Welcome back to the program. 00:00:01.36\00:00:02.70 I'm Nick Evenson here with Dr. James Marcum 00:00:02.73\00:00:04.07 and before the break, we were talking about nerves... 00:00:04.10\00:00:06.07 and Dr. Marcum, explain a little bit more to me about 00:00:06.10\00:00:08.57 how the nerves affect our pain signals. 00:00:08.60\00:00:10.97 Well this is going to be a very important point for 00:00:11.01\00:00:13.98 each of our viewers to understand, as we talk about 00:00:14.01\00:00:17.31 pain in this series - remember, when a person has pain, 00:00:17.55\00:00:21.62 there's a reason why and we want to understand why. 00:00:21.65\00:00:25.85 Pain creates stress on the system and just that stress 00:00:25.89\00:00:30.66 makes pain more dangerous because it affects the brain. 00:00:30.69\00:00:35.86 So stress affects every cell in the body including the brain. 00:00:35.90\00:00:39.30 How our brain perceives and what it knows about pain, 00:00:39.33\00:00:42.80 you know, it talks about a response... 00:00:44.04\00:00:45.74 For instance, if we know this pain is not a big deal, 00:00:45.77\00:00:48.38 if it's not going to kill us, if we have that knowledge, 00:00:49.21\00:00:51.81 we can handle pain much better. 00:00:51.85\00:00:53.38 The body goes, "No, this is just a little bitty ant bite, 00:00:53.42\00:00:56.79 it's not going to kill you, I can sort of turn off all 00:00:56.82\00:00:59.15 all this effort that I'm doing the pain, 00:00:59.19\00:01:01.59 and then it turns off the stress. 00:01:01.62\00:01:03.49 So that's a very important thing. 00:01:03.53\00:01:04.93 Your brain is involved in pain. 00:01:04.96\00:01:07.03 So the more you know about pain, 00:01:07.06\00:01:08.63 the better you're going to deal with pain. 00:01:08.66\00:01:10.77 Then you'll want to think about, "Well how does my provider 00:01:10.80\00:01:13.60 deal with pain?" 00:01:13.64\00:01:14.97 And the first thing he's going to do is, 00:01:15.00\00:01:16.34 hopefully, he is going to say, "Well let's find 00:01:16.37\00:01:18.01 the cause of pain - so we're not treating a symptom." 00:01:18.04\00:01:20.94 We want to treat a cause and not a symptom. 00:01:20.98\00:01:24.38 And, unfortunately, modern medicine treats 00:01:24.41\00:01:27.25 symptoms and not... it's a money-maker. Yeah 00:01:27.28\00:01:29.92 And we see all the people that are taking pills, 00:01:29.95\00:01:32.65 4 billion written in the United States, 00:01:32.69\00:01:34.72 more than all the world combined. Wow! 00:01:34.76\00:01:37.03 That's big time! Yeah 00:01:37.06\00:01:38.39 And narcotics, the death rate keeps going up 00:01:38.43\00:01:40.86 because they're not getting at causes and pain 00:01:40.90\00:01:43.03 medicines, it's all about risks and benefits. 00:01:43.06\00:01:45.67 And sometimes, there is a benefit to taking 00:01:45.70\00:01:47.64 medicines for the short-term. Right 00:01:47.67\00:01:49.64 But if we can get off it, that's the key. 00:01:49.67\00:01:51.64 And then the next thing is find the cause and then 00:01:52.24\00:01:54.24 understand whether the nerve itself is damaged. 00:01:54.28\00:01:57.45 We call that a neuropathy. 00:01:57.48\00:01:59.41 If the nerve itself is damaged, 00:01:59.45\00:02:01.58 it's going to experience pain differently. 00:02:01.62\00:02:04.39 You know, for instance... you don't get enough oxygen; 00:02:04.42\00:02:06.86 you don't get enough water; you don't get a right nutrient; 00:02:06.89\00:02:09.49 you don't get enough rest. 00:02:09.52\00:02:10.93 That nerve function is going to be compromised. 00:02:10.96\00:02:13.96 So the way it perceives pain is going to be different as well. 00:02:14.00\00:02:17.83 So those are all some things that I really wanted 00:02:17.87\00:02:20.57 our audience to understand. 00:02:20.60\00:02:22.30 This will help them as we move forward 00:02:22.34\00:02:24.51 especially if they have pain or if they want to help 00:02:24.54\00:02:26.88 their neighbors who have pains. Right 00:02:26.91\00:02:28.48 Now, we have a lot of questions that have come into our website, 00:02:28.84\00:02:31.41 haven't we? Yeah, we sure do! 00:02:31.45\00:02:32.78 The first one that someone writes in is: 00:02:32.81\00:02:35.45 "What is the definition of chronic pain, 00:02:35.48\00:02:38.15 and how to differentiate acute pain from chronic pain?" 00:02:38.19\00:02:42.12 Well, acute pain is pain that goes away, okay? 00:02:42.16\00:02:47.03 You find the cause, it goes away. 00:02:47.06\00:02:49.26 You sprained your ankle, you rest it, 00:02:49.30\00:02:50.97 eventually it goes away. 00:02:51.00\00:02:52.33 That's more of an acute pain. 00:02:52.37\00:02:53.94 We call that a nociceptive pain because the nerve is fine, 00:02:53.97\00:02:57.24 if you can get at the cause which is usually 00:02:57.27\00:02:59.31 resting it and let it heal on it's own. 00:02:59.34\00:03:01.24 That pain usually gets better! 00:03:01.28\00:03:03.45 Chronic pain is a pain, no matter where it's from, 00:03:03.48\00:03:06.51 that goes on for a long period of time 00:03:06.55\00:03:09.72 or it's a pain that we can't identify the etiology. 00:03:09.75\00:03:13.66 Now when a person has pain, 00:03:13.69\00:03:15.66 if a doctor can't find out what it is, 00:03:15.69\00:03:18.16 that doesn't mean there's not a cause. Right 00:03:18.19\00:03:21.03 He just hasn't found out what it is. 00:03:21.06\00:03:23.13 Many times the doctor can't find out what the cause is 00:03:23.16\00:03:26.63 because pain is very complicated. 00:03:26.67\00:03:29.60 Pain not only has to do with what causes it, 00:03:29.64\00:03:31.97 but how your brain perceives pain; 00:03:32.01\00:03:33.98 how your emotional system is working; 00:03:34.01\00:03:36.71 how your stress system is working. 00:03:36.75\00:03:39.58 You can have pain just from chronic stress 00:03:39.61\00:03:42.18 and emotional problems. 00:03:42.22\00:03:44.35 You know, so pain is a very complicated disease process. 00:03:44.39\00:03:48.79 Right, and a lot of different causes 00:03:48.82\00:03:50.16 that could be playing a role. 00:03:50.19\00:03:51.59 Yeah, and the most common causes of pain are... 00:03:51.63\00:03:54.83 There's chronic back pain that's probably the #2. 00:03:54.86\00:03:59.50 Chronic pain where we don't 00:03:59.53\00:04:00.94 know the cause - that's probably #1. 00:04:00.97\00:04:03.30 Arthritis is high up on the list. 00:04:03.34\00:04:06.81 Headaches that's a very common cause of pain. 00:04:06.84\00:04:09.98 Nerve pain - you know people have chronic nerve pain, 00:04:10.01\00:04:13.72 these neuropathies that occur. 00:04:13.75\00:04:16.79 And those are probably the most common causes of pain. 00:04:16.82\00:04:19.39 So chronic pain, pain that goes on for a long 00:04:19.42\00:04:22.69 period of time where we don't know what the cause is, 00:04:22.72\00:04:25.43 those are the most common causes. 00:04:25.46\00:04:27.30 And if they want to know, they can go to our website: 00:04:27.33\00:04:29.33 heartwiseministries.org 00:04:29.36\00:04:31.63 we'd be happy to answer their questions, 00:04:31.67\00:04:34.40 and also, we have a prayer page there, Nick, 00:04:34.44\00:04:37.07 where we actually pray for people 00:04:37.11\00:04:38.94 because we believe in the power of prayer. 00:04:38.97\00:04:40.88 God can do things miraculously. 00:04:40.91\00:04:42.84 Sometimes He helps us through prayer right away. 00:04:42.88\00:04:47.32 Sometimes He teaches us how to take care 00:04:47.35\00:04:49.65 of our body better, so there are many ways that 00:04:49.68\00:04:51.85 God can help in this healing process. 00:04:51.89\00:04:54.92 You know, on the topic of chronic pain 00:04:54.96\00:04:56.32 we have another person who wrote in and asks: 00:04:56.36\00:04:58.33 "What is the role of a chronic pain specialist?" 00:04:58.36\00:05:01.40 Well, we saw Dennis Ford and he is a chronic pain specialist, 00:05:01.43\00:05:07.14 and you want to pick someone that's good like Dr. Ford. 00:05:07.17\00:05:11.24 You don't want to pick someone that will just 00:05:11.27\00:05:13.98 give you a medication and how do you know if someone is good? 00:05:14.01\00:05:17.35 Well, they are usually board certified in pain medicine. 00:05:17.38\00:05:20.58 That's usually all they do. 00:05:20.62\00:05:22.48 A lot of times, they have specialties like Dr. Ford 00:05:22.52\00:05:25.89 had a specialty in anesthesiology which is 00:05:25.92\00:05:28.89 so he knows how to inject the pain medicines chronically, 00:05:28.92\00:05:31.96 but I think a couple of good things when you interview 00:05:31.99\00:05:34.50 a pain specialist... 00:05:34.53\00:05:35.86 You want a pain specialist that will get at the cause, 00:05:35.90\00:05:39.50 to teach you about pain and the complexities 00:05:39.53\00:05:42.80 like we've talked about. 00:05:42.84\00:05:44.17 How the mental status affects the pain. 00:05:44.21\00:05:46.68 Also, I think it's very important that you want 00:05:46.71\00:05:48.41 someone that will get at the cause of the pain, 00:05:48.44\00:05:50.41 and try to minimize the risks of the treatment. 00:05:50.45\00:05:53.82 And there are many different treatments for chronic pain, 00:05:53.85\00:05:56.65 and we're going to get into those as the series goes, 00:05:56.69\00:05:59.42 but we want to minimize the dangers and sit there and 00:05:59.45\00:06:02.29 explain, you know, and it's not a long-term thing. 00:06:02.32\00:06:04.73 We just want to use this until we can get at the cause, 00:06:04.76\00:06:07.53 and I think cognitive therapy is very important 00:06:07.56\00:06:10.30 in the treatment of chronic pain. 00:06:10.33\00:06:11.77 What is cognitive therapy? 00:06:11.80\00:06:13.60 Well that's teaching a person's brain 00:06:13.64\00:06:15.77 how to deal with chronic pain because the more the 00:06:15.80\00:06:18.87 brain knows, the less stress and inflammation 00:06:18.91\00:06:21.48 it turns on, the less it makes the pain - it deactivates 00:06:21.51\00:06:25.61 some of the chronic pain stimuli. 00:06:25.65\00:06:27.18 So to summarize that, it sounds like cognitive therapy 00:06:27.22\00:06:29.78 helps train the brain how to respond appropriately 00:06:29.82\00:06:32.45 to different levels of pain. 00:06:32.49\00:06:33.82 For instance, it might be whenever you have pain, 00:06:33.86\00:06:37.03 take 10 deep breaths because that turns down stress. 00:06:37.06\00:06:41.00 It might be that whenever you start having pain, 00:06:41.03\00:06:43.20 maybe you laugh or pray or turn on some music. 00:06:43.23\00:06:46.20 Do something in the brain that will help it 00:06:46.23\00:06:48.57 turn on endorphins; some of the chemicals... 00:06:48.60\00:06:51.57 Oxytocin - that's another chemical that helps fight 00:06:51.61\00:06:54.41 chronic pain that your brain turns on. 00:06:54.44\00:06:57.31 And if you can start learning your brain to turn on this, 00:06:57.35\00:07:00.58 and handle the pain better, 00:07:00.62\00:07:01.95 it's going to make it less pronounced. Right 00:07:01.98\00:07:04.29 So that's why I say a good pain doctor will do all of those. 00:07:04.32\00:07:07.79 So those are just some general tips for someone. 00:07:07.82\00:07:10.43 So who would go to a pain doctor? 00:07:10.46\00:07:12.03 Well, someone that usually they can't find the cause of pain. 00:07:12.06\00:07:15.23 You know, they need a little expertise, 00:07:15.26\00:07:18.20 and it's a lot of people that go there, 00:07:18.23\00:07:19.93 they're in chronic pain, they can't find the cause 00:07:19.97\00:07:22.27 of pain or they have chronic back pain 00:07:22.30\00:07:25.34 that no one seems to understand how to solve it or arthritis. 00:07:25.37\00:07:29.38 Those are the most common referrals to a pain physician. 00:07:29.41\00:07:33.42 You know, we've got another question here, 00:07:33.45\00:07:35.38 someone write in: "I've taken narcotics for pain, 00:07:35.42\00:07:38.29 is there any possible way to get off of these medications?" 00:07:38.32\00:07:41.32 You know, a lot of people get hooked on these, 00:07:41.36\00:07:43.19 and it's a real problem. 00:07:43.22\00:07:44.56 Yeah and narcotics, you know, there's a place for narcotics, 00:07:44.59\00:07:48.83 and I've taken care of patients with cancer pain, 00:07:48.86\00:07:53.13 and that can be very painful and sometimes 00:07:53.17\00:07:55.37 the risk of a narcotic is worth the benefits... 00:07:55.40\00:07:58.27 But if someone is on a narcotic and they have to stay on it 00:07:58.31\00:08:02.04 because you can't get at the cause of the pain, 00:08:02.08\00:08:03.81 you want to take it very safely. 00:08:03.85\00:08:06.01 You want to try to use other treatments 00:08:06.05\00:08:08.78 so you don't have to escalate the dose to keep the dose 00:08:08.82\00:08:11.42 as low as possible and you want to tell them of the side effects 00:08:11.45\00:08:14.92 especially it can turn off the respiratory system; 00:08:14.96\00:08:17.83 it can slow the bowels... 00:08:17.86\00:08:19.76 And then if you can get at the cause, 00:08:19.79\00:08:22.33 then you have a chance of coming off the narcotics... 00:08:22.36\00:08:24.97 And sometimes, if a person has chronic back pain, 00:08:25.00\00:08:28.00 sometimes there are other modalities that can be used 00:08:28.04\00:08:31.11 like a TENS Unit, physical therapy, heat, 00:08:31.14\00:08:35.78 the cognitive ways that we mentioned, 00:08:35.81\00:08:37.65 and sometimes they can be turned down on the medications. 00:08:37.68\00:08:40.88 Sometimes you can mix and match. 00:08:40.92\00:08:42.48 You can use maybe a narcotic with an anti-inflammatory. 00:08:42.52\00:08:45.55 But the first thing we want in the approach to pain, Nick, 00:08:45.59\00:08:48.56 is we want, if possible, to get at the cause of pain, 00:08:48.59\00:08:51.83 and just the fact that this person is on chronic 00:08:51.86\00:08:55.53 pain medicines for a long period of time, 00:08:55.56\00:08:57.80 just the fact that they want to get off of the medication 00:08:57.83\00:09:00.90 is a good sign. A step in the right direction. 00:09:00.94\00:09:02.97 A lot of people take pain medications 00:09:03.14\00:09:05.44 and they really aren't in chronic pain. 00:09:05.47\00:09:07.74 They do it because of that feel-good, that buzz. 00:09:07.78\00:09:11.05 They use it to block out some of the world, 00:09:11.08\00:09:14.15 and if they keep getting more and more, 00:09:14.18\00:09:15.62 that's when we see the overdoses, 00:09:15.65\00:09:17.52 the real problems that we get into. 00:09:17.55\00:09:19.49 So we're going to put this person on our prayer list, Nick, 00:09:19.52\00:09:22.92 and we're going to pray for them that they'll help 00:09:22.96\00:09:25.29 find the right people that will 00:09:25.33\00:09:27.03 help them get off this medicine, 00:09:27.06\00:09:28.40 but I applaud them for trying to work on this, 00:09:28.43\00:09:31.07 and the first place I would go, I'd say, "Yes, there's a way 00:09:31.10\00:09:33.77 to get off pain medicines and the first place to go 00:09:33.80\00:09:36.20 is the Ultimate Physician. 00:09:36.24\00:09:37.77 Ask God to help you find the right people to help you 00:09:37.81\00:09:41.81 figure out ways you can do this because sometimes 00:09:41.84\00:09:43.68 you're your own best doctor. Yeah 00:09:43.71\00:09:45.61 Now, there's a lot of folks that are always interested in 00:09:45.65\00:09:48.58 natural remedies and I think that's a 00:09:48.62\00:09:50.39 great solution when it's appropriate. 00:09:50.42\00:09:52.19 Is chronic pain a place where 00:09:52.22\00:09:54.82 natural remedies can be appropriate? 00:09:54.86\00:09:56.32 Well, it is and sometimes people with chronic pain 00:09:56.36\00:10:00.23 have chronic pain because they're hurting themselves. 00:10:00.30\00:10:04.13 So one of the natural remedies if you are hurting in the 00:10:04.17\00:10:08.77 arm is maybe to quit using the arm. 00:10:08.80\00:10:10.97 You know, maybe rest and that's one of the Biblical treatments 00:10:11.01\00:10:14.81 for life is, you know, God taught us to 00:10:14.84\00:10:17.05 work 6 days and take a day off and rest. Right 00:10:17.08\00:10:19.58 When we rest, the body heals better. 00:10:19.61\00:10:21.92 Another thing is nightly rest, Nick. 00:10:21.95\00:10:24.59 You know, we were supposed to work in the day 00:10:24.62\00:10:26.59 and rest at night because we can heal. 00:10:26.62\00:10:29.02 The circadian rhythms go down. 00:10:29.06\00:10:30.63 Some of the stress chemistry goes down. 00:10:30.66\00:10:32.79 So we heal better when we rest better. 00:10:32.83\00:10:35.23 So one of the best natural remedies is just rest. 00:10:35.26\00:10:38.53 We talked about the nerves... 00:10:38.57\00:10:40.34 The nerves have to be happy so they can heal. 00:10:40.37\00:10:43.51 And some of the things that help nerves 00:10:43.54\00:10:45.51 are things just simple as water. 00:10:45.54\00:10:47.41 And getting appropriate nutrition... 00:10:47.44\00:10:49.34 There are certain diets that cause inflammation in the body. 00:10:49.38\00:10:52.41 We know that high fat and high 00:10:52.45\00:10:54.08 protein diets cause inflammation. 00:10:54.12\00:10:56.28 We also know that people that eat more whole food 00:10:56.32\00:10:58.72 plant-based nutrition, they do better with inflammation. 00:10:58.75\00:11:02.62 There are natural things like physical therapy that helps. 00:11:02.66\00:11:06.83 There is natural ginger that is one of the things 00:11:06.86\00:11:10.50 that works for chronic pain because 00:11:10.53\00:11:11.93 it is an anti-inflammatory, sometimes that helps. 00:11:11.97\00:11:15.64 There are natural things like prayer and worship. 00:11:15.67\00:11:19.54 Worship turns down the stress chemistry, 00:11:19.57\00:11:22.04 that helps with chronic pain, decreases inflammation. 00:11:22.08\00:11:25.31 Having healthy relationships, 00:11:25.35\00:11:27.45 one of the best ways is laughing. Really? 00:11:27.48\00:11:30.52 Yeah, if I had a good joke, I'd tell it to you, 00:11:30.55\00:11:33.22 but I'm not a funny person, Nick, are you a funny person? 00:11:33.25\00:11:36.39 Do you have a joke you could throw out? 00:11:36.42\00:11:37.76 I don't, I don't. 00:11:37.79\00:11:39.13 You know, I used to do jokes with my kids, 00:11:40.13\00:11:43.20 but they never thought it was funny. 00:11:43.23\00:11:44.77 Did you hear the joke about the rope? 00:11:44.80\00:11:47.40 Uh uh... "Skip it" - do you get it? 00:11:47.44\00:11:49.60 "Skip it," but anyway the point is, Nick, is 00:11:49.64\00:11:52.51 laughter is the best medicine. 00:11:53.21\00:11:55.08 Chemically, it makes a chemical called "endorphins," 00:11:55.11\00:11:58.41 so there are a lot of things that we can do. 00:11:58.45\00:12:00.95 Did you know that when you have a pet, 00:12:00.98\00:12:02.48 that can help with chronic pain. Really? 00:12:02.52\00:12:04.32 Yeah, you can look at the pet and the pet 00:12:04.35\00:12:06.32 looks back at you... we call that "eye-hugging." 00:12:06.35\00:12:09.29 And when you eye-hug a pet like that, 00:12:09.32\00:12:11.06 you make a chemical called "Oxytocin," 00:12:11.09\00:12:13.36 go up in your body... that helps with 00:12:13.40\00:12:15.40 inflammation and chronic pain. Sure 00:12:15.43\00:12:17.13 Now those are all little natural remedies. 00:12:17.43\00:12:20.07 Did you know that getting out in the sunshine 00:12:20.10\00:12:21.94 helps with chronic pain? 00:12:21.97\00:12:23.67 Vitamin D can help with chronic pain, 00:12:23.71\00:12:25.87 especially if it has to do with bones and muscles. 00:12:25.91\00:12:28.34 So there's a host of different things 00:12:28.38\00:12:30.15 that can treat chronic pain. 00:12:30.18\00:12:31.95 It sounds like the more we live in harmony with God's design 00:12:31.98\00:12:35.12 and His plan for our lives, 00:12:35.15\00:12:36.48 the better off we will be.
