care right away. 00:04:11.34\00:04:15.27 Sometimes when a person is having a heart attack, 00:04:15.30\00:04:18.11 right away we bring him in, we go up into the heart, 00:04:18.15\00:04:20.83 we find out where that blockage is and we open it up. 00:04:20.86\00:04:23.90 We put a metal device that holds that artery open, 00:04:23.93\00:04:26.94 so blood can flow out to that muscle 00:04:26.97\00:04:29.03 so that muscle can work properly. 00:04:29.07\00:04:31.10 And if we can get a heart attack within the first hours, 00:04:31.13\00:04:34.34 there's almost never residual damage done to the heart. 00:04:34.38\00:04:37.08 It's when people sit at home and they don't get help 00:04:37.12\00:04:39.79 "It's heart burn, I'm not worried about it." 00:04:39.82\00:04:42.07 That's when we have a lot of problems with this. 00:04:42.10\00:04:45.44 I don't even wanna ask cause I've done the same. 00:04:45.47\00:04:48.74 So, since he had a good physical but he's got this family history 00:04:48.78\00:04:54.44 Is he at risk for a heart attack? 00:04:54.47\00:04:56.16 Yes, all of us in America are at risk of a heart attack. 00:04:56.20\00:04:59.78 In fact,90% of the heart attacks 00:04:59.81\00:05:02.53 that we have in America are acquired. 00:05:02.57\00:05:04.67 That means we give them to ourselves, 00:05:04.71\00:05:06.78 what lifestyles we have. 00:05:06.81\00:05:08.37 So he has a family history, so is definitely at risk. 00:05:08.40\00:05:11.91 So he could have a heart attack. 00:05:11.94\00:05:13.63 I wanna just throw up a slide here 00:05:13.67\00:05:15.36 and sort of give you the magnitude 00:05:15.39\00:05:17.05 for cardiovascular disease. 00:05:17.08\00:05:18.27 Myocardial infarction in the United States. 00:05:18.31\00:05:21.46 For instance, people don't realize this but it accounts 00:05:21.50\00:05:23.80 deaths annually. 00:05:23.83\00:05:26.81 That's almost 1,000,000 deaths every year. 00:05:26.85\00:05:30.82 That is a lot of people that die. 00:05:30.85\00:05:32.98 One heart attack happens every 34 seconds. 00:05:33.02\00:05:37.45 And every year 00:05:37.48\00:05:39.01 400,000 to 460,000 people never make it to the Emergency Room. 00:05:39.05\00:05:43.41 They collapse, they fall over dead 00:05:43.44\00:05:45.49 before they reach the Emergency Room. 00:05:45.53\00:05:48.11 I think those are staggering statistics. 00:05:48.15\00:05:50.66 And you have to really be aware that a heart attack 00:05:50.70\00:05:53.57 is gonna be the thing that you have to worry about 00:05:53.60\00:05:56.44 whenever you have chest pain or discomfort. 00:05:56.47\00:05:58.72 So Jim, is there a genetic predisposition to this 00:05:58.76\00:06:02.47 as well as our lifestyle? 00:06:02.50\00:06:04.72 Yes, we know that some people have bad genes. 00:06:04.76\00:06:08.02 And if you have bad genetics and you have bad lifestyle habits, 00:06:08.06\00:06:13.59 this accelerates the development of this problem. 00:06:13.62\00:06:16.74 And this problem is usually called arteriosclerosis. 00:06:16.77\00:06:19.83 A big name that says cholesterol gets jumped up inside 00:06:19.86\00:06:22.89 the arteries of our heart. 00:06:22.92\00:06:24.80 Not only in our heart, this is going on in the blood vessels 00:06:24.84\00:06:27.51 to our brain which can cause strokes. 00:06:27.54\00:06:29.41 In the aorta which can weaken it and cause aneurysms. 00:06:29.45\00:06:32.88 In the legs, in the kidneys. 00:06:32.91\00:06:34.43 All the blood vessels can be affected 00:06:34.47\00:06:36.30 by this disease process. 00:06:36.33\00:06:37.90 If you have a family history, this even puts you 00:06:37.94\00:06:40.18 at a higher risk of having a heart attack. 00:06:40.21\00:06:42.36 I have to throw something in here because I remember 00:06:42.40\00:06:45.52 for the longest time when I'd hear the word cholesterol 00:06:45.56\00:06:48.65 and they'd say to avoid cholesterol. 00:06:48.68\00:06:50.67 I think,ok,what's got cholesterol in it? 00:06:50.70\00:06:53.52 What's got cholesterol in it? 00:06:53.55\00:06:54.86 And then when I finally learned I was embarrassed. 00:06:54.90\00:06:57.52 But would you please explain to our audience 00:06:57.55\00:07:00.36 what has cholesterol in it, how do you avoid these things? 00:07:00.40\00:07:04.86 For instance, one egg has about 200 milligrams of cholesterol. 00:07:04.90\00:07:09.33 The recommended daily requirement for cholesterol 00:07:09.36\00:07:12.26 is 300. But most people do not even 00:07:12.29\00:07:15.49 realize the fact that you do not need to eat any cholesterol 00:07:15.52\00:07:18.95 in your diet. Did you know that? 00:07:18.98\00:07:20.47 Your liver makes all the cholesterol your body needs. 00:07:20.51\00:07:23.79 However, in westernized societies through the years 00:07:23.82\00:07:26.86 we've developed this diet that's high in cholesterol. 00:07:26.89\00:07:29.90 We have lots of meat in our diet, 00:07:29.93\00:07:31.62 animal based products which are high in cholesterol. 00:07:31.66\00:07:34.42 Of course, the organ that has the most is the liver. 00:07:34.46\00:07:37.19 And I know some people that love to eat liver. 00:07:37.22\00:07:40.28 But liver is packed, packed with cholesterol. 00:07:40.31\00:07:43.75 So, the animal based diets, the cheese, the eggs, 00:07:43.78\00:07:47.19 all of these are packed with cholesterol. 00:07:47.22\00:07:49.96 This is the enemy when we're dealing 00:07:49.99\00:07:52.51 with cardiovascular disease. 00:07:52.54\00:07:53.88 So, it's smarter to go more towards the plant based diets. 00:07:53.92\00:07:58.05 Yes, that is the ideal goal. A lot of people have problems. 00:07:58.09\00:08:02.19 This is sort of the ideal goal. 00:08:02.22\00:08:04.03 We wanna move people towards that. 00:08:04.07\00:08:05.81 But I found people that sometimes they just 00:08:05.85\00:08:07.60 can't give up everything at once. 00:08:07.63\00:08:09.03 So we try to encourage them, to make changes slowly. 00:08:09.07\00:08:11.78 First try this, maybe substitute this, eat more fresh 00:08:11.81\00:08:14.79 fruits and vegetables, then they're able to get it. 00:08:14.82\00:08:17.94 As they feel better and more alive and more healthy 00:08:17.98\00:08:21.03 They're gonna say: "I don't think I can eat this anymore," 00:08:21.07\00:08:23.29 "cause I feel so much better." 00:08:23.32\00:08:24.54 I will attest to that. I was one that made 00:08:24.58\00:08:26.82 the changes slowly and continue to make the changes. 00:08:26.86\00:08:29.63 Here is a question from Anne in Canada. 00:08:29.66\00:08:32.61 And Anne writes: "Is it more likely for a woman" 00:08:32.65\00:08:36.67 "to develop cardiovascular disease or breast cancer?" 00:08:36.71\00:08:39.89 Anne, that is a wonderful question because cancer 00:08:39.92\00:08:43.29 gets so much publicity in the United States. 00:08:43.32\00:08:46.66 Women are more scared of cancer 00:08:46.69\00:08:48.98 than they are of cardiovascular disease. 00:08:49.02\00:08:51.18 But the chances of having a cardiovascular disease 00:08:51.22\00:08:55.01 is much higher than you would expect. 00:08:55.04\00:08:57.82 The next slide will attest to. 00:08:57.85\00:09:00.53 The prevalence of myocardial infarction in women 00:09:00.57\00:09:03.91 is a lot higher than most people think. 00:09:03.94\00:09:06.65 For instance, one in every 2.5 women die 00:09:06.69\00:09:09.33 of cardiovascular disease. I think that's staggering. 00:09:09.37\00:09:12.87 That's unbelievable. 00:09:12.90\00:09:13.90 One in 30 only, die of breast cancer. 00:09:13.94\00:09:16.59 25% of women who actually die of cardiovascular disease 00:09:16.63\00:09:21.70 are unaware of it. 00:09:21.73\00:09:23.29 That's unbelievable. 00:09:23.32\00:09:24.81 There's two things that come to my mind right away. 00:09:24.85\00:09:27.38 Number 1: For breast cancer, once you're 00:09:27.41\00:09:31.03 over the age of 50.Like I get my mammogram annually. 00:09:31.07\00:09:34.53 Between that and self examination 00:09:34.56\00:09:39.09 that's the way that I can know I'm cancer free. 00:09:39.12\00:09:42.01 But you don't really hear a lot about women and heart attacks. 00:09:42.04\00:09:46.09 I mean you don't hear doctors talking to you much about it. 00:09:46.13\00:09:50.17 So, what should I do to be 00:09:50.20\00:09:53.65 tested or to make sure that I'm not developing? 00:09:53.69\00:09:56.24 The first thing to do is to be aware that it's such 00:09:56.28\00:10:00.09 a high prevalence. Look for symptoms. 00:10:00.12\00:10:02.30 When you go and get your routine exam 00:10:02.34\00:10:04.49 bring this up to your doctor. 00:10:04.52\00:10:05.85 "Listen, I think that I might have to be screened" 00:10:05.89\00:10:09.78 "for cardiovascular disease." 00:10:09.81\00:10:11.01 There's things like blood work that can do 00:10:11.05\00:10:12.74 to make sure that your cholesterol is good 00:10:12.77\00:10:14.43 to lower your risk. 00:10:14.46\00:10:15.68 Check your blood pressure. Sometimes if you have some 00:10:15.72\00:10:18.00 symptoms you're not sure about, they can screen you 00:10:18.03\00:10:20.24 with a stress test or they walk you to see if you have symptoms. 00:10:20.28\00:10:23.33 There are tests that can measure your blood flow 00:10:23.36\00:10:26.14 and also look at risks that you might develop. 00:10:26.17\00:10:28.94 Those figures, I thought were amazing. 00:10:28.97\00:10:31.38 One in 2.5 vs. one in 30. That's very high. 00:10:31.42\00:10:35.88 To answer to Anne's question, Anne it's much more likely, 00:10:35.92\00:10:40.65 almost 10 times more likely that 00:10:40.68\00:10:42.39 you're gonna have a cardiovascular disease 00:10:42.43\00:10:44.14 than you are gonna have breast cancer. 00:10:44.17\00:10:45.81 And we hear so much more about breast cancer 00:10:45.85\00:10:49.03 than of cardiovascular disease. 00:10:49.06\00:10:50.92 Our third question comes from Raymond in Australia. 00:10:50.96\00:10:56.02 Raymond writes: "My friend Charlie" 00:10:56.05\00:10:59.33 "has chest pain daily. We work as guides in Australia." 00:10:59.37\00:11:03.83 "He says these episodes are heart attacks." 00:11:03.86\00:11:06.65 "Is there a difference between chest pain" 00:11:06.68\00:11:09.44 "and a heart attack?" 00:11:09.47\00:11:10.63 Yes. 00:11:10.66\00:11:11.63 That's the core of sort of diagnosing. 00:11:11.64\00:11:14.08 Lots of things can cause chest pain. 00:11:14.12\00:11:16.63 For instance, chest pain when you touch, 00:11:16.66\00:11:19.14 that can be a muscle. 00:11:19.17\00:11:20.26 Chest pain when you breathe, that could be related to your 00:11:20.30\00:11:23.08 lungs. Some of the hall-marks of chest pain coming from the heart 00:11:23.11\00:11:25.95 is pain when you do something. 00:11:25.98\00:11:27.56 Like you run up a hill, you do some exercise. 00:11:27.60\00:11:30.42 That's when the heart works. 00:11:30.45\00:11:31.76 And if the arteries are blocked up, you're gonna have 00:11:31.80\00:11:34.41 a symptom when the heart has to work hard. 00:11:34.44\00:11:36.40 And when you stop, it usually goes away. 00:11:36.43\00:11:38.36 We call that symptom "angina". 00:11:38.39\00:11:40.50 We also have the other symptoms. 00:11:40.53\00:11:42.57 We have pain that doesn't go away, or discomfort 00:11:42.61\00:11:44.72 that doesn't go away. We call that a "heart attack". 00:11:44.76\00:11:48.08 Let me show you the next slide here. 00:11:48.11\00:11:50.79 We're talking a little bit about heart attack. 00:11:50.83\00:11:52.57 Now there's different types of heart attacks that can happen. 00:11:52.60\00:11:54.83 Remember, heart attack is when the heart muscle 00:11:54.86\00:11:57.70 does not get enough blood and it starts to malfunction. 00:11:57.74\00:12:00.42 One way that can happen is the gradual occlusion of a vessel. 00:12:00.46\00:12:04.33 The cholesterol gradually piles up. 00:12:04.36\00:12:06.88 60%,70%,80%.This could take years to develop. 00:12:06.92\00:12:10.87 That's one. And then it gradually gets clogged up. 00:12:10.90\00:12:13.87 Usually these patients have symptoms of chest pain 00:12:13.90\00:12:16.51 before they have the heart attack. 00:12:16.54\00:12:17.90 The second one is a spasm. 00:12:17.93\00:12:20.67 That's when the artery, for a certain reason 00:12:20.71\00:12:23.55 spasms down and closes. 00:12:23.58\00:12:25.30 When it spasms down, downstream doesn't get enough blood. 00:12:25.34\00:12:28.47 Frequent causes of that would be drug abuse, 00:12:28.50\00:12:30.91 we see people that have cocaine, they have spasms. 00:12:30.95\00:12:33.99 People that smoke sometimes get spasms. 00:12:34.02\00:12:35.94 People that go in high altitudes can have spasms. 00:12:35.97\00:12:38.92 That don't get enough oxygen can have spasms, 00:12:38.96\00:12:41.90 it could be very devastating. 00:12:41.93\00:12:43.53 A third type of heart attack is actually the most common. 00:12:43.57\00:12:47.02 That's when we have a little plaque inside that blows open, 00:12:47.05\00:12:50.83 it ruptures suddenly. 00:12:50.86\00:12:52.22 Nothing predicts it, that plaque ruptures and then 00:12:52.26\00:12:55.30 the body sees that it's damaged. Whenever you've had a cut 00:12:55.34\00:12:58.26 on your hand, you know what happens when you get cut. 00:12:58.29\00:13:00.76 When there's damage, the body clogs it all. 00:13:00.79\00:13:02.57 So suddenly you clog off that artery and downstream 00:13:02.61\00:13:06.31 from that it doesn't have any blood 00:13:06.34\00:13:08.24 and you have a myocardial infarction. 00:13:08.27\00:13:10.12 The last one on the slide is a very uncommon one. 00:13:10.16\00:13:13.44 That was an emboli or some clot in your body 00:13:13.47\00:13:16.06 floating against the arteries. 00:13:16.09\00:13:17.38 That's very uncommon, we usually see that in people with 00:13:17.42\00:13:19.81 heart valves or clots in the heart. 00:13:19.84\00:13:21.74 We don't see that as often. 00:13:21.77\00:13:23.11 But as far as getting back to Raymond here, 00:13:23.15\00:13:26.78 his episodes are probably not heart attacks 00:13:26.82\00:13:30.42 if they're happening daily. 00:13:30.45\00:13:32.10 But if they're happening when he does something, 00:13:32.14\00:13:34.18 that could be a warning that the heart 00:13:34.21\00:13:35.50 is not getting enough blood, that could be angina. 00:13:35.54\00:13:38.46 But also could be a lot of other things. 00:13:38.49\00:13:40.86 In fact, I see people every day that most of the time, 00:13:40.90\00:13:44.38 they have chest pain that it's not the heart, 00:13:44.41\00:13:46.59 but you can't afford to miss it if it is the heart. 00:13:46.63\00:13:48.92 Jim, that's the problem because... 00:13:48.95\00:13:51.93 Recently my husband's best friend had an episode 00:13:51.97\00:13:57.08 of chest pain and he had his wife take him 00:13:57.11\00:14:00.43 to the emergency room and it turned out he had a 90% blockage 00:14:00.47\00:14:04.56 and had he waited any longer, he would have been dead. 00:14:04.59\00:14:08.07 It's what they told him. He said it wasn't that severe 00:14:08.10\00:14:11.55 but if I'm one of these people, I just nearly have to be 00:14:11.58\00:14:15.87 lying on my death bed to go to a doctor. 00:14:15.90\00:14:18.29 Do you just say ok, I'm gonna bite the bullet 00:14:18.33\00:14:22.79 and go and check this out cause it's not worth the risk? 00:14:22.83\00:14:25.77 I would just look at prevalences here, 00:14:25.80\00:14:28.55 I mean cardiovascular disease is so common. 00:14:28.59\00:14:31.89 If I had chest pain in my chest and I didn't know what it was, 00:14:31.93\00:14:35.74 for sure I would get it checked out because look at 00:14:35.77\00:14:38.12 the consequences if you don't. Look at the consequences. 00:14:38.16\00:14:42.29 Jim, I had microvalve prolapse. 00:14:42.32\00:14:45.27 So you probably had pain quite a bit. 00:14:45.31\00:14:47.69 I had pain all the time, I mean very often. 00:14:47.73\00:14:50.27 And I had a cardiologist once tell me, I said: 00:14:50.30\00:14:52.81 "I feel like I'm having a heart attack." 00:14:52.84\00:14:54.89 He said: "Don't worry, you're not." 00:14:54.92\00:14:56.25 And I said: "When will I know if I am?" 00:14:56.29\00:14:58.94 He said: "If you fall on the floor, you don't get up." 00:14:58.98\00:15:01.32 "That was a heart attack." 00:15:01.35\00:15:03.11 I know. So you kind of weigh 00:15:03.15\00:15:04.86 these things but the older I get the more I'm trying to be 00:15:04.90\00:15:08.84 serious about, if it is something that seems to 00:15:08.88\00:15:12.68 continue. You know Shelley, it might feel different from time 00:15:12.72\00:15:16.49 to time, you know, it might feel different than that time. 00:15:16.52\00:15:18.66 Inherently, usually when the heart's involved, 00:15:18.69\00:15:20.69 you also get short of breath, too. 00:15:20.72\00:15:22.57 That might help you distinguish between the two. 00:15:22.61\00:15:25.97 But microvalve prolapse is very difficult because that's 00:15:26.01\00:15:28.61 sort of an ongoing episode of chest pain. 00:15:28.64\00:15:31.32 And when you have that situation you really have to 00:15:31.36\00:15:34.34 know your body and know what's microvalve prolapse 00:15:34.37\00:15:37.32 and what might be different. 00:15:37.35\00:15:38.68 It's a scary feeling when you have any type 00:15:38.72\00:15:41.34 of pain in your chest. 00:15:41.37\00:15:42.36 Absolutely, but for those of you who are at home, 00:15:42.40\00:15:44.92 don't sit around and wait. Had my husband's best friend 00:15:44.96\00:15:48.56 waited, he would not be with us today. 00:15:48.59\00:15:50.61 Ok,here's a question from John in California. 00:15:50.65\00:15:53.63 Ok. And John writes: 00:15:53.66\00:15:55.01 "My grandmother just fell over dead and the doctors said" 00:15:55.04\00:15:58.35 "she had a heart attack." 00:15:58.38\00:15:59.64 "Is it possible to have a heart attack without symptoms?" 00:15:59.68\00:16:02.69 That is an excellent question and the answer 00:16:02.72\00:16:05.51 to that question is yes. 00:16:05.54\00:16:07.29 The groups that tend to have heart attacks 00:16:07.33\00:16:10.22 that don't have symptoms are the elderly and the diabetics. 00:16:10.26\00:16:14.84 Remember diabetics, their nervous systems 00:16:14.87\00:16:17.09 don't feel quite like everyone else's. 00:16:17.13\00:16:19.28 You've seen diabetics, they don't feel the bottom of their 00:16:19.32\00:16:22.72 feet. Sometimes they don't have sensations in their chest. 00:16:22.75\00:16:25.25 Also the elderly, sometimes they just get fatigued 00:16:25.28\00:16:27.84 or short of breath. 00:16:27.87\00:16:29.08 That could be a manifestation of a heart attack. 00:16:29.12\00:16:31.84 This is a good time, I think, to talk about some of the more 00:16:31.88\00:16:35.15 common symptoms of a heart attack. 00:16:35.18\00:16:36.35 So let's go ahead and look at the next slide here. 00:16:36.39\00:16:39.25 Symptoms of myocardial infarction. 00:16:39.28\00:16:42.33 One is chest discomfort. It might be pain, 00:16:42.37\00:16:45.02 it might be a heavy feeling on the chest. 00:16:45.06\00:16:47.64 It might be radiation of burning up into the jaw, the back 00:16:47.68\00:16:51.23 or the left arm, the shoulders. 00:16:51.26\00:16:52.70 Another symptom of a heart attack could be just 00:16:52.74\00:16:55.23 extreme shortness of breath. 00:16:55.26\00:16:56.89 I can't breathe, I can't get my breath. 00:16:56.93\00:16:59.93 These symptoms are not fleeting symptoms, these symptoms 00:16:59.96\00:17:02.36 are ones that stay with you, they're not going away. 00:17:02.40\00:17:04.79 In elderly sometimes we see the only symptom 00:17:04.82\00:17:07.70 being extreme fatigues. 00:17:07.73\00:17:09.54 So those are some symptoms that... 00:17:09.58\00:17:11.38 Can we see the next slide? 00:17:11.41\00:17:12.81 Another one: palpitations. 00:17:12.84\00:17:14.17 Feeling your heart beat too fast. 00:17:14.21\00:17:16.17 Feeling like you can't move on. 00:17:16.21\00:17:18.47 You can have abnormal sensations. 00:17:18.51\00:17:20.74 I've seen through the years almost anywhere in the body, 00:17:20.77\00:17:22.96 it can be in the jaw, it can be in the shoulders. 00:17:22.99\00:17:25.49 Some people, for instance, think they have heart burn. 00:17:25.52\00:17:27.95 And the heart burn is really a heart attack and vice versa. 00:17:27.99\00:17:31.17 They say: "Oh,I'm just having some heart burn." 00:17:31.20\00:17:33.86 And that is a heart attack. 00:17:33.89\00:17:35.71 So there's many different sensations that you can get 00:17:35.75\00:17:38.27 from a heart attack but you just really have to pay attention 00:17:38.31\00:17:41.08 to those and say 00:17:41.11\00:17:42.11 "Hey,I'm gonna get some help right away!" 00:17:42.15\00:17:43.84 My own grandmother died suddenly of a heart attack. 00:17:43.88\00:17:48.80 Just served the meal and just sat down 00:17:48.83\00:17:51.13 and her face fell into her plate. 00:17:51.16\00:17:53.27 But they said that hers was caused from an upset 00:17:53.31\00:17:57.08 in the electrolytes balance, that she was on a very 00:17:57.11\00:18:00.85 severe or strict diet. That her doctor had restricted 00:18:00.88\00:18:05.84 her to like 7 or 800 calories a day. 00:18:05.87\00:18:07.96 So something that drastic can cause a heart attack, right? 00:18:08.00\00:18:16.01 Yes, it could contribute to a heart attack 00:18:16.04\00:18:17.97 but the cause, I'm not so sure because we know lots of things. 00:18:18.01\00:18:22.02 In the vessels, usually it's blockages that really cause it. 00:18:22.05\00:18:26.03 Things like the plaque rupturing due to stress, 00:18:26.06\00:18:28.68 electrolyte abnormalities like she might have had 00:18:28.71\00:18:31.30 which cause a funny rhythm problem. 00:18:31.33\00:18:33.03 Those are all things that could have contributed 00:18:33.07\00:18:35.89 to the heart attack all of the sudden developing. 00:18:35.93\00:18:38.68 What she had happened is called "sudden death syndrome"? 00:18:38.72\00:18:41.73 Yes, sudden death. And most sudden deaths 00:18:41.76\00:18:43.55 are from not so much a heart attack 00:18:43.58\00:18:45.35 but from a rhythm problem. 00:18:45.38\00:18:47.05 The heart rhythm becomes suddenly abnormal. 00:18:47.09\00:18:49.88 When a heart rhythm becomes abnormal, all of the sudden 00:18:49.92\00:18:53.35 it doesn't pump effectively so you get no blood to the brain. 00:18:53.39\00:18:56.42 When no blood goes to the brain, BOOM! You pass out. 00:18:56.45\00:18:59.88 That's why with people that pass out we're always concerned 00:18:59.92\00:19:02.12 whether they have a funny heart beat, which we call arrhythmia. 00:19:02.16\00:19:04.71 Some rhythm problems are related to blockages while some 00:19:04.74\00:19:08.95 rhythm problems come from other reasons, 00:19:08.98\00:19:10.81 like electrolyte abnormalities, people that get dehydrated 00:19:10.84\00:19:13.85 can have these electrical problems. 00:19:13.88\00:19:15.80 People that have a host of other things outside the arteries 00:19:15.84\00:19:18.69 can also have this and that can cause sudden death 00:19:18.72\00:19:21.36 outside of a myocardial infarction. 00:19:21.39\00:19:23.36 My microvalve prolapse causes arrhythmia too, doesn't it? 00:19:23.40\00:19:27.89 Yes, but usually that one's not a dangerous one. 00:19:27.92\00:19:30.52 That's good news. 00:19:30.55\00:19:32.02 Here is a great question from Lacey in New York. 00:19:32.06\00:19:35.97 And Lacey writes: 00:19:36.00\00:19:37.73 "Is there a test that predicts a heart attack?" 00:19:37.77\00:19:41.52 Unfortunately there's no test that predicts a heart attack. 00:19:41.56\00:19:45.54 Lacey, I wish we did have a test that predicted a heart attack, 00:19:45.58\00:19:49.53 cause if we did, I could say 00:19:49.56\00:19:51.07 you're gonna have one, you're not gonna have one. 00:19:51.11\00:19:53.62 I could tell people so they can play in their lives accordingly. 00:19:53.66\00:19:57.16 However, we have some people that walk on a treadmill, 00:19:57.19\00:20:01.05 they walk 9 or 10 minutes on a treadmill, 00:20:01.08\00:20:02.92 they all feel great, have no symptoms at all and then 00:20:02.96\00:20:05.94 down the road they might have one of these plaque ruptures 00:20:05.98\00:20:08.26 and then they have a heart attack. 00:20:08.29\00:20:09.69 We also have people that we do angiograms. 00:20:09.73\00:20:11.94 We take pictures of the arteries of the heart 00:20:11.98\00:20:14.16 and we can actually see these plaques. 00:20:14.19\00:20:16.09 We can still not predict which of these plaques 00:20:16.13\00:20:19.41 are gonna rupture and cause a heart attack. 00:20:19.45\00:20:22.58 That's why we wanna focus on everything we can 00:20:22.62\00:20:25.72 to lower the risk of having a heart attack. 00:20:25.75\00:20:27.61 On our next show we're really gonna focus on ways that we can 00:20:27.65\00:20:31.36 tell people how to lower the risk to the lowest possible 00:20:31.39\00:20:34.59 risk of having a myocardial infarction. 00:20:34.62\00:20:36.19 So,Lacey,I think that was an excellent question. 00:20:36.23\00:20:38.79 Yes. I have had an increase of stress" 00:20:57.99\00:21:01.38 "in my life since the war in Iraq started." 00:21:01.41\00:21:04.13 "My wife is concerned that this might cause a heart attack." 00:21:04.17\00:21:08.28 "How can I lower my chance of having a heart attack?" 00:21:08.31\00:21:11.85 That is an excellent question, Steve. 00:21:11.88\00:21:14.06 That is a type of question, Shelley, that makes 00:21:14.10\00:21:17.10 a heart attack doctor just smile because we know that 00:21:17.14\00:21:20.49 this is a person that wants to prevent heart attacks. 00:21:20.52\00:21:23.72 We've talked about over the course of this program 00:21:23.76\00:21:26.93 that 90% of this disease can be preventable. 00:21:26.96\00:21:29.50 And here we have Steve in Saudi Arabia 00:21:29.53\00:21:32.78 wanting to lower his risk. 00:21:32.81\00:21:34.46 Unfortunately, Steve can't come over here 00:21:34.49\00:21:37.22 and talk to me right now but if Steve was sitting 00:21:37.26\00:21:39.65 right here with me today like you, I'd say: 00:21:39.68\00:21:41.60 Steve, let's talk about ways you can lower your risk. 00:21:41.64\00:21:44.38 Cause this is the most important thing, probably 00:21:44.41\00:21:46.26 some of the most important health decisions 00:21:46.29\00:21:48.30 he'll make ever in his life to lower his risk, 00:21:48.34\00:21:50.32 since the prevalence of this disease is so high. 00:21:50.35\00:21:52.38 The first thing is exercise. 00:21:52.41\00:21:54.41 Exercise helps in a host of ways. 00:21:54.44\00:21:57.29 We'll talk more about them in the next program. 00:21:57.33\00:21:59.47 Some of the things that it does is that it lowers 00:21:59.51\00:22:02.20 the blood pressure, makes the good cholesterol go up, 00:22:02.24\00:22:04.90 the bad cholesterol goes down, 00:22:04.93\00:22:07.05 it makes hormones called endorphins which block 00:22:07.09\00:22:11.40 stress hormones which can sometimes cause 00:22:11.43\00:22:13.91 these plaques to rupture. 00:22:13.94\00:22:15.11 Also exercise lowers the chance of diabetes 00:22:15.15\00:22:18.17 and a host of other things. 00:22:18.20\00:22:19.89 So exercise! I really tell people that 00:22:19.93\00:22:22.18 if you don't do anything else, you have to exercise. 00:22:22.22\00:22:24.45 The second thing that we want to do is lower blood pressure. 00:22:24.49\00:22:28.17 We know that the higher the blood pressure, 00:22:28.20\00:22:30.41 the more likely to have vascular damage. 00:22:30.44\00:22:32.62 And it only makes sense, Shelley The blood just rushing around 00:22:32.65\00:22:35.42 through you. If you have a plaque, 00:22:35.45\00:22:36.97 it's gonna rupture or break open, 00:22:37.00\00:22:38.45 and all this blood pressure will damage things. 00:22:38.49\00:22:41.12 Sort of like pipes, sometimes they talk about 00:22:41.16\00:22:43.76 heart attacks it's glorified plumbing. 00:22:43.79\00:22:45.84 If it's just rushing through, you can have some damage. 00:22:45.88\00:22:49.56 So pay attention to your blood pressure. 00:22:49.59\00:22:51.52 A lot of people don't even know they have blood pressure 00:22:51.56\00:22:53.56 problems because they never get it checked. 00:22:53.59\00:22:56.12 The third thing that I wanna focus them to 00:22:56.16\00:22:58.66 is know what your cholesterol is. 00:22:58.69\00:23:00.49 We talked about cholesterol being a risk factor. 00:23:00.53\00:23:03.31 We know that the higher the level of cholesterol 00:23:03.34\00:23:06.09 the greater the cardiovascular risk. 00:23:06.12\00:23:08.63 And we divide the cholesterol into bad and good cholesterol. 00:23:08.67\00:23:13.04 The good cholesterol is called HDL. 00:23:13.07\00:23:15.38 We want that to be out the roof. 00:23:15.41\00:23:17.65 The bad cholesterol is called the LDL. 00:23:17.69\00:23:20.36 And in just a normal person, with no health problems at all, 00:23:20.40\00:23:23.51 we want the LDL to be at least below 160. 00:23:23.54\00:23:26.90 If we have risk factors, high blood pressure, 00:23:26.94\00:23:30.27 family history, we want it below 130. 00:23:30.30\00:23:33.55 And if you have a known disease we want it to be well below 100. 00:23:33.59\00:23:38.37 In fact, there's been some recent studies done that say 00:23:38.40\00:23:41.10 if we can push the LDL down low enough, 00:23:41.13\00:23:43.79 we can have disease regression. 00:23:43.82\00:23:46.30 That's disease that once was in the body, leaves the body. 00:23:46.34\00:23:50.24 So it's actually reversing the disease. 00:23:50.27\00:23:51.93 Yes, reversing the disease. 00:23:51.96\00:23:52.97 So cholesterol is very important. 00:23:53.01\00:23:54.49 Also,Steve,if you're in a high risk group, 00:23:54.53\00:23:57.34 mum and dad had heart attacks or heart problems 00:23:57.37\00:24:01.04 at a younger age, we recommend those people 00:24:01.08\00:24:04.74 to take aspirin every day. 00:24:04.77\00:24:06.78 Aspirin every day if you're in a high risk group. 00:24:06.82\00:24:10.47 That's the 81 milligram, the little baby aspirin. 00:24:10.51\00:24:14.13 Yes, that's the minimum that you would need. 00:24:14.16\00:24:15.73 Steve, another thing is smoking. 00:24:15.76\00:24:18.42 You might say "I don't smoke at all". 00:24:18.46\00:24:20.51 A lot of people don't realize if you're around smokers 00:24:20.55\00:24:23.36 and if you're in the military, as you might be 00:24:23.39\00:24:25.76 over in Saudi Arabia, you can get half the risk 00:24:25.79\00:24:28.13 from being around second hand smokers. 00:24:28.16\00:24:29.82 So, if someone smokes two packs a day around you 00:24:29.86\00:24:32.88 you get one pack a day and the harmful effects. 00:24:32.91\00:24:35.90 A lot of people don't realize that. 00:24:35.93\00:24:37.64 So stay away from cigarettes! 00:24:37.67\00:24:39.45 And that's why it's been so important in the last few years 00:24:39.49\00:24:41.95 in America that we quit smoking 00:24:41.98\00:24:43.96 in restaurants, in public facilities. 00:24:44.00\00:24:45.67 Think about little children whose parents are smoking 00:24:45.71\00:24:49.10 at home and these children are trying to develop. 00:24:49.14\00:24:52.50 You said that. I had no idea second hand 00:24:52.53\00:24:55.74 smoke was that dangerous. 00:24:55.77\00:24:57.48 It's a major problem, second hand smoke. 00:24:57.52\00:24:59.50 But Steve, there might be something else 00:24:59.53\00:25:01.48 you might wanna look at. 00:25:01.51\00:25:02.60 The last couple ones I think are would getting more and more 00:25:02.64\00:25:05.96 information that these help lower the risk of heart attack. 00:25:05.99\00:25:09.97 One is cutting back on the stress and anxiety in the life. 00:25:10.01\00:25:13.50 We know that people that are into high amounts of stress 00:25:13.54\00:25:16.94 have high levels of epinephrine and these high hormones 00:25:16.97\00:25:20.30 which can cause high blood pressure and plaques to rupture. 00:25:20.34\00:25:22.87 So look at your life and see if there's things that you can 00:25:22.90\00:25:25.19 cut back on the stress. I know if you're in the military 00:25:25.22\00:25:27.86 or if you're in the Saudi Arabia around a lot of things that 00:25:27.89\00:25:30.50 going on in that part of the world, 00:25:30.53\00:25:31.65 it's hard to get away from. 00:25:31.68\00:25:33.64 But look at the stress in your life and see 00:25:33.67\00:25:36.37 what is you can lower that. 00:25:36.40\00:25:37.49 The last thing I think is very important 00:25:37.53\00:25:39.98 in lowering your risk is the proper nutrition. 00:25:40.02\00:25:42.65 You have to eat the right foods. 00:25:42.68\00:25:44.56 We know that people that eat high animal based diets, 00:25:44.60\00:25:48.52 they don't eat a lot of plants, we know that they have 00:25:48.55\00:25:52.44 a higher risk of having a heart attack 00:25:52.47\00:25:54.33 than those who don't. 00:25:54.36\00:25:55.46 And part of it is the cholesterol, the blood pressure 00:25:55.50\00:25:58.64 but also just changes the insides in a bad type of way. 00:25:58.67\00:26:01.74 So those are some things, Steve that you can...that's a start. 00:26:01.78\00:26:05.65 There's a couple of others we didn't mention 00:26:05.68\00:26:07.60 but that would be a good starting point 00:26:07.63\00:26:09.35 if you can get working on those. 00:26:09.38\00:26:10.62 Yes. Amen! 00:26:38.91\00:26:41.02 So you kind of recap for us what we've learned here today? 00:26:41.06\00:26:45.97 Well, myocardial infarction is such a huge topic. 00:26:46.00\00:26:50.39 The couple things we just have to take home with us today, 00:26:50.43\00:26:53.80 that we just must remember if we don't remember anything else. 00:26:53.84\00:26:57.18 So if you're sitting there at home, sort of zoning in, 00:26:57.21\00:26:59.54 you went to the cupboard and got some food 00:26:59.57\00:27:02.03 and you're just now coming back. 00:27:02.06\00:27:03.24 Let me throw up the three things that I think you just have 00:27:03.28\00:27:06.01 to know no matter what. 00:27:06.04\00:27:07.25 Number 1. The most common cause of death 00:27:07.29\00:27:11.10 in society now is myocardial infarction. 00:27:11.14\00:27:14.92 This is a lot bigger than most people even realize. 00:27:14.95\00:27:18.41 and we talked earlier about 43% of deaths. 00:27:18.44\00:27:21.11 People that don't make it in because of the heart attacks. 00:27:21.15\00:27:23.72 So that's the one you have to realize. 00:27:23.75\00:27:25.23 It's more common than breast cancer, 00:27:25.26\00:27:26.96 it's more common than lung disease, 00:27:26.99\00:27:28.74 it's more common than infections, 00:27:28.77\00:27:30.45 it's more common than these Ebola viruses, 00:27:30.49\00:27:33.28 it's more common than having a car accident, 00:27:33.32\00:27:35.74 and yet you just don't hear enough about it. 00:27:35.77\00:27:38.52 And a lot of this is some of the societal norms 00:27:38.56\00:27:41.28 that we're going through. 00:27:41.31\00:27:42.38 The second point is that we cannot ignore the symptoms. 00:27:42.42\00:27:45.05 If you have anything that you think that might be 00:27:45.08\00:27:47.29 related to your heart, you have to get in to the doctor 00:27:47.33\00:27:50.01 and get help right away. 00:27:50.04\00:27:51.47 And the last point is 90% of cardiovascular disease 00:27:51.51\00:27:55.20 if dealt with early could be prevented. 00:27:55.23\00:27:58.01 And getting back to our opening text. 00:27:58.04\00:28:01.40 Psalm 139é14 says: "I will praise Him" 00:28:01.43\00:28:04.72 "for I am fearfully and wonderfully made." 00:28:04.76\00:28:06.62 And what a great chance we have to have our bodies taken care of 00:28:06.66\00:28:11.93 by Lord and Savior that loves us. 00:28:11.96\00:28:13.39 Amen! 00:28:13.42\00:28:15.77
