Yeah. 00:01:22.48\00:01:24.18 How many joints, do you know? Well, we have lots. 00:01:24.21\00:01:26.21 There are lots of them. We have joints in our hands. 00:01:26.25\00:01:28.42 You know, in our fingers, our wrists, our elbows, 00:01:28.45\00:01:31.79 our shoulders, our hips, and the knees. 00:01:31.82\00:01:35.32 Those are the most common. 00:01:35.36\00:01:36.69 We also have joints in our feet. 00:01:36.73\00:01:38.33 We've already spoken about the feet pain. 00:01:38.36\00:01:39.83 Yeah. 00:01:39.86\00:01:41.20 And getting a diagnosis on joint pain 00:01:41.23\00:01:43.77 is very important, you know. 00:01:43.80\00:01:46.70 Probably the most common causes of joint pain, 00:01:46.74\00:01:49.70 regardless of the joint is trauma and overuse. 00:01:49.74\00:01:54.74 When was the last time you had a problem 00:01:54.78\00:01:56.78 with one of your joints? 00:01:56.81\00:01:58.15 And was it trauma or overuse? 00:01:58.18\00:02:00.25 You know, recently I have had some knee pain going on 00:02:00.28\00:02:04.29 and there's, it's a trauma from long time ago. 00:02:04.32\00:02:07.26 Okay. Yeah. 00:02:07.29\00:02:08.62 And when we have trauma to a joint, 00:02:08.66\00:02:11.19 of course if it's just localized trauma 00:02:11.23\00:02:13.93 in the joint area and is made of muscles 00:02:13.96\00:02:16.23 and ligaments and bones. 00:02:16.26\00:02:17.83 If it's localized and it's not too severe, 00:02:17.87\00:02:20.24 usually just immobilization and it goes away. 00:02:20.27\00:02:23.14 For instance, you broke your arm, 00:02:23.17\00:02:24.81 you put it in a cast, 00:02:24.84\00:02:26.17 you wouldn't keep throwing fast balls. 00:02:26.21\00:02:27.54 That's right. 00:02:27.58\00:02:28.91 So let's say you have, 00:02:28.94\00:02:30.28 you know, a problem in your knee. 00:02:30.31\00:02:31.65 Would you keep running races? 00:02:31.68\00:02:33.18 No. No, you would let it heal. 00:02:33.21\00:02:34.55 Yes. 00:02:34.58\00:02:35.92 Because pain tells you that something's not wrong. 00:02:35.95\00:02:38.85 Something's not right. Something's not right. 00:02:38.89\00:02:40.39 Not something's not wrong. 00:02:40.42\00:02:41.76 I need to learn to speak English 00:02:41.79\00:02:43.12 on these programs. 00:02:43.16\00:02:44.49 But anyway, so if the pain is bothering you, 00:02:44.53\00:02:47.30 and it doesn't go away, and you're mobilizing it, 00:02:47.33\00:02:50.03 that could tell you 00:02:50.07\00:02:51.40 that there's a structural problem 00:02:51.43\00:02:52.77 with the joint. 00:02:52.80\00:02:54.24 And that could be a sign 00:02:54.27\00:02:55.60 that you need to see a specialist. 00:02:55.64\00:02:57.37 And when you go, 00:02:57.41\00:02:58.74 they'll do some examining on you. 00:02:58.77\00:03:00.24 Sometimes they can figure it out with 00:03:00.28\00:03:01.71 which is examining the joint 00:03:01.74\00:03:03.08 to see if there's cartilage problem with that. 00:03:03.11\00:03:04.61 Right. 00:03:04.65\00:03:05.98 Usually they'll need to do an x-ray to see if the bones, 00:03:06.01\00:03:08.32 x-rays are great to see if the bones are damaged. 00:03:08.35\00:03:10.42 Right. 00:03:10.45\00:03:11.89 X-rays don't really look at the soft tissue 00:03:11.92\00:03:13.86 and the connection. 00:03:13.89\00:03:15.22 Sometimes they'll have to get more information 00:03:15.26\00:03:16.83 and they might order an MRI. 00:03:16.86\00:03:18.96 Now can an MRI see the ligaments 00:03:18.99\00:03:20.40 and the tendons and all that? 00:03:20.43\00:03:21.76 Yeah, it can see a lots of stuff. 00:03:21.80\00:03:23.13 Okay. 00:03:23.16\00:03:24.50 And if you get used to looking at them, 00:03:24.53\00:03:25.87 you can tell what's normal and what's abnormal. 00:03:25.90\00:03:28.17 Sometimes in a need, 00:03:28.20\00:03:29.60 they might have to do a procedure 00:03:29.64\00:03:31.64 called an arthroscopy, 00:03:31.67\00:03:33.98 where that's actually they can go inside 00:03:34.01\00:03:35.91 and see what's going on in the joint. 00:03:35.94\00:03:38.18 And if you get you can see 00:03:38.21\00:03:39.55 whether things are where they should be, 00:03:39.58\00:03:41.22 if there's a tear, 00:03:41.25\00:03:42.58 if there's a break of all these different causes. 00:03:42.62\00:03:44.95 So the two most common causes are trauma and overuse. 00:03:44.99\00:03:50.63 Trauma, you know, 00:03:50.66\00:03:51.99 I had some traumatic thing one time to my shoulder, 00:03:52.03\00:03:54.43 and I had what they call a SLAP tear. 00:03:54.46\00:03:56.50 Actually I had a tear in one of my ligaments. 00:03:56.53\00:03:59.67 And what happened was, 00:03:59.70\00:04:01.04 I was trying to fix my car, okay, 00:04:01.07\00:04:03.17 and I was trying to lift it up one day. 00:04:03.20\00:04:05.07 And I wasn't strong enough to lift up the car 00:04:05.11\00:04:06.88 and it tore my shoulder. 00:04:06.91\00:04:08.24 Okay. 00:04:08.28\00:04:09.61 So I went to an orthopedic doctor 00:04:09.64\00:04:10.98 because it didn't go away. 00:04:11.01\00:04:12.35 It kept bothering me. 00:04:12.38\00:04:13.72 In fact I couldn't sleep well at night. 00:04:13.75\00:04:15.08 And that's one problem 00:04:15.12\00:04:16.45 if your joints keeping you from sleeping. 00:04:16.48\00:04:17.82 that's a pretty significant problem. 00:04:17.85\00:04:19.19 Yeah. 00:04:19.22\00:04:20.56 If a joint swelling and it won't go away, 00:04:20.59\00:04:21.92 that's a significant problem. 00:04:21.96\00:04:23.73 If you're having a fever with a joint, 00:04:23.76\00:04:25.73 that's a very significant problem. 00:04:25.76\00:04:28.36 So anyway I figured out, you know, 00:04:28.40\00:04:29.90 this is not the way I want to go. 00:04:29.93\00:04:31.90 So I went and saw the orthopedic, 00:04:31.93\00:04:33.27 and of course he did his testing and he says, 00:04:33.30\00:04:35.80 "you have two options," okay, 00:04:35.84\00:04:37.71 "you can either do surgery on it" okay, 00:04:37.74\00:04:40.18 "or you can do, learn these exercises 00:04:40.21\00:04:42.38 which will help it heal." 00:04:42.41\00:04:43.75 Some physical therapy. Right. 00:04:43.78\00:04:45.11 So I went to the physical therapy, 00:04:45.15\00:04:46.48 did my exercise faithfully and guess what, 00:04:46.51\00:04:48.75 my shoulder healed up on its own. 00:04:48.78\00:04:50.12 Yeah, that's great. So it really worked well. 00:04:50.15\00:04:52.95 What are some other, 00:04:52.99\00:04:54.32 there's other causes besides trauma 00:04:54.36\00:04:55.86 and overuse on our joints. 00:04:55.89\00:04:57.56 Okay. 00:04:57.59\00:04:58.93 Sometimes we can have the tendons, 00:04:58.96\00:05:00.30 the tendons get inflamed. 00:05:00.33\00:05:01.90 We call that tendonitis. 00:05:01.93\00:05:04.17 That could be overused usually things of that. 00:05:04.20\00:05:06.23 Sometimes we can get the bursa, these are sacks of fluid. 00:05:06.27\00:05:09.70 Sometimes those could be at play, 00:05:09.74\00:05:11.07 we call that bursitis, okay. 00:05:11.11\00:05:14.68 There's also things like we can have different diseases, 00:05:14.71\00:05:17.71 disease states that can affect the joints. 00:05:17.75\00:05:20.42 And this is usually the disease states 00:05:20.45\00:05:21.95 where the body attacks itself. 00:05:21.98\00:05:24.05 We call that autoimmune diseases. 00:05:24.09\00:05:26.65 The most common one 00:05:26.69\00:05:28.02 is probably rheumatoid arthritis. 00:05:28.06\00:05:29.99 The body attacks itself. 00:05:30.03\00:05:31.83 But other conditions 00:05:31.86\00:05:33.19 where the body can attack the different joints, 00:05:33.23\00:05:34.73 things like lupus, the body can attack. 00:05:34.76\00:05:37.63 There's another condition called scleroderma. 00:05:37.67\00:05:40.57 One called polyarteritis nodosa. 00:05:40.60\00:05:44.37 Sometimes even infections can cause damage to the joints. 00:05:44.41\00:05:49.11 Sometimes we get infections from a tick. 00:05:49.14\00:05:52.11 You know, Lyme disease and that can hurt our joints. 00:05:52.15\00:05:55.32 Other different joints can be infected by infections as well. 00:05:55.35\00:05:59.75 That's probably the most serious acute cause 00:05:59.79\00:06:02.32 of joint pain. 00:06:02.36\00:06:03.69 So if you're having joint pain 00:06:03.73\00:06:05.06 that didn't have trauma and overuse, 00:06:05.09\00:06:07.16 and you've never had a chronic disease, 00:06:07.20\00:06:09.06 that's something you need to get evaluated 00:06:09.10\00:06:10.57 to make sure you don't have an infection. 00:06:10.60\00:06:13.17 That's a very serious cause of joint pain. 00:06:13.20\00:06:16.47 Sometimes you can have, 00:06:16.50\00:06:17.84 we've talked before about metabolism being off 00:06:17.87\00:06:20.18 and sometimes if your metabolism is off, 00:06:20.21\00:06:22.31 you can make extra stones or crystals. 00:06:22.34\00:06:24.61 Okay. Okay. 00:06:24.65\00:06:25.98 Everyone's heard of GALT. 00:06:26.01\00:06:27.52 GALT sometimes make extra crystals, 00:06:27.55\00:06:29.45 they can get deposited in joints 00:06:29.48\00:06:31.49 and cause pain as well. 00:06:31.52\00:06:33.32 Certain medications can cause joint problems, 00:06:33.36\00:06:36.83 as side effects. 00:06:36.86\00:06:38.19 Chemotherapeutic agents are very common, penicillin, 00:06:38.23\00:06:41.93 certain other agents like chemotherapy 00:06:41.96\00:06:45.03 that can also cause it. 00:06:45.07\00:06:46.40 Right. 00:06:46.43\00:06:47.77 So when, so we want to know, 00:06:47.80\00:06:49.14 you know, these are some general terms. 00:06:49.17\00:06:50.94 When a person comes in with a joint pain, 00:06:50.97\00:06:53.34 one of the first thing the doctor wants to know, 00:06:53.38\00:06:54.98 is this something that's happened overtime 00:06:55.01\00:06:57.38 or have you, it's something that's happened right away. 00:06:57.41\00:06:59.68 Right. 00:06:59.71\00:07:01.05 So he thinks about that he says, well, 00:07:01.08\00:07:02.45 if it happened right away, 00:07:02.48\00:07:03.89 then he's going to go through is it trauma, is it overuse? 00:07:03.92\00:07:07.42 Could it be an infection? 00:07:07.46\00:07:09.39 Could it be one of these rheumatologic diseases? 00:07:09.42\00:07:12.46 What's gonna happen? Right. 00:07:12.49\00:07:14.33 Or then he might say, well, if it didn't happen 00:07:14.36\00:07:16.06 suddenly like that 00:07:16.10\00:07:17.43 if it's happened slowly overtime, 00:07:17.47\00:07:20.14 chronic causes. 00:07:20.17\00:07:21.77 And one of the most common causes 00:07:21.80\00:07:23.51 of chronic joint pain is just guess what, we get old. 00:07:23.54\00:07:27.58 You're right, right. 00:07:27.61\00:07:28.94 And when we get old, things wear out. 00:07:28.98\00:07:31.51 The cartilage wears out, the ligaments wear out, 00:07:31.55\00:07:33.88 things get wear out. 00:07:33.92\00:07:35.25 They don't work as good. 00:07:35.28\00:07:36.62 That's why it's so important for our joints 00:07:36.65\00:07:39.19 to keep that from happening by using our joints 00:07:39.22\00:07:41.76 when we're young appropriately. 00:07:41.79\00:07:43.59 Okay. 00:07:43.63\00:07:44.96 You know, prevent joint problems 00:07:44.99\00:07:46.33 you know, that's why stretching, and exercise, 00:07:46.36\00:07:48.90 and good nutrition, and water, 00:07:48.93\00:07:51.33 and not doing crazy things is important. 00:07:51.37\00:07:54.17 Now crazy things... Yeah. 00:07:54.20\00:07:55.67 Let's qualify for that minute. What are we talking about? 00:07:55.70\00:07:58.27 Well, you know, 00:07:58.31\00:07:59.87 sometimes the brain is younger than the body. 00:07:59.91\00:08:02.74 Okay. Okay. 00:08:02.78\00:08:04.11 I had a 94 year old fellow come in, 00:08:04.15\00:08:06.51 his brain was 64, okay. 00:08:06.55\00:08:08.48 Good for him, right. And his joints though were 94. 00:08:08.52\00:08:11.55 And he decided, he could get up on his, 00:08:11.59\00:08:14.06 is his house and clean the leaves in the gutter. 00:08:14.09\00:08:16.83 His brain said he could do that. 00:08:16.86\00:08:18.46 Probably not a good point in your life to be doing that. 00:08:18.49\00:08:20.43 So he went up and did that and guess what happened? 00:08:20.46\00:08:23.37 He fell off the ladder and broke his pelvis. 00:08:23.40\00:08:25.30 Oh, that's bad. Okay, he almost died from that. 00:08:25.33\00:08:27.54 Wow. Okay. 00:08:27.57\00:08:28.90 So I will say that that is a crazy thing to do. 00:08:28.94\00:08:32.01 There are some risk takers, 00:08:32.04\00:08:33.68 younger people that do all sorts of things. 00:08:33.71\00:08:35.48 They jump off cliffs and parachutes 00:08:35.51\00:08:37.71 and they live for the thrill. 00:08:37.75\00:08:39.21 Sure. Okay. 00:08:39.25\00:08:40.58 Well, sometimes that can put strain on the joints 00:08:40.62\00:08:43.39 quite a bit. 00:08:43.42\00:08:44.75 Sometimes you can have damages and injuries. 00:08:44.79\00:08:46.55 If you look at the football players, 00:08:46.59\00:08:48.09 their joints what they go through. 00:08:48.12\00:08:49.66 Yeah. 00:08:49.69\00:08:51.03 Sometimes there is this common sense says, 00:08:51.06\00:08:52.39 our joints aren't made to do that. 00:08:52.43\00:08:53.90 And if we, if we abuse them at a younger age, 00:08:53.93\00:08:57.97 when we get older they wear out quicker. 00:08:58.00\00:09:00.27 We really experience those consequences really, 00:09:00.30\00:09:02.60 really badly in our old age. 00:09:02.64\00:09:03.97 Your joints get better mileage if you go at 40 instead of 80. 00:09:04.01\00:09:07.94 Yeah. 00:09:07.98\00:09:09.31 And just common things that wear out joints 00:09:09.34\00:09:10.75 is besides overuse and doing crazy things, 00:09:10.78\00:09:12.91 as not drinking enough water, not getting good nutrition. 00:09:12.95\00:09:16.75 These are the basics of joint care. 00:09:16.79\00:09:19.09 But so chronic it might, 00:09:19.12\00:09:20.46 the most common is it just wears out overtime. 00:09:20.49\00:09:22.99 The cartilage wears out, these things get old, 00:09:23.02\00:09:25.53 but other diseases that can do that. 00:09:25.56\00:09:28.13 Arthritis is probably the most common. 00:09:28.16\00:09:30.57 Arthritis over long periods of time, 00:09:30.60\00:09:32.70 either rheumatoid arthritis or osteoarthritis. 00:09:32.73\00:09:36.57 Osteoarthritis is just the body wearing out basically. 00:09:36.60\00:09:39.61 Rheumatoid is where the body attacks the joints. 00:09:39.64\00:09:42.44 And those are the things 00:09:42.48\00:09:43.81 that happen over long periods of time 00:09:43.85\00:09:46.08 and it's very, very serious. 00:09:46.11\00:09:48.25 Now so if you have a joint problem 00:09:48.28\00:09:50.09 and it doesn't go away, how does a doctor evaluate it? 00:09:50.12\00:09:53.42 You know, what are the specific things he does? 00:09:53.46\00:09:56.26 Well, we talked about the MRI. 00:09:56.29\00:09:58.49 If it's an infection, they can put a needle in it. 00:09:58.53\00:10:00.63 If there's fluid, to see what type of fluid it is. 00:10:00.66\00:10:02.63 Okay. 00:10:02.66\00:10:04.00 Sometimes things like these fancy diseases you know, 00:10:04.03\00:10:06.10 all those ones I said scleroderma, rheumatoid, lupus, 00:10:06.13\00:10:09.04 if the body's attacking itself, 00:10:09.07\00:10:10.67 usually find out that in the blood work. 00:10:10.71\00:10:13.64 Now let's talk about these infections for a minute. 00:10:13.68\00:10:15.74 If you have an infection and it's attacking a joint, 00:10:15.78\00:10:18.01 what does that look like on the physiological level, 00:10:18.05\00:10:20.12 is that the ligaments or the bones or what? 00:10:20.15\00:10:22.12 Well, usually what happens is you get an infection 00:10:22.15\00:10:24.55 in the joint space and it starts forming pus. 00:10:24.59\00:10:28.02 Okay, so just there's a fluid that fills the joint 00:10:28.06\00:10:29.99 that shouldn't be there. 00:10:30.03\00:10:31.36 Pass your infection 00:10:31.39\00:10:32.73 and whenever an infection goes somewhere, 00:10:32.76\00:10:34.10 it starts to get big and you know, 00:10:34.13\00:10:35.46 it starts to grow. 00:10:35.50\00:10:36.83 Well. 00:10:36.87\00:10:38.20 And everyone says, hey, let's come and join me here. 00:10:38.23\00:10:39.57 And one becomes two, 00:10:39.60\00:10:40.94 two becomes a hundred and whatever. 00:10:40.97\00:10:42.30 And then it starts and then the body says, guess what? 00:10:42.34\00:10:43.67 There's a bacteria in a joint space 00:10:43.71\00:10:45.81 that's not supposed to be there. 00:10:45.84\00:10:47.18 Yeah. Okay. 00:10:47.21\00:10:48.54 And sometimes that can happen spontaneously, 00:10:48.58\00:10:50.45 sometimes it's after a procedure is done, 00:10:50.48\00:10:53.01 so when it starts happening guess what? 00:10:53.05\00:10:55.42 The body says, "Let's get the immune cells in, 00:10:55.45\00:10:57.52 let's try to treat it our self, then it gets swollen. 00:10:57.55\00:11:00.22 And guess what? 00:11:00.26\00:11:01.59 When the immune cells get there, it gets red. 00:11:01.62\00:11:03.12 You might run a fever because of this attack. 00:11:03.16\00:11:05.93 And then sometimes your body can take care of the infection, 00:11:05.96\00:11:08.86 sometimes it can't. 00:11:08.90\00:11:10.23 Right. 00:11:10.27\00:11:11.60 Well, if you, if I saw a warm joint 00:11:11.63\00:11:12.97 that had fluid in it, you having a fever 00:11:13.00\00:11:15.47 and maybe there's a reason we put a needle in it 00:11:15.50\00:11:17.67 if we needed to and we'd see if it was pussy. 00:11:17.71\00:11:20.14 So then you'd analyze that fluid and figure out 00:11:20.18\00:11:22.48 what was the best treatment from there. 00:11:22.51\00:11:24.11 Yeah. 00:11:24.15\00:11:25.48 Sometimes joints that are worn out overtime 00:11:25.51\00:11:27.55 tend to have inflammation. 00:11:27.58\00:11:29.18 Inflammation tends to have infections. 00:11:29.22\00:11:31.59 Infections and part of the body 00:11:31.62\00:11:32.95 can sometimes get a little bit out of control. 00:11:32.99\00:11:35.62 So these are some basic, 00:11:35.66\00:11:37.29 you know, principles to help people 00:11:37.33\00:11:38.99 understand our joints in general. 00:11:39.03\00:11:41.70 Right. 00:11:41.73\00:11:43.40 So you got to summarize if you have a problem, 00:11:43.43\00:11:44.90 see if you think it's due to wear and tear, okay. 00:11:44.93\00:11:48.07 If the wear and tear won't go away, 00:11:48.10\00:11:50.17 you need to see someone to see 00:11:50.21\00:11:51.54 if it's a structural problem, okay. 00:11:51.57\00:11:53.91 We see this a lot in the shoulder neck. 00:11:53.94\00:11:55.41 You know...
