" 00:01:02.71\00:01:05.45 compasshealth. net 00:01:05.48\00:01:07.42 Dr. DeRose you're a specialist in internal medicine and 00:01:07.45\00:01:11.78 preventive medicine, you've got a masters in public health 00:01:11.81\00:01:14.84 this is kind of a blending of both the medical and the 00:01:14.87\00:01:17.15 ministry in CompassHealth, isn't that right? 00:01:17.18\00:01:20.59 That's exactly right. 00:01:20.62\00:01:21.59 Basically we do things for churches, community groups, 00:01:21.60\00:01:24.99 we do things for the general public as far as materials that 00:01:25.02\00:01:28.27 we write, we have some videos out, book, a number of things 00:01:28.30\00:01:32.56 that really can put more practical health information 00:01:32.59\00:01:34.86 in people's hands, as well as the media things that we do in 00:01:34.89\00:01:37.69 radio and TV. 00:01:37.72\00:01:38.79 You work together as a family and this is kind of a family 00:01:38.82\00:01:42.22 thing as well. 00:01:42.25\00:01:43.22 Well, some of it is. 00:01:43.23\00:01:44.20 Of course a lot of the technical work I'm doing. 00:01:44.21\00:01:46.85 My wife is also a physician but she's a busy 00:01:46.88\00:01:50.40 homeschooling mother as well, trying to run the house, keep 00:01:50.43\00:01:54.48 me in line as well as three children, doesn't give her 00:01:54.51\00:01:58.72 much time to do the medical end of things in CompassHealth. 00:01:58.75\00:02:02.21 She has a large corpas callosum. 00:02:02.24\00:02:03.90 She has to keep all those things together. 00:02:03.93\00:02:05.67 Ok, lets talk then about fungus today. 00:02:05.70\00:02:09.50 What is a fungus and do a lot of people struggle with a 00:02:09.53\00:02:13.71 problems with fungus? 00:02:13.74\00:02:14.91 Well, there's no question fungus or yeast is actually a form of 00:02:14.94\00:02:21.45 infectious condition. 00:02:21.48\00:02:23.23 These are potentially infectious organisms, if you will. 00:02:23.26\00:02:27.16 They're different than virus or bacteria. 00:02:27.19\00:02:30.08 They can cause human illness and disease. 00:02:30.11\00:02:33.45 The most common things that we see in most general practices 00:02:33.48\00:02:37.94 are skin fungus infections but there are a whole host of fungus 00:02:37.97\00:02:42.01 infections that can effect the lungs and other parts of the 00:02:42.04\00:02:44.31 body and can be very serious and even life threatening. 00:02:44.34\00:02:47.32 Candida is it a fungus? 00:02:47.35\00:02:49.28 Candida, sometimes called Candida other times Canada 00:02:49.31\00:02:53.51 but it's one of them histoplasmosis, coccidiomycosis, 00:02:53.54\00:02:58.40 blastomycosis, mucormycosis - people's eyes are already 00:02:58.43\00:03:04.40 glazing over. - laughter - 00:03:04.43\00:03:06.24 These are all just names for fungus. 00:03:06.27\00:03:07.95 These are different types of fungus. 00:03:07.98\00:03:09.52 Some of those are very bad fungus infections 00:03:09.55\00:03:12.63 that are systemic. 00:03:12.66\00:03:13.71 These other funguses, that we call dermatophytids, they're 00:03:13.74\00:03:17.19 funguses that reside on the skin and cause skin problems. 00:03:17.22\00:03:20.32 So the causes - what causes a fungus? 00:03:20.35\00:03:22.61 I'm sure it depends on where it is, but some general causes. 00:03:22.64\00:03:26.08 Well, the general perspective at least on these skin fungus 00:03:26.11\00:03:29.60 infections is that these fungi are ubiquitous - 00:03:29.63\00:03:35.01 they're all over. 00:03:35.04\00:03:36.01 Different host factors, whether it's genetic things, 00:03:36.02\00:03:40.37 whether it's lifestyle things predispose certain people 00:03:40.40\00:03:44.17 to get skin fungal infections. 00:03:44.20\00:03:46.65 We know certain environmental factors have a role. 00:03:46.68\00:03:50.34 For example if your skin is broken down it's more 00:03:50.37\00:03:54.22 susceptible to a fungal infection. 00:03:54.25\00:03:55.82 Fungi in general do best in warm, moist environments. 00:03:55.85\00:04:01.92 So warm, moist conditions that can cause just a 00:04:01.95\00:04:06.21 proliferation of this? 00:04:06.24\00:04:07.80 It can cause a proliferation. 00:04:07.83\00:04:09.20 It really sets the stage, if you will, for ideal environment 00:04:09.23\00:04:13.22 for these organisms. 00:04:13.25\00:04:14.47 We're seeing this more and more in our country because people 00:04:14.50\00:04:18.61 are providing... they're literally... it's like they're 00:04:18.64\00:04:20.63 building houses for fungi. 00:04:20.66\00:04:22.47 Have you ever thought about this? 00:04:22.50\00:04:23.47 No, and I don't think that's what they set out to do, but 00:04:23.48\00:04:25.95 what do you mean? 00:04:25.98\00:04:26.95 Well, as we gain weight and we get these skin folds, especially 00:04:26.96\00:04:31.04 if we gain lots of weight, what happens is those skin folds 00:04:31.07\00:04:35.53 tend to be moist, warm areas and people will often get what we 00:04:35.56\00:04:41.66 call an intertriginous - between the skin layers - dermatitis - 00:04:41.69\00:04:47.56 an infection with these fungi often times, these dramatify. 00:04:47.59\00:04:53.43 And so they get this fungal rash under their abdominal fat 00:04:53.46\00:04:59.61 or in other fatty areas of the body. 00:04:59.64\00:05:01.63 So this is something really that's becoming a big, big 00:05:01.66\00:05:04.87 problem in our country as we're gaining weight 00:05:04.90\00:05:07.43 and becoming larger. 00:05:07.46\00:05:08.75 What about having a depressed immune system? 00:05:08.78\00:05:12.67 This is important too, Don. 00:05:12.70\00:05:14.71 People that have diabetes, people that immune system 00:05:14.74\00:05:18.08 impairment they are more predisposed to these infections. 00:05:18.11\00:05:22.15 So in other words, if you're a diabetic... 00:05:22.18\00:05:26.06 how does that really work? 00:05:26.09\00:05:28.01 Well, actually blood sugar, we've known this for years, 00:05:28.04\00:05:32.73 is something that actually interferes with normal 00:05:32.76\00:05:35.48 immune system processes. 00:05:35.51\00:05:37.27 It's actually not just in diabetes. 00:05:37.30\00:05:39.56 Many of our viewers have probably heard of studies that 00:05:39.59\00:05:42.62 were done many years ago where people ate simple sugars. 00:05:42.65\00:05:46.01 The classic studies with just plain sugar but they've done 00:05:46.04\00:05:49.68 this with juice, honey, and other things. 00:05:49.71\00:05:52.13 You raise the blood sugar quickly and that actually 00:05:52.16\00:05:55.26 interferes with the ability of the white blood cells to fight 00:05:55.29\00:05:59.23 micro-organisms. 00:05:59.24\00:06:00.40 So whatever's happening with blood sugar is likely part of 00:06:00.43\00:06:05.69 the connection with diabetes and why diabetes has 00:06:05.72\00:06:08.58 immune suppressing effects. 00:06:08.61\00:06:10.43 We also know that those high blood sugars complex, they join 00:06:10.46\00:06:14.39 up with important proteins in the body, and this probably also 00:06:14.42\00:06:17.88 plays a role - especially in uncontrolled diabetes. 00:06:17.91\00:06:21.18 So depression of the immune system comes as a result of 00:06:21.21\00:06:23.72 the elevating sugars, but I've also heard that when someone is 00:06:23.75\00:06:26.95 obese or someone overweight they're in a state of infection, 00:06:26.98\00:06:31.60 is that right or wrong? 00:06:31.63\00:06:32.76 Well, it's an inflammatory condition, if you will. 00:06:32.79\00:06:35.53 What happens is, as we gain weight, especially around the 00:06:35.56\00:06:39.47 middle, this visceral fat, this fat around the organs, 00:06:39.50\00:06:42.79 it actually stimulates some inflammatory hormones in the 00:06:42.82\00:06:47.45 body that do have a number of deleterious processes. 00:06:47.48\00:06:52.08 It seems like this is some of what drives insulin 00:06:52.11\00:06:55.09 resistance some of these metabolic products that are 00:06:55.12\00:06:58.98 actually inflammatory chemicals. 00:06:59.01\00:07:01.65 So diabetes these other things if you have them, they certainly 00:07:01.68\00:07:05.01 can set you up for fungal infections as well. 00:07:05.04\00:07:07.71 What about Cushing's syndrome? 00:07:07.74\00:07:10.95 Well, Cushing's syndrome is another basically hormonal 00:07:10.98\00:07:14.38 condition. 00:07:14.41\00:07:15.38 Often times people with Cushing's will have excess 00:07:15.39\00:07:18.39 weight so they have those skin fold problems, they have 00:07:18.42\00:07:21.75 the disordered blood sugar - we could go through a whole list 00:07:21.78\00:07:25.00 of examples like this - HIV infection, we could talk about 00:07:25.03\00:07:29.63 people who have had transplants, kidney and liver transplants. 00:07:29.66\00:07:33.98 These people are taking anti-rejection drugs that are 00:07:34.01\00:07:37.27 suppressing their immune system. 00:07:37.30\00:07:38.71 So the common denominator that we have here are these 00:07:38.74\00:07:42.28 host factors - whether it's a drug you're taking or whether 00:07:42.31\00:07:45.82 it's a disease - it's increasing the risk. 00:07:45.85\00:07:47.74 So we've got the host factors that increase the risk for 00:07:47.77\00:07:50.44 fungal infections and then we've got the environmental factors. 00:07:50.47\00:07:54.05 One of the things that people don't often think about 00:07:54.08\00:07:56.65 when it comes to the environmental factors is the 00:07:56.68\00:07:58.64 clothes they wear. 00:07:58.67\00:07:59.96 Like clothes that cause moisture and all those bad stuff? 00:07:59.99\00:08:03.55 That's right! 00:08:03.58\00:08:04.55 Nylon for example holds in moisture. 00:08:04.56\00:08:07.50 Cotton is the best type of clothing to wear. 00:08:07.53\00:08:10.63 If you've got athlete's foot - wear cotton socks. 00:08:10.66\00:08:16.49 Wear more breathable shoes and if you can wear sandals 00:08:16.52\00:08:20.57 or if you don't have to wear socks in the summer time 00:08:20.60\00:08:24.54 wear the sandals and go without the socks. 00:08:24.57\00:08:27.59 What you want to do is you want to get a cool, 00:08:27.62\00:08:30.16 cooler at least, dry, and more air circulating. 00:08:30.19\00:08:36.56 These are the things that we want to keep in mind when you're 00:08:36.59\00:08:40.48 trying to suppress these fungal infections or remove 00:08:40.51\00:08:43.28 the possibility for them arising. 00:08:43.31\00:08:45.06 Are there genetic factors that drive fungal infections? 00:08:45.09\00:08:48.77 There seem to be genetic factors. 00:08:48.80\00:08:50.60 I'm going to be honest with you, I'm not an expert 00:08:50.63\00:08:54.22 in the genetics of host factors when it comes to 00:08:54.25\00:08:57.96 fungal infection but there definitely seems to be some 00:08:57.99\00:09:01.80 pre-dispositions host wise as far as the individual whether 00:09:01.83\00:09:06.08 it has to do with skin quality, whether it has to do with skin 00:09:06.11\00:09:10.13 oil production. 00:09:10.16\00:09:11.78 There's a variety of things that theoretically may be 00:09:11.81\00:09:14.45 impacting how well a person can fend off these organisms 00:09:14.48\00:09:20.15 that are all around us. 00:09:20.18\00:09:21.15 You have something written down here - atopy? 00:09:21.16\00:09:25.13 That refers to atopic conditions. 00:09:25.16\00:09:30.29 These are allergic conditions. 00:09:30.32\00:09:32.65 Skin is often affected in this, like young children many times 00:09:32.68\00:09:37.58 have an atopic problem. 00:09:37.61\00:09:40.33 And they may get facial rashes or other things. 00:09:40.36\00:09:42.89 There maybe some food relationships. 00:09:42.92\00:09:44.78 The bottom line - if you've got an allergic condition that has 00:09:44.81\00:09:48.50 skin manifestations, that is lowering resistance to the skin 00:09:48.53\00:09:54.04 to other problems and so these fungi can then get in and cause 00:09:54.07\00:09:58.73 a fungus infection. 00:09:58.76\00:09:59.73 Now if someone has a question normally they would see their 00:09:59.74\00:10:01.81 general physician and maybe a dermatologist 00:10:01.84\00:10:04.18 for some of these things? 00:10:04.21\00:10:05.18 Exactly. 00:10:05.19\00:10:06.16 If you know it's a fungus infection or it's just related 00:10:06.17\00:10:08.15 to the skin, dermatologist... I mean they're the experts 00:10:08.18\00:10:10.93 when it comes to these skin infections. 00:10:10.96\00:10:12.54 Many general practitioners, though, have a lot of experience 00:10:12.57\00:10:15.36 dealing with some of the more simple things that occur 00:10:15.39\00:10:18.21 frequently because they just see it so often. 00:10:18.24\00:10:20.01 So, we're going to look at some practical ways 00:10:20.04\00:10:22.32 to treat these things. 00:10:22.35\00:10:25.14 Are we ready to move there? 00:10:25.17\00:10:26.31 Is there anything else we need to understand? 00:10:26.34\00:10:27.46 No, I think that's where we want to go because we want 00:10:27.49\00:10:30.24 to make the program practical. 00:10:30.27\00:10:31.24 Ok, let's look at some of those practical things. 00:10:31.25\00:10:33.98 Work us through some things we can do. 00:10:34.01\00:10:35.52 Because these fungal infections can effect all different parts 00:10:35.55\00:10:39.22 of the body, one of the common ones is athlete's foot 00:10:39.25\00:10:42.25 we've mentioned it already. 00:10:42.28\00:10:43.25 Let's talk about that first of all. 00:10:43.26\00:10:44.84 Strategies again. 00:10:44.87\00:10:47.26 Remember the environment you want to keep cool and dry. 00:10:47.29\00:10:51.46 We've talked about the clothing. 00:10:51.49\00:10:53.16 We've talked about something that may have passed by people. 00:10:53.19\00:10:58.35 That is, when it comes to moisture content, they may have 00:10:58.38\00:11:01.95 read between the lines, but where is the most common 00:11:01.98\00:11:04.85 place that you're exposed to moisture when it comes 00:11:04.88\00:11:06.58 to your feet? 00:11:06.59\00:11:07.83 Probably in the shower, sometimes a collective shower 00:11:07.86\00:11:10.13 maybe where all the other athlete's have taken a shower. 00:11:10.16\00:11:12.88 Ok, you're right. 00:11:12.91\00:11:13.91 Is that where you pick it up? 00:11:13.94\00:11:14.93 Is it contagious? 00:11:14.96\00:11:16.06 Yes, there is evidence that people who are using communal 00:11:16.09\00:11:19.54 bath facilities, like athletes, are at higher risk 00:11:19.57\00:11:23.76 of these things. 00:11:23.79\00:11:24.76 My mother really wanted me to be a "sissy" when I was a boy, 00:11:24.77\00:11:27.97 you know that? 00:11:28.00\00:11:29.00 No, I didn't know that, but... 00:11:29.03\00:11:30.09 'Cause she didn't succeed, hopefully, is what you 00:11:30.12\00:11:31.89 were thinking! - laughter - 00:11:31.92\00:11:33.22 But what she wanted me to do, was when I was a high school kid 00:11:33.25\00:11:36.51 she wanted me to wear thongs, sandals, in the shower room! 00:11:36.54\00:11:40.81 Because of this athlete's foot? 00:11:40.84\00:11:42.64 Yes! 00:11:42.67\00:11:43.64 I wasn't going to do that! 00:11:43.65\00:11:44.62 And so you got athlete's foot? 00:11:44.63\00:11:46.26 Did I tell you this already? 00:11:46.29\00:11:47.80 No, but I can almost guess! 00:11:47.83\00:11:48.83 I did and then I started wearing the thongs so I wouldn't give 00:11:48.86\00:11:51.49 it to anyone else! 00:11:51.52\00:11:52.49 Whether that helped or not... but you see the point? 00:11:52.50\00:11:55.54 Mom knew best again! 00:11:55.57\00:11:57.53 I should have listened to her. 00:11:57.56\00:11:59.24 But this is an important risk factor. 00:11:59.27\00:12:01.92 But here's the other point. 00:12:01.95\00:12:03.38 We're talking about treatment now. 00:12:03.41\00:12:04.58 You got the athlete's foot already. 00:12:04.61\00:12:06.14 What do you do when you come out of the shower? 00:12:06.17\00:12:08.49 You dry your feet off, you open them up to air, you put sandals 00:12:08.52\00:12:12.96 on maybe... I don't know. 00:12:12.99\00:12:13.96 But how do you dry your feet? 00:12:13.97\00:12:15.07 This is the question. 00:12:15.10\00:12:16.07 With a towel. 00:12:16.08\00:12:17.05 Well, you know what? 00:12:17.06\00:12:18.03 Often people dry the area that's effected by the fungus 00:12:18.07\00:12:22.21 incorrectly. 00:12:22.24\00:12:23.35 If they've got it affecting their feet, under a layer of fat 00:12:23.38\00:12:26.30 in their abdomen, in their private area, and around their 00:12:26.33\00:12:29.21 bottom, these are all common places to have these 00:12:29.25\00:12:32.02 fungal infections, and people say, "I got to keep it dry. " 00:12:32.05\00:12:35.12 So they go ahead and get that terry towel and they dry between 00:12:35.15\00:12:38.57 their toes or they dry in some other areas - that's wrong. 00:12:38.61\00:12:41.99 It's wrong because the skin when it's moist is very 00:12:42.02\00:12:45.73 susceptible to physical agitation or 00:12:45.76\00:12:49.33 physical irritation. 00:12:49.37\00:12:50.56 So what you do is dab it. 00:12:50.60\00:12:53.62 You blot it dry. 00:12:53.66\00:12:54.97 To get it really, really well dry, use a blow dryer 00:12:55.00\00:12:59.35 on a low heat setting. 00:12:59.39\00:13:01.30 Spread you toes apart, use the blow dryer after you've 00:13:01.33\00:13:04.42 blotted it dry, just kind of pat on top of the feet or 00:13:04.45\00:13:07.92 under the areas that you're treating and then get it 00:13:07.96\00:13:11.40 really dry before you put the clothing on. 00:13:11.43\00:13:13.65 If you didn't listen to your mother about the thongs 00:13:13.69\00:13:16.07 it's not going to be too many people listen about the 00:13:16.11\00:13:18.52 blow dryer but - laughter - but that's still good advice. 00:13:18.55\00:13:20.93 Yes, it is good advice. 00:13:20.96\00:13:22.41 I'd say my mom's advice was good. 00:13:22.44\00:13:24.08 We've got to give all the wisdom that's out there 00:13:24.11\00:13:27.56 because you know what? 00:13:27.60\00:13:29.45 Most people don't get motivated to prevent something, 00:13:29.49\00:13:32.96 they get motivated to treat it. 00:13:33.00\00:13:34.09 That was the case for me. 00:13:34.13\00:13:35.27 Aside from drying it off and aside from doing all the things 00:13:35.30\00:13:37.56 you said that are natural things, are there any 00:13:37.59\00:13:39.32 medications that you need for athlete's foot? 00:13:39.36\00:13:41.02 There are medications many people have used, the ointments, 00:13:41.06\00:13:45.15 the lotions, the over-the-counter things. 00:13:45.19\00:13:47.08 And those things are fine. 00:13:47.11\00:13:48.28 They're often effective. 00:13:48.32\00:13:49.42 The problem is most people don't use them long enough. 00:13:49.46\00:13:54.57 Lot's of times it's the itch cycle or something like that, 00:13:54.60\00:13:59.68 that you noted on your webpage. 00:13:59.71\00:14:02.22 Talk to us about the itch cycle. 00:14:02.25\00:14:04.69 There's what we sometimes call the scratch-itch cycle. 00:14:04.72\00:14:08.91 I can tell you, Don, listen, we're on the air, maybe your 00:14:08.95\00:14:13.11 mom told you this, "Don't scratch yourself, 00:14:13.14\00:14:15.68 people are watching you!" 00:14:15.72\00:14:17.45 During the public times or during the waking hours 00:14:17.49\00:14:23.55 we can say even if no one is watching us, we're going to say, 00:14:23.59\00:14:26.62 "Look it, the Doctor told me if I scratch this area it's 00:14:26.66\00:14:29.73 just making it worse because I'm disturbing the skin integrity. " 00:14:29.76\00:14:33.67 What happens at night? 00:14:33.71\00:14:35.75 What happens when you're sleeping and you've got 00:14:35.79\00:14:38.47 an area that itches? 00:14:38.50\00:14:39.47 What do you think you do? 00:14:39.48\00:14:40.45 You scratch it. 00:14:40.46\00:14:41.43 You don't have any higher cortical control while 00:14:41.44\00:14:44.05 you're asleep. 00:14:44.09\00:14:45.06 So it is important many times if the skin is being disturbed 00:14:45.07\00:14:49.66 to break that scratch-itch cycle. 00:14:49.69\00:14:51.36 Many of these anti-fungal preparations, medications, 00:14:51.40\00:14:56.51 will have something for the itch in it as well. 00:14:56.55\00:15:01.15 A classic one will be some Hydrocortisone. 00:15:01.19\00:15:04.02 Now this is ironic because cortisone drugs actually 00:15:04.05\00:15:08.88 suppress the immune system. 00:15:08.92\00:15:10.86 So they are not good over the long haul? 00:15:10.89\00:15:12.67 They are not good over the long haul. 00:15:12.71\00:15:14.42 They also, the more potent ones, can thin the skin. 00:15:14.46\00:15:18.17 Hmmm, not good, because that makes it easier to be 00:15:18.20\00:15:21.88 introducing secondary pathogens perhaps. 00:15:21.92\00:15:25.38 That's right. 00:15:25.41\00:15:26.51 So really, although these are sometimes included and we can 00:15:26.54\00:15:29.63 get away with it sometimes, some people if they have a real 00:15:29.66\00:15:32.99 resistant fungal infection, using these common preparations 00:15:33.02\00:15:36.07 that have the steroids in them will only frustrate the problem. 00:15:36.11\00:15:39.13 We're talking with Dr. David De Rose. 00:15:39.16\00:15:40.84 We're talking about fungal infections and other things 00:15:40.88\00:15:44.08 that can afflict us in America with our skin. 00:15:44.12\00:15:47.55 Join us when we come back. 00:15:47.58\00:15:49.19 Have you found yourself wishing that you could 00:15:50.19\00:15:52.22 shed a few pounds? 00:17:42.24\00:17:44.75 Is there a difference? 00:17:44.78\00:17:45.75 Oh no, very different. 00:17:45.76\00:17:46.73 Eczema is not a fungal infection but eczema is related in that, 00:17:46.74\00:17:51.31 like the scenario we were talking about earlier, any type 00:17:51.34\00:17:54.71 of skin problem can lower the resistance of that skin 00:17:54.74\00:18:01.29 to fungal infection. 00:18:01.32\00:18:03.36 Eczema actually has some seasonal variations 00:18:03.39\00:18:07.82 in people when the air is colder and dryer, containing less 00:18:07.85\00:18:12.49 moisture. 00:18:12.52\00:18:13.49 People that are predisposed to eczema will often have 00:18:13.50\00:18:14.81 more problems. 00:18:14.84\00:18:15.83 Doctors and nurses we're at high risk, washing our hands 00:18:15.86\00:18:20.19 so frequently, often get a hand eczema. 00:18:20.22\00:18:24.05 So the idea is to moisturize as well as washing the skin. 00:18:24.08\00:18:29.26 Use a good moisturizer as well after you wash. 00:18:29.29\00:18:32.99 The reason it came to my mind is because one of the treatments 00:18:33.02\00:18:35.73 you were suggesting for a fungal infection is something 00:18:35.76\00:18:38.19 that sometimes they use with eczema that being a 00:18:38.22\00:18:41.72 cortisone based cream. 00:18:41.75\00:18:43.89 Exactly. 00:18:43.92\00:18:44.89 The cortisone does not actually treat the fungus. 00:18:44.90\00:18:46.47 So just using a cortisone cream on a fungal infection will 00:18:46.50\00:18:50.66 actually tend to make it worse. 00:18:50.69\00:18:52.02 What they often try to get away with doing... 00:18:52.05\00:18:56.24 I'm more of a purist. 00:18:56.27\00:18:59.59 I sensed that. - laughter- 00:18:59.62\00:19:01.11 You did? - laughter- 00:19:01.14\00:19:02.11 I like to treat the problem. 00:19:02.12\00:19:03.91 So I tend not to use the cortisone containing 00:19:03.95\00:19:06.81 preparations initially. 00:19:06.84\00:19:07.81 But many of my colleagues will say, "Look it De Rose get off 00:19:07.84\00:19:11.44 the purist kick and just be realistic, you can give a little 00:19:11.47\00:19:16.60 cortisone mixed in with these things. " 00:19:16.63\00:19:18.52 There are many commercial preparations and like I said 00:19:18.55\00:19:21.21 many people can get away with using these but others don't. 00:19:21.24\00:19:24.90 They've got to drop the cortisone out of the preparation 00:19:24.93\00:19:27.73 then use something else for the itching. 00:19:27.76\00:19:29.25 Whether it's an antihistamine orally or something topical 00:19:29.29\00:19:33.22 I'll tell you an interesting thing. 00:19:33.26\00:19:35.05 A lot of people don't know this but Calamine lotion, 00:19:35.09\00:19:38.90 you know a lot of people think of using it for poison ivy, 00:19:38.93\00:19:42.39 well it's a drying lotion. 00:19:42.42\00:19:44.01 So you could use it for a fungal problem. 00:19:44.04\00:19:46.64 You could actually use it... it's not a treatment for a 00:19:46.67\00:19:50.30 fungus but if you've been treating... if you've got like a 00:19:50.33\00:19:53.13 chronic fungal infection, chronic athlete's foot problem, 00:19:53.16\00:19:56.02 you don't want to take one of the oral anti-fungal medications 00:19:56.05\00:19:59.61 because you're afraid of the side effects. 00:19:59.64\00:20:01.00 And there are some reasons to be afraid. 00:20:01.03\00:20:02.81 Some of these are very potent drugs that cause 00:20:02.84\00:20:05.13 liver irritation and other things. 00:20:05.16\00:20:06.97 You say, "Look it, I just want to keep the area very clean, 00:20:07.00\00:20:10.88 dry, cool. " 00:20:10.91\00:20:12.22 The Calamine lotion or a Caladryl that includes Benadryl 00:20:12.25\00:20:16.58 with an anti-itching compound may actually help. 00:20:16.61\00:20:18.90 If you can break the cycle sometimes with that itching, 00:20:18.93\00:20:22.66 sometimes that in and of itself will allow it to heal and that 00:20:22.69\00:20:25.86 may be what was perpetuating it. 00:20:25.89\00:20:27.47 So, just like in athlete's foot you said to dry it by blotting 00:20:27.50\00:20:32.55 or dabbing rather than irritating it because that's 00:20:32.58\00:20:35.78 almost like a itch cycle. 00:20:35.81\00:20:37.21 Exactly. 00:20:37.24\00:20:38.21 So, if you have things in other places, your private areas, 00:20:38.22\00:20:39.87 you'd do the same kind of thing - keep it dry. 00:20:39.90\00:20:42.75 Well, the private areas are really difficult areas to 00:20:42.78\00:20:49.84 handle sometimes. 00:20:49.87\00:20:50.92 There are several reasons for that. 00:20:50.95\00:20:52.00 One of them is there is the micro biologic environment 00:20:52.03\00:20:58.04 there is different. 00:20:58.07\00:20:59.04 Especially if you think of where the colon empties out. 00:20:59.05\00:21:02.70 We're continually exposing that area to some of the 00:21:02.73\00:21:06.82 bi products of our digestive system. 00:21:06.85\00:21:09.45 Some of the strategies that seem to make a difference 00:21:09.48\00:21:13.45 in people that have problems in those areas is actually 00:21:13.48\00:21:17.54 looking very closely at their diet, avoiding lots of 00:21:17.57\00:21:19.95 simple sugars. 00:21:19.99\00:21:21.17 The theory behind that is we're going to provide less food for 00:21:21.20\00:21:24.34 the yeast that thrive on those simple sugars 00:21:24.38\00:21:28.00 in that environment. 00:21:28.03\00:21:29.05 Another strategy that is sometimes used is probiotics, 00:21:29.08\00:21:32.58 these so-called "good bacteria" like lactobacillus. 00:21:32.61\00:21:36.07 These can be taken orally. 00:21:36.11\00:21:37.59 And there are some suggestions that this may help to address 00:21:37.63\00:21:40.87 some of the problems that you may have in the rectal area 00:21:40.90\00:21:44.10 or other areas. 00:21:44.14\00:21:45.33 So we're talking about two different things - things that 00:21:45.37\00:21:48.84 stop the itching cycle and all that, but then the actual 00:21:48.87\00:21:52.31 treatment of the fungus - what should people take 00:21:52.34\00:21:54.34 for that, it's not just sometimes go away on it's own? 00:21:54.38\00:21:57.42 No, actually many times you do need something. 00:21:57.45\00:21:59.77 There are some natural strategies that work. 00:21:59.81\00:22:02.29 It depends where the fungal infection is 00:22:02.32\00:22:04.77 whether or not you use these. 00:22:04.80\00:22:06.82 Garlic has anti-fungal properties for example. 00:22:06.86\00:22:11.00 Some people have had success using garlic powder. 00:22:11.03\00:22:15.11 It's not something that I've actually used but the natural 00:22:15.14\00:22:18.37 things that I tend to use first are tea tree oil. 00:22:18.41\00:22:23.30 This you have on the webpage? 00:22:23.34\00:22:25.75 That's right, it's on the webpage. 00:22:25.79\00:22:27.48 Tea tree oil has natural anti-fungal properties. 00:22:27.51\00:22:31.82 You have to be careful with it though because there's a 00:22:31.85\00:22:34.78 significant percentage of people that do get allergic to the 00:22:34.81\00:22:38.85 tea tree oil. 00:22:38.88\00:22:39.85 So you need to be mindful of that. 00:22:39.86\00:22:41.03 If that doesn't work, what is your second line? 00:22:41.07\00:22:42.89 Well, not necessarily the second line, but another one that's a 00:22:42.92\00:22:46.95 actually can be effective is vinegar. 00:22:46.99\00:22:49.38 Vinegar actually has anti-fungal properties. 00:22:49.42\00:22:52.71 Garlic powder? 00:22:52.75\00:22:54.08 Garlic powder and calendula is another herb. 00:22:54.11\00:22:57.09 Calendula, what's that? 00:22:57.13\00:22:58.18 Calendula is an herb that has some anti-fungal properties. 00:22:58.21\00:23:01.95 There's a number of these herbs and natural products 00:23:01.99\00:23:04.84 that can be used for people that gravitate in that direction. 00:23:04.87\00:23:08.02 And then there are some of the over-the-counter, you don't need 00:23:08.05\00:23:11.16 a prescription, anti-fungals: Lotrimines, Monistat, 00:23:11.20\00:23:15.20 Mycostatin, things like this that can be used on 00:23:15.24\00:23:19.21 fungal areas. 00:23:19.24\00:23:20.21 Usually they work quite good - Tinactin powders 00:23:20.22\00:23:22.78 and things like this. 00:23:22.81\00:23:23.97 Many times they do work well. 00:23:24.01\00:23:26.29 And then there's probably a whole class of drugs or 00:23:26.33\00:23:29.44 regiments that you can only get from your 00:23:29.47\00:23:31.05 doctor, dermatologist or your general practice person? 00:23:31.08\00:23:34.46 Yes, especially if you've got a resistant fungal infection 00:23:34.50\00:23:37.84 and it's going to require something oral. 00:23:37.88\00:23:39.62 Some of these prescription drugs can be very expensive, they need 00:23:39.66\00:23:44.93 to be monitored, and some of them do have 00:23:44.96\00:23:46.84 significant toxicity. 00:23:46.87\00:23:48.52 Do you make a difference on the webpage between topical 00:23:48.56\00:23:51.58 anti-fungal preparations and systemic drugs? 00:23:51.61\00:23:54.18 I appreciate you highlighting that because sometimes 00:23:54.22\00:23:58.12 the terminology we use in professional circles is not 00:23:58.16\00:24:02.03 familiar to lay people. 00:24:02.06\00:24:03.34 Topical is what you put on the skin. 00:24:03.37\00:24:06.41 Generally these topical things are readily available, 00:24:06.44\00:24:10.06 many options at least, over the counter. 00:24:10.09\00:24:12.15 The thing is, Don, while we're talking about the topical, 00:24:12.18\00:24:14.52 I'll mention the systemic again. 00:24:14.55\00:24:16.76 That's the oral things you take that go through out the body. 00:24:16.79\00:24:19.77 People need to make sure that they treat these things. 00:24:19.80\00:24:23.75 Usually what we'll say is a minimum sometimes of two weeks. 00:24:23.78\00:24:27.58 And make sure you continue the treatment for a week after 00:24:27.61\00:24:31.27 the infection is cleared. 00:24:31.30\00:24:32.60 That's one rule of thumb for going about this. 00:24:32.63\00:24:34.86 So it's like an antibiotic? 00:24:34.89\00:24:35.89 Yes, a lot of people will treat it, they'll knock it out, and 00:24:35.92\00:24:39.83 what they've really done is they've allowed, at least 00:24:39.86\00:24:42.87 theoretically, that fungus or yeast to become resistant 00:24:42.90\00:24:48.04 to whatever because they've knocked it down enough 00:24:48.07\00:24:50.12 but they didn't wipe it out. 00:24:50.15\00:24:51.46 So make sure you use it for a week after you've eradicated 00:24:51.49\00:24:56.07 it - at least visibly eradicated. 00:24:56.10\00:24:58.55 You have some specifics here in applying vinegar - kind of a 00:24:58.58\00:25:03.51 formula: 1/2 calendula, 1/2 oz. myrrh, 1/2 qt. vinegar 00:25:03.54\00:25:10.74 you have these on your webpage for actual treatments if you're 00:25:10.77\00:25:15.46 going that natural way. 00:25:15.49\00:25:16.48 That's right. 00:25:16.51\00:25:17.48 Myrrh is another agent that's been used in these preparations 00:25:17.49\00:25:20.54 like you point out. 00:25:20.57\00:25:21.54 So there are recipes that have been used in more 00:25:21.55\00:25:24.25 natural combinations and that's one of them. 00:25:24.28\00:25:26.40 Fungus under the nails? 00:25:26.43\00:25:27.66 Fungus under the nails or nail fungus it's really a fungus 00:25:27.69\00:25:31.14 that actually infects the nail itself. 00:25:31.17\00:25:33.21 Those can be very resistant especially the toe nail. 00:25:33.24\00:25:36.66 There is research data done in the medical research literature 00:25:36.69\00:25:41.83 showing that tea tree oil in a percentage of people is 00:25:41.86\00:25:45.47 an effective treatment. 00:25:45.50\00:25:46.66 Now when I say "effective treatment" we're not talking 00:25:46.69\00:25:49.45 50, 60, 70 percent. 00:25:49.48\00:25:51.75 It's a minority of people but if someone wants to go with a 00:25:51.78\00:25:55.39 natural treatment, you're talking a 3- 6 months 00:25:55.42\00:25:57.80 whatever you do. 00:25:57.83\00:25:58.80 If you're doing something topical on the toenails 00:25:58.81\00:26:00.89 they're slow to grow and you need to be committed to the 00:26:00.92\00:26:04.70 long haul for applying this treatment topically. 00:26:04.73\00:26:08.54 Garlic?
