Yes. 00:01:11.63\00:01:13.10 Have you ever experienced oral pain? 00:01:13.13\00:01:15.04 I think everyone has, Nick, 00:01:15.07\00:01:16.40 if you've ever got bopped in the mouth 00:01:16.44\00:01:18.41 or bitten your tongue that counts as oral pain. 00:01:18.44\00:01:22.11 Lots of oral pain is caused by trauma. 00:01:22.14\00:01:24.78 You know something's in your mouth. 00:01:24.81\00:01:26.15 It's not right. 00:01:26.18\00:01:27.52 Some, you know a lot of people have suffered 00:01:27.55\00:01:28.95 through cavities and problems with their mouth, 00:01:28.98\00:01:31.72 and, you know, we're going to talk a little bit 00:01:31.75\00:01:33.29 about that today. 00:01:33.32\00:01:34.66 But we've been talking in general 00:01:34.69\00:01:36.16 about all the different causes of pain. 00:01:36.19\00:01:38.73 And pain is not very fun. 00:01:38.76\00:01:41.20 And I think what we've learned is that 00:01:41.23\00:01:43.23 if we can get the cause of pain, 00:01:43.26\00:01:44.83 that's the best way to deal with it. 00:01:44.87\00:01:46.57 That's right. 00:01:46.60\00:01:47.94 And if it's a dangerous pain, 00:01:47.97\00:01:49.30 you know we need to get help right away. 00:01:49.34\00:01:52.11 And then, of course, 00:01:52.14\00:01:53.48 chronic pain is a whole different ballgame 00:01:53.51\00:01:55.98 and we sort of been addressing that as well. 00:01:56.01\00:01:59.11 But oral pain it is something that affects many people 00:01:59.15\00:02:01.88 and I think the probably the specialists in oral pain 00:02:01.92\00:02:05.12 are probably dentists. 00:02:05.15\00:02:07.89 Now I understand you have a daughter 00:02:07.92\00:02:09.26 who's recently experienced some oral pain. 00:02:09.29\00:02:10.63 Yes, yes. 00:02:10.66\00:02:11.99 She had her wisdom teeth taken out. 00:02:12.03\00:02:14.16 Okay. And that was very painful. 00:02:14.20\00:02:15.76 They said it was crowding her mouth, 00:02:15.80\00:02:17.73 and she had a wisdom teeth taken out, 00:02:17.77\00:02:19.70 and this really hurt her. 00:02:19.73\00:02:21.07 You know when they took them out 00:02:21.10\00:02:22.50 and she's getting better and that caused her oral pain, 00:02:22.54\00:02:25.11 and she's just now getting over that. 00:02:25.14\00:02:26.98 But you know if you're having lots of oral pain 00:02:27.01\00:02:29.01 that doesn't go away, 00:02:29.04\00:02:30.58 the specialist to really have it checked out, 00:02:30.61\00:02:32.41 look at the mouth, 00:02:32.45\00:02:33.78 look at the teeth is the dentist. 00:02:33.82\00:02:35.15 That's right. 00:02:35.18\00:02:36.52 And we recently had the opportunity to go 00:02:36.55\00:02:37.89 to the Davis family dental care, 00:02:37.92\00:02:39.42 and Dr. Joel Davis is one of my good friends. 00:02:39.45\00:02:41.82 And he's a recent graduate of dental school 00:02:41.86\00:02:44.09 and you know sometimes 00:02:44.13\00:02:45.46 people are afraid of young dentists. 00:02:45.49\00:02:47.10 Both are on top of things though. 00:02:47.13\00:02:48.46 They've had the latest training, right? 00:02:48.50\00:02:50.10 And so Dr. Davis shared with us 00:02:50.13\00:02:52.33 about some of the most common causes 00:02:52.37\00:02:53.77 he sees in his practice, 00:02:53.80\00:02:55.24 and some of the latest and greatest techniques 00:02:55.27\00:02:57.27 he's learned to treat the symptoms, 00:02:57.31\00:02:58.81 let's have a look. 00:02:58.84\00:03:00.64 I tell all my patients that there are two battles 00:03:00.68\00:03:03.68 that we fight in dentistry, 00:03:03.71\00:03:05.05 one is tooth decay 00:03:05.08\00:03:06.41 and the other is periodontal disease. 00:03:06.45\00:03:08.48 So tooth decay is a cavity, it's destroying the tooth. 00:03:08.52\00:03:11.22 Periodontal disease is more like high blood pressure. 00:03:11.25\00:03:13.72 You don't know you have it until it's measured for, 00:03:13.76\00:03:15.76 but periodontal disease is a slow destruction 00:03:15.79\00:03:18.13 of the supporting tissues 00:03:18.16\00:03:19.86 that hold the tooth in the mouth. 00:03:19.89\00:03:21.66 Periodontal disease is usually not painful, 00:03:21.70\00:03:24.07 it's a slow progress of chronic problem 00:03:24.10\00:03:26.94 that needs to be treated with a deep cleaning, 00:03:26.97\00:03:30.21 but it usually is not painful, until the tooth becomes loose. 00:03:30.24\00:03:33.61 At that point, it's almost too late to save the tooth. 00:03:33.64\00:03:37.48 Almost every time a patient comes into the dental office 00:03:37.51\00:03:40.02 with pain in their mouth, 00:03:40.05\00:03:41.68 it's because they've had a cavity 00:03:41.72\00:03:43.18 that has gone untended to untreated for too long. 00:03:43.22\00:03:47.29 And the tooth decay is destroyed the tooth 00:03:47.32\00:03:49.62 all the way down to the nerve on the inside of the tooth. 00:03:49.66\00:03:52.29 It's caused them to have intense pain 00:03:52.33\00:03:54.63 that lasts for two or three minutes at a time 00:03:54.66\00:03:56.60 which is severe 8, 9 out of 10. 00:03:56.63\00:04:00.07 Sometimes, it's the worst pain patients have ever experienced. 00:04:00.10\00:04:03.54 But if it's keeping you awake at night, constantly throbbing, 00:04:03.57\00:04:08.04 we know that the nerves involved, 00:04:08.08\00:04:09.41 and you're going to need some treatment right away. 00:04:09.44\00:04:12.05 Other things that bring patients in 00:04:12.08\00:04:14.85 might be pain of a sore. 00:04:14.88\00:04:17.99 You know an ulcer or some type of canker sore 00:04:18.02\00:04:20.59 that formed on the gums or on the lips 00:04:20.62\00:04:22.76 you know we can treat those 00:04:22.79\00:04:24.13 with some topical creams or rinses. 00:04:24.16\00:04:27.20 Temperamental with their joint disorder 00:04:27.23\00:04:29.23 or TMD is a disorder of the joint 00:04:29.26\00:04:32.97 that can be painful or you might just have popping 00:04:33.00\00:04:37.44 or clicking when you open and close. 00:04:37.47\00:04:39.87 If a patient locks open, 00:04:39.91\00:04:43.11 that can be very uncomfortable and kind of scary. 00:04:43.14\00:04:45.65 But typically, 00:04:45.68\00:04:47.12 joint problems are not painful as more of a chronic. 00:04:47.15\00:04:51.79 You hear a kind of a crepitus or grinding sound in the joint 00:04:51.82\00:04:54.49 when you open and close or a popping and clicking. 00:04:54.52\00:04:56.93 And when, when should you come in 00:04:56.96\00:04:58.29 when you have tooth pain? 00:04:58.33\00:04:59.66 Well, the answer will always be 00:04:59.69\00:05:01.03 the sooner we can catch that problem, 00:05:01.06\00:05:03.57 the smaller it is 00:05:03.60\00:05:04.93 and the easier it should be to fix. 00:05:04.97\00:05:06.74 The longer you wait, it's going to cost a lot more, 00:05:06.77\00:05:09.47 it's going to hurt your wallet a lot more 00:05:09.50\00:05:11.81 just because it's a lot more complex to fix. 00:05:11.84\00:05:14.98 Prevention is worth everything. 00:05:15.01\00:05:17.25 If we can stop things from happening 00:05:17.28\00:05:19.21 such as tooth decay, 00:05:19.25\00:05:20.58 periodontal disease is worth everything. 00:05:20.62\00:05:21.95 I have patients all the time to ask me. 00:05:21.98\00:05:24.82 "I've got this special rinse, 00:05:24.85\00:05:26.25 this special paste or this special brush." 00:05:26.29\00:05:29.22 What works is the thing that you actually use 00:05:29.26\00:05:32.23 and you need to remove plaque off the teeth daily 00:05:32.26\00:05:35.23 with a brush and with floss. 00:05:35.26\00:05:37.10 It's traditional, it's tried and true. 00:05:37.13\00:05:40.10 Flossing and brushing is what works 00:05:40.14\00:05:42.04 because for removing the plaque off the tooth 00:05:42.07\00:05:44.54 on a daily basis, 00:05:44.57\00:05:46.07 then we are eliminating the causative factor 00:05:46.11\00:05:48.78 for tooth decay, 00:05:48.81\00:05:50.15 which is bacteria or periodontal disease 00:05:50.18\00:05:52.28 which is bacteria once again that is trapped in the plaque 00:05:52.31\00:05:55.42 or the tartar that's on the tooth. 00:05:55.45\00:05:58.02 So it really comes down to really good home hygiene. 00:05:58.05\00:06:01.59 And at the dental office, 00:06:01.62\00:06:02.96 that's one of our goals is to help educate you 00:06:02.99\00:06:06.36 and enable you to do a great job at home. 00:06:44.73\00:06:48.74 is more to do every day. 00:06:48.77\00:06:50.17 And if we do that, we can avoid a lot of pain. 00:06:50.21\00:06:52.11 You know another friend told me one time, 00:06:52.14\00:06:53.78 "You have to brush your teeth, 00:06:53.81\00:06:55.14 just the ones you want to keep." 00:06:55.18\00:06:56.51 This... 00:06:56.54\00:06:57.88 Well, you know that's interesting, Nick, 00:06:57.91\00:06:59.25 because if you think about you know oral pain, 00:06:59.28\00:07:01.82 you know we have our teeth and our gums, 00:07:01.85\00:07:04.02 what did you know I talked to Dr. Davis once about this 00:07:04.05\00:07:06.86 as what did we do before we had toothbrushes 00:07:06.89\00:07:08.96 you know we've always had toothbrushes. 00:07:08.99\00:07:10.69 That's true. 00:07:10.73\00:07:12.06 And he was telling me that 00:07:12.09\00:07:13.43 the reason a lot of the cavities, 00:07:13.46\00:07:14.86 gum pain, and problem happens is because of processed sugar. 00:07:14.90\00:07:18.40 Yeah, sure. 00:07:18.43\00:07:19.77 And if we can get processed sugar out of our lives, 00:07:19.80\00:07:22.27 we can save a lot of oral problems. 00:07:22.30\00:07:24.94 That's right. 00:07:24.97\00:07:26.31 And also you know the canker sores 00:07:26.34\00:07:27.98 and some of those things you know 00:07:28.01\00:07:29.34 that's great we put them so topical stuff on it 00:07:29.38\00:07:31.81 to get a feel better total. 00:07:31.85\00:07:33.18 But if we can do the things 00:07:33.21\00:07:34.55 if process sugar our lifestyles are contributing to it, 00:07:34.58\00:07:37.35 if we can do that, it's going to help a lot. 00:07:37.39\00:07:39.59 Yeah, certainly. 00:07:39.62\00:07:40.96 Now you know I guess 00:07:40.99\00:07:42.32 we should talk about all the different causes, 00:07:42.36\00:07:43.69 other causes of oral pain, too. 00:07:43.73\00:07:45.06 Yeah. 00:07:45.09\00:07:46.43 What are some of those 00:07:46.46\00:07:47.80 and if you experience any others? 00:07:47.83\00:07:49.16 Well, I mean you know as a cardiologist, 00:07:49.20\00:07:50.73 I run into people with different, 00:07:50.77\00:07:52.40 and so we're going to talk to people 00:07:52.43\00:07:54.14 and if they have any questions go to our website. 00:07:54.17\00:07:56.20 We'll give it over to Dr. Davis 00:07:56.24\00:07:57.64 'cause we might not be able to handle 00:07:57.67\00:07:59.31 all these oral questions, 00:07:59.34\00:08:00.68 but think let's think about 00:08:00.71\00:08:02.04 we talked about our teeth and gums. 00:08:02.08\00:08:03.55 Okay. 00:08:03.58\00:08:04.91 But a lot of that can from trauma okay. 00:08:04.95\00:08:07.85 Right. 00:08:07.88\00:08:09.22 People might have braces 00:08:09.25\00:08:10.59 or special things in their mouth. 00:08:10.62\00:08:12.32 They might be professions or things that hit their mouth. 00:08:12.35\00:08:15.79 You know that would cause trauma, 00:08:15.82\00:08:17.26 that could cause pain in the mouth. 00:08:17.29\00:08:19.49 Why it's so serious if we have a lesion in the mouth 00:08:19.53\00:08:22.10 is if we think about it if the mouth bleeds, 00:08:22.13\00:08:24.03 there's lots of bacteria in the mouth. 00:08:24.07\00:08:26.27 And you can be set up for infections in the mouth. 00:08:26.30\00:08:29.14 One of the serious infections that I'm concerned about is 00:08:29.17\00:08:31.77 if you have a long-term dental abscess 00:08:31.81\00:08:34.31 which is an infection, 00:08:34.34\00:08:35.68 and it should go untreated and it gets in the bloodstream, 00:08:35.71\00:08:39.21 guess what the, the bacteria in the blood 00:08:39.25\00:08:41.28 is circulating all through the body. 00:08:41.32\00:08:43.02 Causes more stress on the heart. 00:08:43.05\00:08:44.52 Yeah, more stress on the heart 00:08:44.55\00:08:45.89 and if you have an abnormal heart valve, 00:08:45.92\00:08:48.12 that bacteria that's floating through says, 00:08:48.16\00:08:50.89 "I'm getting a little hungry. 00:08:50.93\00:08:52.26 What can I latch on to? 00:08:52.29\00:08:53.63 Oh, there's a heart valve. 00:08:53.66\00:08:55.00 I didn't about that, let me grab onto that." 00:08:55.03\00:08:56.56 So he grabs on just, "Mmm, little tasty." 00:08:56.60\00:08:59.20 Let me invite my friends over, and before long, 00:08:59.23\00:09:01.84 we could have an infection on the heart valve, 00:09:01.87\00:09:03.87 we call that endocarditis. 00:09:03.91\00:09:06.71 Also dental care 00:09:06.74\00:09:08.34 and it is causing a lot of inflammation 00:09:08.38\00:09:10.61 and inflammation is not good for the body either 00:09:10.65\00:09:12.95 especially, heart disease. 00:09:12.98\00:09:14.32 So we want to keep that mouth healthy 00:09:14.35\00:09:16.05 just to prevent heart disease infections there, 00:09:16.08\00:09:18.62 so trauma is a big deal. 00:09:18.65\00:09:20.26 We don't want to get are or you know so, Nick, 00:09:20.29\00:09:22.56 I would advise don't get in fights. 00:09:22.59\00:09:24.66 Okay. 00:09:24.69\00:09:26.16 If you're playing basketball and you've got some rough guys, 00:09:26.19\00:09:28.73 you might want to wear mouth guard, 00:09:28.76\00:09:31.07 things of that nature. 00:09:31.10\00:09:32.63 If you're wearing braces, you know you want to be careful 00:09:32.67\00:09:35.30 that you don't get a sharp wire that's banging into things. 00:09:35.34\00:09:37.77 Right. 00:09:37.81\00:09:39.14 Those are some just some common things of trauma, 00:09:39.17\00:09:40.98 so trauma is up there pretty high of oral pain. 00:09:41.01\00:09:44.61 Now every time I go to my dentist, 00:09:44.65\00:09:46.31 he pulls out the tongue, he kind of pokes around, 00:09:46.35\00:09:48.18 feels around. 00:09:48.22\00:09:49.55 I assume he's checking for cancer. 00:09:49.58\00:09:50.92 Yes, he is. 00:09:50.95\00:09:52.29 Now can that be a source of real pain in the mouth 00:09:52.32\00:09:53.66 or how would you know other than him checking? 00:09:53.69\00:09:55.02 Well, you might feel a mass. 00:09:55.06\00:09:56.99 If the cancer is pushing on a nerve 00:09:57.03\00:09:59.13 or pushing on an important structure, 00:09:59.16\00:10:00.80 it could cause pain as well. 00:10:00.83\00:10:02.86 So that that's a big, big issue if you have oral cancer, 00:10:02.90\00:10:05.50 who's at risk of having cancer. 00:10:05.53\00:10:08.00 A lot of people that chew tobacco, okay, 00:10:08.04\00:10:10.61 that's a carcinogen that gets in there. 00:10:10.64\00:10:12.54 So those are people 00:10:12.57\00:10:13.91 that are at a higher risk of having oral cancer, 00:10:13.94\00:10:15.74 but he will look for cancer 00:10:15.78\00:10:17.11 but cancer is a cause of referred pain. 00:10:17.15\00:10:19.88 And also sinuses. 00:10:19.91\00:10:21.32 You know sometimes sinus pain can be referred into the jaw. 00:10:21.35\00:10:26.32 And you might feel like a pressure 00:10:26.35\00:10:27.99 on the upper jaw that kind of thing. 00:10:28.02\00:10:30.26 It might be due to pain. 00:10:30.29\00:10:32.29 Other types of pain is infections, okay, 00:10:32.33\00:10:35.30 have you ever had a strep throat? 00:10:35.33\00:10:36.67 I haven't really. 00:10:36.70\00:10:38.03 Yeah, oh, have ever have a sore throat? 00:10:38.07\00:10:39.40 Yep, I had a sore throat. 00:10:39.43\00:10:40.77 Well, many people have had sore throats. 00:10:40.80\00:10:42.57 And sore throats can be from an infection, 00:10:42.60\00:10:44.74 either a bacterial infection or a viral infection. 00:10:44.77\00:10:48.84 Lots of kids around five, six age gets strep throat. 00:10:48.88\00:10:52.38 And that's very painful. 00:10:52.41\00:10:53.75 And if you look in their throats 00:10:53.78\00:10:55.12 and they say ah, you'll see all sorts of you know 00:10:55.15\00:10:57.29 so white splotchy blots, tonsils, tonsillitis, 00:10:57.32\00:11:01.69 you know we can get that tonsils infected 00:11:01.72\00:11:04.16 that can cause a lot of pain. 00:11:04.19\00:11:05.96 Usually, when you swallow, 00:11:05.99\00:11:08.16 so those are another couple of causes of chronic pain. 00:11:08.20\00:11:11.60 So you've probably... 00:11:11.63\00:11:12.97 Every question I asked you, Nick, 00:11:13.00\00:11:14.47 you've never had problems with 00:11:14.50\00:11:15.87 so have you ever had pain in your mouth? 00:11:15.90\00:11:17.24 I've had sore throat. 00:11:17.27\00:11:18.61 Okay, so you had the sore throat. 00:11:18.64\00:11:19.97 Yeah, I had a sore throat. 00:11:20.01\00:11:21.34 I don't know, 00:11:21.38\00:11:22.71 what's difference between sore throat and strep throat, 00:11:22.74\00:11:24.08 I guess I think strep being a bit more serious. 00:11:24.11\00:11:25.45 It's a cause, you know, 00:11:25.48\00:11:26.82 that you know strep is a bacterial infection. 00:11:26.85\00:11:29.32 Whereas sometimes you just get a viral infection 00:11:29.35\00:11:31.65 or you know common cold or rhinovirus is a common virus 00:11:31.69\00:11:34.92 that can cause a sore throat but it's not, it's not quite, 00:11:34.96\00:11:39.43 they don't get the white blotches. 00:11:39.46\00:11:40.83 You don't run afoul of that kind of thing. 00:11:40.86\00:11:42.96 So I think we've talked about causes of pain in the mouth, 00:11:43.00\00:11:45.93 teeth, gums, trauma, cancer, infections. 00:11:45.97\00:11:50.27 We've talked about the sinuses. 00:11:50.31\00:11:52.24 And if you have it, you've got to get it looked at. 00:11:52.27\00:11:53.98 That's right. 00:11:54.01\00:11:55.34 Now let's go back to processed sugar. 00:11:55.38\00:11:56.71 You mentioned that earlier, 00:11:56.75\00:11:58.08 that's a really, really tough one 00:11:58.11\00:11:59.88 to eliminate from your diet. 00:11:59.91\00:12:01.48 It is, but it's sort of a contributor 00:12:01.52\00:12:04.02 to all sorts of problems. 00:12:04.05\00:12:05.75 Cavities, bacteria in the mouth, 00:12:05.79\00:12:07.72 they like that processed sugar. 00:12:07.76\00:12:09.52 It can create, 00:12:09.56\00:12:10.89 weakens your immune system or likely to get infections, 00:12:10.93\00:12:14.06 more you know, more like the gum health, 00:12:14.10\00:12:15.93 the gums aren't happy when you get a lot of processed sugar. 00:12:15.96\00:12:18.83 So if we can get rid of processed sugar, 00:12:18.87\00:12:21.07 we can eliminate a lot of the problems 00:12:21.10\00:12:23.97 of chronic mouthing. 00:12:24.01\00:12:25.34 If you do that and you get rid of processed sugar
