Welcome back to the program. 00:00:01.40\00:00:02.77 I'm Charles Mills here with Dr. James Marcum 00:00:02.80\00:00:04.49 and we're talking about the causes of death 00:00:04.52\00:00:07.48 in this world and one of the major causes, 00:00:07.51\00:00:10.36 of course, is heart disease. 00:00:10.39\00:00:11.76 And we're talking about stroke here. 00:00:11.79\00:00:13.93 Make the connection for us, Dr. Marcum, 00:00:13.96\00:00:15.91 between heart disease and stroke. 00:00:15.94\00:00:19.03 How are they connected? 00:00:19.06\00:00:20.43 Well, you know, they're connected 00:00:20.46\00:00:21.83 'cause the heart pumps blood and oxygen 00:00:21.86\00:00:23.58 and nutrients to everything. Right. 00:00:23.61\00:00:25.70 So if the heart doesn't work good, 00:00:25.73\00:00:27.25 of course, the brain is not gonna work good 00:00:27.28\00:00:29.20 but the same process is having in the arteries 00:00:29.23\00:00:31.61 of the heart can also happen 00:00:31.64\00:00:33.01 and the arteries of brain requires cholesterol. 00:00:33.04\00:00:35.64 In addition, blood clots 00:00:35.67\00:00:37.04 which can originate in the heart. 00:00:37.07\00:00:38.68 It can be pumped to the brain 00:00:38.71\00:00:40.76 which causes these tubes to be clogged up. 00:00:40.79\00:00:44.31 And, really, I want to let our audience know 00:00:44.34\00:00:46.55 the stroke is a medical emergency 00:00:46.58\00:00:48.65 just like heart attack. 00:00:48.68\00:00:50.05 I mean, if you're not moving your arms, 00:00:50.08\00:00:51.76 if you're not thinking right, 00:00:51.79\00:00:53.16 if you're not speaking right, 00:00:53.19\00:00:54.74 severe headache, if you can't see, 00:00:54.77\00:00:56.73 this is a medical emergency 00:00:56.76\00:00:58.14 and you want to get help right away 00:00:58.17\00:01:00.30 'cause if we don't get help you could be devastated. 00:01:00.33\00:01:03.69 It could be something that damages your body. 00:01:03.72\00:01:06.14 You might not ever get it back. 00:01:06.17\00:01:07.85 So stroke is a medical emergency. 00:01:07.88\00:01:09.86 Get help right away and it's just so scary 00:01:09.89\00:01:13.50 that people are out there that have had stroke 00:01:13.53\00:01:15.46 and what they've had just live through and suffer through. 00:01:15.49\00:01:17.64 And unfortunately, our treatments for stroke 00:01:17.67\00:01:19.68 aren't as advanced as they are for the heart. 00:01:19.71\00:01:22.30 The blood vessels are smaller. 00:01:22.33\00:01:23.95 The brain's a little bit trickier to navigate. 00:01:23.98\00:01:26.33 So it's so important that if you have 00:01:26.36\00:01:28.01 a family history of stroke 00:01:28.04\00:01:29.65 or if you have risk factors of stroke 00:01:29.68\00:01:31.87 that you address it early 00:01:31.90\00:01:33.82 before you run into the problem with these symptoms. 00:01:33.85\00:01:36.55 All right, well, let's get to some questions 00:01:36.58\00:01:38.28 from the Heartwiseminstries.org website. 00:01:38.31\00:01:40.98 Here is the first one on this subject. 00:01:41.01\00:01:42.43 Can atrial fibrillation cause a stroke? 00:01:42.46\00:01:44.69 Yes, it can. 00:01:44.72\00:01:46.09 And we talked a little bit about 00:01:46.12\00:01:47.49 how the blood circulates the body. 00:01:47.52\00:01:49.95 Let's talk about that. 00:01:49.98\00:01:51.35 If the top part of the heart is not squeezing well 00:01:51.38\00:01:54.03 it could get a blood clot. 00:01:54.06\00:01:55.43 And we see that in atrial fibrillation. 00:01:55.46\00:01:57.25 That blood clot can be pumped 00:01:57.28\00:01:58.65 to other places including the head. Okay. 00:01:58.68\00:02:01.47 So it's been estimated that almost 15% of all strokes 00:02:01.50\00:02:05.38 come from atrial fibrillation. 00:02:05.41\00:02:07.20 Atrial fibrillation is a very common abnormality. 00:02:07.23\00:02:10.38 Some people don't even know they have it. 00:02:10.41\00:02:12.35 That's why we always talk about blood thinners 00:02:12.38\00:02:14.76 and people that have atrial fibrillation 00:02:14.79\00:02:16.54 to lower the risk of getting a blood clot. 00:02:16.57\00:02:18.81 Also people that have weak hearts, Charles. 00:02:18.84\00:02:21.32 If you're heart's weak sometimes 00:02:21.35\00:02:22.72 the bottom doesn't squeeze very good. 00:02:22.75\00:02:24.95 If the blood doesn't move sometimes you can get a clot 00:02:24.98\00:02:27.22 from a weak heart that can pump into the brain. 00:02:27.25\00:02:30.11 Now there's another condition. 00:02:30.14\00:02:31.99 Sometimes you have blood clots in your legs. 00:02:32.02\00:02:34.84 And if you have blood clots in your legs 00:02:34.87\00:02:36.42 usually those clots go to the lungs. 00:02:36.45\00:02:38.26 We called that a pulmonary embolism. 00:02:38.29\00:02:40.71 But in some instances there could be 00:02:40.74\00:02:42.16 a hole in the heart. 00:02:42.19\00:02:43.56 And a blood clot could cross to that hole 00:02:43.59\00:02:45.44 and then pump up to the brain, too. 00:02:45.47\00:02:47.64 So, you know, there is a condition called 00:02:47.67\00:02:49.29 an atrial septal defect where there is a hole 00:02:49.32\00:02:51.72 in one of the chambers of the heart 00:02:51.75\00:02:53.45 where actually clot from leg could pass to that. 00:02:53.48\00:02:55.93 So blood clots could pump into the brain. 00:02:55.96\00:02:58.56 That could cause a stroke 00:02:58.59\00:02:59.96 because once it gets to a small blood vessel, guess what? 00:02:59.99\00:03:02.54 No blood can get through that part of the brain 00:03:02.57\00:03:04.99 that that blood vessel nourishes dies. 00:03:05.02\00:03:07.75 If it's your talking part, all of a sudden you can't talk. 00:03:07.78\00:03:10.98 If it's your seeing part, all of a sudden you can't see. 00:03:11.01\00:03:13.49 If it's your moving part, 00:03:13.52\00:03:14.89 you might have the left or the right side go numb. 00:03:14.92\00:03:16.88 Then all of a sudden you have a medical emergency. 00:03:16.91\00:03:19.66 All right, this really underlines 00:03:19.69\00:03:21.28 the fact that we should be moving. 00:03:21.31\00:03:22.85 That we should have an exercise program 00:03:22.88\00:03:24.33 because you're saying that when we are lazy, 00:03:24.36\00:03:27.37 when we are sedentary and our heart 00:03:27.40\00:03:29.54 is just slowly pumping 00:03:29.57\00:03:31.15 and everything that this pulling of the blood, 00:03:31.18\00:03:33.56 this clotting could happen? 00:03:33.59\00:03:36.12 Well, mainly if the heart's damaged, Charles. 00:03:36.15\00:03:38.34 Now if the heart's damaged and weak 00:03:38.37\00:03:39.82 but, you know, if you're laying on a couch 00:03:39.85\00:03:41.86 and your heart's strong, 00:03:41.89\00:03:43.26 you're not that that big of a risk. 00:03:43.29\00:03:44.66 If you lay there for weeks and weeks 00:03:44.69\00:03:46.06 and months and months? Right, right. 00:03:46.09\00:03:47.46 But what we want to do, though, is if you lay around 00:03:47.49\00:03:48.86 and don't do anything what happens is 00:03:48.89\00:03:50.51 you might have high blood pressure 00:03:50.54\00:03:52.00 'cause you're gaining weight. You might get diabetes. 00:03:52.03\00:03:53.88 You might have high cholesterol. 00:03:53.91\00:03:55.28 Those are the things-- those risk factors 00:03:55.31\00:03:57.17 that can raise a chance of you having a stroke. 00:03:57.20\00:03:59.46 And since our treatment for stroke is not very good 00:03:59.49\00:04:02.40 and since it's a leading cause of death 00:04:02.43\00:04:04.45 we've got to do everything we can right now 00:04:04.48\00:04:06.69 wherever you're at everything you can 00:04:06.72\00:04:08.68 to lower the risk of stroke. 00:04:08.71\00:04:10.39 And if you're having a stroke get in right away. 00:04:10.42\00:04:12.71 I can't tell you how many people I've seen through the years. 00:04:12.74\00:04:15.05 Oh, it's nothing serious, you know? I can't move. 00:04:15.08\00:04:18.03 It's gonna go away or you know, it will go away. 00:04:18.06\00:04:21.20 I don't wanna wake up my wife 00:04:21.23\00:04:23.10 and then the next thing 00:04:23.13\00:04:24.50 they can't even move part of their body. 00:04:24.53\00:04:25.90 It's just devastating. 00:04:25.93\00:04:27.30 So atrial fibrillation is a cause of stroke. 00:04:27.33\00:04:29.64 All right. Question number two from Heartwiseministries.org. 00:04:29.67\00:04:33.44 What should be done for a family history of strokes? 00:04:33.47\00:04:36.94 Everyone in my family has had a stroke at some point. Wow. 00:04:36.97\00:04:39.55 Yeah, and it's just the things we've talked about. 00:04:39.58\00:04:41.57 You'd be concerned if someone said that. 00:04:41.60\00:04:42.97 I would be very concerned. 00:04:43.00\00:04:44.37 And I would tell them, you know, we got a-- 00:04:44.40\00:04:45.77 just like heart disease, we've got to lower the stresses 00:04:45.80\00:04:47.58 as we've got to lower the stress of high blood pressure. 00:04:47.61\00:04:50.10 If they carry extra weight we got to loose weight. 00:04:50.13\00:04:52.28 Control the diabetes. 00:04:52.31\00:04:54.15 Some people need a cholesterol medication. 00:04:54.18\00:04:56.66 Get in an exercise program 00:04:56.69\00:04:59.32 and then lower all the other stressors 00:04:59.35\00:05:01.35 that might have--sleep apnea is a stressor on the body. 00:05:01.38\00:05:04.32 So anything that raises blood pressure 00:05:04.35\00:05:06.02 and put stress is gonna raise a chance of having a stroke. 00:05:06.05\00:05:08.83 Very similar to what we tell someone with heart disease. 00:05:08.86\00:05:11.48 And we want to lower that but if you've have 00:05:11.51\00:05:12.88 a family history things tend to run in families. 00:05:12.91\00:05:15.43 And if these arteries are getting blocked up in the brain 00:05:15.46\00:05:18.29 you're gonna be at a higher risk of having a stroke. 00:05:18.32\00:05:20.50 You know, if everyone in the family has had a stroke 00:05:20.53\00:05:24.67 that can talk about genetics. 00:05:24.70\00:05:26.07 We could also talk about lifestyle. 00:05:26.10\00:05:27.59 Yes. Maybe whole family had bad lifestyle, you know? 00:05:27.62\00:05:30.11 Maybe, everyone smokes cigarettes 00:05:30.14\00:05:31.66 and had high blood pressure and weighed a lot 00:05:31.69\00:05:34.56 but these are things that, 00:05:34.59\00:05:35.96 you know, family history can be changed, you know? 00:05:35.99\00:05:37.99 We talked about bad genetics. 00:05:38.02\00:05:39.39 You can change your genetics 00:05:39.42\00:05:40.81 by the way you live sometimes. 00:05:40.84\00:05:42.21 Absolutely. Okay, next question from heartwiseministries.org. 00:05:42.24\00:05:46.84 Is it possible to regain function 00:05:46.87\00:05:50.37 and I'm sure that this person would like to know 00:05:50.40\00:05:52.23 if we can regain full function after a stroke? 00:05:52.26\00:05:54.84 After the clot has gotten there 00:05:54.87\00:05:56.64 and something has been damaged can you get it back? 00:05:56.67\00:05:59.91 Yeah, and that's why it's so important 00:05:59.94\00:06:02.04 to get treatment right away 00:06:02.07\00:06:03.93 if you think you've had a stroke. 00:06:03.96\00:06:05.76 If you can get into the hospital 00:06:05.79\00:06:07.21 and they can give a blood thinner within the, 00:06:07.24\00:06:09.04 you know, four or four and a half hours 00:06:09.07\00:06:10.59 or if they can go in and pull out the clot, 00:06:10.62\00:06:12.53 you're brain can be totally, completely better. 00:06:12.56\00:06:15.52 Now sometimes we have a stroke and the strokes there at home 00:06:15.55\00:06:18.71 and we can grow collateral blood vessels, okay? 00:06:18.74\00:06:21.61 What we can--you know, retrain the brain that's been damaged. 00:06:21.64\00:06:24.78 Sometimes that occurs and that's why its very important 00:06:24.81\00:06:27.40 especially the six months after they've had a stroke 00:06:27.43\00:06:31.06 to go through stroke rehab where they try to retrain 00:06:31.09\00:06:34.36 the brain to do things 00:06:34.39\00:06:35.76 depending on what part of brain has been damaged. 00:06:35.79\00:06:38.72 You might see a speech therapist. 00:06:38.75\00:06:40.63 You might see an occupational therapist, 00:06:40.66\00:06:42.78 other people to help retrain the brain. 00:06:42.81\00:06:44.83 Sometimes you have to start over. 00:06:44.86\00:06:46.58 And we've seen the greatest benefits 00:06:46.61\00:06:48.04 in the first six months. 00:06:48.07\00:06:49.54 However, some people, you know, it's like after a heart attack 00:06:49.57\00:06:52.64 I want people to start exercising, 00:06:52.67\00:06:54.28 doing things even if they've never done it before 00:06:54.31\00:06:56.47 to help the heart regain its function. 00:06:56.50\00:06:59.05 Some people after stroke they don't go 00:06:59.08\00:07:01.30 and get anything done. 00:07:01.33\00:07:02.70 They just say, oh, you've had a stroke. 00:07:02.73\00:07:04.10 Go home, live with it. 00:07:04.13\00:07:05.50 The doctor fixed it. You can go. 00:07:05.53\00:07:06.90 Yes, or they haven't done anything for it. 00:07:06.93\00:07:08.67 They've just had the stroke. 00:07:08.70\00:07:10.07 The part of the brain hasn't been tried to retrain. 00:07:10.10\00:07:12.24 So it's very important to get stroke rehab. 00:07:12.27\00:07:14.56 And they've shown that up to six months 00:07:14.59\00:07:16.00 you can retrain parts of the brain. 00:07:16.03\00:07:17.55 So even if you don't get it on time sometimes, 00:07:17.58\00:07:20.14 if you have the appropriate therapy 00:07:20.17\00:07:21.54 after the stroke it could be retrained. 00:07:21.57\00:07:23.81 Now unfortunately, if the brain is damaged 00:07:23.84\00:07:26.39 you have to learn things over again. 00:07:26.42\00:07:28.06 Like, what if you can't swallow well and, 00:07:28.09\00:07:30.28 you know, the food's getting into your lungs? 00:07:30.31\00:07:32.23 What if you can't move 00:07:32.26\00:07:33.63 and you're getting bedsores on you? 00:07:33.66\00:07:35.85 What if you're having seizures? 00:07:35.88\00:07:37.86 What if you can't do a lot of the things you knew? 00:07:37.89\00:07:39.69 What if you're eyes can't close 00:07:39.72\00:07:41.09 and you need drops in your eyes 00:07:41.12\00:07:42.49 just to keep them moist and help your vision? 00:07:42.52\00:07:44.49 So there is a lot of things that go along with the stroke 00:07:44.52\00:07:46.91 even if you can't get to retrain the brain 00:07:46.94\00:07:49.23 that must be done to care for a person with the stroke. 00:07:49.39\00:07:52.38 So it's very important that the caregivers 00:07:52.41\00:07:54.27 as well as the person themselves learn everything they can 00:07:54.30\00:07:56.74 not only to retrain the brain but the brain 00:07:56.77\00:07:58.99 can't retrain the brain doing the things 00:07:59.02\00:08:00.89 you do to function well. 00:08:00.92\00:08:02.62 One of the scariest things after stroke, Charles, 00:08:02.65\00:08:05.07 that I see is people can't swallow. Oh, dear. 00:08:05.10\00:08:07.26 And they have a condition called aspiration 00:08:07.29\00:08:09.81 because the muscles in the neck don't do well. 00:08:09.84\00:08:12.80 So they eat some food, instead of going to stomach 00:08:12.83\00:08:14.66 it goes around to the lungs. 00:08:14.69\00:08:16.34 And they get pneumonia and they have, 00:08:16.37\00:08:17.94 what we call, aspiration 'cause they can't swallow. 00:08:17.97\00:08:20.41 Some of those patients actually need feeding tubes 00:08:20.44\00:08:22.94 early on rather than going home and trying to swallow 00:08:22.97\00:08:25.89 and not being able to swallow 00:08:25.92\00:08:27.29 and all the food ends up in the lungs. 00:08:27.32\00:08:28.80 That's called aspiration. 00:08:28.83\00:08:30.62 So there's lots of things in stroke that can help. 00:08:30.65\00:08:33.21 So not only that you care something is very important 00:08:33.24\00:08:36.16 but after a stroke you also need a lot of care, 00:08:36.19\00:08:38.77 a lot of rehab, a lot of education, 00:08:38.80\00:08:40.81 both for the person as well as the family. 00:08:40.84\00:08:43.15 I had one person that I helped care for. 00:08:43.18\00:08:45.47 Everything is fine but she can't talk. 00:08:45.50\00:08:47.72 She can't talk. 00:08:47.75\00:08:49.12 You know, how hard it is to communicate 00:08:49.15\00:08:50.52 when you can't talk? Yes. 00:08:50.55\00:08:51.92 I mean, that's a major issue, you know? 00:08:51.95\00:08:53.33 So she's had to learn how to communicate 00:08:53.36\00:08:55.83 without being able to talk 00:08:55.86\00:08:57.40 and because one of her arms doesn't move well 00:08:57.43\00:08:59.59 she can't write as well. 00:08:59.62\00:09:01.10 So there's all sorts of issues 00:09:01.13\00:09:02.69 when parts of your brain doesn't work well. 00:09:02.72\00:09:05.94 We got another question, Dr. Marcum, here 00:09:05.97\00:09:07.65 from the Heartwiseministries.org website. 00:09:07.68\00:09:10.22 What is a thrombolytic and how is it used 00:09:10.25\00:09:14.16 in the acute treatment of stroke? Yeah. 00:09:14.19\00:09:16.32 Thrombolytic if we think that the cause of a stroke was 00:09:16.35\00:09:20.70 due to blood clot not a bleed 00:09:20.73\00:09:23.25 we give strong medications called thrombolytic, 00:09:23.28\00:09:26.36 thrombo meaning thrombos, lytic means break it apart 00:09:26.39\00:09:29.56 so they give this medicine in the body 00:09:29.59\00:09:32.28 and it goes and breaks up that clot 00:09:32.31\00:09:34.25 so all the sudden that brain gets enough blood to it 00:09:34.28\00:09:37.58 and this is the main extreme treatment 00:09:37.61\00:09:40.58 if it can be done very quickly. 00:09:40.61\00:09:42.43 There are some risk of thrombolytics 00:09:42.46\00:09:44.94 and the main risk of a thrombolytics guess what? 00:09:44.97\00:09:47.39 Its bleeding 'cause it causes you to bleed 00:09:47.42\00:09:49.36 so if you're bleeding already 00:09:49.39\00:09:50.88 I mean, you give a thrombolytic 00:09:50.91\00:09:52.28 you're gonna bleed more so that's why 00:09:52.31\00:09:53.68 they have to make sure what the cat scan 00:09:53.71\00:09:55.25 that there is no blood in the brain, 00:09:55.28\00:09:56.66 its not bleeding type of stroke before they give it 00:09:56.69\00:09:59.08 but thrombolytics before we have thrombolytics 00:09:59.11\00:10:01.12 it was very hard to treat a stroke at all 00:10:01.15\00:10:03.25 'cause we really didn't have anything to off it 00:10:03.28\00:10:04.65 but now we have that 00:10:04.68\00:10:06.05 for an acute stroke, we also have some devices 00:10:06.08\00:10:08.67 modern medicines as devices 00:10:08.70\00:10:10.07 that we can go up in the brain 00:10:10.10\00:10:11.47 and certain type of individuals we can treat that 00:10:11.50\00:10:13.51 but bottomline is people have to get to a stroke thinner 00:10:13.54\00:10:17.00 rather quick if they're having symptoms 00:10:17.03\00:10:19.21 to get this type of therapy. Oh, boy. 00:10:19.24\00:10:21.38 I'm just amaze at modern medicine 00:10:21.41\00:10:22.91 and I hope that listeners, 00:10:22.94\00:10:24.70 you are understanding that modern medicine has a place 00:10:24.73\00:10:28.57 and very important place but you know what, 00:10:28.60\00:10:30.67 Dr. Marcum, I wanna do everything I can-- 00:10:30.70\00:10:32.81 Do not needed. But not needed ever. 00:10:32.84\00:10:34.68 That's what I'm looking for. Okay. 00:10:34.71\00:10:36.08 Absolutely, Charles. 00:10:36.11\00:10:37.48 Our last question here we have time for this is a T.I.A. 00:10:37.51\00:10:40.42 the same thing as a stroke. What is that T.I.A.? 00:10:40.45\00:10:43.07 You know, T.I.A. they are different than the stroke 00:10:43.10\00:10:45.26 'cause the T.I.A. is called T transient ischemic attack 00:10:45.29\00:10:49.59 so it doesn't last as long, so its usually lesson 24 hours. 00:10:49.62\00:10:54.00 You might have a neurologic symptoms 00:10:54.03\00:10:55.64 then all the sudden the cause of it 00:10:55.67\00:10:57.04 gets better on its own spontaneously. 00:10:57.07\00:10:59.27 But you still had that symptom 00:10:59.30\00:11:01.01 we'll thank goodness it got better 00:11:01.04\00:11:02.70 but this could be a precursor for a stroke. 00:11:02.73\00:11:05.70 So people that have T.I.A. we wanna make sure 00:11:05.73\00:11:07.87 their risk factors are maintained 00:11:07.90\00:11:10.05 we gonna make sure there is no plots places 00:11:10.08\00:11:12.23 in the body that they could have a clot, 00:11:12.26\00:11:13.65 make sure they don't have 00:11:13.68\00:11:15.05 Atrial Fibrillation or weak heart, 00:11:15.08\00:11:16.45 make sure they don't have a whole in the heart, 00:11:16.48\00:11:18.16 we also wanna make sure that there is no blockages 00:11:18.19\00:11:20.93 in the arteries very important if you have T.I.A. 00:11:20.96\00:11:23.57 to get a major evaluation to make sure 00:11:23.60\00:11:25.75 there is nothing that can be done 00:11:25.78\00:11:27.15 'cause if you have a little stroke 00:11:27.18\00:11:28.55 the big one could be right around the corner. 00:11:28.58\00:11:30.56 Oh, Boy, okay. we'll take a short break now 00:11:30.59\00:11:33.18 and when we come back let's spent sometime 00:11:33.21\00:11:35.69 with the master physician.
