Okay - let's jump into it! 00:00:10.67\00:00:12.27 Our first viewer asks: How do I come off the 00:00:12.31\00:00:14.64 sleeping pill "Ambien?" 00:00:14.68\00:00:16.01 Yeah, well the sleeping pill "Ambien," 00:00:16.04\00:00:18.15 helps people sleep well at night. 00:00:18.18\00:00:20.42 The benefit of it is it knocks you out. 00:00:20.45\00:00:22.28 The risk is that it might hurt your cognition 00:00:22.32\00:00:24.62 and your ability to drive a car. 00:00:24.65\00:00:26.29 Well, if we want to get off of it, let's figure out ways 00:00:26.32\00:00:29.42 we can help a person sleep better. 00:00:29.46\00:00:31.03 What's the cause of them not sleeping well? 00:00:31.06\00:00:33.23 Is it sleep apnea? 00:00:33.26\00:00:34.76 Is it a medicine making their blood sugar go low? 00:00:34.80\00:00:37.27 Is it a stimulant like caffeine or alcohol, 00:00:37.30\00:00:40.07 eating late at night? 00:00:40.10\00:00:41.44 Is it television? Is it light? 00:00:41.47\00:00:43.77 Those are the things if we can find out the cause 00:00:43.81\00:00:46.21 of them not sleeping well, 00:00:46.24\00:00:47.58 maybe we can get the ability to taper that 00:00:47.61\00:00:50.35 medicine to off so maybe they won't have to take that pill 00:00:50.38\00:00:53.52 the rest of their life. Great question! 00:00:53.55\00:00:56.08 But again, it just lets us know that it is possible 00:00:56.12\00:00:59.05 if someone will work with you to come off 00:00:59.09\00:01:00.89 some of these medications. How about a statin? 00:01:00.92\00:01:02.79 Remind us what is a statin and is it possible to come off of. 00:01:02.82\00:01:04.73 A statin is a medication to lower cholesterol. Okay 00:01:04.76\00:01:07.53 So, if you can lower your cholesterol 00:01:07.56\00:01:09.16 with a natural means and your 00:01:09.20\00:01:10.67 risk-benefit profile is acceptable, 00:01:10.70\00:01:12.77 that's another one you might be going off. 00:01:12.80\00:01:14.80 So let's say that, Nick, you don't have any medical 00:01:14.84\00:01:17.54 problems and you come to me with an LDL cholesterol of 200, 00:01:17.57\00:01:21.24 that's too high and I'm going to say, Nick... 00:01:21.28\00:01:23.68 "You have an option, you can either start eating 00:01:23.71\00:01:25.81 right and exercise or you could take a statin." 00:01:25.85\00:01:28.38 A statin will lower your risk, so at that point, 00:01:28.42\00:01:32.62 it's probably beneficial to you, but if you can lower your risk 00:01:32.65\00:01:35.19 other ways, maybe you won't have to be married to that. 00:01:35.22\00:01:37.93 Now some people that have genetic problems, 00:01:37.96\00:01:40.03 need to be on medicines like that. 00:01:40.06\00:01:41.56 Okay, well that was an easy one. 00:01:41.60\00:01:43.43 How about Coumadin, I hear a lot of people 00:01:43.47\00:01:45.30 talk about not wanting to take Coumadin. 00:01:45.33\00:01:47.14 Why do people shy away from it? 00:01:47.17\00:01:48.94 Coumadin is a blood thinner, and Coumadin has a risk of 00:01:48.97\00:01:53.07 bleeding and the generic Coumadin called "warfarin," 00:01:53.11\00:01:56.61 it is rat poison - they used to use it to kill rats. 00:01:56.64\00:01:58.88 That's not good. Right 00:01:58.91\00:02:01.18 But Coumadin does prevent clots. 00:02:01.22\00:02:02.95 So if a person has a blood clot in their heart 00:02:02.98\00:02:05.42 which could pump up to their brain and cause a stroke, 00:02:05.45\00:02:08.79 or a blood clot in their legs which could pump 00:02:08.82\00:02:10.76 to their lungs and cause a pulmonary embolism, 00:02:10.79\00:02:13.09 then the risk of Coumadin might be worth it. 00:02:13.13\00:02:17.10 Now I don't like to give people that medication, 00:02:17.13\00:02:19.60 but if it's very risky, we do, but we monitor it 00:02:19.63\00:02:22.54 very carefully - we monitor for bleeding. 00:02:22.57\00:02:25.57 We let them know about the risk and benefits; 00:02:25.61\00:02:28.28 we let them know if other medications interact 00:02:28.31\00:02:30.65 with it and we monitor their blood very closely 00:02:30.68\00:02:32.98 to make sure it doesn't get too thick or too thin. 00:02:33.01\00:02:35.68 Now sometimes, it might be possible 00:02:35.72\00:02:38.05 to come off the Coumadin, but it's a rare person 00:02:38.09\00:02:41.52 that can come off that because usually when they are 00:02:41.56\00:02:43.63 putting them on it, means they have gotten 00:02:43.66\00:02:44.99 a blood clot. 00:02:45.03\00:02:46.36 So to get off that medicine, you've got to eliminate 00:02:46.39\00:02:48.56 the chance of a blood clot coming back again, 00:02:48.60\00:02:51.00 sometimes that's very hard to do. All right 00:02:51.03\00:02:53.87 We've got another question... 00:02:53.90\00:02:55.40 This viewer asks: "What is the safest antibiotic 00:02:55.44\00:02:58.57 and is it true that many allergies to penicillin 00:02:58.61\00:03:01.04 are not true allergies?" 00:03:01.08\00:03:02.71 First of all, many allergies of 00:03:02.74\00:03:04.51 penicillin are not true allergies. 00:03:04.55\00:03:06.21 You know, people just say they are... 00:03:06.25\00:03:07.88 We can certainly have a person go to an allergist 00:03:07.92\00:03:10.29 and test for that... The first question is: 00:03:10.32\00:03:13.96 "Antibiotics," are they safe antibiotics? 00:03:14.02\00:03:17.59 Well, there are safe antibiotics but the best thing about 00:03:17.63\00:03:20.96 antibiotics is try not to use them. 00:03:21.00\00:03:23.26 Many antibiotics are given for virus infections... 00:03:23.30\00:03:26.53 they don't even need to be given and people become 00:03:26.57\00:03:29.60 resistant to antibiotics, especially people that 00:03:29.64\00:03:31.97 eat animals! 00:03:32.01\00:03:33.84 Seventy to 80% of all the antibiotics used are 00:03:33.88\00:03:36.38 passed on to us in some form from animals that we eat. 00:03:36.41\00:03:40.38 So we get sort of antibiotic resistance when we have 00:03:40.42\00:03:43.25 some low grade antibiotics in us all the time - that's not good! 00:03:43.28\00:03:47.22 So we've heard about antibiotic resistance in our society. 00:03:47.26\00:03:50.76 We've heard about methicillin resistant staph... 00:03:50.79\00:03:53.60 things of that nature, so we want to limit 00:03:53.63\00:03:55.86 our exposures to antibiotics and take them 00:03:55.90\00:03:58.00 when we really need it, but if we really don't 00:03:58.03\00:04:00.54 need an antibiotic - if it's a virus, 00:04:00.57\00:04:02.34 we won't want to take antibiotics from animals; 00:04:02.37\00:04:05.97 we don't want to take those type of things, 00:04:06.01\00:04:07.88 then antibiotics can be used effectively, 00:04:07.91\00:04:10.18 but nowadays, we're not using them too effectively. 00:04:10.21\00:04:12.65 If we use them ineffectively, we can damage our 00:04:12.68\00:04:15.48 bacterial flora and it damages 00:04:15.52\00:04:18.35 the trillium bacteria's that messes us up. 00:04:18.39\00:04:20.49 When those bacteria damages, they turn on chemicals 00:04:20.52\00:04:23.32 that puts stress on our body 00:04:23.36\00:04:24.96 causes all sorts of problems with that. Right 00:04:24.99\00:04:26.76 Some people even have to take probiotics, 00:04:26.80\00:04:28.73 things of that nature, if they're on lots of antibiotics. 00:04:28.76\00:04:32.03 Now, why might some people think they have 00:04:32.07\00:04:34.07 an allergic reaction to penicillin, 00:04:34.10\00:04:36.04 but it's maybe not really allergies? 00:04:36.07\00:04:37.41 Well, they might get a little rash when they take it, 00:04:37.44\00:04:39.54 and that's not a real anaphylactic reaction. 00:04:39.57\00:04:41.88 Just a minor... Yeah, a minor reaction. 00:04:41.91\00:04:43.55 Some people get it to a generic preparation. 00:04:43.58\00:04:46.21 It might not be the allergy to penicillin, 00:04:46.25\00:04:48.25 but it might be the packaging or the delivery system. 00:04:48.28\00:04:51.05 Remember, when a medicine is invented, not only is it 00:04:51.09\00:04:54.89 the active ingredient, but the delivery system, 00:04:54.92\00:04:57.93 how is it packaged... 00:04:57.96\00:04:59.29 how is it delivered in the system? 00:04:59.33\00:05:00.93 it takes chemicals - they might be allergic to those chemicals. 00:05:00.96\00:05:04.03 Generic medicines have different delivery systems 00:05:04.07\00:05:07.04 than pure medications sometimes. 00:05:07.07\00:05:09.77 Now it seems, most medications have side effects, 00:05:09.80\00:05:12.07 but are there any that don't have side effects? 00:05:12.11\00:05:13.44 I have no medicine that doesn't have a 00:05:13.48\00:05:15.21 side effect to some degree. 00:05:15.24\00:05:16.58 Now, some are safer than others like - you know, 00:05:16.61\00:05:18.58 we talked about Tylenol, that has a few side effects 00:05:18.61\00:05:20.98 on the liver, but it's very safe. 00:05:21.02\00:05:23.89 Advil, Motrin - some of them have side effects, 00:05:23.92\00:05:26.35 but they're not terrible. 00:05:26.39\00:05:27.72 The ones I see have side effects, Nick, 00:05:27.76\00:05:29.96 are the ones that people use chronically. 00:05:29.99\00:05:32.33 Prolonged exposures to medications raise the risk 00:05:32.36\00:05:35.70 of side effects. Right 00:05:35.73\00:05:37.07 Getting you over the hump usually doesn't have 00:05:37.10\00:05:39.00 as many side effects unless you have 00:05:39.03\00:05:40.67 one of these terrible immune 00:05:40.70\00:05:42.20 reactions to it. Good information! 00:05:42.24\00:05:44.97 Our next viewer has a question about blood pressure. 00:05:45.01\00:05:48.31 They've been on 3 different medications, 00:05:48.34\00:05:50.25 and they want to know which medication 00:05:50.28\00:05:52.25 may have the least side effects. 00:05:52.28\00:05:54.05 And, you know, the approach to blood pressure is, of course, 00:05:54.08\00:05:57.32 find our what's causing your blood pressure; 00:05:57.35\00:05:59.29 try to have some natural remedies to lower 00:05:59.32\00:06:01.72 blood pressure - like exercising, 00:06:01.76\00:06:03.59 a plant-based diet, lots of water, 00:06:03.63\00:06:06.03 no processed foods, things of that nature. 00:06:06.06\00:06:08.50 Some people still need to be on some medications 00:06:08.53\00:06:10.97 at least to get them over the hump. Sure 00:06:11.00\00:06:12.70 One of the medicines that I use that's very safe is 00:06:12.73\00:06:15.74 one called, "Norvasc." 00:06:15.77\00:06:17.21 That does not slow the heart rate down. 00:06:17.24\00:06:19.44 It makes the arteries a little bit bigger. 00:06:19.47\00:06:21.81 Now I can also accomplish that by people eating lots of greens, 00:06:21.84\00:06:25.45 and cutting back fat as well, but some people 00:06:25.48\00:06:28.02 cannot do that and if they can't do those things, 00:06:28.05\00:06:30.79 then the risk of high blood pressure is greater 00:06:30.82\00:06:33.76 than the risk of the medicine. 00:06:33.79\00:06:35.12 They might benefit to that, but at the same time, 00:06:35.16\00:06:37.53 I would watch that very closely. 00:06:37.56\00:06:39.63 Another medicine I use quite a bit for blood pressure is 00:06:39.66\00:06:42.36 called, "the beta blockers." 00:06:42.40\00:06:44.07 A person that has blood pressure 00:06:44.10\00:06:45.73 because they are stressed out a lot - you know, 00:06:45.77\00:06:48.30 that blocks adrenalin's effect on the blood vessels, 00:06:48.34\00:06:51.64 that might help a lot. 00:06:51.67\00:06:53.14 In someone that's anxious, we might use that. 00:06:53.17\00:06:55.41 Of course, I'd want them to do some other things, 00:06:55.44\00:06:58.21 maybe taking deep breaths, relaxing - you know 00:06:58.25\00:07:01.62 those things that will lower the blood pressure as well, 00:07:01.65\00:07:03.75 but if they can't do those things, 00:07:03.79\00:07:05.22 then sometimes the medicine is worth the side effects... 00:07:05.25\00:07:07.86 But those are two that are fairly safe. 00:07:07.89\00:07:09.96 But every medicine does have side effects. 00:07:09.99\00:07:12.79 The blood pressure medicines, of course, 00:07:12.83\00:07:14.16 can make it go too low. 00:07:14.20\00:07:15.86 So if you're dehydrated or if your kidneys quit working 00:07:15.90\00:07:19.00 or if your liver doesn't work, and you still take 00:07:19.03\00:07:21.27 the medicines, your blood pressure could go too low, 00:07:21.30\00:07:24.01 and you could pass out, you could hurt yourself, 00:07:24.04\00:07:26.14 so there are side effects to each of these medicines, 00:07:26.17\00:07:28.04 and I think what patients need to know is, first of all... 00:07:28.08\00:07:30.95 hopefully, they can try to change their chemistry 00:07:30.98\00:07:33.65 so they don't need a medicine. Sure 00:07:33.68\00:07:35.28 Then if they do need the medicine and their physician 00:07:35.32\00:07:37.55 thinks the risk is worth the benefit, 00:07:37.59\00:07:39.25 know about the risk. 00:07:39.29\00:07:40.62 Know when to take it, know what to look for, 00:07:40.66\00:07:42.79 know what the complications. 00:07:42.82\00:07:44.33 And I think when people learn about their medications, 00:07:44.36\00:07:47.23 they can take them much safer. 00:07:47.26\00:07:49.46 That's good advice! Okay 00:07:49.50\00:07:51.07 We have another viewer who asks: 00:07:51.10\00:07:52.53 "I take medication for my stomach, 00:07:52.57\00:07:54.17 is there a chance I can come off of this?" 00:07:54.20\00:07:56.40 And again, it's according to why they take the medications 00:07:56.44\00:08:00.51 for the stomach, okay? 00:08:00.54\00:08:02.24 I assume it's for acid. Okay 00:08:02.28\00:08:05.15 The body makes extra acid, it causes reflux... 00:08:05.18\00:08:09.15 that's a bothersome. 00:08:09.18\00:08:10.52 And a lot of people take these antacids, 00:08:10.55\00:08:12.72 and there are a bunch of different ones on the market... 00:08:12.75\00:08:14.82 Nexium, Prilosec, different ones, some you can even buy 00:08:14.86\00:08:17.56 over-the-counter. Right 00:08:17.59\00:08:19.69 We weren't really made to take these for the rest of our lives. 00:08:19.73\00:08:24.73 And some of those can change the calcium content of the body, 00:08:24.77\00:08:29.30 and so we try to get people off of those. 00:08:29.34\00:08:32.07 Well, if you can get at the cause. 00:08:32.11\00:08:33.74 You know, if it's because we're eating big at night 00:08:33.78\00:08:36.34 well, quit eating big at night. Sure 00:08:36.38\00:08:37.98 If it's because there's an infection in the stomach 00:08:38.01\00:08:39.95 called, "H. pylori," well have that treated. 00:08:39.98\00:08:42.72 If it's because they're eating high protein diets... 00:08:42.75\00:08:45.95 you know, with lots of fat which makes the body 00:08:45.99\00:08:48.62 more acidic, maybe if they can get off that medicine. 00:08:48.66\00:08:51.83 If they're under a lot of stress and worry and concern, 00:08:51.86\00:08:55.66 and that's contributing to their body making a lot of acid, 00:08:55.70\00:08:58.83 maybe that's something they can do. 00:08:58.87\00:09:01.24 So is there a chance they can get off this? Yes 00:09:01.27\00:09:04.01 And sometimes people only have to take the medicines 00:09:04.04\00:09:06.37 for a short period of time until they can get at the cause. 00:09:06.41\00:09:09.64 I have a lot of people, Nick, that will just quit drinking 00:09:09.68\00:09:12.51 caffeine and the acid production 00:09:12.55\00:09:14.05 will come down, they won't need it. 00:09:14.08\00:09:15.52 I have some people that will lose 20 pounds, 00:09:15.58\00:09:17.95 and that will help. Yeah 00:09:17.99\00:09:19.52 I have some people that won't eat big at night, 00:09:19.55\00:09:21.82 and then their acid production will get better. 00:09:21.86\00:09:23.59 I have some people that will quit eating meat, 00:09:23.63\00:09:25.63 and then that will get better! 00:09:25.66\00:09:27.20 So is it possible? Yes... 00:09:27.23\00:09:28.70 But what I've seen is... a lot of people aren't even 00:09:28.73\00:09:31.53 given the options... 00:09:31.57\00:09:34.87 I explained the risks and the benefits, 00:09:35.27\00:09:36.81 and why this is, I think, a good choice for them, 00:09:36.84\00:09:40.14 but some people say, "Nope, I don't want to do it, 00:09:40.18\00:09:42.28 I would rather take the medicine and do what I want." 00:09:42.31\00:09:44.31 Well that is their right and I'm still going to love them 00:09:44.35\00:09:46.85 the best I can and still encourage them 00:09:46.88\00:09:49.78 and try to move them on other steps. 00:09:49.82\00:09:51.59 I'll say, "Well, if you won't get off the medicine, 00:09:51.62\00:09:53.59 maybe you'll drink more water and dilute the acid, 00:09:53.62\00:09:56.29 or maybe you will drink one less coke that day, 00:09:56.32\00:09:59.53 and not have so much phosphoric acid 00:09:59.56\00:10:01.56 which is adding even MORE acid to your body. 00:10:01.60\00:10:03.83 So these are the little things 00:10:03.87\00:10:05.20 that we do that can work with people. 00:10:05.23\00:10:06.70 And I know a common question people ask is: 00:10:06.74\00:10:08.30 "What if I forget to take my medication, 00:10:08.34\00:10:09.84 what should I do?" "Should I double up?" 00:10:09.87\00:10:11.71 Nope! I wouldn't... It's according to what 00:10:11.74\00:10:13.54 the medicine is. Okay 00:10:13.58\00:10:14.94 For instance, if you forget to take your medicine, 00:10:14.98\00:10:17.78 and it's a very important medicine, 00:10:17.81\00:10:19.35 then you want to talk to your doctor and ask them 00:10:19.38\00:10:21.65 what they want you to do. 00:10:21.68\00:10:23.15 If it's a medicine that's not important, 00:10:23.18\00:10:25.02 then maybe you can just miss a dose, 00:10:25.05\00:10:26.39 and then resume the next one. 00:10:26.42\00:10:28.12 For instance, if it's a blood pressure medicine, 00:10:28.16\00:10:29.99 I tell my patients to... "Well, you missed one dose, 00:10:30.03\00:10:33.06 let's just resume with the next." 00:10:33.09\00:10:35.86 If it's a blood thinner medicine, 00:10:35.90\00:10:37.47 and the blood is going to get too thick, 00:10:37.50\00:10:38.93 I'll have them come into the office and check it, 00:10:38.97\00:10:40.74 and see if it's too thick or too thin. Right 00:10:40.77\00:10:42.67 If it's an antibiotic and they are at the end of the infection, 00:10:42.70\00:10:45.84 I might say, "Well, the infection is pretty much over, 00:10:45.87\00:10:47.91 that's okay, but if it's in the beginning of the infection, 00:10:47.94\00:10:51.01 I might say, "Well, you missed a dose - you double up." 00:10:51.05\00:10:53.08 So it really depends on the individual situation... 00:10:53.11\00:10:56.32 to know what they should do if they miss a medication. 00:10:56.35\00:10:58.82 Do you find that happens commonly with your patients? 00:10:58.85\00:11:00.56 Yes, 70% of patients do not take 00:11:00.59\00:11:03.39 the medication as you tell them to. 00:11:03.43\00:11:05.73 Seventy percent - that's a big number. 00:11:05.76\00:11:07.10 Yeah, they either forget the medicine... 00:11:07.13\00:11:09.06 they usually forget it, they don't take it on time, 00:11:09.10\00:11:11.07 they mix and match it with things that interact 00:11:11.10\00:11:13.00 with the medicine... so a large number of people 00:11:13.03\00:11:15.60 don't take the medications as you prescribe it. 00:11:15.64\00:11:17.91 That's why I try to give alternatives for medicines 00:11:17.94\00:11:21.31 and a lot of people make mistakes with their medicines. 00:11:21.34\00:11:24.21 Just like you do... "Oh, I take one and it made me 00:11:24.25\00:11:26.01 feel a little bit better... I'm going to take three."
