I recently returned to the island of Vanuatu, 00:00:31.58\00:00:34.32 to visit Dr. Turnbull and family. 00:00:34.35\00:00:37.05 It had been nearly two years since the last time I was here. 00:00:37.08\00:00:40.28 Vanuatu is a nation comprised of 83 distinct islands 00:00:40.31\00:00:43.92 located in the South Pacific. 00:00:43.95\00:00:45.85 We met up with Dr. Mark 00:00:49.04\00:00:50.40 as the natives call him on the island of Santo, 00:00:50.43\00:00:53.53 where he needed to pick up some supplies. 00:00:53.56\00:00:55.71 Then it was back to the airport 00:01:01.35\00:01:03.13 where we loaded up the Maule M-7 Super Rocket 00:01:03.16\00:01:06.53 before taking off to their home base on the island of Gaua. 00:01:06.56\00:01:09.52 When we arrived at the hangar house 00:01:20.30\00:01:22.12 as usual I received a warm welcome. 00:01:22.15\00:01:24.87 I also couldn't believe 00:01:24.90\00:01:25.93 how many things have changed since my last visit. 00:01:25.96\00:01:28.85 Our Maule M-7 Super Rocket as it is called 00:01:28.88\00:01:32.16 was in need of some repairs. 00:01:32.19\00:01:33.82 We wasted no time getting the job done 00:01:33.85\00:01:36.08 using some of the parts I brought with me 00:01:36.11\00:01:37.80 from the United States. 00:01:37.83\00:01:39.08 Look at all these new parts that we brought with us 00:01:42.65\00:01:44.40 from America, from Alaska bush wheel 00:01:44.43\00:01:46.84 and these guys are really serious about 00:01:46.87\00:01:48.30 making great parts for these Maule aircraft. 00:01:48.33\00:01:50.87 And these are the new struts that we have for the plane 00:01:50.90\00:01:53.31 because this one here that one is bend just a little bit 00:01:53.34\00:01:55.84 which concerns us about safety issues. 00:01:55.87\00:01:58.73 So while we are here we are hoping to get 00:01:58.76\00:02:00.72 some of these new parts on the plane. 00:02:00.75\00:02:02.79 So we have a new tailwheel, extra tires, extra springs 00:02:02.82\00:02:06.50 and then these new struts for the front suspension. 00:02:06.53\00:02:09.86 Yeah, we have a lot of maintenance issues 00:02:09.89\00:02:13.34 which are related to the kind of airstrips 00:02:13.37\00:02:15.45 that we are landing on. 00:02:15.48\00:02:18.21 We are landing on bush airstrips all the time. 00:02:18.24\00:02:20.89 And so, the tire wheel is definitely 00:02:20.92\00:02:22.67 the most abused part of the aircraft. 00:02:22.70\00:02:25.08 Out of curiosity, how often do you use the plane? 00:02:25.11\00:02:27.32 Do you use like once a month or once a week or? 00:02:27.35\00:02:30.24 Jeff, this is--this is flying virtually everyday. 00:02:30.27\00:02:34.98 Sometimes two or three times a day flying into bush airstrips 00:02:35.01\00:02:38.16 two or three times a day. 00:02:38.19\00:02:39.23 Occasionally we have a day or two pass 00:02:39.26\00:02:41.09 where we don't have flights but-- 00:02:41.12\00:02:42.71 Chris, our cameraman was saying, 00:02:42.74\00:02:43.81 most people have a garage with a car in it, 00:02:43.84\00:02:45.68 you've got a garage with an airplane in it. 00:02:45.71\00:02:47.41 That's right, now, this is a very busy aircraft 00:02:47.44\00:02:51.76 but we sure need these new springs because 00:02:51.79\00:02:54.81 they are very, very stressed landing on these airstrips. 00:02:54.84\00:02:59.77 These struts for the front wheels are very-- 00:02:59.80\00:03:03.74 they are an upgrade of the stock ones 00:03:03.77\00:03:07.41 and I believe these are stainless steel, 00:03:07.44\00:03:09.04 very, very strong. 00:03:09.07\00:03:11.26 We have an issue with our small airstrips 00:03:11.29\00:03:14.87 because they are very narrow 00:03:14.90\00:03:16.96 and when we are turning on a bush airstrip 00:03:16.99\00:03:19.09 it's putting a lot of torque on the wheels, 00:03:19.12\00:03:22.29 very, very tight on the wheels. 00:03:22.32\00:03:25.00 So that actually bends them slightly. 00:03:25.03\00:03:27.86 And then when you do your checks on the wheel alignment 00:03:27.89\00:03:31.59 one of the wheels is now-- 00:03:31.62\00:03:33.58 Kind of pull you off the other directions. 00:03:33.61\00:03:34.97 Trying to pull us and I can feel it when I'm landing. 00:03:35.00\00:03:37.66 So we need to do this because known Dr. Turnbull probably for ten years and more 00:05:48.82\00:05:52.03 and I know him really well, and I know he is a charts guy, 00:05:52.06\00:05:54.98 he likes to always figure and calculate everything out 00:05:55.01\00:05:57.14 with charts and schematics and do his research 00:05:57.17\00:05:59.11 and cross all his tears and dot all his eyes 00:05:59.14\00:06:01.62 and when we are looking at getting a plane, 00:06:01.65\00:06:03.34 he looked at all the different kinds of planes we could get 00:06:03.37\00:06:06.57 and Maule was the best kind of plane 00:06:06.60\00:06:08.26 for this type of work environment. 00:06:08.29\00:06:10.44 And he came to the conclusion he set me down, 00:06:10.47\00:06:12.19 he said Jeff, we need to get a Maule 00:06:12.22\00:06:13.96 but then we realized we couldn't afford it. 00:06:13.99\00:06:16.00 And to make a long story short we had a gentleman 00:06:16.03\00:06:18.58 who called up our ministry after seeing 00:06:18.61\00:06:20.35 one of our first Vanuatu videos on 3ABN 00:06:20.38\00:06:23.43 and he saw that we had a need 00:06:23.46\00:06:25.28 and he said I want to donate a plane to work in Vanuatu 00:06:25.31\00:06:29.38 and I asked what kind of plane was it? 00:06:29.41\00:06:31.13 And he says, oh, it's a Maule M-7 00:06:31.16\00:06:33.99 and that was exactly the kind of plane 00:06:34.02\00:06:36.18 that Mark had figured out that we needed 00:06:36.21\00:06:38.01 and that's what the Lord provided for this project 00:06:38.04\00:06:40.01 is just absolutely amazing how it all worked out. 00:06:40.04\00:06:42.50 Since our last visit, a new clinic and patient house 00:06:44.41\00:06:47.40 had been built next to the hangar house. 00:06:47.43\00:06:49.41 We asked Dr. Mark to give us a tour. 00:06:49.44\00:06:53.51 This is our outpatient room 00:06:53.54\00:06:54.95 where I see people for check-ups, 00:06:54.98\00:06:57.12 or day visits or day patients, day cases 00:06:57.15\00:07:00.25 and sometimes it becomes our overflow inpatient room. 00:07:00.28\00:07:05.02 We have somehow sick patients 00:07:05.05\00:07:06.90 or post-operative patients in here at times. 00:07:06.93\00:07:09.51 This is our main inpatient room 00:07:09.54\00:07:11.09 where we look after postoperative patients 00:07:11.12\00:07:14.11 or people who are intense who need an intensive care 00:07:14.14\00:07:17.25 who are very sick 00:07:17.28\00:07:18.57 and so they are taking care off in a nice hygienic environment. 00:07:18.60\00:07:22.00 When they get better, 00:07:22.03\00:07:23.27 they usually go out to our house-- 00:07:23.30\00:07:26.10 patient house which is next door 00:07:26.13\00:07:28.02 which is a more local style building 00:07:28.05\00:07:29.47 a bit more comfortable for them. 00:07:29.50\00:07:31.67 This is our sterilization area 00:07:31.70\00:07:33.73 we use very simple forms of sterilization. 00:07:33.76\00:07:36.55 For example, boiling water 00:07:36.58\00:07:38.50 and we use some simple antiseptics and disinfectants 00:07:38.53\00:07:42.59 which are keeping us from getting any infections. 00:07:42.62\00:07:47.42 We are very impressed 00:07:47.45\00:07:49.67 with low infection rate of our operative patients. 00:07:49.70\00:07:53.39 We have a toilet and shower here which is flushing toilet 00:07:53.42\00:07:56.43 nice hygienic set up here for our patients. 00:07:56.46\00:07:59.76 They really appreciate these buildings 00:07:59.79\00:08:01.94 this is regarded as kind of high quality, 00:08:01.97\00:08:05.07 the most high quality care in this province. 00:08:05.10\00:08:06.96 Very simple structure here 00:08:06.99\00:08:08.31 but very highly appreciated by people. 00:08:08.34\00:08:11.04 This is a very significant room, 00:08:11.07\00:08:12.60 this is where operations take place. 00:08:12.63\00:08:14.17 We do cesarean sections 00:08:14.20\00:08:16.71 and we do some hernia repairs, 00:08:16.74\00:08:20.35 emergency hernia repairs here as well. 00:08:20.38\00:08:23.26 Which are the two most commonly 00:08:23.29\00:08:24.97 needed forms of emergency surgery in this area. 00:08:25.00\00:08:28.63 It is once again we use very simple anesthesia, 00:08:28.66\00:08:32.21 we only use a drug called Ketamine. 00:08:32.24\00:08:35.51 We don't use complex anesthesia 00:08:35.54\00:08:37.10 because of the environment that we are living in but it's-- 00:08:37.13\00:08:40.40 it's complete for what we are doing. 00:08:40.43\00:08:42.73 Through here we have a store room 00:08:42.76\00:08:44.49 where we keep our medicines and our surgical supplies 00:08:44.52\00:08:48.66 which are quite well stocked and we keep them-- 00:08:48.69\00:08:52.65 we keep supplies coming from Australia 00:08:52.68\00:08:54.69 and we have some government supplies 00:08:54.72\00:08:56.60 coming to us from the government pharmacy 00:08:56.63\00:08:58.81 which we really appreciate. 00:08:58.84\00:09:00.34 And so basically we are quite well set up 00:09:00.37\00:09:03.21 in our own specialized field here 00:09:03.24\00:09:06.00 we don't have very fancy equipment, 00:09:06.03\00:09:08.47 we don't have much diagnostic equipment 00:09:08.50\00:09:10.71 but what we are doing here is a big blessing 00:09:10.74\00:09:12.86 and then cases that are very difficult for us 00:09:12.89\00:09:15.85 to have over here, we just take them down 00:09:15.88\00:09:17.26 to Santo or Luganville in the aircraft. 00:09:17.29\00:09:20.45 Okay, when patients come to Gaua 00:09:20.48\00:09:23.89 where they are able to stay in our patient house 00:09:23.92\00:09:25.71 which is as you can see it's a local style building 00:09:25.74\00:09:27.93 with Natanguru roof that's very cool. 00:09:27.96\00:09:30.65 It's very practical building. 00:09:30.68\00:09:32.78 Inside it's pretty tidy, it's nice and clean hygienic. 00:09:32.81\00:09:36.98 The patients have got nice clean rooms and beds 00:09:37.01\00:09:40.56 and a nice floor and most people feel like 00:09:40.59\00:09:44.97 it's not a bad idea to get sick and come to Gaua now. 00:09:45.00\00:09:49.17 This is a really nice place to stay 00:09:49.20\00:09:50.96 and they have their own kitchen, 00:09:50.99\00:09:52.14 they have toilets and shower so. 00:09:52.17\00:09:54.14 I think most of them feel it's pretty good place to-- 00:09:54.17\00:09:57.95 to come and stay when they are sick. 00:09:57.98\00:09:59.49 While we were in Vanuatu, 00:10:06.45\00:10:07.82 we had the opportunity to shadow Dr. Mark 00:10:07.85\00:10:10.31 as he traveled to other islands 00:10:10.34\00:10:11.85 bringing patients to Gaua for treatment. 00:10:11.88\00:10:14.04 This morning I have received a phone call from Sola 00:10:20.58\00:10:23.04 which is the next island north of us 00:10:23.07\00:10:24.85 they called me about a lady 00:10:24.88\00:10:26.56 who has a breech presentation baby 00:10:26.59\00:10:29.94 so she is about to deliver, 00:10:29.97\00:10:31.64 she has had seven children already 00:10:31.67\00:10:34.74 and this is her eighth pregnancy 00:10:34.77\00:10:36.69 and apparently the baby is in not a good position, 00:10:36.72\00:10:40.75 so they have asked me to go and pick her up 00:10:40.78\00:10:42.65 and bring her here to Gaua so we can look after right here. 00:10:42.68\00:10:46.78 So right now, we've refilled and we are ready to go. 00:10:46.81\00:10:49.82 Okay, so we got a lady here 00:11:47.36\00:11:48.88 who I haven't done any examination or anything 00:11:48.91\00:11:51.53 but suggestion is that she is breech 00:11:51.56\00:11:54.91 and she is smoky gravida 00:11:54.94\00:11:57.15 she probably needs a bit of help with this delivery. 00:11:57.18\00:11:59.44 So she is coming to Gaua for that. 00:11:59.47\00:12:01.28 The baby is okay, it's just fun at side 00:13:26.72\00:13:29.55 and it's starting to come out, 00:13:29.58\00:13:31.38 so I have to do a caesarian on this lady. 00:13:31.41\00:13:33.47 Okay, sister-- 00:13:36.51\00:13:39.67 Okay, so we have a transverse lie 00:13:39.70\00:13:41.91 it's not a breech but its a transverse lie. 00:13:41.94\00:13:44.81 The baby is unable to come--come down 00:13:44.84\00:13:48.88 and so we are going to do a caesarian section. 00:13:48.91\00:13:50.68 She also wants to prevent future pregnancy 00:13:50.71\00:13:52.98 so we use the opportunity to help her in that area as well. 00:13:53.01\00:13:57.27 Mr. John is the surprised father of two, it is-- 00:14:35.34\00:14:39.26 none of us realized I suspected 00:14:39.29\00:14:41.17 but I didn't actually pick it up on the scan. 00:14:41.20\00:14:43.53 I think that second baby was hiding right behind. 00:14:43.56\00:14:46.71 And so when we took out the first baby, 00:14:46.74\00:14:49.68 I notice there was another hand saying, hello, I'm in here too. 00:14:49.71\00:14:54.47 So, anyway John is a proud father of nine now. 00:14:54.50\00:14:57.52 Okay, we got a busy day ahead of us, 00:15:09.07\00:15:11.03 we got patient on Mota Lava Island 00:15:11.06\00:15:14.57 who we need to go and get. 00:15:14.60\00:15:15.63 He has got urinary retention. 00:15:15.66\00:15:18.85 Sounds like he is not very stable, 00:15:18.88\00:15:20.76 we also have directors with us, who we need to take to Mota. 00:15:20.79\00:15:25.95 And this afternoon we have a big program at the school so, 00:15:25.98\00:15:31.37 as usual there is lots happening. 00:15:31.40\00:15:32.64 But we will see what's happening on Mota Island as we depart now. 00:15:32.67\00:15:36.80 This gentleman had urinary retention yesterday. 00:16:30.21\00:16:34.62 And I think he advised the nurse practitioner 00:16:42.84\00:16:45.12 to give put a urinary catheter in. 00:16:45.15\00:16:47.71 And so now, he is very much stabilized 00:16:47.74\00:16:50.53 so now we got time, we is a sad reality 00:17:30.71\00:17:33.29 that not every case ends the best. 00:17:33.32\00:17:36.35 What is the series of events on last 24 hours? 00:17:36.38\00:17:40.58 Okay, we had a very busy day, yesterday. 00:17:40.61\00:17:45.36 We have finished up doing a cesarean section on a lady 00:17:45.39\00:17:48.74 who had been in labor for two weeks 00:17:48.77\00:17:53.59 and her membranes had broken three days ago. 00:17:53.62\00:17:57.63 The baby was born okay, 00:17:57.66\00:18:00.48 seem to be okay but during the night the baby died. 00:18:00.51\00:18:04.59 So, it was another one of the experiences 00:18:04.62\00:18:09.52 that people go through 00:18:09.55\00:18:11.79 when they are involved in looking after these patients 00:18:11.82\00:18:13.81 I don't know exactly what happened. experiencing them. 00:18:53.34\00:18:55.53 In 2011, while helping to cut a jungle airstrip 00:19:03.13\00:19:06.38 on the island of Vanua Lava a tragic accident occurred 00:19:06.41\00:19:09.64 that killed 43-year-old Wesley Mooi. 00:19:09.67\00:19:12.26 Darren Vaotuua was there when the accident happened. 00:19:20.94\00:19:24.09 Darren and I decided to return 00:19:24.12\00:19:25.97 to what is now being named Wesley airstrip 00:19:26.00\00:19:28.79 to remember our brother in Christ. 00:19:28.82\00:19:30.98 We just want to say thank you so much 00:20:04.96\00:20:09.52 for warm welcome. unfortunate accident? 00:21:13.49\00:21:16.87 Well, it was the last day of our work here Friday 00:21:16.90\00:21:21.75 and Wesley and Pete have been 00:21:21.78\00:21:23.80 the most experienced chainsaw movers 00:21:23.83\00:21:26.60 had decided to tackle what we call them Nabunga tree 00:21:26.63\00:21:30.26 with big strangler fig type trees here, 00:21:30.29\00:21:35.11 and I was left some place here down the way 00:21:35.14\00:21:38.61 just plaining out and cutting what was on the ground. 00:21:38.64\00:21:44.99 So Peter and Wesley had gone to tackle this Nabunga tree 00:21:45.02\00:21:49.78 after having successfully fallen one. 00:21:49.81\00:21:54.14 Just earlier that day and when I was cutting up 00:21:54.17\00:21:59.10 I heard this crack and I turned around 00:21:59.13\00:22:02.33 and looked up at the tree and that's what left over there 00:22:02.36\00:22:05.77 and he used to have massive arms 00:22:05.80\00:22:09.01 throwing out in every direction hanging out over this airstrip 00:22:09.04\00:22:13.24 and going around the other side. 00:22:13.27\00:22:14.80 I don't think people even can comprehend 00:22:14.83\00:22:16.63 how big these trees are they are so massive. 00:22:16.66\00:22:18.67 They are massive. 00:22:18.70\00:22:20.74 And when I heard the crack just like a domino effect 00:22:20.77\00:22:24.06 one after another at the arms just fell. 00:22:24.09\00:22:26.45 One, bang, bang, bang, bang 00:22:26.48\00:22:29.82 and I turned and I just laughed 00:22:29.85\00:22:31.55 because I thought that they were just doing their work. 00:22:31.58\00:22:37.16 And then a moment later a man comes running up 00:22:37.19\00:22:39.92 to where I was cutting and said Peter had me dead. 00:22:39.95\00:22:45.35 He was saying that Peter was dead 00:22:45.38\00:22:47.47 and I thought I wasn't too strong on bissalama 00:22:47.50\00:22:50.43 I thought hopefully I have got the message wrong and I start. 00:22:50.46\00:22:54.71 This was all chopped up wood, 00:22:54.74\00:22:56.99 You have to climb over things to run down the back there, 00:22:57.02\00:23:01.53 that's where I found Peter and he was okay. 00:23:01.56\00:23:05.43 Peter says to me have you seen Wesley? 00:23:05.46\00:23:09.74 And that was when our stomach just dropped. 00:23:09.77\00:23:12.89 We knew that something wasn't right. 00:23:12.92\00:23:17.81 You could see his chainsaw still turning over 00:23:17.84\00:23:21.21 on the ground further up the way. 00:23:21.24\00:23:24.79 That's when one of the local people 00:23:24.82\00:23:26.87 who had seen it all happen 00:23:26.90\00:23:28.44 came and showed us right where he was. 00:23:28.47\00:23:31.41 And one of the arms 00:23:31.44\00:23:33.94 look like he must have run towards the tree. 00:23:33.97\00:23:37.31 And one of the arms which is as big as the tree in itself 00:23:37.34\00:23:40.00 had fallen down and just crushed him immediately. 00:23:40.03\00:23:45.97 You can get an idea how big these trees are 00:23:46.00\00:23:48.11 by just standing here at the base of it. 00:23:48.14\00:23:50.51 These things can be the size of a small house across 00:23:50.54\00:23:53.74 and they are just a mass of roots and branches 00:23:53.77\00:23:57.50 trying to cut something like this 00:23:57.53\00:23:58.69 down with a chainsaw is really a challenge. 00:23:58.72\00:24:03.88 He was cutting this one, 00:24:03.91\00:24:06.11 time when tree broke him run go this way. 00:24:06.14\00:24:10.34 Yes. 00:24:13.70\00:24:15.01 These men saw when happened 00:24:26.13\00:24:31.61 and it wasn't--everything has been a bit foggy 00:24:31.64\00:24:33.81 because I was further down the way. 00:24:33.84\00:24:37.23 What you can see in this tree was the back cut. 00:24:37.26\00:24:42.73 Oh, he was putting, you know, in the wedge actually 00:24:42.76\00:24:45.28 it look he probably wanted to fall the tree that way 00:24:45.31\00:24:50.21 and while he was putting in this cut 00:24:50.24\00:24:53.12 that's when the limb that Peter was cutting 00:24:53.15\00:24:56.98 was on the other side of the tree. 00:24:57.01\00:24:59.84 So it should have just fallen on the other side of the tree.
