Where do you start? 00:12:31.78\00:12:33.81 -I guess we start right at the beginning. 00:12:34.63\00:12:37.60 The patients come in and I say, 00:12:37.61\00:12:39.84 what are your health problems or 00:12:39.85\00:12:41.66 your health challenges. 00:12:41.67\00:12:43.33 One of them is his weight, which is obvious 00:12:44.21\00:12:47.39 when you see the gentlemen. 00:12:47.40\00:12:49.06 Another one is his diabetes. 00:12:49.27\00:12:51.06 Along with that also goes usually high blood pressure 00:12:52.06\00:12:54.75 which he also had. 00:12:54.76\00:12:56.18 -He also had some sort of a heart problem as well. 00:12:57.87\00:13:02.21 What was that? 00:13:03.27\00:13:04.66 -Well that wasn't his major concern. 00:13:04.67\00:13:07.21 I'm curtain that he has some heart problem because 00:13:07.22\00:13:11.06 the number 1 complication of 00:13:11.07\00:13:13.09 diabetes is some form of heart disease. 00:13:13.10\00:13:15.81 Cardiovascular disease. Either a heart attack 00:13:15.82\00:13:19.75 or stroke, high blood pressure and all of those go 00:13:19.76\00:13:22.81 along together. He definitely had high blood pressure. 00:13:22.82\00:13:25.84 -So it's fairly common that those of us who, 00:13:26.12\00:13:29.51 already have or do have will 00:13:29.87\00:13:32.21 deal with some form of cardiovascular disease. 00:13:32.48\00:13:36.09 -Absolutely. In fact that's the number 1 00:13:36.33\00:13:38.63 common cause of death. 00:13:39.15\00:13:40.69 People think that if they have diabetes, 00:13:41.24\00:13:43.75 they need to be concerned that they 00:13:43.76\00:13:45.87 might loose their toes or feet because 00:13:45.88\00:13:49.63 of gangrene or they might have kidney problems 00:13:49.64\00:13:53.21 because of End stage Renal disease 00:13:53.39\00:13:55.87 and may end up on dialysis. 00:13:55.88\00:13:57.87 or they may go blind because of complications 00:13:57.88\00:14:00.72 of diabetic retinopathy. 00:14:00.73\00:14:02.42 Those do happen but it is far more common 00:14:03.60\00:14:06.72 to have some form of cardiovascular disease. 00:14:06.73\00:14:09.57 -Now I had a lady call me 00:14:09.58\00:14:11.48 concerning her husband yesterday. 00:14:11.49\00:14:13.27 I'm told by her that his physician suggested 00:14:13.75\00:14:19.09 that he go on dialysis. 00:14:19.51\00:14:21.51 At what point do we know that someone needs dialysis? 00:14:21.81\00:14:26.30 You look at various factors but, 00:14:26.60\00:14:29.60 things that we look at are how high the creatinine level 00:14:29.87\00:14:33.60 is in the blood. How high the Blood Urea Nitrogen or BUN is. 00:14:33.61\00:14:38.06 Also how much protein they have in their urine. 00:14:38.07\00:14:41.81 Probably one of the earliest signs of kidney disease 00:14:42.21\00:14:44.87 in diabetes is protein in the urine. 00:14:44.88\00:14:47.57 We look for something called Microalbumin. 00:14:47.58\00:14:50.54 Microalbuminuria and below a curtain number 00:14:51.06\00:14:54.51 it is acceptable but once it goes past 30 00:14:54.52\00:14:58.57 than that's when we start to see protein in the urine. 00:14:58.58\00:15:02.57 It usually continues to get worse. 00:15:02.58\00:15:06.15 Unless the patients make some changes in their lifestyle. 00:15:06.16\00:15:09.48 So we've seen from time to time, 00:15:10.18\00:15:12.09 and I know you've seen it for many many years, 00:15:12.10\00:15:14.48 that those kinds of individuals come to our NEWSTART program 00:15:15.06\00:15:18.72 and all of a sudden they no longer have a renal failure, 00:15:19.36\00:15:24.12 they no longer have high blood pressure, 00:15:24.13\00:15:27.15 and many of the other diseases. 00:15:27.16\00:15:28.84 How is this possible? 00:15:29.39\00:15:31.27 I have to thank God about it because, 00:15:31.72\00:15:35.84 I know that many people come here for various things. 00:15:36.24\00:15:39.21 We put them all through this lifestyle change, 00:15:39.60\00:15:43.06 which almost seems like one thing fixes all. 00:15:43.30\00:15:47.45 Can you comment about that? 00:15:47.84\00:15:49.24 Well as far as 00:15:49.60\00:15:51.81 Specifically we were talking about kidney disease. 00:15:52.84\00:15:55.21 They find that people with kidney disease, 00:15:55.45\00:15:58.75 on the standard American diet, which by the way 00:15:58.76\00:16:02.30 the acronym for standard American diet is Sad. 00:16:02.31\00:16:06.39 standard American diet. High in fat, 00:16:06.40\00:16:09.48 high in refined carbohydrates, high in protein. 00:16:09.49\00:16:13.18 This tends to speed up the progress of kidney disease. 00:16:13.45\00:16:18.69 Putting them on a plant based diet, 00:16:19.06\00:16:21.09 whole plant foods eaten whole, 00:16:21.10\00:16:23.24 we find that the amount of work their 00:16:23.25\00:16:26.09 kidney has to do is much less. 00:16:26.10\00:16:28.21 The blood flow through the kidney is less. 00:16:28.22\00:16:30.60 Oftentimes your creatinine and their Blood Urea Nitrogen or BUN 00:16:30.61\00:16:35.18 come down and they have less protein in the urine. 00:16:35.19\00:16:38.42 We've had patients come here who were told 00:16:39.27\00:16:41.42 that they needed to go on dialysis, 00:16:41.43\00:16:42.81 and with these changes, 00:16:42.82\00:16:44.18 they were able to delay having to go on dialysis. 00:16:44.51\00:16:47.12 In other words, their kidneys worked better 00:16:47.13\00:16:50.21 when they were on plant protein, 00:16:50.51\00:16:52.81 than using the animal protein which 00:16:52.82\00:16:56.06 is so common in the American diet and lifestyle. 00:16:56.07\00:16:58.63 -For the folks who are watching this program right now, 00:16:59.06\00:17:02.39 I feel like you might make a little plug to them right here 00:17:03.06\00:17:08.18 because many of them like the lady I spoke with yesterday, 00:17:08.19\00:17:11.54 the husband just game up. 00:17:11.55\00:17:13.09 Do they give up? Should we be concerned about it? 00:22:17.30\00:22:20.48 What should we do if we find out that we 00:22:20.49\00:22:23.21 are struggling with high blood pressure, 00:22:23.22\00:22:24.63 What is the first thing? 00:22:24.64\00:22:26.18 Right, people find out that they have 00:22:26.19\00:22:28.57 high blood pressure in different settings. 00:22:28.58\00:22:30.66 Very commonly they are in a doctors office 00:22:30.67\00:22:33.21 for some other reason, and we do vitals. 00:22:33.22\00:22:35.27 One of those vitals is blood pressure 00:22:35.28\00:22:37.06 and people say, wow it's running high. 00:22:37.07\00:22:38.72 It all depends on if it is a little bit high, 00:22:39.15\00:22:44.45 as people get older their blood pressure tends to go up, 00:22:44.46\00:22:47.21 or if it is really high and they have 00:22:47.22\00:22:50.81 a Hypertension emergency for example. 00:22:50.82\00:22:53.42 -So sometimes just when they meet you their blood 00:22:53.43\00:22:55.57 pressure goes up because they have that white coat syndrome 00:22:55.58\00:22:57.84 -right. Is that real? It actually is. 00:22:57.85\00:23:00.75 White jacket hypertension. 00:23:00.76\00:23:02.72 I see it very frequently, it's fairly common. 00:23:02.73\00:23:06.57 We try to do a couple things, we try to 00:23:08.48\00:23:11.51 check it again after they calm down a little. 00:23:11.52\00:23:13.66 I've literally even tried to not wear a jacket 00:23:13.67\00:23:16.57 but I think it is actually doctor hypertension 00:23:16.58\00:23:18.57 not just white jacket. -Right, 00:23:18.58\00:23:19.66 So you do it 2 or 3 times but than you 00:23:19.67\00:23:22.72 find out that you do have hypertension, 00:23:22.73\00:23:25.06 what is the next step? What should you do than? 00:23:25.15\00:23:27.15 Let me just emphasise, 00:23:27.16\00:23:28.69 we don't really make a diagnosis, 00:23:28.72\00:23:31.15 if we are measuring slightly elevated 00:23:31.16\00:23:33.42 blood pressure, we don't make a diagnosis 00:23:33.43\00:23:35.06 of hypertension, we like to check it 00:23:35.07\00:23:36.48 on 2 separate settings because 00:23:36.63\00:23:38.81 our blood pressure goes up and down throughout the day. 00:23:38.82\00:23:41.69 Just because we get one spot reading, 00:23:41.87\00:23:45.24 is not a diagnosis unless it is incredibly high. 00:23:45.25\00:23:48.33 -Let's say that it is high blood pressure. 00:23:48.42\00:23:51.39 -Unfortunately many of my colleagues, 00:23:52.21\00:23:54.69 who are not oriented to lifestyle 00:23:54.70\00:23:59.27 approach to the practice of medicine, 00:23:59.28\00:24:01.33 I joke that they tap their tendon 00:24:01.63\00:24:04.51 and than they right out prescription. 00:24:04.52\00:24:06.75 Unfortunately a lot of patients are immediately 00:24:06.76\00:24:08.51 put on a prescription for blood pressure. 00:24:08.52\00:24:10.60 If it is mild or even a lower moderate hypertension, 00:24:10.84\00:24:14.12 really the first thing we aught to do is 00:24:14.21\00:24:16.42 try to maximize lifestyle intervention. 00:24:16.43\00:24:19.15 In most cases, for mild hypertension, 00:24:19.16\00:24:21.69 it can be controlled with lifestyle. 00:24:21.70\00:24:23.15 -So what are some quick things 00:24:23.16\00:24:25.21 that they can do lifestyle wise. 00:24:25.22\00:24:26.45 -There is a number of very simple things, 00:24:26.46\00:24:29.06 one is, you can get exercise. Especially aerobic exercise. 00:24:29.07\00:24:34.42 Things like walking, or bicycling 00:24:34.43\00:24:37.69 which I enjoy doing. 00:24:37.70\00:24:39.06 Exercise by itself is really quite powerful. 00:24:39.24\00:24:42.60 The second thing in terms of diet, is salt. 00:24:43.06\00:24:47.24 A curtain percentage of patients 00:24:47.57\00:24:50.21 are actually very sensitive to salt. 00:24:50.22\00:24:51.66 So if they could reduce their salt intake, 00:24:51.67\00:24:53.57 that would be recommended. 00:24:53.72\00:24:55.57 Especially no adding of salt, 00:24:55.81\00:24:57.51 looking at the labels 00:24:57.52\00:24:59.09 and seeing where it is. 00:24:59.10\00:25:00.42 The third thing is loosing some weight 00:25:00.78\00:25:03.63 and that by itself can often times do the trick. 00:25:03.69\00:25:05.78 Alcohol, caffeine, or tabaco, can raise 00:25:06.21\00:25:10.63 the blood pressure as well. 00:25:10.64\00:25:12.57 -Once they have tried those things, 00:25:12.87\00:25:15.39 and if those things are not working, 00:25:15.40\00:25:17.18 that is when they maybe need to 00:25:17.19\00:25:19.33 talk about some other things 00:25:19.34\00:25:20.84 because it is not something that you want to leave alone. 00:25:20.85\00:25:23.06 -No, high blood pressure or hypertension 00:25:23.07\00:25:25.72 is called the silent killer, that is without symtoms. 00:25:25.73\00:25:29.06 If you don't know about it because you don't have symtoms, 00:25:29.36\00:25:33.42 it can put you at a significant risk of stroke, 00:25:33.43\00:25:36.72 and that is very disabling, to have a stroke 00:25:37.24\00:25:40.21 and loose a function, 00:25:40.22\00:25:41.51 as well as a heart attack as well. 00:25:41.52\00:25:45.60 -Would you agree that hypertension 00:25:46.75\00:25:49.24 is usually related to something else? 00:25:49.25\00:25:50.87 What you said is maybe they are overweight, 00:25:50.88\00:25:53.12 maybe they're this maybe they're that. 00:25:53.13\00:25:54.60 You probably try and help 00:25:54.78\00:25:56.27 them discover that that issue is. 00:25:56.28\00:25:58.09 -Yeah you know, high blood pressure 00:25:58.27\00:26:00.45 falls into 2 categories, 00:26:00.46\00:26:01.87 we probably won't get into that here 00:26:01.88\00:26:03.21 but primary and secondary. 00:26:03.22\00:26:04.27 It can be caused by some other medical conditions. 00:26:04.28\00:26:06.63 Most often it is caused by lifestyle or ageing. 00:26:07.09\00:26:12.09 -Well this was very practical advice to give you something 00:26:12.42\00:26:16.09 if you know of someone who is struggling with hypertension 00:26:16.24\00:26:18.36 or high blood pressure.
