Absolutely! 00:01:51.24\00:01:53.24 So we've got some questions from the website 00:01:53.27\00:01:54.91 that people have written in and the first one 00:01:54.94\00:01:57.35 is from a viewer who asks, "Is it common for chest pain 00:01:57.38\00:02:01.32 to last for more than a month?" 00:02:01.35\00:02:03.59 That's an interesting question, 00:02:03.62\00:02:05.92 and the way we're going to have to approach that... 00:02:05.95\00:02:08.22 I can't get all the details, but if a person 00:02:08.26\00:02:11.19 has chest pain that won't go away for a month, 00:02:11.23\00:02:13.83 that's usually not the heart because some bad happens. 00:02:13.86\00:02:17.73 It's usually not something dangerous - usually 00:02:17.77\00:02:21.30 because it lasted a month and they're still alive. Right 00:02:21.34\00:02:23.71 But, for that person, it's not common, 00:02:23.74\00:02:26.27 so we talked earlier about it, anytime that pain 00:02:26.31\00:02:28.91 won't go away - we need to find out what's causing it. 00:02:28.94\00:02:32.38 So it could be something as complicated as reflux; 00:02:32.41\00:02:37.35 it might be related to muscles. 00:02:37.39\00:02:39.15 One of the things that also can cause chronic chest pain, 00:02:39.19\00:02:41.89 believe it or not, Nick, is stress. Really? 00:02:41.92\00:02:44.73 Stress - now we want to rule everything else out first. 00:02:44.76\00:02:47.60 We want to make sure it's not the heart, 00:02:47.63\00:02:48.96 it's not the 5 things we talked about. Right 00:02:49.00\00:02:51.50 But sometimes what happens in people that are under 00:02:51.53\00:02:53.70 chronic stress, they make adrenalin 00:02:53.74\00:02:55.80 that makes their muscles tight. 00:02:55.84\00:02:57.77 They also make inflammatory chemicals in the body 00:02:57.81\00:03:01.31 which makes inflammation of the muscles. 00:03:01.34\00:03:03.65 And, of course, when you feel it in your chest, 00:03:03.68\00:03:05.81 you get very, very concerned. Right 00:03:05.85\00:03:07.58 You know if it's a headache, 00:03:07.62\00:03:08.95 sometimes they're not as concerned. 00:03:08.98\00:03:10.32 If it's in the back of the leg, they're not as concerned. 00:03:10.35\00:03:12.29 But if it's in the chest, they need to be concerned. 00:03:12.32\00:03:15.22 So, hopefully, this person that has had chest pain 00:03:15.26\00:03:17.83 for more than a month, has already been 00:03:17.86\00:03:19.76 to see the doctor to make sure it's not 00:03:19.79\00:03:21.60 the first big 5, and then he can go down 00:03:21.63\00:03:24.33 the list of all sorts of different things 00:03:24.37\00:03:26.53 that it could be... from shingles to stress, 00:03:26.57\00:03:29.64 to muscles, to GI tract, to chronic other conditions. 00:03:29.67\00:03:35.01 So, it's not common for it to last that long, 00:03:35.04\00:03:37.61 but still this person needs to seek help and find the 00:03:37.65\00:03:40.85 diagnosis so it doesn't continue... 00:03:40.88\00:03:42.68 Because remember, we talked about what happens 00:03:42.72\00:03:45.02 with long-term stress, Nick? 00:03:45.05\00:03:46.39 Let me see if you remember the last program. 00:03:46.42\00:03:47.99 What happens when you are under long-term pain, 00:03:48.02\00:03:50.86 what does it do to the body? 00:03:50.89\00:03:52.23 It turns on reactions... STRESS, right? Yes 00:03:52.26\00:03:55.76 Chemical reactions and that causes deterioration. 00:03:55.80\00:03:59.23 And it could potentiate any bad system. Okay 00:03:59.27\00:04:02.90 So chronic pain makes whatever is causing it - worse... Right 00:04:02.94\00:04:05.91 including your brain. 00:04:05.94\00:04:07.38 So you're saying that there is a real... 00:04:07.41\00:04:10.31 when we have stress chronically, when we have a job 00:04:10.35\00:04:12.91 that we're stressed out every day, 00:04:12.95\00:04:15.48 that kind of thing causes a real chemical reaction in the body. 00:04:15.52\00:04:17.82 Yes, and it could cause chronic chest pain. 00:04:17.99\00:04:19.79 Now I'm not saying it is for this person, 00:04:19.82\00:04:21.59 but it's a possibility. 00:04:21.62\00:04:22.96 That would have to be looked into after everything else 00:04:22.99\00:04:25.69 was ruled out. Right, right. 00:04:25.73\00:04:27.40 Interesting, so would a vacation help? 00:04:27.43\00:04:30.73 Yes, the opposite of stress is rest. Okay 00:04:30.77\00:04:34.57 And remember that text in Matthew, 00:04:34.60\00:04:37.51 "Come unto Me, all you that labor and I will give you rest." 00:04:37.54\00:04:40.38 So if we come to God in worship, He has promised to give us rest, 00:04:40.41\00:04:44.61 and the three types of rest is spiritual rest, 00:04:44.65\00:04:47.18 the mental rest - how many people get mental rest? 00:04:47.22\00:04:50.19 I read recently that the average person 00:04:50.22\00:04:52.15 spends 12 hours a day in the media. Wow 00:04:52.19\00:04:54.89 Twelve hours! That's a lot... 00:04:54.92\00:04:56.26 That's a lot of stressors. Yes 00:04:56.29\00:04:57.73 Depending upon what they're listening to and then 00:04:57.76\00:05:00.46 we talked about that and then you need the physical rest. 00:05:00.50\00:05:03.43 We were originally designed to rest a day a week, 00:05:03.47\00:05:05.97 to rest at night; people are staying up 00:05:06.00\00:05:08.97 all night; they are awake; they don't get that 00:05:09.00\00:05:11.74 circadian rhythm that turns off the stress chemistry. 00:05:11.77\00:05:14.51 If stress chemistry goes on long enough, 00:05:14.54\00:05:16.78 it could cause pain including chest pains. Right, right 00:05:16.81\00:05:19.78 Let's go on to another question here... 00:05:19.81\00:05:21.45 Someone writes in... "How is a CT scan used to 00:05:21.48\00:05:23.92 evaluate chest pain?" 00:05:23.95\00:05:25.32 A CT scan is a very useful test to evaluate chest pain 00:05:25.35\00:05:29.66 that happens right away. 00:05:29.69\00:05:32.16 It's a CAT scan - people have heard that term, 00:05:32.19\00:05:34.30 but it's very good to look for structural things, 00:05:34.33\00:05:37.23 and also we can look for blood clots. 00:05:37.27\00:05:39.43 On the big 5, we use CAT scans to evaluate for 00:05:39.47\00:05:42.80 pulmonary embolism - this could kill a person. 00:05:42.84\00:05:45.24 We use CAT scans to look at aortic dissections, 00:05:45.27\00:05:48.18 that's when arteries are splitting open. Right 00:05:48.21\00:05:50.68 We use that, but it can also look at other causes 00:05:50.71\00:05:53.42 of chest pain like pneumonia, inflammation in the lungs, 00:05:53.45\00:05:57.85 and other different things that can happen. 00:05:57.89\00:05:59.49 It can also pick up if there's abnormal masses in the lungs 00:05:59.52\00:06:03.39 or in the chest including the things that would cause 00:06:03.43\00:06:06.03 cancer - it can pick up cancer. 00:06:06.06\00:06:08.53 It can also look at the lining of the heart called the 00:06:08.56\00:06:11.43 "pericardium," and sometimes that can get infected 00:06:11.47\00:06:14.14 and also cause us chest pain as well. 00:06:14.17\00:06:16.47 So a CAT scan is a very useful 00:06:16.50\00:06:18.77 structure to look at the insides of the body. 00:06:18.81\00:06:22.54 Now it doesn't look inside the organs, 00:06:22.58\00:06:24.31 but it gives us a picture of the outside of the organs 00:06:24.35\00:06:26.78 which can give us clues as to 00:06:26.82\00:06:28.75 what might be causing the chest pain. Right, right 00:06:28.78\00:06:30.99 Now, I haven't ever had a CAT scan, 00:06:31.02\00:06:33.76 is that the one where you lay on the table and they put 00:06:33.79\00:06:35.59 you inside and you're real claustrophobic in there? 00:06:35.62\00:06:37.69 Yeah, you can be claustrophobic, hopefully you aren't, 00:06:37.73\00:06:41.10 and there is small risk of radiation, 00:06:41.13\00:06:44.30 but hopefully, you're not getting that done 00:06:44.33\00:06:47.30 unless it's something that needs to be done. 00:06:47.34\00:06:49.90 In fact, some of the images... you know that image we have 00:06:49.94\00:06:53.14 behind your head is an image from a CAT scan of the head. 00:06:53.17\00:06:57.61 And so it gives us great images 00:06:57.65\00:06:59.41 of the different parts of the body. 00:06:59.45\00:07:01.12 It can also look at the chest. Right, right. 00:07:01.15\00:07:03.55 All right, how about chest pain 00:07:03.59\00:07:06.29 that comes from the stomach can that happen? 00:07:06.32\00:07:08.66 Well, yes it is, and I wanted to bring this, you know, 00:07:08.69\00:07:11.56 this is a great question because you can have acid in the 00:07:11.59\00:07:14.40 stomach, you can have reflux. 00:07:14.43\00:07:16.87 Sometimes that acid crawls upstream into the esophagus. 00:07:16.90\00:07:21.50 If it comes on right away, sometimes it can feel like 00:07:21.54\00:07:24.51 a heart attack and you can't know the difference. 00:07:24.54\00:07:26.17 So it's always better to be safe than sorry, 00:07:26.21\00:07:29.94 but the typical pain that comes from the stomach 00:07:29.98\00:07:32.15 can come when a person lays down because the valve might 00:07:32.18\00:07:35.25 no close well, so acid comes up into the esophagus. 00:07:35.28\00:07:38.95 It might come after we eat because the stomach might 00:07:38.99\00:07:42.22 get worse after we eat and usually after food. 00:07:42.26\00:07:45.06 It might have a funny taste, funny in our mouth... 00:07:45.09\00:07:48.53 It might taste acidy in our mouth. 00:07:48.56\00:07:51.13 But when the pain comes from the stomach, 00:07:51.17\00:07:53.34 which is a very common cause of pain 00:07:53.37\00:07:55.17 from the acid bubbling up, we want to make 00:07:55.20\00:07:57.17 sure it's not a heart pain. Right 00:07:57.21\00:07:58.91 So if a pain is happening when you do something, 00:07:58.94\00:08:01.31 that really doesn't go with your stomach, 00:08:01.34\00:08:03.08 that goes with your heart. Right 00:08:03.11\00:08:04.61 Pain that comes on after you eat - sometimes that can 00:08:04.65\00:08:06.82 be acid, sometimes it could be your heart. 00:08:06.85\00:08:09.12 In fact, when people come to the emergency room, 00:08:09.15\00:08:12.02 and we rule out the big 5, 00:08:12.05\00:08:13.86 the most common causes that it's not, 00:08:13.89\00:08:16.42 again, once we rule out the significant things, 00:08:16.46\00:08:18.66 it usually ends up being something related to the 00:08:18.69\00:08:21.00 stomach or something related to stress, 00:08:21.03\00:08:23.60 but we can't miss the big ones, 00:08:23.63\00:08:25.30 so you gotta find the big ones first. Okay 00:08:25.33\00:08:27.30 How about shingles? Somebody else write in... 00:08:27.34\00:08:30.41 "Can shingles cause pain in the chest - what are shingles? 00:08:30.44\00:08:33.17 Yeah, shingles is not a fun thing to have. 00:08:33.21\00:08:36.34 That's an infection along a nerve root, 00:08:36.41\00:08:39.51 and everyone has heard about the chickenpox. 00:08:39.55\00:08:42.78 Shingles is sort of like the chickenpox and it gets 00:08:42.82\00:08:44.69 along a nerve root, it can cause extreme pain. 00:08:44.72\00:08:48.02 In fact, I saw a 70-year-old not too long ago 00:08:48.06\00:08:51.96 that just had pain all the time. 00:08:51.99\00:08:53.73 You know, they went to the emergency room, 00:08:53.76\00:08:55.46 and we did the tests, we did the blood work, the EKG. 00:08:55.50\00:08:58.83 We did the CAT scan. 00:08:58.87\00:09:00.20 We couldn't find out what the pain was. 00:09:00.24\00:09:01.80 The pain would not go away. 00:09:01.84\00:09:03.30 So a pain that won't go away is either a 00:09:03.34\00:09:05.31 heart attack or one of these bad things or something else. 00:09:05.34\00:09:08.58 So we said, "Wow, we don't really know what it is." 00:09:08.61\00:09:11.31 We don't know and the pain is just excruciating. 00:09:11.35\00:09:13.78 Well, in a day or two, guess what happened? 00:09:13.82\00:09:15.98 They had a characteristic rash, a shingles rash 00:09:16.02\00:09:19.85 that came right where that nerve was going around their chest, 00:09:19.89\00:09:23.59 and we knew that their pain was from the shingles infection, 00:09:23.63\00:09:27.66 and nowadays, if people are exposed to shingles a lot, 00:09:27.70\00:09:32.37 there are even vaccines they can give. 00:09:32.40\00:09:34.44 Some people that have a weakened immune system 00:09:34.47\00:09:37.91 might consider that because stress triggers shingles 00:09:37.94\00:09:41.81 because our immune system can't keep that infection down, 00:09:41.84\00:09:45.21 and it becomes activated, it is very painful and it can 00:09:45.25\00:09:48.08 occur in other places. 00:09:48.12\00:09:49.45 It can occur in the face; it can occur on the body; 00:09:49.48\00:09:52.22 and it can occur in any dermatome where a nerve lives, 00:09:52.25\00:09:55.69 and it is very painful. 00:09:55.72\00:09:57.49 So shingles is a cause of pain. 00:09:57.53\00:10:00.33 It's usually not chronic pain, it's acute pain, 00:10:00.36\00:10:03.23 pain that won't go away, and there are some things 00:10:03.26\00:10:05.27 that we can do to help treat that pain 00:10:05.30\00:10:07.00 until the virus runs its course. 00:10:07.04\00:10:09.17 So the pain from shingles, it could be in the chest 00:10:09.20\00:10:11.27 or it probably could be in other parts of the body as well? 00:10:11.31\00:10:13.24 Yeah, we found out on this one because it was in the chest, 00:10:13.27\00:10:15.71 we evaluated it, we didn't know for sure 00:10:15.74\00:10:17.55 until guess what? The virus came, 00:10:17.58\00:10:20.15 and then we saw the rash along the nerve root, 00:10:20.18\00:10:23.82 and we knew they had shingles. 00:10:23.85\00:10:25.19 So you mentioned earlier that pain is often associated 00:10:25.22\00:10:27.89 with heart attack, but is it possible to have 00:10:27.92\00:10:29.79 heart attack without experiencing pain? 00:10:29.82\00:10:31.69 Yeah, it is, and especially in women. 00:10:31.73\00:10:35.70 Women don't present quite the same as men. 00:10:35.73\00:10:39.00 They have a different pain mechanism in their bodies. 00:10:39.03\00:10:42.74 Their blood vessels in the heart are a little 00:10:42.77\00:10:44.37 bit different - they're smaller. 00:10:44.41\00:10:45.97 Because of their hormonal system, 00:10:46.01\00:10:47.74 they experience things a little bit different. 00:10:47.78\00:10:49.44 Sometimes women will just have shortness of breath, 00:10:49.51\00:10:51.81 extreme shortness of breath instead of chest pain. 00:10:51.85\00:10:54.58 Sometimes if a person has diabetes 00:10:54.62\00:10:57.25 or damage to their nerves... 00:10:57.29\00:10:58.69 Remember we talked in previous programs when the nerves are 00:10:58.72\00:11:01.09 damaged, some people don't feel pain at all. 00:11:01.12\00:11:05.83 We see that occasionally in diabetics, 00:11:05.86\00:11:07.90 and people that are old. 00:11:07.93\00:11:09.60 They might just present with extreme fatigue, 00:11:09.63\00:11:11.77 shortness of breath, a lack of energy, 00:11:11.80\00:11:14.27 and we say, "Wow, what could that be?" 00:11:14.30\00:11:16.57 And we do some testing and wow, it ended up being the heart. 00:11:16.60\00:11:19.14 Now that's not as common, we don't see that as common, 00:11:19.17\00:11:22.14 but it is possible to have a heart attack without pain. 00:11:22.18\00:11:25.01 So when someone comes in with symptoms that we have 00:11:25.05\00:11:27.38 no explanation for, since heart disease is so 00:11:27.42\00:11:30.49 common in our world... 00:11:30.52\00:11:31.95 In fact, heart disease was one of the highest killers 00:11:31.99\00:11:35.42 in the United States last year and as we get westernized, 00:11:35.46\00:11:38.79 and have more stress, more fatty foods, guess what? 00:11:38.83\00:11:42.06 We are going to have more problems with 00:11:42.10\00:11:44.23 cardiovascular disease. Yeah 00:11:44.27\00:11:46.03 We've got just another question here, somebody writes in... 00:11:46.07\00:11:48.87 "Why is chest pain evaluated so quickly in the emergency room?" 00:11:48.90\00:11:53.58 I think I've got a little bit of an idea because you 00:11:53.61\00:11:55.51 mentioned the big 5 earlier: heart attack, 00:11:55.54\00:11:57.71 pulmonary embolism, pneumothorax, 00:11:57.75\00:12:01.02 and a dissection, and what was the other one? 00:12:01.05\00:12:03.05 Unstable angina, that's where it's sort of getting 00:12:03.08\00:12:06.49 to a heart attack, but it's not quite there. 00:12:06.52\00:12:08.62 Well, a person that's having pain right away... 00:12:08.66\00:12:10.83 if they're having a heart attack, 00:12:10.86\00:12:13.09 you have to get treatment right away because when 00:12:13.13\00:12:15.13 the heart doesn't get enough blood, 00:12:15.16\00:12:17.63 it becomes electrically unstable. Right 00:12:17.67\00:12:20.64 It doesn't get enough blood and the heart, you know, 00:12:20.67\00:12:23.10 if it doesn't get enough blood, electricity has problems, 00:12:23.14\00:12:26.74 and it starts to fibrillate, and I don't know if you've 00:12:26.78\00:12:29.34 seen TV where they get the defibrillators out 00:12:29.38\00:12:31.68 and shock the heart. Yeah 00:12:31.71\00:12:33.05 Even a small blockage can cause that to happen. 00:12:33.08\00:12:36.42 The heart gets electrically unstable, 00:12:36.45\00:12:38.05 it goes into funny rhythm. 00:12:38.09\00:12:39.42 They have ventricular tachycardia, 00:12:39.45\00:12:41.92 ventricular fibrillation and sudden, sudden death. 00:12:41.96\00:12:45.09 So that's just one of the reasons it got to be 00:12:45.13\00:12:47.56 jumped on quickly and I can't tell you how many people 00:12:47.60\00:12:49.66 that we've had to shock in the emergency room 00:12:49.70\00:12:51.50 that were having a heart attack.
