What pain is? 00:01:19.94\00:01:21.68 When it's serious? 00:01:21.71\00:01:23.04 And abdominal pain is a big one because everyone has it. 00:01:23.08\00:01:26.11 And whether it comes from, 00:01:26.15\00:01:28.65 you know, eating too much food or, 00:01:28.68\00:01:30.72 you know, not moving the food through the tube, 00:01:30.75\00:01:32.72 it's a problem that we have. 00:01:32.75\00:01:34.89 And I think, you know, in approaching it, you know, 00:01:34.92\00:01:37.93 I want to educate our audience and if can know some, 00:01:37.96\00:01:40.90 you know, some of the big, big physiologic principles. 00:01:40.93\00:01:43.97 Yeah, we can sort of understand this, this so, 00:01:44.00\00:01:46.77 so the digestion system is just a big tube. 00:01:46.80\00:01:50.67 So, Nick, think if it's like a big tube that runs 00:01:50.71\00:01:53.17 from your mouth to when it exits your body. 00:01:53.21\00:01:56.51 And the function of this tube 00:01:56.54\00:01:58.21 is to extract energy from your food. 00:01:58.25\00:02:01.38 That's what it does. 00:02:01.42\00:02:02.75 So it's big tube that pulls energy into your body. 00:02:02.78\00:02:06.25 Holds out the nutrients that you need 00:02:06.29\00:02:08.22 and it passes through the things 00:02:08.26\00:02:09.59 that you don't need. 00:02:09.62\00:02:10.96 It keeps out what goes out. 00:02:10.99\00:02:12.33 So that sort of helps you. Right. 00:02:12.36\00:02:13.70 It's one big tube, it's different types of tissue 00:02:13.73\00:02:15.06 and it goes at different speeds 00:02:15.10\00:02:16.43 and different parts pull different things out, 00:02:16.46\00:02:18.23 but that's what it's for. 00:02:18.27\00:02:19.93 So if you think about it, it starts in the mouth, 00:02:19.97\00:02:22.97 the tube starts in your mouth 00:02:23.00\00:02:24.34 and that's where actually digestion starts. 00:02:24.37\00:02:27.04 And we can help maintain the tube in this part 00:02:27.08\00:02:29.84 by just chewing our food well. 00:02:29.88\00:02:31.95 Yeah. Okay. 00:02:31.98\00:02:33.31 It keep, cuts down the work load downstream. 00:02:33.35\00:02:36.18 So digestion starts there, we want to, 00:02:36.22\00:02:38.55 we call this mastication, a big word mastication chewing. 00:02:38.59\00:02:44.43 Amylase, lipids different amylase chemicals 00:02:44.46\00:02:45.79 start to digest the food, 00:02:45.83\00:02:47.16 we chew it that helps to break it down. 00:02:47.20\00:02:48.53 Now amylase is that like, what's in you saliva? 00:02:48.56\00:02:50.27 Yeah, yeah, that's a saliva, that's a enzyme 00:02:50.30\00:02:53.23 that starts to break down protein 00:02:53.27\00:02:55.04 right there in your saliva. 00:02:55.07\00:02:59.41 And then it starts moving down 00:02:59.44\00:03:01.88 and then as it moves down the tube, 00:03:01.91\00:03:04.08 different things happen. 00:03:04.11\00:03:05.45 You know, first it gets, 00:03:05.48\00:03:06.82 you know, the esophagus you swallow it, 00:03:06.85\00:03:08.18 okay. 00:03:08.22\00:03:09.55 And it decides to go down the right tube. 00:03:09.58\00:03:10.92 You know, there's tube that goes to your lungs 00:03:10.95\00:03:12.29 right there at the same spot. 00:03:12.32\00:03:13.66 Right, we don't want it to go down that tube. 00:03:13.69\00:03:15.02 No, and the epiglottis 00:03:15.06\00:03:16.39 keeps it going down the big tube 00:03:16.42\00:03:17.76 and it goes down the esophagus 00:03:17.79\00:03:19.79 and then it eventually gets into your stomach. 00:03:19.83\00:03:21.73 So there in the stomach really work starts to happen. 00:03:21.76\00:03:24.83 Yeah. 00:03:24.87\00:03:26.20 And there is a little valve leading into the stomach 00:03:26.23\00:03:27.57 called upper esophageal sphincter, okay. 00:03:27.60\00:03:30.21 And it gets in the stomach and then the stomach starts 00:03:30.24\00:03:32.37 digesting the food breaking it down more. 00:03:32.41\00:03:34.71 So if you chewed it up petty well, guess what? 00:03:34.74\00:03:37.65 And if you eat food that's easy to digest, 00:03:37.68\00:03:39.58 the stomach doesn't have as much work to do. 00:03:39.61\00:03:41.98 So it's easy on your stomach if you chew the food well. 00:03:42.02\00:03:44.62 I know, that something that sometimes 00:03:44.65\00:03:45.99 I rush through a meal 00:03:46.02\00:03:47.36 and I'm not doing myself any favor, 00:03:47.39\00:03:48.72 is that where am I? 00:03:48.76\00:03:50.09 Yeah, and also you don't want to hit the stomach 00:03:50.13\00:03:51.79 with too much food all at once because then he's got to do is, 00:03:51.83\00:03:54.13 "Man, my workload is too hard today." 00:03:54.16\00:03:56.90 Yeah. So he likes to rest too. 00:03:56.93\00:03:58.27 So it makes a lot of different chemicals 00:03:58.30\00:03:59.97 to start breaking down the food even further, 00:04:00.00\00:04:02.90 so it can be absorbed downstream 00:04:02.94\00:04:05.14 in the intestines. 00:04:05.17\00:04:06.57 So it breaks down the food especially proteins, 00:04:06.61\00:04:09.48 a lot of, you know, breaks down proteins. 00:04:09.51\00:04:11.68 Hydrochloric acid is made in the stomach 00:04:11.71\00:04:13.75 and that's a pretty powerful acid 00:04:13.78\00:04:15.18 to break down food. 00:04:15.22\00:04:16.55 So the stomach is very acidic, very acidic. 00:04:16.58\00:04:19.19 And we want to keep the acid in the stomach. 00:04:19.22\00:04:21.86 We're gonna learn that some disease conditions happen 00:04:21.89\00:04:24.16 because the acid either goes upstream 00:04:24.19\00:04:26.39 into the esophagus or it causes damage 00:04:26.43\00:04:29.26 within the stomach lining itself. 00:04:29.30\00:04:31.13 I'm assuming that the stomach is made to hold this acid... 00:04:31.17\00:04:33.17 Yes, it is. 00:04:33.20\00:04:34.54 And so it can do that very well. 00:04:34.57\00:04:35.90 But when the acid gets out into other parts of the body... 00:04:35.94\00:04:37.27 Not so very good. Okay. 00:04:37.31\00:04:38.64 Not so very good. 00:04:38.67\00:04:40.01 So anyway, the food eventually gets into the small intestine 00:04:40.04\00:04:42.24 and the large intestine and that's where the water 00:04:42.28\00:04:44.88 and the nutrients are processed. 00:04:44.91\00:04:48.45 The intestines aside, what gets in your body, 00:04:48.48\00:04:51.59 what doesn't get in your body. 00:04:51.62\00:04:53.56 We have a very a trillion bacteria 00:04:53.59\00:04:55.82 live in this tube downstream 00:04:55.86\00:04:57.36 and we call that the microbiome. 00:04:57.39\00:04:59.89 Okay. 00:04:59.93\00:05:01.26 All sorts of species of bacteria 00:05:01.30\00:05:02.93 and we want to have the good ones out 00:05:02.96\00:05:04.53 with the bad because they decide 00:05:04.57\00:05:05.90 what gets in your gut, what gets out of your gut. 00:05:05.93\00:05:08.17 The bacteria down there also make chemicals 00:05:08.20\00:05:10.81 that affect your mood, 00:05:10.84\00:05:12.21 it affects inflammation on their body, 00:05:12.24\00:05:14.38 it affects all sorts of things. 00:05:14.41\00:05:15.84 You know, it can even affect 00:05:15.88\00:05:17.91 you know different disease states 00:05:17.95\00:05:19.98 down in your intestine. 00:05:20.02\00:05:21.42 Well, you know, the microbiome, 00:05:21.45\00:05:23.25 that you talk about all these bacteria, 00:05:23.28\00:05:26.39 I just think of them just dealing with the food 00:05:26.42\00:05:28.96 but it sounds like they play a much bigger role 00:05:28.99\00:05:30.63 than just dealing with food. 00:05:30.66\00:05:31.99 Yes, they help with the food, they help decide 00:05:32.03\00:05:33.36 what gets in and out of the body. 00:05:33.40\00:05:35.03 The bad bacteria have their own things that they do. 00:05:35.06\00:05:39.13 And if you think of our genetics, 00:05:39.17\00:05:40.50 there's more genetic material in the bowels 00:05:40.54\00:05:43.10 than in the rest of the body. 00:05:43.14\00:05:44.47 Really? That's fascinating. Yeah. 00:05:44.51\00:05:45.84 A trillion bacteria they all have DNA. 00:05:45.87\00:05:47.28 DNA makes different things. 00:05:47.31\00:05:49.21 And so that's you know why, you know, 00:05:49.24\00:05:50.61 the brain and the gut are connected 00:05:50.65\00:05:52.35 because it makes chemicals like serotonin 00:05:52.38\00:05:54.48 which affects our moods. 00:05:54.52\00:05:55.88 So if the guts not happy, a lot of it's not happy. 00:05:55.92\00:05:58.82 So anyway, it moves through the big tube, 00:05:58.85\00:06:01.06 the small and large intestine. 00:06:01.09\00:06:02.56 Eventually it gets 00:06:02.59\00:06:04.26 once all the nutrients have been pulled out, 00:06:04.29\00:06:06.56 the waste is excreted. 00:06:06.59\00:06:08.26 Right. And then you're done. 00:06:08.30\00:06:10.13 So that's in short what this big tube does. 00:06:10.17\00:06:13.94 Okay. 00:06:13.97\00:06:15.30 So now when we sort of got a feel for this, it helps us, 00:06:15.34\00:06:17.94 it's gonna help us understand abdominal pain 00:06:17.97\00:06:20.11 and also pain up higher too. 00:06:20.14\00:06:21.58 Right, yeah. 00:06:21.61\00:06:22.94 So if you think what causes malfunction of the tube. 00:06:22.98\00:06:25.88 If we sort of know what it's supposed to do, 00:06:25.91\00:06:27.78 what can cause malfunction? 00:06:27.82\00:06:29.18 We're eating the wrong types of food. 00:06:29.22\00:06:31.29 Okay, eating food with toxins in it. 00:06:31.32\00:06:33.89 You know, that doesn't agree with the system. 00:06:33.92\00:06:36.02 Now what kind of things would be toxins? 00:06:36.06\00:06:37.59 Well, think about it, lets say, you eat spoilt food. 00:06:37.63\00:06:41.43 Okay. It has bacteria in it. 00:06:41.46\00:06:42.83 It's gonna make you get sick and throw up. 00:06:42.86\00:06:45.70 Let's say you eat a lot of chemicals 00:06:45.73\00:06:47.67 that your body can't handle. 00:06:47.70\00:06:49.34 Okay, a lot of people you hear gluten insensitivities 00:06:49.37\00:06:52.74 and a lot of people do poor, for instance, 00:06:52.77\00:06:55.34 if you are a meat eater, 00:06:55.38\00:06:56.71 if you think of all the chemicals 00:06:56.75\00:06:58.08 that come with meat, 00:06:58.11\00:06:59.45 the antibiotics, the growth hormones, 00:06:59.48\00:07:02.98 the disease itself in animals that gets in you. 00:07:03.02\00:07:06.02 Sometimes that can act as a toxin in the body. 00:07:06.05\00:07:09.19 So that's gonna hurt the system, 00:07:09.22\00:07:11.39 the system needs good old water, okay. 00:07:11.43\00:07:13.16 That's right. 00:07:13.19\00:07:14.53 Yeah, little bit of water, it's very important. 00:07:14.56\00:07:15.90 To push it through. 00:07:15.93\00:07:17.27 So not getting enough water could hurt help dysfunctions. 00:07:17.30\00:07:20.24 We know that people who don't drink enough water 00:07:20.27\00:07:22.17 sometimes they get constipated. 00:07:22.20\00:07:24.24 Right. Have you ever been constipated? 00:07:24.27\00:07:27.88 Not too bad, I usually drink a good amount of water. 00:07:27.91\00:07:29.91 You know, I haven't either, thank goodness. 00:07:29.94\00:07:32.41 And when you said they've been practicing 00:07:32.45\00:07:33.85 for 25 years I said, 00:07:33.88\00:07:35.22 "Man! I'm getting up there, you know." 00:07:35.25\00:07:36.72 Yeah, yeah. 00:07:36.75\00:07:38.09 But that's a good thing not to have 00:07:38.12\00:07:39.45 and just keep drinking lots of water, lots of fiber 00:07:39.49\00:07:41.49 are good ways to keep from being constipate. 00:07:41.52\00:07:43.66 Now gravity, the tube needs gravity, so just, 00:07:43.69\00:07:47.50 you know just standing up and moving. 00:07:47.53\00:07:49.53 And when we move and exercise, guess what? 00:07:49.56\00:07:52.07 There's muscles involved in this process as well. 00:07:52.10\00:07:55.54 Muscles in the esophagus 00:07:55.57\00:07:56.94 that put the food down into the stomach. 00:07:56.97\00:07:59.14 The bowels also have, 00:07:59.17\00:08:00.51 we call it motility where the food moves. 00:08:00.54\00:08:02.94 So just a good exercise program 00:08:02.98\00:08:05.08 helps move things through as well. 00:08:05.11\00:08:06.98 Yeah. 00:08:07.02\00:08:08.35 Now if you're a couch potato and don't exercise 00:08:08.38\00:08:09.72 and lay down after you eat every meal, 00:08:09.75\00:08:11.32 the tube can't work as efficiently. 00:08:11.35\00:08:13.56 So this process can't move from A to B as easy. 00:08:13.59\00:08:17.09 So gravity is our friend 00:08:17.13\00:08:18.46 when it comes to moving the food 00:08:18.49\00:08:19.83 through the systems. 00:08:19.86\00:08:21.20 Exactly. Okay. 00:08:21.23\00:08:22.56 One of our greatest friends of this tube is fiber. 00:08:22.60\00:08:24.13 Okay. 00:08:24.17\00:08:25.73 I've read a statistic recently 00:08:25.77\00:08:27.27 that 95 percent of Americans don't get enough fiber. 00:08:27.30\00:08:32.07 This tube needs fiber for couple of reasons. 00:08:32.11\00:08:34.84 Fiber helps move things through, 00:08:34.88\00:08:36.61 it speeds up the motility of the bowel, 00:08:36.64\00:08:39.15 so the bowel moves things through faster. 00:08:39.18\00:08:42.02 Of course that will help with constipation. 00:08:42.05\00:08:44.32 It also helps move water in and out 00:08:44.35\00:08:46.76 but more than that fiber helps regulate 00:08:46.79\00:08:49.42 all the bacteria downstream. 00:08:49.46\00:08:51.06 Right. 00:08:51.09\00:08:52.43 This microbiome, and we know that people 00:08:52.46\00:08:53.80 that don't get enough fiber, 00:08:53.83\00:08:55.16 it hurts the flora or the bacteria in the gut. 00:08:55.20\00:08:59.30 Right, mm-hmm. 00:08:59.33\00:09:00.67 You make more bad bacteria and less good bacteria. 00:09:00.70\00:09:02.77 Mm-hmm. 00:09:02.80\00:09:04.14 So just getting fiber helps in the regulation 00:09:04.17\00:09:05.84 and the movement of the gut bacteria. 00:09:05.87\00:09:08.21 So that's a very good thing. 00:09:08.24\00:09:10.38 And of course other things that affect the tube 00:09:10.41\00:09:13.15 through time is inflammation. 00:09:13.18\00:09:15.42 If you have, what would cause inflammation? 00:09:15.45\00:09:17.55 Eating the wrong foods, having a disease state 00:09:17.59\00:09:20.52 that we're gonna talk about a little bit later. 00:09:20.56\00:09:22.32 If you have a malignancy, that can cause problems. 00:09:22.36\00:09:24.86 So that it gives us a general understanding. 00:09:24.89\00:09:27.43 Well, let's talk about the pain, okay. 00:09:27.46\00:09:29.80 Mm-hmm. 00:09:29.83\00:09:31.17 You know, I'm talking about pain 00:09:31.20\00:09:32.53 usually from the waist down. 00:09:32.57\00:09:33.90 Okay. 00:09:33.94\00:09:35.27 Now sometimes heart pain 00:09:35.30\00:09:36.64 can sort of feel like abdominal pain. 00:09:36.67\00:09:38.01 So you want to make sure you're not having heart pain. 00:09:38.04\00:09:39.37 So if the pain won't go away, 00:09:39.41\00:09:41.21 if a pain happens when you walk, 00:09:41.24\00:09:43.04 those pains need to be evaluated right away. 00:09:43.08\00:09:45.51 Yeah. Okay. 00:09:45.55\00:09:46.88 But chronic abdominal pain that goes on 00:09:46.92\00:09:49.52 for a long periods of time, it's not anything serious, 00:09:49.55\00:09:52.32 that really requires a complete evaluation. 00:09:52.35\00:09:56.02 From looking at all the things, the common sense things 00:09:56.06\00:09:58.76 that we've talked about to looking at things like 00:09:58.79\00:10:01.33 inflammation, infection, malignancies 00:10:01.36\00:10:05.40 and doctors can do that now fairly easy 00:10:05.43\00:10:07.44 by looking down with different scopes. 00:10:07.47\00:10:09.54 We can look above in the esophagus, 00:10:09.57\00:10:12.61 in the stomach we call that an EGD. 00:10:12.64\00:10:14.98 Okay. 00:10:15.01\00:10:16.34 And we can look below through a colonoscopy. 00:10:16.38\00:10:18.51 Now the small bowel is a little bit harder to see, 00:10:18.55\00:10:21.05 but now we have pill cams and other ways 00:10:21.08\00:10:23.49 to look at the small bowel as well. 00:10:23.52\00:10:25.19 Okay, right. 00:10:25.22\00:10:26.55 So we look in there to see if there's a disease state. 00:10:26.59\00:10:29.02 So if a person has pain, Nick, that will not go away, 00:10:29.06\00:10:32.83 that the belly is tight, and they're running a fever 00:10:32.86\00:10:35.56 and they feel bloated 00:10:35.60\00:10:36.93 and things like exploding inside of them, 00:10:36.97\00:10:38.30 that's a trip to the emergency room. 00:10:38.33\00:10:40.27 Okay. Okay. 00:10:40.30\00:10:41.64 A little bit of pain if the belly 00:10:41.67\00:10:43.10 is not tight, no fever, 00:10:43.14\00:10:44.47 you know, that's a keep an eye on type of pain. 00:10:44.51\00:10:46.17 Right, right. 00:10:46.21\00:10:47.54 But the pain that where the belly gets tight, 00:10:47.58\00:10:48.91 you're running a fever, 00:10:48.94\00:10:50.28 you're getting nausea and vomiting, 00:10:50.31\00:10:51.65 those are serious symptoms from the GI tract, 00:10:51.68\00:10:53.95 you need to get help right away. 00:10:53.98\00:10:55.78 Now also when the GI tract, not only you have pain 00:10:55.82\00:10:58.95 but pain associated with diarrhea 00:10:58.99\00:11:01.06 that might be a serious symptom as well. 00:11:01.09\00:11:03.53 Because diarrhea in the bowels 00:11:03.56\00:11:05.09 changing the way they function, 00:11:05.13\00:11:06.73 that could be a sign that you have something, 00:11:06.76\00:11:09.13 I'm going on that's more than just you know, 00:11:09.16\00:11:11.43 the bowel not working well. 00:11:11.47\00:11:12.80 You might have food poisoning. 00:11:12.83\00:11:14.54 You might have some bad inflammation. 00:11:14.57\00:11:16.87 But the real danger 00:11:16.91\00:11:18.24 is if you have lots of diarrhea, 00:11:18.27\00:11:19.61 you lose electrolytes in water. 00:11:19.64\00:11:21.74 And you can get dehydrated pretty quickly 00:11:21.78\00:11:24.61 with severe diarrhea. 00:11:24.65\00:11:26.45 So and talking about this, 00:11:26.48\00:11:28.88 those are some just general approaches 00:11:28.92\00:11:31.49 so you know, we want to make sure 00:11:31.52\00:11:32.85 our maintenance is good, 00:11:32.89\00:11:34.22 we chew well, we get lots of water, 00:11:34.26\00:11:36.83 have lots of gravity. 00:11:36.86\00:11:38.93 The bowel needs rest, you can't eat all the time, you know. 00:11:38.96\00:11:41.16 That's right, yeah. 00:11:41.20\00:11:42.53 That you need to rest your bowel, 00:11:42.56\00:11:43.90 especially we don't want to eat big at night. 00:11:43.93\00:11:45.47 We want to let the bowel rest at night, so it can recover. 00:11:45.50\00:11:49.87 It can sort of have a break, 00:11:49.90\00:11:51.24 it can sort of get itself together. 00:11:51.27\00:11:52.87 You think people will sleep better 00:11:52.91\00:11:54.24 with that strategy? 00:11:54.28\00:11:55.61 Absolutely, absolutely.
