Hello and welcome to Health for a Lifetime. 00:00:49.49\00:00:51.57 I'm your host Don Mackintosh. 00:00:51.60\00:00:52.77 We're glad that you've joined us. 00:00:52.80\00:00:54.13 Today we're talking about a very important subject. 00:00:54.16\00:00:57.47 Many Americans are struggling with a disease called diabetes. 00:00:57.50\00:01:01.01 Perhaps you yourself struggle with this. 00:01:01.04\00:01:03.50 I'm sure that if you don't you know someone that does. 00:01:03.53\00:01:06.38 Today we're going to be joined with someone who is really an 00:01:06.41\00:01:10.28 expert in dealing with this. 00:01:10.31\00:01:11.88 He is a specialist in internal medicine. 00:01:11.91\00:01:14.33 His name is Dr. Victor Herry. 00:01:14.36\00:01:16.68 He's from the Washington, D.C. area where he practices 00:01:16.71\00:01:19.79 and has done so for over 20 years. 00:01:19.82\00:01:23.39 Dr. Herry, we're glad you're with us today. 00:01:23.42\00:01:25.29 Thank you. 00:01:25.32\00:01:26.29 You know, diabetes is something that many people 00:01:26.32\00:01:31.92 have heard about. 00:01:31.95\00:01:32.92 There may be someone out there that doesn't know 00:01:32.93\00:01:34.82 what diabetes is. 00:01:34.85\00:01:36.04 What is it exactly? 00:01:36.07\00:01:37.94 Diabetes actually is a group of disorders in the body resulting 00:01:37.97\00:01:46.24 in elevated blood sugar in the blood stream. 00:01:46.27\00:01:50.03 Underlying all of this is maybe a defect in the pancreas. 00:01:50.06\00:01:59.22 The pancreas is that organ lies right below the stomach and 00:01:59.25\00:02:02.78 that produces insulin. 00:02:02.81\00:02:04.68 In some people the insulin production is not enough 00:02:04.71\00:02:09.46 or absent. 00:02:09.49\00:02:11.03 In some people the insulin production is adequate but the 00:02:11.06\00:02:16.12 body is not accepting the insulin to push the sugar 00:02:16.15\00:02:19.34 into the blood cell. 00:02:19.37\00:02:20.38 The blood sugar remains in the blood stream. 00:02:20.41\00:02:23.37 It causes an elevation of blood sugar and so it is called 00:02:23.40\00:02:26.94 diabetes or hyperglycemia - elevated blood sugar. 00:02:26.97\00:02:31.83 They talk about two types of diabetes - type 1, type 2 00:02:31.86\00:02:36.07 What's the difference between those? 00:02:36.10\00:02:37.54 There are two types. 00:02:37.57\00:02:41.01 Type 1 is sometimes referred to as juvenile onset diabetes. 00:02:41.04\00:02:46.70 That has to do with the death of the cells in the pancreas 00:02:46.73\00:02:52.02 that produces insulin - the absence of those cells. 00:02:52.05\00:02:55.20 It is an autoimmune cell. 00:02:55.23\00:02:58.79 The body destroys those cells, the beta cells of the pancreas. 00:02:58.82\00:03:03.24 so that young person does not produce insulin. 00:03:03.27\00:03:07.93 That usually is from birth or it can happen at any time. 00:03:07.96\00:03:12.01 Anytime from the age 1 to age 18 before the person is an adult. 00:03:12.04\00:03:19.29 Now, you say the body attacks itself. 00:03:19.32\00:03:22.54 Why does it happen? 00:03:22.57\00:03:24.77 Do they know why it happens? 00:03:24.80\00:03:25.77 Why it happens we're not sure. 00:03:25.78\00:03:27.24 It's just autoimmune and that happens with many other 00:03:27.27\00:03:30.45 kinds of diseases. 00:03:30.48\00:03:31.45 The basis of it we're not quite sure why it happens. 00:03:31.46\00:03:35.56 Then there is the type 2. 00:03:35.59\00:03:38.24 The type 2 used to be called adult onset diabetes. 00:03:38.27\00:03:42.63 However, because now it is found in children it is called type 2 00:03:42.66\00:03:49.86 and not adult onset anymore. 00:03:49.89\00:03:51.57 The reason for that is because of changing in life style. 00:03:51.60\00:03:56.45 We find that we have young children who are markedly 00:03:56.48\00:04:00.12 overweight who do not exercise and as a result of that the 00:04:00.15\00:04:04.17 sugar in the blood stream becomes so high that they 00:04:04.20\00:04:08.70 develop diabetes. 00:04:08.73\00:04:10.03 So its changed from being adult onset to type 2 and that's 00:04:10.06\00:04:14.70 really a warning shot. 00:04:14.73\00:04:15.99 That makes this program even more important. 00:04:16.02\00:04:18.06 That's right because children have type 2 diabetes now. 00:04:18.09\00:04:21.26 So what are the symptoms of diabetes? 00:04:21.29\00:04:23.94 How can I know I have it? 00:04:23.97\00:04:24.95 There could be multiple symptoms. 00:04:24.98\00:04:29.34 One of the things one must remember is that if you have 00:04:29.37\00:04:34.13 a family history of diabetes 00:04:34.16\00:04:35.95 then one is to look for diabetes. 00:04:35.98\00:04:39.33 Some of the things like increased thirst. 00:04:39.36\00:04:42.26 One finds that one wants to drink water. 00:04:42.29\00:04:45.23 They're thirsty all the time. 00:04:45.26\00:04:46.58 Increased thirst is an indication. 00:04:46.61\00:04:48.60 Or one goes to the restroom a lot to pass water. 00:04:48.63\00:04:53.77 That too is an indication that there is diabetes. 00:04:53.80\00:04:58.17 So increased thirst, increased running to the bathroom 00:04:58.20\00:05:05.03 to pass water. 00:05:05.06\00:05:06.03 Increased eating, eating a lot, that may be an indication 00:05:06.04\00:05:10.35 that there is diabetes. 00:05:10.38\00:05:11.93 So those are some of the symptoms that one would find 00:05:11.96\00:05:14.70 that might indicate and of course there's a family history 00:05:14.73\00:05:18.47 that that person has diabetes. 00:05:18.50\00:05:20.63 But the only way you can know is to check the numbers. 00:05:20.66\00:05:22.66 Got to check the numbers - so what are normal numbers 00:05:22.69\00:05:28.03 when you get them checked? 00:05:28.06\00:05:29.98 Before one eats, if one is in a fasting state, that is you have 00:05:30.01\00:05:36.75 not eaten before midnight, and early in the morning, let's say 00:05:36.78\00:05:40.46 6:00, you're going to check your blood sugar. 00:05:40.49\00:05:43.04 If your blood sugar is greater than 110 it indicates that 00:05:43.07\00:05:49.96 you may be diabetic. 00:05:49.99\00:05:52.00 Your body is not handling sugar properly. 00:05:52.03\00:05:55.58 That's a different number than when I went to nursing school. 00:05:55.61\00:05:59.49 A number of years ago they said 120. 00:05:59.52\00:06:01.49 But it is now even going down. 00:06:01.52\00:06:03.98 As a matter of fact they're now thinking of putting it 00:06:04.01\00:06:06.47 down to 100. 00:06:06.50\00:06:07.48 So 100 to 110 fasting blood sugar in the morning. 00:06:07.51\00:06:13.27 Now, 2 hours after a normal meal if your blood sugar is greater 00:06:13.30\00:06:19.87 than 140 and even greater than 200 then the indications are 00:06:19.90\00:06:27.96 that you are diabetic. 00:06:27.99\00:06:29.09 So 2 hours after eating. 00:06:29.12\00:06:32.41 We call that post prandial. 00:06:32.44\00:06:34.35 You actually draw the blood right then? 00:06:34.38\00:06:36.70 Yes, draw the blood 2 hours after one eats. 00:06:36.73\00:06:39.49 The only way you can really know then is by those tests? 00:06:39.52\00:06:42.86 Yes, because what happens is there are many people who 00:06:42.89\00:06:46.70 have had observations of themselves. 00:06:46.73\00:06:49.18 For instance, patients would come and say, "I've been loosing 00:06:49.21\00:06:52.89 a lot of weight and I notice I've been running to the 00:06:52.92\00:06:55.22 bathroom. " 00:06:55.25\00:06:56.22 Weight loss, running to the bathroom frequently, I've been 00:06:56.23\00:06:58.63 hungry all the time, I've been drinking a lot of water - those 00:06:58.66\00:07:03.02 are indications but even then, even before that state comes, 00:07:03.05\00:07:08.06 if there's a family history one should annually check to make 00:07:08.09\00:07:14.68 sure that there is no indication that the blood sugar 00:07:14.71\00:07:17.02 is running high. 00:07:17.05\00:07:18.02 So we've talked about what it is, type 1 and type 2. 00:07:18.03\00:07:22.37 We've talked about some symptoms. 00:07:22.40\00:07:23.62 You also have down, what are the causes? 00:07:23.65\00:07:28.08 What causes diabetes? 00:07:28.11\00:07:33.28 First of all I mentioned that diabetes can be genetic. 00:07:33.31\00:07:37.73 So that through the genes. 00:07:37.76\00:07:42.35 However, the environment is very important both the internal 00:07:42.38\00:07:45.62 and the external environment. 00:07:45.65\00:07:47.14 What do I mean by that? 00:07:47.17\00:07:48.19 Now a person who is over weight is subject to diabetes. 00:07:48.22\00:07:54.36 So one cause of diabetes is obesity. 00:07:54.39\00:07:57.25 If we notice that we are becoming heavier and heavier 00:07:57.28\00:08:02.24 every day because we eat and we do not exercise. 00:08:02.27\00:08:06.07 Ok, so lack of exercise. 00:08:06.10\00:08:08.41 That's one cause. 00:08:08.44\00:08:10.69 Another cause of diabetes, as I mentioned, increased weight, 00:08:10.72\00:08:15.47 obesity, lack of exercise, bad habits of eating, eating foods 00:08:15.50\00:08:25.86 high in carbohydrates, overwhelming the body so that 00:08:25.89\00:08:31.12 the body cannot handle those. 00:08:31.15\00:08:32.84 Foods high in carbohydrates would be like potatoes, rice, 00:08:32.87\00:08:37.18 pasta. 00:08:37.21\00:08:38.18 I'm not saying that they are not good, I'm saying if you 00:08:38.19\00:08:40.65 eat too much of them the body is overloaded with the sugar 00:08:40.68\00:08:45.00 and glucose so your body cannot handle it. 00:08:45.03\00:08:47.74 So you've got to watch that. 00:08:47.77\00:08:49.78 We hear about the glycemic index of foods. 00:08:49.81\00:08:53.74 So you're saying foods high in the glycemic index? 00:08:53.77\00:08:56.28 No, I'm glad you mentioned glycemic index - that's correct. 00:08:56.31\00:09:01.89 We also have carbohydrates in almost everything we eat. 00:09:01.92\00:09:06.49 However, the natural complex carbohydrates are broken down 00:09:06.52\00:09:11.38 very easily. 00:09:11.41\00:09:12.49 Whereas, the ones that I have mentioned in the refined foods 00:09:12.52\00:09:15.62 tend to go through the body very slowly and as a result 00:09:15.65\00:09:19.40 converted to glucose into the blood stream which the body 00:09:19.43\00:09:25.16 cannot handle. 00:09:25.19\00:09:26.16 As a result these individuals develop diabetes. 00:09:26.17\00:09:30.23 The complex carbohydrates are broken down slower and these 00:09:30.26\00:09:35.55 processed foods are broken down faster? 00:09:35.58\00:09:37.96 The natural foods are broken down faster, yes. 00:09:37.99\00:09:42.27 And that causes it just to spike? 00:09:42.30\00:09:45.92 Spike in the blood stream. 00:09:45.95\00:09:47.93 So who is it that is really developing diabetes? 00:09:47.96\00:09:51.97 You've mentioned that it's happening with younger people 00:09:52.00\00:09:54.21 now, but what are the at risk populations? 00:09:54.24\00:09:58.40 The at risk populations people are the black Americans. 00:09:58.43\00:10:02.77 They are at risk for diabetes. 00:10:02.80\00:10:07.07 Why is it that they have such a risk? 00:10:07.10\00:10:09.57 It's probably both genetic and lifestyle. 00:10:09.60\00:10:12.64 There are certain areas of the United States, for instance 00:10:12.67\00:10:16.12 in the South East where that population diabetes is very, 00:10:16.15\00:10:21.32 very high in that African American population. 00:10:21.35\00:10:24.75 The lifestyle there in terms of what people eat and in many 00:10:24.78\00:10:31.69 cases lack of exercise activity resulting in a high incidence 00:10:31.72\00:10:36.01 of diabetes. 00:10:36.04\00:10:37.06 So tell me specifically what are they eating? 00:10:37.09\00:10:39.41 Foods that are high in carbohydrates, fat, foods that 00:10:39.44\00:10:47.28 produce high cholesterol and as a result you have increased 00:10:47.31\00:10:51.96 size and result in diabetes. 00:10:51.99\00:10:55.91 Those are the components of the foods but give me some 00:10:55.94\00:10:57.51 dishes, what are the names? 00:10:57.54\00:10:58.51 Rice, pasta, potatoes, ham hocks, all of those. 00:10:58.52\00:11:09.34 Ok, all of those different things. 00:11:09.37\00:11:10.72 People are going to go, "Oh, no!" because they are 00:11:10.75\00:11:14.06 very tasty foods. 00:11:14.09\00:11:16.34 Ok, Black Americans and who else are at risk populations? 00:11:16.37\00:11:20.92 The elderly. 00:11:20.95\00:11:22.42 As you age you tend to loose some of your beta cells 00:11:22.45\00:11:28.19 and as a result if your diet is improper then you are at risk. 00:11:28.22\00:11:32.68 Just getting older? 00:11:32.71\00:11:34.44 Yes. 00:11:34.47\00:11:36.04 The elderly Americans, the Black Americans, any others? 00:11:36.07\00:11:40.96 Those who don't exercise. 00:11:40.99\00:11:42.60 So that could be young or old. 00:11:42.63\00:11:45.10 We've talked about what it is, the causes, and who develops it. 00:11:45.13\00:11:50.94 What about risk factors? 00:11:50.97\00:11:52.80 Anything different we could add there? 00:11:52.83\00:11:54.55 Now I've mentioned some of the risk factors in diabetes. 00:11:54.58\00:11:58.41 I've mentioned exercise, and I'm coming back to diet, 00:11:58.44\00:12:04.38 coming back to weight gain, and age is a risk factor. 00:12:04.41\00:12:11.34 If you notice we've mentioned genetics as a risk factor. 00:12:11.37\00:12:16.14 Know your genetics. 00:12:16.17\00:12:17.14 If your mother, your father, an aunt or an uncle has diabetes 00:12:17.15\00:12:21.33 it means then that you might be prone to the disease. 00:12:21.36\00:12:25.19 Therefore, you need to take precaution. 00:12:25.22\00:12:27.72 That precaution you need to take, first of all is to check 00:12:27.75\00:12:30.90 with your physician for an annual physical. 00:12:30.93\00:12:33.21 Additionally, you need to be active in your lifestyle. 00:12:33.24\00:12:37.93 You need to exercise. 00:12:37.96\00:12:39.92 What are some of the effects of diabetes? 00:12:39.95\00:12:42.71 In other words, again people say, "Look I got a little more 00:12:42.74\00:12:45.28 sugar in my system, I'm just sweeter than I used to be!" 00:12:45.31\00:12:48.65 What are some of the effects of having high sugar in your blood? 00:12:48.68\00:12:54.36 Here is what high sugar does. 00:12:54.39\00:12:56.89 1. It reduces the oxygen capacity of the blood 00:12:56.92\00:13:01.01 and you need oxygen for clear thinking. 00:13:01.04\00:13:03.35 It increases atherosclerosis - the plaques that build up 00:13:03.38\00:13:09.08 on the vessel sides. 00:13:09.11\00:13:10.49 So diabetes increases that. 00:13:10.52\00:13:13.40 It also increases the risk for heart attack. 00:13:13.43\00:13:16.88 The person who is diabetic is two and a half times more likely 00:13:16.91\00:13:22.30 to get a heart attack than the person who is not. 00:13:22.33\00:13:24.80 It also increases stroke. 00:13:24.83\00:13:27.74 Diabetes also increases stroke. 00:13:27.77\00:13:32.19 So we talked about increased heart attacks, 00:13:32.22\00:13:34.84 increased strokes, and increased heart failure. 00:13:34.87\00:13:40.33 All of those are the consequences. 00:13:40.36\00:13:43.24 Not only that but you notice that in some blindness. 00:13:43.27\00:13:47.39 Because of the fact of the elevated blood sugar 00:13:47.42\00:13:52.75 causes premature growth of additional blood vessels. 00:13:52.78\00:13:57.38 These tiny blood vessels choke off the sight and so 00:13:57.41\00:14:03.71 blindness come as a result of diabetes. 00:14:03.74\00:14:06.86 Increased amputation, loss of limbs because diabetes tend to 00:14:06.89\00:14:14.78 effect the very tiny vessels first. 00:14:14.81\00:14:17.35 The vessels of the eye, the vessels of the extremities, 00:14:17.38\00:14:21.95 the vessels of the kidneys - the tiny vessels - those are 00:14:21.98\00:14:26.76 effected by diabetes. 00:14:26.79\00:14:28.30 That's why in African Americans, for instance, who have a high 00:14:28.33\00:14:32.06 incidence of diabetes there is also high incidence of renal 00:14:32.09\00:14:36.06 failure in African Americans. 00:14:36.09\00:14:38.93 That is because of both diabetes and hypertension. 00:14:38.96\00:14:42.93 When you have the combination of diabetes and hypertension 00:14:42.96\00:14:47.48 it is extremely important that you control your blood pressure 00:14:47.51\00:14:52.76 and that you lower your blood sugar. 00:14:52.79\00:14:54.91 Because any of those can lead to premature death. 00:14:54.94\00:14:58.49 All of these things you have mentioned have to do with the 00:14:58.52\00:15:01.56 smallest capillaries like your eyes, the kidneys, and all these 00:15:01.59\00:15:05.00 different things. 00:15:05.03\00:15:06.00 This really seems to effect, first of all, where the small 00:15:06.01\00:15:09.12 intricate vessels are and just impacting that. 00:15:09.15\00:15:14.82 We're talking with Dr. Victor Herry. 00:15:14.85\00:15:15.97 We're talking about diabetes. 00:15:16.00\00:15:17.59 It's a dangerous disease. 00:15:17.62\00:15:19.44 It's not just something to just say, "Hey, I'm sweeter than 00:15:19.47\00:15:21.84 the day before.
