Different types of 00:00:50.40\00:00:53.38 arthritis, in fact there is 100 varieties of 00:00:53.49\00:00:55.56 arthritis we're not gonna talk about all 00:00:55.63\00:00:57.66 one hundred today but we're gonna talk 00:00:57.81\00:00:59.50 about some of the things we might be able 00:00:59.72\00:01:01.31 to do to ease the pain and may be even 00:01:01.37\00:01:04.74 make life a little easier as my guest today 00:01:06.24\00:01:09.39 I have Dr. Allan Handysides, Allan, 00:01:09.48\00:01:11.79 welcome and you're gonna be our guest 00:01:12.11\00:01:14.14 today and I'd like to ask you first what is it? 00:01:14.19\00:01:19.78 What is arthritis? What is arthritis? 00:01:21.25\00:01:22.64 Well it's a simple word, it really means 00:01:24.10\00:01:27.11 inflammation of a joint, and as such there 00:01:27.15\00:01:31.59 are many as you've already said, 00:01:31.62\00:01:33.01 many different kinds of arthritis. 00:01:33.13\00:01:35.12 A joint is classically made of, is the place 00:01:35.93\00:01:39.29 where two bones come together and the 00:01:39.43\00:01:42.38 good Lord made us in such a way that the 00:01:42.48\00:01:44.97 surfaces of those bones are covered with 00:01:45.22\00:01:46.92 cartilage which allows there to be a 00:01:47.05\00:01:49.84 movement that slips and slides rather easily. 00:01:49.87\00:01:52.67 A lubricant is provided to that joint by 00:01:53.66\00:01:57.03 what is called Synovium or the Synovial 00:01:57.18\00:01:59.90 membrane. Now, this is a specialized structure 00:01:59.96\00:02:03.96 that lines the joint but not over this surfaces 00:02:03.99\00:02:08.10 that rub together but on the inside of the 00:02:08.23\00:02:11.02 capsule that holds the joint together and this 00:02:11.19\00:02:14.66 membrane secretes a lubricating fluid that 00:02:15.08\00:02:19.90 actually gives a nice smooth gliding surface. 00:02:19.93\00:02:24.08 Around the the Synovium is the capsule and 00:02:25.08\00:02:28.99 then supporting the joint are various 00:02:29.06\00:02:31.53 ligaments and tendons and then the muscles 00:02:31.62\00:02:35.00 and so we find that we have this very a 00:02:35.54\00:02:37.94 compact place between bones that gives 00:02:38.03\00:02:41.50 both strength and support and yet at the 00:02:41.64\00:02:44.53 same allows for flexibility and therefore 00:02:44.62\00:02:47.67 for movement. We usually define joints 00:02:47.79\00:02:50.52 small joints and large joints the large joint, 00:02:50.72\00:02:53.71 the largest joint of course there is the 00:02:54.03\00:02:55.27 hip joint. We have knee joints, we have 00:02:55.38\00:02:58.36 then smaller joints, the smaller ones which 00:02:58.42\00:03:01.00 are in our fingers and in our feet. 00:03:01.15\00:03:04.70 And we can get arthritis, different forms 00:03:05.35\00:03:07.63 of arthritis effect different joints 00:03:07.68\00:03:10.47 predominantly so there are patterns of 00:03:11.30\00:03:13.60 arthritis. Now, my grandmothers, 00:03:13.73\00:03:16.94 I've got two grandmothers as most 00:03:17.46\00:03:18.79 people, and both of them had rheumatise, 00:03:18.85\00:03:22.35 what do you think this was? 00:03:24.22\00:03:25.19 Well, what is rheumatise? Well, rheumatise, 00:03:26.27\00:03:28.61 I heard many years ago. And my family 00:03:30.58\00:03:31.69 use to be lumbago, and okay, rheumatism, 00:03:31.75\00:03:34.93 and so forth. That term is a term that has 00:03:35.59\00:03:38.69 been used over generations and it's a lay 00:03:38.75\00:03:42.61 term that refers to arthritis. 00:03:42.74\00:03:45.85 Usually it's referred to the form of 00:03:46.48\00:03:48.77 arthritis that came with aging which is 00:03:48.80\00:03:52.18 osteoarthritis, but any of the forms of 00:03:52.40\00:03:55.12 arthritis that might have been called 00:03:55.15\00:03:57.34 rheumatise you know according to the old 00:03:57.39\00:04:01.03 time treatment. Now, I'd like to ask 00:04:01.07\00:04:02.53 you something because my grandmother 00:04:02.57\00:04:04.66 used to be able to forecast the weather, 00:04:04.69\00:04:07.05 is that possible? You know this is, 00:04:08.17\00:04:09.77 it's a very interesting thing and we don't 00:04:09.92\00:04:11.71 quite understand why that is, but many 00:04:11.80\00:04:13.88 people who have arthritis are sensitive to 00:04:13.91\00:04:17.77 possibly barometric changes, pressure 00:04:18.75\00:04:20.79 changes. And we'll be able to say there 00:04:20.86\00:04:23.48 is bad weather coming along. Interestingly my 00:04:23.51\00:04:25.76 wife she doesn't have arthritis that I've been 00:04:25.85\00:04:28.58 able to define but she has had over the 00:04:28.64\00:04:31.41 years joint pains of various variety and she 00:04:31.44\00:04:34.53 will sometimes say I think we're going to 00:04:34.66\00:04:36.21 have a storm because my knees are aching 00:04:36.27\00:04:39.68 and I think that's a fairly common feeling 00:04:40.33\00:04:43.92 among different people that they can 00:04:44.36\00:04:46.34 sense weather changes probably barometric 00:04:46.42\00:04:48.28 changes. Now, there's many kinds of 00:04:48.31\00:04:50.87 arthritis as I mentioned earlier but we're 00:04:50.91\00:04:54.42 gonna have to limit ourselves to may be a 00:04:54.45\00:04:55.96 couple of kinds. Tell me about osteoarthritis? 00:04:56.10\00:04:59.70 Well, we can, if we have time we can talk 00:05:00.67\00:05:03.11 about some of the other forms. 00:05:03.15\00:05:04.39 Osteoarthritis is the commonest form of 00:05:04.52\00:05:06.88 arthritis, its probably going to affect, 00:05:06.93\00:05:08.95 maybe 60 percent of people are going to 00:05:08.98\00:05:12.24 have trouble with osteoarthritis people 00:05:12.27\00:05:14.95 who have a family history of osteoarthritis 00:05:15.00\00:05:19.77 again are more prone to develop it. 00:05:19.80\00:05:23.31 And people who are have some of the 00:05:23.93\00:05:26.16 predisposing factors such as heavy manual 00:05:26.25\00:05:29.43 labor. Well, osteoarthritis is not an 00:05:29.66\00:05:32.43 arthritis that is a disease caused by living 00:05:32.51\00:05:39.36 in an improper way or anything like that. 00:05:39.42\00:05:41.95 Except that it is brought on by heavy 00:05:42.46\00:05:44.87 physical labor. So somebody who works 00:05:45.19\00:05:47.62 a jackhammer for instance or somebody 00:05:47.65\00:05:50.60 who is working as a stone mason with a 00:05:50.63\00:05:54.10 hammer hammering away at rock corner 00:05:54.24\00:05:56.79 with a chisel, chiseling rock. Somebody who 00:05:57.04\00:06:00.75 maybe lifting heavy weights, a minor. 00:06:00.83\00:06:04.17 Well, those kind of people do tend to 00:06:04.72\00:06:06.36 get osteoarthritis, although one can get 00:06:06.45\00:06:10.07 osteoarthritis even doing relatively light 00:06:10.27\00:06:13.38 work and sedentary work. if it's a repetitive 00:06:13.41\00:06:16.19 type of motion, now I myself have osteoarthritis 00:06:16.45\00:06:20.38 in my hands which surprised me because 00:06:20.46\00:06:22.66 having been a physician, I didn't think that 00:06:22.74\00:06:24.78 I was doing a lot of strain. A lot of work, 00:06:24.94\00:06:26.86 but I was doing a lot of surgery, 00:06:27.29\00:06:29.42 I used to operate may be for 12 or 15 00:06:29.70\00:06:32.65 hours a week and using even though 00:06:32.72\00:06:36.83 there's small instruments but I was using 00:06:36.86\00:06:39.58 a repetitive type of motion with sowing 00:06:39.73\00:06:42.72 and cutting and repairing tissues and so forth. 00:06:43.53\00:06:47.37 And I have a little arthri-d in my hands. 00:06:48.06\00:06:50.90 You'll notice my finger, this little finger has 00:06:51.11\00:06:54.93 arthritis. And that may have also have been 00:06:54.96\00:06:57.55 just. One second, what did you call it? 00:06:57.65\00:06:58.62 Stuck, yeah stuck together, arthri-d. 00:06:58.67\00:07:00.69 Yes, you look you see well I can close this 00:07:00.75\00:07:02.59 finger those that joint there has kind of 00:07:02.70\00:07:05.80 stuck up and what's happen with the joint is 00:07:05.88\00:07:08.69 that there has been an overgrowth of bony, 00:07:08.99\00:07:11.79 little tuberosities around the joint the fibrous 00:07:12.97\00:07:16.44 tissue of the capsules become stiffened and 00:07:16.48\00:07:19.84 at a point in time, it was painful. 00:07:20.19\00:07:22.52 I think I sustained an injury actually to this 00:07:23.13\00:07:25.82 little finger as well which has speeded up 00:07:26.03\00:07:28.44 the process, but that's a classic situation 00:07:28.49\00:07:32.38 for osteoarthritis. Many people get hip, 00:07:32.60\00:07:35.97 osteoarthritis of the hip, that can be a very 00:07:36.48\00:07:39.38 serious problem. And osteoarthritis then 00:07:39.92\00:07:43.68 is a wear and tear type of problem, 00:07:43.71\00:07:47.59 on the joints. Now, occasionally I have 00:07:47.88\00:07:50.52 fallbacks, years ago I watched Roger 00:07:50.69\00:07:53.08 Staubach, he was a quarterback for the 00:07:53.34\00:07:55.91 Dallas Cowboys and heard him advertising 00:07:56.10\00:07:58.91 pain killers. Well, are sports like contact 00:07:59.03\00:08:02.23 sports like football, are those players have to 00:08:02.26\00:08:06.59 suffer arthritis? Yes, osteoarthritis later 00:08:06.73\00:08:09.00 on in life, they are indeed, particularly 00:08:09.03\00:08:12.61 players that get tackled are likely to have 00:08:12.78\00:08:16.98 the joints traumatized and trauma just 00:08:17.15\00:08:20.56 like hard work trauma can predispose. 00:08:20.66\00:08:24.11 So, sports as well as some of the manual 00:08:24.15\00:08:26.62 professions could be a danger, one of the 00:08:26.66\00:08:28.62 most severe anthracitic situation that I 00:08:28.85\00:08:32.24 remember encountering was in a former 00:08:32.37\00:08:34.36 hockey player. You know, sometimes we 00:08:34.39\00:08:37.83 look at the stars of hockey or the stars 00:08:37.86\00:08:40.77 of sports and we think wow! What a 00:08:40.81\00:08:43.02 wonderful thing, but we don't understand 00:08:43.05\00:08:44.90 that they are very many casualties along 00:08:44.93\00:08:47.88 the way and for every big price. 00:08:48.08\00:08:49.45 Every star there are hundreds of people 00:08:49.70\00:08:52.29 who played hard may be enjoyed it very 00:08:52.84\00:08:55.31 much and I'm not against good athletic 00:08:55.34\00:08:57.72 activities, but do sustain severe injuries and 00:08:57.75\00:09:02.51 we need if our children and young people are 00:09:02.70\00:09:05.16 playing sports we need to be careful that 00:09:05.28\00:09:07.93 they're not putting themselves at undue risk. 00:09:07.96\00:09:10.71 For instance the pictures in baseball, 00:09:11.36\00:09:13.85 that repetitive strain many of them have 00:09:14.16\00:09:16.87 problems with shoulders and elbows, 00:09:16.97\00:09:19.01 arthritis because of repetitive throwing, 00:09:19.30\00:09:22.51 you know that ball time after time after time, 00:09:23.00\00:09:26.28 that's not what we were made for. 00:09:26.55\00:09:28.21 We were made to function in a balanced way, 00:09:28.43\00:09:31.11 not in a sort of highly focused way that 00:09:31.21\00:09:34.49 stresses one joint to the, you know 00:09:34.68\00:09:38.33 to out of proportion to all the others. 00:09:38.44\00:09:40.29 After listening my grandmothers for years 00:09:41.20\00:09:43.75 complaining about their arthritis is there 00:09:43.89\00:09:46.71 anything I can do or may be its too late 00:09:46.78\00:09:48.99 for me but some people can do to prevent 00:09:49.16\00:09:50.92 it? Well, of course we've already been touching, 00:09:50.95\00:09:54.06 talking about preventing these strains and 00:09:54.13\00:09:56.69 these injuries. So, don't play contact sports. 00:09:56.76\00:09:59.20 Avoid these excessive activities, if we can. 00:09:59.62\00:10:03.26 So, there is anyway though like I'm, 00:10:03.33\00:10:04.81 say I'm a stone mason. Is there anyway 00:10:04.90\00:10:07.55 to avoid this kind of strain? It might be very 00:10:07.65\00:10:10.80 difficult, it might be very difficult, 00:10:10.96\00:10:12.97 one can of course if you have knee 00:10:13.16\00:10:15.06 troubles or hip troubles look to your shoes, 00:10:15.15\00:10:19.01 we need to have cushioned shoe and 00:10:19.38\00:10:21.76 there's been a great advance in the last 00:10:21.79\00:10:23.88 25-30 years since all the different shoe 00:10:23.98\00:10:27.21 manufactures have given comfortable 00:10:27.28\00:10:29.40 springy shoe, that's going to be very 00:10:29.53\00:10:31.78 beneficial to the knees and the hips of 00:10:31.92\00:10:34.11 people who wear those type of shoes. 00:10:34.14\00:10:35.95 When I was a boy we used to wear 00:10:36.19\00:10:37.68 you know stiff hard heels and soles people 00:10:37.97\00:10:43.03 wore boots, sometimes steel heels were 00:10:43.15\00:10:46.13 put on those and that transmitted a lot of 00:10:46.19\00:10:48.57 force through the limbs, so good cushioned 00:10:48.63\00:10:51.56 footwear is a very important thing for 00:10:52.33\00:10:54.95 protecting and then of course it's important 00:10:55.11\00:10:57.41 to wear supportive gloves and protective 00:10:57.77\00:11:00.86 gear in certain activities. You know, 00:11:00.89\00:11:03.82 I occasionally on my lawn tractor I wear 00:11:03.87\00:11:06.67 earmuffs to protect my ears, yes. 00:11:06.70\00:11:08.89 And when I get off the tractor and start 00:11:09.01\00:11:10.78 walking it feels like the sound come in my 00:11:10.94\00:11:15.05 ears, it feels like somebody's pounding 00:11:15.16\00:11:17.81 my ears. Now, when you take the protective, 00:11:17.84\00:11:20.81 no, when I put them on and walk, 00:11:20.88\00:11:23.24 which explains to me that even when I walk 00:11:24.17\00:11:28.03 there is a pounding going on in my joints, 00:11:28.71\00:11:31.71 I can hear it. Were these from the earmuffs, 00:11:31.77\00:11:33.46 it's just amplifies it, it amplifies it. 00:11:33.98\00:11:36.11 You're cutting off all the other sound, 00:11:36.17\00:11:38.02 you're cutting off the external sound and 00:11:38.18\00:11:39.76 then you're aware of, if you were eating 00:11:39.79\00:11:42.63 your honey nut. So, shoes I think it 00:11:42.84\00:11:45.23 would be important wouldn't it be? 00:11:45.26\00:11:46.41 Very important, because you do transmit 00:11:47.06\00:11:49.75 forces through that. Now, there are some 00:11:49.87\00:11:51.26 other things that might increase the risk of 00:11:51.32\00:11:53.80 osteoarthritis and that is obesity, 00:11:55.04\00:11:57.44 is that true? It certainly is. Obesity imagine 00:11:57.72\00:12:02.48 we've talked about work, now imagine that 00:12:02.51\00:12:04.76 you carry a 50 pound sack of potatoes or grain 00:12:04.79\00:12:08.47 or whatever else you want to do. 00:12:08.60\00:12:10.01 Everywhere you go, are extra calories, 00:12:10.25\00:12:12.86 well if you have 50 pounds of overweight 00:12:13.71\00:12:15.58 that's what you do. Yeah! That's right. 00:12:15.67\00:12:16.80 Yeah you're carrying that all the time and 00:12:16.88\00:12:19.30 that's a major strain on your joint and obesity 00:12:19.70\00:12:23.37 being such a common problem. 00:12:23.44\00:12:24.91 Now, there is a definite thing that we can 00:12:25.57\00:12:27.11 do is to take control about our weight. 00:12:27.22\00:12:29.49 Easier said of course than done, 00:12:29.85\00:12:31.31 because you know I feel myself I've got 00:12:31.83\00:12:34.29 this little thing here, it's not because 00:12:34.33\00:12:36.27 I don't know what to do its because 00:12:36.34\00:12:38.83 we need to be motivated, to be have the 00:12:38.86\00:12:42.11 will power not to eat so much. So, how does 00:12:42.14\00:12:45.68 the obese, is that just mean there is extra 00:12:45.88\00:12:48.05 pressure on the joints? Yes, extra pressure 00:12:48.18\00:12:50.23 on the joints, extra wear and extra tear. 00:12:50.26\00:12:52.80 Alright, go on. Sometimes you know we're 00:12:54.32\00:12:56.99 going to have to address osteoarthritis 00:12:57.04\00:13:00.62 though with surgery, you are? 00:13:00.68\00:13:04.30 Yeah, my father had osteoarthritis in the 00:13:04.40\00:13:06.50 knees, as a boy he suffered rickets. 00:13:06.60\00:13:09.65 He was brought up in Newcastle on Tyne, 00:13:10.14\00:13:13.81 its an mining and cold city in Northern 00:13:13.90\00:13:17.87 England. Very cold, very dark, not a lot 00:13:17.90\00:13:20.74 of sunshine and of course they're dressed 00:13:21.28\00:13:23.32 from here right down the toes, they had 00:13:23.35\00:13:25.30 a big hat on as well. Very little sunshine, 00:13:25.33\00:13:27.48 vitamin D deficiency developed and he 00:13:27.91\00:13:30.84 became, he had bowlegs and we used to 00:13:30.91\00:13:33.85 tease him about his bowlegs but it was a 00:13:33.94\00:13:35.36 sad thing really because it was a fact 00:13:35.39\00:13:37.02 that he had rickets as a child. That meant 00:13:37.11\00:13:40.54 that as he walked the pressure on his knee 00:13:40.57\00:13:44.05 joints were transmitted not evenly across 00:13:44.08\00:13:47.16 the joint but to one side of the joint. 00:13:47.19\00:13:49.56 That's right, okay, and so when he got to 00:13:49.59\00:13:51.44 be about 18 years old he was having a 00:13:51.47\00:13:53.71 lot of trouble with his knees and the doctor 00:13:53.74\00:13:56.15 said what we could do is to cut, 00:13:56.20\00:13:57.51 do an osteotomy, cut your bone and realign 00:13:57.77\00:14:01.27 them, so that the weight will be disturbed 00:14:01.30\00:14:02.90 to the other side of the joint and that 00:14:03.05\00:14:04.82 would free you of some of the pain. 00:14:04.85\00:14:06.99 Now, today maybe they would have said 00:14:07.63\00:14:09.37 to him, maybe you can have a joint replacement, 00:14:09.40\00:14:11.92 replacement. Where they take the joint out 00:14:11.95\00:14:13.64 and we will replace the joint, they have 00:14:13.91\00:14:17.04 done surgeries like putting caps on the 00:14:17.08\00:14:19.47 joint, just on the ends of the bones so 00:14:19.50\00:14:22.03 that the surfaces that are rubbing against 00:14:22.11\00:14:23.66 each other are metallic. And we have 00:14:23.69\00:14:26.06 of course arthroscopic surgery which is 00:14:26.09\00:14:28.13 very interesting surgery because you take 00:14:28.21\00:14:30.02 the little tiny tube like thing through which 00:14:30.10\00:14:33.20 you can look and see and they can look inside 00:14:33.23\00:14:36.09 a joint, that's very useful sometimes if a 00:14:36.12\00:14:39.12 joint's damaged with osteoarthritis, 00:14:39.24\00:14:40.72 got little chips of cartilage or bone inside 00:14:40.75\00:14:43.62 that, they can take those out through 00:14:43.70\00:14:45.59 little tiny cuts maybe quarter or half an 00:14:45.62\00:14:48.23 inch in size. People can have that surgery 00:14:48.26\00:14:50.89 done and go away very, very readily. 00:14:50.92\00:14:54.11 Are these surgeries quite successful or do 00:14:54.31\00:14:56.52 they have to be done over again? 00:14:56.57\00:14:57.99 Some times will, replacement surgery will 00:14:58.07\00:15:00.14 have to sometimes to be done over again. 00:15:00.22\00:15:02.51 Maybe what ten years? May be 10-15 years 00:15:02.56\00:15:05.43 and there is a time with the improving joints 00:15:05.46\00:15:09.06 that is getting to be longer and longer 00:15:09.27\00:15:12.26 time that a joint will last. That is why we 00:15:12.46\00:15:14.81 used to say to people well maybe put up 00:15:14.97\00:15:17.36 with your pain a little longer because we 00:15:17.39\00:15:19.11 didn't want to really do a joint replacement 00:15:19.14\00:15:21.85 too soon. Especially if a person was say, 00:15:22.63\00:15:27.96 60 or may be if he could put up with him for 00:15:28.28\00:15:30.52 5 years. What's the problem with having 00:15:30.62\00:15:32.20 replacement, the replacement? Well, the 00:15:32.23\00:15:35.00 problem with the replacement is nothing, 00:15:35.03\00:15:36.47 nothing is as good. Except expense. 00:15:36.94\00:15:38.64 As nothing is as good as what God gave us. 00:15:38.77\00:15:41.71 We are indeed as our program says wonderfully 00:15:41.87\00:15:44.83 made, beautifully made and when you replace 00:15:44.90\00:15:48.44 a joint with a manufactured joint it 00:15:48.47\00:15:51.40 works fine but I've seen them when 00:15:51.50\00:15:53.36 they've been taken out and the ground 00:15:53.39\00:15:55.66 up edges and the ground up surfaces, 00:15:55.69\00:15:58.09 the wear and tear that the joint undergoes 00:15:58.24\00:16:01.92 and our normal joints repair themselves. 00:16:02.17\00:16:05.18 Self-reparative joints and so, marvelous, 00:16:05.31\00:16:07.70 marvelous, it's absolutely fantastic that that 00:16:07.73\00:16:10.30 can take place. So yes, it's good that there 00:16:10.33\00:16:14.53 is a hip or a joint replacement, 00:16:14.56\00:16:16.78 knee replacement but it would be so much 00:16:16.83\00:16:19.50 benefit to preserve what God has given 00:16:19.53\00:16:21.74 us if we could. Now, my grandmother, 00:16:21.77\00:16:23.98 one of my grandmothers was told that if she can 00:16:24.11\00:16:26.48 get relief by standing, now in Texas we 00:16:26.51\00:16:29.70 have red ants, red ants about a half 00:16:29.73\00:16:32.47 inch long and they are sting and they're 00:16:32.50\00:16:35.62 very painful but she was told if she went 00:16:35.65\00:16:38.11 out on this bed and got stung by ants that 00:16:38.14\00:16:40.92 would relief her arthritis. I don't think 00:16:40.95\00:16:43.51 it did, you know anything about this? 00:16:43.54\00:16:45.11 Well, of course I am, that's an alternative 00:16:45.62\00:16:48.18 remedy. What I am going, leading at is that 00:16:48.23\00:16:51.10 are there some kind of relief, is there 00:16:51.23\00:16:54.01 something that we can do to relieve somes 00:16:54.13\00:16:55.26 of this pain in a little surgery. 00:16:55.35\00:16:57.05 Well, of course the simple pain relievers 00:16:57.39\00:17:00.21 have worked well and continue to work well. 00:17:00.70\00:17:03.67 Aspirin as you know is an extract of willow 00:17:04.19\00:17:06.06 bark and was, quite natural, right? 00:17:06.12\00:17:09.82 It was natural originally and then it's been 00:17:09.85\00:17:12.14 synthesized and so Aspirin is a very good 00:17:12.25\00:17:15.58 pain reliever, of course it's hard on the 00:17:15.61\00:17:17.26 stomach, it may cause bleeding in some 00:17:17.40\00:17:19.46 individuals so it's not something that you 00:17:19.60\00:17:20.83 can take easily. Tylenol is less irritative 00:17:20.86\00:17:26.05 to the stomach and it metabolizes to some 00:17:26.21\00:17:28.48 of the similar ingredients. So, Tylenol 00:17:28.51\00:17:30.67 will work. Then we have a whole class of 00:17:30.75\00:17:32.34 medications called non-steroidal 00:17:32.49\00:17:34.42 anti-inflammatory and they've been used with 00:17:34.45\00:17:37.11 some degree of benefit but they also are 00:17:37.14\00:17:39.76 irritated to the stomach. The latest ones 00:17:39.79\00:17:43.28 which have been in the news are Vioxx and 00:17:43.31\00:17:45.57 we've also seen some problems with that they 00:17:46.88\00:17:49.93 are what you call COX-2 inhibitors. 00:17:50.00\00:17:52.44 The COX-2 inhibitor interfere with the 00:17:52.95\00:17:55.68 inflammatory process in the different 00:17:55.75\00:17:57.41 place but they have been found, Vioxx has 00:17:58.13\00:18:01.45 been found to increase your risk of heart 00:18:01.56\00:18:03.31 attack. So, when we talk about treatment 00:18:03.34\00:18:06.80 for arthritis, yes we really do want to avoid 00:18:06.92\00:18:10.74 it if at all possible and then we want to use 00:18:10.77\00:18:13.56 as mild the medication as we possibly can. 00:18:13.59\00:18:15.76 It's important not to forget some very simple 00:18:15.86\00:18:18.78 things that can help for arthritis. 00:18:18.83\00:18:21.00 Heat and cold you know is something that will 00:18:21.65\00:18:24.92 very often help, now we don't usually heat 00:18:24.95\00:18:27.33 and inflame joints, so we can talk about 00:18:27.40\00:18:29.25 rheumatoid arthritis, we don't heat and 00:18:29.28\00:18:31.69 inflame joint but osteoarthritis may benefit 00:18:31.80\00:18:34.03 from warm and we can also alternate that with 00:18:34.08\00:18:36.82 warm and cold. Alright, now, get into the 00:18:37.03\00:18:39.91 details how long if I have a joint that's 00:18:40.13\00:18:44.22 inflamed and hurting how much cold what 00:18:44.25\00:18:47.93 kind of, how do we put cold on that? 00:18:47.96\00:18:49.47 Ice pack. Well, yes an ice pack would be a 00:18:49.55\00:18:51.87 good way, a heating pad will be fine, 00:18:51.90\00:18:53.94 heating pad that we mild, or if you're using, 00:18:53.97\00:18:56.37 you you must be very careful using hot 00:18:56.42\00:18:58.89 water bottles and those sort of things 00:18:58.92\00:19:00.49 because it's gonna burn the skin. 00:19:00.52\00:19:01.88 And if there is a diabetic that's being 00:19:02.23\00:19:03.78 treated or if there is any evidence of nerve 00:19:03.84\00:19:06.54 insensitivity there, got to be very, 00:19:07.25\00:19:09.62 very careful with heat. But you can put 00:19:09.73\00:19:11.74 warm packs around you, usually put a 00:19:11.85\00:19:14.68 warm packet something that you may heat 00:19:14.81\00:19:16.70 or if it can be an electrical pad that you 00:19:17.35\00:19:19.50 heat. You apply that around the affected 00:19:19.61\00:19:21.76 area or over the affected area usually 00:19:21.83\00:19:24.19 for five, six, seven minutes, then you 00:19:24.29\00:19:27.17 remove it and then you can have an ice 00:19:27.29\00:19:28.93 pack. And an ice pack you can make by 00:19:28.97\00:19:31.40 crushed ice inside a towel or, okay, 00:19:31.50\00:19:34.49 a plastic bag first and then, and you apply 00:19:34.56\00:19:37.31 that and then you just cool the joint, 00:19:37.34\00:19:39.35 you then alternate the hot, you can do 00:19:39.48\00:19:41.59 it with water. Actually very, very good 00:19:41.62\00:19:45.11 way to heat and cool if you can immerse 00:19:45.14\00:19:47.78 if its an ankle or something you can 00:19:47.86\00:19:49.46 immerse it in warm water then in cold 00:19:49.58\00:19:51.31 water. Again for five minutes-ten minutes 00:19:51.34\00:19:53.87 you always want to end up with a cold 00:19:53.95\00:19:56.37 application, good, for osteoarthritis because 00:19:56.64\00:19:59.09 the cold chills it out and the warm brings 00:19:59.12\00:20:02.19 more blood there so it improves the healing. 00:20:02.22\00:20:04.67 Now, the, I have a brother-in-law that 00:20:05.22\00:20:07.40 took one of these thermal foils, 00:20:08.08\00:20:09.55 these heating pads? Yes. That they have a 00:20:09.58\00:20:11.52 cut off switch on them, which is automatic, 00:20:11.55\00:20:13.31 but he was very clever he propped 00:20:13.96\00:20:15.92 something up against that cut off switch 00:20:16.00\00:20:18.05 and left it on and went to sleep and he 00:20:18.35\00:20:20.09 couldn't leave himself with a big burn 00:20:20.12\00:20:21.52 on his skin, and when woke up his fingers 00:20:21.55\00:20:24.39 were very inflamed and they had to remove 00:20:24.66\00:20:27.31 one of this fingers and half of the other. 00:20:27.51\00:20:29.23 I've seen people with burns the size of 00:20:29.42\00:20:31.29 saucers because of heating pads, 00:20:31.32\00:20:33.40 be very careful. These automatic cut off 00:20:33.43\00:20:35.28 switches are there for a purpose, very, 00:20:35.35\00:20:37.91 right, and so don't try to outsmart them. 00:20:37.94\00:20:41.57 You know, I've seen that we are going to 00:20:41.79\00:20:43.52 not have a lot of time, some people will 00:20:43.58\00:20:45.21 be suffering with rheumatoid arthritis 00:20:45.24\00:20:46.59 that's exactly right and that's our next 00:20:46.62\00:20:47.72 topic. Next part of our topic what about, 00:20:47.94\00:20:50.04 now what's the difference between 00:20:50.21\00:20:51.22 rheumatoid and osteo? Rheumatoid is not 00:20:51.37\00:20:54.85 a wear and tear situation, you know the 00:20:54.88\00:20:57.27 Adventist health study showed that people 00:20:57.30\00:20:59.05 who were vegetarians actually had less 00:20:59.08\00:21:02.01 rheumatoid arthritis not the other kinds 00:21:02.88\00:21:04.98 of arthritis not gouty arthritis not, 00:21:05.01\00:21:06.97 we are not talking about that. But vegetarians 00:21:07.21\00:21:09.37 had less rheumatoid arthritis then did the 00:21:09.40\00:21:12.45 others, it wasn't a very, very major 00:21:12.48\00:21:14.70 reduction because some of the rheumatoid 00:21:15.45\00:21:16.83 arthritis is hereditary, it occurs much more 00:21:16.86\00:21:20.49 in women then it occurs in men. But it's an 00:21:20.55\00:21:23.47 inflammatory form of arthritis, the synovial 00:21:23.53\00:21:28.48 membrane which I mentioned at the beginning 00:21:28.52\00:21:30.03 becomes inflamed and it becomes inflamed 00:21:30.55\00:21:33.86 probably because antibodies are attacking 00:21:33.89\00:21:36.39 it. Now, what triggers that immune response 00:21:36.46\00:21:41.60 that the body is rejecting its own joint 00:21:41.63\00:21:44.13 we're not sure about it, it may be that there 00:21:44.24\00:21:46.28 are infectious agents that are doing that, 00:21:46.31\00:21:48.23 it may be that there are something in the 00:21:48.56\00:21:50.74 environment there maybe some toxic or 00:21:50.79\00:21:53.26 something that we are eating that is 00:21:53.40\00:21:56.22 stimulating this and it may not be the same 00:21:56.92\00:21:59.10 factor for everybody, but the end 00:21:59.13\00:22:01.34 result is that the joint membranes become 00:22:01.37\00:22:04.53 very inflamed and those inflammatory 00:22:05.03\00:22:07.64 products that are poured into the joint 00:22:07.70\00:22:09.86 damage the cartilage. So, people who have 00:22:10.19\00:22:13.15 rheumatoid arthritis are subject to joint 00:22:13.54\00:22:16.78 damage as a result of inflammatory process 00:22:16.81\00:22:19.55 and today we're much more aggressive 00:22:20.04\00:22:22.15 in treating rheumatoid arthritis than we 00:22:22.51\00:22:24.30 used to be because it's recognized that, 00:22:24.35\00:22:27.13 that joint damage can take place early 00:22:27.40\00:22:30.18 in the onset. So, lets suppose I've been 00:22:30.42\00:22:34.22 diagnosed with rheumatoid arthritis but 00:22:34.27\00:22:37.26 I've heard from the supplement people that 00:22:37.29\00:22:41.25 they are some alternatives out there, 00:22:41.42\00:22:43.53 is it wise to experiment with some of those 00:22:43.93\00:22:46.21 alternatives while that process is going 00:22:46.24\00:22:48.80 on damaging my joints. Chondroitin has been 00:22:48.83\00:22:53.46 recommended as a treatment, some of 00:22:54.99\00:22:58.39 those alternatives, "alternatives" may not 00:22:58.50\00:23:03.34 be alternatives for too long because once 00:23:03.57\00:23:06.60 they are shown to work they will then be 00:23:06.69\00:23:09.99 incorporated into the normal treatment. 00:23:10.02\00:23:13.21 But, a person who has an acute flare-up, 00:23:13.70\00:23:15.71 for instance somebody may start with 00:23:15.80\00:23:18.01 rheumatoid arthritis in a very, very 00:23:18.04\00:23:20.44 dramatic way. Rheumatoid arthritis 00:23:21.89\00:23:24.22 usually tends to be bilateral in its 00:23:24.25\00:23:26.10 distribution, it may effect the joints of the 00:23:26.13\00:23:29.57 hands, the elbow, knees, it doesn't usually 00:23:29.60\00:23:32.31 effect the vertebral column, the spinal 00:23:32.38\00:23:33.92 column. But if you have a very sudden 00:23:33.98\00:23:37.10 onset of rheumatoid arthritis then that is 00:23:37.18\00:23:41.52 probably not the way for alternatives. 00:23:41.62\00:23:44.53 How are they treated? Well they, 00:23:44.56\00:23:45.84 what do they do for, if you got it? 00:23:45.87\00:23:47.21 They have many different treatments, 00:23:47.43\00:23:49.33 some of the newer treatments that have 00:23:49.81\00:23:52.01 been used actually interfere with the 00:23:52.12\00:23:56.40 immune process itself, and so what they 00:23:58.07\00:24:01.40 do as they try to intercept one of them 00:24:01.45\00:24:04.93 is called Etanercept, it tries to intercept 00:24:04.96\00:24:08.80 this reaction of rejection of your own 00:24:09.34\00:24:13.14 synovial tissue and these are very, very 00:24:13.55\00:24:17.16 interesting medications, they are working 00:24:17.60\00:24:20.97 well, but I don't believe personally that 00:24:21.00\00:24:24.69 we have had a long enough experience to 00:24:24.84\00:24:27.80 be able to say definitively that they 00:24:27.83\00:24:29.96 can't have some serious side-effects 00:24:30.25\00:24:32.56 and so, since they is a cure insight? 00:24:32.99\00:24:35.07 I don't know that there is a, that we 00:24:35.53\00:24:37.83 could say that at this point in time we have 00:24:37.92\00:24:39.90 a cure insight, I think our understanding 00:24:39.93\00:24:42.30 of rheumatoid has become very, very 00:24:42.33\00:24:44.57 profound. Rheumatologist understand in a, 00:24:44.60\00:24:48.21 to a great extend and it could well be 00:24:48.34\00:24:51.20 that research into rheumatoid is going to 00:24:51.31\00:24:53.75 give us a benefit. I think I'd like to sort 00:24:53.85\00:24:57.60 of rather than focus on the drug therapy 00:24:57.72\00:24:59.85 and people are gonna have to see their 00:24:59.89\00:25:00.93 doctor for the drug therapy. I get that 00:25:00.99\00:25:02.16 yeah. I would like to talk about some 00:25:02.19\00:25:05.50 of the things that people can do who 00:25:05.56\00:25:07.68 have rheumatoid arthritis. Yes, some of 00:25:07.77\00:25:09.15 the treatments, some of the home, 00:25:09.18\00:25:11.50 type of, some of the methods of management, 00:25:11.84\00:25:13.93 okay. It's very, very important for a person 00:25:14.13\00:25:16.86 with rheumatoid arthritis to avoid trauma 00:25:16.93\00:25:19.03 to the joints and yet, like lifting heavy 00:25:19.14\00:25:22.07 weights, like lifting heavy weights, running, 00:25:22.22\00:25:24.01 straining, running putting damn, putting 00:25:24.07\00:25:26.80 excessive pressure and yet it's very, 00:25:27.03\00:25:29.94 very important that the range of 00:25:30.04\00:25:32.63 movement of that joint should be 00:25:32.68\00:25:34.77 maintained. So, lifting weights is probably, 00:25:34.80\00:25:39.16 need to be maintain. We need to maintain 00:25:39.24\00:25:41.53 the range of movement, we need to 00:25:41.68\00:25:44.38 maintain the strength, we need to protect 00:25:44.43\00:25:48.15 from trauma, so this is a situation where 00:25:48.51\00:25:51.10 we need gentle exercise, not, not a 00:25:51.13\00:25:55.57 strenuous exercise and the strengthening, 00:25:55.68\00:25:58.40 the muscle strengthening may be done 00:25:58.52\00:26:00.48 through isometric type of exercises. 00:26:00.71\00:26:02.84 You all remember Charles Atlas, I'm sure 00:26:02.92\00:26:04.95 you don't remember, some of the older 00:26:05.71\00:26:07.31 people remember Charles. I saw ads that 00:26:07.34\00:26:08.87 actually its Charles Atlas method, that's 00:26:08.90\00:26:11.18 where we flex our muscles but we don't 00:26:11.21\00:26:13.08 move the joint, that can maintain the 00:26:13.16\00:26:15.37 strength without traumatizing the joint. 00:26:15.41\00:26:19.15 So that is very important, swimming is a 00:26:19.60\00:26:22.59 weight less situation but you can go 00:26:22.67\00:26:26.27 through the full range of exercise, now you 00:26:26.35\00:26:28.57 always remember, the rheumatologist when 00:26:28.68\00:26:30.13 I was in Edinburgh Medical School there, 00:26:30.19\00:26:32.63 he said if you ever grow and you guys 00:26:32.87\00:26:34.68 are going to grow in prestige in the medical 00:26:34.71\00:26:37.83 fashion one day you might be incharge of a 00:26:37.94\00:26:39.96 rheumatology unit, remember the most 00:26:40.36\00:26:42.80 important piece of equipment is a 00:26:43.20\00:26:44.78 therapeutic swimming pool, and whirlpools 00:26:44.81\00:26:47.73 and swimming pools are a very, very 00:26:47.77\00:26:49.66 useful way for people with arthritis to be 00:26:49.69\00:26:52.12 able to exercise and yet not to damage 00:26:52.15\00:26:55.16 their joints. Well, this has been a pleasure 00:26:55.27\00:26:58.87 today, discussing this topic I think this 00:26:58.90\00:27:02.90 condition is wear and tear our human 00:27:03.12\00:27:05.19 body certainly effects a lot of people and 00:27:05.35\00:27:08.80 lets just, in about 30 seconds recap of 00:27:09.31\00:27:12.84 some of the things that we can do ourselves. 00:27:12.98\00:27:15.29 Increase our flexibility, increase our 00:27:16.02\00:27:18.82 strength gently, gently, because we don't 00:27:18.96\00:27:22.20 want to put any pressure right on our 00:27:22.23\00:27:24.19 joints. Is that we can apply hot and cold, 00:27:24.25\00:27:28.96 yes that, mild hot, hydro therapy, 00:27:29.54\00:27:31.36 hot and cold and watch the thermo for, 00:27:31.59\00:27:33.28 don't let that automatic thing work, 00:27:33.47\00:27:35.98 'cause you're gonna burn yourself, 00:27:36.07\00:27:37.25 you don't want to loose your fingers like 00:27:37.89\00:27:39.29 I have? Not for that reason, for another 00:27:39.32\00:27:41.29 reason but, so these are some of the 00:27:41.32\00:27:44.00 things we can do and be sure you get 00:27:44.03\00:27:46.80 good health.
