It is a problem, 00:01:43.20\00:01:45.80 it is a concern and needs to be treated 00:01:45.83\00:01:47.50 and there are other doctors that say 00:01:48.17\00:01:50.12 yes testosterone levels do decreases with age, 00:01:50.15\00:01:54.82 but there is no Andropause, 00:01:55.22\00:01:59.07 that's in anyway analogous to menopause. 00:01:59.10\00:02:01.93 I tend to lean toward that latter school of thought 00:02:03.01\00:02:06.35 that there is not some dramatic climatic event 00:02:06.38\00:02:09.86 in the male hormonal life cycle if you will, 00:02:09.89\00:02:14.14 but by the same token there are real changes 00:02:14.29\00:02:16.70 in testosterone that occur overtime. 00:02:16.73\00:02:18.67 Okay, so you know maybe 00:02:18.85\00:02:20.57 another factor could be drug companies 00:02:20.60\00:02:22.53 that seem to be very happy to find the things 00:02:22.56\00:02:25.29 they can make drugs for 00:02:25.32\00:02:26.29 although that's positive in the many ways, 00:02:26.30\00:02:28.21 you know, someone suggest that, you know, 00:02:28.24\00:02:32.64 they create needs by saying this is a problem, 00:02:32.67\00:02:35.67 that's the problem, you don't think that's a fact. 00:02:35.70\00:02:37.22 Well, I haven't really seen it at least in the things 00:02:37.25\00:02:40.50 I have been reading, it seemed to be something 00:02:40.53\00:02:42.26 being driven by drug companies. 00:02:42.29\00:02:43.84 I mean, sure if there are certain proprietary drugs 00:02:43.87\00:02:47.24 that are being studied may be but the law have to realize 00:02:47.27\00:02:49.78 that these sex steroids 00:02:49.81\00:02:52.88 and what's being marketed are often 00:02:52.91\00:02:54.73 not the sex steroids themselves, 00:02:54.76\00:02:56.33 it's not that they're telling people 00:02:56.36\00:02:57.76 to line up for testosterone shots. 00:02:57.79\00:02:59.38 A lot of people who are promoting this Andropause 00:02:59.41\00:03:02.42 for example through the Internet are people 00:03:03.14\00:03:05.05 that are trying to sell you a whole raft of supplements 00:03:05.08\00:03:07.62 that are designed to boost your testosterone levels, 00:03:07.65\00:03:10.57 make you feel better, more youthful 00:03:10.60\00:03:12.54 in all aspects of human performance. 00:03:12.57\00:03:16.04 What can we do to measure 00:03:16.38\00:03:19.43 to see whether or not the levels are normal? 00:03:19.46\00:03:21.87 Is there a measurement? 00:03:21.90\00:03:22.93 Well actually, there is a measurement 00:03:22.96\00:03:24.53 and you can simply measure testosterone level. 00:03:24.56\00:03:27.25 The problem was just the 00:03:27.55\00:03:28.84 conventional testosterone measurement. 00:03:28.87\00:03:30.85 I'll just illustrate it in the case of an individual, 00:03:30.88\00:03:34.04 who talked with me very recently. 00:03:34.07\00:03:35.68 He had come in contact with someone who, 00:03:36.38\00:03:40.15 I don't know how they do this, to be honest with you, 00:03:40.18\00:03:43.15 but they are phone based and Internet based physicians. 00:03:43.18\00:03:46.73 I guess they will order lab tests for people. 00:03:46.76\00:03:48.76 So this guy had some doctor order this lab panel 00:03:49.05\00:03:53.00 to see if he was testosterone deficient, 00:03:53.03\00:03:57.46 this is a man in his maybe 70s. 00:03:57.49\00:03:59.32 And they just measure what we call a random testosterone, 00:03:59.66\00:04:03.91 what that means is irrespective of 00:04:03.94\00:04:06.65 what time of day 00:04:06.68\00:04:07.65 they measure this blood testosterone level. 00:04:07.66\00:04:09.62 The problem with that Don is the standard that we use 00:04:09.65\00:04:13.09 for looking at whether testosterone is high or low, 00:04:13.12\00:04:15.52 is the measurement made 00:04:15.55\00:04:16.79 between 6:00 and 8:00 am in the morning. 00:04:16.82\00:04:19.02 Why then? 00:04:19.05\00:04:20.02 That's when testosterone typically peaks 00:04:20.03\00:04:22.30 especially in younger man it's very consistent, 00:04:22.33\00:04:25.71 as we get older there may not be as much of a peak 00:04:25.74\00:04:28.13 but that's the time we're looking for 00:04:28.16\00:04:29.72 the maximal level we wanna get it, 00:04:29.75\00:04:31.64 this guy had his level drawn at some other time. 00:04:31.67\00:04:33.57 His level was virtually normal, 00:04:33.60\00:04:35.58 a few points below and on top of that they measured 00:04:35.61\00:04:38.48 an even better test which is free testosterone. 00:04:38.51\00:04:41.81 Okay free meaning in the blood stream. 00:04:41.84\00:04:44.40 Yes, it's in the blood stream not bond to something else, 00:04:44.43\00:04:47.29 many of the compounds in our body 00:04:47.79\00:04:49.43 are hooked up to carriers. 00:04:49.46\00:04:51.47 And so testosterone hooks up to 00:04:51.50\00:04:54.16 something called sex hormone binding globulin 00:04:54.19\00:04:57.53 and that's inactive basically 00:04:58.33\00:05:00.24 while its hooked up to that carrier. Okay. 00:05:00.27\00:05:02.70 The free testosterone is The active testosterone, 00:05:02.73\00:05:05.17 so in the case I am sharing with you, 00:05:05.20\00:05:06.85 this 70-year-old man has this 00:05:06.88\00:05:09.52 blood testosterone measurement made, 00:05:09.55\00:05:12.04 not between 6:00 and 8:00 in the morning, 00:05:12.07\00:05:14.21 close to normal 00:05:14.24\00:05:15.21 then the free testosterone is totally normal. 00:05:15.35\00:05:18.15 Okay. And what happens? 00:05:18.18\00:05:19.59 What! Yeah! What happens? 00:05:19.62\00:05:21.26 This fellow over The phone 00:05:21.75\00:05:23.06 tells him he is well on testosterone. 00:05:23.61\00:05:25.39 And has him to order his product. 00:05:25.42\00:05:26.73 Yeah he has got to order all these natural supplements 00:05:26.76\00:05:29.62 that are gonna raise his testosterone 00:05:29.65\00:05:31.73 and make him, you know, 00:05:31.76\00:05:32.73 feel more like a man. 00:05:32.74\00:05:33.94 Whatever is supposed to happen to him 00:05:33.97\00:05:35.95 and this is really what I would say, you know, 00:05:35.98\00:05:40.33 you are talking about drug companies 00:05:40.36\00:05:42.24 kind of commandeering this 00:05:42.27\00:05:44.11 I am saying there are all kinds of people, 00:05:44.14\00:05:46.45 they are trying to find commercial advantage 00:05:46.48\00:05:48.66 in making this into a great, 00:05:48.69\00:05:51.03 I mean think about it, 00:05:51.06\00:05:52.03 the population is aging and so if you can get every man 00:05:52.04\00:05:55.79 as he gets older convinced that 00:05:55.82\00:05:57.46 he is going through Andropause 00:05:57.49\00:05:58.91 and you have got all kinds of stuff to sell him 00:05:58.94\00:06:00.56 and you could get rich. You could. 00:06:00.59\00:06:03.29 Unfortunately at my website, we don't sell any supplements. 00:06:03.32\00:06:06.43 That's good, yeah, on your website 00:06:06.46\00:06:08.34 you do have something about this subject, 00:06:08.37\00:06:09.84 don't you? 00:06:09.87\00:06:10.84 Right, we've got information, but no supplements, 00:06:10.85\00:06:13.11 my ministry is information ministry, 00:06:13.45\00:06:16.47 we are not trying to sell. 00:06:16.50\00:06:19.60 You are now gonna have someone call up you 00:06:19.63\00:06:21.11 and you gonna do these tests. 00:06:21.14\00:06:22.72 No, I don't order lab tests over phone or anything. 00:06:22.75\00:06:25.66 And the name of that ministry is compass health 00:06:25.69\00:06:27.93 and it's compasshealth.net, compasshealth.net 00:06:27.96\00:06:31.64 You got it. 00:06:31.67\00:06:32.64 And has a lot of information, 00:06:32.65\00:06:33.76 well, then what are normal levels for testosterone. 00:06:34.19\00:06:39.24 Well, we actually use in this country 00:06:39.46\00:06:41.96 a measurement that's a little bit different 00:06:41.99\00:06:44.11 then is used internationally. 00:06:44.14\00:06:45.52 We use something called nanograms per deciliter, 00:06:45.55\00:06:47.97 most people won't know what that measurement means. 00:06:48.00\00:06:51.24 The average person didn't even 00:06:51.27\00:06:52.26 had their testosterone level measured. 00:06:52.29\00:06:54.02 But, the normal range in the US units 00:06:54.22\00:06:57.94 that we are using 300 to 1000, 00:06:57.97\00:07:00.59 that's the range between 6:00 and 8:00 in the morning, 00:07:01.09\00:07:04.50 that's when we're looking at this peak testosterone level. 00:07:04.53\00:07:08.03 So, there is other things to interact with this, 00:07:08.06\00:07:11.05 I see here on your web page 00:07:11.08\00:07:12.68 you say that the pituitary 00:07:12.71\00:07:14.72 and hypothalamus are also related, 00:07:14.75\00:07:17.86 and this and of course they all talk to each other, 00:07:17.89\00:07:19.60 so, just because your level's low 00:07:19.63\00:07:20.95 doesn't mean it's that problem, 00:07:21.51\00:07:23.38 it could be some problem 00:07:23.41\00:07:24.41 with your pituitary or hypothalamus. 00:07:24.44\00:07:26.36 No, you are exactly right, I mean, 00:07:26.39\00:07:27.54 we know the testosterone is made from the testicles 00:07:27.57\00:07:30.12 and so if you have a low testosterone level, 00:07:30.15\00:07:33.39 it doesn't mean you're testicles necessarily are failing 00:07:33.42\00:07:36.78 or could be a pituitary problem. 00:07:36.81\00:07:38.22 That's exactly right, if your pituitary is not making LH, 00:07:38.25\00:07:41.04 Luteinizing hormone, it's the signal 00:07:41.07\00:07:43.12 that tells the testosterone to make this testosterone, 00:07:43.15\00:07:47.83 this male androgen, then that could be 00:07:47.86\00:07:50.35 the problem or the hypothalamus, 00:07:50.38\00:07:51.35 which is in the brain telling the pituitary 00:07:52.81\00:07:55.04 at the base of the brain to put out the LH 00:07:55.07\00:07:57.99 if it's got a problem, 00:07:58.02\00:07:59.17 if you've had a stroke or something else, 00:07:59.20\00:08:01.05 a tumor in the brain. So really Don, 00:08:01.08\00:08:03.73 if you do have a testosterone measurement made, 00:08:03.76\00:08:07.94 okay. Right. 00:08:07.97\00:08:08.94 And we will make it personal, so we have got someone 00:08:08.95\00:08:11.84 who is viewing today and they say boy 00:08:11.87\00:08:13.92 this is all interesting and they wonder if I am, 00:08:13.95\00:08:16.07 you know, got a low testosterone level, 00:08:16.10\00:08:17.64 they go to their doctor 00:08:17.67\00:08:18.64 and the normal range between 300 and 1000, 00:08:18.88\00:08:21.66 they have a testosterone measurement 00:08:21.69\00:08:23.41 that comes back a 150, that's very low 00:08:23.44\00:08:26.51 that should prompt some diagnostic evaluation, 00:08:26.54\00:08:30.60 why the testosterone level is that low. 00:08:30.63\00:08:33.10 Yeah, because it could be the hypothalamus, 00:08:33.13\00:08:35.47 it could be the pituitary, it could be 00:08:35.50\00:08:36.83 all kinds of different things, 00:08:36.86\00:08:37.95 but it's not the end of all is what you are saying. 00:08:37.98\00:08:40.48 Exactly right! 00:08:40.51\00:08:41.48 Okay what are some factors that influence these levels, 00:08:41.49\00:08:46.15 these testosterone levels? 00:08:46.18\00:08:47.63 Well there are a variety of factors 00:08:47.66\00:08:48.94 and just to go back to this one scenario, 00:08:48.97\00:08:51.75 I gave you, this 70-year-old gentlemen. 00:08:51.78\00:08:53.62 Who called up and. 00:08:53.65\00:08:54.62 Yeah, was sold this raft of supplements, 00:08:54.63\00:08:56.20 when talking with this fellow, 00:08:56.66\00:08:58.00 I mean if you met him, 00:08:58.03\00:09:00.32 you would need to talk with him at all, 00:09:00.35\00:09:02.86 you could just see him and you'd realize 00:09:02.89\00:09:04.11 he was overweight. Okay. 00:09:04.14\00:09:06.20 One of the things That Drives 00:09:06.23\00:09:07.51 testosterone levels down is overweight. 00:09:07.54\00:09:09.96 Is that right? 00:09:10.39\00:09:11.36 So, here this guy instead of spending his money 00:09:11.37\00:09:13.79 on a health club membership or better yet, 00:09:13.82\00:09:15.93 get some new walking shoes. 00:09:15.96\00:09:17.02 He's buying all these supplements 00:09:17.05\00:09:18.97 and really if he just had his testosterone measurement, 00:09:20.89\00:09:24.49 measured between 6:00 and 8:00 in the morning, 00:09:24.52\00:09:25.94 it probably would have been normal. 00:09:25.97\00:09:27.03 But, if he was concerned Beyond 00:09:27.06\00:09:28.59 that just exercising more could have 00:09:28.62\00:09:31.80 made the big difference 00:09:31.83\00:09:33.58 and also lowered his risk of 00:09:33.61\00:09:35.54 a whole list of other problems. 00:09:35.57\00:09:36.81 So, losing weight, I mean, every pound, 00:09:36.84\00:09:38.80 the weight you gain is a whole 00:09:38.83\00:09:40.73 bunch of extra blood vessels, 00:09:40.76\00:09:42.28 as this you know body only produces 00:09:42.31\00:09:44.38 so much and if you're 150 pounds, 00:09:44.41\00:09:46.03 but you move to 300, you still have the same 00:09:46.06\00:09:48.88 you know production level, 00:09:48.91\00:09:51.08 is that way it goes down and up? 00:09:51.11\00:09:52.39 Well I can't tell you I understand 00:09:53.02\00:09:54.20 all the physiology between the obesity, 00:09:54.23\00:09:56.39 testosterone relationship, but it is 00:09:56.42\00:09:58.80 a consistent relationship if someone is overweight 00:09:58.83\00:10:01.60 and they lose weight. 00:10:01.63\00:10:02.78 They can expect those testosterone levels to rise. 00:10:02.81\00:10:05.26 So, you have here on the web page 00:10:05.29\00:10:07.00 that other things can cause it to increase or decrease, 00:10:07.03\00:10:10.06 one is acute illness, any kind of illness, pneumonia. 00:10:10.09\00:10:14.26 Exactly if you've been acutely sick 00:10:14.71\00:10:17.17 those testosterone levels could be suppressed, 00:10:17.20\00:10:19.49 you know, a good way of looking 00:10:19.52\00:10:20.49 at this done is our bodies were designed so that 00:10:20.50\00:10:24.06 when we are under stress 00:10:24.09\00:10:25.99 the most essential things are preserved 00:10:26.49\00:10:29.47 and the least essential things are cutback first 00:10:29.50\00:10:32.09 and so we find this in women as well as men. 00:10:32.12\00:10:34.61 If the body is under stress, one of the first things 00:10:34.64\00:10:37.21 that suffers is reproductive capacity, 00:10:37.24\00:10:39.50 so just like the young ladies 00:10:39.53\00:10:41.17 that are doing you know aggressive gymnastics, 00:10:41.20\00:10:44.39 their hormonal levels may drop 00:10:44.42\00:10:47.06 and they stop having her menstrual periods, 00:10:47.09\00:10:48.76 men when they are under lots of stress, acute stress, 00:10:48.79\00:10:51.42 whether it's physical illness or something else 00:10:51.45\00:10:53.71 we may see those testosterone levels drop. 00:10:53.74\00:10:55.50 What about chronic illness? 00:10:55.53\00:10:56.95 Chronic illness is also on the list. 00:10:56.98\00:10:58.88 So, this is why you Know 00:10:59.40\00:11:01.31 if we just look at one thing in isolation, 00:11:01.34\00:11:03.05 you just say look at the testosterone level. 00:11:03.08\00:11:04.96 Not at all. You're missing the big picture. 00:11:04.99\00:11:06.82 So, diabetes, lung problems, all those 00:11:06.85\00:11:09.09 different kind of things can cause it to go down. 00:11:09.12\00:11:11.01 Exactly all these things were on the list and treatment 00:11:11.04\00:11:13.65 of the underlying problem is where we should focus. 00:11:13.68\00:11:15.60 Then what about medications, 00:11:15.63\00:11:16.97 other medications make it go down. 00:11:17.00\00:11:18.54 There are medications, 00:11:18.57\00:11:19.80 that could affect testosterone levels, 00:11:19.83\00:11:21.58 and some of those medications 00:11:21.61\00:11:22.68 are commonly used drugs in America. 00:11:22.71\00:11:26.15 Like you have done here for heart disease, 00:11:26.18\00:11:27.79 Digoxin. Exactly, Digoxin, 00:11:27.82\00:11:30.01 which many of our listeners and viewers 00:11:30.04\00:11:33.16 may be aware of for their heart 00:11:33.19\00:11:35.30 for whether it's Atrial Fibrillation, 00:11:35.33\00:11:37.27 whether they're on Lanoxin formulation or something else, 00:11:37.63\00:11:41.57 this is a common drug 00:11:41.84\00:11:43.02 and it can depress testosterone levels. 00:11:43.05\00:11:45.10 So, you also list here 00:11:45.13\00:11:46.10 Narcotics and excessive alcohol. 00:11:46.11\00:11:48.33 Well you know the alcohol connection is really, 00:11:49.00\00:11:52.47 I was gonna say a sobering one, 00:11:52.82\00:11:54.72 but that may not be appreciated by you Don, 00:11:54.75\00:11:59.02 who has a little bit different sense of humor. 00:11:59.05\00:12:01.24 It wasn't supposed to be a joke, 00:12:01.27\00:12:03.29 but I was searching for some other word 00:12:03.32\00:12:04.75 to convey the same thing. 00:12:04.78\00:12:05.75 But alcohol is something that really is a reminder to us 00:12:05.76\00:12:09.96 because we're touting alcohol so much, 00:12:09.99\00:12:11.61 I'm not, but many in the public 00:12:11.64\00:12:13.38 health community are touting 00:12:13.41\00:12:14.92 this is something beneficial and it does interfere 00:12:14.95\00:12:17.27 with testosterone levels 00:12:17.30\00:12:18.27 as well as a number of other things. 00:12:18.28\00:12:19.57 Yes, so I suppose you know people 00:12:19.67\00:12:21.66 who are drinking and different things, 00:12:21.69\00:12:23.15 if they are worried at all about their testosterone, 00:12:23.18\00:12:25.02 they'd stop drinking. You are exactly right, 00:12:25.05\00:12:27.75 blood pressure too, alcohol is something 00:12:28.33\00:12:30.50 that raises blood pressure. 00:12:30.53\00:12:31.52 And since we're on the subject, 00:12:31.55\00:12:32.86 it raises breast cancer risk in women for sure. 00:12:33.79\00:12:36.39 Males in breast cancer connection with the alcohol, 00:12:37.23\00:12:41.09 I have not seen data. 00:12:41.12\00:12:42.13 So, narcotics, pain killers, alcohol, 00:12:42.51\00:12:44.77 but you also list as something 00:12:44.80\00:12:46.21 you can really down the testosterone, 00:12:46.24\00:12:48.47 cigarette smoking. 00:12:48.50\00:12:49.47 You know the list just continues to grow and grow 00:12:50.12\00:12:54.20 about the problems of tobacco smoking. 00:12:54.23\00:12:56.18 Many people are getting the message done, 00:12:56.95\00:12:58.73 but it said that many of our young people 00:12:58.76\00:13:01.31 are still picking up cigarettes and becoming
