Yeah. 00:15:04.71\00:15:05.86 She is really excited getting off these medications. 00:15:05.89\00:15:09.93 Well, see that was basically her primary thing 00:15:09.96\00:15:13.19 that she is wanting to do is 00:15:13.22\00:15:14.54 there is couple of things for cholesterol medicine 00:15:14.57\00:15:17.48 and her blood pressure medicine. 00:15:17.51\00:15:18.58 Those were, she is heard of focused 00:15:18.61\00:15:20.24 on that throughout the program 00:15:20.27\00:15:21.38 and she want to go ahead and get off those things. 00:15:21.41\00:15:24.16 I like to talk to you for a moment 00:15:24.19\00:15:27.31 having had taken both those medications myself 00:15:27.34\00:15:31.73 I get off the blood pressure medications, 00:15:31.76\00:15:34.13 it never elevated or is continued to drop. 00:15:34.16\00:15:37.50 But we know there that some people on the cholesterol meds, 00:15:37.53\00:15:42.30 they may come here with cholesterol 270, 280 00:15:42.33\00:15:46.34 and we are expected to drop scientifically 00:15:46.37\00:15:49.29 and all of a sudden it goes up. 00:15:49.32\00:15:51.27 Sometimes that happens. 00:15:51.30\00:15:52.33 That does happen. 00:15:52.36\00:15:53.40 Yeah. Yeah. 00:15:53.43\00:15:54.46 And why is that? What is that all about? 00:15:54.49\00:15:57.65 Well, one thing we have to remember is just 00:15:57.68\00:16:00.79 almost every laboratory value 00:16:00.82\00:16:03.09 will have some natural degree of variation. 00:16:03.12\00:16:05.32 So there can be, there can be that where as just 00:16:05.35\00:16:09.89 when you take a blood test, 00:16:09.92\00:16:12.03 it's really like throwing a dart at a point 00:16:12.06\00:16:14.32 and sometimes little high, sometimes little low 00:16:14.35\00:16:16.47 that's what the nature of lab. 00:16:16.50\00:16:17.58 So that can explain little minor, minor changes. 00:16:17.61\00:16:21.67 Other times there can be thriven underlying cause 00:16:21.70\00:16:24.63 for what that might be. 00:16:24.66\00:16:26.11 And I know through the years 00:16:26.14\00:16:27.94 I've come to this campus to sort of cover for physicians 00:16:27.97\00:16:31.42 as they are on vacation or they are speaking somewhere. 00:16:31.45\00:16:34.19 And so over the years I've seen, 00:16:34.22\00:16:36.13 most people do really well to achieve the goals 00:16:36.16\00:16:40.84 but for other people, some question mark. 00:16:40.87\00:16:42.90 Why did they not lose this much weight 00:16:42.93\00:16:45.07 as they are hoping to or why does, 00:16:45.10\00:16:46.61 you know, their cholesterol go up or down 00:16:46.64\00:16:48.41 or special like triglycerides will tend to bound around. 00:16:48.44\00:16:52.18 And there can be really a variety of reasons 00:16:52.21\00:16:54.42 that we have to sort of look into their specific situation 00:16:54.45\00:16:57.87 to find out what might be the reason for that so. 00:16:57.90\00:17:00.81 I want to ask you a direct question. 00:17:00.84\00:17:02.73 I mean, I've been, I battled with this my own life. 00:17:02.76\00:17:06.86 I know that in my experience 00:17:06.89\00:17:10.40 I'm taking these cholesterol meds 00:17:10.43\00:17:12.82 and they bring the numbers down 00:17:12.85\00:17:15.85 but is that a real number, 00:17:15.88\00:17:18.27 because as soon as I stop the med 00:17:18.30\00:17:20.79 the number goes right back up. 00:17:20.82\00:17:23.01 Well, that's a good point. 00:17:23.04\00:17:25.10 So, so what it is, medications they work. 00:17:25.13\00:17:28.89 They work, on specifics sort of biochemistry of our bodies. 00:17:28.92\00:17:33.24 And so they can and in fact the FDA 00:17:33.27\00:17:36.26 will not allow medication to come on the market 00:17:36.29\00:17:38.80 unless it actually shows an actual impact. 00:17:38.83\00:17:41.41 They have to show that first 00:17:41.44\00:17:42.58 before they are allowed to sell it. 00:17:42.61\00:17:44.49 And so they generally do work at least in terms of changing 00:17:44.52\00:17:49.38 the factors that were majoring. 00:17:49.41\00:17:51.85 For example, cholesterol medicines 00:17:51.88\00:17:53.29 or looking at cholesterol and some of the other lipids 00:17:53.32\00:17:55.95 or fats in the blood. 00:17:55.98\00:17:57.94 And so they can go ahead and work on that, 00:17:57.97\00:18:00.57 but the challenge really is 00:18:00.60\00:18:03.52 and I've seen this in my medical career 00:18:03.55\00:18:06.18 as different studies come out is 00:18:06.21\00:18:09.29 that typically this is how it works. 00:18:09.32\00:18:11.87 You have study that text look at population to be able to see 00:18:11.90\00:18:18.46 do some blood test for example and compare that with outcomes. 00:18:18.49\00:18:21.97 Whether its cardiac outcome or cancer 00:18:22.00\00:18:24.53 or whatever the outcome may be 00:18:24.56\00:18:26.60 and they find that if somebody's vitamin D levels 00:18:26.63\00:18:29.27 elevated or lower, somebody's cholesterol 00:18:29.30\00:18:31.18 or somebody's hypertension 00:18:31.21\00:18:32.59 or all of these different things, 00:18:32.62\00:18:34.38 what are they correlate with 00:18:34.41\00:18:35.99 and they find that there is correlation. 00:18:36.02\00:18:38.41 So you would think that oh, 00:18:38.44\00:18:41.11 if you could just get the cholesterol down 00:18:41.14\00:18:42.70 then we should be able to prevent heart attacks 00:18:42.73\00:18:44.47 because people in the population study 00:18:44.50\00:18:46.83 with low cholesterol level has had, 00:18:46.86\00:18:49.69 found to have reduced rates of heart attacks. 00:18:49.72\00:18:53.40 But unfortunately over time what happens is, 00:18:53.43\00:18:56.84 when we develop a medicine 00:18:56.87\00:18:58.47 through research to be able to adjust 00:18:58.50\00:19:01.86 these lab values and we expect that 00:19:01.89\00:19:05.78 if you improve the lab value that you would expect 00:19:05.81\00:19:08.59 that there will be a good, ultimate clinical outcomes. 00:19:08.62\00:19:11.62 Some times when we do the actually 00:19:11.65\00:19:13.68 what's called perspective studies or studies 00:19:13.71\00:19:15.84 where you do an intervention, you treat patients 00:19:15.87\00:19:19.30 and you go ahead and see over time 00:19:19.33\00:19:21.29 how does that actually impact really 00:19:21.32\00:19:23.85 the important outcome such as mortality, 00:19:23.88\00:19:27.22 you know, death, or other such outcomes. 00:19:27.25\00:19:31.25 We find that it didn't really match what we anticipated 00:19:31.28\00:19:35.10 based on those original population studies. 00:19:35.13\00:19:38.00 And as a physician I can be frustrated some times. 00:19:38.03\00:19:41.48 You think you know what's gonna work 00:19:41.51\00:19:44.40 and then when you do the studies 00:19:44.43\00:19:45.65 mainly it doesn't quite come out the way we had hoped. 00:19:45.68\00:19:49.09 So from my perspective 00:19:49.12\00:19:52.92 this where the lifestyle medicine perspective 00:19:52.95\00:19:55.91 is we would like to be able to try 00:19:55.94\00:19:58.15 to get people back to their natural physiology 00:19:58.18\00:20:01.29 and that is to where the way our body is designed as to-- 00:20:01.32\00:20:05.13 designed to try to maintain our health 00:20:05.16\00:20:07.69 or if we get out of that equilibrium 00:20:07.72\00:20:11.25 to try to bring us back to a healthy state. 00:20:11.28\00:20:14.20 And so that's where we try to take people 00:20:14.23\00:20:16.38 who have been living a sort of a modern 00:20:16.41\00:20:18.76 unhealthy lifestyle where, through lifestyle choices 00:20:18.79\00:20:22.26 there are sort of insulting their body, 00:20:22.29\00:20:25.21 their health, their physiology. 00:20:25.24\00:20:27.31 We try to get them back to a natural lifestyle 00:20:27.34\00:20:29.88 so that all of those mechanisms 00:20:29.91\00:20:31.43 that have been given to us in our bodies, 00:20:31.46\00:20:33.55 its a costly bring us back to house, 00:20:33.58\00:20:35.76 to health that we sort of relieved 00:20:35.79\00:20:38.29 the body of the unnatural stresses, 00:20:38.32\00:20:41.47 through an unhealthy lifestyle choices 00:20:41.50\00:20:43.13 try to get them back to, 00:20:43.16\00:20:44.60 where the body is able to get back to the level to help 00:20:44.63\00:20:47.62 that the body is designed to achieve. 00:20:47.65\00:20:50.46 So with regards to Shela, 00:20:50.49\00:20:52.97 Shela would be fine as long as she continues 00:20:53.00\00:20:59.64 this change in lifestyle. 00:20:59.67\00:21:01.94 I eating plant based diet or plant based lifestyle, 00:21:01.97\00:21:07.81 her numbers will continue to drop, 00:21:07.84\00:21:10.95 she will continue to lose weight 00:21:10.98\00:21:13.25 and that will bring the numbers down. 00:21:13.28\00:21:15.22 She will continue to exercise. 00:21:15.25\00:21:17.56 So she is in good shape in other words. 00:21:17.59\00:21:21.02 I think she is in good shape. 00:21:21.05\00:21:22.32 It was-- when we came in 00:21:22.35\00:21:25.39 basically the question of, 00:21:25.42\00:21:26.45 how much her cholesterol was within normal limits, 00:21:26.48\00:21:29.48 and the question was to what extent is that 00:21:29.51\00:21:32.15 as a result of the cholesterol lowering medicine 00:21:32.18\00:21:34.62 that she is on and, or to what extent is lifestyle, 00:21:34.65\00:21:40.32 her lifestyle having an impact. 00:21:40.35\00:21:42.74 So during this intensive lifestyle intervention program 00:21:42.77\00:21:45.92 it was decided to let's go ahead and suspend her 00:21:45.95\00:21:50.19 or take her off of both of the medicines 00:21:50.22\00:21:52.83 that she is wanting to get. 00:21:52.86\00:21:54.15 And so while under this physician supervised program 00:21:54.18\00:21:57.25 let's go ahead and take her off those 00:21:57.28\00:21:59.31 and then maximize lifestyle intervention to see 00:21:59.34\00:22:02.55 does she really need those medicines or not, okay. 00:22:02.58\00:22:06.28 And so I just saw her earlier today 00:22:06.31\00:22:08.43 and we ran through the second laps 00:22:08.46\00:22:10.82 and what have turned out what as I was expected is 00:22:10.85\00:22:14.34 her cholesterol levels in fact did go up 00:22:14.37\00:22:16.87 because she was off the medicine 00:22:16.90\00:22:18.67 even though she is eating a zero cholesterol diet here. 00:22:18.70\00:22:21.67 you know, So what gives clearly 00:22:21.70\00:22:23.65 the cholesterol medicines were reducing her cholesterol 00:22:23.68\00:22:27.23 and then when we took her off 00:22:27.26\00:22:28.55 that it sort of came back to the level 00:22:28.58\00:22:30.29 that she would normally be having. 00:22:30.32\00:22:33.06 So then the question is 00:22:33.09\00:22:34.96 how should we look in terms of the future for strategy? 00:22:34.99\00:22:42.34 And so what I've shared with here is 00:22:42.37\00:22:44.47 let's go ahead and continue with her 00:22:44.50\00:22:47.52 off the both her cholesterol 00:22:47.55\00:22:49.39 and her blood pressure medicine. 00:22:49.42\00:22:50.53 She has done very well for blood pressure 00:22:50.56\00:22:52.69 and then have her repeat those laps in six weeks 00:22:52.72\00:22:55.65 to see as the lifestyle continues to kick in, 00:22:55.68\00:22:59.36 does she really need that medicine. 00:22:59.39\00:23:01.70 For example does she have this familial hypercholesterolemia 00:23:01.73\00:23:05.67 or can she, you know, can she be off 00:23:05.70\00:23:07.67 and that's what we will find out in the six weeks. 00:23:07.73\00:23:10.22 Well, Shela is close enough 00:23:10.25\00:23:11.71 by where she could come visit us 00:23:11.74\00:23:14.48 and continue seeing our doctors on fresh vegetables and fruits? 00:24:45.50\00:24:50.14 Well, John, this is a question 00:24:50.17\00:24:51.47 that's been asked many times over many years. 00:24:51.50\00:24:53.55 It's a great question and it's not a simple answer. 00:24:53.58\00:24:56.97 Some people in rural parts of America are getting 00:24:57.00\00:24:59.87 very excited about organic foods 00:24:59.90\00:25:01.62 and by the way, great options. 00:25:01.65\00:25:03.47 I work at a center called 00:25:03.50\00:25:05.02 Weimer Center of Health and Education 00:25:05.05\00:25:06.93 and we have a certified organic farm here. 00:25:06.96\00:25:08.91 So we believe in organic produce. 00:25:08.94\00:25:11.11 But I've noticed as I traveled around the country, 00:25:11.14\00:25:13.07 sometimes you go to some little out of the way place 00:25:13.10\00:25:15.72 and you look at that organic produce 00:25:15.75\00:25:17.43 and it looks like its been 00:25:17.46\00:25:18.77 sitting there for maybe a month. 00:25:18.80\00:25:21.14 It's kind of shriveled up and looking in poor condition. 00:25:21.17\00:25:24.42 Actually some of that poor condition fresh produce 00:25:24.45\00:25:28.22 whether it's organic or not 00:25:28.25\00:25:29.61 may have less nutrients in it than frozen produce. 00:25:29.64\00:25:34.15 In general if you just go right to the farm 00:25:34.18\00:25:35.98 and you can eat the food right out the farm 00:25:36.01\00:25:38.17 that's gonna be the most nutritionally dense, 00:25:38.20\00:25:40.48 the best nutrition that you can get. 00:25:40.51\00:25:42.44 But there are loses as food is transported. 00:25:42.47\00:25:46.13 So fresh food sometimes 00:25:46.16\00:25:49.39 if it's been in transit a long time, 00:25:49.42\00:25:51.91 sit in the shelf a long time 00:25:51.94\00:25:53.27 actually can have significant nutritional loses. 00:25:53.30\00:25:56.86 Frozen foods preserve those nutrients pretty well. 00:25:56.89\00:25:59.88 Some things are preserved better than others. 00:25:59.91\00:26:02.27 And then canned foods because of a heating process 00:26:02.30\00:26:04.94 some of the nutrients or what we call heat labeled 00:26:04.97\00:26:07.80 that means the heating process destroys 00:26:07.83\00:26:09.96 some of those nutrients, vitamin C would be an example. 00:26:09.99\00:26:13.03 So the best rule of thumb 00:26:13.06\00:26:14.61 listen, if you are canning your own foods 00:26:14.64\00:26:16.16 out of your garden, that's great. 00:26:16.19\00:26:17.46 Include those in your diet. 00:26:17.49\00:26:18.97 If sometimes you are using canned items that's not wrong. 00:26:19.00\00:26:22.48 But if you can use more fresh fruits 00:26:22.51\00:26:24.19 that's going to be the best 00:26:24.22\00:26:25.25 especially if they are grown from your own garden 00:26:25.28\00:26:27.34 or you are getting them from a local source, 00:26:27.37\00:26:29.50 they have not been in transit 00:26:29.53\00:26:31.12 for many days or weeks or months 00:26:31.15\00:26:34.01 and beyond that frozen food is also good alternative. 00:26:34.04\00:26:37.47 Bottom line don't get so preoccupied 00:26:37.50\00:26:39.49 with all these questions. 00:26:39.52\00:26:40.63 Just eat more fruits and vegetables, 00:26:40.66\00:26:42.87 whole gains, nuts and seeds 00:26:42.90\00:26:44.14 and you are gonna have better health 00:26:44.17\00:26:45.72 but those best sources 00:26:45.75\00:26:47.28 fresh right out of the garden you can't beat it. 00:26:47.31\00:26:49.22 That's why many people 00:26:49.25\00:26:50.48 are moving to community gardens to personal gardens. 00:26:50.51\00:26:53.82 Think about that next time 00:26:53.85\00:26:55.36 when you are in the store just think boy, 00:26:55.39\00:26:57.35 I could be growing some of these own
