COPD is "chronic obstructive pulmonary disease" 00:00:11.47\00:00:14.86 and it's one of the leading causes of death 00:00:14.89\00:00:17.43 still in our world... 00:00:17.46\00:00:18.83 And one of the leading causes we talked about last segment 00:00:18.86\00:00:22.12 is from cigarettes but how do I make the diagnosis? 00:00:22.15\00:00:24.93 Well #1, a person might have a symptom, 00:00:24.96\00:00:28.17 and that symptom might be shortness of breath 00:00:28.20\00:00:30.75 which makes sense - if their lungs are damaged. 00:00:30.78\00:00:33.48 They might be prone to infections; 00:00:33.51\00:00:35.52 they might get wheezing, you know... 00:00:35.55\00:00:38.42 ...they might breathe like that 00:00:38.45\00:00:40.83 Another thing, they might be tachypneic... 00:00:40.86\00:00:43.22 That's a fancy word to say you're breathing fast. 00:00:43.25\00:00:45.84 I might walk in and you're going ... and I'd say, 00:00:45.87\00:00:49.03 "Wow, they might have some breathing problems. " 00:00:49.06\00:00:50.93 And they just came from their car... 00:00:50.97\00:00:52.34 It's not like they climbed a mountain or anything. 00:00:52.37\00:00:53.99 And they might get short of breath real easy. 00:00:54.02\00:00:55.39 So that might be some of the symptoms that we have... 00:00:55.42\00:00:57.62 shortness of breath - so any person with 00:00:57.65\00:00:59.48 shortness of breath and is a smoker, 00:00:59.51\00:01:00.96 I automatically think... 00:01:00.99\00:01:02.36 "Do they have chronic obstructive pulmonary disease?" 00:01:02.39\00:01:04.00 How would I prove it? 00:01:04.03\00:01:05.40 Well one way I could prove it 00:01:05.43\00:01:06.80 is to see if their oxygen level were low. 00:01:06.83\00:01:09.07 And here we have a device called "a pulse oximeter," 00:01:09.10\00:01:12.56 and this measures the oxygen level in your blood, 00:01:12.59\00:01:15.95 and it should be greater than 92% 00:01:15.98\00:01:19.04 So if you would put your finger in there, Charles... 00:01:19.07\00:01:21.37 And we'll make sure the camera can see this 00:01:21.40\00:01:23.24 Aim it in that direction, okay my finger is in there. All right 00:01:23.27\00:01:26.62 I'm going to make sure the on-button is on. 00:01:26.65\00:01:29.11 So I turn the on-button on. 00:01:29.14\00:01:30.57 And that's the on-button over here... There we go 00:01:30.60\00:01:32.96 Oh, is it on? Okay 00:01:32.99\00:01:34.36 Yeah, now it's starting to pick up here. 00:01:34.39\00:01:38.15 Okay, I'm going to do this here... 00:01:38.18\00:01:40.77 And... there we go! I see a light. 00:01:40.80\00:01:46.43 Now we're measuring... There I am... Okay 00:01:46.46\00:01:48.57 So the bottom number is how much your heart is beating. 00:01:48.60\00:01:52.07 And now it's going to check the oxygen level in your blood, 00:01:52.10\00:01:54.53 and it's 97% when you're sitting here. 00:01:54.56\00:01:56.79 Is that good? That's VERY good! 00:01:56.82\00:01:58.87 So that would argue against 00:01:58.90\00:02:00.78 chronic obstructive pulmonary disease. 00:02:00.81\00:02:02.27 Your oxygen level is very high at rest, 00:02:02.30\00:02:04.37 and your heart rate is very good, Charles, 00:02:04.40\00:02:06.34 in the 80s, so that's very good. 00:02:06.37\00:02:07.92 Another way we might do this... 00:02:07.95\00:02:09.59 Okay, I'm going to turn this off here. 00:02:09.62\00:02:11.15 ...is I might get you behind an x-ray machine, 00:02:11.18\00:02:14.56 and then I might take pictures of you and I might get an x-ray. 00:02:14.59\00:02:17.34 Now we probably can't see this, 00:02:17.37\00:02:18.76 but this is your heart right here, 00:02:18.79\00:02:20.58 and these are the lungs, 00:02:20.61\00:02:22.56 and in people who have chronic obstructive pulmonary disease 00:02:22.59\00:02:25.20 their lungs are enlarged. 00:02:25.23\00:02:27.21 This actually would work if you just... there you go! 00:02:27.24\00:02:29.12 Yeah... There you go! Oh, there we go 00:02:29.15\00:02:30.80 They have hyperinflated lungs. 00:02:30.83\00:02:33.15 So an x-ray would give me a good feeling whether 00:02:33.18\00:02:35.76 a person had chronic obstructive pulmonary disease. 00:02:35.79\00:02:37.87 So this is an anterior-posterior view, 00:02:37.90\00:02:40.66 and this is another view where we see behind the heart. 00:02:40.69\00:02:45.43 So this is the lungs behind the heart, 00:02:45.46\00:02:47.57 and there are some characteristics 00:02:47.60\00:02:48.97 that we would see in the lungs... 00:02:49.00\00:02:50.48 The lungs are usually hyperexpanded. 00:02:50.51\00:02:52.65 I call them BIG lungs! 00:02:52.68\00:02:54.45 So if I had a low pulse-ox, 00:02:54.48\00:02:56.32 someone that was short of breath, 00:02:56.35\00:02:58.21 and had some big lungs, 00:02:58.24\00:02:59.73 I might say that they had 00:02:59.76\00:03:01.24 chronic obstructive pulmonary disease, 00:03:01.27\00:03:03.08 and might initiate treatment. 00:03:03.11\00:03:04.73 And the lungs get big because they're trying to work... 00:03:04.76\00:03:07.92 Well actually, the air can't move in and out. 00:03:07.95\00:03:10.31 So the air sacs, the alveoli, get BIGGER 00:03:10.34\00:03:13.52 because they have obstructions. 00:03:13.55\00:03:14.92 They can't get the air in and out as easily... I got ya! 00:03:14.95\00:03:16.87 Now another thing that we do 00:03:16.90\00:03:18.27 is we do a test called a "blow test" 00:03:18.30\00:03:21.22 pulmonary function test. 00:03:21.25\00:03:22.99 Now sometimes... I don't have one with me, 00:03:23.02\00:03:25.22 but we can do a cheap pulmonary function test. 00:03:25.25\00:03:27.58 And what I do is I have a person 00:03:27.61\00:03:29.07 hold out a piece of paper, a tissue, at arm's length, 00:03:29.10\00:03:32.51 and blow on it. 00:03:32.54\00:03:33.91 And in the office, someone with 00:03:33.94\00:03:35.31 chronic obstructive pulmonary disease - it will look like this 00:03:35.34\00:03:37.29 ...when they blow as hard as they can. 00:03:37.32\00:03:40.52 It won't look like this... 00:03:40.55\00:03:43.82 So, it will barely move when they blow at tissue. 00:03:43.85\00:03:46.25 So if they have all these things; 00:03:46.28\00:03:47.88 low pulse-ox, big lungs, they can't move this, 00:03:47.91\00:03:51.07 I might say that you have 00:03:51.10\00:03:52.47 chronic obstructive pulmonary disease. 00:03:52.50\00:03:54.45 But not only does chronic obstructive pulmonary disease 00:03:54.48\00:03:56.66 hurt your lungs, but it hurts your entire body... why is that? 00:03:56.69\00:03:59.88 ...Because you CAN'T CARRY OXYGEN around! 00:03:59.91\00:04:02.05 It can hurt the heart, the parts of the heart can work harder. 00:04:02.08\00:04:05.35 Because when you have low oxygen levels, 00:04:05.38\00:04:07.40 the blood vessels constrict, 00:04:07.43\00:04:09.18 and it makes the right side of the heart have to pump hard 00:04:09.21\00:04:12.05 through the blood vessels. 00:04:12.08\00:04:14.05 So chronic obstructive pulmonary disease is a 00:04:14.08\00:04:15.75 serious problem; it's pretty easy to diagnose. 00:04:15.78\00:04:18.55 But most people that do have it, 00:04:18.58\00:04:20.05 have some type of a smoking history. 00:04:20.08\00:04:22.54 Now people with COPD, would they also have 00:04:22.57\00:04:25.16 hypertension under these circumstances? 00:04:25.19\00:04:26.56 Well they could, they could... 00:04:26.59\00:04:27.98 They have high blood pressure in their lungs 00:04:28.01\00:04:30.28 because it's constricting, 00:04:30.31\00:04:31.74 but they also might have it peripherally 00:04:31.77\00:04:33.72 because of the nicotine. 00:04:33.75\00:04:35.54 And if they are a smoker, they might sure get... 00:04:35.57\00:04:37.06 Any time you're hypoxic, low oxygen, 00:04:37.09\00:04:39.89 can also constrict blood vessels. 00:04:39.92\00:04:42.43 ...But the body loves oxygen. 00:04:42.46\00:04:44.96 You are going to have to help me with this word here... 00:04:44.99\00:04:46.68 This is a question on heartwiseministries. org 00:04:46.71\00:04:50.35 "Should persons with COPD get a Pneumovax?" 00:04:50.38\00:04:55.61 It's like a Hoover? No, it's not a vacuum cleaner. 00:04:55.64\00:04:59.32 But you know, vaccinations - we hear about them all. 00:04:59.35\00:05:01.86 One of them is the Pneumovax, 00:05:01.89\00:05:03.30 and people that smoke cigarettes 00:05:03.33\00:05:05.49 and have chronic obstructive pulmonary disease, 00:05:05.52\00:05:07.45 vaccines are very controversial as you know... 00:05:07.48\00:05:10.16 Some people worry about them 00:05:10.19\00:05:11.58 causing immune reactions and reacting to them. 00:05:11.61\00:05:13.75 But we always look at the risk and benefits; 00:05:13.78\00:05:16.17 and someone that has bad lungs that can't get out secretions, 00:05:16.20\00:05:20.24 that have chronic obstructive pulmonary disease 00:05:20.27\00:05:22.02 with low oxygen, can you imagine IF they had an infection! Oh my! 00:05:22.05\00:05:25.88 If they had an infection, it could KILL them. 00:05:25.91\00:05:27.78 So in this instance, I would say, 00:05:27.81\00:05:29.75 a Pneumovax in a person with bad lungs, 00:05:29.78\00:05:31.93 the risk is worth the benefits on this situation... 00:05:31.96\00:05:36.56 So people with bad lungs usually benefit from having a Pneumovax. 00:05:36.59\00:05:39.68 Here's another question from the heartwiseministries. org website 00:05:39.71\00:05:42.70 "I have COPD - "chronic obstructive pulmonary disease" 00:05:42.73\00:05:46.41 and have frequent problems causing 00:05:46.44\00:05:49.18 4 hospitalizations in 2 years. 00:05:49.21\00:05:51.87 Do you have any suggestions to keep me out of the hospital?" 00:05:51.90\00:05:54.40 Now of course, I'm sure the first question you would ask is, 00:05:54.43\00:05:56.30 "Do you smoke?" Right 00:05:56.33\00:05:57.85 And that's the question, we want to get at the cause, 00:05:57.88\00:06:00.86 and if they are a smoker, then we want to do 00:06:00.89\00:06:03.97 everything we can to reverse that habit. 00:06:04.00\00:06:06.78 And we've already talked about 00:06:06.81\00:06:08.30 some of the things we can do earlier... 00:06:08.33\00:06:09.80 And we can pray about it, 00:06:09.83\00:06:11.20 we can replace a good habit with a bad habit; 00:06:11.23\00:06:14.01 we can love them; we can pick a smoking date; 00:06:14.04\00:06:15.87 get the power of the Holy Spirit. 00:06:15.90\00:06:17.27 I think you want to replace a bad habit with a GOOD habit. 00:06:17.30\00:06:19.05 That's a good idea, Charles! I messed that up! 00:06:19.08\00:06:21.38 And that is a bad habit to have... There you go! 00:06:21.41\00:06:24.81 Yes, that's one thing we want to do. 00:06:24.84\00:06:26.59 The second thing we want to do is 00:06:26.62\00:06:27.99 make sure they have enough oxygen. 00:06:28.02\00:06:30.44 Some times they have to wear oxygen. 00:06:30.47\00:06:32.12 If they're oxygen-deprived, that can make the body weaker. 00:06:32.15\00:06:35.23 So if they need oxygen, we want to give them 00:06:35.26\00:06:36.95 supplemental oxygen especially at night. 00:06:36.98\00:06:39.78 We want too make sure they don't have sleep apnea. 00:06:39.81\00:06:42.23 We want to give them oxygen at night. 00:06:42.26\00:06:43.63 Another thing we want to advise them to do is have the 00:06:43.66\00:06:45.58 vaccination so they don't get infections. 00:06:45.61\00:06:47.96 Another thing that I encourage people is to start on a 00:06:47.99\00:06:50.43 walking program... because the better the blood vessels are... 00:06:50.46\00:06:53.83 the better the circulation is, 00:06:53.86\00:06:55.23 the better you can move what oxygen you HAVE 00:06:55.26\00:06:57.71 around their body. 00:06:57.74\00:06:59.72 And then lastly, we say... 00:06:59.75\00:07:01.12 "Well let's try to do things that would hurt the lungs" 00:07:01.15\00:07:03.85 And things that would hurt the lungs would be 00:07:03.88\00:07:05.32 being around people that are infected. 00:07:05.35\00:07:07.53 Being outside in some allergenic situations... 00:07:07.56\00:07:10.75 You know, they might need to wear a mask. 00:07:10.78\00:07:12.61 They might have to be careful around fragrances. 00:07:12.64\00:07:15.55 Some people have noxious fumes which makes 00:07:15.58\00:07:18.21 already lungs that are compromised even worse 00:07:18.24\00:07:21.07 more prone to have problems. 00:07:21.10\00:07:23.10 Those things might help keep a person out of the hospital. 00:07:23.13\00:07:26.24 I've seen simple things make the biggest difference. 00:07:26.27\00:07:28.80 Now if they don't do anything, 00:07:28.83\00:07:30.29 and their lungs are BAD, GUESS WHAT... 00:07:30.32\00:07:32.37 they're not going to get better! 00:07:32.40\00:07:34.16 They're going to STAY bad... 00:07:34.19\00:07:35.56 So they get around someone that's sick, 00:07:35.59\00:07:36.96 they can't get rid of the infection... 00:07:36.99\00:07:38.36 BOOM! They're back in! 00:07:38.39\00:07:39.76 IV antibiotics, ventilator, if they can't quit smoking, 00:07:39.79\00:07:42.32 the lungs aren't going to get better... 00:07:42.35\00:07:43.88 they're going to get worse! 00:07:43.91\00:07:45.28 They're going to get MORE short of breath. 00:07:45.31\00:07:46.70 We'll have to turn up the oxygen higher and higher! 00:07:46.73\00:07:48.77 It's one of the saddest things I've ever seen... 00:07:48.80\00:07:51.00 seeing someone slowly die from 00:07:51.03\00:07:54.79 chronic obstructive pulmonary disease, 00:07:54.82\00:07:56.40 and yet it is still a problem in America. 00:07:56.43\00:07:58.52 That's why we have to reach out to our young ones out there, 00:07:58.55\00:08:01.12 and if you are a young person that's thinking about smoking 00:08:01.15\00:08:04.02 or even smoking... DON'T DO IT 00:08:04.05\00:08:06.62 It's one of the most addicting habits you can ever have, 00:08:06.65\00:08:09.15 and it can literally destroy your life, 00:08:09.18\00:08:11.32 and if you live with some else, 00:08:11.35\00:08:13.00 you can actually destroy their life. 00:08:13.03\00:08:14.84 And that's why when they stopped smoking in restaurants, 00:08:14.87\00:08:18.61 people saying... "Listen you're poisoning ME, I don't WANT IT" 00:08:18.64\00:08:21.46 You know, it was about someone else's rights too... 00:08:21.49\00:08:23.86 And sometimes when you just say that to a smoker, 00:08:23.89\00:08:26.15 it helps give them more power to quit smoking. 00:08:26.18\00:08:28.80 I'm going to ask you about the environment itself, 00:08:28.83\00:08:31.75 I mean, we live in a pretty toxic world... 00:08:31.78\00:08:34.06 We have the diesel fumes and the car fumes, 00:08:34.09\00:08:37.04 and the airplane fumes and everything... 00:08:37.07\00:08:39.02 Can you get this COPD simply by living in Los Angeles 00:08:39.05\00:08:45.30 when the smog was there? 00:08:45.33\00:08:46.87 Well you know, cigarettes is just one chemical 00:08:46.90\00:08:49.99 that can damage the lungs. 00:08:50.02\00:08:51.39 There are other things that cause 00:08:51.42\00:08:52.79 chronic obstructive pulmonary disease too. 00:08:52.82\00:08:54.95 Any type of long chemical exposures to the lungs 00:08:54.98\00:08:57.38 could cause it... damage the lungs. 00:08:57.41\00:08:59.52 There's a genetic condition called, 00:08:59.55\00:09:01.46 "alpha antitrypsin-1 deficiency" 00:09:01.49\00:09:03.83 which can damage the lungs as well. 00:09:03.86\00:09:05.90 So there are other things that can cause the lungs 00:09:05.93\00:09:08.21 to be obstructed besides cigarettes, 00:09:08.24\00:09:11.27 but that's just one of the most common ones. 00:09:11.30\00:09:13.06 That's why we talk about it the most, 00:09:13.09\00:09:16.12 and I don't know if this is true 00:09:16.15\00:09:17.66 but I heard that the Marlboro man died from lung cancer. 00:09:17.69\00:09:22.43 I don't know if that's true, 00:09:22.46\00:09:23.83 but I thought I heard that at one point. 00:09:23.86\00:09:25.46 You know, I want our listeners and our viewers to have 00:09:25.49\00:09:29.42 some idea what they can do to protect themselves... 00:09:29.45\00:09:32.94 I mean, if they stop smoking, that's fine, that's good... 00:09:32.97\00:09:35.76 If someone in the family smokes, what do you do? 00:09:35.79\00:09:41.41 Do you move out? 00:09:41.44\00:09:42.81 Do you tell that person to go away? 00:09:42.84\00:09:44.21 Do you get an air cleaner for your home? 00:09:44.24\00:09:47.22 What do you DO to help in these environments? 00:09:47.25\00:09:50.48 Yeah, well we frequently have that... 00:09:50.51\00:09:52.54 I'll come in and someone has had a heart attack; 00:09:52.57\00:09:55.67 the other one has not had a heart attack, 00:09:55.70\00:09:57.61 but continues to smoke... 00:09:57.64\00:09:59.27 So they continue to give them that risk. 00:09:59.30\00:10:01.58 So I said, "Listen, you love this person, 00:10:01.61\00:10:04.19 you want to try to help, try to smoke OUTSIDE." 00:10:04.22\00:10:06.78 "Try to leave the house well-ventilated, 00:10:06.81\00:10:08.98 so at least the smoke can leave the house. " 00:10:09.01\00:10:11.04 But you've smelled how smoke gets on the clothes; 00:10:11.07\00:10:14.32 it permeates every pore of your body. 00:10:14.35\00:10:16.70 I could smell the smoker 10 feet away. 00:10:16.73\00:10:19.78 We encourage to get outside, 00:10:19.81\00:10:22.12 try to get the smoker to move outside. 00:10:22.15\00:10:24.32 That's sort of the best way to work in the short-term, 00:10:24.35\00:10:27.00 but in the long-term, we want them, 00:10:27.03\00:10:28.63 you know - because they love their spouse so much, 00:10:28.66\00:10:30.96 to gradually quit smoking as well. Very good 00:10:30.99\00:10:34.58 I guess we can say that if you 00:10:34.61\00:10:36.10 don't do it for yourself, do it for other people. 00:10:36.14\00:10:38.87 But you know Charles, a lot of people don't realize 00:10:38.90\00:10:40.55 that chronic obstructive pulmonary disease... 00:10:40.58\00:10:42.37 it still kills many other people in the world today. 00:10:42.41\00:10:45.35 It's still killing people today after all these years. 00:10:45.38\00:10:49.17 We'll take a short break and when we come back, 00:10:49.20\00:10:51.17 a conversation with the Master Physician...
