00:01:08.96\00:01:10.87 But as I have practiced medicine, 00:01:10.90\00:01:13.54 the longer I have been in medicine, the more I realize 00:01:13.57\00:01:16.82 that this disease is very wide spread. 00:01:16.85\00:01:21.45 A lot of times people don't know that they have it, 00:01:21.48\00:01:24.83 in fact we have people who have had a large number of years 00:01:24.86\00:01:30.04 of experience with diabetes before it was ever made 00:01:30.07\00:01:33.51 by the laboratory. 00:01:33.54\00:01:34.68 So we have things that we can say that the laboratory says 00:01:34.71\00:01:39.10 they have diabetes so we will look back in the patient's 00:01:39.13\00:01:42.27 chart and see yes, you've had diabetes for many many years. 00:01:42.30\00:01:47.91 The major way of making the diagnosis of diabetes is by 00:01:47.94\00:01:52.54 the laboratory tests. 00:01:52.57\00:01:53.94 We can get a good idea about the person's metabolism of 00:01:53.97\00:02:00.34 sugar and the whole metabolic process that has to do with 00:02:00.37\00:02:06.59 dealing with sugar, salt, fats, proteins, and all of these 00:02:06.62\00:02:14.10 are involved in the diabetic syndrome. 00:02:14.13\00:02:16.98 We can get a lot of idea about this by the patient's history, 00:02:17.01\00:02:20.76 but to actually make the diagnosis, we need to have 00:02:20.79\00:02:25.09 laboratory testing, so I would like to show you 00:02:25.12\00:02:28.22 what we see with the laboratory. 00:02:28.25\00:02:30.62 Here you see a graph that shows the time on the bottom, 00:02:30.65\00:02:36.95 and the level of the blood sugar along the left side. 00:02:36.98\00:02:41.11 Notice that the blood sugar starts at 70 in this person, 00:02:41.14\00:02:46.14 and at one hour it is up to about 130, then at 00:02:46.17\00:02:53.31 one and a half hours it has come down a little bit, 00:02:53.34\00:02:55.86 then at two hours it's just slightly below where it started, 00:02:55.89\00:03:00.81 maybe 69, just a little below where it started, 00:03:00.84\00:03:05.91 now that's normal, or essentially normal. 00:03:05.94\00:03:09.03 There are a little variations on this, but essentially 00:03:09.06\00:03:11.50 that's normal. 00:03:11.53\00:03:12.50 Now lets see what a mild diabetic would be, 00:03:12.51\00:03:15.99 it starts at about 90- 95, maybe 100, then goes up 00:03:16.02\00:03:22.93 rather sharply and higher than it does in the normal 00:03:22.96\00:03:25.52 and does not come back down to normal in two hours. 00:03:25.55\00:03:29.02 Now lets take a person who is a severe diabetic, 00:03:29.05\00:03:32.41 it starts at a higher level, it can start at 110, 140, 00:03:32.44\00:03:37.07 it can even be at 200, and then as you can see it is 00:03:37.10\00:03:41.03 almost going off the graph over there at 250, 00:03:41.06\00:03:44.64 it can go even higher than that, 3, 4, 500. 00:03:44.67\00:03:49.19 At two hours it isn't anywhere near back down to normal, 00:03:49.22\00:03:54.27 it isn't even turning downward. 00:03:54.30\00:03:56.23 Sometimes people will keep this kind of curve 00:03:56.26\00:03:59.39 much of their lives, the higher the blood sugar is, 00:03:59.42\00:04:04.18 the greater the likelihood the person is to be producing 00:04:04.21\00:04:08.02 too much insulin, and we will talk with you about 00:04:08.05\00:04:11.01 what too much insulin does for you. 00:04:11.04\00:04:13.27 Before that I would like to show you one of those things 00:04:13.30\00:04:16.90 that we use to treat diabetics with and that is diet. 00:04:16.93\00:04:20.63 In the diet we promote a purely vegetarian diet, 00:04:20.66\00:04:24.59 and one might think, would I be able to do a vegetarian 00:04:24.62\00:04:29.74 diet and I have heard that it takes spending hours every day 00:04:29.77\00:04:33.90 in the kitchen to produce a vegetarian diet 00:04:33.93\00:04:37.03 and I will tell you to both of those, yes you can do Exactly! 00:09:37.31\00:09:41.64 and when you come back it is ready to pop into the oven 00:09:41.67\00:09:44.56 and heat for 30- 40 minutes and it is ready for you to eat, 00:09:44.59\00:09:49.50 that sounds good. 00:09:49.53\00:09:51.39 I think every diabetic should understand about the 00:09:51.42\00:09:54.91 vegetarian diet so that when you are able to produce this 00:09:54.94\00:10:00.63 for yourself, you can see that there are advantages 00:10:00.66\00:10:03.72 to your blood sugar after the body becomes accustomed 00:10:03.75\00:10:06.55 to it you will do much better. 00:10:06.58\00:10:08.35 Now I have asked Dr. Wynn Hoarsly 00:10:08.38\00:10:10.53 who is one of our staff physicians at 00:10:10.56\00:10:12.40 Uchee Pines who deals with diabetes a lot to join me 00:10:12.43\00:10:15.76 in this... 00:10:15.79\00:10:16.76 Thank you for coming Dr. Wynn Horsley and together 00:10:16.77\00:10:21.56 we are going to tell you a few things about diabetes. 00:10:21.59\00:10:24.34 Dr. Horsley has had quite a lot of experience with diabetes 00:10:24.37\00:10:28.57 of course since we treat so many diabetics at Uchee Pines 00:10:28.60\00:10:31.52 and I would like to know something about how you 00:10:31.55\00:10:34.80 approach this whole problem, how you think about it, 00:10:34.83\00:10:36.90 how you explain it to the patient and that kind of thing. 00:10:36.93\00:10:39.73 First of all I like approaching it just because of what you 00:10:39.76\00:10:44.17 said is exactly my experience, it's a rather major disease, 00:10:44.20\00:10:49.39 but you can get such excellent results in many cases. 00:10:49.42\00:10:54.44 - It is such a major disease that I describe it to patients 00:10:54.47\00:10:58.05 as a process of acceleration of the aging process. 00:10:58.08\00:11:03.08 - Yes, and of the worst killers of the aging process, 00:11:03.11\00:11:06.66 like arteriosclerosis, and heart attacks, 00:11:06.69\00:11:08.94 it accelerates that whole badness. 00:11:08.97\00:11:13.18 - Joint problems, kidney problems, eye problems, 00:11:13.21\00:11:17.73 and neuropathy, and all sorts of things are worse and start 00:11:17.76\00:11:23.96 getting complications. 00:11:23.99\00:11:25.32 - When I get into it with the patients we make a list 00:11:25.35\00:11:29.42 of the complications that diabetes gives, 00:11:29.45\00:11:31.73 who cares about a high blood sugar if it doesn't cause 00:11:31.76\00:11:34.53 any trouble. - That's right. 00:11:34.56\00:11:36.42 - One thing that I think is worth while early along is 00:11:36.45\00:11:42.23 to see just what is it that's going on at a cellular level, 00:11:42.26\00:11:46.70 so I would like to take a moment to look at the 00:11:46.98\00:11:50.61 board here. - Very good, I see you've got 00:11:50.64\00:11:53.11 a diagram there. - Yeah, this is my attempt 00:11:53.14\00:11:55.83 of sketching a cell. - Looks good! 00:11:55.87\00:11:58.26 - Here is the nucleus with all that library of information 00:11:58.29\00:12:03.00 and here is a blood vessel going by and you could take this 00:12:03.03\00:12:08.14 as even one of the tiny arterioles that finally 00:12:08.17\00:12:12.03 narrows down to a capillary where the red blood cells 00:12:12.06\00:12:15.72 are going through single file, it's so narrow. 00:12:15.75\00:12:18.41 Now of course this blood is carrying all the nutrients 00:12:18.44\00:12:23.31 to the cells, and the one that concerns us right now is the 00:12:23.34\00:12:25.60 glucose. - Uhhuh 00:12:25.63\00:12:26.60 - This glucose will come from the blood then and be out here 00:12:26.63\00:12:35.69 outside the cell, ready to go in. 00:12:35.72\00:12:37.49 Most people I believe that are not in health work 00:12:37.52\00:12:43.94 professionally, may not have done the studies about how 00:12:43.97\00:12:48.14 unique cells are. 00:12:48.17\00:12:49.75 They really are like a fortified castle, they don't let just 00:12:49.78\00:12:55.26 anything, even in the tissue right around here in the body 00:12:55.29\00:12:58.05 just automatically go in. 00:12:58.08\00:12:59.38 The environment is extremely different outside here, 00:12:59.41\00:13:02.66 here's the chemical symbols for salt, we all know that 00:13:02.69\00:13:06.43 blood tastes salty, inside the cell there is hardly 00:13:06.46\00:13:09.10 any of the sodium it makes for salt, instead you've got 00:13:09.13\00:13:12.93 potassium, I'm going to give you the chemical symbol K. 00:13:12.96\00:13:15.61 Now the cell membrane, the outside of the cell is 00:13:15.64\00:13:19.17 constantly keeping the sodium out and pumping in the potassium 00:13:19.20\00:13:24.45 well the glucose that is coming here is the fuel 00:13:24.48\00:13:31.67 of the body, the cells need it, but even glucose does not 00:13:31.70\00:13:35.18 have automatic entry into the cell. 00:13:35.21\00:13:36.88 The only way that glucose can get in is through a 00:13:36.91\00:13:42.15 special mechanism, and I like to compare it to a locked door 00:13:42.18\00:13:47.11 that even glucose can not go through this door 00:13:47.14\00:13:50.03 with this lock on it here, unless you have a key to 00:13:50.06\00:13:54.15 unlock it, and the key is insulin. 00:13:54.18\00:13:58.31 Insulin is the substance in our body that unlocks the cells 00:13:58.34\00:14:03.29 so that the glucose can pass inside, the insulin is produced 00:14:03.32\00:14:06.81 in the pancreas, one or our abdominal organs. 00:14:06.84\00:14:08.95 If you didn't have enough insulin then you can't get 00:14:08.98\00:14:16.31 that glucose getting into the cell like it should 00:14:16.34\00:14:18.71 so you get the glucose piling up outside, and the cell 00:14:18.74\00:14:24.50 is starving. 00:14:24.53\00:14:25.84 I've heard diabetes described as starvation in the midst 00:14:25.87\00:14:29.00 of plenty and I think that is a very apt description. 00:14:29.03\00:14:31.26 This very situation of not having enough insulin 00:14:31.29\00:14:39.58 is one kind of diabetes that now days they generally 00:14:39.61\00:14:43.59 refer to diabetes... by the was the full name is 00:14:43.62\00:14:46.47 diabetes mellitus, the sweet kind of diabetes where your 00:14:46.50\00:14:49.68 blood sugar is to high. 00:14:49.71\00:14:51.00 Diabetes mellitus we will abbreviate this way, Type I 00:14:51.03\00:14:55.27 now this is not the majority of diabetics, 00:14:55.30\00:14:57.99 they used to call this juvenile diabetes and I would say 00:14:58.02\00:15:02.50 10% maybe even less of diabetics are completely what we would 00:15:02.53\00:15:07.83 call type I, where there is insufficient insulin. 00:15:07.86\00:15:12.07 By the way, when we do lab testing routinely, 00:15:12.10\00:15:16.86 I think people certainly anyone who is diabetic knows 00:15:16.89\00:15:19.03 how you keep checking for glucose, but now with advances 00:15:19.06\00:15:24.21 in lab technology they can even check for insulin level. 00:15:24.24\00:15:27.13 It is a little more complicated and a lot more expensive. 00:15:27.16\00:15:30.06 - Oh yes! Absolutely! But a real interest here 00:15:30.09\00:15:33.85 because there really is what's the most common 00:15:33.88\00:15:37.58 kind of diabetes, the situation is not this one. 00:15:37.61\00:15:41.91 You do have the problem of glucose building up here 00:15:41.94\00:15:45.98 a person has high blood sugar so yes they are diabetic 00:15:46.01\00:15:48.81 and the cells aren't getting enough glucose in, 00:15:48.84\00:15:53.72 but when they check on the amount of insulin 00:15:53.75\00:15:56.70 you've got plenty, in fact it's often more 00:15:56.73\00:15:59.89 than the normal amount of insulin. 00:15:59.92\00:16:01.66 This would be the most common kind of diabetes that's 00:16:01.69\00:16:06.64 diabetes Mellitus II, what in the past has been called 00:16:06.67\00:16:11.50 adult onset diabetes. 00:16:11.53\00:16:13.47 - Why isn't it still called adult type? 00:16:13.50\00:16:15.96 - Well, in fact I just heard an endocrinologist tell me 00:16:15.99\00:16:19.94 that a friend of his is treating a number of adolescents 00:16:19.97\00:16:23.77 who have type II diabetes, which seems... 00:16:23.80\00:16:25.97 - That would be 11, 12, 13 year old. 00:16:26.00\00:16:28.56 - People at that age, it's sad to see that happening but it is. 00:16:28.59\00:16:34.56 Another reason for not using the terms juvenile and 00:16:34.60\00:16:39.88 adult onset is that as a person goes on with 00:16:39.91\00:16:43.99 type II diabetes, producing extra insulin, it can exhaust 00:16:44.02\00:16:48.85 the pancreas and start turning into Type I. 00:16:48.88\00:16:53.30 Now those type I diabetics of course are older people 00:16:53.33\00:16:57.44 who have already gone through a whole length of time, "years" 00:16:57.47\00:17:00.57 of being type II, so these are not juveniles, 00:17:00.60\00:17:03.07 so again the term juvenile doesn't really fit. 00:17:03.10\00:17:06.40 It's worth mentioning here about this problem of insulin 00:17:06.43\00:17:13.06 resistance which characterizes Type II diabetes. 00:17:13.09\00:17:17.67 They have lots of insulin, lots of the key, 00:17:17.70\00:17:20.65 but it doesn't seem to be opening the lock, 00:17:20.68\00:17:22.31 so we could think of this as the lock being rusty, 00:17:22.34\00:17:25.80 and we are not able to make the key function. 00:17:25.83\00:17:28.40 - The cell won't let the insulin work. 00:17:28.43\00:17:31.88 - That's right, so in medical lingo the rusty lock is just 00:17:31.91\00:17:36.47 called insulin resistance. 00:17:36.50\00:17:38.48 Now the work that we do is largely with Type II diabetes, 00:17:38.51\00:17:49.94 and I think we need to make clear in our talk here, 00:17:49.97\00:17:52.70 that we are talking to people who have Type II. 00:17:52.73\00:17:57.46 If it is Type I, you really need further evaluation 00:17:57.49\00:18:02.02 and you will need something more than just the 00:18:02.05\00:18:04.68 diet recommendations. 00:18:04.71\00:18:06.12 - You need good professional help. 00:18:06.15\00:18:07.56 - Absolutely! 00:18:07.59\00:18:08.56 - Well I am always very interested in Type II diabetes, 00:18:08.57\00:18:13.93 because that is the one that is so responsive 00:18:13.96\00:18:16.78 to the things that we can do in the home. 00:18:16.81\00:18:19.07 Exercising, diet, regularity, drinking plenty of water, 00:18:19.10\00:18:25.65 all of those things. 00:18:25.68\00:18:26.68 - The most dramatic results of all that we see at 00:18:26.71\00:18:33.44 Uchee Pines are with even one added step, and I feel 00:18:33.47\00:18:38.26 a little unsure about whether to recommend this to people 00:18:38.29\00:18:41.83 at home, it's fasting. - Yes! 00:18:41.86\00:18:45.73 I like fasting for people and anybody can fast for a day. 00:18:45.76\00:18:50.01 - Oh yes, no problem for a day, and I really don't feel bad 00:18:50.04\00:18:54.36 about telling people to try even another day, 00:18:54.39\00:18:57.34 if they want to try two, but I wouldn't recommend 00:18:57.37\00:18:59.30 more than three days... - I should say anybody 00:18:59.33\00:19:02.87 can fast because a Type I diabetic should not fast. 00:19:02.90\00:19:06.85 - That's right. - And people with gout 00:19:06.88\00:19:08.13 should not fast. 00:19:08.16\00:19:09.27 - There's another important point that we need to make here, 00:19:09.30\00:19:11.73 and that is, even if he is a Type II diabetic 00:19:11.76\00:19:15.80 and he is taking medications, if he is going to fast, 00:19:15.83\00:19:19.12 he must stop those medications. 00:19:19.15\00:19:20.76 - Tell me what do you think about the non-insulin types 00:19:20.79\00:19:25.77 of drugs that will control the blood sugar? 00:19:25.80\00:19:29.59 How do you feel about those? 00:19:29.62\00:19:31.56 - Well in my residency, we used those and we had 00:19:31.59\00:19:36.25 various problems...
