I know we have been talking a lot about 00:01:59.98\00:02:02.55 pain and we are going to try to sum up and answer some 00:02:02.58\00:02:05.29 questions we haven't answered, but where do you think we should 00:02:05.32\00:02:08.12 start today? Well, I am going to go through 00:02:08.16\00:02:10.43 and kind of quiz you on the different types of pain 00:02:10.46\00:02:12.63 people might be experiencing at home and you can kind of 00:02:12.66\00:02:15.26 tell us what type of physician they should go see. Yeah, 00:02:15.30\00:02:17.23 You tell me the type of pain and I will the type of specialist 00:02:17.27\00:02:21.40 that you should see. I'll also mention the most 00:02:21.44\00:02:24.71 dangerous thing about that type of pain in that part 00:02:24.74\00:02:27.08 of the body. So we are going to start off 00:02:27.11\00:02:28.44 with pain in the mouth, oral pain. 00:02:28.48\00:02:30.85 Ok, oral pain. Two sets of doctors would be very helpful 00:02:30.88\00:02:34.28 for that type of pain. One is a dentist because they are in 00:02:34.32\00:02:37.52 the mouth all the time, and the other one that gets 00:02:37.55\00:02:39.79 called in all the time is an E.N.T., Ears, Nose, and Throat. 00:02:39.82\00:02:43.49 They are specialists that deal with the ear, nose, and throat 00:02:43.53\00:02:46.56 and that typically has oral pain. Um, there is not a lot 00:02:46.59\00:02:50.97 of oral pain that I see kill people right away, so that's 00:02:51.00\00:02:54.10 good. Right! You know, they don't have to run to the 00:02:54.14\00:02:56.40 Emergency Room all the time right away, so those are the 00:02:56.44\00:02:59.31 two types of doctors for that type of pain. 00:02:59.34\00:03:01.51 Alright! How about if someone is experiencing abdominal pain. 00:03:01.54\00:03:04.78 Abdominal pain could be a medical emergency, 00:03:04.81\00:03:08.08 especially if we rupture our big tube, if we are bleeding 00:03:08.12\00:03:12.49 inside. So if the pain is rapid and won't go away 00:03:12.52\00:03:15.82 associated with a fever, the belly is tight, nausea, 00:03:15.86\00:03:19.33 lots of diarrhea, you probably need to go to the Emergency 00:03:19.36\00:03:22.03 Room right now. ok. The specialists that deal 00:03:22.06\00:03:24.93 with that pain is usually Gastroenterologists, 00:03:24.97\00:03:28.14 GI specialist. They work with that pain frequently 00:03:28.17\00:03:32.07 um, if a blood vessel is involved they might do that. 00:03:32.11\00:03:34.84 General Surgeon's also work with that type of pain 00:03:34.88\00:03:37.81 quite a bit in the abdomen. But you want to make sure 00:03:37.85\00:03:40.48 that it is not heart pain either because sometimes 00:03:40.52\00:03:43.35 heart pain can simulate belly pain in some instances. 00:03:43.39\00:03:46.65 Alright, what about pain of the sex organs or down in the 00:03:46.69\00:03:50.43 private parts. What kind of doctor should you see? 00:03:50.46\00:03:52.49 Well, genital pain is either a Urologist or a Gynecologist. 00:03:52.53\00:03:58.17 Those are the doctor's that really specialize in that 00:03:58.20\00:04:00.80 type of pain. Now a good provider or good 00:04:00.84\00:04:04.54 family physician, a good internist, a good nurse 00:04:04.57\00:04:07.38 practioner, all of them might be able to help 90% of that type 00:04:07.41\00:04:10.31 of pain. But I am just talking for the 10% that might need 00:04:10.35\00:04:13.98 a specialist. Right! What about the nervous system. 00:04:14.02\00:04:17.09 There can be a lot of complex issues with the nervous system, 00:04:17.12\00:04:20.29 who do they see? Well, the Neurologist. 00:04:20.32\00:04:23.99 Neurologists are very good at doing that. They are experts 00:04:24.03\00:04:29.06 at evaluating the neurological system when your regular doctor 00:04:29.10\00:04:34.24 needs a little extra help. Like I said, a lot of these 00:04:34.27\00:04:37.44 conditions most doctors can handle pretty competently, 00:04:37.47\00:04:41.81 and you only need a specialist maybe 10 or 20% of the time. 00:04:41.84\00:04:45.41 What about joints and muscle aches? 00:04:45.45\00:04:48.38 Orthopedic doctors are very good to go to. 00:04:48.42\00:04:51.02 Sometimes good Physical Therapists and good 00:04:51.05\00:04:54.52 Chiropractors are other alternatives depending on the 00:04:54.56\00:04:57.76 nature of the joint pain and what type of professionals 00:04:57.79\00:05:00.40 you have in your area. 00:05:00.43\00:05:01.76 What about if someone is having chronic headaches and 00:05:01.80\00:05:04.23 that type of thing, who should they see? 00:05:04.27\00:05:05.60 That's another one, Neurologist is usually the one 00:05:05.63\00:05:07.80 to go for a chronic headache or a good Internal Medicine 00:05:07.84\00:05:09.97 doctor because an Internal Medicine doctor sort of looks 00:05:10.01\00:05:12.37 at everything. Those are some specialists that might evaluate 00:05:12.41\00:05:15.11 that type of pain. 00:05:15.14\00:05:16.68 Ok. The last one here I have on my list is going to be pretty 00:05:16.75\00:05:19.21 easy for you, someone having chest pains, who do they go see? 00:05:19.25\00:05:22.78 I think they need to see a "Cardiologist" if you are having 00:05:22.85\00:05:25.52 chest pain. It is different if someone hits you in the chest 00:05:25.55\00:05:29.52 and you know what is causing it, that's different but 00:05:29.56\00:05:31.69 for pain that is undiagnosed, you don't know what it is 00:05:31.73\00:05:35.20 a "Cardiologist" and any pain that is above the waist 00:05:35.23\00:05:39.23 that is unexplained, that is ongoing, I think you need 00:05:39.27\00:05:42.27 evaluation right now, it can't wait because it could be a 00:05:42.30\00:05:45.51 heart attack and then any pain that happens when you are 00:05:45.54\00:05:48.98 walking and doing something on exertion, that's when the heart 00:05:49.01\00:05:51.51 works, that might be very also suggestive of heart. 00:05:51.55\00:05:55.48 The reason I bring this up is because the heart pain 00:05:55.52\00:05:57.89 is probably the most common cause of acute pain 00:05:57.92\00:06:01.52 that's out there. It is the most common cause of death 00:06:01.56\00:06:04.36 in the United States, it's the most common cause of 00:06:04.39\00:06:07.13 lots of problems, so I am very sensitive to heart pain. 00:06:07.16\00:06:10.63 Yep, yep! That is so important, 00:06:48.34\00:06:51.31 Because how would you know that something was wrong 00:06:51.34\00:06:53.21 if you didn't have a signaling system. 00:06:53.24\00:06:55.41 Right! You would never know. It's like a true North, 00:06:55.44\00:06:58.55 You know if you are doing well you are not having pain. 00:06:58.58\00:07:00.68 Well things might be pretty good because God made us 00:07:00.72\00:07:05.02 to have pain when things aren't going well, 00:07:05.05\00:07:07.22 so that's why it's so important. Also if your having pain, 00:07:07.26\00:07:11.16 pain is a form of stress on the body. 00:07:11.19\00:07:13.53 It's great if you need to warn yourself to get help right away, 00:07:13.56\00:07:17.80 that's what it's for, but pain long term, turns on stressors 00:07:17.83\00:07:22.07 on the body which can damage it further, so chronic pain 00:07:22.10\00:07:25.77 is not good, it makes the adrenalin go up, 00:07:25.81\00:07:27.98 you don't think as good, your cognition, 00:07:28.01\00:07:30.05 the way you figure things out aren't as good, 00:07:30.08\00:07:33.05 people in chronic pain, they make mistakes easier at work, 00:07:33.08\00:07:38.09 they can't figure out simple things sometimes, 00:07:38.12\00:07:40.42 so chronic pain really affects not only the part that's 00:07:40.46\00:07:43.32 having the pain, but indirectly it affects your entire body. 00:07:43.36\00:07:47.00 Chronic pain even affects the way you worship, 00:07:47.03\00:07:50.07 people in chronic pain can't worship as well as people 00:07:50.10\00:07:53.23 that don't have chronic pain because of the brain 00:07:53.27\00:07:55.70 and how it is reacting and all the stress chemistry 00:07:55.74\00:07:58.47 that's turned on. God understands this and 00:07:58.51\00:08:01.54 He works with us, but that is some of the two biggest 00:08:01.58\00:08:04.88 reasons pain is important is that it is an alarm system 00:08:04.91\00:08:07.55 that something is not right, and two a longstanding pain 00:08:07.58\00:08:11.22 is dangerous to the body. 00:08:11.25\00:08:12.59 That's why there are some instances if you can't figure 00:08:12.62\00:08:14.86 out what's causing the pain, the pain needs to be treated 00:08:14.89\00:08:17.33 because the damage from the pain is probably more risky 00:08:17.36\00:08:20.73 to you than the treatment for the pain. 00:08:20.76\00:08:22.23 Alright. That makes a lot of sense to me. Yeah! 00:08:22.26\00:08:25.20 Good. Let's talk about what to do when someone's having pain. 00:08:25.23\00:08:29.54 We just talked about the lists, the big lists, the different 00:08:29.57\00:08:31.84 types of pain and who people should see, but I know 00:08:31.87\00:08:34.24 a lot of folks a tendency to just try to tough it out 00:08:34.28\00:08:36.68 and I try and do the same thing, why is that a bad idea? 00:08:36.71\00:08:39.01 Because it could kill you. Ok. You could die if your 00:08:39.05\00:08:42.68 judgment is wrong. 00:08:42.72\00:08:44.19 I've seen people through my life that have chest pain 00:08:44.22\00:08:47.76 they think it's nothing serious, they stay at home, 00:08:47.79\00:08:49.62 they have a heart attack, an arrhythmia and it's over with. 00:08:49.66\00:08:52.06 Half the time, the first time we know that someone's having 00:08:52.09\00:08:55.16 a heart attack is when... I mean we know that they have 00:08:55.20\00:08:58.70 cardiovascular disease is when they have a heart attack. 00:08:58.73\00:09:01.00 It's a disease, and it causes sudden death. 00:09:01.04\00:09:05.61 Let's say they can't breathe, you are all of a sudden 00:09:05.64\00:09:09.08 real short of breath. That could be a "pulmonary embolism". 00:09:09.11\00:09:11.68 Let's say you "pass out", that could be a sign that 00:09:11.71\00:09:15.02 the blood vessel or there is something wrong with your brain. 00:09:15.05\00:09:17.19 Now those aren't pains, but those are dramatic things 00:09:17.22\00:09:20.69 that happen. The severe headache, the worse headache 00:09:20.72\00:09:23.46 in your life, if you can't explain it, that can be a 00:09:23.49\00:09:25.89 broken blood vessel which which could be life threatening. 00:09:25.93\00:09:28.40 all of a sudden your belly starts hurting and it gets 00:09:28.43\00:09:31.93 bigger and you get nauseated. Well that could be a serious 00:09:31.97\00:09:35.27 sign that there is something wrong with your belly. 00:09:35.30\00:09:37.64 So pain in those areas could be a sign that something is 00:09:37.67\00:09:41.84 threatening your life, so unless you know it is not 00:09:41.88\00:09:44.85 life threatening, you need to get to someone to see if it 00:09:44.88\00:09:47.32 is life threatening and give you some advice, 00:09:47.35\00:09:49.58 and help you go from there. So pain happens for a reason. 00:09:49.62\00:09:53.96 Now earlier in the series, we talked about bones and you 00:09:53.99\00:09:58.43 mentioned that bone health, or bone pain specifically, 00:09:58.46\00:10:02.26 can be very, very... one of the worse pains that someone 00:10:02.30\00:10:05.10 could experience. Why is that? 00:10:05.13\00:10:06.47 Well bones have lots of nerves in them, you know, there's 00:10:06.50\00:10:09.24 lots of things going on in the bone and when a bone is broken, 00:10:09.27\00:10:12.17 the nerves, blood supply, that's altered as well. 00:10:12.21\00:10:15.48 Have you ever broken a bone? Not yet! Ok! 00:10:15.51\00:10:18.18 Have you ever chipped a tooth? No, I never actually chipped 00:10:18.21\00:10:22.58 a tooth? You never chipped a tooth. Man, you're not helping 00:10:22.62\00:10:24.29 me out here. Well you know if you chip a tooth or break 00:10:24.32\00:10:27.39 a bone, the air hits the tooth and you know it gives you 00:10:27.42\00:10:30.46 that chilling. The nerve is exposed right away, 00:10:30.49\00:10:32.06 and it makes this terrible pain, 00:10:32.09\00:10:34.00 and some of the worse pain there is is when a malignancy has 00:10:34.03\00:10:38.10 metastasized to the bone and cancer gets in the bones, 00:10:38.13\00:10:42.10 that's some of the worse and severe pain there is. 00:10:42.14\00:10:45.24 Trauma, like a break in the bone, that is severe pain. 00:10:45.27\00:10:48.98 When a point tenders and you touch it here, Ohhh! 00:10:49.01\00:10:52.35 It hurts, well, that's because there is such extreme pain 00:10:52.38\00:10:55.32 and the nerves are so sensitive to bone pains. 00:10:55.35\00:10:57.65 Why those nerves seem to be extra sensitive 00:10:57.69\00:11:00.29 well bones are so important, they are structured 00:11:00.32\00:11:03.22 the backbone of us...You know a little piece of skin that's 00:11:03.26\00:11:06.13 not that important, but if your bone breaks, that's a 00:11:06.16\00:11:08.53 big deal, you know your whole system, that's the foundation, 00:11:08.56\00:11:12.27 that's the boards of your body. Right! So that's a big deal 00:11:12.30\00:11:15.27 with that. Infection in the bones, that seems to hurt 00:11:15.30\00:11:18.24 a whole lot, so those are some of the causes of bone pain 00:11:18.27\00:11:21.98 and bone pain really hurts a lot and we can sort of sort 00:11:22.01\00:11:25.31 through this with modern medicine, 00:11:25.35\00:11:26.68 what's causing that pain. 00:11:26.72\00:11:28.22 Sure. This next question is one I can relate to, 00:11:28.25\00:11:31.15 I've experienced this, and that's a question about 00:11:31.19\00:11:34.72 ligaments and tendons. I've had issues with both personally 00:11:34.76\00:11:38.63 but what are these parts and what are they important, 00:11:38.66\00:11:41.63 and why do they cause pain? 00:11:41.66\00:11:43.03 A lot of people don't know... We talked about the bones 00:11:43.06\00:11:46.47 okay, the bone is sort of the skeleton in the body, 00:11:46.50\00:11:49.54 we look at those with x-rays. Now a ligament, okay, a ligament 00:11:49.57\00:11:56.04 is the tissue that connects the bones to the joint. 00:11:56.08\00:12:00.58 The ligaments, bones to joints okay, and what the tendons 00:12:00.62\00:12:05.55 connect muscles to bones, so I would think of ligaments 00:12:05.59\00:12:09.06 and tendons as things that connect things together. 00:12:09.09\00:12:11.63 Bones to joints, muscles to bones, things of that nature, 00:12:11.66\00:12:16.33 their connectors. If you have had... You know every question 00:12:16.36\00:12:21.47 I have asked you, you said no. You didn't hurt your teeth, 00:12:21.50\00:12:24.97 and you didn't break a bone. Did you ever play with 00:12:25.01\00:12:27.48 Lincoln Logs? Oh sure! And Tinker Toys? Yeah, Um hum. 00:12:27.51\00:12:31.18 Good. Okay. You know how you have those Tinker Toys you have 00:12:31.21\00:12:33.88 these little things that connect things together. Yeah! Yeah! 00:12:33.92\00:12:36.28 Sort of like ligaments and muscles. Well whenever these 00:12:36.32\00:12:40.12 things get lax, the bone is affected because the bone 00:12:40.16\00:12:44.33 changes position and you could have inflammation when all this 00:12:44.36\00:12:47.70 happens. The bone because of repetitive movement, 00:12:47.73\00:12:50.53 you know remember all these move the body and design us 00:12:50.57\00:12:53.34 to even do things. If they are not in the right positions 00:12:53.37\00:12:56.30 guess what else is not in the right position? 00:12:56.34\00:12:58.47 Mr. Nerves too. The nerves too. The nerve goes hey, 00:12:58.51\00:13:02.44 this ligament and this tendon are laxed, they are inflamed 00:13:02.48\00:13:05.91 the bone is not happy, I am not happy either. 00:13:05.95\00:13:08.52 So I am going to tell Mr. Brain that I am having pain and 00:13:08.55\00:13:11.45 something's wrong. Right! 00:13:11.49\00:13:12.99 So those are some things that all sort of involve 00:13:13.02\00:13:15.59 and it is so critical because that's what keeps us moving 00:13:15.62\00:13:18.03 and doing things. That sort of like the backbone of our bodies 00:13:18.06\00:13:21.26 so that's what ligaments and tendons that connect bones 00:13:21.30\00:13:24.00 and muscles to different things. 00:13:24.03\00:13:25.37 Alright!
