Okay? 00:01:39.33\00:01:40.66 And inflammation is as we've learned 00:01:40.70\00:01:42.36 in other parts of the body, inflammation can cause pain. 00:01:42.40\00:01:44.83 Right. 00:01:44.87\00:01:46.20 Inflammation immune cells come to this area 00:01:46.23\00:01:48.67 can cause pain and also can cause 00:01:48.70\00:01:51.07 disruption in normal function. 00:01:51.11\00:01:52.97 Okay. Okay. 00:01:53.01\00:01:54.38 And there's certain things 00:01:54.41\00:01:55.81 can cause inflammation in this part of the body. 00:01:55.84\00:01:59.25 Some women, as a reproductive, for pregnancies, 00:01:59.28\00:02:03.25 they use IUDs and sometimes IUDs 00:02:03.28\00:02:06.22 can cause inflammation, of course, infections. 00:02:06.25\00:02:09.22 Especially the sexually transmitted 00:02:09.26\00:02:11.33 diseases of Gonorrhea, Chlamydia, 00:02:11.36\00:02:13.90 those can cause that. 00:02:13.93\00:02:15.36 There's conditions we have 00:02:15.40\00:02:16.73 chronic inflammation endometriosis, 00:02:16.77\00:02:18.93 some people have heard of that. 00:02:18.97\00:02:20.87 Some chronic infections can cause this. 00:02:20.90\00:02:22.87 It has a lot of pelvic pain. 00:02:22.90\00:02:25.07 You might diagnose this 00:02:25.11\00:02:26.44 because when they examine this area, 00:02:26.47\00:02:28.31 there's lot of tenderness in this area. 00:02:28.34\00:02:30.55 It might hurt when you have relations 00:02:30.58\00:02:32.55 in this area. 00:02:32.58\00:02:33.92 And one of the big reasons 00:02:33.95\00:02:35.28 why I think we need to bring it up is 00:02:35.32\00:02:36.85 'cause people with pelvic inflammatory disease, 00:02:36.89\00:02:39.62 they have increased risk of Ectopic Pregnancies. 00:02:39.65\00:02:42.76 Okay. 00:02:42.79\00:02:44.13 Okay, because when the egg is fertilized, 00:02:44.16\00:02:45.49 you know, it can't move through the tubes as well. 00:02:45.53\00:02:47.60 Right. 00:02:47.63\00:02:48.96 So it can't get to the, 00:02:49.00\00:02:50.33 so they have ectopic pregnancies, 00:02:50.37\00:02:51.70 that can be very dangerous. 00:02:51.73\00:02:53.27 And so, that's, you know, 00:02:53.30\00:02:55.20 they say if you have pelvic inflammatory disease, 00:02:55.24\00:02:57.64 the chance of an ectopic pregnancy 00:02:57.67\00:03:00.34 increases 10 times. 00:03:00.38\00:03:01.81 Oh, wow. 00:03:01.84\00:03:03.18 And that's, you know, 00:03:03.21\00:03:04.55 we have fertilization in the tube, 00:03:04.58\00:03:06.61 and then it doesn't move and then the tube 00:03:06.65\00:03:08.38 can have something serious happen... 00:03:08.42\00:03:09.75 Yeah. 00:03:09.78\00:03:11.12 In that, so this is a big problem. 00:03:11.15\00:03:14.89 Another situation that happens, 00:03:14.92\00:03:17.26 if you have pelvic inflammatory disease, 00:03:17.29\00:03:19.66 it makes it harder to have a baby later on, 00:03:19.69\00:03:22.50 because, of course, 00:03:22.53\00:03:23.87 the tubes aren't working as well, 00:03:23.90\00:03:25.33 it's harder to work through those tubes 00:03:25.37\00:03:27.74 to get what needs to be accomplished. 00:03:27.77\00:03:30.81 So those are two things that we need to talk about 00:03:30.84\00:03:33.88 pelvic inflammatory disease. 00:03:33.91\00:03:35.44 If you're having lots of tenderness in that area, 00:03:35.48\00:03:38.41 see your doctor, they wonder if they can do some examination 00:03:38.45\00:03:41.52 to see if you might have pelvic inflammatory disease 00:03:41.55\00:03:43.72 if it's due to infection, 00:03:43.75\00:03:45.09 of course, they would treat that. 00:03:45.12\00:03:46.79 And they might want to be very sensitive to you 00:03:46.82\00:03:49.29 if you ever were to get pregnant, 00:03:49.32\00:03:51.03 so watch out for ectopic pregnancies. 00:03:51.06\00:03:52.83 Right. 00:03:52.86\00:03:54.20 We've got another viewer who writes in and asks, 00:03:54.23\00:03:55.93 "What is Peyronie's disease 00:03:55.96\00:03:57.47 and what's the treatment for it?" 00:03:57.50\00:03:58.83 Yeah. 00:03:58.87\00:04:00.20 Peyronie's disease is a disease that affects about 1 in 20 men. 00:04:00.24\00:04:03.94 Okay. Okay. 00:04:03.97\00:04:05.31 That's a fair number of 5%. Yeah, mm-hm. 00:04:05.34\00:04:06.84 Okay. 00:04:06.88\00:04:08.21 And this is really a simple way 00:04:08.24\00:04:09.71 to think of it as arthritis of the penis, okay? 00:04:09.74\00:04:12.51 Inflammation of the penis and specifically 00:04:12.55\00:04:14.65 you have connective tissue that's very painful. 00:04:14.68\00:04:18.32 And it can lead to a lot of scar tissue. 00:04:18.35\00:04:21.02 And when the scar tissue develops, 00:04:21.06\00:04:23.59 it can cause lots of pain. 00:04:23.63\00:04:25.79 Now the good news, 00:04:25.83\00:04:27.26 the good news is even though lots of people have it, 00:04:27.30\00:04:30.40 20% of those that have it spontaneously improve, 00:04:30.43\00:04:34.94 so a lot of them just get better on their own 00:04:34.97\00:04:36.40 and it goes away. 00:04:36.44\00:04:37.77 Mm-hm. 00:04:37.81\00:04:39.14 However, if it progresses and is very painful, 00:04:39.17\00:04:41.41 some might need surgery 00:04:41.44\00:04:43.21 or some might even need a prosthesis if it progresses. 00:04:43.24\00:04:45.98 Now those are the most serious cases. 00:04:46.01\00:04:47.55 Yeah. 00:04:47.58\00:04:48.92 The doctors that specialize in that are urologists. 00:04:48.95\00:04:51.62 Mm-hm. Okay? 00:04:51.65\00:04:52.99 And they would know the questions to ask, 00:04:53.02\00:04:55.06 the detailed questions that would go into that, 00:04:55.09\00:04:58.23 a lot of men don't like to mention that, 00:04:58.26\00:05:01.96 you know, they won't even mention that things hurt. 00:05:02.00\00:05:03.47 Right, sure, sure. 00:05:03.50\00:05:04.83 But when a man has his penis that hurts, 00:05:04.87\00:05:06.84 there's some serious things that it can be. 00:05:06.87\00:05:08.50 It can be an infection, it can be Peyronie's, 00:05:08.54\00:05:11.04 and they want to look and make sure 00:05:11.07\00:05:12.41 all the parts are working well. 00:05:12.44\00:05:14.24 They're doing what they should do 00:05:14.28\00:05:15.61 and make sure there's nothing more serious going on. 00:05:15.64\00:05:18.41 So the urologist is the person to go to 00:05:18.45\00:05:21.15 when you have this type of symptom. 00:05:21.18\00:05:22.82 Now, I generally think, for some reason, 00:05:22.85\00:05:24.89 I think of arthritis being more in the hands or places 00:05:24.92\00:05:27.82 where there's bone structures, but really you're saying 00:05:27.86\00:05:29.22 it's in soft tissue? 00:05:29.26\00:05:30.59 Yes, soft tissue, the connective tissue. 00:05:30.63\00:05:31.96 The connective tissue? 00:05:31.99\00:05:33.33 The connective tissue and that's throughout the body, 00:05:33.36\00:05:35.33 okay. 00:05:35.36\00:05:36.70 You know, you have connective tissue 00:05:36.73\00:05:38.07 in the ear, unfortunately, 00:05:38.10\00:05:39.43 it doesn't cause too many problems in the ear, 00:05:39.47\00:05:40.84 but yes, you can have inflammation that's abnormal, 00:05:40.87\00:05:43.30 it's plaques. 00:05:43.34\00:05:45.04 You know, there's connective tissue plaques 00:05:45.07\00:05:46.57 and it pushes on things it shouldn't put on. 00:05:46.61\00:05:48.74 And when the normal function's disrupted, guess what, 00:05:48.78\00:05:52.11 the nerves say, "Oh, I'm not happy here." 00:05:52.15\00:05:54.58 There's lots of nerves in this parts of the body, 00:05:54.62\00:05:56.99 and they're used to being very sensitive to things. 00:05:57.02\00:05:59.12 And when things aren't right, the nerves go, 00:05:59.15\00:06:00.89 "Oh, this plaque is disrupting my normal function, 00:06:00.92\00:06:03.93 I'm going to cause pain. 00:06:03.96\00:06:05.46 I'm going to tell the host that something's not wrong." 00:06:05.49\00:06:08.26 So, our duty when we have pain, we should say, 00:06:08.30\00:06:10.80 "Well, something's not wrong, let me find out what it is." 00:06:10.83\00:06:13.84 Because chronic pain is not good. 00:06:13.87\00:06:15.54 So if something's not feeling well, 00:06:15.57\00:06:17.11 get it checked out. 00:06:17.14\00:06:18.47 Exactly, exactly. Yeah. All right. 00:06:18.51\00:06:19.84 We've got another question, also again referring to men. 00:06:19.87\00:06:23.28 "What causes pain in the scrotum?" 00:06:23.31\00:06:25.21 Yeah and... 00:06:25.25\00:06:26.58 Is this a common thing that people experience or... 00:06:26.61\00:06:28.08 Actually, actually, I'm not a urologist, 00:06:28.12\00:06:30.52 so I don't know, 00:06:30.55\00:06:31.89 but when I was preparing for this program, 00:06:31.92\00:06:35.22 it's one of the most common questions 00:06:35.26\00:06:37.26 that's not asked. 00:06:37.29\00:06:38.63 Oh, really? 00:06:38.66\00:06:40.00 So I said, "Well, we're gonna ask this, 00:06:40.03\00:06:41.36 you know, that we're gonna ask this question today and see." 00:06:41.40\00:06:43.60 Not talked about a lot. 00:06:43.63\00:06:44.97 Talk about it, but there's many different causes, 00:06:45.00\00:06:47.74 and the number one cause of scrotal pain is trauma, okay. 00:06:47.77\00:06:53.38 Okay. 00:06:53.41\00:06:55.04 Trauma, you know, we talked about it, 00:06:55.08\00:06:57.48 you know, bicycles, 00:06:57.51\00:06:59.58 you know, constant repetitious pain on this area. 00:06:59.61\00:07:02.52 Some people get inadvertently hit in this area, 00:07:02.55\00:07:05.55 causes lots of pain. 00:07:05.59\00:07:07.56 Whenever the body has trauma to it, 00:07:07.59\00:07:10.26 it starts to swell, inflammation, 00:07:10.29\00:07:12.99 lots of nerves are activated, the body tries to fix itself. 00:07:13.03\00:07:16.36 Sometimes, the pain increases pressure in an organ, 00:07:16.40\00:07:19.20 swelling in an organ that causes major problems. 00:07:19.23\00:07:22.27 Right. 00:07:22.30\00:07:23.64 So trauma is probably the number one 00:07:23.67\00:07:25.01 cause of scrotal pain. 00:07:25.04\00:07:26.47 I tend to think of trauma in that area 00:07:26.51\00:07:29.11 usually goes away on its own. 00:07:29.14\00:07:30.55 Yes, usually, it does. 00:07:30.58\00:07:31.91 Now if it's prolonged for a period of time, what's next? 00:07:31.95\00:07:36.05 That's right, sometimes you can actually have 00:07:36.08\00:07:38.12 damage to the testis, and the other tissue in there, 00:07:38.15\00:07:42.02 epididymis, etcetera. 00:07:42.06\00:07:43.39 Mm-hm. 00:07:43.43\00:07:44.76 And if it's damaged, then you really sometimes things 00:07:44.79\00:07:46.26 can get torsed, turned around, and doesn't move as well. 00:07:46.29\00:07:49.36 Sometimes the tubes can get obstructed, okay. 00:07:49.40\00:07:53.44 And that causes lot of pain as well. 00:07:53.47\00:07:55.87 And what would happen if the pain continues? 00:07:55.90\00:07:58.27 You would probably see a urologist 00:07:58.31\00:08:00.51 and they would do a complete physical, 00:08:00.54\00:08:02.71 they'd also do an ultrasound 00:08:02.74\00:08:05.11 to see if there's been serious damage, 00:08:05.15\00:08:07.15 to see if the testes or the tubes have been damaged. 00:08:07.18\00:08:10.09 Sometimes, they actually find 00:08:10.12\00:08:13.59 that the scrotum has been damaged, 00:08:13.62\00:08:15.92 not by trauma, but it's damaged by cancer. 00:08:15.96\00:08:18.76 Okay. 00:08:18.79\00:08:20.13 Testicular cancer is rare but it does exist, 00:08:20.16\00:08:23.20 and testicular cancer is another cause of pain 00:08:23.23\00:08:27.27 to the scrotal area. 00:08:27.30\00:08:28.84 And if it's diagnosed early, it can be very well treated. 00:08:28.87\00:08:32.07 There's some famous athletes that have had that in the past, 00:08:32.11\00:08:35.38 and it's been Very well publicized. 00:08:35.41\00:08:37.51 But I'd say, you know, if you're having pain 00:08:37.55\00:08:39.58 in the scrotal area, see if it's trauma, 00:08:39.61\00:08:42.25 if the pain doesn't go away, it needs help right away. 00:08:42.28\00:08:44.82 And then a man can examine this area themselves. 00:08:44.85\00:08:47.82 If you feel anything that's hard, 00:08:47.86\00:08:49.26 that's like a rock, 00:08:49.29\00:08:50.76 that would be consistent with something that shouldn't be, 00:08:50.79\00:08:53.29 and you need to get it checked in to right away. 00:08:53.33\00:08:54.66 Right, right. 00:08:54.70\00:08:56.03 So those are some of the more common causes 00:08:56.06\00:08:57.73 on trauma, torsion, obstruction of the tube, 00:08:57.77\00:09:00.70 see a urologist. 00:09:00.74\00:09:02.07 He'd do some ultrasound, make the diagnosis, 00:09:02.10\00:09:04.04 and in severe cases, they have lots of modern medicine 00:09:04.07\00:09:07.34 that they can do to take care of the problems. 00:09:07.38\00:09:09.58 I was gonna ask, is it often surgery 00:09:09.61\00:09:11.48 that they use or... 00:09:11.51\00:09:12.85 Well, sometimes, they have to do, 00:09:12.88\00:09:14.22 rarely they have to do an orchiectomy, okay, 00:09:14.25\00:09:16.08 and that's where they, you know, take the testes out, 00:09:16.12\00:09:18.49 and that can be done. 00:09:18.52\00:09:19.85 Right. 00:09:19.89\00:09:21.22 And that would be the most extreme case. 00:09:21.26\00:09:23.29 And sometimes, they can manipulate 00:09:23.32\00:09:24.73 and relieve the pressure as well, 00:09:24.76\00:09:26.36 but you don't want it to, 00:09:26.39\00:09:28.90 especially if you're young and want to have a family, 00:09:28.93\00:09:31.57 you'd want to get it evaluated because a chronic problem 00:09:31.60\00:09:34.47 could hurt your fertility down the road as well. 00:09:34.50\00:09:36.57 Sure. Right. 00:09:36.60\00:09:37.94 So that's why it's something to have evaluated soon, 00:09:37.97\00:09:40.31 and a lot of men don't like to talk about problems 00:09:40.34\00:09:43.18 in their reproductive organs you know. 00:09:43.21\00:09:44.55 Yeah, right. 00:09:44.58\00:09:45.91 They'll talk about stones, they'll talk about the pain 00:09:45.95\00:09:47.35 that they got, playing ball with junior, you know, 00:09:47.38\00:09:49.92 "I got hit in the mouth, and now I can't breathe anymore 00:09:49.95\00:09:51.95 and my shoulder's hurting" and you know... 00:09:51.99\00:09:53.32 Right. 00:09:53.36\00:09:54.69 They like those type of pains 00:09:54.72\00:09:56.06 but when it gets personal pains, they don't, 00:09:56.09\00:09:57.43 but I'm telling them that it's okay. 00:09:57.46\00:09:59.76 Yeah. 00:09:59.79\00:10:01.16 Don't let a little problem become a big problem. 00:10:01.20\00:10:03.73 Now you mentioned cancer a minute ago, 00:10:03.77\00:10:05.57 and we've got another question. 00:10:05.60\00:10:06.94 Someone writes in, "how is bladder cancer diagnosed?" 00:10:06.97\00:10:09.30 Yeah. well, that's a good question 00:10:09.34\00:10:10.67 'cause cancer can occur anywhere in the body, right? 00:10:10.71\00:10:13.31 Right. 00:10:13.34\00:10:14.68 It can occur in the urogenital tract, 00:10:14.71\00:10:16.04 it can happen in the kidneys, it can happen through 00:10:16.08\00:10:17.98 in the bladder cancer, 00:10:18.01\00:10:19.51 it can happen in the reproductive organs 00:10:19.55\00:10:21.78 as well. 00:10:21.82\00:10:23.15 Bladder cancer is usually diagnosed 00:10:23.18\00:10:25.35 by someone having a symptom that they don't feel right. 00:10:25.39\00:10:28.12 Yeah. 00:10:28.16\00:10:29.49 It's something out of the ordinary. 00:10:29.52\00:10:30.89 When they go to their doctor, they might have something, 00:10:30.93\00:10:32.89 they usually have blood in the urine, you know, 00:10:32.93\00:10:34.90 something like that will show up, 00:10:34.93\00:10:36.43 they might have some pain or some presenting symptoms. 00:10:36.46\00:10:39.03 And the doctor would go 00:10:39.07\00:10:40.50 and they do a test where they can actually go 00:10:40.54\00:10:42.77 into the bladder, they can look in the bladder, 00:10:42.80\00:10:44.67 and they can see if there are cells 00:10:44.71\00:10:46.31 that shouldn't be there, 00:10:46.34\00:10:47.68 a growth that shouldn't be there. 00:10:47.71\00:10:49.04 They will take that growth out, okay, 00:10:49.08\00:10:51.41 and look at it under the microscope. 00:10:51.45\00:10:53.65 Now other cancers that are, you know, 00:10:53.68\00:10:55.28 bladder cancer happens in the bladder, of course, 00:10:55.32\00:10:57.75 but we've heard a lot about prostate cancer, okay and... 00:10:57.79\00:11:00.79 Right. 00:11:00.82\00:11:02.16 Men can, when they get their prostate examined, 00:11:02.19\00:11:03.83 you can feel nodules on the prostate, 00:11:03.86\00:11:05.59 feels like marbles, 00:11:05.63\00:11:07.03 and the doctor can go up and biopsy that 00:11:07.06\00:11:09.30 and see if there's a cancer there. 00:11:09.33\00:11:11.03 But, of course, cancer in the testes 00:11:11.07\00:11:12.77 feels like a marble, 00:11:12.80\00:11:16.20 in the female tract, uterine cancer, ovarian cancer, 00:11:16.24\00:11:19.97 those are cancers that can press on organs 00:11:20.01\00:11:22.31 and have pain as well. 00:11:22.34\00:11:23.68 Yeah. 00:11:23.71\00:11:25.05 So whenever you have pain in the urogenital system, 00:11:25.08\00:11:26.58 you know, they would say, 00:11:26.61\00:11:27.95 "Well, where's the pain coming from?" 00:11:27.98\00:11:29.32 And you say, "Well, it's coming from here." 00:11:29.35\00:11:30.79 So your doc will say, "Well, let me see if I can find out 00:11:30.82\00:11:32.85 what's causing the pain." 00:11:32.89\00:11:34.32 They would think infection but on the list, 00:11:34.36\00:11:36.46 they'd say, "Well, could they have 00:11:36.49\00:11:37.83 a possibility of a cancer?" 00:11:37.86\00:11:39.53 And they would evaluate that with ultra-sounds, CAT scans. 00:11:39.56\00:11:42.96 If they thought it was a cancer, 00:11:43.00\00:11:44.33 the next step would be to biopsy it 00:11:44.37\00:11:46.30 and see exactly what it is. 00:11:46.33\00:11:47.87 Right, right. 00:11:47.90\00:11:49.90 You know, we're just about out of time today, 00:11:49.94\00:11:51.27 but what would you say is the most important 00:11:51.31\00:11:53.78 take home point that we've discussed today?
