Hello and welcome to Health for A Lifetime. 00:00:50.72\00:00:52.28 Today, we're gonna talk about something that 00:00:52.31\00:00:54.48 maybe many of our viewers have experienced 00:00:54.66\00:00:57.28 and that is looking forward to that first baby. 00:00:57.31\00:01:00.08 Being excited about that new bundle of joy 00:01:00.11\00:01:02.74 that's coming into your home. 00:01:02.77\00:01:04.10 Sometimes people say, Why wait? 00:01:04.13\00:01:07.59 Maybe something should come early. 00:01:07.62\00:01:09.24 But, we're going to learn today that having a baby too soon 00:01:09.27\00:01:13.15 is really quite damaging, 00:01:13.18\00:01:14.90 and how to avoid that, if possible. 00:01:14.93\00:01:17.15 Talking with us today about that is Dr. Neil Nedley. 00:01:17.18\00:01:19.77 Welcome, Dr. Nedley. 00:01:19.80\00:01:20.94 Thank you. Good to be here. 00:01:20.97\00:01:22.49 You're a friend to this program. 00:01:22.52\00:01:24.15 Some may have just joined us, but many times, we've talked. 00:01:24.18\00:01:26.89 You're actually an internal medicine specialist. 00:01:26.92\00:01:29.59 You practice in Ardmore, Oklahoma. - Correct. 00:01:29.62\00:01:31.08 You see people that have just had babies? 00:01:31.11\00:01:35.46 Normally, it's a pediatric type person or an OB type doctor 00:01:35.49\00:01:39.70 that sees these types of people that have had 00:01:39.73\00:01:41.75 births where there's a premature birth. 00:01:41.78\00:01:44.40 But how would you as an internal medicine relate to this? 00:01:44.43\00:01:48.49 Well, I often see many of these pregnant women in my 00:01:48.52\00:01:52.31 internal medicine office because they have 00:01:52.34\00:01:54.36 internal medicine problems. - Okay. 00:01:54.39\00:01:56.16 And, of course, that increases the risk of premature births. 00:01:56.19\00:02:00.01 And so, we're trying to do everything possible to 00:02:00.04\00:02:03.31 prevent the premature delivery. 00:02:03.34\00:02:05.49 So, we want to get that mom healthy; 00:02:05.52\00:02:07.70 all the different things we'll talk that about today. 00:02:07.73\00:02:09.73 Now this is a concern 00:02:09.76\00:02:10.86 among researchers here in America, isn't it? 00:02:10.89\00:02:13.38 It is a concern. In fact, just very recent studies have shown 00:02:13.41\00:02:18.67 that it's a growing problem in America. 00:02:18.70\00:02:21.59 In fact, we have a graphic that illustrates 00:02:21.62\00:02:24.63 that US health leaders are trying to craft a blueprint on 00:02:24.66\00:02:28.41 how to cut the US premature birth rate. 00:02:28.44\00:02:32.13 The problem with it is that so many people, 00:02:32.16\00:02:36.03 so many kids are being born premature now 00:02:36.06\00:02:39.22 In fact, it's increased by 20 percent since 1990. 00:02:39.25\00:02:44.21 Half a million babies are born premature 00:02:44.24\00:02:47.93 in this country every year. 00:02:47.96\00:02:50.17 And that means before 37 weeks. 00:02:50.20\00:02:52.13 So it's not just one week premature 00:02:52.16\00:02:53.80 or two weeks premature. 00:02:53.81\00:02:54.86 To actually get the definition of prematurity 00:02:54.87\00:02:57.79 you have to be more than four weeks early. 00:02:57.86\00:03:00.32 And, if you are more than four weeks early, 00:03:00.35\00:03:03.74 that increases your risk of a host of problems later on. 00:03:03.77\00:03:07.64 I know this isn't part of it, but what about coming late? 00:03:07.67\00:03:09.93 Is that a problem, too? 00:03:09.96\00:03:11.17 Coming late can be a problem. 00:03:11.20\00:03:12.97 Particularly if it's over two weeks late. 00:03:13.00\00:03:14.99 In some studies, it can increase the risk of some problems 00:03:15.02\00:03:19.22 However, that is not near as common a problem in America 00:03:19.25\00:03:23.62 as coming early; which is the far more common problem. 00:03:23.65\00:03:26.87 So, when they come premature, there also is the problem of... 00:03:26.90\00:03:31.41 Is it something to do with weight? 00:03:31.44\00:03:33.69 It is. Actually, if they're coming premature, 00:03:33.72\00:03:38.33 there actually is low birth weight associated with that. 00:03:38.36\00:03:42.39 And when you have low birth weight 00:03:42.42\00:03:43.98 then that also increases your risk of a number of problems. 00:03:44.01\00:03:46.85 So, let's talk, then about premature 00:03:46.88\00:03:50.82 birth, but more prenatal influences. 00:03:50.85\00:03:55.18 What kind of things are influencing this? 00:03:55.21\00:03:59.19 Identifying those would be one thing. 00:03:59.22\00:04:02.56 It may be obvious how to avoid them if we just identify them. 00:04:02.59\00:04:06.34 But, let's talk about those. 00:04:06.44\00:04:07.50 Well, some of the prenatal influences that could lead to 00:04:07.71\00:04:10.88 premature birth are going to be smoking, drinking, 00:04:10.91\00:04:15.47 more in vitro fertilization. 00:04:15.51\00:04:18.74 That's one of the reasons why it's increased. 00:04:18.97\00:04:20.92 The in vitro fertilization; 00:04:20.95\00:04:23.02 particularly when you have multiple pregnancies. 00:04:23.05\00:04:25.39 That also increases the risk 00:04:25.42\00:04:27.32 that prematurity is going to come about. 00:04:27.33\00:04:29.45 So, how does that work with the in vitro fertilization? 00:04:29.48\00:04:31.98 Is it not as closely... 00:04:32.01\00:04:34.20 Is it not as firmly grounded there in the... 00:04:34.21\00:04:37.48 Why would that be premature? 00:04:37.51\00:04:40.10 Well, often they're using more than one sperm 00:04:40.13\00:04:45.21 and more than one egg to do the in vitro fertilizations 00:04:45.24\00:04:49.02 so they get a pregnancy. 00:04:49.05\00:04:51.32 And so, to increase the chance that they'll have a pregnancy 00:04:51.35\00:04:56.00 often there can be two or three babies born; four or five. 00:04:56.01\00:05:01.25 That's where you get the seven babies born, eight babies born. 00:05:01.29\00:05:06.44 And the more babies that are inside the womb 00:05:06.47\00:05:09.73 of course, the bigger that womb gets. 00:05:09.76\00:05:11.78 And the larger it gets, the greater the risk that it's gonna 00:05:11.81\00:05:15.24 try to expel those contents before it's time. 00:05:15.27\00:05:19.41 And then smoking and alcohol? 00:05:19.44\00:05:20.65 How does that cause premature? 00:05:20.68\00:05:22.17 Do you know the mechanism of that? 00:05:22.20\00:05:24.54 There have been studies in regards to the mechanism. 00:05:24.57\00:05:28.16 It's actually more than one mechanism. 00:05:28.19\00:05:30.16 There's over 2000 chemicals in cigarette smoke. 00:05:30.20\00:05:33.32 Some of those chemicals can actually 00:05:33.35\00:05:37.11 lead to uterine contractions; 00:05:37.15\00:05:39.25 can lead to decreased circulation of certain portions 00:05:39.29\00:05:42.40 of the uterus. 00:05:42.44\00:05:43.58 One of the reasons why fibroids of the uterus 00:05:43.61\00:05:46.28 also can increase the risk of premature delivery 00:05:46.31\00:05:49.31 because of the decreased circulation in that area 00:05:49.34\00:05:54.05 and smoking does affect the circulation. 00:05:54.08\00:05:56.52 So, I'm sure second hand smoke would as well. 00:05:56.55\00:05:58.52 Second hand smoke does, to a certain extent. 00:05:58.55\00:06:01.42 So, if you're not a smoker and you're carrying a baby 00:06:01.43\00:06:04.39 you don't want your husband to smoke around you either. 00:06:04.40\00:06:07.15 Okay. And then alcohol? 00:06:07.18\00:06:08.35 Yes. Alcohol also has a significant role to play. 00:06:08.38\00:06:12.36 Alcohol increases the risk of blood pressure problems. 00:06:12.39\00:06:15.81 A lot of people are unaware of the fact 00:06:15.82\00:06:17.72 that just two ounces of alcohol a day 00:06:17.75\00:06:19.73 is all it takes to raise a woman's blood pressure. 00:06:19.76\00:06:22.47 And when she increases her blood pressure 00:06:22.50\00:06:26.13 it increases the risk of premature delivery. 00:06:26.16\00:06:28.75 Okay, so, no smoking, no drinking and 00:06:28.78\00:06:33.84 then also watching this 00:06:33.87\00:06:35.76 fertility things in terms of in vitro fertilization. 00:06:35.79\00:06:41.50 What are some other causes of premature... 00:06:41.51\00:06:44.54 Well, maternal obesity. 00:06:44.57\00:06:45.80 Maternal obesity? 00:06:45.83\00:06:47.56 That's right. And so, when the woman is overweight, she 00:06:47.93\00:06:50.67 increases her risk of actually having an underweight baby. 00:06:50.70\00:06:55.38 How does that work? 00:06:55.41\00:06:56.51 Maternal obesity also increases the risk of high blood pressure. 00:06:56.54\00:07:00.74 And there's probably some other mechanisms involved, as well, 00:07:00.75\00:07:03.88 in that process. 00:07:03.91\00:07:05.62 We recommend, actually if someone is overweight and 00:07:05.65\00:07:09.75 they're having a baby they don't need to gain as much weight 00:07:09.78\00:07:14.28 as someone who is of normal weight. 00:07:14.31\00:07:17.05 In fact, a recent obstetrics task force looked at this 00:07:17.08\00:07:22.32 and a person who is obese at the time that they conceive 00:07:22.35\00:07:28.13 they really should only gain about 15 pounds 00:07:28.16\00:07:30.96 during their pregnancy or maybe a little more. 00:07:30.99\00:07:33.21 But certainly not the 25 pounds that's recommended 00:07:33.24\00:07:35.96 for someone who is of normal weight 00:07:35.99\00:07:38.38 at the time when they get pregnant. 00:07:38.41\00:07:40.55 Maybe there's not enough room in there for the baby. 00:07:40.56\00:07:42.83 They want to get it out of there. 00:07:42.86\00:07:44.46 Well, that could be part of it as well as we'd talked about 00:07:44.49\00:07:47.39 when there is more pressure 00:07:47.42\00:07:49.55 on that it does increase the risk of delivery. 00:07:49.58\00:07:53.79 I saw a CAT scan, not of the lady of course but 00:07:53.82\00:07:57.32 looking at the obesity of someone inside 00:07:57.35\00:08:00.00 we see someone obese outside. 00:08:00.03\00:08:01.54 Inside there's also a lot of adipose tissue in there. 00:08:01.57\00:08:05.47 A lot of adipose tissue in there, and 00:08:05.50\00:08:07.35 A lot of fat cells. Right. 00:08:07.38\00:08:08.68 And if you're wearing tight jeans on top of that 00:08:08.71\00:08:12.22 you've increased the pressure even more. 00:08:12.25\00:08:14.46 Baby says, Get me out of here. 00:08:14.49\00:08:15.87 Anything else then in terms of premature... 00:08:16.97\00:08:20.52 There are a host of factors that 00:08:20.55\00:08:22.42 increase the risk of premature delivery. 00:08:22.45\00:08:24.95 Diabetes is one of them. 00:08:24.98\00:08:26.77 And if the diabetes is not controlled in the mother. 00:08:26.78\00:08:30.58 And so checking for gestational diabetes, 00:08:30.61\00:08:33.44 checking those blood sugars, 00:08:33.47\00:08:34.87 getting on the right diet so the blood sugars don't go above 140 00:08:34.90\00:08:38.59 an hour after eating is an important step 00:08:38.62\00:08:41.27 in preventing a premature delivery. 00:08:41.30\00:08:43.57 We don't know why they come out early based on that. 00:08:43.60\00:08:46.74 It's just, we need to avoid it. 00:08:46.77\00:08:48.00 That's right. We need to avoid it. 00:08:48.03\00:08:49.79 So Diabetes, making sure that... 00:08:49.82\00:08:52.43 And this happens a lot. 00:08:52.46\00:08:53.63 When someone gets pregnant, it just kind of 00:08:53.66\00:08:55.02 puts them over the edge and they 00:08:55.05\00:08:56.63 get diabetes when they never had it before. 00:08:56.66\00:08:58.35 So they need to get checked. 00:08:58.38\00:08:59.40 If they have the gene for diabetes. 00:08:59.43\00:09:01.27 In other words, if their mother was a diabetic, or father. 00:09:01.30\00:09:04.25 Or maybe their mother and father aren't old enough 00:09:04.28\00:09:07.64 to get to diabetes yet. 00:09:07.67\00:09:08.90 But if they have the gene there, even among grandparents 00:09:08.93\00:09:11.62 that really increases their risk of gestational diabetes. 00:09:11.65\00:09:15.23 When you say they have the gene, 00:09:15.26\00:09:16.31 they shouldn't get genetic testing, they just know their 00:09:16.34\00:09:18.08 family history, is what you're talking about. 00:09:18.12\00:09:19.77 Yeah. If they know their family history. 00:09:19.80\00:09:21.56 If that is there... If they do get gestational diabetes 00:09:21.59\00:09:26.68 it is a true sign that 00:09:26.71\00:09:29.14 that mother does indeed have the gene. 00:09:29.17\00:09:31.69 And so that means she is going to develop 00:09:31.72\00:09:34.31 diabetes later on in life unless she stays normal weight, 00:09:34.34\00:09:38.85 stays lean, stays exercising and eats the right food 00:09:38.88\00:09:41.81 and even if you have the gene for diabetes 00:09:41.84\00:09:43.55 you'll never develop diabetes 00:09:43.58\00:09:45.40 if you're on an ideal lifestyle program. 00:09:45.43\00:09:47.64 Some other things that increase the risk. 00:09:47.67\00:09:51.93 Caffeine increases the risk. 00:09:51.96\00:09:54.37 A study by Yale University back in 2003 showed that if you 00:09:54.40\00:09:58.67 drink a 10 ounce glass or cup of coffee per day, 00:09:58.70\00:10:03.27 it does significantly increase the risk of preterm delivery 00:10:03.30\00:10:07.96 as well as low birth weight babies. 00:10:07.99\00:10:09.59 So just one cup of Starbucks or whatever it is.. 00:10:09.62\00:10:13.27 That's right. 00:10:13.30\00:10:14.70 That's not something someone who is 00:10:14.73\00:10:17.93 pregnant or thinking of getting pregnant 00:10:17.96\00:10:19.87 or trying to get pregnant should be drinking. 00:10:19.90\00:10:21.74 Exactly. 00:10:21.77\00:10:23.28 Was there any factor as to when 00:10:23.31\00:10:24.88 they drank that during the pregnancy 00:10:24.91\00:10:26.46 in terms of heightened risk or was it anytime? 00:10:26.49\00:10:28.86 Particularly in the first two trimesters 00:10:28.89\00:10:33.16 it increases the risk. 00:10:33.19\00:10:34.94 So at the time you get pregnant, that's a good time to... 00:10:34.97\00:10:38.86 Actually before you get pregnant... 00:10:38.89\00:10:41.59 If you're planning on getting pregnant 00:10:41.62\00:10:43.24 go through the caffeine withdrawal first. 00:10:43.27\00:10:45.66 You'll have headaches for a couple of days 00:10:45.69\00:10:47.37 but it'll be worth it in the long run. 00:10:47.38\00:10:49.91 So, basically everything you know to be bad, 00:10:49.94\00:10:52.19 stop doing it before you get pregnant. 00:10:52.22\00:10:54.30 Exactly. You would actually... 00:10:54.33\00:10:57.01 That way your dream can be fulfilled 00:10:57.04\00:10:59.49 of having the perfect baby. 00:10:59.52\00:11:01.92 Everyone wants to have the perfect baby 00:11:01.95\00:11:06.08 and there are ways actually of 00:11:06.11\00:11:08.73 greatly increasing your likelihood of having 00:11:08.76\00:11:11.19 that perfect baby. 00:11:11.22\00:11:12.79 Does age have anything to do with premature births? 00:11:12.82\00:11:15.47 Age of the mother? Yes. 00:11:15.50\00:11:17.52 It does. 00:11:17.55\00:11:18.79 Mothers who are over the age of 35 when they get pregnant 00:11:18.82\00:11:22.91 they have an increased risk of preterm delivery. 00:11:22.94\00:11:25.78 And even more vital than that is mothers 00:11:25.81\00:11:28.23 who are under the age of 16 00:11:28.26\00:11:30.59 have a significant risk of preterm delivery. 00:11:30.62\00:11:34.03 In other words, when you are over 35 00:11:34.06\00:11:40.27 are you saying don't try to have a baby? 00:11:40.30\00:11:43.89 No, I'm just telling you the statistics on that 00:11:43.92\00:11:47.27 that are associated with it. 00:11:47.30\00:11:48.95 I think if you're over the age of 35 00:11:48.98\00:11:51.45 you can still have babies. 00:11:51.48\00:11:52.75 In fact a recent study 00:11:52.78\00:11:54.26 showed that if you have a baby and you're aged 45 00:11:54.29\00:11:57.93 you're four times as likely to live to age 90. 00:11:57.96\00:12:01.59 People ought to be less risk of living to age 90 00:12:03.48\00:12:06.67 but there might be some sort of motivation 00:12:06.70\00:12:08.55 to see your kids and your grandkids grow up. 00:12:08.58\00:12:11.16 So having babies in 40's is okay, 00:12:11.19\00:12:13.86 but you would really need to have some close prenatal 00:12:13.89\00:12:16.66 follow-up. 00:12:16.69\00:12:18.09 You need to really do the prenatal exams. 00:12:18.12\00:12:20.31 in fact, that's one of the risks as well. 00:12:20.34\00:12:22.20 We're talking about risks of preterm delivery. 00:12:22.23\00:12:24.13 One of the risks is if that girl just doesn't see the doctor 00:12:24.16\00:12:27.87 as scheduled for her prenatal exams. 00:12:27.90\00:12:30.74 You do need to see your gynecologist 00:12:30.77\00:12:34.05 for those prenatal exams. 00:12:34.08\00:12:35.96 And then, of course, the ultrasounds need to be done 00:12:35.99\00:12:38.24 making sure that the baby 00:12:38.27\00:12:39.73 is developing normally and healthfully. 00:12:39.74\00:12:42.10 And if you do that over the age of 35, 00:12:42.13\00:12:44.96 I think your risk can be almost as low as someone who's 30. 00:12:44.99\00:12:48.07 Certainly it needs to be avoided under the age of 16. 00:12:48.10\00:12:52.63 So, I'm not gonna ask how old your wife is, 00:12:52.66\00:12:55.18 and I'm not gonna ask how old mine is. 00:12:55.21\00:12:56.46 But at some point, this might be good information for them 00:12:56.49\00:12:59.48 if they want to have another child. 00:12:59.51\00:13:00.84 Yeah, absolutely. 00:13:00.87\00:13:02.41 My wife did have a couple of babies after the age of 35. 00:13:02.44\00:13:08.76 One in her 40's. 00:13:08.79\00:13:11.49 They were very healthy, 00:13:11.52\00:13:13.02 but she also paid very close attention 00:13:13.05\00:13:15.39 to what I am talking about here today. 00:13:15.40\00:13:17.45 We're talking with Dr. Neil Nedley. 00:13:17.48\00:13:19.45 We're talking about prenatal influences. 00:13:19.48\00:13:21.76 When we come back, you may be saying well, 00:13:21.79\00:13:23.76 I see what I shouldn't do and maybe that seems... 00:13:23.79\00:13:26.92 that's interesting, but when we come back 00:13:26.93\00:13:29.45 you're gonna to see how important it really is. 00:13:29.48\00:13:31.80 What are the impacts on children. 00:13:31.83\00:13:35.49 When we come back, we'll take a closer look 00:13:35.52\00:13:38.38 not only at what the impact can be 00:13:38.41\00:13:40.18 but what we can do to avoid problems. 00:13:40.21\00:13:42.75 So join us when we come back. 00:13:42.78\00:13:44.62
