Today, we are 00:00:52.00\00:00:54.04 gonna talk about maybe many of you are 00:00:54.12\00:00:56.26 experiencing and that's low back pain and talking 00:00:56.52\00:00:59.24 with us about this is Dr. John Clark, he is an 00:00:59.36\00:01:02.40 orthopedic surgeon and they see you when they 00:01:02.56\00:01:04.27 wanna get rid of the low back pain. Yeah, 00:01:04.93\00:01:06.73 that's right. Is this, how, how, how many people 00:01:06.79\00:01:11.24 are struggling with this in America? You know, 00:01:11.36\00:01:14.67 probably lot more people than we know, but those 00:01:14.70\00:01:17.11 who have come to medical attention about 20% 00:01:17.23\00:01:20.70 of the population this year will have an episode of 00:01:20.78\00:01:24.26 low back pain. Over a lifetime, 80% of people 00:01:24.50\00:01:28.63 experience low back pain. That's a lot. That's a lot 00:01:28.88\00:01:32.90 it's is also very expensive for workers compensation, 00:01:33.17\00:01:36.04 is the number one most expensive diagnosis for 00:01:36.26\00:01:39.51 workers' compensation. So, I mean this is an 00:01:39.62\00:01:41.85 obvious question, but what is your most memorable 00:01:41.91\00:01:45.46 patient that you have seen with low back pain. 00:01:45.74\00:01:48.28 Well, you know, I saw one recently that comes to 00:01:48.85\00:01:50.99 mind. So, gentlemen who had five years earlier had 00:01:51.10\00:01:56.15 an episode where he slipped and fell while carrying a 00:01:56.18\00:01:59.12 heavy object at work. And the pain in his back 00:01:59.15\00:02:03.85 was excruciating, he had drove him to go see the 00:02:04.12\00:02:06.43 doctor after he saw the doctor, they did an MRI and 00:02:06.63\00:02:09.51 they decided he needed a surgery. They took him to 00:02:09.54\00:02:13.09 surgery, took out a disc and he has had for six 00:02:13.18\00:02:16.66 months after that. He did fine, but then it was all the 00:02:16.74\00:02:20.47 same again. And the pain was driving him to seek more 00:02:20.64\00:02:24.89 medical help, he had gotten more MRIs and so finally 00:02:25.02\00:02:28.09 they said there is nothing more we can do. 00:02:28.12\00:02:30.76 Well, then he came to see me for a second opinion 00:02:31.48\00:02:33.69 to see what he could figure out. He was telling me 00:02:33.73\00:02:36.56 that he no longer had energy to do anything, 00:02:36.72\00:02:39.13 he had this low aching pain in his back, he couldn't 00:02:39.18\00:02:42.59 go out with his kids and play basketball, and he is 00:02:42.68\00:02:45.30 wishing he could. He was in his early 50s and his 00:02:45.44\00:02:47.96 kids were in their teenage years and he was just 00:02:48.49\00:02:50.54 feeling like he is getting left out. I think he was 00:02:50.57\00:02:52.98 really coming to me because he wanted disability. 00:02:53.07\00:02:55.84 But you didn't give him disability. You, you must 00:02:57.55\00:03:00.82 have suggested something else or you wouldn't tell 00:03:00.93\00:03:02.54 me the story. Yeah, I don't see disability as the 00:03:02.71\00:03:06.47 best solution necessarily and so I talked with him a 00:03:06.69\00:03:10.15 little bit and discussed with him what was happening, 00:03:10.27\00:03:13.40 I did an exam I could tell there was nothing that was, 00:03:13.89\00:03:17.37 you know, pressing as far as a neurological deficit or 00:03:17.98\00:03:21.42 anything. The low back pain was in the middle of his 00:03:21.46\00:03:24.26 back, it radiated little bit down to the side of his 00:03:24.41\00:03:27.16 back and, and pelvis, sometimes to his legs. 00:03:28.81\00:03:32.78 He told me that he was working at a job, 00:03:34.43\00:03:36.22 where he stock shelves in a local department store 00:03:36.27\00:03:40.19 and he was getting to where he couldn't do that 00:03:40.83\00:03:43.03 anymore, it was starting to really bug him. 00:03:43.08\00:03:45.51 So, what do you tell him to do? Well, I decided to 00:03:46.91\00:03:50.26 talk to him about this. I said, now sir you know, 00:03:50.36\00:03:53.39 you have got this big problem you had it for five 00:03:53.58\00:03:55.48 years it's destroying your life. Would you be willing 00:03:55.64\00:03:58.78 to try something just for one week? I mean even 00:03:58.90\00:04:01.68 if it was really, you know, difficult and challenging, 00:04:01.82\00:04:04.69 but maybe we can get you through this and he 00:04:04.78\00:04:06.49 said sure I'll try anything for one week. I said, 00:04:06.54\00:04:08.59 you know, you know, there is a program we will 00:04:08.71\00:04:11.05 try for one week and, and at the end of that week 00:04:11.18\00:04:14.61 I'll reevaluate you and see how you are doing. 00:04:14.82\00:04:17.50 So, he says okay. So, I said, alright. First thing, 00:04:17.75\00:04:20.76 I want you to do is draw the schedule here for me. 00:04:21.28\00:04:24.45 Now, I talked him about his lifestyle, his diet and 00:04:27.57\00:04:28.54 everything. So, I kind of knew what he was already 00:04:28.55\00:04:29.94 doing. Okay. And so I started down the schedule 00:04:30.13\00:04:32.94 I said the first thing you do when you get up in the 00:04:33.10\00:04:35.06 morning as I want you to drink two glasses of water. 00:04:35.27\00:04:40.23 Two glasses of water or more, it might be good for 00:04:41.00\00:04:42.91 people watching to take down this and they can try 00:04:43.02\00:04:44.71 it for a week, right? That's right, if you are having 00:04:44.79\00:04:46.99 low back pain. Two glasses of water; start up. 00:04:47.07\00:04:49.36 That's right, two glasses of water and as soon as you 00:04:49.82\00:04:53.33 get up in the morning start the hydration early. 00:04:53.42\00:04:56.26 Okay. And then when you eat breakfast I want you 00:04:56.90\00:04:59.34 to eat something specific for breakfast. I asked him 00:04:59.37\00:05:02.38 if he had ever eaten oatmeal. He says, oh, yeah, 00:05:02.47\00:05:04.10 I have. I said, alright I want you to eat oatmeal 00:05:04.37\00:05:07.29 for breakfast. All week. All week. For the first 00:05:07.60\00:05:10.50 week. First week. I told him now this is a necessarily 00:05:10.53\00:05:14.00 the way you are gonna live the rest of your life. 00:05:14.03\00:05:15.91 But this week. But this week you are on a therapeutic 00:05:16.06\00:05:18.81 program. Alright. So, he said okay, I said now with 00:05:19.07\00:05:21.91 your oatmeal, I don't want you to put oils on it, 00:05:21.94\00:05:24.38 butter, margarine or anything like that. Any dates. 00:05:24.45\00:05:27.58 You can put in a few dates or raisins, but and keep the 00:05:28.23\00:05:31.69 salt to minimum, no sugar. Okay. Because we wanna 00:05:31.77\00:05:34.71 really avoid refined things and then as soon as 00:05:34.85\00:05:39.34 you finish eating breakfast. I want you to take a 00:05:39.66\00:05:43.57 walk for 10 minutes. And that's after each meal of 00:05:43.71\00:05:47.55 the day, you know, and so 10 minute walk in the 00:05:47.60\00:05:50.60 open air even if it's raining or snowing just put on 00:05:50.92\00:05:53.57 appropriate clothing, get outside also with each 00:05:53.80\00:05:57.40 meal take 500 mg of vitamin C, and then between 00:05:57.51\00:06:04.62 breakfast and lunch about two hours after 00:06:04.81\00:06:08.82 breakfast I want you to take two more glasses of water, 00:06:08.87\00:06:13.57 but avoid drinking with your meals just drink your 00:06:14.39\00:06:16.53 water between meals. Now, if you are used to 00:06:16.68\00:06:18.61 eating between meals I told him and you start 00:06:18.75\00:06:21.45 getting hungry, drink a glass of ice water, and just 00:06:21.53\00:06:26.36 douche the stomach in cold water and it usually 00:06:26.52\00:06:28.84 fixes the problem. Then when you get the lunch time 00:06:28.97\00:06:31.84 on your special program for this week, I want you 00:06:32.28\00:06:34.66 to eat nothing but vegetables preferably raw as much 00:06:34.76\00:06:38.43 as you want, but don't eat vegetables that have 00:06:38.46\00:06:42.13 lots of oils or salt. You can eat cooked vegetables, 00:06:42.31\00:06:46.34 but make sure they are not overly, they have sugar 00:06:46.53\00:06:50.98 with them or oil with them and then on your salad 00:06:51.04\00:06:54.14 make sure you don't eat too much salad dressing, 00:06:54.20\00:06:56.14 low fat if you have to have some or else make a 00:06:56.43\00:06:58.88 good dressing and then of course with your lunch, 00:06:58.95\00:07:01.64 your vitamin C, your 10 minute walk between lunch 00:07:01.76\00:07:04.82 and supper another two glasses of water. 00:07:04.90\00:07:08.23 And for supper I want you to eat nothing, 00:07:09.23\00:07:12.77 but fruit preferably fresh fruit. Fruit that is in his 00:07:12.93\00:07:17.68 natural state as possible with all the fiber, 00:07:17.94\00:07:20.47 no refining, no juices, and eat at least three hours 00:07:20.61\00:07:25.88 before bedtime, take your vitamin C, walk immediately 00:07:26.04\00:07:30.17 after supper for 10 minutes and then between supper 00:07:30.51\00:07:34.34 and bedtime, drink your two glasses of water again 00:07:34.39\00:07:37.99 that will make a total of 8 glasses of water for the day. 00:07:38.43\00:07:41.01 Eight glasses and then what are the 1500 mg of 00:07:41.88\00:07:44.99 vitamin C? That's right, lot's of vitamin C, lots of 00:07:45.78\00:07:49.14 water, good diet, lots of exercise and then I told 00:07:49.28\00:07:53.64 him be sure you avoid things like coffee, tea, 00:07:53.67\00:07:57.71 alcohol, things that would give you back trouble. 00:07:58.19\00:08:02.66 And so that was the program that I put him on and 00:08:03.32\00:08:06.82 I send him out of the office. So, why did you tell 00:08:06.90\00:08:09.05 him to drink so much water? Well, you know, 00:08:09.15\00:08:11.88 the discs in your back are largely made out of 00:08:13.13\00:08:18.22 water and we can put up a graphic here and on our 00:08:18.26\00:08:22.04 graphic we have a picture of a vertebrae above 00:08:22.12\00:08:25.12 and a vertebrae below, and I've colored the middle 00:08:25.55\00:08:27.97 disc yellow. Those discs are made up of about 70 00:08:28.45\00:08:33.29 to 80% water. For in the water your disc kind a 00:08:33.32\00:08:36.44 shrinks. That's right, it's like a tire that runs out 00:08:36.57\00:08:39.37 of air, your disc runs out of water and most people 00:08:39.40\00:08:42.81 live their lives in dehydration. Now, if we put 00:08:43.28\00:08:46.87 up the next graphic, the point there is that if you 00:08:46.91\00:08:50.56 look at the disc it only has a blood supply to the 00:08:50.78\00:08:54.87 edge of it to the, what we call the annulus. 00:08:55.12\00:08:58.73 And when the blood supplies the annulus, 00:08:59.44\00:09:01.68 the rest of the disc is dependent upon diffusion or 00:09:02.07\00:09:06.63 soaking through of the nutrients into the disc and 00:09:06.85\00:09:10.36 the oxygen into the disc and all has to soak 00:09:10.43\00:09:12.99 through and or pump through by the action of 00:09:13.08\00:09:17.09 your walking. These discs are especially vulnerable 00:09:17.14\00:09:21.74 to dehydration to poor nutrition to anything 00:09:22.13\00:09:25.38 that affects the vessels running to them. 00:09:25.50\00:09:28.41 So, drinking plenty of water keeps the disc full, 00:09:28.91\00:09:31.51 it keeps the medium in the disc that lets nutrients 00:09:31.71\00:09:34.30 flow from the vessels to the centre of it. So, you have 00:09:34.52\00:09:37.78 plan, you had to drink a lot of water, so the enough 00:09:38.06\00:09:40.15 water to send, if you had a walk, so it pushes and 00:09:40.27\00:09:43.14 squeezes it through there by diffusion these were the 00:09:43.29\00:09:46.06 ideas. That's exactly right. And then let me guess, 00:09:46.16\00:09:50.01 the oatmeal and all of the healthy food was, 00:09:50.39\00:09:52.62 the vessels would be nice and clean and deliver 00:09:52.65\00:09:55.67 everything he needs for his disc. That's exactly 00:09:55.86\00:09:58.80 right, and to talk little bit more about the walking. 00:09:58.85\00:10:01.35 The reason I had him walk after each meal was 00:10:02.19\00:10:05.74 because when you just finished eating, 00:10:05.98\00:10:09.17 you have a lot of nutrients pouring out of your 00:10:09.98\00:10:13.79 stomach, out of your intestines into your blood 00:10:14.08\00:10:16.15 stream. And if the blood becomes stagnate during 00:10:16.29\00:10:19.68 that period of time, it tends to slug up and that 00:10:19.84\00:10:24.49 decreases the oxygen and nutrients to the disc. 00:10:24.57\00:10:28.40 Also walking as it pumps the disc helps to move 00:10:29.03\00:10:32.95 nutrients in and out of the disc, further if you are 00:10:33.25\00:10:36.35 walking out in the open air; you have a lot higher 00:10:36.48\00:10:39.40 oxygen in your bloodstream. And as the oxygen 00:10:39.59\00:10:43.02 that were trying to get to these discs, to get 00:10:43.18\00:10:45.82 these cells active again, pumped up and fluffy. 00:10:46.19\00:10:49.11 Pumped up and fluffy, so that they end up healing 00:10:49.71\00:10:52.44 any damage done to the disc and that they also 00:10:52.67\00:10:56.41 remove irritations, waste products. So, why no oil 00:10:56.83\00:11:01.21 or sugar for breakfast. Oils and sugar. 00:11:01.29\00:11:04.43 I usually have my oatmeal and my brown sugar, 00:11:04.46\00:11:06.56 so why did you take that away from me? 00:11:06.94\00:11:08.10 Well, because I'm mean. No, really because the 00:11:08.24\00:11:12.76 sugar in the diet. You know, we won't created 00:11:12.85\00:11:14.92 the sugar, you have to go look in for the sugar in 00:11:15.16\00:11:17.77 order to find it the way you find is you have to boil 00:11:17.83\00:11:20.60 down a whole bunch of sugarcane or beets or 00:11:20.72\00:11:22.55 something and then you make this white chemical 00:11:22.91\00:11:25.03 looking stuff that's granular and you put in your 00:11:25.18\00:11:27.46 food is not really good for it. We can talk a lot 00:11:27.62\00:11:31.14 about that, but sugar when it gets in the 00:11:31.22\00:11:33.24 bloodstream does a couple of things, one when 00:11:33.41\00:11:37.06 you eat high glycemic index foods that is foods that 00:11:37.17\00:11:40.36 have a lot of sugar. Foods that put a lot of 00:11:40.46\00:11:42.84 sugar in the bloodstream fast; it creates the 00:11:42.91\00:11:47.17 environment for sugar to stick to proteins permanently. 00:11:47.24\00:11:51.36 We called it glycosylation. When the sugar sticks 00:11:51.89\00:11:55.18 to the vessels that surround the disc, then the disc 00:11:55.26\00:11:59.29 cannot get nutrients because the nutrients can't 00:11:59.79\00:12:02.14 go by the sugar barrier. When the sugar gets high 00:12:02.28\00:12:05.95 in the bloodstream it sticks to the red blood 00:12:05.98\00:12:07.97 cells and they become stuck together or what 00:12:08.15\00:12:12.29 we called glycosylated, hemoglobin A1C is what we 00:12:12.46\00:12:16.17 called it and you can test for it, but when you get 00:12:16.32\00:12:18.77 these, the sugar sticking all over everything 00:12:19.12\00:12:21.58 pretty soon blood cells stick to the sides of blood 00:12:21.94\00:12:25.22 vessels, the sugar doesn't allow nutrients to pass 00:12:25.30\00:12:28.83 from the blood cell to the blood vessel to the adjacent 00:12:28.86\00:12:32.57 tissues. It's a big problem, further the blood cells 00:12:32.72\00:12:36.24 all stick together, it becomes stacked up, 00:12:36.29\00:12:39.06 the affect we called a Rulo effect. Rulo. Rulo 00:12:39.49\00:12:44.14 is kind of a French word, its kind a like that, 00:12:44.17\00:12:46.59 I like. Roulette, like Roulette. Well, I don't know 00:12:46.62\00:12:49.77 about that's right, but anyway the cells all stick 00:12:49.80\00:12:52.49 together in a stack and they won't make it 00:12:52.65\00:12:55.28 through the capillaries like they should. 00:12:55.45\00:12:57.35 So, you have, you gave him the water, you gave him 00:12:57.98\00:13:00.97 this diet with no sugar, but also no fat, 00:13:01.10\00:13:05.89 no oils you said. That's right; oils have the same 00:13:06.66\00:13:10.05 affect of sugars of causing the cells to all 00:13:10.24\00:13:13.26 stack together. Okay, so they make it viscous and 00:13:13.67\00:13:16.61 doesn't get the oxygen or other nutrients there. 00:13:16.73\00:13:18.94 That's right. Okay, well I want to see what 00:13:19.02\00:13:21.91 happens with this man. And I'm sure everybody 00:13:22.08\00:13:23.88 else does too. Well, I think 00:15:33.89\00:15:37.11 we have a list of it. We can look through it and 00:15:37.14\00:15:38.79 summarize it again. And we will look at that list 00:15:38.92\00:15:41.88 and then we will catch up to what we were and we 00:15:42.22\00:15:45.12 were looking at some of these different areas 00:15:45.15\00:15:47.59 that you were talking about we will pick that up 00:15:47.66\00:15:49.75 again. Vitamin C 500 mg 00:16:07.69\00:16:13.09 and then for lunch. I think he walked right after 00:16:13.94\00:16:18.54 breakfast as well too, isn't that correct? Yes. 00:16:18.58\00:16:20.08 And then you could drink a couple of glasses 00:16:20.77\00:16:23.77 after, after, after breakfast sometime there couple 00:16:24.06\00:16:26.60 of hours afterwards. In lunch, vegetables of any 00:16:26.63\00:16:29.99 kind raw preferred no oils or different things on 00:16:30.13\00:16:33.68 them, we talked about why? You would want to put 00:16:33.79\00:16:36.06 it that on them. It's test week. 00:22:36.94\00:22:39.04 And I started picking up that this was a stress 00:22:39.73\00:22:41.82 related phenomenon. Part of taking a walk in 00:22:41.97\00:22:45.52 the outdoors after each meal is about relieving 00:22:45.55\00:22:48.72 stress. So, you tell this guy never to take tests 00:22:48.87\00:22:51.30 or what? Well, I don't think it would have work; 00:22:51.37\00:22:55.09 there is something's you can't tell people, 00:22:55.48\00:22:56.88 won't you. He said, you know, when I kept silent my 00:23:09.59\00:23:12.11 bones growl throughout my groaning all the day 00:23:12.36\00:23:15.13 you know. That was interesting. And the hand 00:23:15.18\00:23:16.62 was heavy upon me, so for about a year I 00:23:16.91\00:23:19.51 guess we figured out you know, Psalm 32 00:23:19.58\00:23:22.45 coupled with Psalm 51, we realize that he was 00:23:22.74\00:23:25.48 really going through that, so he was experiencing 00:23:25.65\00:23:27.53 probably low back pain at least some kind of 00:23:28.24\00:23:30.08 pain in his bones because of that. That's right, 00:23:30.29\00:23:33.87 you know, studies have shown that people who 00:23:33.92\00:23:36.16 are under stress especially if the stress is related 00:23:36.27\00:23:39.94 to childhood abuse have a higher incidence of 00:23:40.11\00:23:43.29 low back pain. That's right, so like you know, 00:23:43.51\00:23:45.79 you got your firemen, let's say, you know, 00:23:45.95\00:23:49.81 I have got a friend who is a chapel for firemen, 00:23:50.24\00:23:51.89 I used to be a paramedic and everything, 00:23:52.13\00:23:54.18 these people are under terrible stress, but they 00:23:54.41\00:23:56.11 also have all kinds of back problems, we usually 00:23:56.18\00:23:58.51 associated just with the kind of lifting their joint, 00:23:58.66\00:24:01.38 but we are suggesting is it could be actually 00:24:01.50\00:24:03.59 related to the stress itself. That's right, 00:24:03.72\00:24:06.68 you know, we have people who injure their 00:24:06.86\00:24:08.53 backs and are being lifelong involute. Well, 00:24:08.77\00:24:12.88 that's interesting because bones heal in the 00:24:12.92\00:24:15.07 matter of weeks, ligaments tendons and joints heal 00:24:15.23\00:24:19.22 in a matter of you know, few months, 00:24:19.28\00:24:21.32 well why are they spending the rest of the 00:24:21.69\00:24:23.37 life with pain. So, this man he was like, 00:24:23.67\00:24:26.73 you have given him like a seven day test, 00:24:27.08\00:24:29.04 you went quite Daniel one with the 10 days, 00:24:29.16\00:24:30.82 test but your seven day test with kind of Daniel's 00:24:30.94\00:24:34.13 diet here, what happened when he came back 00:24:34.54\00:24:37.26 to your office, was he, what were the results? 00:24:37.60\00:24:42.12 Well, you know, I thought he would leave and 00:24:42.46\00:24:44.59 just go home and say, Oh fooly, what's that 00:24:44.68\00:24:46.82 doctor think that he is a nut, well anyway he went 00:24:47.03\00:24:49.96 home and he was gone and he came back in a week 00:24:50.19\00:24:53.53 and as he walked down the hall, I tried to watch 00:24:53.60\00:24:56.96 him, to see what. His gait was like. What his gait 00:24:56.99\00:25:00.20 was like and I can't tell a whole lot, he was, 00:25:00.36\00:25:02.35 was like and I can't tell a whole lot, he was, 00:25:02.38\00:25:03.35 you know, he is kind a hit his feelings well and I 00:25:03.36\00:25:05.19 just couldn't tell I was thinking okay well. Get out 00:25:05.22\00:25:08.05 the disability papers. Well, he came in, he sat 00:25:08.08\00:25:10.99 down and says, well so how do you go, he said, 00:25:11.15\00:25:14.44 you know, within three days, I could tell a 00:25:14.57\00:25:17.85 dramatic difference, I no longer had that died 00:25:17.98\00:25:22.23 heavy tired feeling in my back, but you know I 00:25:22.73\00:25:25.79 peed an awful lot. You know, I ended up 00:25:26.12\00:25:27.96 drinking a lot of ice water because I was always 00:25:28.03\00:25:30.32 hungry because I'm used to eating between meals 00:25:30.40\00:25:33.49 and he says, I didn't feel a lot, but during breaks 00:25:34.14\00:25:37.08 I was wide awake when everybody else was 00:25:37.57\00:25:39.57 asleep. And he says, I had no trouble on my jobs, 00:25:39.94\00:25:43.98 stocking shelves, he says, this is just you know, 00:25:44.11\00:25:46.41 great thank you so much. How successful 00:26:55.97\00:26:58.94 are surgeries as a treatment for low back pain. 00:26:58.99\00:27:01.95 Certain types of problems with the back need 00:27:03.72\00:27:06.57 surgery, but there is a lot of excess surgery 00:27:06.72\00:27:09.53 done in our world, it's because the surgeon gets 00:27:09.75\00:27:14.66 paid by doing surgery not by telling patients about 00:27:14.88\00:27:17.92 a low fat vegetarian diet. And so there is 00:27:18.31\00:27:22.30 a lot of, you know, financial incentive to the 00:27:22.48\00:27:25.29 way our system works, pain is rewarded and 00:27:25.41\00:27:28.18 that's a whole another topic to get into, 00:27:28.99\00:27:30.56 but surgery often times you end up with the patient 00:27:30.63\00:27:34.36 that needs another surgery and another surgery. 00:27:34.50\00:27:36.92 Okay. And about 50% of time it does not work. 00:27:36.95\00:27:40.49 So, really this is not the best option, 00:27:41.24\00:27:43.27 the surgical option it is just is like catch up 00:27:43.41\00:27:46.23 work really if you go that way.
