Talking with us about 00:00:56.39\00:00:59.02 this problem is Dr. Philip Mills, 00:00:59.03\00:01:00.70 he's a specialist in physical medicine, 00:01:01.01\00:01:02.98 and you've been practicing that for, 00:01:03.31\00:01:04.63 you know over 20 years, isn't that right? 00:01:04.87\00:01:07.49 That is correct, and so one of the things that 00:01:07.50\00:01:09.18 you really see is you know the problem 00:01:09.19\00:01:11.72 we're talking about today which is stroke. 00:01:11.73\00:01:13.27 A lot of people in America have strokes, 00:01:13.28\00:01:15.32 yes it's the, it's the most common 00:01:15.57\00:01:18.51 rehabilitation problem that we see. 00:01:18.52\00:01:20.66 And what exactly is a stroke? 00:01:21.37\00:01:22.99 Well, it all depends when you're asking 00:01:23.71\00:01:26.60 the question. Anciently, in Bible 00:01:26.61\00:01:28.09 times, the stroke was actually leprosy, 00:01:28.10\00:01:30.98 the Jews regarded this as a judgment from 00:01:30.99\00:01:33.87 God, they call it a stroke from God or 00:01:33.88\00:01:36.43 the finger of God. But in the Middle Ages 00:01:36.44\00:01:39.39 when a person suddenly became 00:01:39.68\00:01:41.60 unable to speak or lost the ability to move 00:01:42.63\00:01:46.53 one side, it was is if they were struck 00:01:46.54\00:01:49.82 because it happens so suddenly, they called 00:01:49.83\00:01:51.91 that a stroke. Now, today we understand 00:01:51.92\00:01:54.84 that what happened was that they lost 00:01:54.85\00:01:57.65 blood supply either from a bleed or 00:01:57.66\00:02:00.12 actually from a clot and so there was no 00:02:00.13\00:02:02.91 oxygen to a particular portion of the body 00:02:02.92\00:02:05.25 and that's why they had this sudden onset 00:02:05.82\00:02:08.56 of symptoms which we call a stroke or a 00:02:08.57\00:02:11.39 cerebrovascular accident (CVA). 00:02:11.40\00:02:14.13 So, a certain part of the brain just loses 00:02:14.38\00:02:16.84 blood supply and that's a stroke. 00:02:16.85\00:02:18.89 Yes, okay how does a blood clot form in the 00:02:18.90\00:02:22.74 brain, why would that happen? 00:02:22.75\00:02:24.50 Well, a blood clot is actually a very 00:02:25.15\00:02:27.96 complicated process because you don't 00:02:28.40\00:02:30.92 want it to clot when it shouldn't clot. 00:02:30.93\00:02:33.17 But you need it to clots when you 00:02:33.76\00:02:34.85 needed to clot. So, God designed 00:02:34.86\00:02:38.37 a cascade, a series of events, checks and 00:02:38.38\00:02:41.54 balances to make sure that when it clotted 00:02:41.55\00:02:44.86 it was an appropriate time to clot. 00:02:45.13\00:02:46.85 Well, in the case of a stroke for some 00:02:47.32\00:02:49.66 reason the blood clots in the vessel itself 00:02:49.67\00:02:53.81 and if that is in the brain we called it a stroke. 00:02:54.90\00:03:00.49 If it clots maybe in the heart one of the 00:03:01.19\00:03:04.48 valves and then breaks lose and floats 00:03:04.49\00:03:07.67 up to the brain, we call it an embolus, 00:03:07.85\00:03:11.45 okay. Now, the other way you can get a 00:03:11.77\00:03:14.65 stroke is from a bleed and that's the opposite 00:03:14.66\00:03:19.43 of a clot, there it should clot but it 00:03:19.44\00:03:21.92 doesn't clot instead the blood just pours 00:03:21.93\00:03:25.06 out and fills the brain. And so you don't 00:03:25.07\00:03:29.77 know which of these it is until you kind a 00:03:29.78\00:03:31.76 look at it through testing or something. 00:03:31.77\00:03:34.37 They can, they can find out through CT 00:03:34.38\00:03:37.01 scans and other studies which it is 00:03:37.02\00:03:40.58 and it's important to know because 00:03:41.06\00:03:42.72 depending on which type of stroke that you 00:03:42.73\00:03:45.29 have, it will depend very dramatically on 00:03:45.30\00:03:49.23 what type of treatment your to give. 00:03:49.24\00:03:52.28 The, the bleed kind of stroke is actually the 00:03:52.93\00:03:56.78 most dangerous as far as death that can kill 00:03:56.79\00:04:00.29 you very rapidly. On the other hand 00:04:00.30\00:04:03.21 surprisingly enough if a person survives a 00:04:03.63\00:04:06.19 bleed; it may leave you with the least of 00:04:06.20\00:04:08.87 the residuals, so its sort of if it doesn't kill 00:04:08.88\00:04:11.79 you it actually maybe in some cases the 00:04:11.80\00:04:14.98 best, the better kind of stroke to have. 00:04:14.99\00:04:17.42 I suppose that's because the blood 00:04:17.43\00:04:18.90 supply's not cut off forever to that area, 00:04:19.45\00:04:21.39 is that what you mean? That's correct, and 00:04:21.85\00:04:23.50 also since the blood is a normal component 00:04:23.51\00:04:26.53 the body knows how to take care of blood, 00:04:27.21\00:04:29.07 it can absorb the blood and come back quite well. 00:04:29.08\00:04:33.26 So, you've said this is the number one thing 00:04:33.27\00:04:35.32 that people see your specialty for. 00:04:35.33\00:04:38.34 How, big is it in terms of the other problems 00:04:39.83\00:04:42.62 that are causing problems for people in America? 00:04:42.63\00:04:45.86 Well, it's big, it's the third largest killer in 00:04:47.24\00:04:51.91 the United States and over age 60 it's a very 00:04:51.92\00:04:56.13 major cause of disability. In fact seven thousand 00:04:56.14\00:05:00.49 people will have strokes this year 00:05:00.50\00:05:02.86 that's one every 45 seconds, I figured that 00:05:03.21\00:05:06.08 during of this program 40 people in 00:05:06.09\00:05:09.41 America will have had strokes. 00:05:09.42\00:05:11.23 That's a huge number so you said in your, 00:05:12.77\00:05:15.48 your fact that you sent to me about 157,000 00:05:15.49\00:05:19.40 people per year. And, that's dying, that's 00:05:19.41\00:05:25.36 the ones that die. What I've said to you 00:05:25.37\00:05:27.82 is there's just a brand new sixty seven page 00:05:27.83\00:05:31.62 report of all the statistics of strokes, 00:05:31.63\00:05:35.54 it's very fascinating to go through and look 00:05:35.55\00:05:38.64 at these statistics every three minutes. 00:05:38.65\00:05:42.18 You have, a death from that, a death 00:05:43.23\00:05:45.66 from a stroke. And it says here something 00:05:45.67\00:05:49.12 about blacks versus whites, 00:05:49.13\00:05:51.20 more prevalence with African Americans 00:05:51.68\00:05:53.87 then whites. For reasons that are 00:05:53.88\00:05:55.61 not entirely clear black males have the 00:05:55.62\00:05:58.42 highest incident of stroke in this country. 00:05:58.43\00:06:01.35 You said the cost per year is in the billions for this. 00:06:02.12\00:06:04.60 Billions of dollars for stroke. 00:06:04.61\00:06:06.45 So, we've talked a little bit already about 00:06:06.94\00:06:08.87 what causes strokes but we wanna revisit 00:06:08.88\00:06:11.41 that again, I mean we've talked about a 00:06:11.42\00:06:13.15 bleed; we've talked about a clot. 00:06:13.16\00:06:14.90 What are some other causes we don't 00:06:16.31\00:06:17.64 normally think about? Well, I don't know 00:06:17.65\00:06:20.18 what we normally think about but you 00:06:20.19\00:06:22.88 have to consider age, genetics, 00:06:22.89\00:06:26.78 race as we mentioned, then the causes that 00:06:29.95\00:06:34.91 we can do something about include such 00:06:34.92\00:06:38.02 things as smoking, alcohol, certain 00:06:38.03\00:06:43.35 dietary factors and certain diseases, diabetes. 00:06:43.36\00:06:49.59 So, what about men versus women, 00:06:49.60\00:06:51.24 who have more. Men have more strokes 00:06:51.69\00:06:54.15 as a, as a group but women actually have 00:06:55.32\00:06:58.74 more strokes because they live longer. 00:06:58.75\00:07:00.76 I see, so other diseases that can 00:07:01.09\00:07:04.21 really predispose you say like HIV, HIV, 00:07:04.22\00:07:07.62 well or HIV behavior. HIV can in a small, 00:07:07.63\00:07:13.79 in small cases contribute to stroke 00:07:13.80\00:07:17.29 danger but really you have many of us 00:07:17.79\00:07:22.87 concerned about those types of behaviors 00:07:22.88\00:07:25.18 that cause HIV. We should actually 00:07:25.19\00:07:28.85 be also very concerned about 00:07:28.86\00:07:30.76 certain types of behaviors that cause strokes. 00:07:30.77\00:07:34.29 What we do today contributes to what 00:07:34.60\00:07:37.67 disease we have tomorrow. And everybody knows 00:07:37.68\00:07:40.37 that's true in the case of HIV, but some 00:07:40.38\00:07:43.58 people sort of think that strokes and 00:07:43.59\00:07:48.09 cancer, diabetes, heart attacks, never will 00:07:48.64\00:07:53.33 happen to them, well there is nothing they 00:07:53.34\00:07:54.89 can do about it. They don't realize that the, 00:07:54.90\00:07:57.45 that the lifestyle they choose today is going 00:07:57.46\00:08:01.32 to determine the problems they have tomorrow. 00:08:01.33\00:08:04.75 So, no smoking, no alcohol, proper diet 00:08:04.76\00:08:08.17 all these different things are gonna 00:08:08.18\00:08:09.62 protect you against it. Yes, what I choose to 00:08:09.95\00:08:12.58 do today is going to determine how I am 00:08:12.59\00:08:15.21 twenty years from now? So, someone comes in 00:08:15.22\00:08:17.61 or calls you on the phone or comes to the 00:08:17.62\00:08:19.36 emergency room or sees their family 00:08:19.37\00:08:20.87 physician, what are the symptoms they 00:08:20.88\00:08:22.75 look for to say whether not 00:08:22.76\00:08:24.77 someone's having a stroke. Well, of course the 00:08:24.78\00:08:26.59 number one symptom that you think about 00:08:26.60\00:08:28.71 in a stroke is sudden onset of weakness in 00:08:28.72\00:08:35.21 a side of the body, arms, legs, on the 00:08:36.30\00:08:39.15 right or on the left. There can be sudden 00:08:39.16\00:08:42.45 onset of numbness on the side of the body, 00:08:42.46\00:08:44.78 you think of the side of the body. 00:08:44.79\00:08:46.17 There can be some difficulties in 00:08:46.98\00:08:49.68 speeches, either they have garbled speech 00:08:49.69\00:08:54.86 or difficulty in, in speech and 00:08:54.87\00:08:59.34 understanding speech, confusion. 00:08:59.35\00:09:01.87 They can have present with dizziness and 00:09:02.89\00:09:06.96 these are various types. Blindness? Blindness 00:09:06.97\00:09:09.51 in one eye, one side of the body can be a stroke. 00:09:09.52\00:09:13.11 Stumbling around does or what do you 00:09:13.12\00:09:15.39 call that attacks here or something, yes. 00:09:15.40\00:09:17.17 And that's another sign, yes, okay. 00:09:17.59\00:09:20.09 So, these symptoms if they come on, 00:09:20.10\00:09:22.08 they usually come on when suddenly or 00:09:22.09\00:09:23.97 over hours? They come on suddenly 00:09:23.98\00:09:26.64 or over maximum of maybe three days, 00:09:27.10\00:09:30.55 if something is coming on gradually 00:09:30.95\00:09:33.23 over weeks to months then you think not in 00:09:33.24\00:09:38.69 terms of a stroke you think more in terms of 00:09:38.70\00:09:40.73 a, of a tumor or some other, other process. 00:09:40.74\00:09:44.86 A stroke by nature is sudden; however you 00:09:45.10\00:09:49.48 can still have an underlying tumor that 00:09:49.49\00:09:53.92 maybe causes a bleed or a clot, so we have 00:09:54.53\00:09:57.44 to check it out. So, you still have to 00:09:57.45\00:09:58.94 investigate just because they have a 00:09:59.26\00:10:01.12 stroke doesn't mean that there can't be 00:10:01.13\00:10:02.85 another cause contributing to the stroke. 00:10:02.86\00:10:05.39 So, you, do you see strokes a lot when 00:10:05.40\00:10:09.64 they, people first time them, are you like one 00:10:09.65\00:10:12.09 of the first people they call? 00:10:12.10\00:10:13.39 No, I see them at the end of the medical 00:10:14.08\00:10:18.65 process in the rehab phase but actually 00:10:18.66\00:10:21.81 because of the first aspect there is, 00:10:21.82\00:10:24.62 actually three aspects of stroke treatment, okay. 00:10:24.63\00:10:28.31 There is the emergency phase and that's very 00:10:28.32\00:10:31.16 important, we should talk about that, 00:10:31.17\00:10:32.99 then there is the hospital phase and 00:10:33.22\00:10:36.62 then there is the phase which most excites 00:10:36.63\00:10:38.95 me which is the rehab phase. 00:10:38.96\00:10:41.03 So, if you think you're having a stroke 00:10:41.59\00:10:44.88 what should you do? The immediate 911 00:10:44.89\00:10:50.20 call, because if it's a certain type of stroke 00:10:50.21\00:10:54.02 that is a clot type of stroke. 00:10:54.03\00:10:56.08 Then you have a window of opportunity of one 00:10:58.67\00:11:02.54 hundred and eighty minutes from the time 00:11:02.55\00:11:05.07 you first have your symptoms through 00:11:05.08\00:11:07.26 some of the modern wonderful treatments 00:11:07.27\00:11:09.99 that they have for strokes today. 00:11:10.00\00:11:11.58 And so, during that time if you're outside 00:11:12.36\00:11:15.95 of that window of opportunity, it can't 00:11:15.96\00:11:18.60 work, is that like some of those clot 00:11:18.61\00:11:20.21 busting drugs and stuff. Exactly, so you have 00:11:20.22\00:11:23.71 this time, during that time that means 00:11:23.72\00:11:25.75 transporting to the emergency room, 00:11:25.76\00:11:27.81 that means the testing time, that means the 00:11:28.04\00:11:31.44 decision that this is not a bleed that 00:11:31.45\00:11:34.03 you're an appropriate candidate and you 00:11:34.04\00:11:37.09 may think three hours is a long time. 00:11:37.10\00:11:39.00 But, it's not in an emergency room, 00:11:39.01\00:11:40.33 speed is of the essence and doing the 00:11:40.34\00:11:44.09 right thing at the right time is the essences of 00:11:44.10\00:11:46.84 good medicine. So, after you get that 00:11:46.85\00:11:48.50 done, lets say you have excellent team 00:11:48.51\00:11:51.16 there that gets you right in, you're, you're 00:11:51.17\00:11:53.10 under the two hours or, the three hours, 00:11:53.11\00:11:55.73 yeah right you're between two and 00:11:55.97\00:11:57.83 three hours there, you're all done. 00:11:57.84\00:11:59.02 And they know it's a stroke, they know it's 00:11:59.36\00:12:02.14 a clot, what's the next thing that happen, 00:12:02.15\00:12:03.95 what kind of gold standard treatment comes next? 00:12:03.96\00:12:06.28 Well, then they would do one of the clot 00:12:06.54\00:12:07.89 busters, that you were talking about and 00:12:07.90\00:12:11.04 hopefully with proper care and the blessing 00:12:11.05\00:12:15.78 of the Lord. Then you will have very little 00:12:15.79\00:12:19.23 residual, in many cases however you 00:12:19.24\00:12:23.69 can't get for some reason or another that 00:12:23.70\00:12:27.41 treatment care. And so, then you 00:12:28.06\00:12:30.44 were in the hospital because if you look at 00:12:30.45\00:12:34.41 the statistics, if you will have a Transient 00:12:34.42\00:12:37.98 Ischemic Attack, which is a very mild 00:12:37.99\00:12:39.70 stroke, temporary kind of stroke. 00:12:39.71\00:12:41.56 And you have a stroke you're at high risk 00:12:42.32\00:12:46.35 before the stroke you either progress or you 00:12:47.21\00:12:49.85 can have a recurrent stroke. 00:12:49.86\00:12:51.41 So, you want to be in the hospital to see if 00:12:52.55\00:12:55.68 there's other testing, other medicine that 00:12:55.69\00:12:59.02 needs to be given to prevent future strokes. 00:12:59.03\00:13:03.60 So, you're there to stabilize and during 00:13:04.05\00:13:08.12 that time of stabilization you start 00:13:08.13\00:13:10.93 early rehab intervention. In a good hospital 00:13:10.94\00:13:15.41 what you'll have as a stroke team. 00:13:16.52\00:13:18.44 And you go through the evaluation in 00:13:19.09\00:13:22.16 emergency room, you go to the hospital with 00:13:22.17\00:13:24.59 the stroke team and they're immediately 00:13:24.60\00:13:26.73 implementing the proper measures to 00:13:26.99\00:13:29.41 prevent future strokes and minimize the 00:13:29.65\00:13:33.00 damage of the stroke. Once that's all done 00:13:33.01\00:13:35.03 then they go to a rehab specialist like yourself. 00:13:35.04\00:13:37.41 Yeah, but I want to talk about what needs 00:13:37.42\00:13:40.14 to happen in the acute setting and hospital 00:13:40.15\00:13:42.72 setting, because one of the problems that 00:13:42.73\00:13:45.14 can be missed, in strokes it's very 00:13:45.15\00:13:48.61 common if you're weak on one side that 00:13:48.62\00:13:51.93 includes your swallow. I see. 00:13:51.94\00:13:54.76 So because you have problems with 00:13:55.21\00:13:57.04 swallow, you might get pneumonia. 00:13:57.05\00:14:00.05 You have a danger of swallowing into the 00:14:00.53\00:14:03.46 lungs and that's not designed to digest 00:14:03.47\00:14:05.74 your food and then you get what's called 00:14:05.75\00:14:08.53 Pneumonitis. So, to prevent that they've 00:14:08.54\00:14:12.00 develop special diets, special studies to see 00:14:12.01\00:14:15.46 whether you're what's called Aspirator that 00:14:15.47\00:14:17.97 is swallowing into the lungs and then you 00:14:17.98\00:14:23.12 and put them on this diet. 00:14:24.09\00:14:25.60 So, you wanna minimize complications, other 00:14:25.86\00:14:28.38 things that you do in the acute setting 00:14:28.39\00:14:30.77 is make sure you're protecting range of 00:14:31.55\00:14:34.52 motion, you're protecting the skin 00:14:34.53\00:14:36.94 so that you're not getting precious sores 00:14:37.52\00:14:39.09 and moving them around enough, yes 00:14:39.10\00:14:40.47 and putting things on them. And these are kind of 00:14:40.48\00:14:42.63 things that relatives can do as they, 00:14:42.64\00:14:44.51 if their relative goes in, they can be saying, 00:14:44.52\00:14:46.55 hey make sure, then check the swallowing, 00:14:46.56\00:14:49.18 make sure this, make sure that, a great list 00:14:49.19\00:14:51.71 to have around just in case that would happen. 00:14:51.72\00:14:53.56 Yes, and in a good rehab, with a rehab 00:14:53.57\00:14:57.14 team they're going to do all this, 00:14:57.15\00:14:59.56 they're gonna know what needs to be done. 00:14:59.97\00:15:01.69 We're talking with Dr. Philip Mills, 00:15:02.51\00:15:03.96 he's a rehab specialist, he deals 00:15:04.28\00:15:05.87 with strokes everyday. There was several 00:16:50.83\00:16:52.05 other different things we watch out. 00:16:52.06\00:16:53.42 How do we know when they're ready to 00:16:54.17\00:16:56.03 go to a rehab hospital? Well, you're actually 00:16:56.04\00:16:58.66 looking for two key components; 00:16:58.67\00:17:00.86 rehab is very intensive training, 00:17:00.87\00:17:03.37 so you need to be able to tolerate two hours, 00:17:04.00\00:17:06.92 have the endurance to tolerate two hours of 00:17:06.93\00:17:10.45 therapies a day. And then the other 00:17:10.46\00:17:13.41 key component, they have to be able 00:17:14.01\00:17:16.19 to learn from day to day and you can 00:17:16.20\00:17:18.49 measure that if there is no carry over then 00:17:18.50\00:17:21.88 it's difficult for person to be benefited by the 00:17:21.89\00:17:26.01 intensive rehab. What do they do there? 00:17:57.42\00:18:00.16 Well, rehab hospital or a rehab wing in a 00:18:00.52\00:18:06.22 hospital is where you have very intense 00:18:06.23\00:18:09.97 physical, occupational and speech therapy 00:18:11.57\00:18:15.82 when we're talking about strokes. 00:18:16.16\00:18:18.51 So this is like re-educating how 00:18:18.58\00:18:20.05 to walk, how to talk, how to eat, how to all 00:18:20.06\00:18:22.45 those different things. Yes, there are 00:18:22.46\00:18:23.73 activities that are necessary in life. 00:18:23.74\00:18:26.99 Now I've actually visited the hospital 00:18:27.86\00:18:29.84 that you've served in and they have all 00:18:29.85\00:18:31.34 these, they have stairs, they have a 00:18:31.35\00:18:33.17 little kitchen, they have, 00:18:33.18\00:18:35.65 they throw balls back and forth for hand eye 00:18:37.60\00:18:39.69 coordination, they have a whole bunch 00:18:39.70\00:18:41.51 of therapists that work with their peoples 00:18:41.52\00:18:43.76 hands and all these, that's what you're 00:18:43.77\00:18:45.09 talking about. Yes, and with that it's very 00:18:45.10\00:18:48.43 individualized, it's very directed 00:18:48.44\00:18:51.70 and what your aim is, is to make sure that 00:18:53.20\00:18:56.44 they can dress themselves if 00:18:56.45\00:18:58.80 possible, you want them independent, 00:18:58.81\00:19:01.27 you want them to be independent in 00:19:01.95\00:19:03.97 tolerating themselves, you want them to be 00:19:03.98\00:19:06.78 independent in eating, so they can feed 00:19:06.79\00:19:09.78 themselves. You want them as independent 00:19:09.79\00:19:13.25 as possible in walking, or transforming themselves, 00:19:13.26\00:19:17.06 transfer or assisting in this. 00:19:17.07\00:19:20.33 If it's not walking then you want them 00:19:20.79\00:19:22.46 as independent as possible in a wheelchair. 00:19:22.47\00:19:24.83 And this is probably quite discouraging not 00:19:26.64\00:19:29.09 only the person that has a stroke but also 00:19:29.10\00:19:30.78 to their relatives. Well, they can be, 00:19:30.79\00:19:33.44 because it changes a person almost 00:19:34.01\00:19:36.47 overnight, if you have a severe stroke from 00:19:36.48\00:19:40.72 one with complete activity in their hands 00:19:40.73\00:19:43.61 and their legs to one, to one that has 00:19:43.62\00:19:45.76 difficulty in coordination of hands and legs. 00:19:45.77\00:19:49.13 Particularly early on and maybe some 00:19:49.14\00:19:52.30 difficulty in communication. 00:19:52.31\00:19:54.36 So, I mean I think they're probably just 00:19:55.06\00:19:56.69 reaching out for any kind of hope and 00:19:56.70\00:19:58.51 encouragement they can find in that setting. 00:19:58.52\00:20:00.46 And, and rehab therapists by enlarge 00:20:01.14\00:20:05.37 are wonderful, causing people, 00:20:05.38\00:20:07.86 yes and because stroke isn't into the 00:20:08.26\00:20:10.33 world, stroke is just new challenges and 00:20:10.34\00:20:14.62 new ways to do old things, which is 00:20:14.63\00:20:18.18 dressing and eating and all the things 00:20:18.74\00:20:21.25 there are necessary for life, it maybe a 00:20:21.26\00:20:22.81 little slower, it maybe a little different. 00:20:22.82\00:20:24.88 But, our aim is to bring you back to 00:20:25.39\00:20:28.30 those goals of caring for yourself. 00:20:28.74\00:20:30.83 Strengthening that which remains and 00:20:31.61\00:20:34.19 being able to do the first works, yes so just be. 00:20:34.20\00:20:37.04 Now, of course the best thing to do is to 00:20:37.83\00:20:41.68 avoid strokes, to begin with or prevent 00:20:41.69\00:20:44.35 them, what can we do to prevent strokes 00:20:44.36\00:20:47.40 Dr. Mills? Well, I like to say that life is like 00:20:47.41\00:20:51.00 golf, we want to have as few strokes as 00:20:51.01\00:20:53.98 possible. Okay, all the Tiger Woods 00:20:53.99\00:20:57.23 fans would like that, yeah, that's true so 00:20:57.24\00:20:59.91 you wanna decrease even having one, yes. 00:20:59.92\00:21:03.32 And so the question is how do we decrease 00:21:04.74\00:21:09.13 the blood clotting or the bleed in the brain, 00:21:09.58\00:21:13.06 what we need to look at what makes the 00:21:13.60\00:21:15.37 blood clot abnormally or what makes the 00:21:15.38\00:21:18.44 brain bleed, when it's not suppose to. 00:21:18.45\00:21:21.89 And let's look at the bleeding problem first, 00:21:22.43\00:21:24.65 okay with bleeding you can think of high 00:21:25.01\00:21:29.40 blood pressure, which puts pressure on the 00:21:29.41\00:21:33.01 blood vessels and they can burst. 00:21:33.36\00:21:37.60 So, you watch our blood pressure, 00:21:38.94\00:21:40.17 make sure we don't have high salt, 00:21:40.18\00:21:41.60 make sure we don't have overweight, 00:21:41.82\00:21:43.64 make sure we don't have all those 00:21:43.65\00:21:44.71 different things. Exactly, so that blood 00:21:44.72\00:21:48.51 pressure is not something that 00:21:48.52\00:21:50.04 typically kills you instantly, 00:21:50.57\00:21:52.47 it's something that's slow, kills you overtime. 00:21:52.88\00:21:56.43 And so it's important to check and measure 00:21:56.98\00:21:59.69 your blood pressure and make sure that 00:21:59.70\00:22:02.80 isn't a problem. My patients typically 00:22:02.81\00:22:05.89 would have a triad of problems, they would 00:22:05.90\00:22:08.72 have diabetes, they would have high 00:22:08.73\00:22:11.77 blood pressure and then they'd have other 00:22:12.36\00:22:17.62 high cholesterol. Other problems such 00:22:17.93\00:22:20.38 as cholesterol, these triad and associated 00:22:20.39\00:22:27.71 diseases don't just add to your risk. 00:22:27.72\00:22:32.30 They multiply your risk. I think probably for 00:22:33.00\00:22:36.69 everyone you would say stop smoking but 00:22:36.70\00:22:39.11 would you say that for someone who had a stroke? 00:22:39.12\00:22:40.65 Well, you want to stop smoking before 00:22:41.31\00:22:44.11 you have the stroke, does it really 00:22:44.12\00:22:45.91 predispose you to stroke. It's a , it's a major 00:22:46.22\00:22:49.26 component to, to strokes. Now you have to 00:22:49.27\00:22:52.41 think of stroke as a heart attack of the 00:22:52.42\00:22:55.35 brain or you might think of the 00:22:55.36\00:22:57.88 heart attack as a stroke of heart. 00:22:59.57\00:23:01.52 In fact if you look at the 67 pages of 00:23:02.71\00:23:05.77 documentation of causes and the 00:23:05.78\00:23:09.34 statistics of strokes, it's actually strokes 00:23:09.35\00:23:12.27 and heart attacks, because these are 00:23:12.28\00:23:14.35 blood vessel disorders and what's bad for the 00:23:14.63\00:23:19.23 heart, is also bad for the health; 00:23:19.24\00:23:21.04 Is bad for health. So, you, you talk also 00:23:21.27\00:23:23.58 about the electrolyte to get calcium, 00:23:23.59\00:23:25.58 potassium, magnesium, all those for just 00:23:25.59\00:23:29.08 optimal functioning of the body, 00:23:29.09\00:23:30.67 how are those related to stroke? 00:23:30.68\00:23:32.09 Well, what you want is to have healthy 00:23:32.85\00:23:35.35 nutrition to the brain. Okay, and so that 00:23:35.36\00:23:39.73 your blood vessels are in the maximum 00:23:39.74\00:23:43.29 health possible. Now, you're not a 00:23:44.82\00:23:46.73 prophet or the summer prophet. 00:23:46.74\00:23:48.11 But what do you think the direction is in 00:23:48.99\00:23:51.73 terms of future medical breakthroughs concerning 00:23:51.74\00:23:55.91 stroke treatment or therapy, 00:23:55.92\00:23:57.93 all these different things? 00:23:57.94\00:23:59.03 Well, predicting tomorrow's surprise is 00:24:00.49\00:24:07.19 really difficult, right and what technology 00:24:07.20\00:24:09.83 we will do we don't know, but they are breakthroughs 00:24:09.84\00:24:15.70 that the Bible predicted in the area of stroke. 00:24:15.71\00:24:19.33 Isaiah said there was going to come a time 00:24:19.34\00:24:22.14 where the lame man would leap as a hart, 00:24:22.83\00:24:26.64 and the tongue of the dumb would sing. 00:24:27.34\00:24:31.20 So, that's, that's definitely talking 00:24:31.45\00:24:33.03 about someone that had a stroke. 00:24:33.04\00:24:34.46 Well, those are the two key components 00:24:34.47\00:24:36.00 of strokes, you're weak, you're lame and 00:24:36.01\00:24:39.38 you have communication difficulties and this 00:24:39.39\00:24:41.90 said that this breakthrough would 00:24:41.91\00:24:43.31 occur when Christ broke through the 00:24:43.32\00:24:46.42 clouds and came in the clouds of the heaven. 00:24:46.43\00:24:49.13 So, that's the breakthrough that 00:24:49.14\00:24:50.45 I see in the future that's going to really 00:24:50.48\00:24:52.59 solve the problem with stroke. 00:24:52.60\00:24:53.59 It wasn't that also the healing that was done 00:24:53.60\00:24:55.69 in Acts 3 or you had, that was the quote, 00:24:55.70\00:24:58.21 quotation from that, I mean the man was 00:24:58.22\00:25:00.56 unable to walk, unable to enter the 00:25:00.57\00:25:02.79 temple and rejoice, he probably had a stroke 00:25:02.80\00:25:05.30 didn't he? Well, I don't know what 00:25:05.31\00:25:06.62 he had, but it certainly could have been a stroke. 00:25:06.63\00:25:09.42 Interesting, so of course we also may 00:25:10.68\00:25:13.02 look to Christ as the ultimate healer for 00:25:13.03\00:25:16.71 these, these types of things talk to me a 00:25:16.72\00:25:18.42 little bit as we close, we've got about two 00:25:18.43\00:25:20.21 minutes left. You know, the family 00:25:20.22\00:25:22.42 people, the family members that are 00:25:22.43\00:25:23.72 supporting someone what can they do to 00:25:23.73\00:25:25.56 support someone that's had a stroke. 00:25:25.57\00:25:27.15 I mean the person who is being 00:25:27.83\00:25:28.92 supported in the hospital, in the acute 00:25:28.93\00:25:31.21 phase and also in rehabilitation hospital. 00:25:31.22\00:25:34.10 And then that, that comes to an end and 00:25:34.60\00:25:37.27 they go home what kind of things can that 00:25:37.28\00:25:39.62 family member be doing to just kind of 00:25:39.63\00:25:41.66 get themselves ready to be helping others 00:25:41.67\00:25:44.57 and I know, you know it's a big change for 00:25:44.58\00:25:46.78 people but what do you tell people, 00:25:46.79\00:25:48.39 how do you help them? Well, there is, two 00:25:48.40\00:25:52.28 factors here the first one is sometimes they 00:25:52.29\00:25:55.10 feel guilty if they're not able to provide 00:25:55.11\00:25:58.95 assistance and the loved one if they've 00:25:58.96\00:26:01.08 had severe stroke may need extended care 00:26:01.09\00:26:04.39 facilities, really can't do it in a home setting. 00:26:04.40\00:26:07.20 Now, if a parent or a loved one is not able 00:26:07.70\00:26:13.34 to feed themselves, if they're not able to 00:26:13.35\00:26:16.04 tolerate themselves, if they're not able to 00:26:16.05\00:26:18.56 provide some assistance in transfers. 00:26:18.57\00:26:22.52 It really, you can't take care of them at 00:26:23.51\00:26:25.92 home, they need some external help. 00:26:25.93\00:26:28.44 And they shouldn't feel guilty about that? 00:26:28.79\00:26:30.30 No, because it's actually gonna hurt 00:26:30.31\00:26:32.42 them to try and do that. Yeah, and they won't, 00:26:32.43\00:26:33.93 you really can't care for the person 00:26:34.62\00:26:36.85 efficiently as is needed, so they can 00:26:36.86\00:26:40.38 need an extended care. But, if they have 00:26:40.39\00:26:42.04 those things and they go home? 00:26:42.05\00:26:43.63 Then you can assist them, now we have to 00:26:44.45\00:26:47.06 remember stroke is a continuum. 00:26:47.07\00:26:48.39 And many people have strokes and they 00:26:48.40\00:26:50.09 have very little in terms of the 00:26:50.10\00:26:52.52 impairment. Can you over help someone 00:26:52.53\00:26:54.04 who has had stroke? Yes, you want them 00:26:54.05\00:26:55.61 to be as independent as possible. 00:26:55.62\00:26:58.14 You may need to provide a ramp so that 00:26:58.48\00:27:02.01 they have ease and safety of entry, 00:27:02.02\00:27:04.54 you want to provide a, a way for them to be 00:27:05.53\00:27:10.19 able to get into and out of their bathroom, 00:27:10.20\00:27:13.26 so grab bars maybe necessary. 00:27:13.27\00:27:15.52 These are the sorts of things that you wanna 00:27:15.91\00:27:17.58 do in the home to make a stroke ready. 00:27:17.59\00:27:20.37 You know, I watch my grandfather take 00:27:21.04\00:27:22.98 care of my grandmother who had six strokes. 00:27:22.99\00:27:25.04 And as he was taking care of her, he would 00:27:25.77\00:27:28.16 sing wonderful words of life. 00:27:28.17\00:27:30.46 And he learned to how to in that setting 00:27:30.82\00:27:33.21 still show love and help his wife. 00:27:33.61\00:27:36.16 It was a great testimony to me 00:27:36.17\00:27:37.31 and the family.
