Absolutely 00:00:59.59\00:01:01.52 And we've been talking about mitral valves today, 00:01:01.56\00:01:04.06 and aortic valves and all these valves in our body, 00:01:04.09\00:01:07.30 and a lot of people don't know much about 00:01:07.33\00:01:09.40 these heart valves, but it's something that's important 00:01:09.43\00:01:11.97 to know because I have people that come in and say, 00:01:12.00\00:01:14.14 "My doctor says I have a heart murmur," 00:01:14.17\00:01:15.94 and they're scared to death. Yes 00:01:15.97\00:01:17.91 And really, all a heart murmur is 00:01:17.94\00:01:19.97 is when you listen, you hear abnormal flow. 00:01:20.01\00:01:24.05 You hear that swishing sound you're talking about. 00:01:24.08\00:01:25.71 Yes, you can have a heart murmur 00:01:25.75\00:01:27.08 and have a completely normal heart valve. 00:01:27.12\00:01:30.02 If a person is dehydrated, sometimes you hear 00:01:30.05\00:01:32.62 an innocent murmur. 00:01:32.65\00:01:33.99 When women are pregnant, their hearts change 00:01:34.02\00:01:37.13 because it has to work so hard. 00:01:37.16\00:01:38.69 We almost always hear murmurs in pregnant women, 00:01:38.73\00:01:41.20 but just because you hear that you have a murmur, 00:01:41.23\00:01:44.23 that doesn't necessarily mean it's dangerous. 00:01:44.27\00:01:46.97 And a lot of people come to us because either their doctor 00:01:47.00\00:01:50.61 has heard a murmur... 00:01:50.64\00:01:52.44 The most serious condition is they have a symptom like 00:01:52.47\00:01:55.54 shortness of breath, can't breathe when they do things, 00:01:55.58\00:01:58.41 and then their doctor hears a murmur and then they come to us 00:01:58.45\00:02:00.98 and then we find out, "Oh yeah, you do have a murmur 00:02:01.02\00:02:02.95 that's correlating with your symptom." 00:02:02.98\00:02:04.82 We find out that the valve is not doing well, 00:02:04.85\00:02:07.29 and then we talk about all of our options to the patient. 00:02:07.32\00:02:09.66 So the murmur and symptom together 00:02:09.69\00:02:11.36 is when you know there's a problem. 00:02:11.39\00:02:13.13 Yes, if you're having a heart murmur, 00:02:13.16\00:02:14.86 and you're having symptoms, especially shortness of breath, 00:02:14.93\00:02:17.50 extreme fatigue - that might be a sign that you 00:02:17.53\00:02:20.84 might need a little more help on the valves. 00:02:20.87\00:02:22.60 All right - here's another question from 00:02:22.64\00:02:24.64 heartwiseministries.org 00:02:24.67\00:02:26.44 It says, "What is "endocarditis" 00:02:26.47\00:02:28.78 and how is it diagnosed?" It sounds kind of serious! 00:02:28.81\00:02:31.85 Yeah, endocarditis just means inflammation 00:02:31.91\00:02:34.92 of the endocardium of the heart. 00:02:34.95\00:02:37.02 Specifically when it comes to valve disease, 00:02:37.05\00:02:39.62 a lot of people pick up infections. 00:02:39.65\00:02:42.46 Let's say you cut yourself here or somewhere, 00:02:42.49\00:02:46.29 and you didn't know it, and diabetics have lots of 00:02:46.33\00:02:48.76 infections in their feet that they can't even feel their feet 00:02:48.80\00:02:51.17 because they have nerves that don't feel well. 00:02:51.20\00:02:53.17 So let's say you get an infection and you don't 00:02:53.20\00:02:55.00 even know it, and sometimes mouth infections, 00:02:55.04\00:02:57.87 the infection has been there a long time, 00:02:57.91\00:02:59.51 you don't really have it treated. 00:02:59.54\00:03:01.14 But bacteria grow there, staph grows there, 00:03:01.18\00:03:05.01 strep grows there, some type of bacteria, 00:03:05.05\00:03:07.42 and it gains access to the bloodstream. 00:03:07.45\00:03:10.29 Well first, usually a person gets pretty sick, 00:03:10.32\00:03:12.42 but those bacteria are circulating through the blood 00:03:12.45\00:03:15.06 going everywhere and it happens to see a heart valve, 00:03:15.09\00:03:18.49 and says, "Hmm, this is a nice place to land, 00:03:18.53\00:03:21.36 I think I'm going to stick here and, boy, I have a little 00:03:21.40\00:03:24.07 food source too that's constantly moving by me," 00:03:24.10\00:03:27.50 especially if it sees a heart valve that's been damaged. 00:03:27.54\00:03:30.51 It says, "Well, I'm just going to make a home here." 00:03:30.54\00:03:33.01 So he grabs onto the valve, and then all of a sudden, 00:03:33.04\00:03:35.84 he is holding onto the valve, getting some food to eat, 00:03:35.88\00:03:37.61 and says, "Well, let me grow a little bit." 00:03:37.65\00:03:39.11 So he grows a little family and before you know it, 00:03:39.15\00:03:41.62 there's an infection on that valve and actually the bacteria 00:03:41.65\00:03:45.65 are actually eating up the valve. 00:03:45.69\00:03:48.06 Physically eating the valve. Yeah 00:03:48.09\00:03:49.52 And we can have endocarditis bacteria on a heart valve 00:03:49.56\00:03:53.23 that destroys the heart valve, 00:03:53.26\00:03:54.73 but it's usually from some infection. 00:03:54.76\00:03:56.40 Now the common ones we also get is from people that use drugs. 00:03:56.43\00:04:00.37 You know, they take the IV syringe, that's dirty, 00:04:00.40\00:04:03.24 and has bacteria and then and inject, 00:04:03.27\00:04:06.14 and that infection, of course, multiplies, 00:04:06.17\00:04:08.94 and it has a tendency to get on heart valves too. 00:04:08.98\00:04:11.55 So when it gets on a heart valve, 00:04:11.58\00:04:12.95 we call that "endocarditis," 00:04:12.98\00:04:15.05 and sometimes if the valve is not leaking too bad, 00:04:15.08\00:04:18.32 we can destroy it with IV antibiotics, 00:04:18.35\00:04:21.56 but sometimes there's so much damage to the heart valve 00:04:21.59\00:04:24.79 that we have to replace the valve. 00:04:24.83\00:04:26.46 In fact, one of my patients that I've had for about 20 years now, 00:04:26.49\00:04:30.03 when he was in his 20s, he used to do a lot of drugs. 00:04:30.07\00:04:32.83 He used to do heroin and he actually got endocarditis 00:04:32.87\00:04:36.57 on both heart valves and he comes in really 00:04:36.60\00:04:39.51 short of breath with fever. 00:04:39.54\00:04:41.54 The blood cultures showed that he had bacteria in his blood. 00:04:41.58\00:04:44.61 We looked at his heart valves and sure enough 00:04:44.65\00:04:46.58 there was a big old vegetation on that heart valve. 00:04:46.61\00:04:49.68 The valve was leaking terribly, so we had to settle him 00:04:49.72\00:04:52.79 down with antibiotics and then the surgeon put 00:04:52.82\00:04:55.42 2 metal valves in him and he's still doing okay today, 00:04:55.46\00:04:58.39 and thank goodness he gave up the drugs. Oh my, yes. 00:04:58.43\00:05:01.00 But he has to take blood thinners the rest 00:05:01.03\00:05:02.86 of his life now, but that's a very common type of 00:05:02.90\00:05:06.20 valve problem and we call that "endocarditis." 00:05:06.23\00:05:08.40 But any infection that a person has on his body, 00:05:08.44\00:05:11.07 if the infection has access to the bloodstream, 00:05:11.11\00:05:13.94 it could make you very sick quickly. 00:05:13.98\00:05:16.28 Now the fancy term when it's just in the blood is called, 00:05:16.31\00:05:19.05 "bacteremia," but if it gets on a heart valve, 00:05:19.08\00:05:23.28 you know that's bacteremia that has formed endocarditis, 00:05:23.32\00:05:26.49 and that is potentially a life-threatening infection. 00:05:26.52\00:05:29.69 Now you mentioned, during the break - you and I were talking 00:05:29.72\00:05:31.99 and you said that they can do these valve replacements 00:05:32.03\00:05:34.43 without opening the heart? 00:05:34.46\00:05:35.86 They don't have to go down through the chest? 00:05:35.90\00:05:37.23 Yeah and that's new technology we've had especially for the 00:05:37.27\00:05:41.17 aortic valve. Yes 00:05:41.20\00:05:42.67 Nowadays, they call it "TAVR" transaortic valve replacement. 00:05:42.70\00:05:48.08 And they can actually go through vessels in the groin, 00:05:48.11\00:05:50.95 and we can put catheters with an artificial valve... 00:05:50.98\00:05:54.78 and while the person is not knocked out, 00:05:54.82\00:05:57.89 they can actually deploy that valve in the aortic position. 00:05:57.92\00:06:01.99 Now we mainly use it for people that are older in age, 00:06:02.02\00:06:05.36 that might not be a good surgical risk 00:06:05.39\00:06:07.60 or who have a lot of other medical problems, 00:06:07.63\00:06:09.50 and only have one valve problem. 00:06:09.53\00:06:11.13 But so far, we've been doing it for a few years now, 00:06:11.17\00:06:14.60 the results look very promising and every year as we've done it, 00:06:14.64\00:06:18.01 we've refined the procedure so it's a lot less risky. 00:06:18.04\00:06:21.04 No one, when they're older, 80 or 90 years old, 00:06:21.08\00:06:23.48 you don't want to have a big incision, 00:06:23.51\00:06:25.25 and pull the chest open and put a valve in. 00:06:25.28\00:06:27.68 But do they have to stop the heart for this? 00:06:27.72\00:06:29.28 No they don't. They don't even stop the heart? 00:06:29.32\00:06:30.82 The heart is going and they sort of just like exchange 00:06:30.89\00:06:33.59 one valve for another within the blood vessels, 00:06:33.62\00:06:35.76 and that is a remarkable use of technology. 00:06:35.79\00:06:39.46 But most valves, Charles, you know, most of them 00:06:39.49\00:06:41.53 they can put in metallic valves. 00:06:41.56\00:06:44.80 They can put a metallic valve here, here, 00:06:44.83\00:06:46.60 and through the years, there have been different 00:06:46.63\00:06:47.97 companies that do that. 00:06:48.00\00:06:49.34 We also can get replacement valves from pigs and cows, 00:06:49.37\00:06:53.38 and sometimes the aortic root is also damaged and what get 00:06:53.41\00:06:58.05 what we call an "allograft." 00:06:58.08\00:06:59.45 We get not only the valve, but the whole original part 00:06:59.48\00:07:02.62 of the aorta and replace the aorta and valve. 00:07:02.65\00:07:05.29 So a whole section they do. Yes, a whole section. 00:07:05.32\00:07:07.06 We can take the old section out, put in a new section, 00:07:07.09\00:07:09.89 sew it in and "boom," they have a valve. 00:07:09.92\00:07:12.99 Modern technology, modern medicine. 00:07:13.03\00:07:14.36 Yeah and that's a great place for modern medicine... 00:07:14.40\00:07:16.70 It is if you have an infection of the heart; 00:07:16.73\00:07:18.60 if you have the leaky heart valves from whatever reason 00:07:18.63\00:07:21.57 or you have a genetic problem. 00:07:21.60\00:07:23.37 Now most of the valves I see now are the aortic valve 00:07:23.41\00:07:26.24 that doesn't open - that's aortic stenosis and it's 00:07:26.27\00:07:29.24 usually caused by either a bicuspid genetic aortic valve, 00:07:29.28\00:07:32.81 or it's caused by the aging process and we see that a lot. 00:07:32.85\00:07:36.65 A person would say, "I'm short of breath," 00:07:36.69\00:07:39.22 the doctor would hear a murmur, would lead to this 00:07:39.25\00:07:41.19 echocardiogram and they would say, "Yes, there's too much 00:07:41.22\00:07:43.96 pressure in your heart, the valve is not opening well, 00:07:43.99\00:07:46.90 we can help you out with that." 00:07:46.93\00:07:48.33 Now you've mentioned several of the symptoms of valve 00:07:48.36\00:07:51.63 abnormalities, are there some other symptoms 00:07:51.67\00:07:53.94 we should be aware of that you haven't talked about yet? 00:07:53.97\00:07:55.84 Well, you know, I guess the main ones is because the 00:07:55.87\00:07:59.41 valve directs blood flow... you know, you want blood 00:07:59.44\00:08:01.94 heading in the right direction. 00:08:01.98\00:08:03.45 You don't want it leaking backwards, 00:08:03.48\00:08:04.91 and also you want blood getting out of the heart. Yeah 00:08:04.95\00:08:07.28 So either - you know to make this simple, 00:08:07.32\00:08:09.02 the problem with the valve is either it doesn't open... 00:08:09.05\00:08:11.59 that's called "stenosis," or it's leaky... 00:08:11.62\00:08:14.79 that's called "regurgitation." 00:08:14.82\00:08:16.96 So we have aortic valve stenosis and regurgitation, 00:08:16.99\00:08:20.00 we also have tricuspid valve - that's the valve that 00:08:20.03\00:08:22.53 is on the right side of the heart, 00:08:22.56\00:08:24.30 and we also have mitral valve regurgitation and stenosis. 00:08:24.33\00:08:27.80 The most common cause of mitral stenosis is probably 00:08:27.84\00:08:30.74 rheumatic heart disease where an infection damages the 00:08:30.77\00:08:34.11 valve and the valve doesn't open well. 00:08:34.14\00:08:36.41 And all of these would cause symptoms of shortness of breath 00:08:36.44\00:08:39.31 when you're doing things. 00:08:39.35\00:08:40.68 It can also make the heart weak which could cause 00:08:40.72\00:08:43.08 fatigue and tiredness. 00:08:43.12\00:08:44.45 If the heart went into failure, 00:08:44.49\00:08:46.19 you might get fluid in your lungs, 00:08:46.22\00:08:48.19 not be able to lay down flat. 00:08:48.22\00:08:49.79 You might have legs that were swollen up or even 00:08:49.82\00:08:52.29 a belly that was swollen up. 00:08:52.33\00:08:53.73 So those type of symptoms, you would go to a doctor 00:08:53.76\00:08:56.97 and he would say, "Well, let me order an echocardiogram 00:08:57.00\00:08:58.87 to look at the valves," and then he would find out what it is, 00:08:58.90\00:09:01.67 and he'd say, "Well here are our options." 00:09:01.70\00:09:03.77 Usually medications helps with blood pressure, 00:09:03.81\00:09:06.84 and it might help with an infection, 00:09:06.88\00:09:08.21 but usually when a heart valve gets really, really damaged, 00:09:08.24\00:09:10.91 it's time to either replace it or repair it. 00:09:10.95\00:09:14.08 Some skilled surgeons can actually repair valves, 00:09:14.12\00:09:16.69 and usually the ones they repair is the mitral valve 00:09:16.72\00:09:19.49 that prolapses - they can fix that prolap. 00:09:19.52\00:09:22.02 They work on a section called "the PT" and they do an 00:09:22.06\00:09:24.46 angular resection - it's a very elaborate surgery that's done, 00:09:24.49\00:09:27.70 but the skill that those surgeons have is just amazing. 00:09:27.73\00:09:31.03 I mean, I've heard of heart surgery on 00:09:31.07\00:09:33.44 babies in the mother. Yes 00:09:33.47\00:09:34.80 They open up the mother, they open up the baby 00:09:34.84\00:09:36.87 do heart surgery, close up everything and then 00:09:36.91\00:09:38.74 3 months later, they're born and everything is fine. 00:09:38.77\00:09:40.58 Yeah and that's a wonderful place for modern technology, 00:09:40.61\00:09:43.91 you know, to deal with these genetic defects, 00:09:43.95\00:09:46.01 these congenital defects in babies that now they can live 00:09:46.05\00:09:49.35 a lot healthier lives. 00:09:49.38\00:09:50.85 And just the bicuspid valves... you know, 200 years ago, 00:09:50.89\00:09:53.82 there was no treatment for that. 00:09:53.86\00:09:55.19 The men that had that and the women that had that 00:09:55.22\00:09:57.33 would die in their 40s and 50s and now they can 00:09:57.36\00:09:59.69 live a lot longer than that just because we have 00:09:59.73\00:10:01.70 the technology to replace that valve. 00:10:01.73\00:10:03.67 You know, one of the most amazing things as I was looking 00:10:03.70\00:10:05.73 at your little model here and the way the valves are designed, 00:10:05.77\00:10:10.14 I want to share something with you... 00:10:10.17\00:10:11.51 Make a valve over here so we can see it, okay. 00:10:11.54\00:10:14.28 Here's the valve, now lift it a little bit so we 00:10:14.31\00:10:16.78 can see it on the camera, a little higher, there we go. 00:10:16.81\00:10:18.98 There's the valve... On the C and O Canal, 00:10:19.01\00:10:21.68 my wife and I go bicycle riding on the C and O Canal a lot. 00:10:21.72\00:10:24.85 This is exactly the way that the locks are designed. 00:10:24.89\00:10:30.56 On the C and O... when the water comes this way, 00:10:30.59\00:10:33.33 it comes on to a bridge like this, on to a valve like this, 00:10:33.36\00:10:37.30 and because of the way it's designed, it won't open. 00:10:37.33\00:10:39.33 The pressure of the water coming down actually keeps it closed. 00:10:39.37\00:10:42.54 If you go this way, you'll open it up easily, 00:10:42.57\00:10:45.17 but this way, it will not let the water come out. 00:10:45.21\00:10:47.74 So it's just like modern science is using biblical science 00:10:47.78\00:10:52.95 to design the locks in the C and O Canal. That's amazing! 00:10:52.98\00:10:55.65 And what's interesting is if a person does need a heart 00:10:56.48\00:10:59.15 valve and you're out there to talk about it, the surgeon 00:10:59.19\00:11:01.79 will help guide you to the best type of valve. 00:11:01.82\00:11:04.69 And if a person is an older person, sometimes we put in 00:11:04.73\00:11:07.70 a bioprosthetic valve so they don't have to take Coumadin, 00:11:07.73\00:11:11.30 whereas a younger person that the valve has to last many, 00:11:11.33\00:11:14.07 many years, the bioprosthetic valves tend to wear out too 00:11:14.10\00:11:17.61 because they're tissue. 00:11:17.64\00:11:18.97 But the metal valves don't wear out, so the surgeon would 00:11:19.01\00:11:21.04 help guide you through the correct valve to use. 00:11:21.08\00:11:25.01 And, of course, a lot of these are genetics, 00:11:25.05\00:11:27.32 but the things that keep valves healthy are the things 00:11:27.35\00:11:29.68 that keeps everyone healthy... exercise, eating the right foods 00:11:29.72\00:11:34.42 ... those type of things will keep the valves healthy. 00:11:34.46\00:11:36.39 But sometimes an infection comes in that's unexplained 00:11:36.42\00:11:39.39 and causes endocarditis or rheumatic fever or something 00:11:39.43\00:11:43.10 bad happens - you know, genetics are passed on 00:11:43.13\00:11:45.17 and they are born with these valves. 00:11:45.20\00:11:46.53 So this is something that modern medicine is 00:11:46.57\00:11:48.17 great and we just wanted everyone to understand 00:11:48.20\00:11:50.37 a little bit about valves today. 00:11:50.41\00:11:52.07 Well very good, the take-home message I'm getting 00:11:52.11\00:11:54.18 from this is if you have a heart murmur and then you 00:11:54.21\00:11:56.98 combine it with symptoms, 00:11:57.01\00:11:58.35 then you have something to worry about 00:11:58.38\00:11:59.95 because you can live with a murmur for a long time 00:11:59.98\00:12:01.35 without the symptoms.
