Yeah. 00:02:54.54\00:02:55.87 And if you think, when I think about a person 00:02:55.90\00:02:57.77 that comes in and evaluates a headache, 00:02:57.81\00:02:59.77 the first thing I want to know is, 00:02:59.81\00:03:01.41 do I think the headache's caused by chemical imbalances, 00:03:01.44\00:03:04.45 okay? 00:03:04.48\00:03:05.81 What do I mean by that? 00:03:05.85\00:03:07.72 The nerves and the blood vessels 00:03:07.75\00:03:09.68 being out of whack, 00:03:09.72\00:03:11.05 muscles in the neck. 00:03:11.09\00:03:12.82 If there are chemical imbalances, 00:03:12.85\00:03:14.32 we call that primary headaches, okay? 00:03:14.36\00:03:17.53 Now, if there are bigger problems, 00:03:17.56\00:03:20.10 we call that secondary 00:03:20.13\00:03:21.66 and that's usually structural problems in the brain. 00:03:21.70\00:03:24.73 Those can be life threatening. 00:03:24.77\00:03:27.50 For instance, some of the structural problems 00:03:27.54\00:03:29.27 that you might have in the brain 00:03:29.30\00:03:30.64 if you had an aneurysm, okay? 00:03:30.67\00:03:32.27 Or if you had a intracerebral bleed, 00:03:32.31\00:03:34.84 that could be life threatening if not treated right away. 00:03:34.88\00:03:38.45 I mean, we're gonna talk a little bit 00:03:38.48\00:03:39.81 about how you might know that 00:03:39.85\00:03:41.18 that's type of your headache. 00:03:41.22\00:03:42.55 So there's basically two big types of headaches 00:03:42.58\00:03:44.35 and a lot of it you can find out 00:03:44.39\00:03:45.92 without even testing at all. 00:03:45.95\00:03:47.36 You can just ask the patient lots of questions. 00:03:47.39\00:03:51.26 Some of the common causes of primary headaches are, 00:03:51.29\00:03:54.70 the most common is tension headaches, okay? 00:03:54.73\00:03:57.67 And tension headache is when the muscles 00:03:57.70\00:03:59.77 of the head get tight. 00:03:59.80\00:04:02.10 You know, if the muscles in your arm get tight, 00:04:02.14\00:04:04.37 it can cause pain, okay? 00:04:04.41\00:04:06.24 If it gets, you know, we got a charley horse. 00:04:06.27\00:04:07.61 Kind of a cramp in the leg. 00:04:07.64\00:04:08.98 Yeah, you have the charley horse, 00:04:09.01\00:04:10.35 you don't worry about that 'cause you know, 00:04:10.38\00:04:11.71 it's the muscles in the legs. 00:04:11.75\00:04:13.08 Right. 00:04:13.11\00:04:14.45 Occasionally we have cramping in the muscles of the chest 00:04:14.48\00:04:15.82 and people should get worried 00:04:15.85\00:04:17.19 'cause it could feel like a heart attack. 00:04:17.22\00:04:18.55 Well, the muscles of the head can also get tight too. 00:04:18.59\00:04:21.79 In this world, we are under so much stress. 00:04:21.82\00:04:24.36 We make the stress chemistries, adrenaline, the cytokines. 00:04:24.39\00:04:28.43 It can make muscles tight. 00:04:28.46\00:04:30.47 I mean, you can have what we call tension headaches. 00:04:30.50\00:04:33.54 Typically, tension headaches are usually headaches. 00:04:33.57\00:04:36.04 If you talk to the patients, they say, 00:04:36.07\00:04:37.41 oh, it's behind their eyes 00:04:37.44\00:04:39.17 or at the base of their neck, okay? 00:04:39.21\00:04:41.24 Those are characteristic places for tension headaches, 00:04:41.28\00:04:44.25 especially behind the eyes. 00:04:44.28\00:04:46.55 So that's a type of primary headache. 00:04:46.58\00:04:48.72 Another type of primary headache might occur like this. 00:04:48.75\00:04:51.85 You have a headache behind the eye, 00:04:51.89\00:04:54.69 usually one eye that awakens you 00:04:54.72\00:04:56.22 in the middle of night. 00:04:56.26\00:04:57.93 And that can be termed a cluster headache, okay? 00:04:57.96\00:05:01.06 And that can be very devastating. 00:05:01.10\00:05:03.06 Sometimes there's triggers for cluster headaches. 00:05:03.10\00:05:06.07 Some triggers might be alcohol, cigarettes, 00:05:06.10\00:05:08.74 they might trigger these headaches. 00:05:08.77\00:05:10.17 It disrupts the vessels 00:05:10.21\00:05:11.77 in the internal functioning of the brain enough, 00:05:11.81\00:05:14.38 so you have this severe pain. 00:05:14.41\00:05:16.71 Another type of headache 00:05:16.75\00:05:18.51 that almost everyone has heard of is migraine headaches. 00:05:18.55\00:05:21.35 Yeah, right. 00:05:21.38\00:05:22.72 And that's still a... 00:05:22.75\00:05:24.09 one of these headaches that are, you know, 00:05:24.12\00:05:26.29 they are not caused by structural problem, 00:05:26.32\00:05:28.19 they are more of the chemistry of the brain. 00:05:28.22\00:05:30.36 And a migraine headache, usually they might have it 00:05:30.39\00:05:32.79 or you might feel it coming before it does. 00:05:32.83\00:05:35.40 You might, you might have some numbness in your tongue, 00:05:35.43\00:05:37.87 in your fingers. 00:05:37.90\00:05:39.47 Sometimes it has triggers too. 00:05:39.50\00:05:42.10 Sometimes the light can trigger it. 00:05:42.14\00:05:44.41 Sometimes dehydration, 00:05:44.44\00:05:45.87 certain foods with nitrates can trigger it. 00:05:45.91\00:05:48.71 Lack of sleep can trigger these headaches 00:05:48.74\00:05:50.45 and they are very devastating too. 00:05:50.48\00:05:52.91 Some of the treatments of these types of headaches 00:05:52.95\00:05:55.15 are medicinal to relieve the pain. 00:05:55.18\00:05:57.45 If we can, we want, of course, get at the cause. 00:05:57.49\00:06:00.66 Getting at the cause is the key if possible, 00:06:00.69\00:06:03.86 'cause they devastate... 00:06:03.89\00:06:05.59 You remember, chronic pain 00:06:05.63\00:06:06.96 turns on chronic stress chemistry 00:06:07.00\00:06:08.56 which damages other parts of the body 00:06:08.60\00:06:10.70 including the higher parts of the brain 00:06:10.73\00:06:12.73 which does our thinking. 00:06:12.77\00:06:14.40 So that's one big type of headaches, okay? 00:06:14.44\00:06:17.77 When would a headache be life threatening, okay? 00:06:17.81\00:06:21.34 What are some things I should worry about? 00:06:21.38\00:06:23.45 Well, if the headache's chronic every single day, 00:06:23.48\00:06:26.18 never goes away, that might be serious. 00:06:26.21\00:06:28.58 You need to get to see someone about that. 00:06:28.62\00:06:30.79 The doctor that specializes in headaches 00:06:30.82\00:06:32.89 is called a neurologist. 00:06:32.92\00:06:34.79 They specialize in headaches. 00:06:34.82\00:06:36.36 If a headache comes after a cough, 00:06:36.39\00:06:38.89 that might be dangerous. 00:06:38.93\00:06:40.70 If it's a worse headache in your life, 00:06:40.73\00:06:43.03 that might be dangerous. 00:06:43.06\00:06:44.87 Headaches usually have to have relationships. 00:06:44.90\00:06:47.07 That can be a dangerous signal for dangerous headaches. 00:06:47.10\00:06:50.14 And all these headaches need to be evaluated right away. 00:06:50.17\00:06:52.94 And the way we sort of tell if it's a serious headache 00:06:52.97\00:06:55.41 is with a scan. 00:06:55.44\00:06:56.78 We look at the structure, 00:06:56.81\00:06:58.15 the structures of the brain that they look okay, 00:06:58.18\00:07:00.55 we can know it's not a serious headache. 00:07:00.58\00:07:02.38 And sometimes, a patient will come into the emergency room 00:07:02.42\00:07:05.42 and they have the worst headache of their life. 00:07:05.45\00:07:07.26 We'll do the scan and first we say, 00:07:07.29\00:07:08.72 "Oh, it's not dangerous." 00:07:08.76\00:07:10.56 It must be one of these other primary types of headaches, 00:07:10.59\00:07:13.19 the tensions, the clusters or the migraine. 00:07:13.23\00:07:15.53 We can symptomatically help that with medications, 00:07:15.56\00:07:18.37 get you over the hump 00:07:18.40\00:07:19.73 and then help you educate on the cause of the brain 00:07:19.77\00:07:22.50 to prevent it from happening long term. 00:07:22.54\00:07:25.01 But the structural problems, we can't do a whole lot about. 00:07:25.04\00:07:28.34 You know, a lot of us are born with, you know, 00:07:28.38\00:07:30.31 if you have an aneurysm, that's a weak spot, 00:07:30.35\00:07:33.58 the blood vessel if it bleeds in the brain. 00:07:33.62\00:07:36.08 Sometimes you can't do too much about that 00:07:36.12\00:07:37.85 other than control the blood pressure before. 00:07:37.89\00:07:40.26 But for primary headaches, 00:07:40.29\00:07:41.62 anything that improves brain health 00:07:41.66\00:07:44.33 will lower your risk of that. 00:07:44.36\00:07:46.23 What would improve brain health? 00:07:46.26\00:07:47.76 Drinking water, getting rest and eating the right foods. 00:07:47.80\00:07:52.63 Some of the chemicals, you know, you've heard of MSG, 00:07:52.67\00:07:55.47 some of those chemicals can adversely affect your brain. 00:07:55.50\00:07:59.17 Low blood sugar can give you a headache. 00:07:59.21\00:08:01.61 High blood sugar can give you headache. 00:08:01.64\00:08:03.38 So anything that interferes with the normal function 00:08:03.41\00:08:06.65 of the brain can cause the brain to malfunction 00:08:06.68\00:08:09.55 that could create the nerves in the brain saying, 00:08:09.58\00:08:11.39 "Hey, things aren't right. 00:08:11.42\00:08:12.75 I'm going to cause a headache. 00:08:12.79\00:08:14.12 I'm going to get Nick's attention." 00:08:14.16\00:08:15.49 And then Nick says, "I have a headache. 00:08:15.52\00:08:16.86 I don't feel so good." 00:08:16.89\00:08:18.23 And then he's going to figure it out. 00:08:18.26\00:08:19.59 You know, and if it's not serious, 00:08:19.63\00:08:20.96 you know, you've to make sure you, 00:08:21.00\00:08:22.33 you know, slept it off, drank some water. 00:08:22.36\00:08:24.63 If it's something more serious, you'll get help right away. 00:08:24.67\00:08:28.50 There's all sorts of structural type of headaches too 00:08:28.54\00:08:31.07 that we need to sort of talk a few minutes about. 00:08:31.11\00:08:33.48 Sinuses. 00:08:33.51\00:08:34.98 Sinuses that get infected or you have allergies, 00:08:35.01\00:08:38.51 those are frequently cause of headaches. 00:08:38.55\00:08:41.02 Those type of headaches are usually associated 00:08:41.05\00:08:43.08 when that hurts when you press over the sinuses. 00:08:43.12\00:08:45.59 You might have a green drainage, 00:08:45.62\00:08:47.52 something like that might be happening. 00:08:47.56\00:08:50.06 Of course, you can have headaches 00:08:50.09\00:08:51.43 if you have a concussion. 00:08:51.46\00:08:53.09 If you bang your head, you know. 00:08:53.13\00:08:54.46 You are playing football and have a lot of... 00:08:54.50\00:08:55.83 Mental trauma? Yes. 00:08:55.86\00:08:57.20 So if you are having headaches after you bang your head, 00:08:57.23\00:08:59.27 that could be a structural problem. 00:08:59.30\00:09:01.54 So that would need a more thorough evaluation. 00:09:01.57\00:09:05.04 You know, you heard of concussions. 00:09:05.07\00:09:06.74 And almost all the gentlemen 00:09:06.78\00:09:08.11 that have concussions have headaches. 00:09:08.14\00:09:10.35 Some of them have bleeds, some of them don't have bleeds 00:09:10.38\00:09:13.01 and it's just chronic damage to the brain 00:09:13.05\00:09:14.92 but that would be something we need to look into. 00:09:14.95\00:09:18.29 Sometimes if you have a headache 00:09:18.32\00:09:19.99 and a fever or a rash, 00:09:20.02\00:09:22.32 that could be a sign of infection, 00:09:22.36\00:09:24.59 especially if you had a stiff neck 00:09:24.63\00:09:27.60 and a rash and a fever, 00:09:27.63\00:09:29.10 that could be meningitis, 00:09:29.13\00:09:31.07 that could be due to an infection. 00:09:31.10\00:09:32.80 That's a potentially serious structural problem 00:09:32.83\00:09:36.00 in the brain. 00:09:36.04\00:09:37.64 There's lots of toxins 00:09:37.67\00:09:39.01 that can really hurt the brain really quick. 00:09:39.04\00:09:41.44 We think of some of the ones we mentioned, 00:09:41.48\00:09:43.14 MSG but carbon monoxide, 00:09:43.18\00:09:45.25 frequently that can hurt the brain 00:09:45.28\00:09:46.72 cause you to have these headaches. 00:09:46.75\00:09:49.08 Other types of headaches that are more structurally 00:09:49.12\00:09:51.29 are visual problems. 00:09:51.32\00:09:53.09 You know, if you have a bad vision, 00:09:53.12\00:09:55.12 if you can't see where your vision is changing. 00:09:55.16\00:09:57.23 Something's damaged your vision. 00:09:57.26\00:09:58.59 Sometimes the left and the right eye aren't right, 00:09:58.63\00:10:00.86 sometimes that put stress on the system, 00:10:00.90\00:10:03.10 that can create headache-type pains too. 00:10:03.13\00:10:05.37 If your eyes are straining a lot, 00:10:05.40\00:10:06.84 that can cause it. 00:10:06.87\00:10:08.20 Looking at the computer screen hour after hour, 00:10:08.24\00:10:11.07 watching television many hours. 00:10:11.11\00:10:13.11 You know, not giving your eyes some rest like they should. 00:10:13.14\00:10:16.44 That can cause chronic headaches as well. 00:10:16.48\00:10:20.08 Hangovers, you know, 00:10:20.12\00:10:21.85 a lot of people drink alcohol nowadays. 00:10:21.88\00:10:24.55 They drink too much, they develop hangovers. 00:10:24.59\00:10:27.16 That can cause problems. 00:10:27.19\00:10:28.79 A lot of people don't realize it 00:10:28.82\00:10:30.16 but certain medications can also cause headaches. 00:10:30.19\00:10:33.86 The overuse of pain medications. 00:10:33.90\00:10:36.10 You wouldn't think it can cause a headache 00:10:36.13\00:10:37.77 but the overuse of pain medications 00:10:37.80\00:10:39.43 can actually cause headaches. 00:10:39.47\00:10:41.60 So when a person comes in with a headache 00:10:41.64\00:10:43.81 and if you want to look at all the different possible causes, 00:10:43.84\00:10:46.44 from the eyes to the sinuses, 00:10:46.47\00:10:48.24 to structural problems to muscles, 00:10:48.28\00:10:50.31 to the ingestions they put in, 00:10:50.35\00:10:52.21 whether they are sleeping enough, 00:10:52.25\00:10:53.82 whether they've been having some trauma, 00:10:53.85\00:10:55.75 whether they are drinking enough water. 00:10:55.78\00:10:57.22 So it's really a very complex evaluation 00:10:57.25\00:11:00.26 but it needs to be done 'cause it could, 00:11:00.29\00:11:01.79 you know, occasionally, 00:11:01.82\00:11:03.53 a person has a bleed or a bad concussion 00:11:03.56\00:11:05.99 leading to a seizure, 00:11:06.03\00:11:07.36 those could all be life threatening. 00:11:07.40\00:11:09.16 So let's talk a little bit more about the chemical imbalance. 00:11:09.20\00:11:13.00 Are we talking about like maybe not drinking enough water 00:11:13.03\00:11:15.10 and so there's not the right chemicals in the brain, 00:11:15.14\00:11:18.24 or if you are drinking alcohol, 00:11:18.27\00:11:19.61 that's causing imbalance of chemicals in the brain? 00:11:19.64\00:11:21.14 Yes, it does that. 00:11:21.18\00:11:22.51 And remember, 80 to 85 percent of the brain is water, okay? 00:11:22.54\00:11:25.98 And usually when people drink alcohol, you know, 00:11:26.01\00:11:28.25 it affects the way the brain works, okay? 00:11:28.28\00:11:30.99 Not only does it water 00:11:31.02\00:11:32.35 but it affects actually the way the brain works, 00:11:32.39\00:11:34.06 the brain doesn't think as clear, okay? 00:11:34.09\00:11:36.09 But a lot of drugs do that, okay? 00:11:36.12\00:11:38.66 Cocaine, methamphetamines, LSD, 00:11:38.69\00:11:41.90 which is a big one that affect the way the brain works. 00:11:41.93\00:11:44.83 So any of those could potentially 00:11:44.87\00:11:46.27 lead to brain problems. 00:11:46.30\00:11:47.74 We want to stay away from all of those. 00:11:47.77\00:11:51.71 Some people, Nick, that have high blood pressure. 00:11:51.74\00:11:54.11 If the blood pressure's high all the time, 00:11:54.14\00:11:55.81 that can led to chronic headache, 00:11:55.84\00:11:57.35 so you always want to check their blood pressure. 00:11:57.38\00:12:00.35 You know, after some procedures that people get done, you know, 00:12:00.38\00:12:03.49 have you ever heard of a spinal tap? 00:12:03.52\00:12:05.52 You know, when they do those? 00:12:05.55\00:12:07.19 Some people after that can have headaches. 00:12:07.22\00:12:09.22 You know, they call it a spinal headache. 00:12:09.26\00:12:11.66 Some people, I've seen a couple of this through years 00:12:11.69\00:12:13.73 that they wear goggles when they either swim or bike, 00:12:13.76\00:12:17.37 the goggles are too tight. 00:12:17.40\00:12:18.73 It's from the pressure.
