I am a board certified 00:00:46.07\00:00:48.90 family physician and I am the 00:00:48.93\00:00:50.79 Medical Director of Vibrant Health Family 00:00:50.82\00:00:53.08 Medicine at St. Louis, Missouri. 00:00:53.11\00:00:54.88 Today, we want to discuss prevention 00:00:54.91\00:00:57.40 and screening across the lifespan. 00:00:57.43\00:00:59.72 Let's get an idea what we mean by prevention 00:01:01.26\00:01:04.43 versus screening. Let's say that you have a 00:01:04.46\00:01:08.12 brand new car and every 3 months you take 00:01:08.15\00:01:12.67 it to auto zone for them to check the tires, 00:01:12.70\00:01:15.37 check the oil and check under the hood, 00:01:16.07\00:01:21.19 but you don't ever do anything yourself, 00:01:22.28\00:01:25.02 you don't put the air in the tires, 00:01:25.05\00:01:28.00 you don't change the oil, you use 00:01:28.03\00:01:32.06 whatever gas you want to use. 00:01:32.09\00:01:33.69 Would that be prevention for your car 00:01:35.12\00:01:38.47 or would that just be screening? 00:01:38.50\00:01:40.35 You have got it, it would be screening. 00:01:41.69\00:01:44.27 Simply just looking for a problem 00:01:44.30\00:01:46.62 that is different from prevention, 00:01:48.14\00:01:49.85 where you actively try to do something to make 00:01:49.88\00:01:53.75 a difference or to prevent something 00:01:53.78\00:01:55.40 from happening that's why you use 00:01:55.43\00:01:57.21 the correct gasoline designed for that car, 00:01:57.24\00:02:01.28 that's why you check the oil levels, 00:02:01.31\00:02:03.67 that's why you check the tires, 00:02:03.70\00:02:06.03 that's why you winterize the car. 00:02:06.98\00:02:08.80 That is prevention and in addition 00:02:09.73\00:02:11.81 you want to take it into the checks 00:02:11.84\00:02:13.87 so that other things are detected that 00:02:13.90\00:02:16.41 you may not have detected. 00:02:16.44\00:02:17.49 That is the same concept that we are 00:02:18.73\00:02:21.01 talking about for human beings because truly 00:02:21.04\00:02:26.45 we are faithfully Wonderfully Made 00:02:26.48\00:02:27.93 and we want to do the best that we can do 00:02:27.96\00:02:30.00 to prevent certain diseases occurring 00:02:30.03\00:02:33.22 and to screen for other diseases so that 00:02:33.25\00:02:37.08 we can intervene quickly. And so 00:02:37.11\00:02:41.26 let's talk about prevention for the 00:02:41.29\00:02:44.32 infants and toddlers. The number one cause 00:02:44.35\00:02:48.55 of death for infants and toddlers are 00:02:48.58\00:02:52.15 unintentional injury or accidents. 00:02:52.18\00:02:54.61 In the infant, accidents are a concern leaving 00:02:55.80\00:03:02.19 the child unattended, thinking that 00:03:02.22\00:03:04.60 the infant will not rollover, he can fall 00:03:04.63\00:03:07.84 and have an injury, not properly restraining 00:03:07.87\00:03:11.06 that infant in a car seat. Can the infant 00:03:11.09\00:03:14.83 in your arms thinking that if those are impact 00:03:14.86\00:03:18.74 you would hold on to that infant. 00:03:18.77\00:03:20.81 But in fact physiologically speaking 00:03:20.84\00:03:23.36 you would not because with the impact 00:03:23.39\00:03:26.40 you would automatically release that infant 00:03:26.43\00:03:29.21 who would then proceed towards the windscreen 00:03:29.24\00:03:31.99 and get a very damaging injury. 00:03:32.02\00:03:36.21 So infants should be properly restrained 00:03:36.24\00:03:39.24 in their car seats. What else causes death 00:03:39.27\00:03:43.36 in infants? Sudden infant death syndrome. 00:03:43.39\00:03:46.73 What can we done to prevent that? 00:03:47.47\00:03:49.28 There is a large campaign called 00:03:50.12\00:03:52.14 Back to Sleep, that is put your infant to sleep 00:03:52.17\00:03:56.26 on his or her back. Not on the front, 00:03:56.29\00:04:00.03 on the front seat shown to increase the risk 00:04:00.06\00:04:04.05 for sudden infant death, where there is no 00:04:04.08\00:04:07.23 obvious reason as to why the child died. 00:04:07.26\00:04:10.37 Place your child to sleep on the back 00:04:11.29\00:04:13.81 or on the sides to prevent sudden 00:04:13.84\00:04:17.54 infant death. If you are a smoker, 00:04:17.57\00:04:20.96 you will increase the risk for 00:04:21.92\00:04:24.23 sudden infant death in your child. 00:04:24.26\00:04:26.30 If you breastfeed your infant you will decrease 00:04:27.85\00:04:31.28 the risk for sudden infant death. 00:04:31.31\00:04:33.00 So there are three major things that 00:04:33.76\00:04:35.85 you can do to prevent sudden infant 00:04:35.88\00:04:37.85 death syndrome. Back to sleep, 00:04:37.88\00:04:41.12 breastfeed your infant and clearly avoid 00:04:41.15\00:04:46.72 smoking around that child. Even if you go to 00:04:46.75\00:04:51.46 smoke outside those particles of smokes 00:04:51.49\00:04:53.97 are still on your clothes and so when you 00:04:54.00\00:04:56.50 come to pick up your infant, 00:04:56.53\00:04:58.03 your infant is inhaling that smoke 00:04:58.06\00:05:00.89 from your clothes. So sudden infant death 00:05:00.92\00:05:05.38 and accidents are a concern in infants 00:05:05.41\00:05:08.77 and what about toddlers. There number one killer 00:05:08.80\00:05:12.61 is also unintentional injury or accidents. 00:05:12.64\00:05:16.44 Toddlers just like to investigate 00:05:16.47\00:05:20.40 that's what they do. They go into places 00:05:20.43\00:05:24.47 they take things out, they put things 00:05:24.50\00:05:26.45 in their mouth. It is important that 00:05:26.48\00:05:30.32 you toddler proof your home, 00:05:30.35\00:05:32.18 get down in your hands and knees, 00:05:32.21\00:05:34.47 crawl around your house. Look for things 00:05:34.50\00:05:37.53 that a toddler or crawling infant 00:05:37.56\00:05:40.23 will get into. Codes that are hanging 00:05:40.26\00:05:43.02 down too low, lose screws, outlets that are 00:05:43.05\00:05:48.58 unprotected, that little curious finger will put, 00:05:48.61\00:05:51.93 little finger right into that outlet 00:05:51.96\00:05:54.50 and cause electrocution, unintentional injury, 00:05:54.53\00:05:58.29 accidents, drowning. Leaving a toddler 00:05:59.14\00:06:03.75 or infant unsupervised in a bath. 00:06:03.78\00:06:07.87 Hard to believe, but even just a few inches 00:06:08.88\00:06:11.96 of water can cause drowning 00:06:11.99\00:06:13.49 an infant or toddler. Leaving buckets 00:06:13.52\00:06:18.40 unattended that have water in it, 00:06:18.43\00:06:20.26 toddler will go down to go into that bucket 00:06:20.29\00:06:23.28 make it stuck, make it stuck in the toilet 00:06:23.31\00:06:26.66 and cause drowning and so having a safety 00:06:26.69\00:06:32.29 latch on that toilet seat would be 00:06:32.32\00:06:34.81 very important. It's important so 00:06:34.84\00:06:38.04 you can talk to your physician, 00:06:38.07\00:06:39.40 your family doctor, your child's doctor 00:06:39.43\00:06:42.85 and ask them for a checklist so that 00:06:42.88\00:06:45.51 you can safe proof your home from these 00:06:45.54\00:06:49.75 unintentional injuries. And what about 00:06:49.78\00:06:53.62 the young child? The young child is a 00:06:53.65\00:06:56.88 vivacious and very active, loves to ride 00:06:56.91\00:07:01.41 the bicycle but parents it's up to 00:07:01.44\00:07:04.86 you to make sure that you impress upon 00:07:04.89\00:07:07.67 your young child to always wear a 00:07:07.70\00:07:10.81 bicycle helmet. And you need to show by example 00:07:10.84\00:07:15.00 if you don't wear a helmet your child 00:07:15.03\00:07:18.06 certainly will not wear a helmet. 00:07:18.09\00:07:19.74 But if your child is riding along 00:07:19.77\00:07:22.26 and it is not have to be at high speed 00:07:22.29\00:07:24.08 and bums onto a curve on a stone and goes over 00:07:24.11\00:07:28.10 that handle bar. When that child's head hits 00:07:28.13\00:07:31.34 the concrete which one do you think is going 00:07:31.37\00:07:34.21 to give? It's not the concrete, 00:07:34.24\00:07:36.98 traumatic head injuries in children occur 00:07:37.90\00:07:41.28 because they are not wearing 00:07:41.31\00:07:43.31 protective headgear. The young child, 00:07:43.34\00:07:48.69 what about the teenager, car accidents, 00:07:48.72\00:07:54.65 riding in a car without wearing a seatbelt 00:07:56.03\00:07:59.51 it's unfortunate that teenagers think that 00:07:59.54\00:08:02.94 they will live forever and that they 00:08:02.97\00:08:04.60 can do daring acts. They drive at high speed 00:08:04.63\00:08:10.06 unrestrained sometimes under the 00:08:10.09\00:08:13.08 influence of alcohol. That is a recipe 00:08:13.11\00:08:16.16 for a head injury it may not be death, 00:08:16.19\00:08:19.08 but that teenager will never be the same again. 00:08:19.11\00:08:22.79 Prevention, prevention, prevention is the key. 00:08:22.82\00:08:27.89 Do not drive in a car with somebody else 00:08:28.51\00:08:31.39 has been on to the influence of alcohol. 00:08:31.42\00:08:33.22 Always wear your seatbelt when you drive. 00:08:33.25\00:08:36.60 Always wear a helmet if you're riding a bicycle 00:08:38.15\00:08:41.09 or if you're rollerblading. 00:08:41.12\00:08:42.95 If there is risk for a head injury with the 00:08:44.51\00:08:48.06 solid object you need to wear 00:08:48.09\00:08:50.87 protective headgear. Also in teenagers 00:08:50.90\00:08:57.99 depending on your location, 00:08:58.02\00:09:00.56 infection is also a common killer. 00:09:00.59\00:09:05.42 In particular, we are talking about 00:09:05.45\00:09:08.35 sexually transmitted diseases 00:09:08.38\00:09:10.52 and in particular we are referring to HIV 00:09:10.55\00:09:14.68 in subsequent age. It is important that teenagers 00:09:14.71\00:09:21.46 recognize the danger of multiple sexual partners 00:09:21.49\00:09:26.13 or even sexual activity before marriage. 00:09:26.16\00:09:29.63 Multiple sexual partners increases the risk for 00:09:30.41\00:09:34.12 sexual transmit disease, Herpes, Chlamydia, 00:09:34.15\00:09:37.98 HIV and HIV can result in death. 00:09:38.01\00:09:42.95 Although some of the other 00:09:42.98\00:09:45.02 sexually transmitted disease do not result 00:09:45.05\00:09:47.06 in death they certainly result in problem 00:09:47.09\00:09:50.09 such as infertility. And so our recommendation 00:09:50.12\00:09:53.83 for teenagers is chastity not just 00:09:53.86\00:09:57.15 abstinence, but chastity a lifestyle of purity 00:09:57.18\00:10:00.69 before marriage and continued into marriage 00:10:02.08\00:10:05.49 with one partner, your husband or wife. 00:10:05.52\00:10:10.28 And teens, so from teenagers we go onto 00:10:11.39\00:10:14.37 young adults and what we are finding 00:10:14.40\00:10:20.13 as we traverse the lifespan is that it goes 00:10:20.16\00:10:23.74 from unintentional injuries to more 00:10:23.77\00:10:26.67 lifestyle conditions that become the major 00:10:26.70\00:10:30.66 killers and so in the young and even 00:10:30.69\00:10:36.06 middle age adult accidents continued 00:10:36.09\00:10:38.67 to be the number one killer. 00:10:38.70\00:10:40.96 Again wearing your seatbelts, 00:10:41.79\00:10:44.99 not doing things that are dangerous, 00:10:46.17\00:10:48.44 driving under the influence of alcohol, 00:10:49.19\00:10:51.87 these are all important preventive strategies 00:10:51.90\00:10:55.83 to decrease the risk for injury and death. 00:10:55.86\00:11:00.20 But as we approach the middle years 00:11:01.00\00:11:03.72 and on into the older years then what comes 00:11:03.75\00:11:08.18 to the forefront is heart disease 00:11:08.21\00:11:11.00 and cancer. Cancer, we can prevent 00:11:11.03\00:11:18.14 as well as screenful, what are the cancers 00:11:18.17\00:11:21.93 that we are concerned about? 00:11:21.96\00:11:23.71 When in young man we are concerned about 00:11:24.50\00:11:27.57 testicular cancer and that can easily be 00:11:27.60\00:11:31.45 screenful by the young man himself doing 00:11:31.48\00:11:33.99 monthly testicular exams you may notice a lump 00:11:34.02\00:11:37.48 on the testicle. It's not necessary painful 00:11:37.51\00:11:41.28 and is not necessary large, 00:11:41.31\00:11:42.98 but it is something that is unusual 00:11:43.01\00:11:46.73 and different. If you find this 00:11:46.76\00:11:49.38 you need to see your physician 00:11:49.41\00:11:50.94 so the appropriate workup can be done. 00:11:50.97\00:11:53.36 Testicular cancer. Colorectal cancer is the 00:11:53.39\00:12:00.95 number two cancer killer. Number one is 00:12:01.74\00:12:05.81 lung cancer and we associate lung cancer 00:12:05.84\00:12:10.69 with tobacco smoking. So avoiding tobacco smoke 00:12:10.72\00:12:16.52 will dramatically decrease your risk 00:12:16.55\00:12:19.46 or prevent lung cancer from occurring 00:12:19.49\00:12:23.86 but not just you actively smoking 00:12:24.94\00:12:28.42 we are also concerned about passive smoking. 00:12:28.45\00:12:32.27 What is passive smoking? You inhaling 00:12:32.30\00:12:36.68 the cigarette smoke from somebody else may be 00:12:36.71\00:12:40.38 your husband or wife smokes. 00:12:40.41\00:12:42.74 Well they increase in their risk as well as 00:12:43.73\00:12:46.57 your risk for smoking related illnesses 00:12:46.60\00:12:49.89 and is not just lung cancer, heart disease, 00:12:50.46\00:12:54.33 bladder cancer, cervical cancer, renal cancer. 00:12:54.36\00:13:00.31 There are problems with actively smoking 00:13:00.34\00:13:03.97 and passive smoking, so we do want you to avoid 00:13:04.00\00:13:10.23 tobacco smoke in all its forms. 00:13:10.26\00:13:12.93 Some my patients say I don't smoke, 00:13:12.96\00:13:15.31 but I chew tobacco. Well that's a problem 00:13:15.34\00:13:17.94 because when you chew tobacco you increase 00:13:17.97\00:13:20.88 the risk for cancer of the mouth and throat 00:13:20.91\00:13:25.74 and if you are smoking and drinking alcohol 00:13:25.77\00:13:29.89 together that is a very bad combination. 00:13:29.92\00:13:33.40 Smoking will initiate a cancer. 00:13:34.29\00:13:36.63 Alcohol will fan the flames of that, 00:13:36.66\00:13:42.73 so it is a promoter we say the initiator 00:13:42.76\00:13:47.15 is a tobacco smoke, the promoter is the alcohol. 00:13:47.18\00:13:50.38 Alcohol and tobacco smoke increases the risk 00:13:50.41\00:13:53.54 for head and neck cancers. So how would 00:13:53.57\00:13:57.48 you want to prevent those quit smoking, 00:13:57.51\00:14:00.73 don't drink alcohol. So tobacco smoking 00:14:00.76\00:14:07.42 and lung cancer is well established. 00:14:07.45\00:14:09.92 What about the second cancer? 00:14:09.95\00:14:13.22 That's a leading cause of death that is 00:14:13.25\00:14:16.09 colorectal cancer. How do we prevent 00:14:16.12\00:14:19.67 that there was a large trial that was done 00:14:19.70\00:14:23.63 that look to the lifestyle that promoted 00:14:23.66\00:14:27.64 colorectal cancer and it was found that a diet 00:14:27.67\00:14:32.15 that was plant based fruits, vegetable, 00:14:32.18\00:14:37.23 whole grains, low fat dairy, lean meats 00:14:37.26\00:14:41.18 if at all was associated with a very low risk 00:14:41.21\00:14:45.54 of colorectal cancer. On the other hand, 00:14:45.57\00:14:48.48 a diet that had cakes, pastries, ice cream, 00:14:48.51\00:14:54.16 pan cakes, pretzels, soda's that was 00:14:54.19\00:14:59.93 associated with a high risk of 00:14:59.96\00:15:02.77 colorectal cancer. So, we see prevention 00:15:02.80\00:15:07.38 would be to change your diet and your lifestyle. 00:15:07.41\00:15:12.37 Also exercises shown to lower 00:15:12.40\00:15:14.53 colorectal cancer risk. But how do we screen 00:15:14.56\00:15:19.10 for colorectal cancer? We screen 00:15:19.13\00:15:23.50 by doing a process called colonoscopy. 00:15:23.53\00:15:26.44 Some people say I don't have any problems, 00:15:27.41\00:15:31.56 I don't feel any abdominal pain, 00:15:31.59\00:15:33.31 I don't have a change in bowel habit, 00:15:33.34\00:15:35.35 I don't have blood in my stool. 00:15:35.38\00:15:37.05 So I don't need to get a colonoscopy done. 00:15:37.08\00:15:40.24 Wrong, that is the very time that 00:15:41.08\00:15:44.48 you need to have a colonoscopy done 00:15:44.51\00:15:46.56 because that's when they will find the cancer 00:15:46.59\00:15:48.99 in its earliest stages as a tiny polyp. 00:15:49.02\00:15:52.59 They find that tiny polyp that's an 00:15:52.62\00:15:54.80 early cancer they snip it off, 00:15:54.83\00:15:57.19 100 percent cure. On the other hand, 00:15:57.22\00:16:01.37 if you wait until you have symptoms of 00:16:01.40\00:16:04.73 weight loss, abdominal pain, 00:16:04.76\00:16:07.17 blood in the stool by that time you may have 00:16:07.20\00:16:10.37 a large cancer that is actually spread 00:16:10.40\00:16:14.07 into the lymph nodes and on into the liver 00:16:14.10\00:16:16.59 and to other vital organs of the body 00:16:16.62\00:16:18.45 in which case there is not 100 percent cure. 00:16:18.48\00:16:22.43 When do we screen for colon cancer? 00:16:24.61\00:16:27.33 The current guidelines recommend that persons 00:16:28.10\00:16:32.17 50 years or older with no other risk facts for 00:16:32.20\00:16:36.58 colorectal cancer should be screened for 00:16:36.61\00:16:40.23 colon cancer. And the best screening would be 00:16:40.26\00:16:44.61 if a colonoscopy done by a trained specialist, 00:16:44.64\00:16:49.11 a gastroenterologist or a colorectal surgeon. 00:16:49.14\00:16:53.33 On the other hand if your mother, 00:16:54.57\00:16:58.04 father, brother or sister had colon cancer 00:16:58.07\00:17:02.13 when they were 40 years old then you would 00:17:02.16\00:17:07.12 need to be screened 10 years prior to that 00:17:07.15\00:17:10.29 because it takes about 5 to 7 years 00:17:10.32\00:17:13.05 for a colon cancer to grow. 00:17:13.08\00:17:14.66 So if your first degree relative had 00:17:15.47\00:17:19.04 colon cancer 40, you should get screened 00:17:19.07\00:17:22.29 at 30. But if there is no family history 00:17:22.32\00:17:26.72 or no other underlying medical condition 00:17:27.74\00:17:30.28 such as ulcerative colitis that would 00:17:30.31\00:17:33.00 increase your risk for colon cancer 00:17:33.03\00:17:35.18 then the only screening time would began 00:17:35.21\00:17:38.51 at age 50. Please don't put it off, 00:17:38.54\00:17:41.65 in the hands of a well trained specialist 00:17:41.68\00:17:45.69 the procedure can be done quickly 00:17:46.90\00:17:49.02 and uneventfully. And then it would not 00:17:49.05\00:17:51.94 have to be done for another 10 years 00:17:51.97\00:17:53.52 and of course during those 10 years 00:17:55.20\00:17:57.07 you are gonna be doing all those things that 00:17:57.10\00:17:58.98 we know decreases the risk for colon cancer 00:17:59.01\00:18:02.00 as far as diet is concerned. 00:18:02.03\00:18:04.06 Fruits, vegetables, whole grains, 00:18:04.09\00:18:07.16 foods that are rich in calcium. 00:18:07.19\00:18:09.15 Exercise that decreases inflammation 00:18:11.04\00:18:14.64 because the current thought is that 00:18:15.67\00:18:17.38 the colon cancers actually a part of an 00:18:17.41\00:18:20.78 inflammatory process that is going on. 00:18:20.81\00:18:24.19 Certainly fiber is important 00:18:25.87\00:18:27.66 when it comes to colon health, 00:18:27.69\00:18:29.70 but more so then fiber red meat has been linked 00:18:31.56\00:18:36.03 quite strongly with colorectal cancer 00:18:36.06\00:18:38.97 and so if you want to prevent colon cancer 00:18:40.12\00:18:43.21 you will avoid red meats, you will 00:18:43.24\00:18:46.87 exercise, you will avoid the regular use of 00:18:46.90\00:18:51.68 cakes, pastries, ice creams, sodas. 00:18:51.71\00:18:54.24 Instead you will eat fruits, vegetables, 00:18:54.27\00:18:57.70 whole grains and foods that are rich in calcium 00:18:57.73\00:19:02.58 such as green leafy vegetables. 00:19:02.61\00:19:04.19 So colon cancer that can be screened for, 00:19:06.65\00:19:09.49 it can be prevented. What is some of the 00:19:09.52\00:19:12.00 other cancers in woman? Breast cancer. 00:19:12.03\00:19:14.94 In woman between ages 40 and 50, 00:19:16.02\00:19:19.44 the number one cause of death is an 00:19:19.47\00:19:21.61 heart disease, it's in fact breast cancer 00:19:21.64\00:19:24.84 and so breast health is very important 00:19:24.87\00:19:28.63 that should be a strong focus in those years. 00:19:28.66\00:19:31.44 What are some of the things that we know 00:19:31.47\00:19:34.15 that help prevent breast cancer? 00:19:34.18\00:19:37.55 You have got it. Exercise a plant based 00:19:38.70\00:19:43.09 diet these will decrease the risk of 00:19:43.12\00:19:46.81 breast cancer, but if you have a 00:19:46.84\00:19:49.61 family history you are clearly at higher risk, 00:19:49.64\00:19:52.61 if your mother or sister or maternal aunt 00:19:52.64\00:19:56.82 had breast cancer you will clearly 00:19:56.85\00:19:59.44 at a higher risk. That doesn't mean it is 00:19:59.47\00:20:02.67 inevitable, it means that you have to more 00:20:02.70\00:20:05.58 aggressive to do those things to protect 00:20:05.61\00:20:08.44 your breast. Fiber in fact a special kind of 00:20:08.47\00:20:13.90 fiber that we find in flax is very important 00:20:13.93\00:20:16.74 for breast health, lignans very important 00:20:16.77\00:20:21.28 ground flax seed sprinkled on whole grain 00:20:21.31\00:20:24.85 cereal will go a long way to protect 00:20:24.88\00:20:28.07 your breasts from breast cancer as we exercise, 00:20:28.10\00:20:32.45 as we eating the cruciferous vegetables 00:20:32.48\00:20:35.94 broccoli, cauliflower, Brussels sprouts, kale. 00:20:35.97\00:20:39.10 There is nothing new under the sun 00:20:40.67\00:20:42.78 as song of Solomon says still a diet 00:20:42.81\00:20:48.14 that is healthy coupled with physical activity 00:20:49.16\00:20:52.56 is what promotes cellular health whether 00:20:52.59\00:20:57.04 it's health of the breast tissue, 00:20:57.07\00:20:58.72 whether it's health of the colon tissue, 00:20:58.75\00:21:00.81 whether it's health of the testicular tissue, 00:21:00.84\00:21:02.94 cellular health and integrity is governed 00:21:03.91\00:21:07.98 by the lifestyle and activity that is 00:21:08.01\00:21:10.96 occurring in ones body.
