Hello and welcome to this segment of "Wonderfully Made" 00:00:56.94\00:01:01.18 I'm Dr. Neil Nedley, practicing physician 00:01:01.21\00:01:04.41 and author of the book entitled... 00:01:04.44\00:01:06.55 "DEPRESSION THE WAY OUT" 00:01:06.58\00:01:08.36 which is actually the title of our presentation today. 00:01:08.39\00:01:11.92 Depression is an epidemic, according to the 00:01:11.95\00:01:15.87 Surgeon General of the United States in America 00:01:15.90\00:01:18.90 and these numbers of depressed people actually are continuing 00:01:18.93\00:01:23.77 to go up... and this does not have to be! 00:01:23.80\00:01:26.28 It's a sad fact, and it's really a fact due to lack of education, 00:01:26.31\00:01:30.98 and lack of motivation, as well as some mild forms of 00:01:31.01\00:01:36.53 brainwashing that has taken place here in America... 00:01:36.56\00:01:42.09 and I would like to actually go through each one of those 00:01:42.10\00:01:44.81 statements that I've made in a little more detail 00:01:44.84\00:01:47.42 so you can see exactly where I'm coming from 00:01:47.45\00:01:50.39 in this segment. 00:01:50.42\00:01:52.58 First of all, the impact of depression 00:01:52.59\00:01:54.64 worldwide is astounding! 00:01:54.67\00:01:56.56 The number of people developing depression has increased 00:01:56.59\00:02:00.22 since 1915. 00:02:00.25\00:02:02.80 Now we have been studying this disease since the turn 00:02:02.83\00:02:05.73 of the century at 1900... 00:02:05.76\00:02:08.12 And so, when you consider from 1915 on... 00:02:08.15\00:02:11.97 each successive generation has had more and more depression 00:02:12.00\00:02:15.66 It really raises some questions! 00:02:15.69\00:02:18.03 First of all, I don't think anyone would question 00:02:18.06\00:02:20.82 that today we have more FUN things to do than ever before 00:02:20.83\00:02:26.32 in human history, but even though we have more 00:02:26.35\00:02:29.06 fun things to do, there are more people that are depressed 00:02:29.09\00:02:34.04 than ever before in human history. 00:02:34.05\00:02:36.42 First of all, that should tell us that doing these 00:02:36.45\00:02:39.36 fun things does not necessarily PREVENT or even TREAT depression 00:02:39.37\00:02:44.44 It may make one feel better just temporarily, 00:02:44.47\00:02:48.28 or transiently while they are doing the fun thing... 00:02:48.31\00:02:51.30 But after the fun thing is over, 00:02:51.33\00:02:52.98 often the individual will go down into a lower spirit 00:02:53.01\00:02:57.12 than they were before they did the entertaining activity. 00:02:57.15\00:03:02.50 The disease of depression is also striking at an earlier age. 00:03:02.53\00:03:06.68 In fact, we used to call this disease "the midlife crisis" 00:03:06.71\00:03:10.47 because it would change people's behavior so much 00:03:10.50\00:03:13.33 that there appeared to be a crisis standpoint, 00:03:13.36\00:03:16.70 a nervous breakdown that would occur in midlife 00:03:16.71\00:03:19.84 But, unfortunately, this disease is now striking very 00:03:19.87\00:03:23.78 frequently in this country by age 25. 00:03:23.81\00:03:26.74 In fact, there are many people, even in their adolescent years, 00:03:26.77\00:03:30.23 who are experiencing depression... 00:03:30.26\00:03:33.58 And, of course, as the Surgeon General reported in his 00:03:33.61\00:03:38.38 very exhaustive report on mental health 00:03:38.41\00:03:41.21 that came out recently... 00:03:41.22\00:03:42.70 Even children under the age of 8 can have major depression... 00:03:42.73\00:03:48.51 and this was unheard of in previous generations. 00:03:48.52\00:03:53.50 Overall, the risk of depression has increased over time 00:03:53.53\00:03:56.72 for ALL ages, and so it's not just a disease 00:03:56.73\00:04:00.37 of the adolescents, or those age 25... 00:04:00.40\00:04:03.20 It's not just a disease of those in midlife... 00:04:03.23\00:04:06.26 It actually is even more frequent during this particular 00:04:06.27\00:04:11.52 generation in the LATER generation... 00:04:11.53\00:04:14.40 The older an individual gets, the more likely they are 00:04:14.43\00:04:18.59 to suffer from this disease that we call "major depression" 00:04:18.62\00:04:24.62 The impact of depression worldwide is astounding 00:04:24.65\00:04:28.03 as you might guess. 00:04:28.06\00:04:29.79 At the time that we are speaking today, 00:04:29.82\00:04:32.06 200 million people are suffering from depression. 00:04:32.09\00:04:36.17 As a practicing internal medicine physician, 00:04:36.20\00:04:39.45 like myself, there are studies revealing that up to 00:04:39.48\00:04:43.60 1 in 3 people that I see, as an internal medicine physician, 00:04:43.63\00:04:48.21 actually has major depression. 00:04:48.24\00:04:51.68 Now this was shocking news to even myself, 00:04:51.71\00:04:54.55 after I became an internist... I went to medical school, 00:04:54.58\00:04:57.47 did my specialty training... actually has started practice 00:04:57.50\00:05:01.03 And a very interesting article came across my desk 00:05:01.06\00:05:05.04 in a very reputable internal medicine journal called... 00:05:05.07\00:05:07.79 "The Archives of Internal Medicine" 00:05:07.82\00:05:10.47 and it took a look at outpatient internal medicine practices. 00:05:10.50\00:05:13.98 And it found that up to 1 in 3 people 00:05:14.01\00:05:17.05 that internists see on a daily basis, 00:05:17.06\00:05:20.16 are suffering from depression. 00:05:20.19\00:05:22.77 Now, as an internal medicine physician, I must tell you 00:05:22.80\00:05:26.79 that I was not trained really 00:05:26.82\00:05:29.93 to understand the disease depression. 00:05:29.96\00:05:33.63 Now I had been through medical school... 00:05:33.66\00:05:35.34 I had performed psychiatric consultations. 00:05:35.37\00:05:39.09 I had done psychiatric rotations 00:05:39.12\00:05:41.60 I had sat through grand rounds with high-credentialed 00:05:41.63\00:05:46.07 psychiatrists explaining the disease depression, 00:05:46.10\00:05:49.12 as well as other mental diseases such as 00:05:49.15\00:05:51.31 bipolar disorder or schizophrenia, 00:05:51.34\00:05:53.68 or certain anxiety disorders. 00:05:53.71\00:05:55.81 And so I had learned these things in the process of 00:05:55.84\00:05:58.61 my training, as well as the process of my education 00:05:58.62\00:06:03.12 But what I was really trained to do was to treat disease 00:06:03.15\00:06:06.80 of the internal organs... that's why it's called an 00:06:06.83\00:06:09.03 internal medicine physician, as we are treating diseases 00:06:09.06\00:06:12.09 of the heart, the lungs, the intestinal tract, the kidneys, 00:06:12.12\00:06:16.33 the internal organs. 00:06:16.36\00:06:18.25 And what we are mainly utilized by in most sectors is to 00:06:18.28\00:06:24.22 find out why a person is suffering from certain symptoms. 00:06:24.25\00:06:30.48 We must ask WHY... is a very important question 00:06:30.51\00:06:33.55 for an internal medicine physician because if we 00:06:33.58\00:06:35.70 don't answer the WHY, actually a fatality can result 00:06:35.73\00:06:39.33 ...particularly if it's a serious disease. 00:06:39.34\00:06:41.68 So when other physicians such as family practitioners 00:06:41.71\00:06:45.22 or surgeons, or other specialties, 00:06:45.25\00:06:47.64 cannot really find out exactly what's going on, 00:06:47.67\00:06:50.74 they then refer that patient to me often... 00:06:50.77\00:06:53.66 to try to find out exactly what the disease is. 00:06:53.69\00:06:57.22 And, of course, they also wonder if it has to do with the 00:06:57.25\00:07:01.10 internal organs, and so that's why... partly they do this. 00:07:01.13\00:07:05.62 But when I realized, back to our main point, 00:07:05.63\00:07:08.09 that 1 out of 3 people that I was seeing were potentially 00:07:08.12\00:07:11.95 depressed, it kind of depressed me 00:07:11.98\00:07:15.01 because I didn't feel adequately prepared to handle those cases. 00:07:15.04\00:07:19.05 Yes, I was familiar with the medications of depression... 00:07:19.08\00:07:21.93 I had to be familiar with them 00:07:21.96\00:07:23.50 because so many of my patients were taking these medicines 00:07:23.53\00:07:26.47 ...I had to know their effects and their side effects, 00:07:26.50\00:07:28.91 as well as what to expect when taking these medicines. 00:07:28.94\00:07:33.12 I was familiar with counseling techniques for depression... 00:07:33.15\00:07:36.54 But yet I thought, if 1 out of 3 of my patients have this 00:07:36.57\00:07:41.65 this is potentially something I'm seeing more frequently 00:07:41.68\00:07:45.03 than any of the standard internal medicine diseases... 00:07:45.06\00:07:48.98 such as heart disease, or kidney disease, 00:07:49.01\00:07:52.08 or gastrointestinal disease. 00:07:52.09\00:07:54.37 And so I then embarked upon a serious study to try to find out 00:07:54.40\00:07:59.91 as much about this disease process as possible. 00:07:59.94\00:08:03.21 And in the process of finding out, 00:08:03.24\00:08:06.52 from standard medical sources, 00:08:06.55\00:08:08.42 and the standard medical literature, 00:08:08.45\00:08:09.98 I began to realize that there was much more to this 00:08:10.01\00:08:14.07 disorder... mental disorder called "depression" 00:08:14.10\00:08:18.56 than I had ever learned before. 00:08:18.59\00:08:21.62 There was much more to it than just a biological problem 00:08:21.65\00:08:25.39 where there was a chemical imbalance... 00:08:25.42\00:08:27.31 much more to it than just a social problem 00:08:27.34\00:08:30.47 where counseling was necessary, 00:08:30.50\00:08:32.97 and that MOST of the causes of depression 00:08:33.00\00:08:36.37 were actually being completely overlooked by the standard 00:08:36.40\00:08:41.40 healthcare practitioners that these people were seeing. 00:08:41.43\00:08:46.89 And this, of course, is an unfortunate situation. 00:08:46.92\00:08:50.00 When we have people who are depressed, and are seeing 00:08:50.01\00:08:53.61 practitioners for this disorder, 00:08:53.64\00:08:56.50 and they don't even recognize... 00:08:56.53\00:08:59.09 the patient doesn't, and neither do the caregivers, 00:08:59.10\00:09:01.37 the underlying reasons for the problem. 00:09:01.40\00:09:05.19 And if you never get to the underlying reasons of a problem, 00:09:05.20\00:09:08.59 you will NEVER find a cure for the problem. 00:09:08.62\00:09:11.79 The individual will always be dependent on 00:09:11.82\00:09:14.95 medication, or counseling, in order to keep 00:09:14.98\00:09:20.09 from being depressed. 00:09:20.12\00:09:21.64 And this is what we are finding in America today... 00:09:21.67\00:09:23.99 The antidepressant medications are skyrocketing... 00:09:24.02\00:09:27.25 As far as the numbers that are being used, we are dealing 00:09:27.28\00:09:31.07 with a disease that with JUST the medications, 00:09:31.10\00:09:34.10 people are spending billions of dollars a year 00:09:34.13\00:09:37.09 for these medications. 00:09:37.12\00:09:38.28 They take them like their morning vitamins... 00:09:38.31\00:09:40.62 and if they happen to miss a few doses, 00:09:40.65\00:09:43.64 they go into the doldrums of severe depression, 00:09:43.67\00:09:46.97 and they feel like they are going to be dependent 00:09:47.00\00:09:49.25 on these medications for life. 00:09:49.26\00:09:51.42 This is a sad fact, and one that does not have to be. 00:09:51.45\00:09:55.02 The important thing to understand, 00:09:55.05\00:09:58.59 as part of this presentation is... 00:09:58.62\00:10:00.70 Even though depression is common, you can find 00:10:00.73\00:10:04.35 the cause of depression, 00:10:04.38\00:10:06.31 and if you treat that underlying cause, then you can actually 00:10:06.34\00:10:10.49 have a cure for depression. 00:10:10.52\00:10:13.07 And in our experience, when this systematic approach 00:10:13.10\00:10:16.41 is outlined, that I have embarked on as part of my 00:10:16.44\00:10:19.68 serious study and implementation over 90% of patients 00:10:19.71\00:10:23.46 are able to be CURED from depression in a 20-week 00:10:23.49\00:10:27.06 period of time. 00:10:27.09\00:10:28.50 And this sounds, of course, very surprising 00:10:28.51\00:10:32.01 if you're a psychiatrist, or a psychologist, 00:10:32.04\00:10:34.23 that these types of numbers are accurate, 00:10:34.26\00:10:38.21 but indeed, in our practice, they are accurate... 00:10:38.24\00:10:40.98 And I think that it's not just dependent on me, 00:10:41.01\00:10:43.66 if the principles outlined are applied by psychologists 00:10:43.69\00:10:48.66 and psychiatrists, and other healthcare practitioners 00:10:48.69\00:10:51.84 as well as if patients become informed 00:10:51.87\00:10:54.65 about their disease process, there is no reason why 00:10:54.68\00:10:58.64 9 out of 10 depressed patients should still be depressed. 00:10:58.67\00:11:02.38 They can be, indeed, cured from their disease. 00:11:02.41\00:11:06.11 Now we mentioned the billions of dollars... 00:11:06.12\00:11:09.40 The Wall Street Journal recently came out with an article 00:11:09.43\00:11:12.64 showing that 70 billion dollars are spent on the disease of 00:11:12.67\00:11:18.36 depression every year. 00:11:18.39\00:11:20.30 That actually is a figure that goes above the cost of 00:11:20.33\00:11:24.51 heart disease in this country. 00:11:24.54\00:11:26.34 And we all know that heart disease is the #1 cause of death 00:11:26.37\00:11:29.71 and there are all sorts of costs associated with heart disease 00:11:29.74\00:11:33.05 But this 70 billion dollar figure puts this into a MAJOR 00:11:33.08\00:11:38.20 cost problem in the United States 00:11:38.23\00:11:40.09 and that cost is not only the medications, 00:11:40.12\00:11:42.07 not only seeing the doctor, 00:11:42.08\00:11:44.58 not only paying for the psychologist to perform 00:11:44.61\00:11:48.34 psychotherapy, but also the cost as far as lost productivity 00:11:48.35\00:11:52.35 when you're not productive on the job, 00:11:52.38\00:11:53.96 or even lost days at work because of severe depression 00:11:53.99\00:11:57.87 in not being able to function. 00:11:57.90\00:12:01.62 After the attack on America on September 11, 2001... 00:12:01.65\00:12:07.46 we found that this disease actually went up that much more. 00:12:07.47\00:12:13.48 And, of course, we don't know exactly until probably 00:12:13.51\00:12:17.36 years later what the impact of this is going to be. 00:12:17.39\00:12:21.15 But a study done not long after the attack, 00:12:21.18\00:12:24.04 showed that 71% of Americans felt depressed after that 00:12:24.05\00:12:30.23 September 11th attack. 00:12:30.26\00:12:32.62 That's, of course, a very high figure... 00:12:32.63\00:12:34.74 50% were having trouble concentrating, 00:12:34.77\00:12:37.34 and fully one-third were having insomnia, 00:12:37.37\00:12:40.22 either early morning awakenings where they couldn't 00:12:40.25\00:12:42.42 get back to sleep, or they were actually not being able 00:12:42.45\00:12:46.30 to sleep at all. 00:12:46.33\00:12:47.68 And these are symptoms, not being able to concentrate 00:12:47.71\00:12:50.67 and insomnia are some of the standard symptoms 00:12:50.70\00:12:53.60 for depression. 00:12:53.63\00:12:55.40 Well, before we get into the actual cure, 00:12:55.43\00:12:58.48 and how to find the cause of the disease, 00:12:58.51\00:13:00.70 I think it's important to come up with definitions. 00:13:00.73\00:13:03.51 Depression does have a well-defined way of determining 00:13:03.54\00:13:11.16 whether it's present or not... 00:13:11.19\00:13:13.26 And you can get this in psychiatric literature 00:13:13.29\00:13:15.97 through what we call the DSM-IV criteria 00:13:16.00\00:13:18.39 but you do NOT have to be a psychiatrist, 00:13:18.42\00:13:20.88 and you do NOT have to be a psychologist 00:13:20.91\00:13:23.07 to diagnose depression. 00:13:23.10\00:13:25.48 In fact, I dare say that everyone who is watching 00:13:25.51\00:13:29.95 and listening right now, if they pay attention 00:13:29.98\00:13:33.22 to the WAY to diagnose depression, 00:13:33.25\00:13:36.41 whether you're a lay person, or whether you're an engineer 00:13:36.44\00:13:39.12 whether you've never set foot in a doctor's office, 00:13:39.15\00:13:41.80 and never had any healthcare training, 00:13:41.83\00:13:43.54 you WILL be able to accurately diagnose whether you 00:13:43.57\00:13:47.49 are depressed, or whether someone that you 00:13:47.52\00:13:49.99 LOVE is depressed. 00:13:50.02\00:13:51.85 And this is an important step to take because 00:13:51.88\00:13:54.69 many people are suffering from depression but don't even 00:13:54.72\00:13:57.97 recognize that they're suffering from it. 00:13:58.00\00:14:00.60 And if you don't even RECOGNIZE the problem, 00:14:00.63\00:14:02.50 then you're that much further AWAY from getting a cure. 00:14:02.53\00:14:06.98 Just like an alcoholic cannot undergo appropriate treatment 00:14:07.01\00:14:11.85 for alcoholism and recover from alcoholism until he 00:14:11.88\00:14:16.04 recognizes that he's an alcoholic... 00:14:16.07\00:14:18.20 So it is true for a depressed patient. 00:14:18.23\00:14:20.47 Unless they actually recognize that they are suffering from 00:14:20.50\00:14:23.73 major depression, then they're not going to undergo 00:14:23.76\00:14:27.15 the analysis to find out why and then to be plugged in 00:14:27.18\00:14:31.53 to the absolute cure for depression. 00:14:31.56\00:14:34.80 So let's go over the definition of depression... 00:14:34.83\00:14:39.11 First of all, in order to have major depression, 00:14:39.14\00:14:41.80 you CANNOT have recently faced obvious emotional trauma 00:14:41.83\00:14:48.54 And, of course, being in New York recently, 00:14:48.57\00:14:52.64 helping out with the grief, and depression and recovery 00:14:52.67\00:14:56.49 efforts there, the people that have just suffered 00:14:56.52\00:15:00.33 through that are NOT necessarily suffering from major depression. 00:15:00.36\00:15:04.44 They're suffering from grief... 00:15:04.47\00:15:05.69 that is what we would call situational depression. 00:15:05.72\00:15:09.18 It's normal to be depressed when you lose a 00:15:09.21\00:15:11.68 host of loved ones, when you've lost your residence, 00:15:11.71\00:15:14.17 when you've lost your job... 00:15:14.20\00:15:15.46 those type of things bring about a situational depression. 00:15:15.50\00:15:20.25 This is not what we're emphasizing here today... 00:15:20.28\00:15:23.95 What we are emphasizing is actually major depression 00:15:23.98\00:15:27.41 where there is NO recent emotional trauma... 00:15:27.44\00:15:30.85 And that means no trauma for the last 2 weeks prior to this. 00:15:30.88\00:15:35.17 Now, if you're still having symptoms of depression 00:15:35.20\00:15:37.96 4 to 6 months after an event like what happened at the 00:15:37.99\00:15:41.19 World Trade Center, then that is abnormal... 00:15:41.22\00:15:43.50 And then you do have to actually undergo some process 00:15:43.53\00:15:48.37 to find out why you're having a complication with the 00:15:48.40\00:15:52.42 grief aspect of the disease. 00:15:52.45\00:15:54.89 But if you haven't faced obvious recent emotional trauma, 00:15:54.92\00:15:59.16 and you have 5 of the following 9 symptoms, 00:15:59.19\00:16:03.95 you have major depression. 00:16:03.96\00:16:06.15 Let's go over each one of these 9 symptoms... 00:16:06.18\00:16:09.56 First is deep sadness. 00:16:09.59\00:16:11.40 Individuals with depression... when they say they're depressed 00:16:11.41\00:16:14.21 this is what they normally feel 00:16:14.24\00:16:15.56 is a deep, overwhelming sense of sadness. 00:16:15.59\00:16:18.42 But not everyone with depression will have sadness. 00:16:18.43\00:16:21.27 In fact, many people will just have a feeling of emptiness... 00:16:21.30\00:16:24.43 And they'll think this emptiness if because they're 00:16:24.46\00:16:26.52 missing something in their life, maybe missing some 00:16:26.55\00:16:31.38 romantic relationship that they would like 00:16:31.41\00:16:34.02 And it may not be due that they're missing anything at all; 00:16:34.05\00:16:37.78 it may be because they're actually suffering 00:16:37.81\00:16:39.74 from depression. 00:16:39.75\00:16:41.21 Now, but they may be missing something underlying 00:16:41.24\00:16:43.90 that's causing that. 00:16:43.93\00:16:45.12 But it's not necessarily just the lack of a romantic 00:16:45.13\00:16:48.17 relationship bringing that about. 00:16:48.21\00:16:49.81 So if you have deep sadness, or a sense of emptiness, 00:16:49.84\00:16:52.78 that would qualify as 1 of those 9 symptoms. 00:16:52.81\00:16:58.51 The 2nd symptom is apathy. 00:16:58.54\00:17:00.88 Apathy is where individuals just don't seem to 00:17:00.91\00:17:04.21 care anymore in regards to what goes on as 00:17:04.24\00:17:08.56 part of their daily routine. 00:17:08.59\00:17:10.55 They have LOST the desire and the joy and the zest for life. 00:17:10.58\00:17:16.47 The third 3rd symptom is agitation. 00:17:16.48\00:17:20.75 Some individuals that have depression will not experience 00:17:20.78\00:17:24.93 the sadness or the emptiness, but they will just be agitated, 00:17:24.94\00:17:27.82 and they will frequently lose their concentration 00:17:27.85\00:17:31.49 as a result of this. 00:17:31.52\00:17:33.32 The 4th symptom, and one that we tend to see quite a bit 00:17:33.35\00:17:36.39 is sleep disturbances. 00:17:36.42\00:17:38.59 Now with the sleep disturbances, it can go either way... 00:17:38.62\00:17:41.13 Most people with severe depression will actually 00:17:41.16\00:17:44.25 suffer insomnia... like we mentioned the people in America 00:17:44.28\00:17:47.97 after the World Trade Center attack were suffering. 00:17:48.00\00:17:50.77 But some people with depression will actually be too sleepy... 00:17:50.80\00:17:54.36 And they will be sleeping more than 9 hours a day, 00:17:54.39\00:17:57.29 and be falling asleep during their job, 00:17:57.32\00:18:00.65 and just having this overwhelming sense of 00:18:00.68\00:18:03.58 needing to sleep, and be sleeping too much. 00:18:03.61\00:18:05.83 That can ALSO be a sign of depression... 00:18:05.86\00:18:09.87 So, that one can go either way, and if it does 00:18:09.90\00:18:12.65 it qualifies for one of those 9 symptoms. 00:18:12.66\00:18:15.66 The 5th symptom is lack of concentration... 00:18:15.69\00:18:19.51 And this is one that I tend to see quite a bit 00:18:19.54\00:18:22.35 in my medical practice. 00:18:22.39\00:18:23.72 In fact, when an individual goes from a healthy state of mind, 00:18:23.75\00:18:26.66 to a depressed state of mind, they will frequently 00:18:26.67\00:18:29.77 NOT be able to even make simple decisions without 00:18:29.80\00:18:33.77 involving others. 00:18:33.80\00:18:35.55 And just even the decision as far as what to wear 00:18:35.58\00:18:38.61 for an event, can produce a lot of anxiety... 00:18:38.64\00:18:43.21 And the individual will then try to get other people 00:18:43.24\00:18:45.78 in their family involved to help them out to decide 00:18:45.79\00:18:48.03 what to wear for an event, where prior to being depressed, 00:18:48.06\00:18:51.97 they were fully capable, and recognize they were fully 00:18:52.00\00:18:54.92 capable of making that decision. 00:18:54.95\00:18:57.15 There are other examples of more complex decisions as well 00:18:57.18\00:19:01.17 in regards to what to do about a work situation 00:19:01.20\00:19:04.52 that isn't the best. 00:19:04.55\00:19:05.64 A depressed patient will feel helpless in regards to 00:19:05.67\00:19:09.28 being able to make a wise decision to resolve a problem 00:19:09.31\00:19:15.29 in the work environment. 00:19:15.32\00:19:16.56 Or if they do make that decision 00:19:16.59\00:19:18.68 often it will be a hasty decision, and an 00:19:18.71\00:19:22.09 erroneous decision to make. 00:19:22.12\00:19:24.77 The 7th symptom is a feeling of worthlessness. 00:19:24.80\00:19:29.88 This is characteristic among many depressed patients. 00:19:29.91\00:19:33.46 They don't feel useful to themselves... 00:19:33.49\00:19:35.30 They don't feel useful to their family... 00:19:35.33\00:19:37.40 They don't feel useful to their friends... 00:19:37.43\00:19:39.96 And this really involves the loss of meaning and purpose 00:19:39.97\00:19:44.31 in life that can occur when an individual is depressed. 00:19:44.34\00:19:48.53 The 8th symptom is morbid thoughts. 00:19:48.56\00:19:52.54 And, individuals with depression some of them, 00:19:52.55\00:19:55.78 ...fortunately not the majority of them have morbid thoughts; 00:19:55.81\00:19:58.98 this is more of a minority symptom. 00:19:59.01\00:20:01.03 But those that have these thoughts, will not only 00:20:01.06\00:20:05.05 sometimes have thoughts of endangering OTHERS, 00:20:05.08\00:20:10.52 but they can also have thoughts of endangering themselves, 00:20:10.55\00:20:13.55 and that's really when people even can get into that 00:20:13.58\00:20:17.76 stage where suicide becomes contemplated... 00:20:17.79\00:20:21.66 And that is one of the top 10 leading causes of death 00:20:21.69\00:20:25.85 in our country... is suicide. 00:20:25.88\00:20:28.64 And this is WHY this disease deserves some serious attention. 00:20:28.67\00:20:33.83 The 9th symptom and the one that I see MOST frequently 00:20:33.86\00:20:36.57 in my practice of internal medicine is fatigue. 00:20:36.60\00:20:40.21 Individuals with this will come to my office and say... 00:20:40.24\00:20:43.83 "Doctor, I just don't have the energy I used to have" 00:20:43.86\00:20:47.54 "I am tired, and can you tell me why I'm tired?" 00:20:47.57\00:20:52.19 Now, as part of this process, I have to take what's called a 00:20:52.22\00:20:55.27 "history"... where I'm actually asking a lot of questions 00:20:55.30\00:20:58.69 to the patient, and part of what I will ask them 00:20:58.72\00:21:01.91 are the rest of these 8 symptoms to see if they 00:21:01.94\00:21:04.24 actually qualify for major depression. 00:21:04.25\00:21:07.56 If they have 5 of those 9 symptoms, 00:21:07.57\00:21:09.98 they would have major depression, 00:21:10.01\00:21:11.61 or if they just have 2 of those 9 symptoms, 00:21:11.64\00:21:14.52 they would have what's called "subsyndromal depression" 00:21:14.55\00:21:17.75 Between 2 and 4 symptoms means that you're on 00:21:17.78\00:21:20.48 your way to major depression unless steps are taken 00:21:20.51\00:21:23.21 to avoid the major depression... 00:21:23.24\00:21:26.86 And so it's important to recognize even if you have 00:21:26.89\00:21:29.26 2 of those 9 symptoms. 00:21:29.29\00:21:30.97 Now it's also important to recognize that these symptoms 00:21:31.00\00:21:33.36 have to be present the majority of the time for at least 2 weeks 00:21:33.39\00:21:36.97 before you actually are diagnosed with depression. 00:21:37.00\00:21:41.63 Some of us can have some of these feelings transiently 00:21:41.66\00:21:44.23 throughout the course of a week's time, maybe on occasion 00:21:44.26\00:21:47.51 but that doesn't qualify for major depression. 00:21:47.52\00:21:50.11 If you have it the majority of time for the last 2 weeks, 00:21:50.14\00:21:52.93 that WOULD indeed qualify for major depression. 00:21:52.96\00:21:56.94 Making this first step in this first diagnosis 00:21:56.97\00:22:00.72 of major depression is of utmost importance. 00:22:00.75\00:22:03.45 And I know may people... we could go through 00:22:03.48\00:22:06.21 my own patient list... we could go through even 00:22:06.22\00:22:08.95 celebrities who have ignored this disease, 00:22:08.98\00:22:12.02 even CEOs of MAJOR corporations have tried to 00:22:12.05\00:22:16.11 ignore this disease and push it off and tell themselves 00:22:16.14\00:22:19.41 that they're NOT suffering from it... 00:22:19.44\00:22:20.99 and that can be a set up for disaster because they're not 00:22:21.02\00:22:24.35 going to address the problem, 00:22:24.38\00:22:27.05 and really find the help that they need. 00:22:27.08\00:22:32.03 Depression would be bad enough if it just did 00:22:32.06\00:22:35.35 what we talked about. 00:22:35.38\00:22:37.34 Those 9 symptoms are NOT joyous symptoms... 00:22:37.37\00:22:41.08 they're not happy symptoms. 00:22:41.11\00:22:43.15 But unfortunately, depression does much worse than this... 00:22:43.18\00:22:46.93 It can actually increase the risk of death significantly. 00:22:46.96\00:22:52.15 It increases the risk of stroke by 50% in those that 00:22:52.18\00:22:57.19 have major depression. 00:22:57.22\00:22:59.17 It also increases the risk of heart disease in men. 00:22:59.20\00:23:03.38 If someone has survived a heart attack, 00:23:03.41\00:23:05.64 it's actually very important for them NOT to get depressed 00:23:05.67\00:23:09.74 as a result of that heart attack. 00:23:09.77\00:23:11.54 Those that suffer from depression have a 2.5 fold 00:23:11.55\00:23:14.61 increased likelihood of dying from sudden death 00:23:14.64\00:23:17.62 AFTER their heart attack... than ones who are NOT depressed 00:23:17.65\00:23:21.76 And that's even more important than whether they are 00:23:21.79\00:23:24.30 put on medication after their heart attack or not... 00:23:24.33\00:23:27.29 their mental state. 00:23:27.32\00:23:29.59 In addition to that, it increases the 00:23:29.62\00:23:31.70 risk of death from cancer... 00:23:31.74\00:23:33.24 Whether an individual has the same tissue type of cancer, 00:23:33.27\00:23:38.29 in the same place that spread to body organs 00:23:38.30\00:23:40.31 their mental health will often determine exactly 00:23:40.34\00:23:43.52 how long they survive even from terminal cancer. 00:23:43.55\00:23:46.70 It also increases the risk of death from pneumonia, 00:23:46.73\00:23:49.73 and, as we mentioned earlier, it increases the risk of 00:23:49.76\00:23:52.88 death from suicide. 00:23:52.91\00:23:54.20 All of those causes of death that I've just mentioned to you 00:23:54.21\00:23:56.93 are in the top 10 causes of death in this country... 00:23:56.96\00:24:00.17 And it's really hard to estimate exactly how much our mental 00:24:00.20\00:24:04.40 health is playing a toll on the entire 00:24:04.43\00:24:08.18 top 10 causes of death. 00:24:08.21\00:24:12.26 Now, depression would be bad enough if it did 00:24:12.29\00:24:15.46 just those things, but unfortunately, 00:24:15.49\00:24:17.56 the effects of depression tells us that it's more 00:24:17.59\00:24:20.87 than a mental illness... 00:24:20.90\00:24:22.32 and some of those effects are just bothersome 00:24:22.33\00:24:24.36 effects but can still end up having a person 00:24:24.39\00:24:29.02 see a doctor like myself. 00:24:29.06\00:24:31.16 First of all, can cause a chronic headache. 00:24:31.20\00:24:33.76 I had a head elder of a church, not my own church 00:24:33.77\00:24:38.93 that I attend, but another church, come to me 00:24:38.96\00:24:42.44 who had been to a neurologist... 00:24:42.47\00:24:44.75 Actually, first he had gone to his family practitioner 00:24:44.78\00:24:47.34 because of severe headaches, 00:24:47.37\00:24:49.68 and the family practitioner put him on some medication 00:24:49.69\00:24:53.59 for a tension headache, and then when his headaches 00:24:53.62\00:24:57.34 didn't go away, performed an MRI, a sophisticated scan 00:24:57.37\00:25:00.77 of the brain to see if there was a brain tumor, 00:25:00.80\00:25:02.94 or something like that causing it... 00:25:02.95\00:25:04.71 And then referred the patient to a neurologist. 00:25:04.74\00:25:08.58 The neurologist reviewed the MRI, and said that it was 00:25:08.61\00:25:11.18 completely normal, and ended up stepping up the medication 00:25:11.21\00:25:16.64 from just being a tension headache medication, 00:25:16.67\00:25:18.74 to an actual NARCOTIC medication to take care of the pain. 00:25:18.75\00:25:22.14 Well, unfortunately, this individual was having 00:25:22.17\00:25:24.23 to require more and more narcotics in order 00:25:24.26\00:25:26.97 to control the pain... 00:25:26.98\00:25:28.89 And then finally, he heard about us... and our ability to get 00:25:28.90\00:25:32.41 to the root causes of disease, and so scheduled 00:25:32.44\00:25:35.18 an appointment with me. 00:25:35.21\00:25:36.80 Fortunately, he was there with his wife, 00:25:36.83\00:25:39.32 and came in to the room, and I started asking him 00:25:39.35\00:25:41.73 about his headaches. 00:25:41.76\00:25:43.03 And, as a result of him having headaches, 00:25:43.04\00:25:45.56 I started asking him other questions... 00:25:45.60\00:25:47.76 What about fatigue? Do you have fatigue? 00:25:47.80\00:25:49.93 "Oh yes, I have fatigue" He admitted to that. 00:25:49.96\00:25:52.09 I said, "Well, what about feelings of worthlessness?" 00:25:52.12\00:25:54.95 "Oh no, I don't have a problem with that. " 00:25:54.98\00:25:56.68 And his wife would be there in the corner... 00:25:56.71\00:25:59.24 and she would nod her head 00:25:59.27\00:26:01.04 "Yes, he does have a problem with that" 00:26:01.07\00:26:03.17 And then I'd say, "What about lack of concentration, 00:26:03.20\00:26:06.02 any problems there?" 00:26:06.05\00:26:07.23 And he'd say, "Oh no, no problems there. " 00:26:07.26\00:26:09.24 And she would nod her head, 00:26:09.27\00:26:10.52 "Yes, he has significant problems with lack of 00:26:10.55\00:26:12.55 concentration" 00:26:12.58\00:26:13.79 And so I ended up saying, 00:26:13.82\00:26:15.48 "Well sir, your wife is not agreeing with you here... 00:26:15.51\00:26:18.88 well, maybe you ought to talk and see which one is right 00:26:18.91\00:26:21.45 and, as they talked it out there in front of me, 00:26:21.48\00:26:23.62 he had to admit that his wife was right about these things. 00:26:23.65\00:26:26.73 It turns out, he was suffering from 6 of the 9 major 00:26:26.76\00:26:31.02 diagnostic criteria for depression... 00:26:31.03\00:26:33.51 And the reason for his headache... 00:26:33.54\00:26:34.77 was major depression. 00:26:34.80\00:26:36.96 Once we made THAT diagnosis, we could then find out 00:26:36.99\00:26:39.79 the cause of his major depression 00:26:39.82\00:26:41.53 and actually get to the CURE of his headaches... 00:26:41.56\00:26:44.63 And so that story helps to illustrate how 00:26:44.66\00:26:48.05 the mental state can actually affect the entire body, 00:26:48.08\00:26:52.29 and cause problems remotely removed sometimes 00:26:52.32\00:26:55.81 from the mind. 00:26:55.82\00:26:58.07 Now, it can also cause osteoporosis... 00:26:58.10\00:27:00.47 when combined with stress... 00:27:00.50\00:27:02.43 It can actually cause HIV to progress to AIDS 00:27:02.46\00:27:06.63 In addition to this, it can increase the risk of 00:27:06.66\00:27:10.31 return to addictive behavior, such as drinking... 00:27:10.34\00:27:13.63 And on top of this, it can end up increasing the risk of 00:27:13.66\00:27:18.52 asthma, medical problems. 00:27:18.55\00:27:20.93 It can increase the stress hormones, 00:27:20.96\00:27:22.86 and DECREASE the memory significantly. 00:27:22.89\00:27:26.59 These effects of depression are, indeed, paramount 00:27:26.62\00:27:30.01 for us to understand. 00:27:30.02\00:27:31.71 And I would like to leave you, at this point in time, 00:27:31.74\00:27:35.92 with the overwhelming sense of the need to GET to the cause 00:27:35.93\00:27:41.99 of the disease. 00:27:42.02\00:27:43.22 DEPRESSION is just a constellation of symptoms... 00:27:43.25\00:27:46.93 In order to get to a cure, you HAVE to get 00:27:46.94\00:27:49.74 to the CAUSE of the disease, 00:27:49.77\00:27:51.88 and this is what we will be discussing in.. 00:27:51.91\00:27:54.10 Part 2 of "DEPRESSION, THE WAY OUT" 00:27:54.14\00:27:56.13
