00:00:56.07\00:00:58.29 Let's talk with Dr. James Marcum 00:00:58.32\00:00:59.80 about what diabetes is and how to recognize it 00:00:59.83\00:01:02.36 when it comes in knocking at your door. 00:01:02.39\00:01:04.27 Dr. Marcum, first, what is it? 00:01:04.30\00:01:06.71 Well, hopefully we won't come knocking 00:01:06.74\00:01:09.26 on your door, Charles. 00:01:09.29\00:01:11.06 You know, we've been talking 00:01:11.09\00:01:12.46 about some serious, serious issues here 00:01:12.49\00:01:14.31 about leading causes of death 00:01:14.34\00:01:15.89 and, you know, its diabetes is scary. 00:01:15.92\00:01:19.44 And the rate of diabetes has been climbing dramatically. 00:01:19.47\00:01:22.70 It's going up all the time. 00:01:22.73\00:01:24.67 Everyone's affected by diabetes. 00:01:24.70\00:01:27.23 And when I think about diabetes, 00:01:27.26\00:01:28.81 I want to make something that everyone can understand. 00:01:28.84\00:01:31.57 And diabetes is really abnormalities in glucose, 00:01:31.60\00:01:35.64 that sugar metabolism, 00:01:35.67\00:01:37.61 abnormalities in glucose metabolism. 00:01:37.64\00:01:40.58 And when this happens, what happens in the body? 00:01:40.61\00:01:43.16 The blood sugar goes up. 00:01:43.19\00:01:45.20 When the blood sugar goes up, the body goes, 00:01:45.23\00:01:46.95 "Oh, man, I got a lot of blood sugar, 00:01:46.98\00:01:48.48 got to do something with it." 00:01:48.51\00:01:49.88 Insulin levels go up. 00:01:49.91\00:01:51.62 And when insulin levels go up, there's some damage 00:01:51.65\00:01:54.10 that occurs to the endothelia, which is the inside lining 00:01:54.13\00:01:57.10 of every blood vessel in the body. 00:01:57.13\00:01:59.41 So when insulin levels go high 00:01:59.44\00:02:00.94 because of this extra blood sugar, 00:02:00.97\00:02:03.32 it causes all sorts of problems. 00:02:03.35\00:02:04.72 Now there's two types of diabetes. 00:02:04.75\00:02:07.23 Well, there's actually more than that, 00:02:07.26\00:02:08.63 but the ones that we want to focus on today is type 1 00:02:08.66\00:02:11.93 and that used to be called juvenile diabetes, 00:02:11.96\00:02:14.42 but now it's called insulin-dependent diabetes. 00:02:14.45\00:02:17.45 In those cases which are the minority of patients 00:02:17.48\00:02:19.79 that have diabetes, the immune system attacks 00:02:19.82\00:02:23.07 these cells in the pancreas called the Islets of Langerhans 00:02:23.10\00:02:27.19 and there's these beta cells that immune system destroys. 00:02:27.22\00:02:30.32 So these patients are dependent on insulin their entire life. 00:02:30.35\00:02:34.29 That's the majority of people. 00:02:34.32\00:02:35.96 Most other people have type 2 diabetes. 00:02:35.99\00:02:38.77 We used to call it adult-onset diabetes. 00:02:38.80\00:02:41.49 But nowadays, we just call it type 2. 00:02:41.52\00:02:44.14 Now that tends to be a diabetes, a lifestyle diabetic situation. 00:02:44.17\00:02:49.60 Now there's other types that you get 00:02:49.63\00:02:51.00 when your pregnancy that's called gestational diabetes. 00:02:51.03\00:02:54.39 There is some that's called by kidneys 00:02:54.42\00:02:55.98 and call that nephrogenic diabetes. 00:02:56.01\00:02:58.40 There's also diabetes insipidus. 00:02:58.43\00:03:00.48 There's bronze diabetes. 00:03:00.51\00:03:01.88 So you're gonna hear many different types. 00:03:01.91\00:03:03.66 But for our viewers today, I want us to focus on type 1, 00:03:03.69\00:03:06.80 which is insulin-dependent, okay? 00:03:06.83\00:03:09.04 The body needs insulin forever 00:03:09.07\00:03:10.65 and that's usually younger people. 00:03:10.68\00:03:12.05 And type 2 diabetes is the kind we get 00:03:12.08\00:03:14.46 because of this abnormal glucose metabolism. 00:03:14.49\00:03:17.82 So a person that has diabetes, the symptoms they might have, 00:03:17.85\00:03:21.74 if you might have it, you get very thirsty, okay. 00:03:21.77\00:03:24.45 Why do you get so thirsty? 00:03:24.48\00:03:25.85 Well, you get thirsty because your blood sugar is really high. 00:03:25.88\00:03:28.80 When the osmoles or the concentration 00:03:28.83\00:03:30.57 of blood's really high, 00:03:30.60\00:03:31.97 you tend to go to the bathroom more. 00:03:32.00\00:03:33.69 So that's another symptom, you go to the bathroom more 00:03:33.72\00:03:36.11 because you're losing fluids 00:03:36.14\00:03:37.51 because of this high concentration of sugar, 00:03:37.54\00:03:39.11 you want to drink more. 00:03:39.14\00:03:40.51 So you're also very thirsty, very-- 00:03:40.54\00:03:42.25 but usually we find it out on routine blood work. 00:03:42.28\00:03:45.34 And so a person that we think might have diabetes, 00:03:45.37\00:03:47.49 you might come to us and there's many different ways 00:03:47.52\00:03:49.48 that might get me a clue. 00:03:49.51\00:03:50.88 One thing is your urine. 00:03:50.91\00:03:52.28 You might be losing sugar in your urine. 00:03:52.31\00:03:53.88 You've probably heard of that before. 00:03:53.91\00:03:55.76 And another thing that we do is we can test your blood. 00:03:55.79\00:03:58.67 And if we measure your blood randomly 00:03:58.70\00:04:01.02 and it's greater than 200, 00:04:01.05\00:04:03.02 you know, we want the sugar in the cell 00:04:03.05\00:04:05.39 where to doing something to make energy. 00:04:05.42\00:04:07.20 We don't want it in the blood. Yes. 00:04:07.23\00:04:08.72 So if it's in the blood that means 00:04:08.75\00:04:10.12 it's not getting into the cells 00:04:10.15\00:04:11.52 and there's a reason why, we're gonna talk about later. 00:04:11.55\00:04:13.67 So if it's not getting in the cells, that's a problem. 00:04:13.70\00:04:16.77 So prolonged exposure to high blood sugar 00:04:16.80\00:04:20.23 and high insulin damages the lining of the blood vessels. 00:04:20.26\00:04:23.53 So if it's over 200, or if it's a fasting, 00:04:23.56\00:04:25.82 if it's greater than 125 twice, now in other countries, 00:04:25.85\00:04:29.90 it's different, I think it's 6 millimoles around the world. 00:04:29.93\00:04:33.56 But if you had those blood sugars, 00:04:33.59\00:04:34.96 then that means that you probably are a diabetic. 00:04:34.99\00:04:37.25 If you get it later in life, 00:04:37.28\00:04:38.65 you will be classified a type 2 diabetic. 00:04:38.68\00:04:42.16 There's another way we measured it 00:04:42.19\00:04:44.13 is we take something called hemoglobin A1c. 00:04:44.16\00:04:47.77 We call that glycosylated hemoglobin. 00:04:47.80\00:04:50.12 Hemoglobin's iron molecule, glycosylated means 00:04:50.15\00:04:52.98 there's glucose molecules tied on to that. 00:04:53.01\00:04:55.32 It won't make sense if hemoglobin is floating 00:04:55.35\00:04:57.10 around the blood and a bunch of sugar jumps on board 00:04:57.13\00:04:59.63 that number's gonna go higher 00:04:59.66\00:05:01.03 'cause we don't want a lot of sugar in the blood. 00:05:01.06\00:05:02.83 Okay, so if that number is greater than 7.5% 00:05:02.86\00:05:05.89 of the hemoglobin, has these extra sugars on it, 00:05:05.92\00:05:08.44 you're a diabetic too. 00:05:08.47\00:05:10.66 Another way, an old fashion way, 00:05:10.69\00:05:12.58 was this glucose tolerance test 00:05:12.61\00:05:14.78 and I don't know if you heard of that. 00:05:14.81\00:05:16.18 We used to drink a lot of cola 00:05:16.21\00:05:18.63 where they had a lot of sugar in it 00:05:18.66\00:05:20.03 and we used to check your blood sugar at certain hours 00:05:20.06\00:05:22.50 and if the blood sugar stayed up 00:05:22.53\00:05:23.90 above a certain level, you had diabetes. 00:05:23.93\00:05:26.85 So that's how we make the diagnosis of diabetes. 00:05:26.88\00:05:29.57 And, unfortunately, in this world, 00:05:29.60\00:05:30.99 most people have type 2 diabetes. 00:05:31.02\00:05:34.69 Why it's such a big problem is 'cause, 00:05:34.72\00:05:36.36 as you know, when we're prolonged exposed 00:05:36.39\00:05:38.95 in our body or bloodstream 00:05:38.98\00:05:40.35 to high levels of insulin and glucose, 00:05:40.38\00:05:42.37 there's metabolic abnormalities that become. 00:05:42.40\00:05:45.02 Now it's okay in the short term if you have it, 00:05:45.05\00:05:47.17 but in the long term, day in and day after, 00:05:47.20\00:05:49.38 we get damage to our organ systems. 00:05:49.41\00:05:52.44 Damage, you know, to the organ systems. 00:05:52.47\00:05:53.84 Everyone's, you know, let's start from the top down. 00:05:53.87\00:05:57.13 One of the worst ones we have is damages 00:05:57.16\00:05:59.23 to the small blood vessels in the eyes. 00:05:59.26\00:06:01.30 Have you heard of that? 00:06:01.33\00:06:02.70 Yeah, well, they call it retinopathy, 00:06:02.73\00:06:04.32 the eyes--the blood vessels of the eye damage. 00:06:04.35\00:06:06.59 The doctor will look in your eyes 00:06:06.62\00:06:07.99 and he can see characteristic changes. 00:06:08.02\00:06:09.93 It's usually in the small blood vessels. 00:06:09.96\00:06:12.31 In fact, the leading cause of blindness 00:06:12.34\00:06:15.77 in the world is due to diabetes. 00:06:15.80\00:06:19.34 And it only makes sense as the diabetes rate goes up, 00:06:19.37\00:06:21.96 there's gonna be more, more people with blindness. 00:06:21.99\00:06:24.34 The body wasn't made to deal with these chemicals 00:06:24.37\00:06:26.63 over a long period of time. 00:06:26.66\00:06:28.59 The second, another complication is 00:06:28.62\00:06:31.31 it hurts the long nerves in the body, 00:06:31.34\00:06:33.24 especially your feet. 00:06:33.27\00:06:34.72 So you always hear about diabetics 00:06:34.75\00:06:36.32 checking their feet to see if there's sores. 00:06:36.35\00:06:38.51 Because you can have a condition called neuropathy. 00:06:38.54\00:06:41.48 The nerves don't function well. 00:06:41.51\00:06:42.89 So you actually don't feel your feet. 00:06:42.92\00:06:45.02 So you can be walking. 00:06:45.05\00:06:46.42 Let's say you're walking on a hike 00:06:46.45\00:06:47.82 and get a rock in your shoe 00:06:47.85\00:06:49.78 and it can actually bore in your foot 00:06:49.81\00:06:51.81 and it can cause bleeding and you won't even feel it. 00:06:51.84\00:06:54.92 This is a sad disease. 00:06:54.95\00:06:56.32 Yes, it is a sad disease. 00:06:56.35\00:06:57.83 So that's another thing, it can cause neuropathy 00:06:57.86\00:07:00.41 you don't feel and sometimes 00:07:00.44\00:07:01.81 even in the hands because those are long nerves 00:07:01.84\00:07:03.96 and it has to do with the blood supply, 00:07:03.99\00:07:05.94 the insulin, the same things we've been talking about. 00:07:05.97\00:07:08.32 But sometimes also you can get infections 00:07:08.35\00:07:10.64 that you don't even know about. 00:07:10.67\00:07:12.38 The immune system is actually damaged by diabetics. 00:07:12.41\00:07:15.61 Diabetics are much more prone to infection 00:07:15.64\00:07:18.33 because these high levels of this abnormal 00:07:18.36\00:07:20.96 glucose metabolism damages our killer cells, 00:07:20.99\00:07:24.27 the white blood cell count. 00:07:24.30\00:07:25.67 So we're more prone to infections 00:07:25.70\00:07:27.20 and everyone hears about diabetics, 00:07:27.23\00:07:28.69 you know, you're more prone 00:07:28.72\00:07:30.09 to getting this infection or that infection. 00:07:30.12\00:07:32.98 So other organ systems, of course, the heart. 00:07:33.01\00:07:35.91 You know, 80% of diabetics die 00:07:35.94\00:07:38.64 from a heart attack, heart disease 80%. 00:07:38.67\00:07:41.61 It damages not only the blood vessels in the eyes, 00:07:41.64\00:07:44.30 but it damages the blood vessels in the heart. 00:07:44.33\00:07:47.06 It causes a lot of problems 00:07:47.09\00:07:48.46 within the heart so the heart disease. 00:07:48.49\00:07:49.86 It also damages the blood vessels in the legs, 00:07:49.89\00:07:52.09 also damages the blood vessels in the brain. 00:07:52.12\00:07:54.21 So stroke rates, peripheral vascular, 00:07:54.24\00:07:56.63 all that increases in diabetics. 00:07:56.66\00:07:58.49 It's not too uncommon to hear diabetics 00:07:58.52\00:08:01.48 getting amputations because of this damage long term. 00:08:01.51\00:08:05.36 One of the most common organs it damages is kidneys. 00:08:05.39\00:08:08.40 Kidneys are damaged from diabetes. 00:08:08.43\00:08:10.85 In fact, the number one cause, the number one cause 00:08:10.88\00:08:14.09 of dialysis is long-term diabetes effects on the kidneys. 00:08:14.12\00:08:19.22 So it involves major organ system. 00:08:19.25\00:08:21.30 Now if this doesn't get it, 00:08:21.33\00:08:22.70 you know, I'm talking to people that had these diagnoses 00:08:22.73\00:08:25.36 and say listen, it damages your entire body 00:08:25.39\00:08:27.97 through this abnormal glucose metabolism, 00:08:28.00\00:08:30.58 but what's even worse, 00:08:30.61\00:08:32.14 you know, some people say listen, 00:08:32.17\00:08:33.54 you know, when you talk to a man 00:08:33.57\00:08:35.58 about the big one the impotence, 00:08:35.61\00:08:37.66 you know, it can cause impotence 00:08:37.69\00:08:39.67 and this is a major issue in men. 00:08:39.70\00:08:41.07 I say, listen, the diabetes can cause impotence. 00:08:41.10\00:08:43.44 And that all of a sudden gets our attention. 00:08:43.47\00:08:45.45 And I say, "Listen, I don't want 00:08:45.48\00:08:47.46 any more end-organ damage here, 00:08:47.49\00:08:50.13 but left unchecked, diabetes damages the body." 00:08:50.16\00:08:54.43 In fact, a study was done not too long ago 00:08:54.46\00:08:57.33 about treatments for diabetes and they found out that, 00:08:57.36\00:09:00.06 you know, even if your blood sugar 00:09:00.09\00:09:01.46 is under good control in a diabetic person, 00:09:01.49\00:09:04.94 you still lose about 10 years of life. 00:09:04.97\00:09:08.61 So you want to cure type 2 diabetes 00:09:08.64\00:09:11.61 and in our next program, we're gonna talk 00:09:11.64\00:09:13.15 about specific ways to cure it, 00:09:13.18\00:09:14.64 but right now we want to focus on all the bad things 00:09:14.67\00:09:16.91 that happen in diabetes. 00:09:16.94\00:09:19.52 I was just remembering a case not too long ago 00:09:19.55\00:09:23.04 where a young girl with type 1 diabetes, 00:09:23.07\00:09:25.29 insulin-dependent, came to the emergency room 00:09:25.32\00:09:28.92 and what happens in diabetes, Charles, 00:09:28.95\00:09:30.87 is any type of stress on the body 00:09:30.90\00:09:33.50 because it's gonna-- you know, 00:09:33.53\00:09:34.90 when you fight or flight guess 00:09:34.93\00:09:36.30 what happens to your blood sugar? 00:09:36.33\00:09:37.70 It goes up even more, okay, 00:09:37.73\00:09:39.81 and when it goes up even more guess what happens? 00:09:39.84\00:09:42.18 They don't have insulin. 00:09:42.21\00:09:43.58 So they can't keep up with all this blood sugar. 00:09:43.61\00:09:45.02 So they literally don't get blood sugar 00:09:45.05\00:09:47.91 to use into their cell for energy. 00:09:47.94\00:09:49.95 So the stress they have to breakdown fats 00:09:49.98\00:09:52.48 and other things for energy. 00:09:52.51\00:09:54.08 And when they breakdown a lot of fats, 00:09:54.11\00:09:55.52 it makes a lot of acid production. 00:09:55.55\00:09:57.82 And when the acid starts being producing, 00:09:57.85\00:09:59.74 it damages the body. 00:09:59.77\00:10:01.14 We call it diabetic ketoacidosis. 00:10:01.17\00:10:03.52 Ketones are formed, become acids, 00:10:03.55\00:10:05.43 a very dangerous situation. 00:10:05.46\00:10:07.14 So we start putting insulin into the body 00:10:07.17\00:10:09.04 so we can get some blood sugar into the cells. 00:10:09.07\00:10:11.38 We try to figure out what triggered it. 00:10:11.41\00:10:13.09 And, frequently, in this young girl, 00:10:13.12\00:10:15.09 even though she was in her early 30s, 00:10:15.12\00:10:17.15 what triggered her event was a heart attack. 00:10:17.18\00:10:19.96 The heart attack caused the stress. 00:10:19.99\00:10:22.11 The stress made her to go in diabetic ketoacidosis. 00:10:22.14\00:10:24.85 And, unfortunately, in diabetics, 00:10:24.88\00:10:26.92 the blood vessels of the heart tend to be very small 00:10:26.95\00:10:29.52 because of the damage that's done by this terrible disease. 00:10:29.55\00:10:33.25 And there wasn't a lot we could do to help her heart. 00:10:33.28\00:10:35.73 So this is a devastating disease, Charles, 00:10:35.76\00:10:38.14 that effects it, and one thing 00:10:38.17\00:10:39.54 we want to let all people know is, 00:10:39.57\00:10:41.28 you know, if you're having, you know, high blood sugar, 00:10:41.32\00:10:44.11 it needs to be screened for. 00:10:44.14\00:10:45.58 And especially if you're having symptoms, 00:10:45.61\00:10:47.20 if you're thirsty, going to the bathroom all the time, 00:10:47.23\00:10:50.59 carry some extra weight, 00:10:50.62\00:10:52.13 feeling a little bit fatigued and tired, 00:10:52.16\00:10:54.91 you need to get checked by one of these ways to see 00:10:54.94\00:10:57.01 if you have diabetes because there's lots of things 00:10:57.04\00:10:59.56 we can do to cure diabetes at least type 2 diabetes. 00:10:59.59\00:11:03.84 Type 1, we can't really cure that, 00:11:03.87\00:11:06.20 but type 2, we can cure that doing a lot of things for it. 00:11:06.23\00:11:09.46 You know, when you give that description, 00:11:09.49\00:11:13.48 you would think that this would be something 00:11:13.51\00:11:16.97 that everyone would work really hard 00:11:17.00\00:11:20.05 and do whatever necessary to eradicate, 00:11:20.08\00:11:23.77 yet diabetes is on the rise 00:11:23.80\00:11:26.99 and becoming epidemic in this country. 00:11:27.02\00:11:29.49 What don't we know? 00:11:29.52\00:11:30.89 What message is not getting out there? 00:11:30.92\00:11:32.55 Yeah, and again, we've talked 00:11:32.58\00:11:34.20 about all of these diseases that are killing us, 00:11:34.23\00:11:36.87 we wanted to keep these two words. 00:11:36.90\00:11:38.64 What was the original plan in our lives 00:11:38.67\00:11:41.25 and what's the chronic exposure? 00:11:41.28\00:11:43.21 Long-term exposure. What's the long-term exposure? 00:11:43.24\00:11:45.88 And we're gonna find out as we talk more 00:11:45.91\00:11:47.92 the problem with type 2 diabetes is fat, too much fat, 00:11:47.95\00:11:54.25 too much fat in the diet, too much fat in you. 00:11:54.28\00:11:58.45 And when you have too much fat in you, 00:11:58.48\00:12:00.53 the fat goes into the cell. 00:12:00.56\00:12:02.72 When the fat goes into the cell, 00:12:02.75\00:12:04.20 it makes it much harder for the insulin receptors 00:12:04.23\00:12:07.37 and the blood sugar to go in the cell. 00:12:07.40\00:12:09.29 So it changes our entire metabolism. 00:12:09.32\00:12:11.57 This is a physical thing then. Yes, this is a-- 00:12:11.60\00:12:13.45 Physical presence of fat in the cell-- 00:12:13.48\00:12:15.94 Fat in the cells as well as fat in the body, 00:12:15.97\00:12:18.73 it changes all this. 00:12:18.76\00:12:20.13 And, you know, we've done studies 00:12:20.16\00:12:22.07 and we know if people can get fat out of their cells, 00:12:22.10\00:12:24.57 fat out of their diets, they can actually cure type 2 diabetes. 00:12:24.60\00:12:29.12 And we want to cure it 'cause as we're gonna find out 00:12:29.15\00:12:31.00 in our next program, the medicines that are involved 00:12:31.03\00:12:33.56 have some bad side effects. 00:12:33.59\00:12:35.58 So the problem with type 2 diabetes 00:12:35.61\00:12:37.57 the way I see it is it's a fat problem. 00:12:37.60\00:12:40.05 And, unfortunately, nowadays, 00:12:40.08\00:12:42.39 now follow me here 00:12:42.42\00:12:44.30 today just 1/3rd of people in the United States 00:12:44.33\00:12:46.93 are considered obese, 60% overweight. 00:12:46.96\00:12:50.88 Overweight usually carry extra fat. 00:12:50.91\00:12:53.54 Extra fat grows, you're gonna have abnormal insulin receptors, 00:12:53.57\00:12:56.64 abnormal insulin levels. 00:12:56.67\00:12:58.07 The glucose metabolism is gonna change. 00:12:58.10\00:13:00.05 We're not gonna be able to get glucose into the cell 00:13:00.08\00:13:01.98 because of all the fats. 00:13:02.01\00:13:03.38 We're gonna have this high blood sugar. 00:13:03.41\00:13:04.81 We're gonna see all this damage 00:13:04.84\00:13:06.71 that begins to occur in the body. 00:13:06.74\00:13:08.85 Again, it's a chronic exposure to fat 00:13:08.88\00:13:11.65 that we weren't really designed to have. 00:13:11.68\00:13:13.82 So just like we've talked about whether heart disease, 00:13:13.85\00:13:16.27 whether it be stress, 00:13:16.30\00:13:17.67 we talked about cancer exposures, 00:13:17.70\00:13:19.41 now we're putting our body, 00:13:19.44\00:13:20.89 we were never designed to eat a high-fat diet. 00:13:20.92\00:13:24.31 This has just occurred over years and years. 00:13:24.34\00:13:26.48 I heard one professional call this 00:13:26.51\00:13:28.40 a food borne illness diabetes-- Type 2. 00:13:28.43\00:13:31.37 Type 2. That's correct.
