Sounds familiar. Yes. 00:02:18.08\00:02:20.52 And whenever it doesn't get the things 00:02:20.55\00:02:22.40 it needs to survive it doesn't do well. 00:02:22.43\00:02:25.71 When the brain doesn't get the nutrients it needs 00:02:25.74\00:02:28.23 both in oxygen and blood sugar a symptom develops. 00:02:28.26\00:02:31.74 And that symptom develops based on what part of the brain 00:02:31.77\00:02:34.74 is not getting enough blood. 00:02:34.77\00:02:37.24 And one of the acronyms that we use to talk about 00:02:37.27\00:02:40.08 stroke symptoms, okay, is this word called F.A.S.T. 00:02:40.11\00:02:44.77 F might stand for the facial. You might have a facial droop. 00:02:44.80\00:02:48.61 A stands for arms so we quickly have people 00:02:48.64\00:02:50.80 bring up their arms and if there one arm's going slower, 00:02:50.83\00:02:54.05 they're waggling a different way that might be sign 00:02:54.08\00:02:56.57 the part of the brain that controls the arm 00:02:56.60\00:02:58.95 is not getting enough of these nutrients. S is speech. 00:02:58.98\00:03:02.62 Frequently we have in stroke speech abnormalities. 00:03:02.65\00:03:05.95 Sometimes people might have eye problems. 00:03:05.98\00:03:08.30 People might have-- they can't move 00:03:08.33\00:03:10.21 the left or right side of their body. 00:03:10.24\00:03:12.26 These are symptoms of the stroke and the symptom that 00:03:12.29\00:03:14.66 that you get depends on where the stroke is occurring. 00:03:14.69\00:03:18.29 Stroke is devastating but if it can be addressed quickly 00:03:18.32\00:03:22.57 with blood flow sometimes a stroke can be totally 00:03:22.60\00:03:25.89 reversible and we have about three and a half to four hours 00:03:25.92\00:03:28.78 once the stroke happens to do something about it. 00:03:28.81\00:03:32.08 So if a person, Charles, if they think they might be 00:03:32.11\00:03:34.05 having a stroke and we'll talk about 00:03:34.08\00:03:35.90 the kinds of strokes in a moment. 00:03:35.93\00:03:37.74 If you'd like--if you think you might having this, 00:03:37.77\00:03:39.15 either you're not talking well, your vision's deranged, 00:03:39.18\00:03:43.08 you can't hold your arm straight, 00:03:43.11\00:03:44.61 part of it is numb or weak, you can't think right, 00:03:44.64\00:03:47.82 you need to get to the hospital right away 00:03:47.85\00:03:49.44 'cause time is of the essence just like in a heart attack. 00:03:49.47\00:03:52.15 We wanna bring the nutrients to the brain 00:03:52.18\00:03:54.19 so it starts working again. I had a person recently. 00:03:54.22\00:03:57.44 Her main symptom of a stroke was she could understand words 00:03:57.47\00:04:01.28 but she could not speak words. 00:04:01.31\00:04:04.38 You know, another person I saw recently, 00:04:04.41\00:04:05.97 they couldn't move the left side of their body 00:04:06.00\00:04:07.78 or they had a facial droop. 00:04:07.81\00:04:09.95 So when they go to the hospital the first thing 00:04:09.98\00:04:12.02 we wanna do is are you having a stroke? 00:04:12.05\00:04:14.90 Well, the brain is not working. Something's wrong. 00:04:14.93\00:04:17.16 So we take them to a C.A.T. scan and M.R.I. 00:04:17.19\00:04:19.95 and once we have that done 00:04:19.98\00:04:21.58 we decide what type of treatments 00:04:21.61\00:04:23.51 and luckily depending on what kind of stroke it is 00:04:23.54\00:04:26.51 we could initiate treatment. 00:04:26.54\00:04:28.33 Now there's different types of strokes, Charles. 00:04:28.36\00:04:30.50 One is a type of stroke where the-- 00:04:30.53\00:04:32.23 oh, you got your artery there 00:04:32.26\00:04:33.63 i do right there. i have my let me-we can show that-- 00:04:33.66\00:04:35.03 faithful artery right here. Yeah. 00:04:35.06\00:04:36.43 Sometimes, you know, we can have 00:04:36.46\00:04:37.83 an artery in the brain that spasms. 00:04:37.86\00:04:39.23 An artery is supposed to look like this. 00:04:39.26\00:04:40.65 It's supposed to be opened, right? Right. 00:04:40.68\00:04:42.39 It can spasm and things-- 00:04:42.42\00:04:44.71 a spasm can mimic a stroke 00:04:44.74\00:04:46.41 'cause the artery shuts down and opens up. 00:04:46.44\00:04:49.05 There's conditions, inflammatory conditions 00:04:49.08\00:04:51.40 where sometimes this happens. 00:04:51.43\00:04:54.03 Things that mimic stroke is low blood sugar. 00:04:54.06\00:04:56.76 If you have low blood sugar that might mimic a stroke. 00:04:56.79\00:04:59.14 If you weren't getting enough oxygen 00:04:59.17\00:05:00.66 that could mimic a stroke. 00:05:00.69\00:05:02.10 So we always check the blood sugar 00:05:02.13\00:05:03.67 and oxygen right away 00:05:03.70\00:05:05.07 in anyone that comes in with the brain not working well. 00:05:05.10\00:05:08.05 Another thing that can cause a stroke 00:05:08.08\00:05:09.54 is if you have a blood clot 00:05:09.57\00:05:11.35 that can come somewhere else through the body, okay? 00:05:11.38\00:05:14.16 And arteries in the brain can get blocked just like 00:05:14.19\00:05:16.55 the arteries of the heart. 00:05:16.58\00:05:17.95 They can get filled up with cholesterol. 00:05:17.98\00:05:19.87 And if you have a blood clot that's traveling 00:05:19.90\00:05:21.83 from somewhere else in the body 00:05:21.86\00:05:23.30 and it goes to a small blood vessel 00:05:23.33\00:05:25.15 then that part of the brain doesn't get enough blood. 00:05:25.18\00:05:28.43 Now blood clots can originate from the heart. 00:05:28.46\00:05:30.78 We talked about a rhythm called atrial fibrillation 00:05:30.81\00:05:33.64 that where the blood doesn't move to the heart. 00:05:33.67\00:05:35.32 Sometimes the heart is weak 00:05:35.35\00:05:36.95 and the blood sits in the bottom of the heart 00:05:36.98\00:05:38.98 and pumps it up. 00:05:39.01\00:05:40.38 So and when a person has a stroke it could be 00:05:40.41\00:05:43.32 what we call an embolic type of stroke. 00:05:43.35\00:05:45.34 Now you can also have blockages like this 00:05:45.37\00:05:47.70 where the blood just doesn't flow through. 00:05:47.73\00:05:49.19 This is a major blockage right there. 00:05:49.22\00:05:50.62 Yeah, and that can cause a stroke, too. 00:05:50.65\00:05:52.82 So whenever the blood doesn't get enough oxygen 00:05:52.85\00:05:56.36 and blood sugar you could have a stroke 00:05:56.39\00:05:58.79 'cause the blood is notgetting there. 00:05:58.82\00:06:00.40 Now there is another type of stroke 00:06:00.43\00:06:01.89 that happens about 13% of the time. 00:06:01.92\00:06:04.18 That's when the artery breaks open. 00:06:04.21\00:06:07.12 Let's say a weak spot. 00:06:07.15\00:06:08.52 You've heard of aneurysm? Oh, yes. 00:06:08.55\00:06:09.92 Yeah, that's a weak spot in a blood vessel. 00:06:09.95\00:06:12.36 And if that would have break open 00:06:12.39\00:06:13.76 the blood oozes out into the brain. 00:06:13.79\00:06:16.04 We call that a hemorrhagic blood stroke. 00:06:16.07\00:06:19.40 That stroke can be very devastating 00:06:19.43\00:06:21.19 and if that's seen on a C.A.T. scan 00:06:21.22\00:06:23.13 the treatment for that is immediately opening up 00:06:23.16\00:06:25.55 and fixing that blood vessel. 00:06:25.58\00:06:26.97 We can sometimes clip an aneurysm. 00:06:27.00\00:06:29.03 That's important to know 00:06:29.06\00:06:30.43 because if it's because of a blood clot 00:06:30.46\00:06:33.03 the treatment is very different. 00:06:33.06\00:06:34.47 If you have a blood clot the treatment 00:06:34.50\00:06:35.87 is going to be blood thinners, okay? 00:06:35.90\00:06:37.98 But you don't want to give blood thinners to people 00:06:38.01\00:06:39.90 that are bleeding 'cause they bleed more. 00:06:39.93\00:06:41.72 So that's why they have to get a C.A.T. scan 00:06:41.75\00:06:43.54 really quick if you think you're having a stroke. 00:06:43.57\00:06:45.98 If it's not a bleeding stroke then what we try to do 00:06:46.01\00:06:48.43 is we can give some medicines to break open, 00:06:48.46\00:06:50.67 open that artery up. 00:06:50.70\00:06:52.31 And if that sometimes-- And a C.A.T. scan can see this? 00:06:52.34\00:06:55.52 C.A.T. scan or M.R.I.--well, they can see if you're bleeding. 00:06:55.55\00:06:57.69 If you're bleeding, okay. Right, they can see-- 00:06:57.72\00:06:59.09 'cause that's gonna affect treatment. All right. 00:06:59.12\00:07:00.49 Now we make our decisions 00:07:00.52\00:07:01.89 based on a clinical history you know? 00:07:01.92\00:07:03.78 If the brain looks okay and it won't show 00:07:03.81\00:07:05.61 a stroke immediately we can say clinically 00:07:05.64\00:07:07.63 they're having a stroke, okay? 00:07:07.66\00:07:09.61 The brain doesn't show a bleed 00:07:09.64\00:07:11.01 so we would treat that with blood thinners. 00:07:11.04\00:07:13.11 And there's some powerful blood thinners that we use. 00:07:13.14\00:07:15.12 One is called T.P.A. which breaks up blood clots. 00:07:15.15\00:07:18.47 But nowadays we have a procedure where we can 00:07:18.50\00:07:20.70 actually go up into the brain with the device 00:07:20.73\00:07:23.71 and pull out the clot. Oh, my. 00:07:23.74\00:07:25.65 Now that's only at a few centers 00:07:25.68\00:07:27.64 and these are advanced strokes centers 00:07:27.67\00:07:29.80 but the key to treatment for the brain 00:07:29.83\00:07:31.53 is just like the heart. 00:07:31.56\00:07:33.48 You got to get treatment fast. 00:07:33.51\00:07:35.33 Now if the heart doesn't get enough blood 00:07:35.36\00:07:37.13 it sometimes has a dangerous heart rhythm. 00:07:37.16\00:07:39.66 Guess what happens to the brain? 00:07:39.69\00:07:41.11 The brain has a dangerous sort of rhythm. 00:07:41.14\00:07:43.61 It's called the seizure. 00:07:43.64\00:07:45.03 If a brain doesn't get enough blood 00:07:45.06\00:07:47.34 or it doesn't get enough oxygen it has a seizure. 00:07:47.37\00:07:49.63 So frequently strokes can precipitate seizures, too. 00:07:49.66\00:07:53.06 So the key is to recognize you're not thinking right. 00:07:53.09\00:07:56.37 Your body is not moving right. 00:07:56.40\00:07:58.46 Immediately get to the emergency room 00:07:58.49\00:08:00.34 and see if treatment could be done to help abort the stroke 00:08:00.37\00:08:02.95 whether it be an embolic stroke 00:08:02.98\00:08:04.71 with the blood clots floating somewhere else. 00:08:04.74\00:08:06.67 It might be that there is blockages in the arteries. 00:08:06.70\00:08:09.13 We now know that some arteries have spasms in it as well, 00:08:09.16\00:08:12.63 but stroke is so devastating because once the brain dies 00:08:12.66\00:08:15.69 things are really terrible you know? 00:08:15.72\00:08:17.83 The heart dies you can get away with a lot less heart 00:08:17.86\00:08:21.19 but when the brain starts dying and you can't move or speak 00:08:21.22\00:08:24.05 or you can't walk as well, 00:08:24.08\00:08:25.61 you can't feed yourself that's really devastating. 00:08:25.64\00:08:28.60 And nowadays, stroke-- the people that survive stroke 00:08:28.63\00:08:32.07 often can't do the things 00:08:32.10\00:08:33.88 and have the lifestyle that they want to. 00:08:33.91\00:08:36.05 And unfortunately once the damage is done 00:08:36.08\00:08:38.53 it's not like heart disease. 00:08:38.56\00:08:39.93 We don't have as fancier things, you know? 00:08:39.96\00:08:41.33 We can't, you know, shock the brain 00:08:41.36\00:08:43.16 if it's having a seizure, you know? 00:08:43.19\00:08:44.63 We have to give medicines. 00:08:44.66\00:08:46.03 We can't get back parts that are permanently damaged. 00:08:46.06\00:08:49.53 Now sometimes we can through stroke rehab 00:08:49.56\00:08:51.87 we can get people moving again and feeling better 00:08:51.90\00:08:54.31 and sometimes the brain can re-route some blood to that 00:08:54.34\00:08:57.00 but often people do not get the brain function back 00:08:57.03\00:09:00.16 and that's just really devastating. 00:09:00.19\00:09:01.66 And that's why we want to talk about stroke 00:09:01.69\00:09:03.94 since it's a cause of-- leading cause of death in the world. 00:09:03.97\00:09:06.32 We want people to know what a symptom of a stroke is 00:09:06.35\00:09:08.98 and that they have to get to the emergency room right away. 00:09:09.01\00:09:11.43 And there are developing treatments we can do to limit 00:09:11.46\00:09:14.13 the damage that might be occurring from a stroke. 00:09:14.16\00:09:16.67 This brings up an interesting thing. 00:09:16.70\00:09:18.50 I know a lot of people, Dr. Marcum, 00:09:18.53\00:09:20.58 say I don't want to cry wolf here. 00:09:20.61\00:09:23.15 I don't want to have this symptom 00:09:23.18\00:09:25.14 which is really not an issue. 00:09:25.17\00:09:28.21 A little bit of slurred speech, a little blurred vision, maybe, 00:09:28.24\00:09:30.93 a little weakness in the arm, I don't wanna run 00:09:30.96\00:09:33.14 to Dr. Marcum every time this happens 00:09:33.17\00:09:34.88 because aren't there are other things that sort of mimic 00:09:34.91\00:09:37.37 a heart attack or a stroke? Yeah. 00:09:37.40\00:09:39.82 Migraine headaches can mimic a stroke. 00:09:39.85\00:09:42.75 Frequently low blood sugar can mimic a stroke. 00:09:42.78\00:09:45.94 Low oxygen can mimic a stroke 00:09:45.97\00:09:48.78 but also a stroke can be a stroke. Yes, yes. 00:09:48.81\00:09:51.41 And we also know that there are certain people 00:09:51.44\00:09:53.14 that are high risk of developing a stroke. 00:09:53.17\00:09:55.59 People that have had strokes in the family, 00:09:55.62\00:09:57.86 people that have high blood pressure, 00:09:57.89\00:09:59.78 people that have diabetes, people that have heart disease, 00:09:59.81\00:10:03.70 people that have blockages in the arteries of the neck, 00:10:03.73\00:10:06.39 people that have weak heart 00:10:06.42\00:10:08.66 because there could be a blood clot that forms in it. 00:10:08.69\00:10:11.64 Those are people that are higher risk 00:10:11.67\00:10:13.47 of having a stroke. 00:10:13.50\00:10:14.87 So if they have a symptom and you're in a high risk group, 00:10:14.90\00:10:17.40 I want to stay at home and say 00:10:17.43\00:10:18.80 oh, it might be a migraine headache. 00:10:18.83\00:10:20.88 Now I would get help right away. 00:10:20.91\00:10:22.28 Now younger people-- there's a story 00:10:22.31\00:10:24.29 I heard not too long ago. 00:10:24.32\00:10:25.69 One person thought that they were 00:10:25.72\00:10:27.09 just having a migraine headache. 00:10:27.12\00:10:28.49 Well, what was really happening is the artery 00:10:28.52\00:10:30.17 in the brain was, what we call, dissecting. 00:10:30.20\00:10:32.49 It was ripping. oh. 00:10:32.52\00:10:33.93 And they got in there, luckily they got on time. 00:10:33.96\00:10:36.08 The neurosurgeon was able to fix that problem. 00:10:36.11\00:10:38.62 But I think it's better to be safe than sorry 00:10:38.65\00:10:40.90 because, you know, if you're wrong, 00:10:40.93\00:10:42.50 you have an emergency room visit and they say oh, it's a migraine 00:10:42.53\00:10:45.75 or a low blood sugar but if you're wrong, 00:10:45.78\00:10:47.94 if you're wrong about it 00:10:47.97\00:10:49.34 you could be devastated and limited 00:10:49.37\00:10:50.83 'cause you got a very narrow window to get help. 00:10:50.86\00:10:54.08 Now you're saying that the brain the can live 00:10:54.11\00:10:56.51 without oxygen longer than the heart. 00:10:56.54\00:10:58.11 Is that--is that what I hear you say? 00:10:58.14\00:10:59.51 Well, it's the--you know, the brain is so specialized, 00:10:59.54\00:11:02.68 you know, The heart just all it does, 00:11:02.71\00:11:04.21 you know, bless its heart, is it pumps blood everywhere. 00:11:04.24\00:11:06.81 That's true. So it's a pump. 00:11:06.84\00:11:08.21 So it can get by with a little less pump but the brain, 00:11:08.24\00:11:10.73 you know, well--can you get by with a part 00:11:10.76\00:11:12.80 that doesn't your arm? That's true. 00:11:12.83\00:11:14.24 Can you get part that doesn't move your feet? 00:11:14.27\00:11:16.03 Can you get part that, you know, 00:11:16.06\00:11:17.43 doesn't think or controls the eyes? 00:11:17.46\00:11:18.96 So it's really specific to blood vessels. 00:11:18.99\00:11:20.94 So the brain, you know, sometimes it can get by but 00:11:20.97\00:11:23.64 the brain is so complicated and it does so many things 00:11:23.67\00:11:27.03 for the body including controlling the heart. 00:11:27.06\00:11:29.80 So the brain function is just remarkable. 00:11:29.83\00:11:31.81 And since the stroke is such a real problem and we've seen 00:11:31.84\00:11:34.14 stroke rates go up 00:11:34.17\00:11:35.72 as prevalence of diabetes gone up because of obesity, 00:11:35.75\00:11:39.19 the prevalence of high blood pressure 00:11:39.22\00:11:40.59 is gone up because of our diet 00:11:40.62\00:11:41.99 which are filled with sodium, 00:11:42.02\00:11:43.66 the prevalence of these blockages go up 00:11:43.69\00:11:45.42 'cause we're eating lots of cholesterol. 00:11:45.45\00:11:47.61 So as the stroke rates have gone up 00:11:47.64\00:11:49.59 we need to work harder and harder 00:11:49.62\00:11:51.01 about doing the preventive things 00:11:51.04\00:11:52.55 at a young age to prevent it. 00:11:52.58\00:11:54.73 Now I heard not recently about 00:11:54.76\00:11:57.20 if we could prevent these things, 00:11:57.23\00:11:58.97 if we could live healthy lifestyle and control 00:11:59.00\00:12:01.60 these risk factors of stroke and heart attack 00:12:01.63\00:12:03.79 before people were 35 we could also prevent 00:12:03.82\00:12:07.54 all the stuff that's happening as people get older. 00:12:07.57\00:12:09.75 And we know that stroke tends to run 00:12:09.78\00:12:11.47 in elderly people more than younger people. 00:12:11.50\00:12:14.16 So if we could prevent-- live those first 35 years 00:12:14.19\00:12:17.04 as healthy as we can, the second 35 years 00:12:17.07\00:12:20.30 are gonna do really, really well 'cause this seems 00:12:20.33\00:12:22.47 to be a cumulative disease that we get. 00:12:22.50\00:12:24.57 So we are facing some things that we can do 00:12:24.60\00:12:28.55 in our first 35 years. 00:12:28.58\00:12:29.98 Now we talk about diet, exercise and all those things. 00:12:30.01\00:12:33.84 Let's move past the 35 years. 00:12:33.87\00:12:35.31 Let's move into our generation. Right. 00:12:35.34\00:12:38.20 Do the things that we do before 35 00:12:38.23\00:12:40.34 still help us after 35? 00:12:40.37\00:12:42.11 Absolutely, Charles, and, you know, 00:12:42.14\00:12:43.81 it would be great if we did all these things 00:12:43.84\00:12:45.45 in the first 35 years of our life 00:12:45.48\00:12:47.42 but sometimes we are not smart enough 00:12:47.45\00:12:48.82 to do that. Right. 00:12:48.85\00:12:50.22 So we finally catch on. 00:12:50.25\00:12:51.62 Hey, I need to do these things but if you're right now-- 00:12:51.65\00:12:53.68 if you're at a risk and want to lower your risk of stroke, 00:12:53.71\00:12:55.79 yes, do these things, you know?
