Narrator: Adventist World Aviation has mission outposts 00:00:01.43\00:00:04.43 stationed all around the world with the mission of brining 00:00:04.47\00:00:08.74 humanitarian aid and the love of Jesus, 00:00:08.77\00:00:11.37 too difficult to reach areas. 00:00:11.41\00:00:13.98 Often the missions keep very busy flying med-evac flights, 00:00:15.14\00:00:18.81 delivering critically ill patients from remote jungles 00:00:19.11\00:00:22.48 to a place where they can receive a higher level of care. 00:00:22.52\00:00:25.92 These flights are not only lifesaving, but they share 00:00:26.92\00:00:31.59 the love of Jesus and bring hope to the hopeless. 00:00:31.63\00:00:35.33 Jud: Nicaragua, is a very unique project as are most of 00:00:36.53\00:00:40.44 AWA projects. 00:00:40.47\00:00:41.80 Projects, they are in areas, where access is limited. 00:00:41.84\00:00:45.71 They are in places where they are serving people, 00:00:45.74\00:00:47.68 who have unique needs for access to medicine. 00:00:47.71\00:00:53.52 Have unique needs as far as transportation. 00:00:53.55\00:00:55.88 And the airplane is able to provide that service to those 00:00:55.92\00:00:59.55 people in a very special way. 00:00:59.59\00:01:01.72 It has a huge impact on the lives of the people 00:01:02.32\00:01:05.33 living in these areas. 00:01:05.36\00:01:06.70 You see this project meets the needs of the Miskito people 00:01:06.80\00:01:10.73 in a wide variety of ways. 00:01:10.77\00:01:12.60 Narrator: One of Adventist World Aviation's outposts is situated 00:01:58.91\00:02:02.85 in the northeastern corner of Nicaragua. 00:02:02.88\00:02:05.32 Here Clint Hanley and his family dedicate their lives 00:02:05.95\00:02:09.49 to serving the local Miskito Indians. 00:02:09.52\00:02:12.26 Clint: I'm Clint Hanley and I live in northeast Nicaragua, 00:02:12.79\00:02:17.43 in the upper corner along the boarder of the Rio Cocoa 00:02:17.47\00:02:20.47 with the border of Honduras and I live here with my family, 00:02:21.37\00:02:27.11 2 children and my wife, who is a nurse 00:02:27.14\00:02:29.08 and we fly this airplane, a Cessna 172. 00:02:29.11\00:02:33.42 For medical work all around this region, 00:02:33.45\00:02:38.09 covering about 150 miles in all directions. 00:02:38.12\00:02:42.52 Narrator: Clint and his wife Marilyn live in Tronquera, 00:02:42.92\00:02:45.63 Nicaragua on a mission base, that they constructed themselves 00:02:45.66\00:02:49.36 over 10 years ago. 00:02:49.40\00:02:51.27 In the beginning, there was no infrastructure to support 00:02:52.30\00:02:55.04 any communications, or medical evacuations. 00:02:55.07\00:02:58.44 After a few years of hard work, the Hanley's 00:02:59.44\00:03:02.64 procured an air plane, built a runway themselves 00:03:02.68\00:03:05.91 and successfully implemented the only humanitarian aid 00:03:05.95\00:03:09.98 aviation service in the country. 00:03:10.02\00:03:12.75 This airplane is the first and only aircraft that flies medical 00:03:13.56\00:03:16.99 evacuations from remote villages and delivers patients 00:03:17.03\00:03:20.93 to a larger town called Puerto Cabezas. 00:03:20.96\00:03:24.10 Clint: Since we started flying 7 years ago, I have flown 530 00:03:24.63\00:03:28.30 patients from remote communities or from hospital to hospital 00:03:28.34\00:03:32.07 and approximately 50-80% of those would have, 00:03:32.11\00:03:37.25 the doctors say, would have died for sure, 00:03:37.28\00:03:39.45 had they not had transport. 00:03:39.48\00:03:41.18 The rest of them needed transport, because they were 00:03:41.22\00:03:44.92 in great pain, they had broken femurs 00:03:44.95\00:03:47.46 or something like that. 00:03:47.49\00:03:48.82 That if they would have transported by road it would 00:03:48.86\00:03:50.89 have been a lot worse for them, but they probably 00:03:50.93\00:03:52.99 wouldn't have died. 00:03:53.03\00:03:54.36 So it's been a big help to the people and we're happy 00:03:54.40\00:03:57.37 to be a small part of it. 00:03:57.40\00:03:58.77 Before we came, the people got to the hospital by boats, 00:03:58.80\00:04:01.44 by road, by truck. 00:04:01.47\00:04:02.84 To give you an example, to fly from here to San Carlos 00:04:02.87\00:04:06.74 is 35 minutes for me, to get them to the hospital 00:04:06.78\00:04:10.21 from the time I get the call, until they actually arrive 00:04:10.25\00:04:13.31 in the hospital is approximately one and a half hours. 00:04:13.35\00:04:15.88 If they were to take a boat, there's no roads so they would 00:04:15.92\00:04:19.72 have to take a boat, there's an average of one boat a day, 00:04:19.75\00:04:23.02 but sometimes there will be a day or two or three, that go by 00:04:23.06\00:04:25.36 that there's no boats. They're random. 00:04:25.39\00:04:26.80 And the boats take 12 hours. And usually they leave 00:04:26.83\00:04:30.13 in the morning. 00:04:30.17\00:04:31.50 So if the accident happens later in the day, they're having 00:04:31.53\00:04:34.30 to wait through the night, until the next morning and then hoping 00:04:34.34\00:04:36.74 that they can find a boat with room in it, then they ride that 00:04:36.77\00:04:38.97 boat 12 hours to get to the hospital. 00:04:39.01\00:04:40.74 That's fairly average for a lot of the patients that need 00:04:40.78\00:04:43.98 to come to the hospital. 00:04:44.01\00:04:45.35 So you can see where if it's a snake bite, a lot of the people 00:04:46.21\00:04:49.18 we carry out pregnant mothers and babies and either 00:04:49.22\00:04:52.85 the pregnancy has a problem, or the mothers has had the baby 00:04:52.89\00:04:55.36 right away or there's a complication 00:04:55.39\00:04:56.89 between the mother and the baby. 00:04:56.93\00:04:58.26 About 70% of our transports are related to that. 00:04:58.29\00:05:01.96 Narrator: It took many years of working with government 00:05:02.00\00:05:05.07 officials, but Clint was finally able to win their trust. 00:05:05.10\00:05:08.57 Clint: There is none, none other available in all of Nicaragua 00:05:34.33\00:05:37.70 doing this. 00:05:37.73\00:05:39.07 And so it's a vital piece of help that the government 00:05:39.10\00:05:43.47 and no other aid agency was able to offer to the people 00:05:43.51\00:05:47.08 to save lives in transport. 00:05:47.11\00:05:48.51 Narrator: Clint is now well known amongst the natives 00:05:50.15\00:05:52.35 of this region, everyone knows that they can call on him 00:05:52.38\00:05:56.15 when they are in need, or have an emergency. 00:05:56.18\00:05:59.45 Relationships like these aren't easy to develop but the Hanley's 00:06:00.82\00:06:05.03 have lived here for 10 years, and the natives have come 00:06:05.06\00:06:08.66 to accept the missionaries, as their own Miskito people. 00:06:08.70\00:06:12.87 Travel in this region of Nicaragua is very difficult. 00:06:14.87\00:06:17.87 This region is covered with thick lush jungles 00:06:17.91\00:06:22.04 and winding rivers. 00:06:22.08\00:06:23.91 Road systems were never fully developed here. 00:06:24.81\00:06:27.62 This makes transportation tough. Natives travel in large 00:06:27.65\00:06:32.15 over packed buses or by river. 00:06:32.19\00:06:35.26 A trip that would take a 1.5 hour flight, would take 00:06:36.26\00:06:39.39 2 or 3 days, if they were to travel by land. 00:06:39.43\00:06:43.03 Often land travel isn't possible simply because there are 00:06:43.06\00:06:46.87 no road systems to reach many of the remote areas 00:06:46.90\00:06:50.11 These Miskito Indians live in isolation. 00:06:51.11\00:06:53.98 When an emergency occurs, many don't survive mild injuries 00:06:54.98\00:06:59.81 simply because they cannot reach medical care. 00:06:59.85\00:07:03.35 In this part of Central America, passengers often ride on top 00:07:06.05\00:07:10.43 of a fully loaded cargo bus, which is a common yet dangerous 00:07:10.46\00:07:15.23 mode of transportation. 00:07:15.26\00:07:16.77 Today, Clint gets word that one of these cargo buses overturned 00:07:17.80\00:07:21.77 in a ditch and many of its passengers were badly wounded, 00:07:21.80\00:07:26.37 some killed. 00:07:26.41\00:07:27.74 Clint: So it's Dr Zunigate in Waspam, and he has a child 00:07:27.81\00:07:33.18 with some brain trouble. 00:07:33.21\00:07:34.98 And they need to go to Managua. 00:07:35.62\00:07:37.32 So what I do in this case is transfer him from the hospital 00:07:37.35\00:07:40.42 in Waspam to Puerto Cabezas. 00:07:40.46\00:07:42.72 Narrator: Clint immediately sprints into action and travels 00:07:42.76\00:07:45.73 by car the hour journey to the nearest medical facility. 00:07:45.76\00:07:50.00 This hospital has very few supplies and they have no way 00:07:50.03\00:07:53.60 of providing care for critically injured individuals. 00:07:53.64\00:07:57.01 This medical facility can treat simple wounds, but they have 00:07:58.01\00:08:01.98 no way of helping the critically injured. 00:08:02.01\00:08:04.31 Many of the workers at the hospital know Clint well. 00:08:05.48\00:08:08.45 He flies med-evacs for them almost on a weekly basis. 00:08:08.48\00:08:12.22 Clint asks to see the worst patient, so he can arrange 00:08:13.05\00:08:16.22 transport to Puerto Cabezas. 00:08:16.26\00:08:18.76 Clint: And the person that we're going to transport tomorrow, 00:08:19.76\00:08:21.56 we're going to take, they said they have a fracture 00:08:21.60\00:08:24.37 of their skull, and something in their, microcephaly or something 00:08:24.40\00:08:28.80 I don't know exactly what it is. 00:08:28.84\00:08:30.37 But they say that they have stabilized him, but they need 00:08:30.41\00:08:33.44 transport into Managua for surgery. 00:08:33.48\00:08:35.24 And they can't be transported by road because 00:08:35.28\00:08:37.21 they are too fragile. 00:08:37.35\00:08:38.68 So they can't, that there is no plane leaving Puerto Cabezas 00:08:38.71\00:08:42.15 until tomorrow, so they have stabilized him and they are 00:08:42.18\00:08:45.19 keeping him here until they are ready to meet 00:08:45.22\00:08:47.32 the airplane in Port. 00:08:47.36\00:08:49.02 Narrator: The workers inform Clint that there is a 00:08:50.23\00:08:53.43 2 year old boy, who has a skull fracture and they suspect 00:08:53.46\00:08:58.17 that he has a concussion. 00:08:58.20\00:09:00.30 He drifts in and out of consciousness. 00:09:00.34\00:09:03.30 Clint talks to his nurse and tries to get more information 00:09:04.31\00:09:06.98 about the boy. 00:09:07.01\00:09:08.34 Clint: Yesterday there was an accident in a truck 00:09:11.88\00:09:13.82 that rolled over. 00:09:13.85\00:09:15.18 The truck came from a small town and the truck rolled. 00:09:17.19\00:09:21.36 And it was a very bad accident, and that's why we are 00:09:24.33\00:09:27.56 transferring him. 00:09:27.60\00:09:28.93 And so they need a specialist in Port to be able 00:09:28.96\00:09:31.53 to take care of him. 00:09:31.57\00:09:32.90 He has a concussion. They suspect that there is 00:09:32.93\00:09:34.64 a fractured skull. 00:09:34.67\00:09:36.00 And there is no specialist brain surgeons anywhere on this coast, 00:09:36.04\00:09:40.11 the Atlantic coast to be able to check. 00:09:40.14\00:09:41.71 So he has to go to Managua to have specialty brain surgery 00:09:41.74\00:09:44.45 to check why he's having these convolutions and problems. 00:09:44.48\00:09:48.18 That's why his arms are tied, because he's having convolutions 00:09:48.22\00:09:50.52 and problems with that. 00:09:50.55\00:09:51.89 Narrator: The hospital in Puerto Cabezas, where Clint usually 00:09:52.25\00:09:55.19 takes his patients is not equipped to handle 00:09:55.22\00:09:58.03 an injury of this severity. 00:09:58.06\00:09:59.69 Clint and his medical staff make arrangements to have this 00:10:00.50\00:10:04.03 patient flown to the capital city of Managua. 00:10:04.07\00:10:07.04 This is unfortunate news because it means they will not be able 00:10:08.57\00:10:12.17 to make the trip today. 00:10:12.21\00:10:13.88 They must wait until tomorrow to fly this patient 00:10:14.48\00:10:17.05 to receive the care he so desperately needs. 00:10:17.35\00:10:20.72 Early the next morning, Clint prepares to leave and collect 00:10:22.58\00:10:26.35 his awaiting patient, a young 2 year old boy, who was crushed 00:10:26.39\00:10:31.29 under an over turned cargo bus. 00:10:31.33\00:10:33.70 Before he can take off, he must first inspect his aircraft 00:10:34.70\00:10:38.10 and do a preflight inspection. 00:10:38.13\00:10:40.10 This procedure requires Clint to look over every part 00:10:40.94\00:10:44.47 of the aircraft, as well check the oil and fuel. 00:10:44.51\00:10:48.11 It's important to make sure everything is fully operational 00:10:48.71\00:10:51.48 before they begin any flight. 00:10:51.51\00:10:53.11 Ones the plane is cleared for flight, Clint taxies from the 00:10:53.72\00:10:57.25 hangar to the runway. 00:10:57.29\00:10:58.75 Clint and his family cleared and constructed this runway 00:10:59.55\00:11:02.66 8 years ago. 00:11:02.69\00:11:04.09 Having the hangar and runway just steps away from the Hanley 00:11:04.39\00:11:07.76 home, expedites the med-evac process. 00:11:07.80\00:11:10.97 Clint now does a run-up to to make sure the engine 00:11:12.00\00:11:14.87 and avionics are all running properly. 00:11:14.90\00:11:17.11 He has an electronic pre-flight checklist, that he goes through, 00:11:17.51\00:11:20.84 before he makes each flight. 00:11:20.88\00:11:22.48 Safety and protocol are extremely important, 00:11:22.98\00:11:26.18 when it comes to aviation. 00:11:26.21\00:11:28.02 Clint takes no short cuts, when it comes to safety. 00:11:28.82\00:11:32.99 Pressure is good. Charging. Suctions good. Set to go. 00:11:40.36\00:11:45.20 Narrator: Everything seems to be in order and Clint is ready 00:11:47.60\00:11:50.74 to take off and head to Waspam to pick up his very sick patient 00:11:50.77\00:11:55.61 Brendell helps his father by clearing the runway. 00:11:56.11\00:11:58.81 Today, there are a few wild horses that wondered 00:11:59.41\00:12:02.02 onto the runway to graze. 00:12:02.05\00:12:04.09 Well, once the runway is cleared Clint guides this plane down 00:12:10.26\00:12:13.83 the gravel runway and races off into the skies. 00:12:13.86\00:12:17.87 It's a short flight to Waspam. The skies look promising today. 00:12:31.98\00:12:36.58 This is a familiar journey for Clint; many of his patients 00:12:36.99\00:12:40.36 must be picked up from Waspam and taken to Puerto Cabezas. 00:12:40.39\00:12:44.99 Narrator: Today's flight is the only option of transportation 00:14:16.05\00:14:18.99 for this young boy. 00:14:19.02\00:14:20.36 If he were to travel by road, he would likely have to travel 00:14:20.59\00:14:24.56 in a similar bus that he had his accident in. 00:14:24.59\00:14:27.70 Additionally, the roads are terribly bad, 00:14:28.00\00:14:30.93 riddled with potholes. 00:14:31.23\00:14:32.70 This young boy, who has a fractured skull and likely 00:14:33.47\00:14:37.51 a brain injury, would have to bounce down an incredibly 00:14:37.54\00:14:41.44 bad dirt road for over five hours. 00:14:41.48\00:14:45.51 This journey would definitely worsen his condition 00:14:46.31\00:14:49.48 and potentially end his life. 00:14:49.52\00:14:52.02 But thank God, this young boy won't have to risk 00:14:53.02\00:14:56.06 land travel today. 00:14:56.09\00:14:57.53 Adventist World Aviation is honored to provide lifesaving 00:14:58.53\00:15:02.83 med-evac flights in difficult to reach areas of the world. 00:15:02.86\00:15:06.23 Just by looking out the window of the aircraft, one can easily 00:15:07.24\00:15:10.51 see how rural and thick the jungle is here. 00:15:10.54\00:15:15.28 There are no roads visible and often villagers have to travel 00:15:16.28\00:15:20.22 down a slow winding river to reach aid. 00:15:20.25\00:15:23.55 But with lifesaving flights, the patient can be brought 00:15:24.55\00:15:28.89 to the aid they so desperately need. 00:15:28.92\00:15:31.96 Narrator: Soon the sight of Waspam comes into view. 00:15:46.17\00:15:49.51 This town is used as a port since it is situated 00:15:49.54\00:15:53.35 on the Rio Cocoa River. 00:15:53.38\00:15:55.68 They do have a small runway that is mostly only used by Clint. 00:15:56.38\00:16:00.16 Because the runway doesn't have constant air traffic control, 00:16:00.19\00:16:03.73 the locals don't observe safety guidelines and use the runway 00:16:03.76\00:16:08.33 as part of their daily road system. 00:16:08.36\00:16:10.37 People drive, walk and let their livestock graze on the runway. 00:16:11.07\00:16:15.07 So for this reason, Clint makes a pass over the runway first 00:16:16.07\00:16:19.57 and flies low to the ground to scare off anyone that might be 00:16:19.77\00:16:23.65 on the runway. 00:16:23.68\00:16:25.01 He then turns the aircraft around and approaches 00:16:26.15\00:16:29.38 for final decent. 00:16:29.42\00:16:31.29 This runway is very narrow and not very well maintained 00:16:46.00\00:16:48.94 by the town. Clint must fully concentrate on his landing 00:16:48.97\00:16:52.44 to make sure he stays on the narrow path. 00:16:52.47\00:16:55.81 Clint skillfully brings the aircraft down and makes contact 00:17:03.69\00:17:07.89 with the gravel. 00:17:07.92\00:17:09.26 He has arrived safely to collect his patient. 00:17:09.62\00:17:12.23 Thankfully, the ambulance is waiting at the gate and is ready 00:17:13.23\00:17:15.96 to load the young boy. 00:17:16.00\00:17:17.43 The ambulance pulls up next to the plane. 00:17:17.97\00:17:20.44 There's a surprise when the rear doors of the ambulance opens; 00:17:20.74\00:17:24.54 why there are 2 young patients that need emergency med-evacs. 00:17:25.04\00:17:28.58 The nurse carrying the unconscious young boy is first 00:17:28.68\00:17:32.88 to get out of the ambulance. 00:17:32.91\00:17:34.38 But there is another patient, a scared young girl 00:17:34.98\00:17:38.42 with a broken femur and a broken back. 00:17:38.45\00:17:41.72 Due to the severity of her fractures, she cannot physically 00:17:41.76\00:17:46.53 endure the pain of land transportation. 00:17:46.56\00:17:50.13 Her frail back won't allow for that dangerous journey by truck. 00:17:50.43\00:17:55.37 Military personnel struggle getting her into the aircraft, 00:17:56.37\00:17:59.84 will be very tricky and they must load her 00:17:59.87\00:18:02.81 with the utmost care. 00:18:02.84\00:18:04.38 She is terribly frightened, and it is quite evident, 00:18:04.41\00:18:08.08 that she is in enormous pain. 00:18:08.12\00:18:11.19 A nurse will accompany the 2 children since there isn't 00:18:12.19\00:18:14.66 enough room to take family members along 00:18:14.69\00:18:17.19 in this small aircraft. 00:18:17.23\00:18:18.86 The nurse must also monitor the condition of the small boy 00:18:19.86\00:18:22.86 during flight. 00:18:22.90\00:18:24.23 So, ever so carefully, Clint helps the military men load 00:18:25.37\00:18:30.54 the young girl into the plane. 00:18:30.57\00:18:32.21 Supplies here are very limited and there is no stretcher 00:18:32.24\00:18:36.21 available to use for transportation. 00:18:36.24\00:18:38.38 All they have are blankets, that they pick up the young girl in 00:18:38.41\00:18:43.12 and carry her to the plane. 00:18:43.15\00:18:45.45 Undoubtedly this exacerbates the young girl's pain 00:18:46.25\00:18:49.42 and tears stream down her cheeks. 00:18:49.66\00:18:52.43 After a few attempts, the men finally situate the young girl 00:18:53.23\00:18:56.97 into the back of the airplane. 00:18:57.00\00:18:59.43 It's a tearful and heart wrenching sight as the young 00:19:00.24\00:19:03.20 girl says good bye to her mother. 00:19:03.24\00:19:05.94 This young girl must make this journey with only the 00:19:06.94\00:19:09.48 companionship of a stranger as her guardian. 00:19:09.51\00:19:12.91 Adventist World Aviation always makes a practice to offer 00:19:12.95\00:19:17.95 return flights for their patients after they have 00:19:17.99\00:19:21.22 received their medical treatment. 00:19:21.26\00:19:22.82 Although it's very scary now, the patient will not be left 00:19:23.83\00:19:27.56 stranded in Managua and will be safely returned to her mother. 00:19:27.60\00:19:31.77 Clint and his passengers have no time to waste. 00:19:32.77\00:19:35.80 He is now leaving with an extra passenger, 00:19:36.60\00:19:39.07 that he had not anticipated. 00:19:39.11\00:19:41.08 Within moments, Clint and the patients are racing 00:20:16.38\00:20:18.91 down the runway. 00:20:18.95\00:20:20.28 The nurse in the back seat looks anxiously out the window 00:20:20.82\00:20:23.99 as the ground rushes past her. 00:20:24.02\00:20:26.22 She has never been in an airplane before 00:20:26.25\00:20:28.86 and the experience is all so very new to her. 00:20:28.89\00:20:32.29 Clint skillfully lifts the aircraft into the sky 00:20:48.18\00:20:51.15 and turns the plane southward towards their destination. 00:20:51.18\00:20:55.25 This flight will take just over an hour. 00:20:56.05\00:20:58.32 Making this journey by ground would be impossible. 00:20:58.69\00:21:01.79 From the air, it's clear to see, that there are no roads 00:21:02.39\00:21:05.26 for the ground travel. 00:21:05.29\00:21:06.63 As far as the eye can see, there is just dense jungle 00:21:07.60\00:21:11.70 with only rivers cutting through the trees. 00:21:11.73\00:21:13.90 Without this flight, these patients might not survive 00:21:14.70\00:21:17.84 the accident they endured. 00:21:17.87\00:21:20.11 The injuries these 2 young patients sustained 00:21:56.24\00:21:58.91 from the bus accident are life threatening. 00:21:58.95\00:22:02.12 As Clint glances back, he sees that the young boy has slipped 00:22:02.15\00:22:06.15 into unconsciousness. 00:22:06.19\00:22:08.06 The nurse holds him tight as his body goes limp. 00:22:09.56\00:22:12.99 The young girl's frail body also lies perfectly still. 00:22:14.00\00:22:17.97 She has multiple bone fractures and she must remain as still 00:22:18.57\00:22:22.50 as possible to keep from injuring herself further. 00:22:22.54\00:22:25.84 Clint keeps a copy of all medical notes of the patients 00:22:26.84\00:22:29.78 he transports.
