And why you might want that. 00:00:14.91\00:00:16.75 What are some of the risks involved? 00:00:16.78\00:00:18.95 Yeah, there's always risk and there's risk 00:00:18.98\00:00:21.25 of having the surgery itself, 00:00:21.28\00:00:23.25 which mainly includes bleeding, 00:00:23.28\00:00:25.49 there's a small risk of dying from any procedure. 00:00:25.52\00:00:28.19 There's a small risk of stroke and that, 00:00:28.22\00:00:30.93 but we also talk about the benefits of not doing it. 00:00:30.96\00:00:34.16 Usually the valve gets old fairly quickly, 00:00:34.20\00:00:36.83 and then you not only lose quality of life 00:00:36.87\00:00:39.13 but you die from this. 00:00:39.17\00:00:42.04 Now some people say, 00:00:42.07\00:00:43.41 "No, I don't want anything done." 00:00:43.44\00:00:45.04 It's ready, it's my time 00:00:45.07\00:00:46.61 and they choose not to have it done 00:00:46.64\00:00:48.31 and that's fine. 00:00:48.34\00:00:49.68 I know that, you know, 00:00:49.71\00:00:51.15 it's hard to make these decisions. 00:00:51.18\00:00:52.55 I know that you have a grandmother 00:00:52.58\00:00:54.38 that watches these programs on. 00:00:54.42\00:00:55.78 I do have grandma if you're watching, "Hello." 00:00:55.82\00:00:57.52 Okay. If you're watching. 00:00:57.55\00:00:58.89 Okay, but when people get older things do wear out. 00:00:58.92\00:01:01.72 And that's why it's important to have a detailed discussion 00:01:01.76\00:01:04.49 about what's available, the risk and benefits 00:01:04.53\00:01:07.06 and some people say," No, this is not for me. 00:01:07.10\00:01:09.63 I don't wanna take the risk of that. 00:01:09.66\00:01:11.43 I don't wanna have that. I'm ready. 00:01:11.47\00:01:13.50 I'm ready to meet Jesus." And that's okay. 00:01:13.54\00:01:16.20 But technology has improved, it's become a lot safer. 00:01:16.24\00:01:20.14 And some people opt to have these procedures, 00:01:20.18\00:01:22.41 whether they be a new valve. 00:01:22.44\00:01:24.25 Now valve disease is a very complex, 00:01:24.28\00:01:27.58 we can't talk about everything. 00:01:27.62\00:01:28.95 Right. 00:01:28.98\00:01:30.32 But the things I want people to know about 00:01:30.35\00:01:31.69 is if you're short of breath 00:01:31.72\00:01:33.05 or having a symptom, see your doctor. 00:01:33.09\00:01:35.66 And one of the things that causes symptoms 00:01:35.69\00:01:37.36 might be the valve is getting older, 00:01:37.39\00:01:39.69 and we have lots of options 00:01:39.73\00:01:41.36 for valves that are wearing out. 00:01:41.40\00:01:43.90 Now sometimes, you know, we talked about my patient 00:01:43.93\00:01:46.90 has had the symptom for a long time. 00:01:46.94\00:01:49.47 We call that chronic disease. 00:01:49.50\00:01:51.97 Now some people have emergencies. 00:01:52.01\00:01:54.41 The valve just, you know, has a problem right away. 00:01:54.44\00:01:57.35 We call that emergent valve problem. 00:01:57.38\00:02:00.22 And if the valve happens, you know, 00:02:00.25\00:02:02.08 something happens wrong emergently 00:02:02.12\00:02:04.19 something has to be done right away. 00:02:04.22\00:02:05.92 So what would cause that? 00:02:05.95\00:02:07.36 A trauma to the valve, you know, 00:02:07.39\00:02:09.39 an infection to the valve, 00:02:09.42\00:02:11.26 a trauma where something was ripped in the heart 00:02:11.29\00:02:14.13 having a heart attack 00:02:14.16\00:02:15.50 where the blood supply to the valve wasn't good. 00:02:15.53\00:02:18.03 And the valve was affected by that 00:02:18.07\00:02:19.73 caused a lot of leaking something tear. 00:02:19.77\00:02:22.44 So the valve where its scaffolding 00:02:22.47\00:02:24.77 was becomes disrupted, 00:02:24.81\00:02:26.61 those are medical emergency route, 00:02:26.64\00:02:28.44 so that we have to do something right away 00:02:28.48\00:02:30.11 from that that's called an emergency. 00:02:30.15\00:02:32.31 And then sometimes we have a slower symptom 00:02:32.35\00:02:34.52 that develops over time. 00:02:34.55\00:02:36.05 That's a chronic situation. 00:02:36.08\00:02:38.05 So my patient was more of a chronic 00:02:38.09\00:02:40.36 that gradually got worse, but lots of different options 00:02:40.39\00:02:43.66 on the valves on both the mitral and the aortic. 00:02:43.69\00:02:46.70 Sometimes the third valve that we sometimes work on 00:02:46.73\00:02:49.70 is called the tricuspid valve. 00:02:49.73\00:02:53.13 That's the one between the low pressure chamber 00:02:53.17\00:02:55.50 the right ventricle, sometimes we have to do stuff 00:02:55.54\00:02:58.21 with that valve as well. 00:02:58.24\00:02:59.74 So lots of different options on different valves. 00:02:59.77\00:03:02.61 All right, now let's see if I've got this straight. 00:03:02.64\00:03:04.38 We were just talking about the trans aortic valve... 00:03:04.41\00:03:06.11 Yeah. 00:03:06.15\00:03:07.48 Which is controlling the blood flow 00:03:07.52\00:03:09.32 between the left atrium and ventricle? 00:03:09.35\00:03:11.75 In the left ventricle in the body. 00:03:11.79\00:03:13.89 In the body. Okay. 00:03:13.92\00:03:15.26 So it pumps out that valve to the entire body. 00:03:15.29\00:03:17.03 That's the one that let, you know, 00:03:17.06\00:03:18.56 the heart pumps goes out the aorta to entire body. 00:03:18.59\00:03:21.26 That's the aortic valve. 00:03:21.30\00:03:22.63 Okay, now let's talk about the mitral valve, 00:03:22.66\00:03:24.00 the other one in there. 00:03:24.03\00:03:25.37 Yeah. Okay. 00:03:25.40\00:03:26.74 Where's that controlling blood flow to? 00:03:26.77\00:03:28.10 The mitral valve control, you know, the lungs, 00:03:28.14\00:03:30.34 fill up the blood with oxygen, 00:03:30.37\00:03:32.87 and it circulates through the lungs 00:03:32.91\00:03:34.48 and it comes back to the left atrium, 00:03:34.51\00:03:36.95 and then from the left atrium it moves to the left ventricle, 00:03:36.98\00:03:39.91 and the left ventricle then pumps it out to the body. 00:03:39.95\00:03:42.15 Okay. So this is that valve. 00:03:42.18\00:03:44.29 So when there's a problem with the mitral valve, 00:03:44.32\00:03:46.22 what kind of options are there? 00:03:46.25\00:03:47.59 Well, you know, first of all, how would we find out that 00:03:47.62\00:03:50.03 we had a problem, again, shortness of breath, 00:03:50.06\00:03:52.93 sometimes the heart, the flow can back up 00:03:52.96\00:03:56.30 and the heart can start fibrillating, 00:03:56.33\00:03:57.70 that might be a clue. 00:03:57.73\00:03:59.23 Sometimes people can get fatigued and tired, 00:03:59.27\00:04:01.64 but the one I see most is shortness of breath 00:04:01.67\00:04:04.04 when I do things. 00:04:04.07\00:04:05.41 And that's a clue that 00:04:05.44\00:04:06.78 the mitral valve might be involved. 00:04:06.81\00:04:08.88 So what would I do? 00:04:08.91\00:04:10.41 I would get my stethoscope out and I would listen 00:04:10.45\00:04:12.95 and there's things that they teach us in medical school 00:04:12.98\00:04:15.55 and residency where we can tell characteristics 00:04:15.58\00:04:17.69 of which valve is malfunctioning. 00:04:17.72\00:04:19.05 Okay. 00:04:19.09\00:04:20.42 So I would listen to that valve and say, 00:04:20.46\00:04:21.79 "Wow, that valve sounds like 00:04:21.82\00:04:23.16 it's got a holosystolic murmur." 00:04:23.19\00:04:25.46 That's the way they describe it. 00:04:25.49\00:04:27.00 And I said, "Well, I need more information." 00:04:27.03\00:04:29.26 So I would do a test, 00:04:29.30\00:04:30.87 an echocardiogram where we can look at the valve. 00:04:30.90\00:04:33.47 We can look at it from just putting 00:04:33.50\00:04:34.84 some ultrasound on it. 00:04:34.87\00:04:36.87 Sometimes we can swallow a probe and look behind that. 00:04:36.91\00:04:39.97 That's called a transesophageal echocardiogram. 00:04:40.01\00:04:43.31 Sometimes we can get clues about the valves 00:04:43.35\00:04:45.71 through angiography and looking at that. 00:04:45.75\00:04:48.02 We now have MRIs and, you know, 00:04:48.05\00:04:51.09 different types of scans that can look at 00:04:51.12\00:04:53.05 how the valve is opening and closing. 00:04:53.09\00:04:55.66 And once we found out that it was leaking, 00:04:55.69\00:04:57.89 then we try to figure out 00:04:57.93\00:04:59.26 why it's leaking, you know, is it an infection? 00:04:59.29\00:05:02.66 Is it from a lack of blood supply? 00:05:02.70\00:05:04.80 Is it because it's just wearing out? 00:05:04.83\00:05:06.87 Is it something congenital? 00:05:06.90\00:05:08.74 So once we figured out the cause, 00:05:08.77\00:05:10.47 then we could figure out 00:05:10.51\00:05:11.84 what would be the options for treatment. 00:05:11.87\00:05:13.41 Sometimes the valve leaks a little bit, 00:05:13.44\00:05:15.28 and we just keep an eye on it. 00:05:15.31\00:05:16.64 Sure, yeah. 00:05:16.68\00:05:18.01 Okay, but sometimes it leaks a lot. 00:05:18.05\00:05:19.88 And if we don't do something about it, 00:05:19.91\00:05:21.58 it's gonna cause further problems 00:05:21.62\00:05:23.59 down the road. 00:05:23.62\00:05:24.95 So sometimes we choose to treat it right away. 00:05:24.99\00:05:27.02 One of the new develops in cardiology now, you know, 00:05:27.06\00:05:30.66 for years we could, sometimes a surgeon 00:05:30.69\00:05:32.59 can open you up and repair the valve. 00:05:32.63\00:05:35.30 Sometimes you can replace either a metal valve in 00:05:35.33\00:05:38.53 or put a bioprosthetic valve, 00:05:38.57\00:05:40.47 that's been sort of the gold standard, 00:05:40.50\00:05:42.30 but over the last two years, for leaky heart valves 00:05:42.34\00:05:45.47 and for certain valve conditions, 00:05:45.51\00:05:47.21 we have a procedure that we don't cut anyone open. 00:05:47.24\00:05:50.08 It's called a mitral valve clip. 00:05:50.11\00:05:53.28 And this is brand new technology. 00:05:53.31\00:05:55.22 All right, well, let's take a look at that. 00:05:55.25\00:05:56.72 And you can describe what's going on here for us? 00:05:56.75\00:05:58.09 Yeah. 00:05:58.12\00:06:00.06 Well, here is a heart, and that's the mitral valve, 00:06:00.09\00:06:02.59 and see that blue, 00:06:02.62\00:06:03.96 that's the valve leaking a whole lot. 00:06:03.99\00:06:05.89 So the blood instead of going forward 00:06:05.93\00:06:07.96 and out the aorta, which is to your right, 00:06:08.00\00:06:10.20 it's going backwards and it's leaking. 00:06:10.23\00:06:12.77 And that is what causes someone to be short of breath. 00:06:12.80\00:06:14.90 Well, we have this procedure, 00:06:14.94\00:06:17.21 where we actually clip the two valves together. 00:06:17.24\00:06:20.41 And there's an example of clicking it. 00:06:20.44\00:06:22.14 So now we have two orifices, and it will cause a lot less 00:06:22.18\00:06:26.18 of this regurgitation leaking. 00:06:26.21\00:06:28.25 So see how much before and now after 00:06:28.28\00:06:31.32 we notice a lot less leakage backwards, 00:06:31.35\00:06:33.52 and that's a sort of a non-invasive way 00:06:33.56\00:06:36.73 we can use to help the blood 00:06:36.76\00:06:38.29 from the mitral valve from leaking backward. 00:06:38.33\00:06:40.23 Do all patients candidate for this? 00:06:40.26\00:06:42.30 No, not all, only specific patients 00:06:42.33\00:06:45.07 would benefit from this, 00:06:45.10\00:06:46.43 but that's what all the testing can do 00:06:46.47\00:06:48.50 to look at these valves. 00:06:48.54\00:06:50.14 So we can do a lot of different things 00:06:50.17\00:06:53.01 to help the valves. 00:06:53.04\00:06:54.38 Yeah, so I imagine the mitral clip, 00:06:54.41\00:06:55.94 maybe has less risk than actually opening up. 00:06:55.98\00:06:58.98 Yeah, yeah. 00:06:59.01\00:07:00.35 Less risk of infection, quicker recoveries. 00:07:00.38\00:07:02.02 What are some of the other benefit? 00:07:02.05\00:07:03.39 Well, the benefits, main benefits 00:07:03.42\00:07:04.79 is you don't have to have open-heart surgery. 00:07:04.82\00:07:06.89 Yeah. But it's not for everybody. 00:07:06.92\00:07:08.99 And again, there are risks from this. 00:07:09.02\00:07:10.36 And really, you know, 00:07:10.39\00:07:11.73 there's always a risk of bleeding, 00:07:11.76\00:07:13.09 there's still a risk of infection, 00:07:13.13\00:07:14.60 there's still a small risk of stroke and damage, 00:07:14.63\00:07:17.03 but this is for a new technology 00:07:17.07\00:07:19.70 for a special class of patients that have a leaky heart valve. 00:07:19.73\00:07:23.61 And that valve is called mitral regurgitation. 00:07:23.64\00:07:25.67 But the big point today I want for our listeners 00:07:25.71\00:07:27.84 is to know that if you're short of breath, 00:07:27.88\00:07:30.05 see your doctor. 00:07:30.08\00:07:31.41 There are lots of things that we can do, 00:07:31.45\00:07:32.98 lots of different things we can offer. 00:07:33.01\00:07:35.02 Modern medicine has its place in helping these valves out. 00:07:35.05\00:07:38.39 So are there medications that are associated 00:07:38.42\00:07:40.89 with these procedures that patients might have 00:07:40.92\00:07:43.19 to take for a shorter time or a prolonged time after? 00:07:43.22\00:07:45.13 No, it's according to their underlying 00:07:45.16\00:07:47.50 whether the heart's strong, 00:07:47.53\00:07:48.86 whether it's weak, whether the fluids backing up, 00:07:48.90\00:07:51.27 you know, sometimes the fluids backing up they need diuretics. 00:07:51.30\00:07:54.04 Sometimes they need 00:07:54.07\00:07:55.40 different types of blood thinners. 00:07:55.44\00:07:56.91 Sometimes their blood pressure's high. 00:07:56.94\00:07:58.51 They need medicines to strengthen the heart. 00:07:58.54\00:08:00.68 So yes, there are some medications 00:08:00.71\00:08:02.48 that's used to help sort of slow down the aging process 00:08:02.51\00:08:06.28 to help the heart not work as hard 00:08:06.31\00:08:08.65 to help blood clots not developing, 00:08:08.68\00:08:10.55 blood doesn't move as well. 00:08:10.59\00:08:12.42 So yeah, there's medications 00:08:12.45\00:08:13.79 but each individual is so unique, Nick, 00:08:13.82\00:08:16.29 there's not a sort of a standard mix, 00:08:16.32\00:08:18.59 but we look at the patient, see what medicines 00:08:18.63\00:08:20.90 we can do to help the quality of life the best. 00:08:20.93\00:08:23.67 So we talked a little bit about modern medicine, 00:08:23.70\00:08:26.20 how it benefits, 00:08:26.23\00:08:27.57 valves that are that don't open well, 00:08:27.60\00:08:29.54 valves that leak, 00:08:29.57\00:08:30.91 but there's other heart conditions as well. 00:08:30.94\00:08:32.67 That's right. And yet he says, no... 00:09:17.92\00:09:21.42 This story talks about, you know, 00:09:21.46\00:09:22.96 when he ask them to bring him pulse and water. 00:09:22.99\00:09:27.16 Yeah. Basically, a healthy diet. 00:09:27.20\00:09:29.66 But it was more than a healthy diet. 00:09:29.70\00:09:31.57 It was about worship as well 00:09:31.60\00:09:33.40 'cause he was gonna stay true 00:09:33.44\00:09:34.77 to the things he was taught as a child. 00:09:34.80\00:09:36.57 Remember that text, "In all things acknowledge Him, 00:09:36.60\00:09:39.17 and He will direct the paths." 00:09:39.21\00:09:40.54 So they're acknowledging God 00:09:40.58\00:09:41.94 to remembering the way they were raised. 00:09:41.98\00:09:44.45 That's why I brought up kids, you know, we all have kids, 00:09:44.48\00:09:46.21 we all wanna teach them these ways. 00:09:46.25\00:09:47.92 So when they grow up, they're not gonna forget him. 00:09:47.95\00:09:50.22 So he even in a foreign country, 00:09:50.25\00:09:52.85 he stuck with what he was taught 00:09:52.89\00:09:54.22 by mom and dad. 00:09:54.26\00:09:55.62 He stuck with his beliefs, a healthy diet, 00:09:55.66\00:09:58.26 in worship and in a short period of time 00:09:58.29\00:10:00.80 he was how much? 00:10:00.83\00:10:02.16 Ten times better than 00:10:02.20\00:10:04.17 all the magicians and astrologers. 00:10:04.20\00:10:06.00 God blessed Daniel for his worship, 00:10:06.03\00:10:08.67 for his staying close 00:10:08.70\00:10:10.04 to biblical prescriptions in nutrition. 00:10:10.07\00:10:11.97 He was blessed with a better body. 00:10:12.01\00:10:13.98 God made him a special person. He was in charge of everything. 00:10:14.01\00:10:19.31 Yeah, I think, when we think about the story of Daniel, 00:10:19.35\00:10:21.35 it's easy to just think about the nutrition. 00:10:21.38\00:10:23.02 Well, plants are better for you, 00:10:23.05\00:10:24.42 obviously, but 10 times better, that's more than... 00:10:24.45\00:10:27.19 Oh, yeah. 00:10:27.22\00:10:28.56 Just nutrition that's definitely worshiping God 00:10:28.59\00:10:30.43 working in his life to do something supernatural. 00:10:30.46\00:10:32.99 Yeah. Yeah. And he did. 00:10:33.03\00:10:34.36 And this is a very powerful study 00:10:34.40\00:10:35.90 that we talked about how worship and staying true 00:10:35.93\00:10:38.43 to God's Word in all aspects of life, 00:10:38.47\00:10:41.37 you know, and how it's a blessing 00:10:41.40\00:10:43.30 and, you know, some sometimes God blesses us 00:10:43.34\00:10:45.27 more than others. 00:10:45.31\00:10:46.64 But Daniel was the second in command of two kingdoms, 00:10:46.68\00:10:51.31 you know, of the world at that time. 00:10:51.35\00:10:53.72 And he was a smart guy, and he also worshiped 00:10:53.75\00:10:57.02 remember later on in the study, no matter what, 00:10:57.05\00:10:59.72 he was gonna worship and sometimes at great peril, 00:10:59.75\00:11:02.69 he stuck with that plan of worship, you know, 00:11:02.72\00:11:05.89 and that made a big difference in his life. 00:11:05.93\00:11:07.93 God blessed him as he followed God's plan for his life. 00:11:07.96\00:11:11.37 So I think that's good. 00:11:11.40\00:11:13.54 Very interesting and how biblical prescriptions 00:11:13.57\00:11:16.71 are something that we can depend on 00:11:16.74\00:11:18.71 throughout life. 00:11:18.74\00:11:20.11 You know, I'm very curious, 00:11:20.14\00:11:21.48 we talked about valves, you know, 00:11:21.51\00:11:22.98 and things are gonna change in valves. 00:11:23.01\00:11:24.75 If God doesn't take us home, 00:11:24.78\00:11:26.41 what's gonna be 10 or 20 years from now? 00:11:26.45\00:11:28.85 We didn't have mitral clips. 00:11:28.88\00:11:30.79 We didn't have trans aortic valve replacements 00:11:30.82\00:11:32.82 just 10 years ago. 00:11:32.85\00:11:34.29 Technology is growing faster now than ever before. 00:11:34.32\00:11:37.56 Yeah. 00:11:37.59\00:11:38.93 Not only medical technology, but now we're learning 00:11:38.96\00:11:40.80 even about lifestyle technology. 00:11:40.83\00:11:42.50 We're finding out 00:11:42.53\00:11:43.87 how our gut influences our health, 00:11:43.90\00:11:45.50 how our brain, our stress, our rest patterns, 00:11:45.53\00:11:48.50 everything that comes into our brain affects us. 00:11:48.54\00:11:51.17 That's right.
