00:00:37.26\00:00:40.29 At Uchee Pines Institute we deal with pain in a lot of ways 00:00:40.32\00:00:47.81 we deal with a number of different kinds of pain. 00:00:47.84\00:00:51.38 Pain can usually be categorized in two ways: We can talk about 00:00:51.41\00:00:57.47 chronic pain or we can talk about acute pain. 00:00:57.50\00:01:01.28 Acute pain is usually treated in a different way than 00:01:01.31\00:01:05.43 chronic pain, and this is Don Miller who will be talking 00:01:05.46\00:01:09.36 with me today in helping you to understand how we treat pain 00:01:09.39\00:01:13.29 at Uchee Pines Institute. 00:01:13.32\00:01:15.05 Don! - Well Dr. Thrash there are 00:01:15.08\00:01:18.77 various ways that we treat pain and one of the first things 00:01:18.80\00:01:21.81 is what we do within our own minds to make pain worse, 00:01:21.84\00:01:25.33 we call these things pain intensifiers, 00:01:25.36\00:01:27.39 It's basically our reaction to the pain perception. 00:01:27.42\00:01:32.59 The first and probably the greatest thing that intensifies 00:01:32.62\00:01:36.05 our pain is fear, fear does a number of things to the body 00:01:36.08\00:01:40.07 physiologically, fear does a number of things to the body 00:01:40.10\00:01:42.95 mentally. 00:01:42.98\00:01:43.95 Physiologically we tense up our muscles become more rigid, 00:01:43.96\00:01:48.56 you remember back in the days where you used to get 00:01:48.59\00:01:51.04 hypodermic needles shoved into your muscles, 00:01:51.07\00:01:53.34 they would say relax, yeah, really right, relax, 00:01:53.37\00:01:56.22 they have this great big long needle, they are going to ram it 00:01:56.25\00:01:58.25 into your muscle and they tell you to relax, 00:01:58.28\00:01:59.84 well normally you don't and you are all stiff, 00:01:59.87\00:02:01.96 they run this needle into your muscle, and that stiff muscle 00:02:01.99\00:02:06.04 hurts more than it would have or could have relaxed 00:02:06.07\00:02:09.27 in the first place. 00:02:09.30\00:02:10.27 But it is also the fear of the unknown, when we get a pain 00:02:10.28\00:02:13.89 we wake up in the morning and we've got a pain 00:02:13.92\00:02:16.16 and we self diagnose ourselves, hummm, what could this pain be, 00:02:16.19\00:02:20.75 and we start coming up with all types of horror stories, 00:02:20.78\00:02:23.72 we read an article yesterday, we heard a news broadcast 00:02:23.75\00:02:26.77 about this pain and this great disease caused by it 00:02:26.80\00:02:30.31 and pretty soon we are thinking that we have that disease 00:02:30.34\00:02:33.89 and the pain becomes much greater. 00:02:33.92\00:02:36.36 Then we go to a physician, he diagnoses our pain as being 00:02:36.39\00:02:40.39 we slept wrong and all of a sudden the pain is not 00:02:40.42\00:02:42.56 quite as bad anymore. 00:02:42.59\00:02:43.83 Because our pain is somewhat palliated by the fact that 00:02:43.86\00:02:47.45 our fear has been assuaged, it's a nice way to take care of 00:02:47.48\00:02:50.51 the fear and the pain. 00:02:50.54\00:02:52.46 Another thing that intensifies our pain is guilt, 00:02:52.49\00:02:55.33 these are for people who basically pain prone, 00:02:55.36\00:03:00.02 they are guilty, "oh I shouldn't have done that," 00:03:00.05\00:03:03.22 "if only I hadn't done that," if I hadn't eaten that, 00:03:03.25\00:03:06.75 if I hadn't done that, if I hadn't done this thing over here 00:03:06.78\00:03:09.38 I wouldn't have this pain right now, and so this guilt complex 00:03:09.41\00:03:12.93 seems to make the pain much much greater. 00:03:12.96\00:03:16.04 There is also the thing called anger, now we are talking 00:03:16.07\00:03:19.79 again about how the mind is reacting to the pain perception. 00:03:19.82\00:03:24.62 The pain is the same but now we are angry at somebody 00:03:24.65\00:03:29.52 or something, we are angry at the accident, why did that 00:03:29.55\00:03:33.31 happen to me, and often when we start asking those questions 00:03:33.34\00:03:36.93 we become angry at the very one who can take care of 00:03:36.96\00:03:39.78 our problems, that's anger against God. 00:03:39.81\00:03:42.06 So we are angry against God, or we are angry against our 00:03:42.09\00:03:45.54 spouse, or we are angry against our employer, or we are angry 00:03:45.57\00:03:49.20 against ourselves for doing that stupid thing, 00:03:49.23\00:03:51.41 falling down the stairs, tripping over whatever, 00:03:51.44\00:03:53.64 and we get angry and all of these things along the lines 00:03:53.67\00:03:57.68 are making our pain much much greater. 00:03:57.71\00:04:00.34 Really a great pain intensifier is a thing called loneliness, 00:04:00.37\00:04:06.68 people who are alone suffer more pain, we'll talk about 00:04:06.71\00:04:11.06 that a little bit more when I talk about things to assuage 00:04:11.09\00:04:13.73 and take away the pain, but people who are lonely 00:04:13.76\00:04:16.69 seem to have a greater preception of pain. 00:04:16.72\00:04:19.24 We find that those people who are alone suffer more premature 00:04:19.27\00:04:25.52 deaths, matter of fact, double the national average than those 00:04:25.55\00:04:29.19 who are not alone, divorced people are five times more 00:04:29.22\00:04:32.79 likely to commit suicide and die in fatal accidents 00:04:32.82\00:04:36.14 four times more if they are alone, these are sad 00:04:36.17\00:04:39.70 circumstances for a person. 00:04:39.74\00:04:41.48 They did some experiments back in 1956, they had in a hospital 00:04:41.51\00:04:48.26 ward, they had those people in open ward seemed to use 00:04:48.29\00:04:52.60 less pain medications by about.. they took bout 3.2 doses of 00:04:52.63\00:04:56.94 the pain medication to those in private rooms about 13.4 00:04:56.97\00:05:01.46 doses of pain medication. 00:05:01.49\00:05:03.19 Because there was more going on around them, they weren't 00:05:03.22\00:05:06.77 so lonely any more, so we take away the loneliness 00:05:06.80\00:05:10.22 we often take away the pain, the anger, the guilt, take away 00:05:10.67\00:05:14.94 some of these things and we find that they have far less pain. 00:05:14.98\00:05:18.27 So when you are looking at pain, we are not just dealing 00:05:18.30\00:05:21.88 with the physical response in the skin, in the bones, 00:05:21.91\00:05:25.81 in the muscles, we are looking at the minds perception, 00:05:25.84\00:05:28.79 of why do I have this pain, and as we can rest in the Lord 00:05:28.82\00:05:32.43 with this pain as we can accept it as Job says: 00:05:32.47\00:05:36.50 " Though He slay me, yet will I serve Him." 00:05:36.53\00:05:39.89 We need to get that attitude we will find that our pain 00:05:39.92\00:05:42.98 often times will be going away with giving it over and saying 00:05:43.01\00:05:48.09 I am going to accept this and get a right attitude about it. 00:05:48.12\00:05:52.45 Dr. Thrash! 00:05:52.48\00:05:53.72 - We only treat acute pain with drugs such things as anesthesia 00:05:53.75\00:06:01.45 or when there is a severe injury with intense pain, those can be 00:06:01.48\00:06:07.94 properly treated with drugs, but it is easy to see why we 00:06:07.97\00:06:11.43 would not want to treat chronic pain with a drug. 00:06:11.46\00:06:14.29 The person becomes dependent on the drug and increasing 00:06:14.32\00:06:18.58 quantities of the drug are needed until the person 00:06:18.61\00:06:22.22 is hysterical without it, and may even develop a sort of 00:06:22.25\00:06:27.08 personality problem or become a hypochondriac, and they may 00:06:27.11\00:06:30.99 describe their pain as terrible or horrible, but when you look 00:06:31.02\00:06:35.10 at them, they look as if they are coping pretty well. 00:06:35.13\00:06:38.38 So what we need to do is to find ways that we can 00:06:38.41\00:06:42.70 relieve pain and make the person quite comfortable 00:06:42.73\00:06:45.94 without using a powerful drug medication, and some of these 00:06:45.97\00:06:50.61 very important ways to treat chronic pain are heat and cold. 00:06:50.64\00:06:57.44 Or even alternating heat and cold, where we apply the heat 00:06:57.47\00:07:01.38 treatment and then right away apply the cold, even in the same 00:07:01.41\00:07:04.77 treatment or we can apply heat for one type of pain and cold 00:07:04.80\00:07:09.66 for another type of pain, or it can even be that one person 00:07:09.69\00:07:14.57 in one type of pain will enjoy having heat, but another person 00:07:14.60\00:07:20.91 finds that not pleasant, so you must sometimes treat with 00:07:20.94\00:07:25.77 heat for the same kind of illness that the last person 00:07:25.80\00:07:32.44 you treated with cold and it was quite satisfactory. 00:07:32.47\00:07:35.34 Sinusitis should be treated with heat for the most part 00:07:35.43\00:07:40.69 they can sit in a tub of very hot water and that often 00:07:40.72\00:07:44.42 relieves their pain very nicely. 00:07:44.45\00:07:46.07 But cold applied to sinusitis will sometimes make it worse 00:07:46.10\00:07:51.61 but the occasional patient will find the very reverse, 00:07:51.64\00:07:55.78 cold applied makes it better. 00:07:55.81\00:07:58.19 Now heat can also be applied for such things as a sprained 00:07:58.22\00:08:02.96 ankle, but sprained ankles for the most part will be 00:08:02.99\00:08:07.10 better treated with the cold, an ice pack, or something of 00:08:07.13\00:08:11.88 that nature. 00:08:11.91\00:08:12.88 We also find that pressure is good, pressure can be applied 00:08:12.89\00:08:19.78 by the hand when a person has been acutely injured, sometimes 00:08:19.81\00:08:25.05 if you press on the place that they've been injured and where 00:08:25.08\00:08:28.94 they feel pain, it will help a lot. 00:08:28.97\00:08:31.76 You may have noticed that if a finger gets crushed and you 00:08:31.79\00:08:35.22 put pressure on it that that can help quite a lot, 00:08:35.25\00:08:38.32 and then is such areas as tennis elbow we can put pressure. 00:08:38.35\00:08:43.05 I have asked my granddaughter Melissa if she will help me show 00:08:43.08\00:08:47.90 you this kind of treatment for a tennis elbow. 00:08:47.93\00:08:50.42 This is Melissa Thrash, and Melissa is let us say that 00:08:50.45\00:08:55.42 she has been doing some kind of activity that causes a 00:08:55.45\00:09:01.20 rotational movement of her arm, we can easily prevent this kind 00:09:01.23\00:09:07.69 pain from developing by simply applying something like an 00:09:07.72\00:09:12.69 Ace bandage, or if you prefer you can purchase from a 00:09:12.72\00:09:17.90 drug store a little band called a tennis elbow band. 00:09:17.93\00:09:23.90 So this is simply wrapped around the arm in this way 00:09:23.93\00:09:28.19 and the person wears it doing whatever activity they are doing 00:09:28.22\00:09:31.77 whether is shoveling or hoeing, or whatever they may 00:09:31.80\00:09:35.37 be doing, and it's a very simple and nice treatment. 00:09:35.40\00:09:39.21 Then when they have finished with the activity, they simply 00:09:39.24\00:09:42.95 take the band off and that enables them to avoid getting 00:09:42.98\00:09:48.50 the tennis elbow. 00:09:48.53\00:09:49.77 Now another thing that you will find is a real problem 00:09:49.80\00:09:53.50 is that of arthritis, and people have pain in their hands 00:09:53.53\00:09:59.32 and especially when they wake up the next morning 00:09:59.35\00:10:02.46 they have pain in their hands, so I am going to ask Melissa 00:10:02.49\00:10:06.46 to show you how she would handle that with these gloves, 00:10:06.49\00:10:11.15 they look as if they are child's gloves, but they are actually 00:10:11.18\00:10:14.07 large enough for an adult, so the treatment is very simple, 00:10:14.10\00:10:20.20 we simply use a pressure on the hands so that the next 00:10:20.23\00:10:26.11 morning the person wakes up and they will tell you 00:10:26.14\00:10:29.49 that their hands feel much better having just had that 00:10:29.52\00:10:34.66 little bit of pressure on them. 00:10:34.69\00:10:36.11 Now another things that is very important is that of water, 00:10:36.14\00:10:43.83 you might think that water is not a good treatment for pain 00:10:43.86\00:10:49.03 but we find it quite excellent, I don't know exactly why it is 00:10:49.06\00:10:52.84 that it works all the time, and it doesn't work in every kind 00:10:52.87\00:10:56.35 of pain, but in most kinds of pains people will find it better 00:10:56.38\00:10:59.95 if they are simply well hydrated, and you can take 00:10:59.98\00:11:03.31 a glass full of water every ten minutes for an hour, 00:11:03.34\00:11:06.91 that's a glass full of water every ten minutes for an hour, 00:11:06.94\00:11:10.39 and it will often relieve such things as a headache or 00:11:10.42\00:11:14.57 an abdominal pain, or sometimes skeletal pain. 00:11:14.60\00:11:18.82 I think it may separate the nerve fibers that cause those 00:11:18.85\00:11:24.30 kinds of pains and make the person feel a lot better. 00:11:24.33\00:11:27.87 Now Don Miller is going to tell you about some other ways 00:11:27.90\00:11:33.62 you can handle pain, Don Miller! 00:11:33.65\00:11:36.18 - I talked a few minutes ago about helplessness and 00:11:36.21\00:11:39.07 loneliness, loneliness is the one that really seems to get us 00:11:39.10\00:11:42.05 the most. 00:11:42.08\00:11:43.88 Picture this scenario, a child is running down the street 00:11:43.91\00:11:47.07 active with the friends, falls and skins up the knee, 00:11:47.10\00:11:50.25 a little bit of blood, the child sees the blood, immediately is 00:11:50.28\00:11:53.76 scared, runs home to mama crying, it hurts terribly, 00:11:53.79\00:11:58.21 what does mama do? Mama holds the child, 00:11:58.24\00:12:01.16 kisses the child, says it will be all better, and it is amazing 00:12:01.19\00:12:04.47 just those words seem to make it all better, and then puts a 00:12:04.50\00:12:08.18 band aid on it and sends the child back out to play, 00:12:08.21\00:12:10.69 and the child is perfectly fine, doesn't even think about 00:12:10.72\00:12:13.63 it any more, until that night mama kisses baby to sleep 00:12:13.66\00:12:18.71 lays her down, covers her up, turns out the light and now 00:12:18.74\00:12:22.23 what's left, in a dark room, no stimulation, and what happens 00:12:22.26\00:12:27.21 the pain comes all back again. 00:12:27.24\00:12:28.87 Because that is where the mind is, all during the day time 00:12:28.90\00:12:32.23 you had mama sitting there comforting the child, 00:12:32.26\00:12:35.21 you had activities going on all around, you had the other 00:12:35.24\00:12:38.57 playmates, things to do, now there is nothing left to do 00:12:38.60\00:12:43.24 but focus on the pain, and we need to keep from focusing 00:12:43.27\00:12:47.75 on the pain. 00:12:47.78\00:12:49.05 There are ways of doing this, I haven't quite gotten to this 00:12:49.08\00:12:52.99 place yet where you are going into the dentist office, 00:12:53.02\00:12:55.11 and I once tried it, I went into the dentist office and said 00:12:55.14\00:12:57.85 I can do it without the pain medication, thank you, 00:12:57.88\00:13:01.17 and so they put me way back there in the chair and he 00:13:01.20\00:13:04.76 opened my mouth and he went in there with that drill... 00:13:04.79\00:13:06.83 as soon as I heard that zzzz! I tensed and as soon as it 00:13:06.86\00:13:10.69 touched my tooth, he took it back out of my mouth 00:13:10.72\00:13:13.28 and he gave me the shot because everything in my body told him 00:13:13.31\00:13:16.28 that I was really going to be a bad patient that day, 00:13:16.31\00:13:18.94 but if you would find some way to divert the mind to other 00:13:18.97\00:13:24.36 stimulation. 00:13:24.39\00:13:25.36 You only can perceive so many things in your mind, 00:13:25.37\00:13:29.16 right now you are sitting there and you are not really 00:13:29.19\00:13:31.72 aware of the fact that you're back is pushed against the sofa 00:13:31.75\00:13:37.12 or chair and the watch band is pressing against your wrist 00:13:37.15\00:13:40.52 and the top of your socks are against your leg, you don't 00:13:40.55\00:13:43.73 feel or notice all of these things although you do have 00:13:43.76\00:13:46.88 true perception, so if we can get more stimulation going in 00:13:46.91\00:13:51.13 that's pleasurable, we will have a less tendency to focus on 00:13:51.16\00:13:55.82 the pain that's also coming in, and there are ways 00:13:55.85\00:13:58.17 of doing this. 00:13:58.20\00:13:59.17 One of these days I'm going to try going to a dentist office 00:13:59.18\00:14:05.09 with a friend and say as soon as that guy opens my mouth 00:14:05.12\00:14:08.25 and heads for it with the drill, without the Novocain 00:14:08.28\00:14:10.87 I want you to take something as simple as a hairbrush 00:14:10.90\00:14:13.61 and start stroking perhaps my arm or my leg, or you can reach 00:14:13.64\00:14:18.68 up and just take your fingers and give me a little scalp 00:14:18.71\00:14:21.75 massage, and at the same time perhaps take some type of a 00:14:21.78\00:14:25.45 perfume and put it on a handkerchief and let me be 00:14:25.48\00:14:28.46 smelling, so now I've got some olfactory senses coming 00:14:28.49\00:14:31.80 in here and I've got some senses here from touch, 00:14:31.83\00:14:34.62 and perhaps I can take a CD and turn on some music 00:14:34.65\00:14:38.78 so I'm hearing some music going on at the same time, 00:14:38.81\00:14:41.17 with head phones it is even better, you can't hear that zzz! 00:14:41.20\00:14:44.86 going on because basically if you have been inside a 00:14:44.89\00:14:47.48 dentist office, even sitting out into the waiting area when 00:14:47.51\00:14:51.51 you hear the drill you tense up, remember we talked 00:14:51.54\00:14:54.50 about this tensing up, which is one thing that amplifies pain. 00:14:54.53\00:14:58.08 Another thing that you can do is have some type of little 00:14:58.11\00:15:00.64 massage technique, now if someone can give you a massage 00:15:00.67\00:15:03.52 or you can take your foot, this is a little foot massager 00:15:03.55\00:15:07.33 and you just take your foot and you slide it back and forth. 00:15:07.36\00:15:10.02 This gives you a very nice sensation which will sort of 00:15:10.05\00:15:13.67 overwhelm the other sensations going into the body, and so if 00:15:13.70\00:15:17.18 you find ways of sort of over loading the brain with other 00:15:17.21\00:15:22.30 sensations will find that that pain that is usually rather 00:15:22.33\00:15:26.82 acute if we are just focusing on that, will seem to go away. 00:15:26.85\00:15:30.70 I had an experience one time I was in Ukraine in the winter 00:15:30.73\00:15:35.99 time and a neighbor of the place I was working had fallen 00:15:36.02\00:15:40.63 and severely twisted and wrenched his back, 00:15:40.66\00:15:43.97 this big Ukrainian man, 00:15:44.01\00:15:46.90 big burly guy crying, he was in so much pain 00:15:46.94\00:15:50.27 we got him laid down on this couch of the nearest house 00:15:50.30\00:15:54.54 he could not even go home he was in so much pain 00:15:54.57\00:15:56.53 so we put him on the couch of the nearest home and here he was 00:15:56.56\00:16:00.54 and I had a doctor from Ukraine with me, she examined him to 00:16:00.57\00:16:04.94 make sure his back wasn't broken because he walked in 00:16:04.97\00:16:07.12 so it wasn't broken, but now what to do with all this pain 00:16:07.15\00:16:11.15 and so I said well go out and get me an icecicle, 00:16:11.18\00:16:15.21 so they went out and broke a big icecicle, and they are big 00:16:15.24\00:16:18.12 icecicles in Ukraine, big Texas size icecicles, and brought 00:16:18.15\00:16:21.39 this big thing in here and I got a pot of hot water and I 00:16:21.42\00:16:23.57 dipped the end in and sort of smoothed off the edges 00:16:23.60\00:16:25.90 and then I started massaging that back area, well now you had 00:16:25.93\00:16:31.48 a different sensation going on that was cold, 00:16:31.51\00:16:34.32 but at the time I was taking another part of his body 00:16:34.35\00:16:37.91 and I was massaging another part of his body, I forget 00:16:37.94\00:16:40.55 whether it was his shoulder or his head, but I was 00:16:40.58\00:16:43.17 massaging this other part of the body while I was massaging 00:16:43.20\00:16:46.36 the back with the ice and pretty soon he was complaining because 00:16:46.39\00:16:50.26 it was cold. 00:16:50.29\00:16:51.62 But then he got up, he didn't have any more pain, 00:16:51.65\00:16:54.53 not that the situation was fixed he still had a sprained back 00:16:54.56\00:16:58.68 but the ice had taken the pain away enough that he could 00:16:58.71\00:17:01.70 go home that night, pain free and deal with it that way. 00:17:01.73\00:17:05.46 So as we find ways of diverting the mind away from ourselves 00:17:05.49\00:17:11.13 at night time when we are all alone is when we hurt the most, 00:17:11.16\00:17:14.37 so we need to be doing something and I like that saying if you 00:17:14.40\00:17:18.22 can't sleep don't count sheep talk to the Shepherd. 00:17:18.25\00:17:20.63 When you have pain talk to the one who knows all about 00:17:20.66\00:17:24.00 your pain, he went through the pain too, and He'll help 00:17:24.03\00:17:27.75 you through it and so as we talk to Him, as we get other 00:17:27.79\00:17:30.92 senses involved in this process we'll find that pain will be 00:17:30.95\00:17:36.03 saying Sayonara, and that's what we want it to do, 00:17:36.06\00:17:38.96 Dr. Thrash. 00:17:38.99\00:17:40.22 - Another kind of very good treatment for pain is that of 00:17:40.25\00:17:46.54 counter irritation, you may have heard your grandmother 00:17:46.57\00:17:50.25 talk about her liniments, or some neighbor about their 00:17:50.28\00:17:55.53 materials that they rub on come from a store such as Capsaicin 00:17:55.56\00:18:00.47 or Cayenne, it's a red pepper extract and you can get it 00:18:00.50\00:18:04.75 from almost any store, and this kind of red pepper when you 00:18:04.78\00:18:10.68 put it on the skin, will help you greatly with the 00:18:10.71\00:18:14.55 counter irritation. 00:18:14.58\00:18:15.64 Now there are many other liniments, liniments made from 00:18:15.67\00:18:18.50 camphor, from Menthol, and some other counter irritants 00:18:18.53\00:18:24.58 they are a little irritating or icy feeling on the skin 00:18:24.61\00:18:29.31 but when you use it in times of pain, it can actually take 00:18:29.34\00:18:35.60 the pain away. 00:18:35.63\00:18:36.60 Now another type of counter irritation is something 00:18:36.61\00:18:39.35 like this, you may not have seen this because I picked this up in 00:18:39.38\00:18:43.42 Korea and I like it very much because it can be 00:18:43.45\00:18:46.84 self administered if you have a pain in your back you can 00:18:46.87\00:18:51.13 just do this kind of thing and it helps enormously 00:18:51.16\00:18:55.83 for the back pain, and then you can just do it on the other 00:18:55.86\00:18:58.96 side as well. 00:18:58.99\00:19:00.07 But let's say that your pain is not in your back, 00:19:00.10\00:19:02.61 your pain is in your foot, or in your abdomen, will this help 00:19:02.64\00:19:06.51 with that?
