00:01:15.64\00:01:18.17 Very important, Nick, 00:01:18.21\00:01:19.64 and if you think of what we've been talking 00:01:19.67\00:01:21.44 about this series, 00:01:21.48\00:01:22.81 we've been talking about pain in various parts of our body. 00:01:22.84\00:01:25.51 That's right. 00:01:25.55\00:01:26.88 And feet are definitely a part that has pain 00:01:26.92\00:01:30.12 and when you think about pain in your foot, 00:01:30.15\00:01:32.25 if your foot aren't happy, nothing's happy, you know, 00:01:32.29\00:01:35.49 you can't walk, you can't do things, 00:01:35.52\00:01:37.19 you can't exercise, you can't move, 00:01:37.23\00:01:39.09 you don't sleep well at night, 00:01:39.13\00:01:40.70 it hurts every time you put your shoe on, 00:01:40.73\00:01:43.20 it's a big problem. 00:01:43.23\00:01:45.43 So the doctors that take care of feet 00:01:45.47\00:01:47.87 are called podiatrist, they're experts in this. 00:01:47.90\00:01:50.51 That's right, yeah. 00:01:50.54\00:01:51.87 You know, we recently visited our friend Dr. Cheryl Haag, 00:01:51.91\00:01:53.91 at the Foot and Ankle Center 00:01:53.94\00:01:55.38 and she had some very interesting things 00:01:55.41\00:01:57.81 to share about common causes of foot pain, 00:01:57.85\00:02:00.42 let's take a look at the video now. 00:02:00.45\00:02:02.38 Some of the most common things that we see, 00:02:02.42\00:02:04.39 probably number one is heel pain 00:02:04.42\00:02:07.76 or plantar fasciitis associated with heel's burst. 00:02:07.79\00:02:11.86 We also see bunions, hammer toes, 00:02:11.89\00:02:15.43 lot of ingrown toe nails, fractures, diabetic problems. 00:02:15.46\00:02:21.10 Usually when people show up, they're hurting, 00:02:21.14\00:02:24.11 on a scale of zero to ten, if zero is no pain, 00:02:24.14\00:02:27.01 ten is the worst. 00:02:27.04\00:02:28.44 They're usually eight to ten 00:02:28.48\00:02:29.81 by the time they show up on our door step. 00:02:29.84\00:02:31.71 When a patient comes with pain, 00:02:31.75\00:02:33.08 we take a good history obviously, 00:02:33.11\00:02:34.48 wanna know if there's any injury 00:02:34.52\00:02:35.98 or something that is... 00:02:36.02\00:02:38.42 Some event that's contributing to the problem, 00:02:38.45\00:02:41.79 examine their foot, take x-rays, if it's... 00:02:41.82\00:02:45.53 We think there is a soft issue involved, 00:02:45.56\00:02:49.00 we have ultra sound here in the office, 00:02:49.03\00:02:50.97 so that we can... 00:02:51.00\00:02:52.33 X- rays locate bone, 00:02:52.37\00:02:53.70 ultra sound looks at soft issue. 00:02:53.74\00:02:56.50 If it's a problem of the way they're walking, 00:02:56.54\00:02:59.21 we watch them walk, we can evaluate that also. 00:02:59.24\00:03:03.98 People always ask me, what causes this pain? 00:03:04.01\00:03:06.58 If it's not associated with an injury, 00:03:06.61\00:03:08.98 why did I get this pain? 00:03:09.02\00:03:11.45 And you know, we can put a certain amount to hereditary, 00:03:11.49\00:03:16.06 may be just 10% or so but really it's shoes. 00:03:16.09\00:03:19.69 The type of shoe that you wear 00:03:19.73\00:03:21.66 or you do not wear, 00:03:21.70\00:03:23.93 that contributes a lot for all the thousands of steps 00:03:23.97\00:03:26.33 that you take in a day. 00:03:26.37\00:03:27.87 One of the major contributing causes of heel pain 00:03:27.90\00:03:31.37 is the lack of support. 00:03:31.41\00:03:33.07 Stress and strain on the arch of the foot 00:03:33.11\00:03:34.98 gives you heel pain, 00:03:35.01\00:03:36.54 and the most common cause of that 00:03:36.58\00:03:38.11 is wearing flip-flops, sandals, bare feet, 00:03:38.15\00:03:41.78 just shoes that do not have good support. 00:03:41.82\00:03:44.55 I'm just gonna show you 00:03:44.59\00:03:46.25 what I mean by supporting your shoe. 00:03:46.29\00:03:48.36 A shoe, when you're gonna buy, 00:03:48.39\00:03:49.79 it should never be able to bend in half, okay 00:03:49.82\00:03:53.26 there is poor support, 00:03:53.29\00:03:54.63 it should have a good counter in the back, 00:03:54.66\00:03:56.67 it should never flatten down 00:03:56.70\00:03:58.53 and a little bit of fabric and leather, 00:03:58.57\00:04:02.74 but again if you want to have good support, 00:04:02.77\00:04:04.24 you put in orthotic. 00:04:04.27\00:04:05.61 An orthotic is a custom type of arch support 00:04:05.64\00:04:08.21 that goes into the shoe that gives you support, 00:04:08.24\00:04:10.71 so that if you do have arch collapse 00:04:10.75\00:04:13.78 or problems with your arch, 00:04:13.82\00:04:15.52 that makes any shoe give the proper support. 00:04:15.55\00:04:18.55 If it's a problem of heel pain 00:04:18.59\00:04:20.46 which is the most common thing we see, 00:04:20.49\00:04:24.06 obviously we want them to rest, we want to put straps 00:04:24.09\00:04:27.90 or we want to give them arch supports, 00:04:27.93\00:04:29.70 we wanna put them in a better support of shoe 00:04:29.73\00:04:32.23 or a sneaker, 00:04:32.27\00:04:34.04 we want to give them anti-inflammatory 00:04:34.07\00:04:36.04 whether it's by mouth topically 00:04:36.07\00:04:38.17 or cortisone injections, we want to do stretching, 00:04:38.21\00:04:42.08 a lot of times it's just caused by tight tendons, 00:04:42.11\00:04:45.75 there's physical therapy, so there are several modalities 00:04:45.78\00:04:50.62 that we can do for that. 00:04:50.65\00:04:52.55 Everybody thinks, "Oh, you're gonna do surgery." 00:04:52.59\00:04:54.32 No, we're not gonna do surgery on them, okay. 00:04:54.36\00:04:57.19 That's the last thing, 00:04:57.23\00:04:58.69 the least that we wanna think about. 00:04:58.73\00:05:00.50 Probably eight out of ten people 00:05:00.53\00:05:02.66 live happily ever after with heel pain, 00:05:02.70\00:05:04.93 we can get them better, 00:05:04.97\00:05:06.30 may be about 20% that we would have to take on 00:05:06.33\00:05:08.74 where heel pain, if it's removing a heel spur, 00:05:08.77\00:05:11.84 if it's lengthening the plantar fascia, 00:05:11.87\00:05:14.44 taking care of that problem. 00:05:14.48\00:05:15.98 So we try everything, 00:05:16.01\00:05:17.35 a non-surgical conservative approach first. 00:05:17.38\00:05:20.62 Your feet are the foundation of your body, 00:05:20.65\00:05:24.15 they house 25% of your bones, your ligaments and vessels 00:05:24.19\00:05:30.89 and it's so important. 00:05:30.93\00:05:33.13 Again a building is built on a firm foundation, 00:05:33.16\00:05:36.87 it's very, very important that you take care of that 00:05:36.90\00:05:38.73 and prevention is the main thing. 00:05:38.77\00:05:43.07 And we like to thank Dr. Haag for sharing her expertise 00:05:43.10\00:05:45.67 and her time with us. 00:05:45.71\00:05:47.04 And, Dr. Marcum, it sounds like 00:05:47.08\00:05:48.41 there's a lot of different things 00:05:48.44\00:05:49.78 that can cause pain in the feet. 00:05:49.81\00:05:51.15 Yeah, feet pain is foot pain, feet pain, 00:05:51.18\00:05:53.58 whatever you wanna call, it is not a fun thing to have. 00:05:53.62\00:05:56.32 Yeah. 00:05:56.35\00:05:57.69 You know, I've thought a lot about this and, you know, 00:05:57.72\00:05:59.52 I have problem with my feet quite a bit and you say, 00:05:59.55\00:06:02.39 "What causes foot pain? 00:06:02.42\00:06:04.76 you know, what causes... 00:06:04.79\00:06:06.19 When we know about the trauma, like, 00:06:06.23\00:06:07.86 you sprain your ankle or you stub your toe, 00:06:07.90\00:06:10.23 of course, that's gonna cause foot pain 00:06:10.27\00:06:12.20 but most of the foot pain we now 00:06:12.23\00:06:14.30 are coming from chronic problems. 00:06:14.34\00:06:17.37 And, you know, we're just weren't made to wear 00:06:17.41\00:06:19.21 all these tight shoes, and, you know, 00:06:19.24\00:06:21.64 high heel shoes and constrain our feet 00:06:21.68\00:06:25.85 and also, we're not exercising our feet 00:06:25.88\00:06:28.35 like we used to, to keep the muscle strong 00:06:28.38\00:06:31.25 and to keep the support system lasting 00:06:31.29\00:06:33.39 for a long period of time. 00:06:33.42\00:06:34.76 Right. 00:06:34.79\00:06:36.12 So you add that up 00:06:36.16\00:06:37.49 and then soon or later, guess what? 00:06:37.53\00:06:38.86 The part start to wear off, 00:06:38.89\00:06:40.23 how many bones you said, 52 bones? 00:06:40.26\00:06:41.83 Yeah, 52 bones in your feet. 00:06:41.86\00:06:43.70 Yeah, that's a lot of bones and ligaments 00:06:43.73\00:06:45.57 and tendons and muscles and there is a lot, 00:06:45.60\00:06:47.54 a lot of people don't realize 00:06:47.57\00:06:48.90 but, you know, also the blood supply 00:06:48.94\00:06:50.71 to the foot is very important too. 00:06:50.74\00:06:52.47 Right. 00:06:52.51\00:06:53.84 And just have the vascular disease, 00:06:53.88\00:06:55.64 you know, we've talked about 00:06:55.68\00:06:57.01 how prominent vascular diseases in the heart, 00:06:57.05\00:06:59.71 we've talked about in the brain, 00:06:59.75\00:07:01.08 you get dementia, well, you can vascular disease 00:07:01.12\00:07:02.98 in the blood vessels of the feet too 00:07:03.02\00:07:05.12 and if the bones and the ligaments 00:07:05.15\00:07:06.65 and the muscles don't get enough blood, 00:07:06.69\00:07:08.52 guess what happens? 00:07:08.56\00:07:09.89 Pain develops. Right, yeah. 00:07:09.92\00:07:12.09 I mean, another thing is when we sit a lot, you know, 00:07:12.13\00:07:14.63 that puts more, you know, 00:07:14.66\00:07:16.10 sort of an unnatural thing for our feet, 00:07:16.13\00:07:18.23 so that's another thing that causes foot pain 00:07:18.27\00:07:20.34 and you add it all up and soon or later, 00:07:20.37\00:07:22.04 our feet start to hurt. 00:07:22.07\00:07:24.01 Right. Do you have foot pain? 00:07:24.04\00:07:25.77 I sure have. Yeah, it's not fun, is it? 00:07:25.81\00:07:28.11 No, I had an ingrown toe nail 00:07:28.14\00:07:29.91 and I had to go and actually see Dr. Haag 00:07:29.94\00:07:32.31 and she took care of it, 00:07:32.35\00:07:33.68 and I definitely waited longer than I should have 00:07:33.72\00:07:36.05 before I went and saw her. 00:07:36.08\00:07:37.42 Yeah, that means many people out there that have foot pain 00:07:37.45\00:07:39.65 and I'm gonna talk about some of the common causes, 00:07:39.69\00:07:41.92 I mean, you know, you hear these big names, 00:07:41.96\00:07:43.86 so we're gonna throw out some big names. 00:07:43.89\00:07:45.46 Okay. 00:07:45.49\00:07:46.83 Tell you what it is, 00:07:46.86\00:07:48.20 most of the time when you have... 00:07:48.23\00:07:49.56 Some of these conditions, you do have to see a podiatrist 00:07:49.60\00:07:51.67 which is a foot doctor specialist. 00:07:51.70\00:07:54.57 One of things that we see, like she mentioned is, 00:07:54.60\00:07:56.67 plantar fasciitis. 00:07:56.71\00:07:58.14 Right. 00:07:58.17\00:07:59.51 And that's where the fascia gets inflamed, 00:07:59.54\00:08:01.94 it hurts a lot and that could be as simple as, 00:08:01.98\00:08:04.55 because we don't have support on our ankles. 00:08:04.58\00:08:09.38 You know, it doesn't have a good blood supply, 00:08:09.42\00:08:11.15 so it takes a long time to heal that type of inflammation. 00:08:11.19\00:08:15.36 So people that have that, they have to, 00:08:15.39\00:08:17.96 sometimes they put their feet in boots to rest it, 00:08:17.99\00:08:21.63 sometimes they had to take short courses 00:08:21.66\00:08:23.90 of anti-inflammatory medications, 00:08:23.93\00:08:26.77 sometimes they had to get support on their ankle 00:08:26.80\00:08:30.77 and you saw the foot supports. 00:08:30.81\00:08:33.21 Now there are doctors now, Nick, 00:08:33.24\00:08:34.98 they can actually make mouldings of your feet, okay. 00:08:35.01\00:08:37.98 We have the technology now that's exactly 00:08:38.01\00:08:40.32 what type of foot support you need 00:08:40.35\00:08:42.42 to sort of avoid those problems. 00:08:42.45\00:08:44.05 Yeah, custom orthotic. 00:08:44.09\00:08:45.42 Custom orthotic, that's a very common cause of foot pain, 00:08:45.45\00:08:48.29 that's the fashion gets in. 00:08:48.32\00:08:49.72 Now another thing that happens is the tendons, 00:08:49.76\00:08:52.29 the tendons can get inflamed. 00:08:52.33\00:08:54.00 Okay. 00:08:54.03\00:08:55.36 And we call that tendonitis, you know, 00:08:55.40\00:08:56.73 just moving the tendon a lot, sometimes we see that 00:08:56.77\00:08:59.97 people that work at the gym a lot, 00:09:00.00\00:09:01.74 they might be going and doing lot of treadmill work, 00:09:01.77\00:09:04.21 repetitive work, 00:09:04.24\00:09:05.57 where the tendons have to work over and over, 00:09:05.61\00:09:07.48 a lot of runners, sometimes get tendonitis 00:09:07.51\00:09:10.21 where the tendons get inflamed, 00:09:10.25\00:09:12.38 that's a very painful condition, 00:09:12.41\00:09:14.05 of course, the best treatment is to rest, 00:09:14.08\00:09:17.15 if rest doesn't work 00:09:17.19\00:09:18.52 then they can take short courses 00:09:18.55\00:09:19.89 of anti-inflammatory, 00:09:19.92\00:09:21.26 if it's getting in the severe cases, 00:09:21.29\00:09:22.62 they inject steroids into that area, 00:09:22.66\00:09:24.79 'cause steroids help cut down the inflammation. 00:09:24.83\00:09:27.73 Now, of course, 00:09:27.76\00:09:29.10 one of the largest causes of foot problems 00:09:29.13\00:09:31.40 is just trauma to the foot. 00:09:31.43\00:09:33.10 You know, the foot just... 00:09:33.13\00:09:34.47 Sprained ankles and that kind of thing? 00:09:34.50\00:09:35.84 Yeah, it gets hurt, you know, you're running, it gets hurt, 00:09:35.87\00:09:38.07 those type of problems. 00:09:38.11\00:09:39.44 Right. 00:09:39.47\00:09:40.81 If you think about the structures in the foot, 00:09:40.84\00:09:42.18 we've talked about the blood vessels, 00:09:42.21\00:09:43.55 there's also nerves in the feet and if nerves get damaged 00:09:43.58\00:09:47.75 on repetitive use, 00:09:47.78\00:09:49.12 sometimes things in the foot get squished, 00:09:49.15\00:09:52.62 you've heard of things like neuromas, okay, 00:09:52.65\00:09:55.42 those are nerve tissues, diabetes is a condition 00:09:55.46\00:09:59.16 that damages the nerves of the feet as well. 00:09:59.19\00:10:01.86 Usually, they have a lot of burning on the sides 00:10:01.90\00:10:04.00 of the foot when you have, you know, nerve problems. 00:10:04.03\00:10:06.94 Okay. 00:10:06.97\00:10:08.30 A lot of people have cheilosis on their feet, 00:10:08.34\00:10:10.01 because if things not rubbing well, 00:10:10.04\00:10:12.04 you get a foot that's too dry, that's a problem, then, 00:10:12.07\00:10:15.34 you know, socks rub on it, 00:10:15.38\00:10:17.95 here's a medical delusion gout, 00:10:17.98\00:10:19.65 that's typically when at the base of the big toe, 00:10:19.68\00:10:22.22 you get uric acid crystals, 00:10:22.25\00:10:24.55 the uric acid crystals deposit there, 00:10:24.59\00:10:27.06 create a lot of pain as well 00:10:27.09\00:10:28.69 and one of the most common places 00:10:28.72\00:10:30.43 is the base of the big toe, we call that gout. 00:10:30.46\00:10:33.63 Have you heard of bunions? Yeah. 00:10:33.66\00:10:35.43 Okay, bunions are on the side, you get these grows 00:10:35.46\00:10:38.17 on the side of your foot 00:10:38.20\00:10:39.63 because the toes are in that position, 00:10:39.67\00:10:42.87 this area 'cause constant rubbing, 00:10:42.90\00:10:46.07 it can get big and it can hurt quite a bit and of course, 00:10:46.11\00:10:49.21 that sometimes you have a bunionectomy, 00:10:49.24\00:10:51.31 that's where they surgically cut that bunion off. 00:10:51.35\00:10:53.75 You've talked about the ingrown toe nail, 00:10:53.78\00:10:55.78 that's a pretty common thing for people to have 00:10:55.82\00:10:57.75 and that can happen 00:10:57.79\00:10:59.12 just because people don't cut their toes very well. 00:10:59.15\00:11:01.56 Yeah, the toe nails, right.
