Welcome back - on today's program we are discussing 00:00:01.36\00:00:02.70 the gastrointestinal system, and, Dr. Marcum, 00:00:02.73\00:00:05.17 I think there's kind of an overarching principle 00:00:05.20\00:00:07.67 that you might apply to this discussion, is that right? 00:00:07.70\00:00:09.80 Yeah, and hopefully a couple of points that I want to 00:00:09.84\00:00:12.77 point out before we answer questions. 00:00:12.81\00:00:15.21 I think the questions are really going to help us know 00:00:15.24\00:00:17.95 and learn a lot about the GI system and things we can do 00:00:17.98\00:00:20.28 to improve it, but a couple of principles that are: 00:00:20.32\00:00:22.22 1. Don't put bad things in it. 00:00:22.25\00:00:25.75 2. Let it get enough rest, the system, 00:00:25.79\00:00:28.99 let it get enough rest, don't use it all the time. 00:00:29.02\00:00:31.89 3. Give it the basic things it needs to work well. 00:00:31.93\00:00:35.60 And the basic things that the GI system needs to work well 00:00:35.63\00:00:38.83 are water and fiber. Okay 00:00:38.87\00:00:42.87 Water and fiber - very important for the GI system. 00:00:42.90\00:00:45.37 Water, fiber, rest - don't put bad things in, 00:00:45.41\00:00:48.68 that is a great place to start. 00:00:48.71\00:00:50.81 Now, why is the water and fiber so helpful for the GI system? 00:00:50.85\00:00:54.32 Water helps things move through the system well. 00:00:54.35\00:00:57.65 It keeps it lubricated. 00:00:57.69\00:00:59.02 Every cell in the body needs water. 00:00:59.05\00:01:00.79 So the GI system to do its job well, needs water. 00:01:00.82\00:01:04.63 Okay...What about the fiber? 00:01:04.66\00:01:06.06 The fiber helps motility, it helps things move through 00:01:06.09\00:01:09.16 so things don't stay in it for long periods of time. 00:01:09.20\00:01:11.67 You keep the GI system moving. 00:01:11.70\00:01:14.34 When things are static, when things in the GI system 00:01:14.37\00:01:17.04 don't move, you have a greater chance to absorb 00:01:17.07\00:01:19.71 toxins and byproducts of this ecosystem 00:01:19.74\00:01:24.15 that we are going to talk about. Right 00:01:24.18\00:01:25.85 And really, the bowel has its own ecosystem within it. 00:01:25.88\00:01:28.85 It's almost like a world within a world, 00:01:28.88\00:01:31.05 and that's very important. 00:01:31.09\00:01:32.42 We're going to learn a little bit about that 00:01:32.45\00:01:33.92 when we answer a question about probiotics. 00:01:33.96\00:01:36.22 All right, well let's get right to the questions... 00:01:36.26\00:01:37.96 Our first viewer asks: "What is Barrett's esophagus?" 00:01:37.99\00:01:41.43 Barrett's esophagus specifically has to do with this middle 00:01:41.46\00:01:46.07 part called the "esophagus," and the esophagus really 00:01:46.10\00:01:49.94 just transports food from the 00:01:49.97\00:01:51.41 mouth that we start the digestion in, 00:01:51.44\00:01:53.71 and moves it all the way to the stomach. 00:01:53.74\00:01:55.74 There's a valve at the lower part of the esophagus 00:01:55.78\00:01:58.95 that keeps the acid in the stomach. Okay 00:01:58.98\00:02:02.22 But when the stomach is overburdened, 00:02:02.25\00:02:04.35 makes too much acid - what would make it too much acid? 00:02:04.39\00:02:07.16 Eating too much, too much protein. 00:02:07.69\00:02:10.29 There are certain metabolic conditions that cause 00:02:10.33\00:02:13.23 lots of acid to be done. 00:02:13.26\00:02:14.90 Bacteria can cause acid, but if the body makes 00:02:14.93\00:02:17.70 too much acid, you eat late at night and you lay down, 00:02:17.73\00:02:20.70 some people carry extra weight or have hiatal hernias, then the 00:02:20.74\00:02:24.34 acid can slip up this bowel and get into the esophagus. 00:02:24.37\00:02:28.34 Hydrochloric acid in tissue that's not made 00:02:28.38\00:02:31.55 to see hydrochloric acid is not good. 00:02:31.58\00:02:34.22 It damages the cell life. Right 00:02:34.25\00:02:36.08 So when the hydrochloric acid seeps up into the 00:02:36.55\00:02:39.35 esophagus, a lot of people have what we call "heartburn." 00:02:39.39\00:02:43.36 We call that "reflux" because the acid refluxes up. 00:02:43.39\00:02:47.63 They call it "gastroesophageal reflux," 00:02:47.66\00:02:50.73 acid refluxes up into that. 00:02:50.77\00:02:52.80 If it happens long enough, the acid actually damages 00:02:52.83\00:02:56.24 the cells in the esophagus and make them more 00:02:56.27\00:02:59.27 prone to develop cancer. 00:02:59.31\00:03:01.44 That type of esophagus we call "Barrett's esophagus." 00:03:01.48\00:03:05.61 So if lots of acid bubbles up long enough, 00:03:05.65\00:03:07.88 it can damage the cells and it also can convert those 00:03:07.92\00:03:11.59 normal cells into cancer cells. 00:03:11.62\00:03:14.36 Cancer of the esophagus is a very hard thing to deal with. 00:03:14.39\00:03:18.33 Now the things that happen with the esophagus, Nick, 00:03:18.36\00:03:20.90 the main things that happen is the reflux, 00:03:20.93\00:03:23.40 and sometimes the reflux can cause esophagus to get narrowed. 00:03:24.10\00:03:27.47 We call that a "stricture," and a person might 00:03:27.50\00:03:30.07 not be able to swallow. 00:03:30.11\00:03:31.67 Sometimes the esophagus can spasm. 00:03:31.71\00:03:34.21 You known when they have spasm - you eat something 00:03:34.24\00:03:35.91 and it won't go through, that can be a spasm, 00:03:35.94\00:03:38.31 and then, of course, we talked about the cancer... 00:03:38.35\00:03:40.68 those are some of the 3 more common things that happen 00:03:40.72\00:03:42.92 to the esophagus. 00:03:42.95\00:03:44.35 So Barrett's esophagus is when acid refluxes up 00:03:44.39\00:03:47.79 and it damages the cells inside the esophagus. 00:03:47.82\00:03:50.63 So that's something we really want to try and avoid. 00:03:50.66\00:03:52.83 Yes, we do not, and people who have that, 00:03:52.86\00:03:54.76 they have to have regular looks and specimens drawn 00:03:54.80\00:03:57.63 to make sure the cells are not changing into cancer. 00:03:57.67\00:04:00.34 And they want to limit that acid reflux, 00:04:00.37\00:04:02.94 so some of them have to take special antacids. 00:04:02.97\00:04:05.44 And then they want to do lifestyle measures 00:04:05.47\00:04:07.41 that lower their risk of acid going up 00:04:07.44\00:04:09.81 which would include losing weight, 00:04:09.84\00:04:11.75 eating a whole food plant-based diet, 00:04:11.78\00:04:14.98 stay away from a lot of protein-like foods, 00:04:15.02\00:04:17.65 make sure they don't have infections. 00:04:17.69\00:04:19.75 If they have hernias, deal with those as well. 00:04:19.79\00:04:21.86 I didn't realize that acid reflux could be 00:04:21.89\00:04:23.93 a really serious condition like that. 00:04:23.96\00:04:25.33 It can, it can, but one of the more serious things 00:04:25.36\00:04:28.53 is it can cause the esophagus to get sticky inside, 00:04:28.56\00:04:31.93 and when it gets sticky, it sort of squeezes off 00:04:31.97\00:04:34.94 so the food can't get through. Right 00:04:34.97\00:04:36.60 So we have some people that the acid damages the esophagus 00:04:36.64\00:04:39.44 so much, Nick, that they can't get the food through. 00:04:39.47\00:04:42.24 If you can't get the food through, 00:04:42.28\00:04:43.68 you don't get any nutrition... Yeah 00:04:43.71\00:04:45.18 So that is a very... Now is it possible to 00:04:45.21\00:04:47.58 transplant the esophagus? Yes. Really? 00:04:47.62\00:04:49.78 We've had some esophageal transplants, 00:04:49.82\00:04:52.79 but it's very hard to do. 00:04:52.82\00:04:54.16 It's not our first line of treatment. 00:04:54.19\00:04:55.59 Right, not the preferred way to go. 00:04:55.62\00:04:56.96 So that's just part of the tube, that's just some of the things 00:04:56.99\00:04:59.19 that can happen to the esophagus. Okay 00:04:59.23\00:05:00.96 We've got another question and sometimes these questions 00:05:01.00\00:05:04.43 dealing with the GI can be somewhat embarrassing, 00:05:04.47\00:05:06.37 but she says: "I'm a female with fecal incontinence, 00:05:06.40\00:05:09.07 and it is embarrassing, what are my options for treatment?" 00:05:09.10\00:05:11.81 Well I think we need to talk to people about what 00:05:11.84\00:05:13.94 fecal incontinence is. 00:05:13.98\00:05:16.14 Now remember, we've talked about the tube, 00:05:16.18\00:05:18.48 and it has like little devices that keep things from 00:05:18.51\00:05:21.25 going the wrong direction, keep things moving in. 00:05:21.28\00:05:23.42 The one at the mouth and the esophagus 00:05:23.45\00:05:25.52 is called the "epiglottis." 00:05:25.55\00:05:27.49 We have one between the stomach and the esophagus, 00:05:27.52\00:05:29.86 the lower esophageal sphincter, we got some sphincters there. 00:05:29.89\00:05:33.96 Then as it moves through the stomach where digestion 00:05:34.00\00:05:37.37 and all the digestion occurs through the small 00:05:37.40\00:05:39.37 and large intestines, eventually 00:05:39.40\00:05:40.80 the waste have to leave. 00:05:40.84\00:05:42.87 And of course, we have a sphincter down there 00:05:42.90\00:05:45.07 that keeps it in as well. 00:05:45.11\00:05:47.14 Sometimes that sphincter wears out, 00:05:47.18\00:05:50.68 and when it wears out, guess what? 00:05:50.71\00:05:52.78 The poop just comes out. Yeah 00:05:52.81\00:05:54.42 And that can be very embarrassing. 00:05:54.45\00:05:56.48 It's a major problem and it happens 00:05:56.52\00:05:59.09 to a fair number of people. 00:05:59.12\00:06:00.46 We don't hear about it much because 00:06:00.49\00:06:01.82 people are so embarrassed. 00:06:01.86\00:06:03.19 And a lot of people, it happens when people tend to get older, 00:06:03.22\00:06:07.26 and sometimes they have to wear diapers. 00:06:07.30\00:06:09.76 And we hear of stool incontinence. 00:06:09.80\00:06:12.73 We call that "fecal incontinence," 00:06:12.77\00:06:14.30 but it also comes with the urine as well. 00:06:14.34\00:06:16.40 So some people just wear the diapers, 00:06:16.44\00:06:18.27 and we never hear about it. 00:06:18.31\00:06:20.08 Because the tube doesn't have 00:06:20.11\00:06:21.64 ability to keep what's in it, in it. 00:06:21.68\00:06:24.48 So there are a couple of things that people can do 00:06:24.51\00:06:27.18 to help that situation. 00:06:27.22\00:06:28.62 One, is a stool softener to make sure the stool is not 00:06:28.65\00:06:31.95 coming out really hard - that can help protect 00:06:31.99\00:06:34.22 that sphincter down there. 00:06:34.26\00:06:36.19 Another thing is lots of fiber, lots of water, 00:06:36.22\00:06:39.03 we talked about that. 00:06:39.06\00:06:40.40 There is an exercise called "Kegel" exercises that work. 00:06:40.43\00:06:43.63 They can go to the internet and learn how to do 00:06:43.67\00:06:45.77 those special exercises - that helps the muscles. 00:06:45.80\00:06:48.60 So it makes the muscles so 00:06:48.64\00:06:50.27 that sphincter works a little bit better. 00:06:50.31\00:06:52.17 But one of the things I learned not too long ago, 00:06:52.21\00:06:54.58 from Heartwise Ministries, one of our advisors, 00:06:54.61\00:06:57.18 that happens to be a colorectal surgeon. 00:06:57.21\00:06:59.68 He says that there are some very simple surgeries 00:06:59.71\00:07:02.28 that they can do now on that sphincter to tighten it up 00:07:02.32\00:07:05.85 so people don't have to have incontinence. Okay 00:07:05.89\00:07:09.79 So a lot of people don't realize it, but there are surgeons 00:07:09.82\00:07:12.33 that specialize in this and this is a great place 00:07:12.36\00:07:14.63 for modern medicine. 00:07:14.66\00:07:16.10 So a surgeon can do a simple and some of them 00:07:16.13\00:07:17.87 even robotic surgeries that they do that they tighten up 00:07:17.90\00:07:20.97 that sphincter so you don't have 00:07:21.00\00:07:22.47 the problem with fecal incontinence. 00:07:22.50\00:07:24.87 So that's on the lower part of the tube. 00:07:24.91\00:07:27.58 What are some of the parts 00:07:27.61\00:07:28.94 that have the lower part of the tube? 00:07:28.98\00:07:30.31 Well, we talked about incontinence, 00:07:30.35\00:07:31.98 of course cancers happen there. 00:07:32.01\00:07:34.05 We also see hemorrhoids... that's sort of veins 00:07:34.08\00:07:37.02 sort of like varicose veins down there that pooch through. 00:07:37.05\00:07:40.12 That can be very painful, can actually cause bleeding. 00:07:40.16\00:07:43.06 When it gets real dry, we can have 00:07:43.09\00:07:45.93 what we call "fissures," and the skin there is 00:07:45.96\00:07:48.03 very different than other parts of skin, 00:07:48.06\00:07:49.70 and it can have fissures where you have bleeding 00:07:49.73\00:07:51.83 that goes on in that part of the tube. Okay 00:07:51.87\00:07:53.74 So that's why the things we talked about, 00:07:53.77\00:07:55.57 and, you know, the same help that part of the tube 00:07:55.60\00:07:58.17 that helped the other part of the tube. 00:07:58.21\00:07:59.91 We want to avoid toxins, lots of water, 00:07:59.94\00:08:03.01 lots of fiber, but sometimes the tube does get older, 00:08:03.04\00:08:06.68 and this is one instance where modern medicine is very 00:08:06.72\00:08:09.32 helpful and find a good surgeon that can 00:08:09.35\00:08:11.99 fix that lower sphincter. 00:08:12.02\00:08:14.12 And even though these things are maybe embarrassing, 00:08:14.16\00:08:16.22 you know, it's really worth talking to your doctor 00:08:16.26\00:08:18.09 and see what can be done. Yeah it is... 00:08:18.13\00:08:20.13 Our next question comes from a viewer who asks: 00:08:20.16\00:08:22.63 "What are "probiotics, and how can they be useful?" 00:08:22.66\00:08:25.87 Yeah, and that's what we have to spend a lot of time on. 00:08:25.90\00:08:28.60 We are just now learning, in science, that the bowel is a big 00:08:28.64\00:08:33.64 tube that has trillions of bacteria. 00:08:33.68\00:08:37.71 It is its own ecosystem of itself. 00:08:37.75\00:08:40.92 And these trillion bacteria have to live in a very 00:08:41.22\00:08:44.39 special environment. 00:08:44.42\00:08:46.05 If you get one that has overgrown the other, 00:08:46.09\00:08:48.39 it makes chemicals, it makes inflammation, 00:08:48.42\00:08:50.66 it makes byproducts, it does all sorts of bad things. 00:08:50.69\00:08:54.16 A lot of people eat bad foods and they mess up this ecosystem, 00:08:54.20\00:08:58.37 and when the ecosystem is up, it destroys everything. 00:08:58.40\00:09:01.44 In fact, I've got some examples of some people 00:09:01.47\00:09:04.57 that have eaten foods that damaged the ecosystem. 00:09:04.61\00:09:08.38 Here, I have a 52-year-old woman, 00:09:08.41\00:09:10.75 she had daily headaches, everyday lots of migraines 00:09:10.78\00:09:13.85 for years because the ecosystem, 00:09:13.88\00:09:17.62 she damaged her flora and it was making chemicals 00:09:17.65\00:09:20.12 that caused these migraines. Right 00:09:20.16\00:09:22.79 So she found relief just by getting her bowel flora 00:09:22.82\00:09:25.59 back to normal and it stopped the inflammatory 00:09:25.63\00:09:28.53 mediators that were causing her headaches. Wow 00:09:28.56\00:09:31.13 And you know, you have all sorts of different 00:09:31.17\00:09:33.40 damages that can occur from this. 00:09:33.44\00:09:35.80 Another person, this happened to be a 6-year-old girl, 00:09:35.84\00:09:38.74 and she had behavioral problems. 00:09:38.77\00:09:41.71 And, she was violent, disruptive, depressive, 00:09:41.74\00:09:44.55 all sorts of this and her problem was happening 00:09:44.58\00:09:47.08 because she had bacteria overgrowth. 00:09:47.12\00:09:49.62 She was eating the wrong types of food. 00:09:49.65\00:09:51.75 And what happened in this little girl is the case study showed 00:09:51.79\00:09:55.22 that when she was eating lots of animal products... 00:09:55.26\00:09:57.76 Okay, animal products changes our ecosystem because 00:09:57.79\00:10:01.56 a lot of antibiotics are in animal products, 00:10:01.60\00:10:05.07 and that kills off the bacteria. 00:10:05.10\00:10:07.77 Some of the good bacteria are killed, 00:10:07.80\00:10:09.27 some of the bad take over, some of the yeast take over. 00:10:09.30\00:10:12.21 They start making all sorts of bad mediators, 00:10:12.24\00:10:15.01 it changes our genetics and this was actually 00:10:15.04\00:10:17.55 damaging this girl's chemicals that affected her brain, 00:10:17.58\00:10:20.88 and she was having behavioral problems 00:10:20.92\00:10:22.68 just because of the gut. Wow Isn't that pretty interesting? 00:10:22.72\00:10:25.42 Yeah, I wouldn't think that behavioral problems 00:10:25.45\00:10:27.16 or headaches are things that could be related 00:10:27.19\00:10:28.86 to what's happening in your stomach. Right 00:10:28.89\00:10:30.69 And that's why we wanted to talk about this. 00:10:30.73\00:10:33.43 Another case history of a 3- year-old boy with 00:10:33.46\00:10:36.63 autistic-like symptoms. 00:10:36.67\00:10:38.90 They started treating a parasite because his gut 00:10:38.93\00:10:42.30 flora was abnormal. 00:10:42.34\00:10:43.84 It was abnormal, he couldn't get rid of the parasite. 00:10:43.87\00:10:46.41 The parasite triggered the change in this whole ecosystem. 00:10:46.44\00:10:50.38 Remember, this ecosystem has genetics in it. 00:10:50.41\00:10:55.35 There are 100 more genes in the bacteria in your bowel 00:10:55.38\00:10:58.72 than throughout your body, a hundred times more genes. 00:10:58.75\00:11:02.06 So if you have all those genetic material when they are 00:11:02.09\00:11:05.19 turning on all sorts of different proteins 00:11:05.23\00:11:07.30 and different chemical reactions that get absorbed 00:11:07.33\00:11:09.63 into the body... the toxins, the byproducts, 00:11:09.66\00:11:12.30 and it goes places. 00:11:12.33\00:11:13.80 It goes to the heart, it goes to the brain, 00:11:13.84\00:11:15.47 it causes all sorts of bad things to happen. 00:11:15.50\00:11:17.81 So a lot of good health starts in the gut. Right 00:11:17.84\00:11:21.58 Not only the good things we get in, 00:11:21.61\00:11:23.35 but the bad things that we put in that are absorbed 00:11:23.38\00:11:26.31 because of bacteria which is normally protecting us 00:11:26.35\00:11:29.18 from those evil empire, the bacteria is messed up, 00:11:29.22\00:11:32.29 the bad bacteria starts saying, "Well, I have DNA too, 00:11:32.32\00:11:35.39 I'm going to make whatever I want and it goes somewhere." 00:11:35.42\00:11:37.99 It gets in our blood system, it messes up the balances 00:11:38.03\00:11:40.43 in our brain, in our endothelium and then the good 00:11:40.46\00:11:43.23 bacteria are overrun. 00:11:43.26\00:11:44.60 And we've seen that in all the antibiotics we're using 00:11:44.63\00:11:48.07 nowadays in animals. 00:11:48.10\00:11:49.44 So people that eat lots of animal-based nutrition, 00:11:49.47\00:11:53.48 that's bad for the ecosystem. 00:11:53.51\00:11:55.38 People that don't have fiber, that's bad for the system. 00:11:55.41\00:11:58.08 So in short, we mess up this fine balance that we have 00:11:58.11\00:12:01.32 in our system and that system 00:12:01.35\00:12:03.35 literally destroys us if we let it. 00:12:03.39\00:12:05.39 So it goes from anything from depression 00:12:05.42\00:12:08.29 to heart attacks, lots of things. 00:12:08.32\00:12:10.86 The brain and the bowel are intimately connected 00:12:10.89\00:12:13.19 because these trillion bacteria and the genetics 00:12:13.23\00:12:15.56 that goes with it, makes chemicals that throw things 00:12:15.60\00:12:18.13 off balance. 00:12:18.17\00:12:19.50 So once things are out of balance, 00:12:19.53\00:12:21.00 how can that be reversed? 00:12:21.04\00:12:22.94 That's right and that's one of the things, 00:12:22.97\00:12:24.31 you gotta get your bowel happy again, 00:12:24.34\00:12:25.74 and we talked about the basic principles... 00:12:25.77\00:12:27.71 Drink lots of water, lots of fiber, 00:12:27.74\00:12:30.81 and you've heard about these things that detox the system 00:12:30.85\00:12:33.38 like greens - greens are great for detoxing! 00:12:33.42\00:12:36.42 It has all sorts of fiber in it, chemicals like chlorophyll 00:12:36.45\00:12:39.65 which helps set up the good bacteria. 00:12:39.69\00:12:42.16 And nowadays, people that do eat bad foods, 00:12:42.19\00:12:44.96 and don't eat a good diet, I recommend that they take 00:12:44.99\00:12:47.60 a probiotic. Okay 00:12:47.63\00:12:49.03 Probiotics are lots of good bacteria that we introduce 00:12:49.06\00:12:53.13 back into the system. Okay 00:12:53.17\00:12:55.00 In fact, this has gotten so much press recently 00:12:55.04\00:12:58.17 that sometimes people that take too many antibiotics 00:12:58.21\00:13:00.61 they get overgrowth of bad bacteria that 00:13:00.64\00:13:03.04 really cause lots of problems. 00:13:03.08\00:13:04.55 One of them is called, "Clostridium." 00:13:04.61\00:13:06.48 It's even possible now... a lot of people need to take 00:13:06.51\00:13:10.32 probiotics - and even stress can mess up the bowel flora, 00:13:10.35\00:13:14.62 but we found out that sometimes the bowel flora is so messed up, 00:13:14.66\00:13:18.33 you even have to have a transplant of bacteria, 00:13:18.36\00:13:20.70 a good bacteria back into the bowels.
