It appears that IAA MXF files were moved to IAADD (which deletes the original) but the CC was copied not moved. Then new IAA files were made using numbers that had already been used before and the old CC was applied.

Example: IAA451 was moved to IAADD13 but the CC remained

Solution: Delete the cc of any bad programs and start over.
