00:00:12.71\00:00:14.04 Yeah. 00:00:14.08\00:00:15.41 But we're gonna get to some questions 00:00:15.44\00:00:16.85 that people asked in just a moment. 00:00:16.88\00:00:18.91 But I want to talk to you 00:00:18.95\00:00:20.28 about something that is weighing on my mind. 00:00:20.32\00:00:22.32 Okay. 00:00:22.35\00:00:23.69 I've got a couple of young boys and having a couple of kids 00:00:23.72\00:00:26.49 just makes realize I'm getting older. 00:00:26.52\00:00:27.86 Yep. 00:00:27.89\00:00:29.22 And when I get even older, 00:00:29.26\00:00:30.59 say my grandparents' age 00:00:30.63\00:00:32.63 things are going to be lot different in my body. 00:00:32.66\00:00:34.16 Right. 00:00:34.20\00:00:35.53 And hopefully my mind and my body 00:00:35.56\00:00:36.90 age at the same rate, 00:00:36.93\00:00:38.27 so I know what's appropriate to be doing and what's not. 00:00:38.30\00:00:40.50 But as we get older we need to manage risk 00:00:40.54\00:00:42.67 because getting up on ladders 00:00:42.70\00:00:44.14 or climbing up on the roof 00:00:44.17\00:00:45.67 at older ages like you mentioned. 00:00:45.71\00:00:47.04 Yeah. 00:00:47.08\00:00:48.41 It's kind of a not so smart thing to do, right? 00:00:48.44\00:00:49.78 No, we've talked about how our joints just wear out. 00:00:49.81\00:00:53.05 And everything at the body gets older 00:00:53.08\00:00:55.12 at a different speed. 00:00:55.15\00:00:56.48 Right. 00:00:56.52\00:00:57.85 And one of the key things, 00:00:57.89\00:00:59.22 we have to keep our joints healthy as we age 00:00:59.25\00:01:01.02 because that's one of the key things 00:01:01.06\00:01:02.66 for our independence. 00:01:02.69\00:01:04.63 You know when the hips and the knees, 00:01:04.66\00:01:06.43 we can't move to the bathroom, 00:01:06.46\00:01:08.06 we can't take care of ourselves, 00:01:08.10\00:01:09.43 guess what happens. 00:01:09.46\00:01:10.80 We have then someone else take care of us. 00:01:10.83\00:01:12.67 So that's why joint care is so crucial at younger ages. 00:01:12.70\00:01:17.04 Some people say, oh, I don't have to do anything, 00:01:17.07\00:01:18.57 you know. 00:01:18.61\00:01:19.94 I can abuse my joints 00:01:19.97\00:01:21.31 when I'm younger and when I get older 00:01:21.34\00:01:23.24 and those are the people 00:01:23.28\00:01:24.61 that end up not being able to move 00:01:24.65\00:01:25.98 when they're older. 00:01:26.01\00:01:27.35 It's right. 00:01:27.38\00:01:28.72 And heavy people, 00:01:28.75\00:01:30.09 you just can't lug them to the bathroom 00:01:30.12\00:01:31.45 if they can't move their parts. 00:01:31.49\00:01:32.82 So joint care is very important 00:01:32.85\00:01:34.96 in realizing being smart enough 00:01:34.99\00:01:37.03 and wise enough to say listen, 00:01:37.06\00:01:38.39 I have to take care of my joints 00:01:38.43\00:01:39.76 not only when I'm young but when I'm older, 00:01:39.79\00:01:42.46 I might have to take care of them even more. 00:01:42.50\00:01:44.77 And some people don't like to have extra things 00:01:44.80\00:01:47.14 to help their joints do better. 00:01:47.17\00:01:48.60 But I would rather have someone have a walking stick 00:01:48.64\00:01:52.17 or a walker and move around slowly 00:01:52.21\00:01:54.94 than sit in a chair all day and not move at all. 00:01:54.98\00:01:56.88 Yeah. 00:01:56.91\00:01:58.25 'Cause movement is very helpful, 00:01:58.28\00:01:59.61 keep the joints going. 00:01:59.65\00:02:00.98 But sometimes people have to be more careful, 00:02:01.02\00:02:03.49 I mean, some people I've noticed as they age 00:02:03.52\00:02:05.92 and I take care of a lot of people 00:02:05.95\00:02:07.29 between 85 and 100. 00:02:07.32\00:02:09.19 They say well, I can't walk as much 00:02:09.22\00:02:11.59 and I'm embarrassed to have a walker or a device. 00:02:11.63\00:02:15.16 Well, I would rather them do that 00:02:15.20\00:02:16.53 and keep moving than sitting at in a chair all day. 00:02:16.56\00:02:19.27 Yeah. 00:02:19.30\00:02:20.64 We know that sitting would, you know, 00:02:20.67\00:02:22.17 causes the joints to get older quicker 00:02:22.20\00:02:24.64 and keeps us from moving as well 00:02:24.67\00:02:26.57 and we lose our independence sooner. 00:02:26.61\00:02:28.41 So I'd rather have someone get up 00:02:28.44\00:02:29.78 and move every hour 00:02:29.81\00:02:31.15 and protect their joints 00:02:31.18\00:02:32.51 by either a cane or walking stick. 00:02:32.55\00:02:34.45 Some joints need stability with a device 00:02:34.48\00:02:37.35 to help stabilize that joint as we get older. 00:02:37.39\00:02:40.66 But that's important for older people to do that, 00:02:40.69\00:02:43.96 but it's also important for the younger persons 00:02:43.99\00:02:45.89 like you're to start thinking 00:02:45.93\00:02:47.26 about what can I do now to protect my joints 00:02:47.30\00:02:50.03 when I get older. 00:02:50.07\00:02:51.40 Right. 00:02:51.43\00:02:52.77 That's like not doing crazy things 00:02:52.80\00:02:54.14 like we've talked about 00:02:54.17\00:02:55.50 like if you're tennis player 00:02:55.54\00:02:56.87 and this is wearing out will quit wearing it out. 00:02:56.91\00:02:58.24 Yeah. 00:02:58.27\00:02:59.61 You know, if you're a football player 00:02:59.64\00:03:00.98 and you're wearing out your knees, 00:03:01.01\00:03:02.34 maybe you should quit playing football. 00:03:02.38\00:03:04.71 Long distance runners you know, 00:03:04.75\00:03:06.65 God never made us to pound the knees and hips 00:03:06.68\00:03:09.78 26 miles at a time. 00:03:09.82\00:03:12.45 Long term of that if we don't take 00:03:12.49\00:03:14.32 really good care of hydration 00:03:14.36\00:03:16.42 and we don't get really good nourishment, 00:03:16.46\00:03:18.39 if we don't listen to our bodies, 00:03:18.43\00:03:19.89 we can wear out those joints as well. 00:03:19.93\00:03:21.86 Yeah. 00:03:21.90\00:03:23.23 And it's a balance, Nick, because exercise is good, 00:03:23.26\00:03:25.43 it brings nutrition to the joint, 00:03:25.47\00:03:27.14 it brings oxygen to the joint, 00:03:27.17\00:03:28.50 it helps keep the muscle and other functions well, 00:03:28.54\00:03:31.14 but you can literally wear out a joint if you overdo it. 00:03:31.17\00:03:34.91 So I think that's a very important 00:03:34.94\00:03:36.38 both for the young as you think 00:03:36.41\00:03:37.75 about this as you get older and also... 00:03:37.78\00:03:41.35 I have problems with my feet joints, okay. 00:03:41.38\00:03:43.95 And I know if I do too much, guess what's going to happen? 00:03:43.99\00:03:47.02 I'm going to be out of it for long time on a boot, 00:03:47.06\00:03:49.52 I mean, because I've lot of inflammation 00:03:49.56\00:03:51.69 plantar fasciitis. 00:03:51.73\00:03:53.06 So I've learned to be smart about that 00:03:53.09\00:03:54.50 so I can do the basics. 00:03:54.53\00:03:56.03 Right, yeah. 00:03:56.06\00:03:57.40 And like for instance 00:03:57.43\00:03:58.77 last night I was doing some exercise. 00:03:58.80\00:04:01.14 Well, rather than running on my joints 00:04:01.17\00:04:03.24 I decided to sit and do a bicycle 00:04:03.27\00:04:05.97 where I didn't put as much pressure. 00:04:06.01\00:04:07.74 Where I might swim you know, 00:04:07.78\00:04:09.71 that's being intelligent about taking care of your joints 00:04:09.74\00:04:12.75 because you don't want to wear them out. 00:04:12.78\00:04:14.12 That's right. 00:04:14.15\00:04:15.48 And that's something I had to do recently too, I used to, 00:04:15.52\00:04:17.02 you know, the decade ago I was doing activities 00:04:17.05\00:04:18.85 that were much more high impact on my knees and what not, 00:04:18.89\00:04:20.86 but I've to eliminate those because I've decided you know, 00:04:20.89\00:04:22.59 it's just really not worth in. 00:04:22.62\00:04:23.96 Yeah, it's not worth in. All right. 00:05:03.47\00:05:04.80 Well, our first viewer writes in and asks 00:05:04.83\00:05:06.17 what is the most common cause of joint problem? 00:05:06.20\00:05:08.37 Okay, I think we've covered that pretty much all ready. 00:05:08.40\00:05:11.21 Trauma, overuse, 00:05:11.24\00:05:15.28 here is one that we don't like to talk about too much, 00:05:15.31\00:05:17.38 if you carry too much weight on the body, 00:05:17.41\00:05:19.81 it wears out the joints sooner. 00:05:19.85\00:05:21.18 Right. 00:05:21.22\00:05:22.55 So just getting down to ideal body weight. 00:05:22.58\00:05:24.09 I mean other cause of joint problems is inactivity, 00:05:24.12\00:05:26.79 not using the joint, 00:05:26.82\00:05:28.16 if you don't use it, you lose it, 00:05:28.19\00:05:30.13 and lack of stretching. 00:05:30.16\00:05:31.66 People don't warm up 00:05:31.69\00:05:33.03 before they get the joint moving, 00:05:33.06\00:05:35.03 especially as we age we need more stretching, 00:05:35.06\00:05:38.23 more moving of the joints on a regular basis, 00:05:38.27\00:05:40.54 we have to protect it more, 00:05:40.57\00:05:41.90 but those are the most common causes of joint. 00:05:41.94\00:05:43.71 Trauma, too much weight, 00:05:43.74\00:05:45.31 not moving and not stretching enough. 00:05:45.34\00:05:47.88 All right, we got another question here. 00:05:47.91\00:05:49.74 What is the difference between 00:05:49.78\00:05:51.11 osteoarthritis and rheumatoid arthritis? 00:05:51.15\00:05:53.52 You touched on that earlier but... 00:05:53.55\00:05:54.88 Now, arthritis just means inflammation of a joint. 00:05:54.92\00:05:58.39 So anything that can have inflammation of a joint 00:05:58.42\00:06:01.62 is a cut off, right, sometimes it can be bones, 00:06:01.66\00:06:03.83 sometimes it could be connective tissue, 00:06:03.86\00:06:05.69 different things have inflammation of the joint. 00:06:05.73\00:06:07.86 Right. 00:06:07.90\00:06:09.23 Osteoarthritis is just usually from the wear and tear, okay. 00:06:09.26\00:06:13.13 And we do that with the history, 00:06:13.17\00:06:15.87 we can actually take x-rays and we look at the x-rays 00:06:15.90\00:06:18.97 and they can, here we can notice 00:06:19.01\00:06:20.34 that the joints space is becoming less, 00:06:20.38\00:06:22.94 we look for things called osteophytes on the bones, 00:06:22.98\00:06:27.22 different bone cyst, different things like that. 00:06:27.25\00:06:29.85 Very common to have osteoarthritis in the fingers, 00:06:29.88\00:06:33.22 knees, hips and spines. 00:06:33.25\00:06:35.22 We have pain and stiffness, decrease range of motion. 00:06:35.26\00:06:40.40 Morning stiffness are very common. 00:06:40.43\00:06:43.03 Now rheumatoid arthritis is when the body attack itself. 00:06:43.06\00:06:47.97 And usually you can have some of the same type of symptoms 00:06:48.00\00:06:51.27 but you also notice 00:06:51.31\00:06:52.64 that when you look at the blood, 00:06:52.67\00:06:54.01 you notice some inflammatory markers 00:06:54.04\00:06:55.74 where the body is actually attacking itself. 00:06:55.78\00:06:58.01 So it can have some of the similar symptoms of it 00:06:58.05\00:07:01.08 but the cause of it is different. 00:07:01.12\00:07:02.52 One is the body's wearing out, 00:07:02.55\00:07:04.32 the other one's the body is actually attacking itself. 00:07:04.35\00:07:07.06 So what causes the body to attack itself 00:07:07.09\00:07:08.86 as in the case of rheumatoid arthritis? 00:07:08.89\00:07:10.23 It can be lots of things 00:07:10.26\00:07:11.59 and lot of times we don't even know, 00:07:11.63\00:07:13.56 if we know what was causing rheumatoid arthritis 00:07:13.60\00:07:15.40 we would treat it, but it's my theory 00:07:15.43\00:07:17.20 that lot of these things 00:07:17.23\00:07:18.57 when the body attacks itself is its genetics 00:07:18.60\00:07:21.50 or we bring in some type of antigen 00:07:21.54\00:07:23.71 in the body 00:07:23.74\00:07:25.07 that the body doesn't know what to do with it. 00:07:25.11\00:07:26.94 And certain bodies say, I don't know, what is this? 00:07:26.98\00:07:29.41 And then it attacks the joint, it attacks what's there, 00:07:29.44\00:07:33.18 and it does run in families, arthritis does. 00:07:33.21\00:07:35.48 All right, let's talk about another issue. 00:07:35.52\00:07:38.79 Where is the place for physical therapy 00:07:38.82\00:07:40.32 when it comes to joint pain? 00:07:40.36\00:07:41.69 I mean, I know I've visited a physical therapist 00:07:41.72\00:07:43.46 for a lot of different pains 00:07:43.49\00:07:44.83 and it's been very beneficial... 00:07:44.86\00:07:46.19 Yeah. 00:07:46.23\00:07:47.56 Does it help with joints? Yes, it helps with joint pains. 00:07:47.60\00:07:49.03 It's something that I would recommend, 00:07:49.06\00:07:50.60 you see a physical therapist 00:07:50.63\00:07:51.97 because they help correct posturing, 00:07:52.00\00:07:54.47 You might not be moving correctly, 00:07:54.50\00:07:56.57 it's not uncommon 00:07:56.60\00:07:57.94 if you're favoring one part that has a problem 00:07:57.97\00:07:59.97 that the muscles on the other side overdo it. 00:08:00.01\00:08:02.34 Muscle imbalance. Yes, muscle imbalances. 00:08:02.38\00:08:04.68 The physical therapist can train you 00:08:04.71\00:08:06.75 to do specific exercises 00:08:06.78\00:08:08.68 that helps specific muscle groups. 00:08:08.72\00:08:10.05 Right. 00:08:10.09\00:08:11.42 Now I know you can go to the gym and do exercises 00:08:11.45\00:08:13.25 but sometimes those exercises might not be effective 00:08:13.29\00:08:16.09 where you might need more time on this than that, 00:08:16.12\00:08:18.29 because remember we're not trying to just get strong, 00:08:18.33\00:08:20.40 we're trying to treat a joint problem. 00:08:20.43\00:08:22.00 You can make the muscle imbalance worse 00:08:22.03\00:08:23.93 rather than equalizing it I guess. 00:08:23.97\00:08:25.73 Yeah, I think physical therapist are... 00:08:25.77\00:08:27.77 If you're having joint problems, 00:08:27.80\00:08:29.14 I think everyone should see a good physical therapist 00:08:29.17\00:08:31.47 and get evaluated and see the things 00:08:31.51\00:08:33.07 that they can do 'cause you know, 00:08:33.11\00:08:34.44 most of the things that they do, 00:08:34.48\00:08:35.81 they don't have lots of side effects, 00:08:35.84\00:08:37.58 they get to causes or problems and help your body heal itself. 00:08:37.61\00:08:40.72 Yeah, and a good physical therapist, 00:08:40.75\00:08:42.28 they will work with you for a number of weeks 00:08:42.32\00:08:43.65 and then say, you know what, 00:08:43.69\00:08:45.02 if we're not seeing an improvement, 00:08:45.05\00:08:46.39 you know, we need to send you on to someone else, right? 00:08:46.42\00:08:48.09 Yeah, exactly. They know what to do. 00:08:48.12\00:08:49.99 All right, 00:08:50.03\00:08:51.36 so we talked a little bit about this already, 00:08:51.39\00:08:53.23 but how do we prevent joint problems? 00:08:53.26\00:08:55.13 Well, that's a great question. 00:08:55.16\00:08:56.83 I guess one of the most common things is stretching. 00:08:56.87\00:09:00.37 If you sit at a desk every hour move it, 00:09:00.40\00:09:03.20 move every part every hour. 00:09:03.24\00:09:04.84 You know, we should get up and move everything every hour. 00:09:04.87\00:09:07.58 You know, if you don't use it, you'll lose it. 00:09:07.61\00:09:08.94 Right. 00:09:08.98\00:09:10.31 If you don't use it, the parts can get stiff, 00:09:10.35\00:09:12.58 especially if the parts been damaged before. 00:09:12.61\00:09:15.38 So damage part gets stiff a lot quicker 00:09:15.42\00:09:17.49 when you don't move and that can be very painful, 00:09:17.52\00:09:20.16 when they don't move enough. 00:09:20.19\00:09:21.52 So that will help it but what moving does it, 00:09:21.56\00:09:23.36 guess what moving does? 00:09:23.39\00:09:24.73 It brings a new blood supply into the area, 00:09:24.76\00:09:26.66 gets more oxygen to the joints gets, 00:09:26.70\00:09:28.90 you know, some of these joints 00:09:28.93\00:09:30.43 don't have a very big blood supply, 00:09:30.47\00:09:32.97 so just moving increases the blood supply. 00:09:33.00\00:09:36.91 Other things to prevent joint problems 00:09:36.94\00:09:38.37 if you have a weak joint you might want supports. 00:09:38.41\00:09:40.98 Some people wear braces, if it's a foot you know, 00:09:41.01\00:09:44.31 some people wear supportive shoes, 00:09:44.35\00:09:47.05 on the knees you can have braces, 00:09:47.08\00:09:49.15 on the hips you can move with walkers or pads, 00:09:49.18\00:09:53.36 of course we have braces for the joints here, the elbow. 00:09:53.39\00:09:58.19 And these braces are usually just for a short term... 00:09:58.23\00:10:00.50 Hopefully. Right? 00:10:00.53\00:10:01.86 But sometimes people have to wear them long term 00:10:01.90\00:10:03.26 if the joint is irreparably damaged. 00:10:03.30\00:10:05.87 Of course, hydration is so important, 00:10:05.90\00:10:08.37 good nutrition is important and also eating healthy foods, 00:10:08.40\00:10:11.97 you know, healthy food 00:10:12.01\00:10:13.34 is important to how the whole body works, 00:10:13.38\00:10:15.48 so these are some of few things 00:10:15.51\00:10:16.91 that people can do to prevent joint problems. 00:10:16.95\00:10:19.75 Now what about medications? 00:10:19.78\00:10:21.12 So there are medications that can damage the joints 00:10:21.15\00:10:22.65 or there are some that could help? 00:10:22.68\00:10:24.02 Well, you know, it's a query, if you have a joint... 00:10:24.05\00:10:27.02 Certain medications can cause 00:10:27.06\00:10:28.62 side effects to the joints, okay. 00:10:28.66\00:10:30.39 It sort of rare but it could be. 00:10:30.43\00:10:33.16 But certain medications help with joint, 00:10:33.19\00:10:35.43 of course if it's infections you need antibiotics, 00:10:35.46\00:10:37.90 if it's a bacterial infection, if there is inflammation, 00:10:37.93\00:10:41.00 sometimes you need a short course of medicines 00:10:41.04\00:10:43.27 to settle down the inflammation to give the joint time to heal. 00:10:43.30\00:10:46.71 Okay, right. 00:10:46.74\00:10:48.08 So sometimes there is a need for that 00:10:48.11\00:10:49.48 as long as a risk that will worth the benefit. 00:10:49.51\00:10:51.91 And then like we talk today 00:10:51.95\00:10:53.72 surgery is usually sort of a last resort. 00:10:53.75\00:10:56.55 Right, right. 00:10:56.58\00:10:57.92 Now here is an easy one for you. 00:10:57.95\00:10:59.29 You've had a situation similar to this I think, 00:10:59.32\00:11:00.92 how do you know if you have a rotator cuff problem?
