Welcome back to the program. 00:00:01.06\00:00:02.09 I'm Charles Mills here with cardiologist, Dr. James Marcum, 00:00:02.12\00:00:04.68 the founder/director of Heartwise Ministries, 00:00:04.71\00:00:06.72 and we're going to answer some questions that have come in to 00:00:06.75\00:00:09.04 heartwiseministries. org 00:00:09.07\00:00:11.34 on the subject of urological issues - very important. 00:00:11.37\00:00:14.73 We've discussed how important water is to us, 00:00:14.76\00:00:17.43 and that we need to have that to help clean out the toxins 00:00:17.46\00:00:20.39 that collect in our body... 00:00:20.42\00:00:21.80 And our bodies are pretty dirty places inside - 00:00:21.86\00:00:25.12 if you stop and think about it. 00:00:25.15\00:00:26.18 We have a lot of stuff coming in from the environment, 00:00:26.21\00:00:28.16 from our food and the body needs to get rid of that stuff, 00:00:28.19\00:00:30.81 and waste material from just the process 00:00:30.84\00:00:33.25 of being us takes up a lot of materials 00:00:33.28\00:00:36.97 and there's waste involved with that. 00:00:37.00\00:00:38.86 Okay, first question from heartwiseministries. org 00:00:38.89\00:00:41.54 and by the way, we invite you to 00:00:41.57\00:00:42.78 send your questions in to that address, 00:00:42.81\00:00:44.88 and there's a place you click there and you can 00:00:44.91\00:00:46.29 leave your questions for a future show. 00:00:46.32\00:00:48.03 We appreciate that - we build these shows 00:00:48.06\00:00:50.13 around your questions - that's why we know 00:00:50.16\00:00:52.05 we're cutting edge - we're right there ready for you 00:00:52.08\00:00:55.29 because these people are asking and Dr. Marcum is answering. 00:00:55.32\00:00:59.08 "What exactly is a kidney stone and how does it form?" 00:00:59.11\00:01:03.24 Good question! 00:01:03.27\00:01:04.42 Yeah, lots of people have kidney stones. 00:01:04.45\00:01:06.88 They hurt a lot - let me tell you, 00:01:06.91\00:01:08.83 especially when they're in positions 00:01:08.86\00:01:10.42 they're not supposed to be. 00:01:10.45\00:01:11.87 But kidney stones are basically made of different things... 00:01:11.90\00:01:15.85 The most common type of stone 00:01:15.88\00:01:17.51 is called a "calcium oxalate stone" 00:01:17.54\00:01:20.52 and most stones are made of that... 00:01:20.55\00:01:22.65 And what happens is for some unknown reason, 00:01:22.68\00:01:26.25 of course there's lots of reasons, 00:01:26.28\00:01:27.41 but sometimes people make extra calcium; 00:01:27.44\00:01:30.45 sometimes their genetics predispose them; 00:01:30.48\00:01:33.34 sometimes people get dehydrated... 00:01:33.37\00:01:35.47 Well, calcium forms in the kidneys and the tubules, 00:01:35.50\00:01:39.22 and as it's excreted, it gets together and it forms a stone. 00:01:39.25\00:01:42.63 Now when it forms a stone, guess what... 00:01:42.66\00:01:45.62 the urine can't get out or the waste product can't get out, 00:01:45.65\00:01:48.69 and the urine backs up, it stretches out that tube, 00:01:48.72\00:01:52.30 and it causes pain, usually referred pain to the back. 00:01:52.33\00:01:56.08 Now Charles, if you were passing a stone, 00:01:56.11\00:01:59.29 you know we have a lot of interesting things out here; 00:01:59.32\00:02:01.75 of I had this reflex hammer, 00:02:01.78\00:02:04.05 this tests your neurological system. 00:02:04.08\00:02:06.34 This would not help with the stone. Okay 00:02:06.37\00:02:10.67 We can look in the ears, but that would not 00:02:10.70\00:02:12.50 help with the stone. 00:02:12.53\00:02:13.56 Okay, that would not help. 00:02:13.59\00:02:16.05 Taking a blood pressure would not help 00:02:16.08\00:02:17.75 because the blood pressure would 00:02:17.78\00:02:18.81 go high if you're passing a stone. 00:02:18.84\00:02:20.67 So the way we tell, for sure... 00:02:20.70\00:02:22.72 You know, a person comes in with the symptoms... 00:02:22.75\00:02:24.20 There are a couple of ways we tell if a person 00:02:24.23\00:02:25.86 might be passing a stone. 00:02:25.89\00:02:28.06 I'd say, "You know, I think you're passing a stone" 00:02:28.09\00:02:30.21 I might hit you in the flank and you might go "Ooo, Ooo" 00:02:30.24\00:02:32.96 because the ureters are more in the back part of you, 00:02:32.99\00:02:37.61 so if I did this and you jumped, 00:02:37.64\00:02:39.36 that could be a sign that you were passing a stone. 00:02:39.39\00:02:42.25 But what we do is a couple of things... 00:02:42.28\00:02:43.68 We can have a urine - you might notice yourself 00:02:43.71\00:02:45.99 if you're passing a stone because your urine 00:02:46.02\00:02:47.67 might come out bloody because the stone is irritating 00:02:47.70\00:02:50.54 that lining - it might have some blood pass out. 00:02:50.57\00:02:52.97 Another thing that we can do is x-rays and ultrasounds. 00:02:53.00\00:02:56.49 There is something called an "IVP" where we look 00:02:56.52\00:02:58.87 at the tubes to see that. 00:02:58.90\00:03:00.24 Sometimes a CAT scan can show stones, 00:03:00.27\00:03:02.69 but we usually have to do what we call an "imaging test" 00:03:02.72\00:03:05.40 to see if you have a stone. 00:03:05.43\00:03:07.07 Now that won't tell us what type of stone you have. 00:03:07.10\00:03:10.01 Remember what we said, we have calcium oxalate stones, 00:03:10.04\00:03:12.89 we also have another type of stone called "uric acid stones" 00:03:12.92\00:03:17.07 Uric acid stones are very much related to the food we eat, 00:03:17.10\00:03:20.78 and you know, we know that people that have high meat, 00:03:20.81\00:03:22.56 high fat diet have more uric acid stones. 00:03:22.59\00:03:25.26 There is also a type of stone called "struvite" 00:03:25.29\00:03:27.87 So we can't tell just by looking at it, 00:03:27.90\00:03:29.54 but we could tell you have a stone. 00:03:29.57\00:03:31.39 The stone is not good to stay in there. 00:03:31.42\00:03:33.23 Some stones are small and their characteristic 00:03:33.26\00:03:35.84 will let your doctor know that it will pass through. 00:03:35.87\00:03:39.08 Painfully - maybe though, right? 00:03:39.11\00:03:41.20 It could cause some pain. 00:03:41.23\00:03:42.26 A lot of people will drink lots of water, 00:03:42.29\00:03:44.47 3 to 5 liters a day to help pass that stone through 00:03:44.50\00:03:48.82 because you want to just push, push fluid through 00:03:48.85\00:03:51.07 to help the fluid push that through. 00:03:51.10\00:03:53.02 I've seen some people do the - I don't know if that helps, 00:03:53.05\00:03:55.86 it helps to stand up straight, that kind of thing. 00:03:55.89\00:03:59.34 We used to give medicines to dilate that smooth muscle 00:03:59.37\00:04:02.79 so it might help a little soon. 00:04:02.82\00:04:04.54 Sometimes a stone will not pass, 00:04:04.57\00:04:06.85 and we can sometimes tell by the size of the stone, 00:04:06.88\00:04:09.52 the irregularity on imaging test. 00:04:09.55\00:04:12.03 The real danger if that stone won't pass is that 00:04:12.06\00:04:15.04 fluid backs up in the kidneys and there's the condition called 00:04:15.07\00:04:17.56 "hydronephrosis" where the waste products back up into the kidney 00:04:17.59\00:04:22.31 The kidney starts to get bigger and the kidney can actually 00:04:22.34\00:04:24.74 go into failure if you have a stone, 00:04:24.77\00:04:27.94 even though the other side is still working. Right 00:04:27.97\00:04:30.48 So if urine doesn't move out, sometimes it can even 00:04:30.51\00:04:34.24 lead to infections. 00:04:34.27\00:04:35.49 So once we make this diagnosis of a stone, 00:04:35.53\00:04:37.62 either calcium or uric acid, or what type of stone, 00:04:37.65\00:04:40.49 we have to decide whether we're going to pass it. 00:04:40.52\00:04:43.12 Usually, if you're going to pass it on your own, 00:04:43.15\00:04:44.80 it takes lots of pain medications 00:04:44.83\00:04:47.08 to sort of... I can see why, yes... 00:04:47.11\00:04:48.68 Now this is a place where narcotics 00:04:48.71\00:04:50.35 might be a good thing to have because the pain 00:04:50.38\00:04:52.74 from a kidney stone could be so excruciating 00:04:52.77\00:04:55.14 that you might not be able to do 00:04:55.17\00:04:56.58 well if you didn't have pain medicine. 00:04:56.61\00:04:58.24 If the stone does pass, we usually, hopefully, 00:04:58.27\00:05:01.76 we will have the person catch the stone in their urine stream 00:05:01.79\00:05:05.11 so we can examine it under a microscope to see what 00:05:05.14\00:05:08.37 type of stone it is because once we get the 00:05:08.40\00:05:10.80 acute problem done, then we want to say, 00:05:10.83\00:05:12.98 "Hey, I had a stone, I don't want to ever have a stone again, 00:05:13.01\00:05:16.47 what can I do to prevent stone formation?" 00:05:16.50\00:05:19.51 So when they look at the stone through the microscope, 00:05:19.54\00:05:21.52 they can determine the type of makeup, 00:05:21.55\00:05:24.00 and then they would know where it came from, 00:05:24.03\00:05:25.24 then they know why it was formed or how it was formed. 00:05:25.27\00:05:27.80 Right, they can know a lot about the stone. 00:05:27.83\00:05:30.59 You know, they're going to want to look to see if your 00:05:30.62\00:05:32.25 parathyroids are handling calcium good. 00:05:32.28\00:05:34.59 Some foods can precipitate stone formation. 00:05:34.62\00:05:37.57 Sometimes just being dehydrated can trigger stone formation. 00:05:37.60\00:05:42.95 That's if the stone passes naturally. 00:05:42.98\00:05:44.64 What if it doesn't pass naturally? 00:05:44.67\00:05:47.32 Well, there's a procedure now, in certain types of stones, 00:05:47.35\00:05:50.91 called "lithotripsy" 00:05:50.94\00:05:52.74 They use sound waves to make the stone into sort of "gravel" 00:05:52.77\00:05:56.33 so the gravel passes through. 00:05:56.36\00:05:57.81 So they can just ZAP that with sound wave energy. 00:05:57.84\00:06:00.83 Sometimes though, they have to go up and retrieve the stone. 00:06:00.86\00:06:04.46 That's painful. 00:06:04.49\00:06:06.11 Sometimes they have to do surgery to take the stone out. 00:06:06.14\00:06:08.56 And those are the most painful situations you can get into. 00:06:08.59\00:06:11.47 I have a friend now - usually it happens in the males 00:06:11.50\00:06:15.44 usually in the 40s and 50s - 00:06:15.47\00:06:16.90 that's when they start passing stones. 00:06:16.94\00:06:18.63 He's passing so many stones, it's debilitating him, 00:06:18.67\00:06:21.39 and he keeps passing one after another - he keeps making these. 00:06:21.42\00:06:24.35 They looked at his thyroid gland - that was okay. 00:06:24.38\00:06:26.92 They tried to adjust his diet - that was okay. 00:06:26.95\00:06:29.48 He's drinking a lot of water. 00:06:29.51\00:06:30.97 His genetics is just still making lots of stones, 00:06:31.00\00:06:34.94 and he didn't have it 10 years ago, 00:06:34.97\00:06:36.36 but there's something about this age where the genetics 00:06:36.39\00:06:39.38 does change - you start making lots of stones. 00:06:39.41\00:06:42.19 So kidney stones are our big issue, 00:06:42.22\00:06:45.23 but we have medicines that can do stuff about it. 00:06:45.26\00:06:48.03 I was wondering what they used to do about it 00:06:48.06\00:06:49.96 400 years ago. 00:06:49.99\00:06:51.02 I guess they had to pass it then, 00:06:51.05\00:06:53.07 or go into kidney failure - they had no other chances. 00:06:53.10\00:06:56.20 ...But stones are really devastating. 00:06:56.23\00:06:58.62 Again, modern medicine to the rescue... 00:06:58.65\00:07:00.33 If there's a place for it, there it is. 00:07:00.36\00:07:01.84 And things that we can do to help PREVENT stone formation 00:07:01.87\00:07:04.82 that always comes up. 00:07:04.85\00:07:05.88 We want to give everyone a few practical things that they 00:07:05.91\00:07:09.55 can do - one, lots of water! 00:07:09.58\00:07:11.30 If you've had stones, you'll want to hydrate yourself well. 00:07:11.33\00:07:14.12 You want to make sure your doctor checks 00:07:14.15\00:07:16.24 your calcium levels in the parathyroid glands 00:07:16.27\00:07:19.08 regulates calcium. 00:07:19.11\00:07:21.41 Depending on what type of stone, 00:07:21.44\00:07:22.58 they might put you on a special diet. 00:07:22.62\00:07:24.29 We usually recommend - get rid of things like caffeine, 00:07:24.33\00:07:27.36 get rid of things like animal products - as much as you can. 00:07:27.39\00:07:32.43 There are people that drink citric acid - like lemonade. 00:07:32.46\00:07:36.48 That can help calcium oxalate stones, so citric acid eliminate 00:07:36.51\00:07:40.66 sometimes that can help with that. 00:07:40.69\00:07:42.01 Those are some 2 practical things that people could do 00:07:42.04\00:07:45.23 to help prevent stone formations. All right! 00:07:45.26\00:07:48.44 Well let's move from the kidneys into the prostate. Oh boy 00:07:48.47\00:07:52.39 Question here was from a listener and sent into 00:07:52.42\00:07:55.62 heartwiseministries. org 00:07:55.65\00:07:57.12 "What can be done to help an enlarged prostate?" 00:07:57.15\00:08:00.87 I am assuming this is a man writing this... 00:08:00.90\00:08:03.14 "I have to get up 6 or 7 times a night" 00:08:03.17\00:08:06.38 So something is backing it up or why is he getting up so much? 00:08:06.41\00:08:10.22 Well remember, the urethra passes through the prostate, 00:08:10.25\00:08:14.30 and when the prostate gets big, the urine can't pass out as more 00:08:14.33\00:08:18.19 Sometimes you can have infections in the prostate 00:08:18.22\00:08:20.67 ...that's called "prostatitis" 00:08:20.70\00:08:22.28 That's very painful. 00:08:22.31\00:08:23.65 Sometimes the urine can't get out because of constriction, 00:08:23.68\00:08:26.84 and you have to go many times a night. 00:08:26.87\00:08:29.35 Sometimes, 4, 5, 6 times a night you might not have a 00:08:29.38\00:08:32.70 good stream - you might dribble. 00:08:32.73\00:08:35.22 When that starts happening, that's your cue that 00:08:35.25\00:08:37.94 you might have a prostate problem. 00:08:37.97\00:08:39.88 You need to get some help! 00:08:39.91\00:08:41.69 People that sit for prolonged periods of time, 00:08:41.72\00:08:44.02 that can make it worse as well... 00:08:44.05\00:08:45.90 So you'll want to get up and move. 00:08:45.93\00:08:47.51 When people have these symptoms, 00:08:47.54\00:08:50.35 then the doctor that handles that 00:08:50.38\00:08:52.42 is called a "urologist" usually. 00:08:52.45\00:08:54.99 Now a regular medical doctor can feel the prostate 00:08:55.02\00:08:57.67 to see if it's enlarged, especially if it has 00:08:57.70\00:09:00.04 nodules on it because with of the prostate 00:09:00.07\00:09:02.93 can have an enlarged prostate as well. 00:09:02.96\00:09:04.83 And that's such a pleasant exam... 00:09:04.86\00:09:06.28 Well, I guess - I really haven't had one. 00:09:06.31\00:09:08.71 Oh you haven't had one? No no 00:09:08.75\00:09:09.87 You haven't had a prostate exam? No I haven't 00:09:09.91\00:09:11.35 You've NEVER had a prostate exam? I never have 00:09:11.38\00:09:13.44 Never have... Why? 00:09:13.47\00:09:15.00 Well they recommend about age 50 that you 00:09:15.03\00:09:17.23 start looking into it, so it's on the agenda you know. 00:09:17.26\00:09:20.85 It's on the agenda... Okay but you know it's waiting for you! 00:09:20.89\00:09:23.42 I've had my prostate specific antigen checked, 00:09:23.45\00:09:26.49 but I've never really had symptoms of prostate problems. 00:09:26.53\00:09:29.63 ...Of the dribbling and getting up 7 or 8 times, 00:09:29.66\00:09:31.91 but if I did, I would surely know what to do. 00:09:31.94\00:09:34.40 Well they would check your prostate out, 00:09:34.43\00:09:36.03 see if it was enlarged. 00:09:36.06\00:09:37.31 Sometimes they can do ultrasounds 00:09:37.34\00:09:39.05 to learn more about the prostate, 00:09:39.08\00:09:40.88 and there are a couple of things that they can do. 00:09:40.91\00:09:42.89 One of the natural ways that some people treat prostates 00:09:42.92\00:09:46.90 is called "saw palmetto" 00:09:46.93\00:09:49.61 Another natural way, the common sense way, 00:09:49.64\00:09:52.05 is don't drink a lot of fluids at night. That's true! 00:09:52.08\00:09:54.51 If you don't have fluid at night, 00:09:54.55\00:09:55.58 it doesn't bother you because you don't get up 00:09:55.61\00:09:57.05 over and over to wake up and deal with these things... 00:09:57.08\00:09:59.16 And finally, there are some medicines that relax the urethra 00:09:59.19\00:10:02.53 You relax it - so more urine can pass through. 00:10:02.56\00:10:05.41 Sometimes that helps for quite a long time. 00:10:05.44\00:10:07.34 But sometimes, Charles, it gets so bad that something 00:10:07.37\00:10:10.72 big has to be done. 00:10:10.75\00:10:12.27 And, they call that a transurethral resection 00:10:12.30\00:10:16.31 where they go up and they make the prostate, 00:10:16.34\00:10:18.43 as a surgical procedure, where they make it smaller. 00:10:18.46\00:10:21.28 They go in through your orifice there and they scrape 00:10:21.31\00:10:24.10 that out and they make it smaller, 00:10:24.13\00:10:25.35 and usually that lasts for several times, 00:10:25.38\00:10:26.83 and gets the urine going well. 00:10:26.86\00:10:29.69 They numb it up - it's a little painful procedure, 00:10:29.72\00:10:32.29 but some men have to have that and they start 00:10:32.32\00:10:34.22 resting better at night. 00:10:34.25\00:10:35.38 Now, I think in prostate disease and in ureter disease, 00:10:35.41\00:10:40.06 this is a GREAT place for modern medicine. Absolutely! 00:10:40.09\00:10:43.82 Because before this, when these things happened, 00:10:43.85\00:10:46.16 you know - when the aging process happened, 00:10:46.19\00:10:47.79 the prostate got big or genetically, it got big, 00:10:47.82\00:10:50.45 you just didn't get any relief. 00:10:50.48\00:10:51.82 You just couldn't get the urine out. 00:10:51.85\00:10:53.17 You couldn't get the urine out - it backflowed, 00:10:53.20\00:10:54.93 it caused damage to the kidneys. 00:10:54.96\00:10:56.44 You couldn't get your waste out, 00:10:56.47\00:10:57.83 the waste buildup in you; 00:10:57.86\00:10:59.21 you had terrible things happen and you died. 00:10:59.24\00:11:01.73 So this is a great place for modern medicine - 00:11:01.76\00:11:04.40 is treating prostate disease, stone disease, all that, 00:11:04.43\00:11:07.79 but, you know, we've talked a lot today, 00:11:07.82\00:11:09.63 but we want to keep things moving through our bodies... 00:11:09.66\00:11:12.16 Water and healthy diet, less stones form 00:11:12.19\00:11:15.76 when you eat healthier as well. 00:11:15.79\00:11:17.28 Well that really is an important thing for us to know 00:11:17.31\00:11:20.21 because this problem seems to be not isolated 00:11:20.24\00:11:24.54 to this country - it seems to be worldwide... Yeah 00:11:24.57\00:11:27.38 and we really need to address these things. 00:11:27.41\00:11:29.92 I think we have time for one more question here. 00:11:29.95\00:11:32.55 Natural treatment for stones. 00:11:32.58\00:11:34.80 Let's go back to the stones for a minute. 00:11:34.83\00:11:36.35 You mentioned water... 00:11:36.38\00:11:37.48 What else can we do naturally, 00:11:37.51\00:11:39.82 other than drinking water to get rid of stones? 00:11:39.85\00:11:40.89 We talked about stones and, depending upon what type 00:11:40.92\00:11:43.37 of stone it is... uric acid which we get in lemonade 00:11:43.40\00:11:46.18 and citrus products, especially the stones that are 00:11:46.21\00:11:49.89 made from uric acid... 00:11:49.92\00:11:51.25 We want to stay away from animal products 00:11:51.28\00:11:53.54 because that has uric acids. 00:11:53.57\00:11:54.86 We want to avoid caffeine and there's a big long list 00:11:54.89\00:11:57.73 of foods that you can look at that might trigger this, 00:11:57.76\00:12:01.04 and we want to make sure people have a good 00:12:01.07\00:12:02.12 metabolism too, especially of their calcium, 00:12:02.15\00:12:04.79 there are no abnormalities there. 00:12:04.82\00:12:06.03 I would assume that exercise would be important for this too, 00:12:06.06\00:12:08.58 just getting the running in.
