Very bad. 00:02:06.33\00:02:08.49 Now, I think the body is so wonderfully made. 00:02:08.52\00:02:11.39 You know, we've our controls center, 00:02:11.42\00:02:13.03 the brain and all the organs depend on that. 00:02:13.06\00:02:15.71 We have a stomach that can digest stuff 00:02:15.74\00:02:18.18 and get stuff ready for energy. 00:02:18.21\00:02:20.19 Then it gets in our bloodstream, 00:02:20.22\00:02:21.59 the blood takes it everywhere 00:02:21.62\00:02:23.12 and we got this strong structure but the kidney is an organ 00:02:23.15\00:02:26.38 that a lot of people don't understand very well. 00:02:26.41\00:02:29.35 Because you know you can't really feel it. 00:02:29.38\00:02:31.08 They sort of sit in the back, they're like kidney beans. 00:02:31.11\00:02:33.99 But luckily we have two of them. Yes. 00:02:34.02\00:02:36.05 I heard a story the other day 00:02:36.08\00:02:37.67 about a gentlemen wanting to sell his kidney. 00:02:37.70\00:02:40.35 You know, 'cause someone else's kidney has failed. Yeah. 00:02:40.38\00:02:42.40 And they would actually sell these kidneys on the market. 00:02:42.43\00:02:45.21 Now, he got in trouble for it but in some parts, 00:02:45.24\00:02:47.14 you know that because you have two, 00:02:47.17\00:02:48.54 you can take one out and put the other one in. Yeah. 00:02:48.57\00:02:50.30 If you think of the organs that we have two of. 00:02:50.33\00:02:52.43 You know, we have two lungs, so you can live with one lung, 00:02:52.46\00:02:54.96 we have two kidneys, so you can live with one kidney. 00:02:54.99\00:02:56.86 We only have one liver but it regenerates very well. 00:02:56.89\00:02:59.88 We have two legs. 00:02:59.91\00:03:01.28 Wish we had two brains but we don't. 00:03:01.31\00:03:03.00 We have two eyes 00:03:03.03\00:03:04.40 but God designed us in these ways for a reason. 00:03:04.43\00:03:07.73 And the kidneys follow the same theme 00:03:07.76\00:03:10.25 as some of these other organ systems. 00:03:10.28\00:03:12.06 Prolonged exposure to damaging things hurt the system. 00:03:12.09\00:03:15.38 And these damaging things go 00:03:15.41\00:03:16.94 against the way we were designed to be. 00:03:16.97\00:03:20.45 Let's, you know, 00:03:20.48\00:03:21.85 how would you even know you have a kidney problem? 00:03:21.88\00:03:23.70 You know, that's a great question. 00:03:23.73\00:03:25.10 You know, there is-- when evaluating a kidney, 00:03:25.13\00:03:28.13 there's three things I want to know right away. 00:03:28.16\00:03:30.44 Is one, things that can make the kidney look bad 00:03:30.47\00:03:32.97 is if it's not getting enough blood or it's dehydrated. Okay? 00:03:33.00\00:03:36.41 We call that pre-renal because the problem happens 00:03:36.44\00:03:39.51 before it gets to the kidney, right? Yeah. 00:03:39.54\00:03:41.37 Sometimes, the problem happens downstream from the kidneys, 00:03:41.40\00:03:45.66 in the ureters which carry the urine 00:03:45.69\00:03:48.01 to the bladder and out. 00:03:48.04\00:03:49.58 Sometimes, we call that post-obstructive. 00:03:49.61\00:03:52.31 We have problems post-renal, beyond the kidneys. 00:03:52.34\00:03:55.28 And the most common cause of that is like a stone. Yes. 00:03:55.31\00:03:57.54 You know, if you have a stone, 00:03:57.57\00:03:58.94 it blocks things up and there's back pressure in the kidney, 00:03:58.97\00:04:01.13 the kidneys don't look happy. 00:04:01.16\00:04:02.53 So there's pre-renal, post-renal and then 00:04:02.56\00:04:04.55 there is the kidney diseases themselves. 00:04:04.58\00:04:06.69 And things that can damage the kidney is diabetes, 00:04:06.72\00:04:09.66 probably, the number one cause of damage to the kidneys. 00:04:09.69\00:04:12.34 The kidneys' blood-vessels can be damaged. 00:04:12.37\00:04:14.51 The kidney itself can also have problems with it. 00:04:14.54\00:04:16.79 We have conditions like polycystic kidney disease, 00:04:16.82\00:04:19.85 where we get-- it's sort of a genetic disease 00:04:19.88\00:04:22.38 where you get a cyst in the kidney, 00:04:22.41\00:04:23.88 so the kidneys can't work well. 00:04:23.91\00:04:25.94 Sometimes, cancer can get in the kidneys. 00:04:25.97\00:04:28.69 Sometimes, infection can damage the kidneys. 00:04:28.72\00:04:31.46 One of the big things we see 00:04:31.49\00:04:32.86 damaging the kidneys nowadays is medicines. 00:04:32.89\00:04:35.69 Did you know that certain medicines, 00:04:35.72\00:04:37.09 non steroidals like aspirins, 00:04:37.12\00:04:39.02 certain antibiotics can damage the kidney as well? 00:04:39.05\00:04:42.10 I can see why they would. Yeah. 00:04:42.13\00:04:44.26 The kind of--the kidneys are trying to flush waste out. Yes. 00:04:44.29\00:04:49.05 And you put all these toxins in 00:04:49.08\00:04:50.97 from the medicines and everything. 00:04:51.00\00:04:52.37 Yeah, and sometimes the kidney can be overwhelmed. 00:04:52.40\00:04:55.07 Oh, yeah. You can overdo it. 00:04:55.10\00:04:56.50 And sometimes what happens 00:04:56.53\00:04:57.90 is you have three things going on at once. 00:04:57.93\00:04:59.87 And we're seeing that a couple of times is, 00:04:59.90\00:05:01.29 the kidney has been damaged from diabetes and maybe medications, 00:05:01.32\00:05:05.06 there might be a stone in the way 00:05:05.09\00:05:06.68 so the urine can't get up, 00:05:06.71\00:05:08.09 and then the person gets dehydrated 00:05:08.12\00:05:09.76 so that you don't get enough fluid and blood to kidney. Yes. 00:05:09.79\00:05:11.96 When that happens, the kidneys' function can deteriorate quickly 00:05:11.99\00:05:15.31 and a little problem can be a big problem. 00:05:15.34\00:05:17.51 And when the kidney doesn't get waste out, 00:05:17.54\00:05:19.33 if any medicine is floating around in you, 00:05:19.36\00:05:21.57 it stays in you a long time. Yes. 00:05:21.60\00:05:24.17 And unfortunately, when that happens, 00:05:24.20\00:05:26.60 you know, you can have some severe consequences. 00:05:26.63\00:05:28.48 Also in the kidney-- 00:05:28.51\00:05:29.88 it regulates metabolism and all the electrolytes. 00:05:29.91\00:05:32.41 I mean, if your potassium goes high that can be deadly too. 00:05:32.44\00:05:36.53 The kidney removes fluid from the body 00:05:36.56\00:05:38.79 and sometimes a person that has a weak heart 00:05:38.82\00:05:41.04 has bad kidney function not 'cause their kidney is bad, 00:05:41.07\00:05:43.38 because it doesn't get enough blood 00:05:43.41\00:05:44.78 to the kidney to it to do its job. 00:05:44.81\00:05:46.24 So you can back up fluid. 00:05:46.27\00:05:47.98 Things are so interrelated in our body. 00:05:48.01\00:05:49.38 Yes, they are. 00:05:49.41\00:05:50.78 And sometimes when you have fluid backed up, 00:05:50.81\00:05:52.18 you know, if it's from the heart, 00:05:52.21\00:05:53.58 you can go into heart failure and the kidney might need 00:05:53.61\00:05:54.98 a little extra help getting the fluid out. 00:05:55.01\00:05:56.38 We sometimes give a diuretic to that situation. 00:05:56.41\00:05:59.23 So the kidney is right involved in a lot of things 00:05:59.26\00:06:01.57 and it's very important. 00:06:01.60\00:06:02.97 The way we tell about the kidney, 00:06:03.00\00:06:04.52 once we decide, you know, 00:06:04.55\00:06:05.92 whether it's before the kidney or after the kidney 00:06:05.95\00:06:07.61 or in the kidney, is we look at the blood work. 00:06:07.64\00:06:09.85 And the blood work 00:06:09.88\00:06:11.25 gives us some numbers that help us know. 00:06:11.28\00:06:12.65 For instance, the potassium's real high, 00:06:12.68\00:06:14.14 then that could be the kidney, not getting rid of it. 00:06:14.17\00:06:16.05 There's a substance that we measure 00:06:16.08\00:06:17.56 in the blood called creatinine. 00:06:17.59\00:06:19.01 That's a measurement of a metabolite 00:06:19.04\00:06:21.28 that's broken down in the body. 00:06:21.31\00:06:22.69 If that level starts going up, 00:06:22.72\00:06:24.09 we say the kidney is not doing well. 00:06:24.12\00:06:26.18 Sometimes we can look at the urine-- 00:06:26.21\00:06:27.92 if we see protein in urine or an infection in the urine. 00:06:27.95\00:06:31.25 Well, then we can look into the microscope 00:06:31.28\00:06:33.32 to see if there's casts, 00:06:33.35\00:06:34.72 that might give us some information 00:06:34.75\00:06:36.12 about whether the kidney is being damaged. 00:06:36.15\00:06:38.37 That might let us know what's wrong with the kidney. 00:06:38.40\00:06:40.74 Sometimes, we can do ultrasound 00:06:40.77\00:06:42.61 which is pictures of the kidneys to see how big they are. 00:06:42.64\00:06:44.96 In certain conditions, they get smaller 00:06:44.99\00:06:47.15 and in some conditions, you know, 00:06:47.18\00:06:48.55 if they can't get urine out, they get bigger. 00:06:48.58\00:06:50.52 You know, you have terms 00:06:50.55\00:06:51.92 like hydronephrosis, things like that. 00:06:51.95\00:06:54.09 Sometimes we can do biopsy of the kidneys 00:06:54.12\00:06:56.21 to see what type of disease 00:06:56.24\00:06:57.61 or certain connective tissue disease. 00:06:57.64\00:06:59.39 One of the ones that I had to learn 00:06:59.42\00:07:00.79 in medical school is called Wegener's granulomatosis. 00:07:00.82\00:07:03.64 That's a big name of a connective tissue disease 00:07:03.67\00:07:05.81 that causes the kidney. 00:07:05.84\00:07:07.33 But by far when the kidney malfunctions, 00:07:07.36\00:07:10.08 it affects the entire body 00:07:10.11\00:07:11.66 but we do have treatment for kidney failure 00:07:11.69\00:07:14.14 and that treatment is termed dialysis at times. Yeah. 00:07:14.17\00:07:17.58 And unfortunately, many Americans are under dialysis, 00:07:17.61\00:07:20.49 but, you know, what the number one cause 00:07:20.52\00:07:22.29 of the kidney failure is, in America today? 00:07:22.32\00:07:25.74 And probably the entire world? 00:07:25.77\00:07:27.14 The number one cause? No, what is it? 00:07:27.17\00:07:28.68 Diabetes. Really? 00:07:28.71\00:07:30.52 Diabetes untreated, damages the kidney 00:07:30.55\00:07:33.38 and if you can think about things is 00:07:33.41\00:07:35.11 the most common type of diabetes is Type 2 diabetes. 00:07:35.14\00:07:38.49 Type 2 diabetes not treated, 00:07:38.52\00:07:40.67 damages the kidney, leads to dialysis. 00:07:40.70\00:07:42.61 So we want everyone to know, if you have Type 2 diabetes, 00:07:42.64\00:07:45.34 you might save your kidneys' life 00:07:45.37\00:07:47.02 by getting this Type 2 diabetes addressed. 00:07:47.05\00:07:49.75 And as the numbers for Type 2 diabetes goes up, 00:07:49.78\00:07:52.35 guess what's gonna happen to renal disease. 00:07:52.38\00:07:54.65 It's gonna go up, too. 00:07:54.68\00:07:56.25 So guess what's gonna happen to another leading cause of death. 00:07:56.28\00:07:59.04 The kidneys are gonna get blamed again-- 00:07:59.07\00:08:00.44 Climb the charts. It's gonna continue to climb the chart. 00:08:00.47\00:08:02.87 And we're gonna talk to people about, 00:08:02.90\00:08:04.85 you know, kidneys-- we can put them on dialysis. 00:08:04.88\00:08:06.75 So we can keep people alive 00:08:06.78\00:08:08.23 but we don't really necessarily fix the problem. 00:08:08.26\00:08:10.86 And being on dialysis-- 00:08:10.89\00:08:12.64 now dialysis is like an artificial kidney. 00:08:12.67\00:08:14.76 You know, you go three times a week, 00:08:14.79\00:08:16.34 they usually have shunts in your arms 00:08:16.37\00:08:19.27 where they hook you up to the machine 00:08:19.30\00:08:20.73 and they take all the blood out of your body. 00:08:20.76\00:08:22.62 They filter it through that machine, 00:08:22.65\00:08:24.15 it takes the toxins in the volume out 00:08:24.18\00:08:26.29 and then it runs right around and put it back in you. 00:08:26.32\00:08:28.71 That's one type, and we call that hemodialysis 00:08:28.74\00:08:31.63 because it works with the blood. 00:08:31.66\00:08:33.03 There's also one called peritoneal dialysis 00:08:33.06\00:08:35.26 where you put bags, 00:08:35.29\00:08:36.66 you're hooked up to bags and does the same things. 00:08:36.69\00:08:39.01 The advantage of that type of dialysis is you can be mobile. 00:08:39.04\00:08:42.45 Now, one of the things about kidneys is every one wants it, 00:08:42.48\00:08:44.69 you know, we talked about a transplant. 00:08:44.72\00:08:46.16 Every one wants a new kidney, 00:08:46.19\00:08:47.56 to give the kidney a second chance. 00:08:47.59\00:08:48.96 So if a person has a second chance of getting a kidney, 00:08:48.99\00:08:51.33 I want to make sure they don't do things that hurt that kidney. 00:08:51.36\00:08:54.62 You know, when you think about all the things 00:08:54.65\00:08:56.02 that hurt the kidneys, diabetes, 00:08:56.05\00:08:57.90 dehydration, a weak heart, 00:08:57.93\00:09:00.46 stones that block up the uterus, 00:09:00.49\00:09:02.57 all these different things that can damage the kidneys, 00:09:02.60\00:09:04.89 we want to keep those people away 00:09:04.92\00:09:06.41 from those things so they can be healthy. 00:09:06.44\00:09:08.14 So the kidney is a very important organ 00:09:08.17\00:09:10.05 but it usually does what it's supposed to do 00:09:10.08\00:09:12.36 as long as we don't introduce these other things. 00:09:12.39\00:09:14.46 That's the only probably 2 or 3 things 00:09:14.49\00:09:16.28 that can't be tied back to lifestyle. 00:09:16.31\00:09:18.35 Yeah, I was gonna say 00:09:18.38\00:09:19.75 as you were going through that list 00:09:19.78\00:09:21.15 that these are things within our control. Right. 00:09:21.18\00:09:23.45 We can decide, if we want to have this problem or not. 00:09:23.48\00:09:27.12 And that's got to be good news for most people, I think. 00:09:27.15\00:09:29.49 And if you look back about all this disease 00:09:29.52\00:09:31.72 that are killing people all around the country, 00:09:31.75\00:09:33.39 the leading causes, you know, 00:09:33.42\00:09:34.79 most of the diseases that we see 00:09:34.82\00:09:37.17 that are killing people, aren't killing people. 00:09:37.20\00:09:39.74 It's chronically diseased. They don't happen overnight. 00:09:39.77\00:09:43.66 And most of them are caused by our lifestyle. 00:09:43.69\00:09:45.44 They're not caused by genetics 00:09:45.47\00:09:46.97 or something we can't get rid of. 00:09:47.00\00:09:48.64 Now, there are few that we can't. 00:09:48.67\00:09:50.14 You know there are the few conditions 00:09:50.17\00:09:51.54 in the kidneys that are genetics. 00:09:51.57\00:09:52.94 There's a few things that, you know, 00:09:52.97\00:09:54.34 that come in our kidneys and get in our way. 00:09:54.37\00:09:55.74 But lot of the times, it is lifestyle and as, 00:09:55.77\00:09:58.86 if we can't change our lifestyle in the world we live in, 00:09:58.89\00:10:01.54 guess, what's gonna happen. 00:10:01.57\00:10:02.94 We're gonna see more kidney failure, 00:10:02.97\00:10:04.67 more dialysis, that doesn't fix the problem. 00:10:04.70\00:10:07.57 And, of course, with dialysis comes the problems of infections 00:10:07.60\00:10:10.49 comes the problem of drinking too much fluid 00:10:10.52\00:10:13.23 when you shouldn't be drinking too much fluid. 00:10:13.26\00:10:15.31 So there's a load of problems that come 00:10:15.34\00:10:16.87 if your kidneys do fail and you end up on dialysis. 00:10:16.90\00:10:19.91 And the medicines, Charles, 00:10:19.94\00:10:21.31 I can't over emphasize how important medicines are. 00:10:21.34\00:10:23.79 You know, all the medicines, 00:10:23.82\00:10:25.19 lot of the medicines work through the kidneys 00:10:25.22\00:10:27.33 and if you take too many medications, the kidney says, 00:10:27.36\00:10:30.85 "Listen, I'm only going to do so much work today. 00:10:30.88\00:10:33.64 I can't handle this work. 00:10:33.67\00:10:35.69 And you're not giving me enough fluids and I'm dehydrated. 00:10:35.72\00:10:38.65 I just can't handle it, I'm overwhelmed, 00:10:38.68\00:10:41.03 I'm not gonna do my job today." 00:10:41.06\00:10:42.72 And the kidney doesn't do a job 00:10:42.75\00:10:44.12 and the rest of the body finds out real quickly. 00:10:44.15\00:10:46.11 Now, acute renal failure-- 00:10:46.14\00:10:48.56 when the kidney goes down quick, 00:10:48.59\00:10:50.31 if you don't find out about it, you can die from that. Yes. 00:10:50.34\00:10:53.36 Because the potassium builds up, the potassium's high. 00:10:53.39\00:10:55.71 You know, fluid builds up in the body. 00:10:55.74\00:10:57.30 You can die from acute renal failure. 00:10:57.33\00:10:59.60 So if your kidney goes quick, fast-- 00:10:59.63\00:11:01.75 you need to find out pretty quick. 00:11:01.78\00:11:03.60 It's scary but, you know, 00:11:03.63\00:11:05.03 a lot of people don't think 00:11:05.06\00:11:06.43 about these kidney organs as being vital. 00:11:06.46\00:11:08.39 But they're vital to the body. 00:11:08.42\00:11:09.79 But usually if you treat the organ pretty good, 00:11:09.82\00:11:11.54 it does, it takes care of you. 00:11:11.57\00:11:13.73 Dr. Marcum, how do you approach a patient 00:11:13.76\00:11:16.63 that comes in to see you And you know that the problems 00:11:16.66\00:11:20.72 that this person is facing are self-inflicted? 00:11:20.75\00:11:23.59 How do you say that to them? 00:11:23.62\00:11:26.20 Oh, that's a good question, Charles. 00:11:26.23\00:11:27.71 And you know, 00:11:27.74\00:11:29.11 some people take the tough man approach. 00:11:29.14\00:11:31.06 "If you don't do this, you're gonna die." Yeah. 00:11:31.09\00:11:32.86 What I try to do is, I try to wrap truth in love 00:11:32.89\00:11:36.67 and I try to love people and not judge them. 00:11:36.70\00:11:39.24 You know, and I say, "Listen," 00:11:39.27\00:11:40.64 you know, "you're doing some things, 00:11:40.67\00:11:42.25 your diabetes is under good control, 00:11:42.28\00:11:43.83 we see that your kidney function's getting worse. 00:11:43.86\00:11:46.33 You know, we can give you 00:11:46.36\00:11:47.73 some medicines that treat your symptoms 00:11:47.76\00:11:49.13 but if we don't make some lifestyle changes, 00:11:49.16\00:11:51.07 this is what you can expect. 00:11:51.10\00:11:53.10 But I know some ways to get you feeling better. 00:11:53.13\00:11:56.15 I know the Great Physician that we can start talking to 00:11:56.18\00:11:58.76 and make an appointment with him 00:11:58.79\00:12:00.37 that can give you the power to make some changes. 00:12:00.40\00:12:02.36 That can help you learn 00:12:02.39\00:12:03.76 and give you the power to live healthy. 00:12:03.79\00:12:05.17 That can lead you to the doctors, 00:12:05.20\00:12:06.71 that can motivate you to do the things you need to do. 00:12:06.74\00:12:09.32 Are you interested in this?" 00:12:09.35\00:12:10.96 And that's about all you can do, Charles.
