Welcome back. 00:06:05.04\00:06:08.88 I'm glad you joined us. 00:06:08.91\00:06:10.01 If you just tuned in, we were talking about pain tonight. 00:06:10.04\00:06:13.03 We have a specialist 00:06:13.06\00:06:14.09 on pain management Dr. Dennis Ford. 00:06:14.12\00:06:16.79 We are going to introduce him tonight. 00:06:16.82\00:06:18.18 Dennis, thank you for joining us. 00:06:18.21\00:06:19.42 Thank you for asking me. 00:06:19.45\00:06:20.48 Now, tell us, tell me about your journey. 00:06:20.51\00:06:22.80 What got you interested in pain management? 00:06:22.83\00:06:25.04 Now it's a specialty in medicine. 00:06:25.07\00:06:27.54 Oh, about 1995 I woke up one day 00:06:27.57\00:06:30.93 with a crick in my neck 00:06:30.96\00:06:32.00 and I'm thinking, well, this will go away. 00:06:32.03\00:06:34.14 And about two months later 00:06:34.17\00:06:35.20 the pain started going down my arm. 00:06:35.23\00:06:37.33 And I did different and I started having severe pain 00:06:37.36\00:06:40.60 pain score about 8 and so then I went ahead 00:06:40.63\00:06:43.67 and had MRI, I had two herniated cervical disc. 00:06:43.70\00:06:47.48 So I did chiropractic treatment, 00:06:47.51\00:06:49.12 I did acupuncture and still having 00:06:49.15\00:06:52.25 significant amount of pain 00:06:52.28\00:06:54.07 and then fortunately one of my friend said, 00:06:54.10\00:06:56.08 there is another pain doctor that I had been sitting, 00:06:56.11\00:06:58.41 did two epidurals on me, it took my pain away, so like--- 00:06:58.44\00:07:02.19 Explain what is an epidural for everybody? 00:07:02.22\00:07:04.15 Epidural is a steroid injection it's where you-- 00:07:04.18\00:07:06.72 if you have a herniated disc, 00:07:06.75\00:07:08.66 you put a steroid under x-ray guidance 00:07:08.69\00:07:11.19 next to that where the disc is, 00:07:11.22\00:07:12.49 it helps to take the inflammation away. 00:07:12.52\00:07:14.12 Okay, okay, but what happened after that? 00:07:14.15\00:07:16.62 And so for the first week 00:07:16.65\00:07:19.18 I felt like, man, my pain is gone 00:07:19.21\00:07:21.74 and it slowly started coming back 00:07:21.77\00:07:23.98 and then I had a second one and fortunately for me 00:07:24.01\00:07:26.90 I have been-didn't get for about 17 years now. 00:07:26.93\00:07:29.48 Wow, so evidently that calming down 00:07:29.51\00:07:33.31 the inflammation helped your body heal. Yes. 00:07:33.34\00:07:35.66 And then everything is been good. 00:07:35.69\00:07:36.72 I never repeated the MRI but I don't really hurt anymore 00:07:36.75\00:07:39.03 so I avoided surgery and so far so good. 00:07:39.06\00:07:41.69 So, that's what got your interest. 00:07:41.72\00:07:43.30 Now, you've gone to medical school 00:07:43.33\00:07:44.77 and had some training. Yes. 00:07:44.80\00:07:45.91 Tell us about some of the other trainings. 00:07:45.94\00:07:47.37 So, I did family practice early, 00:07:47.40\00:07:49.12 I got to family practice throughout the city 00:07:49.15\00:07:50.92 and then I decided to do anesthesiology, 00:07:50.95\00:07:53.26 so I went to Vanderburgh and did anesthesiology residency. 00:07:53.29\00:07:56.72 I did anesthesiology for 18 years 00:07:56.75\00:07:58.62 and towards the end I started doing pain management 00:07:58.65\00:08:00.46 because I had the pain. 00:08:00.49\00:08:02.02 And I saw there was a need there to help other people. 00:08:02.05\00:08:04.34 Okay, so and how long you have been 00:08:04.37\00:08:06.30 practicing pain management now? 00:08:06.33\00:08:07.83 For about 13, 14 years. 00:08:07.86\00:08:10.20 Now, you have to, I know that when I went to school, 00:08:10.23\00:08:12.94 you know, you go and do your internal medicine 00:08:12.97\00:08:14.99 or whatever, you don't just do 00:08:15.02\00:08:16.66 just specifically pain, do you? 00:08:16.69\00:08:19.00 There is a fellowship now where you do, 00:08:19.03\00:08:20.90 we did anesthesiology 00:08:20.93\00:08:22.41 or physiatry residency in pain. Okay. 00:08:22.44\00:08:26.56 Because I know it's a growing field 00:08:26.59\00:08:27.73 and you can see the reason we are talking about it 00:08:27.76\00:08:29.85 because one in four people have it. Wow. 00:09:02.94\00:09:06.37 So that's far away. 00:09:06.40\00:09:08.00 Yeah, go ahead Mike, you are on with Dr. Ford. 00:09:08.03\00:09:11.14 Okay, I was curious about. 00:09:11.17\00:09:12.72 I've had a back like-- 00:09:12.75\00:09:15.64 everybody is saying it's a pinched nerve. 00:09:15.67\00:09:18.20 And I was in the wreck at 18, 00:09:18.23\00:09:20.15 so I'm 43 and basically I'm just-- 00:09:20.18\00:09:23.27 my guess, my question is 00:09:23.30\00:09:24.70 could that possibly be most of my problem? 00:09:24.73\00:09:27.22 It could be. 00:09:27.25\00:09:28.28 There is a pain radiated down your leg 00:09:28.31\00:09:29.83 or is it localized mainly to your back. 00:09:29.86\00:09:32.50 It varies, it does go down on the leg and back. 00:09:32.53\00:09:34.85 I had times where, you know, 00:09:34.88\00:09:37.71 I felt like I couldn't even get up, 00:09:37.74\00:09:39.69 you know, like I was 90 years old. 00:09:39.72\00:09:41.77 And this is being going on for how long? 00:09:41.80\00:09:44.24 Ah, for a while and I mean, I really didn't looked into, 00:09:44.27\00:09:47.82 you know, about go to a chiropractor, 00:09:47.85\00:09:50.46 and some people say you might need back surgery and all this. 00:09:50.49\00:09:53.23 And, you know, I kind of disagree. 00:09:53.26\00:09:56.46 I thought, well, I don't need to go 00:09:56.49\00:09:57.52 through all more pain and stuff 00:09:57.55\00:09:59.05 but basically I was just kind I guess asking 00:09:59.08\00:10:01.93 that it sound like a pinch nerve to you? 00:10:01.96\00:10:04.18 It could very well be. 00:10:04.21\00:10:05.73 Probably what you need to do is to start off 00:10:05.76\00:10:07.99 I don't know what you have done so far, 00:10:08.02\00:10:09.64 start of with an x-ray, 00:10:09.67\00:10:10.77 and may be probably lead to an MRI of your back 00:10:10.80\00:10:14.05 and then you could do different options depending 00:10:14.08\00:10:16.66 you could start off with chiropractic, physical therapy. 00:10:16.69\00:10:19.80 If you don't get any better, you can see someone like me 00:10:19.83\00:10:22.14 for possibly an epidural steroid injection, 00:10:22.17\00:10:24.92 to see if we can get the pain go away 00:10:24.95\00:10:27.33 and last resort would be surgery if nothing else work. 00:10:27.36\00:10:30.57 But I'm hopeful, lot of times we are very successful 00:10:30.60\00:10:33.24 in one of these options of getting you pain to go away. 00:10:33.27\00:10:36.29 Right, I have already turned to lot of people who-- 00:10:36.32\00:10:38.94 my girlfriend she had that back surgery 00:10:38.97\00:10:40.90 and she is still having chronic pain yet. 00:10:40.93\00:10:44.06 I've always heard people say, 00:10:44.09\00:10:45.35 you know, don't do it, you know, majority. 00:10:45.38\00:10:48.06 That's the last resort typically. 00:10:48.09\00:10:50.91 It's 60% success rate up to 40% failure rate 00:10:50.94\00:10:54.38 with back surgery. Well, what was that? 00:10:54.41\00:10:57.38 Around 60% success rate up to 40% failure 00:10:57.41\00:11:00.37 with back surgery. Thank you. 00:40:48.45\00:40:49.75 And I'm very interested 00:40:49.78\00:40:52.59 because I've been a chronic pain sufferer 00:40:52.62\00:40:55.63 for many, many years now. 00:40:55.66\00:40:58.24 What I'm suffering from is diabetic neuropathy 00:40:58.27\00:41:03.79 and diabetic neuropathy 00:41:03.82\00:41:06.22 when I've had all these different pills, 00:41:06.25\00:41:08.79 they said that actually that 00:41:08.82\00:41:10.47 there is four different kinds of neuropathy 00:41:10.50\00:41:14.08 and I was one of the luckiest gentlemen 00:41:14.11\00:41:15.94 to be able to be blessed with all four 00:41:15.97\00:41:19.87 and they said it's actually the worst case 00:41:19.90\00:41:24.16 that they have actually seen in 27 years of practice. 00:41:24.19\00:41:29.41 My problem is I'm seeing a pain management doctor 00:41:29.44\00:41:34.70 and they've got me on the duragesic pain patch, 00:41:34.73\00:41:40.39 and then they got me on the Lodine, 00:41:40.42\00:41:42.46 they got me on hydrocodone, they got me on Cymbalta, 00:41:42.49\00:41:46.28 all these differing things and it's not getting any better, 00:41:46.31\00:41:49.89 it's just getting worse and they are telling me 00:41:49.92\00:41:51.75 that there is actually no cure for neuropathy. 00:41:51.78\00:41:56.24 I don't have the quality of life that I'm having, 00:41:56.27\00:41:59.62 I mean I have no quality of life or though. 00:41:59.65\00:42:02.05 Okay, Mark, we're going to let 00:42:02.08\00:42:03.11 Dr. Ford give you some suggestion 00:42:03.14\00:42:04.84 and how he handles this type of situation. Sure. 00:42:04.87\00:42:08.42 There is one natural thing you may consider 00:42:08.45\00:42:10.10 adding to that would be R-lipoic acid, 00:42:10.13\00:42:13.82 Lipoic acid has been proven, 00:42:13.85\00:42:15.45 it's a natural substance that is been proven. 00:42:15.48\00:42:18.37 You can take up to 300 milligrams twice a day 00:42:18.40\00:42:20.44 to see if that will help to decrease the neuropathy pains. 00:42:20.47\00:42:23.19 It sound like you are on some Cymbalta 00:42:23.22\00:42:25.17 and the other one I'm sure you are probably on Lyrica, 00:42:25.20\00:42:27.85 those are two for diabetic neuropathy. 00:42:27.88\00:42:30.57 They put me on the Lyrica and it didn't work for me, 00:42:30.60\00:42:33.58 so they got me on started 600 mg on the Lodine now. 00:42:33.61\00:42:38.18 Okay, how is your hemoglobin A1c 00:42:38.21\00:42:40.46 and how well are your blood sugars controlled. 00:42:40.49\00:42:43.60 Blood sugars are doing well, 00:42:43.63\00:42:46.27 the last I went to see that I had was a 6.3. 00:42:46.30\00:42:50.53 Well, it sounds like you are doing pretty good there. 00:42:50.56\00:42:54.16 The only thing I would suggest would keep, 00:42:54.19\00:42:56.73 keep that as best control as you can 00:42:56.76\00:42:59.19 and there is one that I can say 00:42:59.22\00:43:01.80 the Alpha-lipoic or R-lipoic acid, 00:43:01.83\00:43:05.04 lipoics it's a natural thing 00:43:05.07\00:43:07.91 you get in health food store or in internet, 00:43:07.94\00:43:10.46 it's been shown to help decrease neuropathy pains. 00:43:10.49\00:43:13.86 It's called R-lipoic. Good. 00:47:44.98\00:47:48.12 And I was 41-years old 00:47:48.15\00:47:50.23 when I had to have spinal fusion in L4 L5 00:47:50.26\00:47:55.36 and S1 in lower spinal. 00:47:55.39\00:47:58.45 And couple of years later, 00:47:58.48\00:48:01.38 I had problems and I had to go in there 00:48:01.41\00:48:03.49 and take some of it out that hardware 00:48:03.52\00:48:07.12 and then I had a tangent 00:48:07.15\00:48:08.64 a couple years later after that. 00:48:08.67\00:48:11.22 And I'm in chronic pain everyday in my life. 00:48:11.25\00:48:13.76 I'm under pain management. 00:48:13.79\00:48:15.83 I've had shots, I've had epidurals 00:48:15.86\00:48:18.62 and I just live in chronic pain everyday 00:48:18.65\00:48:21.47 in my life without medication. 00:48:21.50\00:48:23.54 I know they tell me, they tried everything 00:48:23.57\00:48:25.30 they could to heal me. 00:48:25.33\00:48:27.72 It's just something I know I'm gonna have to live with, 00:48:27.75\00:48:30.59 you know, that I don't understand 00:48:30.62\00:48:33.67 why all these surgeries didn't help me. 00:48:33.70\00:48:37.47 I could say-- 00:48:37.50\00:48:39.90 up to about 60% of back surgeries 00:48:39.93\00:48:41.94 are successful enough, 00:48:41.97\00:48:43.20 potentially up to 40% are not successful. 00:48:43.23\00:48:45.84 Back is one place that God made on our body 00:48:45.87\00:48:48.04 that man hasn't really gotten great at fixing, 00:48:48.07\00:48:50.78 by taking your gallbladder out 00:48:50.81\00:48:52.06 or doing appendix surgery or your tonsils. 00:48:52.09\00:48:56.65 So have you tried spinal stimulator? 00:48:56.68\00:48:59.70 Yes, I had the tangent, 00:48:59.73\00:49:02.73 and some nerve stimulator thing, 00:49:02.76\00:49:07.50 where I'm actually full button, but what it is. 00:49:07.53\00:49:09.92 No, I'm talking, has your pain doctor talked to you about 00:49:09.95\00:49:12.94 trying the spinal stimulator trial. 00:49:12.97\00:49:14.93 It's where you-- we put it 00:49:14.96\00:49:17.04 spinal stimulator in the epidural space. 00:49:17.07\00:49:19.50 It helps, it's like a tangent, 00:49:19.53\00:49:21.26 on about 1000 times better patients tell me. 00:49:21.29\00:49:23.98 It helps pain from your back down your legs, 00:49:24.01\00:49:26.03 all the way down your legs, it overrides the pain. 00:49:26.06\00:49:29.54 I had to take it out after couple of months, 00:49:29.57\00:49:32.89 it just wasn't working. 00:49:32.92\00:49:34.21 It didn't help you? Okay. 00:49:34.24\00:49:35.99 That was one option that 00:49:36.02\00:49:38.00 Jerry Lewis comedian has got that he loves, 00:49:38.03\00:49:40.12 says but everybody is different, 00:49:40.15\00:49:41.34 sometimes it helps, sometimes it doesn't. 00:49:41.37\00:49:43.72 One other option might be for you would be 00:49:43.75\00:49:45.60 like a spinal pump, may be if your-- 00:49:45.63\00:49:49.30 if the pain medicine doesn't control, 00:49:49.33\00:49:50.84 that's like a last resort top option. 00:49:50.87\00:49:54.49 Yeah, that I know, really, really cheerful 00:49:54.52\00:49:57.80 where he is looking at, not really know. 00:49:57.83\00:50:01.53 But I like I said I've been on pain medicines now for years 00:50:01.56\00:50:05.21 and I really don't, you know, I was a young woman, 00:50:05.24\00:50:09.08 41 years old to me was young to have all that done. 00:50:09.11\00:50:13.26 And still I suffer. 00:50:13.29\00:50:16.16 Yes, I think probably in every once a while, 00:50:16.19\00:50:19.40 every few years or so you sometimes 00:50:19.43\00:50:22.93 we change from one pain medicine to another, 00:50:22.96\00:50:24.73 sometimes yours body becomes tolerant to it, 00:50:24.76\00:50:27.14 and you have to change it to different one 00:50:27.17\00:50:28.95 to get a better pain relief, so. 00:50:28.98\00:50:31.11 Yeah, he changed so many times. 00:50:31.14\00:50:33.29 It sound like you've got a good pain doctor, 00:50:33.32\00:50:36.88 in the pain in chronic pain, 00:50:36.91\00:50:39.36 I hate to say this, but if you get your pain scores 00:50:39.39\00:50:41.37 decrease from say ten or five to seven range, 00:50:41.40\00:50:45.09 we consider that success 00:50:45.12\00:50:46.43 because chronic pain is so difficult to treat 00:50:46.46\00:50:48.40 especially if you had several back surgeries. 00:50:48.43\00:50:50.95 I can lay on my side 00:50:50.98\00:50:52.69 and my way it feels like somebody is ripping them out. 00:50:52.72\00:50:56.04 Well, I can lay on my back 00:50:56.07\00:50:57.85 and it feels like somebody is trying to tear my spinal, 00:50:57.88\00:51:00.63 now that's the only way I know how to explain the pain. 00:51:00.66\00:51:05.77 I can't sleep for long, I can't stand for long, 00:51:05.80\00:51:08.92 and I know they are doing all they can to control it 00:51:08.95\00:51:11.16 but I didn't know, 00:51:11.19\00:51:12.65 you know, if they will ever get it under control. 00:51:12.68\00:51:16.66 Wanda, you know sometimes no mater what medicine does, 00:51:16.69\00:51:20.22 it can't fix the problems. 00:51:20.25\00:51:21.76 You know sometimes there's nerves that can't be fixed, 00:51:21.79\00:51:24.19 even though we do the lifestyle changes 00:51:24.22\00:51:26.09 and everything modern medicine out there, 00:51:26.12\00:51:28.16 this isn't a perfect world. 00:51:28.19\00:51:30.08 And that's why especially in your situation, 00:51:30.11\00:51:32.54 we want to encourage you, 00:51:32.57\00:51:33.73 you know, to keep your faith up, 00:51:33.76\00:51:35.67 realize that there is a relationship with the God 00:51:35.70\00:51:38.03 that can make things perfect in His time. 00:51:38.06\00:51:40.33 Things like laughter and music and serving others 00:51:40.36\00:51:44.06 and being happy might make some endorphins 00:51:44.09\00:51:46.32 that might help a little bit, 00:51:46.35\00:51:47.88 so we will keep you in our prayers 00:51:47.91\00:51:50.28 as time moves forward here, okay. 00:51:50.31\00:51:52.82 Okay, thank you so much.
