Welcome back, on today's program, 00:00:05.83\00:00:07.54 we're talking about pain management, 00:00:07.57\00:00:09.30 and we've received some questions from our website: 00:00:09.34\00:00:11.27 heartwiseministries.org 00:00:11.31\00:00:13.27 And Dr. Marcum, we'd like to get right into those... Okay 00:00:13.31\00:00:16.68 So, the first viewer asks: "I have pains all over, 00:00:16.71\00:00:19.71 how can I tell if pain is something 00:00:19.75\00:00:21.52 I should worry about or not?" 00:00:21.55\00:00:22.88 That is a great question because all of us in life have little 00:00:22.92\00:00:26.72 pains every now and then, and, you know... 00:00:26.76\00:00:29.62 Should I worry about it? Should I go see a doctor? 00:00:29.66\00:00:32.36 Do I try to handle it myself? 00:00:32.39\00:00:34.26 What's the cause of the pain? Right 00:00:34.30\00:00:36.36 Of course, I'm a cardiologist so I see lots of people 00:00:36.40\00:00:39.43 that have chest pain and there are certain patterns of pain 00:00:39.47\00:00:43.74 that alert me that there might be a problem. 00:00:43.77\00:00:47.14 For instance, in the chest... if a pain does not go away, 00:00:47.18\00:00:51.65 lasts prolong periods of time, above the waist, 00:00:51.68\00:00:54.85 that could be the heart attack or something else. 00:00:54.88\00:00:58.02 So that pattern of pain, I would say, if I don't know what it is, 00:00:58.05\00:01:00.99 I wouldn't want to miss a heart attack... Right 00:01:01.02\00:01:02.92 So I would go to the hospital, and have someone 00:01:02.96\00:01:05.13 evaluate you with some EKGs and some blood work. 00:01:05.16\00:01:09.00 And, unfortunately, women... it's very hard for them 00:01:09.03\00:01:12.10 to evaluate pain - sometimes they can feel very atypical. 00:01:12.13\00:01:15.90 So anything above the waist that you can't understand, 00:01:15.94\00:01:19.01 you know, is a problem. 00:01:19.04\00:01:20.68 Pain is not normal, Nick. Right! Okay - pain is not normal. 00:01:20.71\00:01:23.55 If I started taking this and started whacking you, 00:01:23.58\00:01:26.65 you would hurt and you would figure out the pain 00:01:26.68\00:01:28.85 was from me whacking you here. Right 00:01:28.88\00:01:30.75 So if a person can figure out if a trauma caused the pain, 00:01:30.79\00:01:34.62 that's very reasonable. Sure 00:01:34.66\00:01:36.02 It was maybe not dangerous, maybe a trauma, 00:01:36.06\00:01:38.76 the way I was sitting, the back pain - maybe that 00:01:38.79\00:01:41.20 caused it, but above the waist, between the waist and neck, 00:01:41.23\00:01:44.10 if I have pain, that could be very serious, 00:01:44.13\00:01:47.00 including a heart attack. 00:01:47.04\00:01:48.70 The other type of pain that comes from the heart 00:01:48.74\00:01:50.51 is a pain or discomfort when someone is doing something 00:01:50.54\00:01:53.27 because that is when the heart works... 00:01:53.31\00:01:55.34 So if you're going up a hill and it hurts - that's bad. Right 00:01:55.38\00:01:58.38 How about headache pain? 00:01:58.41\00:02:00.58 You know, lots of people have headache pain, 00:02:00.62\00:02:02.35 but if it's a new pain, the worse headache 00:02:02.38\00:02:04.55 you've ever had, that might be 00:02:04.59\00:02:06.09 something you need to look at. 00:02:06.15\00:02:07.62 That might be a serious cause of headache pain 00:02:07.66\00:02:09.79 like an aneurysm or something terrible inside the brain. 00:02:09.82\00:02:13.76 Now sometimes, we just have stress headaches... 00:02:13.80\00:02:15.80 that's not as bad of a pain to worry about. Right 00:02:15.83\00:02:18.67 Pain that tends to go away, 00:02:18.70\00:02:21.80 tends to not be as big of a problem. 00:02:21.84\00:02:24.24 Pain that gets worse over time, tends to be a worse problem. 00:02:24.27\00:02:29.11 So that's a pain to even be more concerned about. 00:02:29.14\00:02:32.61 It seems like people don't worry about 00:02:32.65\00:02:34.98 pain that is far from the brain, you know, pain in the wrist, 00:02:35.02\00:02:39.22 because they know it's not dangerous, 00:02:39.25\00:02:41.19 but pain in the chest, pain when you go to 00:02:41.22\00:02:43.09 the bathroom - that's not a good pain to have. No 00:02:43.12\00:02:45.63 You know, pain that suggests a urinary tract problem. 00:02:45.66\00:02:49.10 Pain when you go to the big commode, 00:02:49.13\00:02:52.87 that should suggest a GI problem. 00:02:52.90\00:02:55.57 Pain when you touch on the bowels, 00:02:55.60\00:02:57.37 that might be a problem too, 00:02:57.41\00:02:58.84 or it might just be an air bubble. 00:02:58.87\00:03:00.51 Also, so pain is something that's not normal, 00:03:00.54\00:03:04.18 that you need to sort of evaluate yourself, 00:03:04.21\00:03:06.48 but if it's not going away, it's worse, 00:03:06.51\00:03:08.68 it doesn't follow a pattern that you know about, 00:03:08.72\00:03:10.95 and it's new - that's when you should get help right away. 00:03:10.99\00:03:14.62 Now chronic pain is a little bit different bird. Okay 00:03:14.66\00:03:18.16 That is pain that will not go away, 00:03:18.19\00:03:21.56 it's been there for a long time. 00:03:21.60\00:03:23.03 Hopefully, it's been evaluated. 00:03:23.06\00:03:25.53 We've talked about the most common cause of chronic pain is 00:03:25.57\00:03:28.14 back pain - sometimes that can be helped quite a bit. 00:03:28.17\00:03:32.57 Sometimes it cannot be helped quite a bit. 00:03:32.61\00:03:35.71 I was reminded, during our break, that one of the best 00:03:35.74\00:03:39.08 things for chronic back pain can be hot and cold, 00:03:39.11\00:03:42.05 alternating it, but there are many other modalities 00:03:42.08\00:03:44.95 to work on chronic pain. 00:03:44.99\00:03:46.32 But I think that so many people are having chronic pain now 00:03:46.35\00:03:49.66 that it's something we're not doing well. Yeah 00:03:49.69\00:03:52.49 And the most common cause of chronic pain is back pain. 00:03:52.53\00:03:56.46 And the most common causes of 00:03:56.50\00:03:58.00 that is - we're not lifting correctly, 00:03:58.03\00:04:00.37 we're not having correct posture, 00:04:00.40\00:04:02.30 we're not exercising well, we have too much weight on, 00:04:02.34\00:04:05.31 and we're trying to do too much. 00:04:05.34\00:04:07.14 All of these things damage our back and once our back 00:04:07.18\00:04:09.94 is damaged, we're not allowing it to heal. 00:04:09.98\00:04:11.88 We're not getting help right away. 00:04:11.91\00:04:13.55 So another thing that's important in chronic pain 00:04:13.58\00:04:15.68 is when you have a new pain, don't ignore it. 00:04:15.72\00:04:18.49 Get some help - don't let it go on and on until it's a 00:04:18.52\00:04:21.52 big problem - it might not be a life-threatening problem now, 00:04:21.56\00:04:25.23 but if it goes on long enough, it could be. 00:04:25.26\00:04:27.76 So have chronic pain evaluated by your doctor. 00:04:27.80\00:04:30.87 And that's some good advice... 00:04:30.90\00:04:32.23 When I was a kid, if we would get hurt, 00:04:32.27\00:04:33.97 we would always try and decide, "is this something we need 00:04:34.00\00:04:35.74 to go to the hospital for or not?" 00:04:35.77\00:04:37.11 But if it's continual pain or if it's unexplainable, or if it's 00:04:37.14\00:04:40.24 getting worse, then you probably ought to get it checked out. 00:04:40.28\00:04:43.35 Yeah, and I've had so many people, through life, 00:04:43.41\00:04:45.51 that they have pain week after week... 00:04:45.55\00:04:47.68 They know it's not dangerous pain, 00:04:47.72\00:04:49.05 but they never do anything about it. Right 00:04:49.08\00:04:50.89 And, before long, it becomes a big problem, 00:04:50.92\00:04:53.96 and then it's much harder to take care of, 00:04:53.99\00:04:57.09 it's much harder to get to the bottom of... 00:04:57.13\00:04:58.86 They hurt their nerves, so it would be a 00:04:58.89\00:05:01.80 chronic issue for them. 00:05:01.83\00:05:04.17 It's just a real hassle, but it's such an 00:05:04.20\00:05:06.10 important topic. 00:05:06.13\00:05:07.74 At our ministry, we sort of track all the different 00:05:07.77\00:05:11.91 callers that come in and chronic pain is one of our 00:05:11.94\00:05:15.44 leading questions that come and it's usually not acute pain. 00:05:15.48\00:05:19.48 People usually know they need to go for help, 00:05:19.51\00:05:21.28 but what I've seen is that one time it is acute pain, 00:05:21.32\00:05:24.19 and they ignore it, and that's when acute pain 00:05:24.22\00:05:26.86 develops into chronic pain. 00:05:26.89\00:05:28.72 Now, we have another question that says: 00:05:28.76\00:05:30.83 "I go to a pain management clinic and the doctor 00:05:30.86\00:05:33.86 wants me to take narcotics, is this a good idea?" 00:05:33.90\00:05:36.90 I think, in my opinion, that narcotics is a last resort. Okay 00:05:36.93\00:05:42.84 And if you do, for whatever reason, have to take a narcotic 00:05:42.87\00:05:45.91 which is a medicine that blocks pain, then you want to take 00:05:45.94\00:05:49.21 as little of it as you can. 00:05:49.24\00:05:51.78 Narcotics over a long haul has side effects of the entire body. 00:05:51.81\00:05:55.88 It makes us not think as good; it depresses our respirations; 00:05:55.92\00:06:00.46 it slows down our bowels; it lowers our blood pressure; 00:06:00.49\00:06:04.93 and what it does is - a little bit of it works for a while, 00:06:04.96\00:06:08.33 but then we get sensitized to it. 00:06:08.36\00:06:10.50 So we need more and more and more to achieve the 00:06:10.53\00:06:13.80 pain effects, but at the same time, 00:06:13.84\00:06:16.14 it's hurting other parts of the body. 00:06:16.17\00:06:18.41 So we build up an immunity to it, it stops working, 00:06:18.47\00:06:21.31 and then it has to be in higher doses which hurts other parts. 00:06:21.34\00:06:24.35 And that's, you know, I wrote a book called: 00:06:24.38\00:06:26.11 "Medicines That Kill," and in my research for the book, 00:06:26.15\00:06:30.29 that was one of the #1 causes 00:06:30.32\00:06:32.35 of medications that kill is narcotics... 00:06:32.39\00:06:35.32 And unfortunately, people use it for pain and some people 00:06:35.36\00:06:38.63 take it and they don't even have pain. 00:06:38.66\00:06:40.30 You know, they don't have that. Right 00:06:40.33\00:06:42.86 And then, we talked about, "Is narcotics a good idea?" 00:06:42.90\00:06:46.47 For mental pain? No, it's not a good idea for mental pain. 00:06:46.50\00:06:49.64 Now, when are narcotics useful? 00:06:49.67\00:06:53.04 I think they're very useful for people that have cancer pain. 00:06:53.07\00:06:57.38 I think people that have cancer pain - cancer is growing 00:06:57.41\00:07:01.22 in the nerves that's causing chronic pain that won't go away, 00:07:01.25\00:07:05.65 whether that be in the brain or other parts of the body... 00:07:05.69\00:07:08.52 That here's nothing else that can be done, 00:07:08.56\00:07:10.23 we can't get at the cause, I think that that case 00:07:10.26\00:07:13.66 the risk of the pain outweighs the risk for the narcotic. Right 00:07:13.70\00:07:17.93 Because when you have chronic pain... 00:07:17.97\00:07:19.47 we talked about all - you don't think good, 00:07:19.50\00:07:21.30 raises the risk of heart disease, 00:07:21.34\00:07:23.10 it weakens your immune system. 00:07:23.14\00:07:24.81 You know, it tears up your body too 00:07:24.84\00:07:26.47 because your body is not made to have chronic pain, 00:07:26.51\00:07:28.88 that's not good either. 00:07:28.91\00:07:30.25 So it's a very good balance and unfortunately, 00:07:30.28\00:07:32.85 not too many people have the right balance... 00:07:32.88\00:07:35.52 And, unfortunately, many people just want the quick fix. 00:07:35.55\00:07:38.75 You know, I want to be out of pain right now! 00:07:38.79\00:07:40.92 I don't want to go through the steps either the 00:07:40.96\00:07:42.62 lifestyle or dealing with it or even resting. 00:07:42.66\00:07:45.49 You know, "I want to keep my busy, fast-paced job. 00:07:45.53\00:07:47.73 Oh, I can't quit work or slow down for a month 00:07:47.76\00:07:50.57 because if I do, something bad is going to happen, 00:07:50.60\00:07:52.90 so just get me through today," 00:07:52.93\00:07:54.54 and they never let their bodies heal. Right 00:07:54.57\00:07:57.11 And there's so much importance from this rest... 00:07:57.14\00:08:00.44 You know, the God of the universe, Nick, 00:08:00.48\00:08:02.24 He worked 6-days and He rested. 00:08:02.28\00:08:04.58 Rest is very important and it's not only the physical rest, 00:08:04.61\00:08:08.88 but the spiritual rest - coming and meeting with God specially, 00:08:08.92\00:08:13.05 and letting Him work in healing you. Yes 00:08:13.09\00:08:15.56 And then there's things that we can do... 00:08:15.59\00:08:17.83 You've heard of "endorphins." Right 00:08:17.86\00:08:19.29 There's natural pain medicines that our body make 00:08:19.33\00:08:22.53 by being happy, by having service. 00:08:22.56\00:08:25.53 There's another pain medicine that helps called, "Oxytocin." 00:08:25.57\00:08:29.67 People with pets and people that love each other 00:08:29.70\00:08:33.27 increase that chemical that can also help with pain. Right 00:08:33.31\00:08:37.15 And now, they are inventing some medications 00:08:37.18\00:08:40.22 that block certain pain receptors. 00:08:40.25\00:08:42.68 That might help if it can block discrete causes of pain, 00:08:42.72\00:08:46.45 and not cause other problems, 00:08:46.49\00:08:48.26 but it doesn't make sense to me just to turn off one receptor 00:08:48.29\00:08:51.13 if we can get at the cause of the pain. Sure 00:08:51.16\00:08:53.46 So these are all really good questions. 00:08:53.50\00:08:55.13 Now, what is a "TENS unit?" 00:08:55.16\00:08:57.40 Yeah, a TENS unit... When a person is diagnosed 00:08:57.43\00:09:01.94 with chronic pain - that means they can't get at the cause, 00:09:01.97\00:09:04.87 that, you know, the pain is going to be there. 00:09:04.91\00:09:06.68 We want to have people have pain relief because 00:09:06.71\00:09:09.84 the risk of the pain is not worth it. 00:09:09.88\00:09:11.28 That's one of the modalities we use. 00:09:11.31\00:09:13.75 It's not a narcotic and there are different types. 00:09:13.78\00:09:16.72 It's sort of a nerve stimulator. 00:09:16.75\00:09:18.62 It stimulates the nerves and it helps them 00:09:18.65\00:09:21.02 maybe not have chronic pain. 00:09:21.06\00:09:22.92 And there are different types that do it. 00:09:22.96\00:09:24.29 It helps with maybe relaxes the muscles 00:09:24.33\00:09:27.03 so the muscle doesn't push on the nerves as much... 00:09:27.06\00:09:29.16 But there are TENS units that we can put topically on us. Okay? 00:09:29.20\00:09:33.00 There are TENS units that can actually be implanted in us. 00:09:33.03\00:09:38.24 That helps quite a bit - people with chronic pain, 00:09:38.27\00:09:40.91 because then they don't have to maybe use narcotics as much. 00:09:40.94\00:09:44.01 Other things we talked about, laughing and having happy things 00:09:44.05\00:09:48.25 happen in service, those help our bodies 00:09:48.28\00:09:50.52 make pain killers, but good posture... 00:09:50.55\00:09:52.42 we talked about water, a healthy diet, 00:09:52.45\00:09:55.59 more of a plant-based, it lowers 00:09:55.62\00:09:57.73 inflammation on the body. 00:09:57.79\00:09:59.29 So treating pain is like treating... 00:09:59.33\00:10:01.20 you know, you want to treat the entire body. 00:10:01.23\00:10:03.40 Do everything that you possibly can do to help the body 00:10:03.43\00:10:06.70 help turn off the pain. Right 00:10:06.74\00:10:09.00 And we have another viewer that asks: 00:10:09.04\00:10:10.74 "What about D-Ribose?" 00:10:10.77\00:10:13.27 D- Ribose... There are lots of different chemicals out there 00:10:13.31\00:10:17.81 that are non-narcotics that help with pain. Okay 00:10:17.85\00:10:20.38 We hear about the anti-inflammatories, 00:10:20.42\00:10:25.32 and we have pain blockers like Tylenol, 00:10:25.35\00:10:27.66 anti-inflammatories, even steroids help with pain 00:10:27.69\00:10:30.49 because it blocks inflammation. 00:10:30.53\00:10:31.86 Well, D-Ribose is another chemical that's been shown 00:10:31.89\00:10:35.23 to help with pain. 00:10:35.26\00:10:36.60 It's usually something that's a natural 00:10:36.63\00:10:39.27 thing that's given that helps. 00:10:39.30\00:10:41.14 There are also creams that help. 00:10:41.17\00:10:42.97 There seems to be some healing power in Aloe vera. 00:10:43.00\00:10:46.68 There are a lot of things out there that have Capsaicin in it. 00:10:46.71\00:10:50.28 Menthol sometimes will help. 00:10:50.31\00:10:51.85 They help these nerves help deal with 00:10:51.88\00:10:54.32 the pains a little bit better... 00:10:54.35\00:10:55.78 This is just one of the many different natural pain relievers 00:10:55.82\00:10:58.69 that are out there that seem to help some people. 00:10:58.72\00:11:01.09 Okay, now here's another question that you kind of 00:11:01.12\00:11:03.12 touched about earlier, but - "Is exercise 00:11:03.16\00:11:05.56 good for chronic pain?" How can exercise help? 00:11:05.59\00:11:08.00 Exercise and movement are very good 00:11:08.03\00:11:10.30 in a couple of different ways. 00:11:10.33\00:11:11.80 One is it helps bring a better blood supply to the area. Okay 00:11:11.83\00:11:17.51 Blood supply is good because within the blood 00:11:17.54\00:11:19.74 comes oxygen and, you know, your healing. 00:11:19.77\00:11:22.88 If you had no blood supply, that causes a lot of pain 00:11:22.91\00:11:28.05 because the nerves don't get the blood it needs 00:11:28.08\00:11:29.92 to function well. Sure 00:11:29.95\00:11:31.29 So nerves need blood to function well. 00:11:31.32\00:11:33.86 Diabetics - you've heard they have a neuropathy, 00:11:33.89\00:11:36.39 they don't get good blood supply to the nerves. 00:11:36.42\00:11:39.06 So the nerves need good blood supply. 00:11:39.09\00:11:40.96 So exercise will help with that. 00:11:41.00\00:11:42.80 Whole food plant-based diet 00:11:42.83\00:11:44.17 will help the nerves get better blood flow. 00:11:44.20\00:11:45.83 It will also improve our posture and our muscles and take 00:11:45.87\00:11:49.20 pressures off that - it also can help us make endorphins, 00:11:49.24\00:11:52.51 and it helps to do a lot of things, 00:11:52.54\00:11:54.11 but we want to do smart exercise. 00:11:54.14\00:11:55.68 If exercise is causing excruciating pain, 00:11:55.71\00:11:59.11 it might not be of benefit. 00:11:59.15\00:12:00.72 You know, massage - that's a very beneficial 00:12:00.75\00:12:03.25 help for pain too because it helps bring extra blood 00:12:03.28\00:12:05.89 into the area and things of that nature. 00:12:05.92\00:12:07.42 That's the whole hot and cold, 00:12:07.46\00:12:09.22 more blood comes in, more blood comes out. 00:12:09.26\00:12:11.23 It brings good things in and it helps our body that way. 00:12:11.26\00:12:14.16 Now, what do doctors do if they 00:12:14.20\00:12:16.80 can't identify the source of the pain? 00:12:16.83\00:12:18.90 Well, what I do is I pray.
