Yeah. 00:00:25.04\00:00:27.87 But we talked in previous programs about heart attacks. 00:00:27.90\00:00:30.57 You know, we talked about the rhythms. 00:00:30.60\00:00:32.42 But there is all-- lot of other things 00:00:32.45\00:00:34.47 that cause hearts to get weak too. 00:00:34.50\00:00:36.25 And we're gonna address these 00:00:36.28\00:00:37.65 and some of the questions we get up. 00:00:37.68\00:00:39.05 Let's go to the heartwiseministries.org website 00:00:39.08\00:00:41.93 and here are some questions that have been sent in. 00:00:41.96\00:00:43.70 Number 1, what exactly does the term, 00:00:43.73\00:00:46.97 heart failure mean? 00:00:47.00\00:00:49.12 Now that sounds like the heart just stops. Is that right? 00:00:49.15\00:00:51.63 The heart's failure means 00:00:51.66\00:00:53.03 that the heart for whatever reason 00:00:53.06\00:00:54.95 doesn't pump blood to the needed organs. 00:00:54.98\00:00:57.57 And the needed organ is the brain. Yes. 00:00:57.60\00:00:59.78 A needed organ is the kidneys. 00:00:59.81\00:01:01.60 So when the heart doesn't pump for whatever reason, 00:01:01.63\00:01:04.10 we call that heart failure. 00:01:04.13\00:01:05.50 It fails to do what it's designed to do. 00:01:05.53\00:01:06.90 Exactly. A symptom of heart failure might be fatigue. 00:01:06.93\00:01:10.71 If the heart doesn't pump blood 00:01:10.74\00:01:12.11 for it sometimes fluid backs up in the lungs. 00:01:12.14\00:01:14.17 You get short of breath, especially, when you lie down. 00:01:14.20\00:01:16.55 Yeah. You can't get rid of the fluid in the body 00:01:16.58\00:01:18.83 so you might swell up in the abdomen or legs. 00:01:18.86\00:01:21.04 Those all might be symptoms of heart failure. Okay. 00:01:21.07\00:01:24.43 So once you make a diagnosis of heart failure 00:01:24.46\00:01:26.61 and we've got- we have x-rays. 00:01:26.64\00:01:28.01 We can look for extra fluids. 00:01:28.04\00:01:29.41 There are some chemicals we can measure in the blood 00:01:29.44\00:01:31.49 that tells us if that person is in heart failure. 00:01:31.52\00:01:33.66 But usually I know if a person is in the heart failure 00:01:33.69\00:01:35.60 because their heart's beating fast. 00:01:35.63\00:01:37.44 Try to keep up with them, they can't breathe well. 00:01:37.47\00:01:39.42 They're swelling up. They'll feel weak and tired. 00:01:39.45\00:01:42.12 You've told me that you can have 00:01:42.15\00:01:43.77 a patient walk into the room 00:01:43.80\00:01:45.65 and you can look at that patient. 00:01:45.68\00:01:47.05 And you can pretty well tell what's going on with the heart. 00:01:47.08\00:01:48.69 Yeah. What are you looking for? You can. 00:01:48.72\00:01:50.09 You know, usually you're looking for them not feeling well. 00:01:50.12\00:01:52.63 Yeah. You know, they're breathing quickly. 00:01:52.66\00:01:54.39 Lot of times a minute. 00:01:54.42\00:01:55.79 You know, they might have pallor 00:01:55.82\00:01:57.19 because they're not getting good blood 00:01:57.22\00:01:58.59 throughout their body. All right. 00:01:58.62\00:01:59.99 So after we determine 00:02:00.02\00:02:01.39 a patient's in heart failure for whatever reason, 00:02:01.42\00:02:03.30 we wanna find out the reason why. 00:02:03.33\00:02:05.68 Why do things happen? 00:02:05.71\00:02:07.16 Unless you understand the reason 00:02:07.19\00:02:08.60 why it's hard to get a good treatment. 00:02:08.63\00:02:10.82 So the next thing we say it could be a rhythm problem. 00:02:10.85\00:02:13.30 And we do that with an E.K.G. 00:02:13.33\00:02:15.03 or monitor or checking the pulse. 00:02:15.06\00:02:16.79 It could be one of these congenital problems. 00:02:16.82\00:02:18.52 Yes. It could be a fast rhythm like atrial fibrillation. 00:02:18.55\00:02:21.33 It might be a slow rhythm. 00:02:21.36\00:02:23.01 So if that's a cause of heart failure, 00:02:23.04\00:02:24.78 we want to correct the rhythm. 00:02:24.81\00:02:26.59 Now sometimes another reason of heart failure 00:02:26.62\00:02:28.79 is the heart pump itself is weak. 00:02:28.82\00:02:30.97 The left ventricle, this big chamber 00:02:31.01\00:02:33.23 that pumps to the entire body is weak. 00:02:33.26\00:02:36.42 Many things can cause that to be weak too. 00:02:36.45\00:02:38.82 The most common is blockages in the arteries. 00:02:38.85\00:02:41.17 We've talked about that in previous programs. 00:02:41.20\00:02:43.48 And the treatment for that would be 00:02:43.51\00:02:44.88 see if we can get more blood out to the heart. 00:02:44.91\00:02:47.13 You can sure tell right here that needs blood. 00:02:47.16\00:02:49.03 I mean look at that--you can see the artery is coming in here. 00:02:49.06\00:02:51.08 There is a big supply coming into the heart all the time. 00:02:51.11\00:02:53.71 So it's very obvious that without that blood supply 00:02:53.74\00:02:56.30 this heart would have a hard time. 00:02:56.33\00:02:57.70 Absolutely, Charles, also heart attacks, 00:02:57.73\00:03:00.78 blockages can make your heart weak. 00:03:00.81\00:03:03.13 The heart gets weak, doesn't pump blood very well. 00:03:03.16\00:03:06.31 Fluid backs up into the body. 00:03:06.34\00:03:08.15 Another cause of heart failure. 00:03:08.18\00:03:10.33 Now sometimes the muscle gets stiff and doesn't relax well. 00:03:10.36\00:03:14.49 The muscle either gets thick 00:03:14.52\00:03:16.06 from pumping against a lot of high blood pressure. 00:03:16.09\00:03:18.45 Sometimes we can have things 00:03:18.48\00:03:19.85 that actually get inside the heart wall. 00:03:19.88\00:03:21.94 We have conditions like sarcoidosis 00:03:21.97\00:03:24.21 where we have different proteins 00:03:24.24\00:03:25.90 that make the heart not expand as well. 00:03:25.93\00:03:28.13 Another condition called amyloidosis or hemochromatosis, 00:03:28.16\00:03:31.94 all these can also make the heart not relax well. 00:03:31.97\00:03:35.59 I mean there is a relaxing phase of the heart. 00:03:35.62\00:03:37.37 So any type of rhythm problem, 00:03:37.40\00:03:39.64 strength of the heart or relaxing condition 00:03:39.67\00:03:42.16 can also precipitate heart failure. 00:03:42.19\00:03:44.20 So your cardiologist, Charles,when you come in, 00:03:44.23\00:03:46.46 he first is gonna decide if you're an heart failure- 00:03:46.49\00:03:48.62 Yeah. Why you're in heart failure 00:03:48.65\00:03:50.72 and then he's gonna come up with the treatment plan. 00:03:50.75\00:03:53.11 Both with modern medicine 00:03:53.14\00:03:54.51 and especially with lifestyle changes. 00:03:54.54\00:03:56.41 Now did we answer her question? I think we did. 00:03:56.44\00:03:58.97 What does heart failure mean? 00:03:59.00\00:04:00.37 It just means that the heart is not doing 00:04:00.40\00:04:02.10 what it's designed to do. Exactly. 00:04:02.13\00:04:03.50 Like brain failure. Let me tell you. 00:04:03.53\00:04:05.91 Brain failure. Brain failure? 00:04:05.94\00:04:07.31 Yes, brain fails when the brain doesn't do 00:04:07.34\00:04:08.80 what it's supposed to do. I have that. All right. 00:04:15.18\00:04:17.77 I wanna insert one of my own here because a lot of times 00:04:17.80\00:04:21.48 when I am just, sort of, sitting at the computer 00:04:21.51\00:04:23.00 and everything my heart just flutters. 00:04:23.03\00:04:26.74 Yeah. Is fluttering dangerous? 00:04:26.77\00:04:28.84 Shall I go see you with a fluttering heart? 00:04:28.87\00:04:30.63 If it continues and lots of people 00:04:30.66\00:04:32.69 have skipped heart beats. Yeah. 00:04:32.72\00:04:34.26 Okay, now if it goes for long periods of time 00:04:34.29\00:04:36.63 then you don't get a good blood pressure to your head. 00:04:36.66\00:04:38.71 You get dizzy or you've a- - Just for a few seconds. 00:04:38.74\00:04:40.92 Yeah, if it's just for a few seconds. 00:04:40.95\00:04:42.32 Probably not but if you start having symptoms 00:04:42.35\00:04:44.40 that lasts longer we put a monitor 00:04:44.43\00:04:46.48 on you and I'll check to see 00:04:46.51\00:04:47.88 if you have a funny heart rhythm that might need some treatment. 00:04:47.91\00:04:50.88 But most people if they lower the stress, 00:04:50.91\00:04:52.94 they get enough sleep, drink enough water, 00:04:52.97\00:04:54.81 cut back on their stimulants-- 00:04:54.84\00:04:56.40 you know, these energy drinks are causing 00:04:56.43\00:04:57.99 lots of problems with rhythm now. 00:04:58.02\00:04:59.91 Yeah, people drink all these energy drinks 00:04:59.94\00:05:01.59 and the heart starts going too fast. 00:05:01.62\00:05:03.45 No blood goes to the head. 00:05:03.48\00:05:04.85 They've serious problems. Wow. Okay. 00:05:04.88\00:05:06.89 So-so lots of things can make the heart not work well. 00:05:06.92\00:05:09.61 So I think we answered that question. I think we did. 00:05:09.64\00:05:11.54 All right. No more brain failures. 00:05:11.57\00:05:12.94 Now what is the main stay in the treatment of heart failure? 00:05:12.97\00:05:16.29 What's the number 1 thing to do? Okay, okay. 00:05:16.32\00:05:18.17 Well, we talked about that. First, we wanna to know 00:05:18.20\00:05:20.21 why the heart's weak, okay? 00:05:20.24\00:05:22.06 Once we deal with that and let's say 00:05:22.09\00:05:23.56 there's nothing that we can do to reverse it. 00:05:23.59\00:05:25.10 There's no-a pacer or a device we can put in the heart. 00:05:25.13\00:05:28.57 We can't do bypass. The heart is just weak, let's say. Yeah. 00:05:28.60\00:05:31.63 When the heart is weak 00:05:31.66\00:05:33.03 it turns on this neural hormonal axis. 00:05:33.06\00:05:35.48 The heart says I am weak. 00:05:35.51\00:05:36.88 So a bunch of chemicals get in the body which make it weaker. 00:05:36.91\00:05:39.86 Those are the stress chemicals. 00:05:39.89\00:05:41.26 Adrenaline, cortisol, angiotensin, 00:05:41.29\00:05:43.99 we make all these chemicals 00:05:44.02\00:05:45.39 that make your heart even weaker. 00:05:45.42\00:05:46.81 So what we wanna do is we wanna block those chemicals. 00:05:46.84\00:05:49.87 So our medications that we give people help block 00:05:49.90\00:05:52.76 those chemicals. which damage the heart further. 00:05:52.79\00:05:55.64 Of course, the natural thing we wanna do is get enough rest, 00:05:55.67\00:05:59.43 exercise, make some endorphins. Eat a healthy diet. 00:05:59.46\00:06:02.87 You know, get good oxygen throughout the body. 00:06:02.90\00:06:04.91 The common sense things. 00:06:04.94\00:06:06.41 But in addition to that we can give medicines like 00:06:06.44\00:06:09.14 beta blockers which block adrenaline. 00:06:09.17\00:06:11.88 We can also give angiotensin blockers, 00:06:11.91\00:06:15.45 they call ACE inhibitors, the blocker chemical. 00:06:15.48\00:06:18.71 So we can block the chemicals that make a heart weaker. 00:06:18.74\00:06:21.35 It can actually make the heart a little bit stronger. 00:06:21.38\00:06:23.76 So the dual treatment for this, first find out why it's weak 00:06:23.79\00:06:27.23 and see if there's anything reversible. Yeah. 00:06:27.26\00:06:29.01 And if it's not reversible give medicines 00:06:29.04\00:06:31.02 to block this neural hormonal axis. 00:06:31.05\00:06:33.37 And then, of course, if there's extra fluid in the body, 00:06:33.40\00:06:35.68 we wanna give a diarrhetic 00:06:35.71\00:06:37.12 which take fluid out of the body. 00:06:37.15\00:06:40.54 We wanna teach people how to eat well. 00:06:40.57\00:06:41.94 And avoid extra salt or do anything 00:06:41.97\00:06:43.34 that will make the heart work harder. 00:06:43.37\00:06:44.94 Years ago, Charles, 00:06:44.97\00:06:46.46 when we didn't have all this modern medicine, 00:06:46.49\00:06:48.48 when we didn't have defibrillators 00:06:48.51\00:06:49.88 to shock a weak heart or these medicines 00:06:49.91\00:06:51.99 to keep the heart from getting weaker, 00:06:52.02\00:06:53.56 you know, what we used to do? 00:06:53.59\00:06:54.96 We use to rest the heart. 00:06:54.99\00:06:56.53 If your arm was broken 00:06:56.56\00:06:57.93 you wouldn't quit throwing fast balls, would you? 00:06:57.96\00:06:59.51 We used to bed rest hearts. 00:06:59.54\00:07:01.04 And so resting the heart helps too, 00:07:01.07\00:07:03.15 which brings me back to some of the biblical treatments 00:07:03.18\00:07:05.44 we've been talking about. 00:07:05.47\00:07:06.84 You know, God gave us a time of rest. God gave us night. 00:07:06.87\00:07:10.25 Well, that helps us to block these chemicals 00:07:10.28\00:07:12.17 which make our body weaker. 00:07:12.20\00:07:13.73 So that's a great treatment for everybody 00:07:13.76\00:07:16.09 whether or not they have a weak heart 00:07:16.12\00:07:17.66 is make sure you get enough rest in your life. 00:07:17.69\00:07:20.05 Both the nightly rest and the God of the universe 00:07:20.08\00:07:22.82 didn't have health problems and he rested one day a week. 00:07:22.85\00:07:25.39 So rest is also a treatment. 00:07:25.42\00:07:26.95 It changes the chemistry just like these medicines should. 00:07:26.98\00:07:30.82 So let's go to another question. Yeah. 00:07:46.16\00:07:47.96 And that's a question that has to do 00:07:47.99\00:07:49.62 because sometimes atrial fibrillation 00:07:49.65\00:07:51.19 that's where the top part 00:07:51.22\00:07:52.73 goes faster than the bottom part. 00:07:52.76\00:07:54.47 Sometimes it goes so fast 00:07:54.50\00:07:56.01 that the heart loses its efficiency. 00:07:56.04\00:07:58.07 Like quivering. Yes. 00:07:58.10\00:07:59.47 If the top part is quivering 00:07:59.50\00:08:00.87 then you go into heart failure. Yeah. 00:08:00.90\00:08:02.27 So the first thing I say when a person-- 00:08:02.30\00:08:03.96 if the heart failure is from the atrial fibrillation 00:08:03.99\00:08:06.07 we wanna see if we can reverse 00:08:06.10\00:08:07.47 the cause of atrial fibrillation. 00:08:07.50\00:08:09.01 That might be sleep apnea, over active thyroid, 00:08:09.04\00:08:11.83 dehydration or it might be that you just have it. 00:08:11.86\00:08:14.40 Now we can get rid it quickly with a shock. 00:08:14.43\00:08:16.46 I can shock the heart and get it back in rhythm. 00:08:16.49\00:08:18.37 I sometimes-we also can sometimes do a fancy oblations 00:08:18.73\00:08:21.94 that can get rid of the rhythm. 00:08:21.97\00:08:23.34 But sometimes no matter what we still have this rhythm. 00:08:23.38\00:08:26.27 Once we slow it down usually the heart functions pretty good. 00:08:26.30\00:08:29.73 It's that fast speed 00:08:29.99\00:08:31.36 that doesn't allow it to fill up with blood 00:08:31.39\00:08:32.91 that causes heart failure but in this process 00:08:32.94\00:08:36.26 the atrium goes fast it's like a bag of worms. 00:08:36.29\00:08:39.34 And when the blood doesn't move 00:08:39.37\00:08:41.33 through it easily you can get a blood clot. 00:08:41.36\00:08:44.38 And if a blood clot pumps out of the heart 00:08:44.41\00:08:46.14 it could go at the head. 00:08:46.17\00:08:47.54 And that could cause a stroke, an embolic stroke. 00:08:47.57\00:08:50.45 So people that have atrial fibrillation, 00:08:50.48\00:08:53.81 once the heart slowdown, 00:08:53.84\00:08:55.21 they're out of heart failure, every doctor should say, 00:08:55.24\00:08:57.48 listen, is this a person that's a candidate for a blood thinner. 00:08:57.69\00:09:01.76 And one of the blood thinners 00:09:01.79\00:09:03.16 we have out now is called coumadin. 00:09:03.19\00:09:04.90 They've developed a couple of new blood thinners 00:09:04.93\00:09:06.82 that are on the market now. 00:09:06.85\00:09:08.22 One's called pradaxa. One's called xarelto. 00:09:08.25\00:09:10.76 They each have their pros and cons. 00:09:10.79\00:09:13.04 And I know a lot of people 00:09:13.07\00:09:14.44 that I talk to never want to take a blood thinner. 00:09:14.47\00:09:17.59 But then also I don't want them to have a stroke 00:09:17.62\00:09:19.80 because the stroke could debilitate them. 00:09:19.83\00:09:21.90 And if you're in atrial fibrillation 00:09:21.93\00:09:23.65 and you don't take a blood thinner 00:09:23.68\00:09:25.05 the chance of you having a stroke 00:09:25.08\00:09:26.45 is about five percent per year. 00:09:26.48\00:09:28.09 So if you go four years you could have a 20% 00:09:28.12\00:09:30.14 chance of having a stroke off a blood thinner. 00:09:30.17\00:09:31.83 So everything about medicines 00:09:31.86\00:09:33.63 is about the risk versus the benefits. 00:09:33.66\00:09:36.07 So this question, should I take a blood thinner 00:09:36.10\00:09:38.86 at least should be talked about it 00:09:38.89\00:09:40.59 to see if you're an adequate risk for it. 00:09:40.62\00:09:42.72 All right. Let's go to the next question here. 00:09:42.75\00:09:44.52 My friend had a pacemaker. 00:09:44.55\00:09:46.98 What type of patient needs a pacemaker? 00:09:47.01\00:09:49.93 And I understand you've one here to show us. 00:09:49.96\00:09:51.33 Yes, I do, Charles. 00:09:51.36\00:09:52.73 And before we get to that I am gonna tell you 00:09:52.76\00:09:55.27 what type of person needs a pacemaker. 00:09:55.30\00:09:57.58 If your heart's going too slow for whatever reason 00:09:57.61\00:10:00.84 you might benefit from a pacemaker, okay? 00:10:00.87\00:10:03.18 And it might be that your heart's just degenerative. 00:10:03.21\00:10:05.51 It's just getting old, rusty. 00:10:05.54\00:10:07.34 The battery doesn't work well. You need a new battery. 00:10:07.37\00:10:10.01 It might be that you're not getting 00:10:10.04\00:10:11.41 enough blood to the battery. 00:10:11.44\00:10:12.99 It might be that you get an infection. 00:10:13.02\00:10:14.87 It might be that you have other conditions 00:10:14.90\00:10:16.72 that make your heart go slow. 00:10:16.75\00:10:18.36 Well, a pacemaker is usually placed right here 00:10:18.39\00:10:21.86 in this area on the non dominant side. Okay. 00:10:21.89\00:10:24.58 And then we-- The heart is right in here-- 00:10:24.61\00:10:26.80 Right in here, uh-huh. 00:10:26.83\00:10:28.20 It's right in here about the size of the fist. 00:10:28.23\00:10:29.60 Now this is probably an elephant's heart here. 00:10:29.63\00:10:31.20 That would be a very large heart. Big heart. Okay. 00:10:31.23\00:10:33.39 So what we do we put the lead--we implant 00:10:33.42\00:10:36.18 this here under the skin. We put the lead in. 00:10:36.21\00:10:38.11 Now the lead has wires that monitor. 00:10:38.14\00:10:40.02 It has computer chip. 00:10:40.05\00:10:41.42 It would go inside and we would actually 00:10:41.45\00:10:43.74 place this inside the left ventricle-- 00:10:43.77\00:10:46.28 the right ventricle of the heart 00:10:46.31\00:10:47.78 And it now would tell wevery single beat 00:10:47.81\00:10:49.87 that would happen and if it would go slow 00:10:49.90\00:10:51.84 there is a sensor there and the heart would start pacing 00:10:51.87\00:10:55.18 the electrical current that came through this battery. 00:10:55.21\00:10:58.08 This is like a probe then-- Yes. 00:10:58.11\00:11:00.09 It's a like a battery of your car.Okay. 00:11:00.12\00:11:03.48 And then you have this lead 00:11:03.51\00:11:04.88 that would give the electrical shock 00:11:04.91\00:11:06.28 that would make it squeeze. 00:11:06.31\00:11:07.68 Is this embedded right into the muscle of the-- 00:11:07.71\00:11:09.34 Yes and actually there's some little spikes that, 00:11:09.37\00:11:11.58 sort of, anchor it into the ventricle. Okay. 00:11:11.61\00:11:13.97 But eventually it grows into the heart. Okay. 00:11:14.00\00:11:16.57 Pacemaker usually lasts about seven years 00:11:16.60\00:11:19.52 before the battery runs out. 00:11:19.55\00:11:21.28 Now we can check it with monitors on the telephone. 00:11:21.31\00:11:24.38 And then when it runs out we just put a new one in 00:11:24.41\00:11:26.62 and take the lead that's already there 00:11:26.65\00:11:28.02 and hook it back up and boom, 00:11:28.05\00:11:29.42 you're good for another seven leads. 00:11:29.45\00:11:30.82 This is minimally invasive because you make 00:11:30.85\00:11:32.22 a very small insertion there. 00:11:32.25\00:11:33.62 Yes, very, very-- it's a very great thing 00:11:33.65\00:11:36.01 for people that are older. 00:11:36.04\00:11:37.65 That have there hearts going too slow 00:11:37.68\00:11:39.54 just because the battery is wearing out. 00:11:39.57\00:11:41.78 Before we had this people would die prematurely. 00:11:41.81\00:11:44.49 Their heart would go slower and slower and slower. 00:11:44.52\00:11:46.86 And then it would stop. 00:11:46.89\00:11:48.26 But now we've pacemaker therapies 00:11:48.29\00:11:49.79 that can help a weak-- a heart beat better. 00:11:49.82\00:11:52.48 It can also help people not go into heart failure. 00:11:52.51\00:11:55.46 Sometimes it goes so slow, 00:11:55.49\00:11:56.86 the lungs fill up with fluids. So it's a great treatment. 00:11:56.89\00:12:00.25 A great place for modern medicine.
