That's right. 00:03:22.30\00:03:23.63 A healing belief. 00:03:23.67\00:03:25.00 There's real chemistry even though 00:03:25.03\00:03:26.37 we can't explain it. 00:03:26.40\00:03:27.74 That's right, that's right. 00:03:27.77\00:03:29.10 Well, let's get to a few of these questions 00:03:29.14\00:03:30.47 on muscle pain. 00:03:30.51\00:03:32.24 The first one is from a viewer who asks, 00:03:32.27\00:03:34.18 what is the treatment for a muscle spasm? 00:03:34.21\00:03:36.18 Okay. 00:03:36.21\00:03:37.55 So first, what is a muscle spasm 00:03:37.58\00:03:38.91 and what causes the spasms? 00:03:38.95\00:03:40.28 Yeah. 00:03:40.32\00:03:41.65 You know, muscle spasms is just what it says, 00:03:41.68\00:03:43.02 the muscle spasm, it contracts 00:03:43.05\00:03:44.39 when you're not telling it to contract. 00:03:44.42\00:03:46.65 Okay, it usually balls up and it hurts. 00:03:46.69\00:03:49.72 Yeah. Okay. 00:03:49.76\00:03:51.39 The first thing is you know, if you have it, you know, 00:03:51.43\00:03:53.56 you're not gonna say, well, what's causing my...? 00:03:53.60\00:03:55.56 No, you want to do something right now. 00:03:55.60\00:03:56.93 Right, yeah. 00:03:56.97\00:03:58.30 And the things that you can do right now is usually 00:03:58.33\00:03:59.67 stretching it into a position. 00:03:59.70\00:04:01.34 You know, you've seen in the leg cramps 00:04:01.37\00:04:03.61 how you change the position of the foot. 00:04:03.64\00:04:06.31 A massage helps with that. Yeah. 00:04:06.34\00:04:08.61 Ice will also help with that. 00:04:08.64\00:04:10.75 And then once you get it to go away, you know, 00:04:10.78\00:04:13.05 fluids will help. 00:04:13.08\00:04:14.42 Then you say what caused it to happen, 00:04:14.45\00:04:16.69 overuse, 00:04:16.72\00:04:18.05 I'm not having enough water, 00:04:18.09\00:04:19.65 I'm not having enough electrolytes, 00:04:19.69\00:04:21.22 those are some of the more common causes 00:04:21.26\00:04:23.36 of acute spasm. 00:04:23.39\00:04:24.96 So is the spasm the some thing as a cramp? 00:04:24.99\00:04:27.00 Yes. Okay. 00:04:27.03\00:04:28.36 Cramp, spasm, we're talking the same... 00:04:28.40\00:04:29.73 Same kind of thing. The same verbiage, Nick. 00:04:29.76\00:04:31.10 Except one's a little bit higher 00:04:31.13\00:04:32.93 and you know spasm. 00:04:32.97\00:04:34.50 You know, and the other one's a little bit lower in cramp. 00:04:34.54\00:04:36.97 And we're counting. 00:04:37.01\00:04:38.34 But it's the same verbiage. 00:04:38.37\00:04:39.71 But those are the, you know, the stretch to relieve 00:04:39.74\00:04:41.68 that spasm to take pressure off the muscle. 00:04:41.71\00:04:44.35 And then of course, you know, massage to sort of work that, 00:04:44.38\00:04:47.68 you know, that muscle, you know, that's contracting, 00:04:47.72\00:04:51.32 work that contraction out. 00:04:51.35\00:04:52.95 Yeah. 00:04:52.99\00:04:54.32 You know, some can be so bad, it's like a really solid muscle 00:04:54.36\00:04:56.19 that's like really cramping so you can feel 00:04:56.22\00:04:58.19 a ball of muscle there that's contacting. 00:04:58.23\00:05:00.50 It's kind of a knot. 00:05:00.53\00:05:01.86 Well, it's not supposed to be doing that. 00:05:01.90\00:05:03.37 Right, right. Yeah. 00:05:03.40\00:05:05.90 Another viewer writes in and asks 00:05:05.93\00:05:07.27 when should muscle pain 00:05:07.30\00:05:08.64 prompt a visit to the emergency room? 00:05:08.67\00:05:10.01 Yeah, that's a good question, 00:05:10.04\00:05:11.37 I think we alluded to that earlier. 00:05:11.41\00:05:14.81 If you have, you know, severe, you know, 00:05:14.84\00:05:16.78 these severity hydration, you going on a walk, 00:05:16.81\00:05:19.15 you pass out. 00:05:19.18\00:05:20.52 But you know people are gonna take you there anyway. 00:05:20.55\00:05:21.88 Yeah. 00:05:21.92\00:05:23.25 If you find grandma that's been on the floor for three days, 00:05:23.28\00:05:25.15 you know that's a trip to the emergency room to see 00:05:25.19\00:05:26.89 if they have muscle breakdown. 00:05:26.92\00:05:28.32 Right. 00:05:28.36\00:05:29.69 But as usually the bane what I wanna tell people 00:05:29.72\00:05:32.46 is if you find someone with a stick stiff neck, 00:05:32.49\00:05:35.30 muscle pain in the neck and a fever, 00:05:35.33\00:05:37.63 that could be a serious infection 00:05:37.67\00:05:39.60 called meningitis. 00:05:39.63\00:05:41.30 So that's muscle pain, stiff neck and a fever, 00:05:41.34\00:05:43.57 that could be a meningitis type situation. 00:05:43.61\00:05:46.24 That would be a trip to the emergency room 00:05:46.27\00:05:48.58 to get that treated right away. 00:05:48.61\00:05:50.31 Right, right. 00:05:50.35\00:05:52.48 What are some... 00:05:52.51\00:05:53.85 So that's a trip to the emergency room. 00:05:53.88\00:05:55.22 That's right. 00:05:55.25\00:05:56.58 There're different places to go when you have muscle pains. 00:05:56.62\00:05:57.95 That's right. That's ER. 00:05:57.99\00:05:59.32 So I was gonna ask, if you're gonna stay at home, 00:05:59.35\00:06:00.69 what are some of the things you can do if your situation 00:06:00.72\00:06:02.76 is deemed not serious enough to go to the ER? 00:06:02.79\00:06:05.33 What are some things you should try at home? 00:06:05.36\00:06:06.93 Okay. 00:06:06.96\00:06:08.30 Well, you know to self treat it rest it, massage it. 00:06:08.33\00:06:12.83 Hydrate it. 00:06:12.87\00:06:14.20 Hydrate it, put some ice on it, 00:06:14.24\00:06:16.40 compression and we have this 00:06:16.44\00:06:19.07 and we call it the RICE treatment. 00:06:19.11\00:06:20.74 Okay, the RICE treatment rest, ice, compress, elevate. 00:06:20.78\00:06:26.51 Okay. 00:06:26.55\00:06:27.88 Okay, if that, now what would require muscle pain 00:06:27.92\00:06:31.02 and a trip to the doctor's office? 00:06:31.05\00:06:32.89 Now I'm not telling you the emergency room, 00:06:32.92\00:06:34.26 a doctor's office. 00:06:34.29\00:06:35.86 If you have a rash with it with muscle aches, okay, 00:06:35.89\00:06:39.33 a tick bite, you've heard of rickettsia and lymes 00:06:39.36\00:06:43.20 that can sometimes a tick bite can cause muscle aches. 00:06:43.23\00:06:46.03 Okay. 00:06:46.07\00:06:47.40 If the muscle aches lasts longer than a week, okay. 00:06:47.44\00:06:51.71 If it's a combination of the muscles red and has pain 00:06:51.74\00:06:56.11 those might be the trips to the doctor 00:06:56.14\00:06:57.91 to dilate a little bit more. 00:06:57.95\00:06:59.68 Okay. 00:06:59.71\00:07:01.05 So that sort of let you know which one should you do it 00:07:01.08\00:07:02.42 at home RICE, what you do if it's little more involved 00:07:02.45\00:07:06.02 because of the doctor and then if it's a lot involved 00:07:06.05\00:07:08.72 you know, this extreme weakness, 00:07:08.76\00:07:10.83 stiff neck then maybe 00:07:10.86\00:07:12.19 that's a trip to the emergency room. 00:07:12.23\00:07:13.70 Right, so another viewer asks, 00:07:13.73\00:07:15.63 what tests might help diagnose muscle pain, 00:07:15.66\00:07:18.77 say at the doctor's office? 00:07:18.80\00:07:20.14 Yeah. 00:07:20.17\00:07:21.50 Well, one of the test that help a lot. 00:07:21.54\00:07:22.87 When a muscle breaks down, 00:07:22.90\00:07:24.41 it spits out protein into the blood 00:07:24.44\00:07:26.94 because the muscle is breaking down. 00:07:26.98\00:07:28.34 Okay. Mm-hmm. 00:07:28.38\00:07:29.71 And so we can measure the muscle products in it, 00:07:29.74\00:07:31.28 and one of the ones 00:07:31.31\00:07:32.65 we measure is creatine phosphokinase or CPK. 00:07:32.68\00:07:35.92 Okay. 00:07:35.95\00:07:37.29 Sometimes we can have, 00:07:37.32\00:07:38.65 when we have muscle break down, 00:07:38.69\00:07:40.02 we can have elevated inflammation. 00:07:40.06\00:07:41.39 We can measure that with a sed rate. 00:07:41.42\00:07:43.29 If we think the muscle is breaking down 00:07:43.32\00:07:44.96 for lots of overuse, 00:07:44.99\00:07:46.39 we can see if there's lactic acid doses developing. 00:07:46.43\00:07:48.83 But that is severe, a severe case. 00:07:48.86\00:07:51.33 Sometimes when there's cancer in the body 00:07:51.37\00:07:54.07 and the cells are metabolically turning over quickly. 00:07:54.10\00:07:56.94 Uric acid in the blood might be a clue for that. 00:07:56.97\00:08:00.88 If we think that the body is attacking itself, you know, 00:08:00.91\00:08:03.68 fibromyalgia or some of those. 00:08:03.71\00:08:05.18 We can measure chemicals like that. 00:08:05.21\00:08:07.42 One that we measure is called "antinuclear antibody," 00:08:07.45\00:08:10.99 ANA we can do that too. 00:08:11.02\00:08:12.75 And then there are some diseases 00:08:12.79\00:08:14.36 we haven't talked about where the muscles aches 00:08:14.39\00:08:16.39 and there's diseases in the muscle. 00:08:16.42\00:08:18.43 You've heard and we haven't talked much about these 00:08:18.46\00:08:20.43 conditions like muscular dystrophy. 00:08:20.46\00:08:22.46 You know, other things that are going on 00:08:22.50\00:08:24.10 where the brain can't control the muscles. 00:08:24.13\00:08:26.10 We can diagnose those things 00:08:26.13\00:08:27.64 with test like electromylogram. 00:08:27.67\00:08:30.17 We measure how the muscle contracts, 00:08:30.21\00:08:32.37 where we can do a biopsy 00:08:32.41\00:08:34.41 to see if there is some inherit thing 00:08:34.44\00:08:36.41 wrong with the muscle. 00:08:36.44\00:08:37.78 Some infections can make the muscles very, very weak. 00:08:37.81\00:08:40.68 I wrote a paper years ago about a patient 00:08:40.72\00:08:43.45 that presented with polymyositis, okay. 00:08:43.49\00:08:46.76 He was a young person that was, his muscles were so weak 00:08:46.79\00:08:50.33 that he couldn't even brush his hair. 00:08:50.36\00:08:52.76 That's how weak all his muscles were. 00:08:52.79\00:08:54.46 And we went through all of these causes 00:08:54.50\00:08:56.46 and they were weak and they were sore. 00:08:56.50\00:08:58.43 And usually they're not sore, they're just weak. 00:08:58.47\00:09:00.50 And his cause was he had AIDS, he had an HIV infection 00:09:00.54\00:09:04.07 and that was causing all these muscles 00:09:04.11\00:09:05.71 all over the body. 00:09:05.74\00:09:07.08 He wasn't sore but they were weak. 00:09:07.11\00:09:08.44 Right, yeah. 00:09:08.48\00:09:09.81 So those are some simple things 00:09:09.84\00:09:11.18 that we can do to sort of diagnose 00:09:11.21\00:09:13.15 what these muscle problems 00:09:13.18\00:09:14.52 and whether they're real serious 00:09:14.55\00:09:15.88 or it's something that's gonna pass. 00:09:15.92\00:09:17.55 Now you mentioned lactic acid doses earlier. 00:09:17.59\00:09:20.36 I ride bikes a lot 00:09:20.39\00:09:21.72 and we always hear about something 00:09:21.76\00:09:23.26 called lactic threshold and it's usually associated 00:09:23.29\00:09:25.79 where your heart rate gets up to a certain number 00:09:25.83\00:09:29.56 where you're producing more lactic acid 00:09:29.60\00:09:31.50 than your blood stream can clear. 00:09:31.53\00:09:33.23 Is that same thing? 00:09:33.27\00:09:34.60 That's the same thing and then guess what? 00:09:34.64\00:09:35.97 And why is that dangerous? 00:09:36.00\00:09:37.34 Well, because you change the pH in your body. 00:09:37.37\00:09:40.08 Yeah, more acid. 00:09:40.11\00:09:41.44 Yes, more acidic, sometimes you can't breathe fast enough 00:09:41.48\00:09:44.68 to burn off the acid. 00:09:44.71\00:09:46.05 And then the muscle can actually break down 00:09:46.08\00:09:48.52 under acidotic conditions. 00:09:48.55\00:09:50.45 So you can't exercise too much. 00:09:50.49\00:09:53.82 Yeah. And it's sort of rare. 00:09:53.86\00:09:55.59 But for instance, 00:09:55.62\00:09:56.96 that's why people that exercise, 00:09:56.99\00:09:58.33 they have to make sure they get their glucose, 00:09:58.36\00:10:00.80 they have to make sure they get their water, 00:10:00.83\00:10:02.56 they make sure that there's a rest. 00:10:02.60\00:10:03.93 And if they start having symptoms 00:10:03.97\00:10:05.53 just stop, don't do it. 00:10:05.57\00:10:07.20 Don't do it, it's not worth becoming dehydrated, 00:10:07.24\00:10:09.57 because a lot of people don't realize it, Nick, 00:10:09.60\00:10:11.21 but when their extreme exercises, 00:10:11.24\00:10:13.27 your temperature 00:10:13.31\00:10:14.64 because your metabolism goes up. 00:10:14.68\00:10:16.01 Right. 00:10:16.04\00:10:17.38 And that high metabolism starts to break down things. 00:10:17.41\00:10:19.35 Remember the body operates 00:10:19.38\00:10:20.98 under a fairly narrow temperature range. 00:10:21.02\00:10:23.35 Hmm, right. 00:10:23.39\00:10:24.72 So when you go above 102 degrees 00:10:24.75\00:10:26.09 above that hyperthermia, okay. 00:10:26.12\00:10:27.96 If you, your body can't regulate that temperature, 00:10:27.99\00:10:30.59 you start breaking down. 00:10:30.63\00:10:32.19 Your brain doesn't think well, 00:10:32.23\00:10:33.90 you start breaking down the body parts... 00:10:33.93\00:10:35.86 Your organs will quit functioning as well. 00:10:35.90\00:10:37.30 Acid doses, it could be real serious 00:10:37.33\00:10:39.07 because hyperthermia is not a good thing to have. 00:10:39.10\00:10:41.90 Yeah. 00:10:41.94\00:10:43.27 But a little bit of rest 00:10:43.30\00:10:44.64 before you get to those serious stages. 00:10:44.67\00:10:46.01 A little bit of rest will help reduce that lactic acid doses. 00:10:46.04\00:10:47.84 Yes, rest, hydration get your body, 00:10:47.88\00:10:51.38 give it what it needs basically. 00:10:51.41\00:10:52.75 All right.
