00:01:33.99\00:01:38.16 in the upper corner along the boarder of the Rio Cocoa 00:01:38.19\00:01:41.56 with the boarder of Honduras and I live here with my family, 00:01:41.60\00:01:48.20 2 children and my wife, who is a nurse and we fly this airplane, 00:01:48.24\00:01:52.77 a Cessna 172, for medical work, all around this region, 00:01:52.81\00:01:59.01 covering about 150 miles in all offers med-evac flights to all the 00:02:18.57\00:02:21.94 neighboring villages that need to transport patients 00:02:21.97\00:02:25.34 to a higher level of medical care. 00:02:25.37\00:02:27.94 There are no safe road systems from the thick jungles 00:02:28.54\00:02:31.35 to Puerto Cabezas, which is the closest 00:02:31.38\00:02:34.72 hospital equipped to handle critical injuries. 00:02:34.75\00:02:37.32 Clint: We devote a lot of our attention and time 00:02:37.35\00:02:40.66 to humanitarian aid. 00:02:40.69\00:02:42.02 The right hand arm of the gospel is being able despite this demanding flight schedule, the Hanley's 00:03:21.83\00:03:25.93 are not satisfied with only offering aviation service. 00:03:25.97\00:03:29.64 Their ministry in Tronquera is so much more. cement walls and roof and floor in it. 00:03:48.32\00:03:54.70 That's been our latest project, that we've been working on 00:03:55.66\00:03:58.07 for the last 6 months or so, is building on this church. 00:03:58.10\00:04:02.30 Santa Rita is about 250 people, about half of the village 00:04:05.91\00:04:09.18 is Seventh-day Adventist. 00:04:09.21\00:04:10.55 Narrator: Today, Clint and his young son Brendell are doing 00:04:11.71\00:04:15.25 that in a very tangible way; they are going to finish 00:04:15.28\00:04:18.79 the construction on a new Adventist church 00:04:18.82\00:04:21.52 in the neighboring village of Santa Rita. 00:04:21.56\00:04:24.79 Jud Wickwire, Vice President of Operations for Adventist 00:04:25.29\00:04:28.83 World Aviation is visiting the project in Nicaragua. 00:04:28.86\00:04:32.70 He also offers to help work on the construction 00:04:32.73\00:04:35.40 of the new church. 00:04:35.44\00:04:37.01 The church construction site is a short drive 00:04:38.84\00:04:41.18 to the neighboring village. 00:04:41.21\00:04:42.84 This region of northeastern Nicaragua has a terrain called 00:04:42.88\00:04:46.85 Pine Savannah. 00:04:46.88\00:04:49.02 A few clay roads, which are very neglected and full of 00:04:49.75\00:04:52.85 pot-holes wind through the trees Traveling these roads 00:04:52.89\00:04:57.43 is very slow. 00:04:57.46\00:05:00.43 Once Miskito people, speaking their language 00:07:18.07\00:07:20.24 is an acceptance into their circle. 00:07:20.27\00:07:22.10 The entire Hanley family has been accepted into the Miskito 00:07:22.50\00:07:25.81 culture, simply because they took the time to learn 00:07:25.84\00:07:29.24 the language of the people they are reaching. 00:07:29.28\00:07:31.85 Jud: The core of any mission work is building relationships. 00:07:32.88\00:07:37.05 And the Hanley's have been here for almost ten years 00:07:37.09\00:07:40.76 and they have, not just built relationships, means to be 00:09:19.19\00:09:23.22 a missionary and to teach him 00:09:23.26\00:09:25.19 the importance of helping others 00:09:25.23\00:09:27.36 Although he is young, Brendell is a missionary to his Skin and all kinds of skin 00:16:03.36\00:16:09.93 topical treatments, that we have too. 00:16:09.96\00:16:12.70 The amount of medicine, that you see here 00:16:13.00\00:16:15.74 using at the current rate, that I am using it will probably 00:16:15.77\00:16:19.57 last a couple years. 00:16:19.61\00:16:20.94 However, occasionally I have been a little more busy 00:16:20.98\00:16:24.91 in mobile clinics and when that happens, we go through 00:16:24.95\00:16:28.68 the medicine quite more quickly. 00:16:28.72\00:16:31.19 It is dispensed rather, rather fast. 00:16:31.22\00:16:34.76 The Miskito people, that live in small villages typically have 00:16:34.79\00:16:40.76 very little medical care available. 00:16:40.80\00:16:44.30 There is a nurse sometimes in their village or another village 00:16:44.33\00:16:47.44 nearby, that is responsible for their healthcare needs. 00:16:47.47\00:16:53.24 However, the nurse is limited by the amount of medicine, 00:16:53.27\00:16:57.08 that they receive from the government clinic. 00:16:57.11\00:16:59.08 So they end up at the clinic with colds, fevers and diarrhea 00:16:59.11\00:17:04.25 are the most common ones. 00:17:04.29\00:17:05.99 Aside from the prevalence of pregnancy. 00:17:06.02\00:17:09.92 Narrator: While Clint and son are away working on the church 00:17:09.96\00:17:13.16 construction, Marilyn is working at the mission base 00:17:13.19\00:17:16.63 tending to a patient, who has stopped by. 00:17:16.67\00:17:19.23 The flow of patients is steady at this mission, 00:17:19.93\00:17:22.67 often Marilyn will see 4-5 patients a day. 00:17:22.70\00:17:26.17 Many of these patients travel great distances 00:17:26.21\00:17:29.21 to the mission base, seeking medical help. 00:17:29.24\00:17:32.31 Today a family has walked over four hours 00:17:32.91\00:17:35.08 to visit Marilyn. 00:17:35.12\00:17:36.65 A mother, her daughter and her child are feeling sick. 00:17:37.05\00:17:41.02 The baby is just 3 months old and has worrisome symptoms. 00:17:41.32\00:17:44.99 Marilyn examines the child first and listens 00:17:45.79\00:17:48.26 for repository sounds. 00:17:48.30\00:17:49.96 She asks the mother a few questions to try and narrow 00:17:50.47\00:17:53.03 down the baby's ailment. 00:17:53.07\00:17:55.20 Marilyn: The baby was sick for a little while and is not eating 00:17:55.70\00:18:01.41 and losing weight. 00:18:01.44\00:18:03.04 It has a little diarrhea, but no repository infection. 00:18:03.24\00:18:08.35 And just the look of the baby, he's very white, he's very light 00:18:11.62\00:18:15.16 skinned, but his mother is light skinned too. 00:18:15.19\00:18:18.36 So he is probably anemic. And he is probably having 00:18:18.39\00:18:23.30 diarrhea, but exactly at what level he needs medication 00:18:23.33\00:18:27.00 I am still not exactly sure. 00:18:27.04\00:18:29.10 She's also complaining, that she doesn't have any milk 00:18:29.14\00:18:31.37 for the baby. 00:18:31.41\00:18:32.74 And that comes from two things. 00:18:32.77\00:18:34.11 One is, three things. She doesn't drink enough water, 00:18:34.14\00:18:39.15 the Miskito people don't drink enough water. 00:18:39.18\00:18:42.65 They don't feed the babies on a schedule. 00:18:42.68\00:18:45.99 They just give them breast whenever they cry, or every 00:18:46.22\00:18:51.13 now and then and so they never, the babies never 00:18:51.16\00:18:53.70 actually drain the breast, so the breast refills with milk. 00:18:53.73\00:18:57.40 So they are not getting good milk. 00:18:57.43\00:19:01.30 Narrator: With Marilyn's understanding of the Miskito 00:19:01.34\00:19:03.41 culture, Marilyn can better diagnose the baby. 00:19:03.44\00:19:06.68 She knows that the baby isn't receiving the nutrients it needs 00:19:06.71\00:19:10.45 and is dehydrated. 00:19:10.48\00:19:12.55 Miskito mothers are uneducated on the importance 00:19:13.15\00:19:15.78 of drinking enough water and their children often suffer. 00:19:15.82\00:19:19.35 It seems in this culture, they intentionally avoid drinking 00:19:20.16\00:19:23.26 water for several reasons. 00:19:23.29\00:19:25.13 Marilyn: One, they're not thirsty. 00:19:26.13\00:19:27.60 So they don't think, that they need it. 00:19:27.63\00:19:29.60 And secondly, if they drink, then they have to go pee, 00:19:29.63\00:19:32.67 which, it's not always great conditions out here. 00:19:32.70\00:19:35.67 People have to go for long ways in the jungle, or they have to 00:19:35.70\00:19:39.44 go on the bus for a long time. 00:19:39.47\00:19:41.78 If they are children, if they give their children a lot of 00:19:41.81\00:19:44.68 water, that means that their children will pee in their 00:19:44.71\00:19:48.12 diapers and then they have a lot more laundry. 00:19:48.15\00:19:50.15 So it's a thing from childhood, that they've learned 00:19:50.19\00:19:54.09 to just not drink so much water. 00:19:54.12\00:19:56.73 Narrator: After speaking more with the mother, Marilyn feels 00:19:57.43\00:20:00.10 confident in a diagnosis. 00:20:00.13\00:20:01.86 The baby isn't getting proper nutrition from the mother 00:20:02.06\00:20:05.03 and is also not getting enough to drink. 00:20:05.07\00:20:07.10 The baby's symptoms all stem from the mother's poor 00:20:07.14\00:20:10.41 education on basic health principles. 00:20:10.44\00:20:12.91 Marilyn: I'm probably going to go ahead and give her vitamins. 00:20:13.94\00:20:18.31 The baby is only three months old. 00:20:25.62\00:20:27.26 So you don't want to give him too much medication. 00:20:28.26\00:20:31.63 Just in case anything. Natives here do not have access to vitamins or prenatal care. 00:21:16.34\00:21:21.08 So Marilyn goes to her pharmacy closet to gather 00:21:21.38\00:21:24.35 a few items to give the patients to help some re-hydration salts, 00:21:41.60\00:21:44.40 so that she has this to drink for a little while. 00:21:44.43\00:21:47.64 She's dizzy and her blood is not rebuilding itself as fast 00:21:47.67\00:21:54.81 as it needs to and so I'm going to do some education 00:21:54.84\00:21:57.01 about good diet too. 00:21:57.05\00:21:59.71 As well as, I talked to her already about drinking 00:21:59.75\00:22:02.98 more water. 00:22:03.02\00:22:04.35 But I'm going to be pretty generous. 00:22:06.22\00:22:08.26 A lot of times I give just thirty, but they've come 00:22:08.29\00:22:10.29 from a long ways away. 00:22:10.33\00:22:11.66 They're from a village it's only about half an hour drive away, 00:22:11.69\00:22:16.97 but it will take them about four hours walk to get here, 00:22:17.00\00:22:20.64 and another four hours to get back home. 00:22:20.67\00:22:23.57 So I am going to be pretty generous. 00:22:23.61\00:22:25.04 She's nursing a baby and her daughter is also nursing a baby, 00:22:25.07\00:22:30.01 so I am going to send plenty of vitamins for them for prenatal 00:22:30.05\00:22:35.15 vitamins which include iron and calcium and folic acid, so that 00:22:35.18\00:22:39.35 they can try to get the mothers to be as healthy as possible. 00:22:39.39\00:22:42.39 Narrator: Nurse Marilyn takes some time to talk with the 00:22:43.89\00:22:46.86 mothers and educate them about the importance of 00:22:46.90\00:22:49.06 nutrition and how to take care of not only of themselves, 00:22:49.10\00:22:52.80 but their young babies. 00:22:52.83\00:22:54.37 Sadly, the diet of the Miskito people is limited to 3 staple 00:22:54.50\00:22:58.81 items: white rice, cassava, and beans. 00:22:58.84\00:23:03.85 Which leaves giant deficiencies in what their 00:23:03.88\00:23:07.12 body needs to stay healthy. 00:23:07.15\00:23:09.18 Marilyn: They only have white rice and white cassava in their 00:23:09.38\00:23:12.62 house from their plantations right now. 00:23:12.65\00:23:15.59 So that means, that she is not recovering from her anemia, 00:23:15.62\00:23:19.59 because she really doesn't have any protein or any vegetables, 00:23:19.63\00:23:22.33 or any vitamins coming into her. 00:23:22.36\00:23:24.17 So I am going to make some suggestions about some of 00:23:25.57\00:23:28.14 those things that they can do, because they don't have the 00:23:28.17\00:23:30.07 resources to be able to buy them. 00:23:30.11\00:23:31.67 And they would have to go to Waspam or Port 00:23:31.71\00:23:34.08 to do that, because they are not available in the village, 00:23:34.11\00:23:36.24 so I am going to make some suggestions for her on that. 00:23:36.28\00:23:40.02 Narrator: The Miskito people are poor subsistence farmers. 00:23:41.82\00:23:45.35 They eat what they can grow on their plantations. 00:23:45.39\00:23:48.06 If they have any extra crops, they sell that small amount 00:23:48.36\00:23:52.13 to get extra money. 00:23:52.16\00:23:53.83 Unfortunately, often their crops yield just enough 00:23:53.86\00:23:57.00 to supply them with food, leaving no extra money 00:23:57.03\00:24:00.50 to buy any other type of food including vegetables. 00:24:00.54\00:24:03.51 Marilyn: So I did a lot of education on what foods would 00:24:03.54\00:24:08.71 have protein in them and what foods would have 00:24:08.74\00:24:10.95 vitamins in them. 00:24:10.98\00:24:12.35 And that she needs to eat more of that. 00:24:12.38\00:24:14.72 Less of things like top ramen, which they are scrapping to try 00:24:14.75\00:24:20.16 to buy, because they think that it has vitamins and stuff in it. 00:24:20.19\00:24:24.59 But it doesn't, so you know they are spending their money 00:24:24.63\00:24:27.76 on things, that don't have as much vitamins in it, water and instructs them 00:25:14.28\00:25:16.81 to drink plenty, before they have to begin back 00:25:16.85\00:25:19.71 on their 4 hour walk home. 00:25:19.75\00:25:22.02 She also tells them, how their water intake can help 00:25:22.05\00:25:26.35 their babies as well. 00:25:26.39\00:25:27.89 The Miskito people are so thankful for Marilyn's advice 00:25:29.92\00:25:32.59 and information about readily accessible. 00:26:37.23\00:26:39.63 Half of this isolated area is without access 00:26:39.66\00:26:42.20 to clean water or toilets. 00:26:42.23\00:26:43.97 About 900 thousand people live without safe 00:26:44.00\00:26:48.30 drinking water. What a pity. 00:26:48.34\00:26:51.27 Water-related diseases exact a huge toll on family health, 00:26:51.31\00:26:56.64 keeping children out of school and stifling economic growth. 00:26:56.68\00:27:01.42 Sadly, nearly 300 children in Nicaragua die each year 00:27:01.45\00:27:06.05 from diarrhea, caused by unsafe drinking water 00:27:06.09\00:27:10.53 and poor sanitation. 00:27:10.56\00:27:12.53 The child that came to visit Marilyn today, could have 00:27:13.56\00:27:17.33 been part of that statistic, had his parents not come 00:27:17.37\00:27:21.40 to the mission airbase today. 00:27:21.44\00:27:23.61 The information they received about drinking plenty of clean 00:27:24.31\00:27:27.24 water, could save this young babies life. 00:27:27.28\00:27:30.48 To fight against the epidemic of unsafe water, the Tronquera 00:27:32.21\00:27:36.28 mission airbase installed a well to provide access to clean 00:27:36.32\00:27:40.29 water to the neighboring villages. 00:27:40.32\00:27:42.99 Clint: This is our well in Santa Rita. 00:27:43.02\00:27:45.83 This is for the whole village.
