Hello and welcome to "Health for a Lifetime" 00:00:51.57\00:00:53.58 I'm your host Don Mackintosh 00:00:53.61\00:00:55.21 Today we're going to be talking about women's health issues, 00:00:55.24\00:00:57.56 and we're going to be talking about a subject 00:00:57.59\00:00:59.78 that we've entitled "Hormone Replacement Therapy" 00:00:59.81\00:01:03.74 Sounds like a BIG long thing, 00:01:03.77\00:01:05.44 but out guest today is going to be able to help us understand 00:01:05.47\00:01:08.31 more about this and how it relates to women's health issues 00:01:08.34\00:01:12.12 Dr. Shadle is with us today, Eric Shadle, 00:01:12.15\00:01:14.38 and you practice in a type of medicine that deals with women 00:01:14.41\00:01:21.09 day in and day out; obstetrics, I guess the way 00:01:21.12\00:01:24.75 we say this and gynecology, is that right? That's right 00:01:24.78\00:01:27.00 So we're dealing with women all day, every day, 00:01:27.03\00:01:30.46 and one of the things that we deal with as women, 00:01:30.49\00:01:34.92 or women deal with is hormones as they change 00:01:34.95\00:01:38.18 over the life cycle and I suppose that this is related to 00:01:38.21\00:01:41.48 what we're going to be talking about. 00:01:41.51\00:01:42.54 What exactly is hormone replacement therapy? 00:01:42.57\00:01:45.54 Well hormone replacement therapy refers to taking estrogen 00:01:45.57\00:01:50.95 and progesterone. 00:01:50.98\00:01:52.39 We call it replacement therapy because it's typically given 00:01:52.42\00:01:56.79 to a menopausal women after she has gone through the menopause 00:01:56.82\00:02:01.72 and isn't making estrogen and progesterone, 00:02:01.75\00:02:04.94 and so we talk about replacing both of those hormones. 00:02:04.97\00:02:10.22 As opposed to ERT or estrogen replacement therapy, 00:02:10.25\00:02:13.84 where we only give the estrogen. 00:02:13.87\00:02:15.67 So progesterone AND estrogen is what we're going to be 00:02:15.70\00:02:18.21 talking about primarily today. 00:02:18.24\00:02:19.36 Primarily, yes, both hormone estrogen and progesterone. 00:02:19.39\00:02:23.69 Are there any risks involved with taking hormones, 00:02:23.72\00:02:26.44 or replacing them? 00:02:26.47\00:02:28.82 Actually, there is... there are several risks 00:02:28.85\00:02:30.92 associated with a HRT or hormone replacement therapy. 00:02:30.95\00:02:35.11 The one that concerns women more than any other is 00:02:35.14\00:02:39.99 breast cancer but there is also thrombosis, 00:02:40.02\00:02:43.15 DVT which is deep venous thrombosis. 00:02:43.18\00:02:45.60 Which is like a blood clot... That's right 00:02:45.63\00:02:48.20 ...Gallbladder disease and hypertension 00:02:48.23\00:02:51.19 or high blood pressure. 00:02:51.22\00:02:52.81 Those are all risks associated with hormone replacement therapy 00:02:52.84\00:02:56.81 And what is the rationale then, to prescribe a medication 00:02:56.84\00:03:01.84 or a hormone like that if there are those 00:03:01.87\00:03:03.76 kind of risks involved? 00:03:03.79\00:03:04.89 Well that's an interesting question and one that 00:03:04.92\00:03:09.87 my colleagues in obstetrics and gynecology will debate 00:03:09.90\00:03:16.20 at about every seminar that I go to on 00:03:16.23\00:03:18.87 hormone replacement therapy... 00:03:18.90\00:03:20.26 But there is specific rationale for prescribing 00:03:20.29\00:03:25.58 hormone replacement therapy and the main one 00:03:25.61\00:03:27.82 is osteoporosis. 00:03:27.85\00:03:29.16 1.3 million women that have osteoporotic fractures each year 00:03:29.19\00:03:36.45 Half of those are vertebral or in the backbone; 00:03:36.48\00:03:40.99 25% of them are in the hip and another 25% are Colles fractures 00:03:41.02\00:03:46.07 which is that fracture in the wrist. 00:03:46.10\00:03:49.03 When you fall down and catch yourself. That's right 00:03:49.06\00:03:50.88 In 1995, the cost of these osteoporotic fractures 00:03:50.91\00:03:55.80 were 13.8 billion dollars, so this is a huge public health 00:03:55.83\00:04:00.70 issue in this country. 00:04:00.73\00:04:02.25 And that is the main rationale for giving women estrogen 00:04:02.28\00:04:07.80 because estrogen has been shown in many retrospective studies 00:04:07.83\00:04:12.05 to reduce the risk for osteoporosis. 00:04:12.08\00:04:15.89 I've also heard that it does something with 00:04:15.92\00:04:17.72 cardiovascular disease or reduces the risk some people say 00:04:17.75\00:04:21.92 There are many retrospective studies, again, that show that 00:04:21.95\00:04:26.00 we reduce the risk of cardiovascular disease 00:04:26.03\00:04:29.42 by taking estrogen but of interest is that there are 00:04:29.45\00:04:32.40 several studies that are ongoing very good prospective studies 00:04:32.43\00:04:38.31 that indicate that actually estrogen or 00:04:38.34\00:04:42.08 hormone replacement therapy not only does NOT 00:04:42.11\00:04:45.74 decrease the risk of cardiovascular disease, 00:04:45.77\00:04:48.16 but may actually INCREASE the risk of heart attack, stroke 00:04:48.19\00:04:52.20 those types of issues... 00:04:52.23\00:04:54.03 So it's not really a good reason to take it. 00:04:54.06\00:04:55.57 It's not a reason to take it at all. 00:04:55.60\00:04:56.99 So you're telling me the BEST reason to take either of 00:04:57.02\00:05:00.11 these would be osteoporosis, 00:05:00.14\00:05:02.22 progesterone or estrogen replacement therapy. 00:05:02.25\00:05:05.34 Yes, even in the face of the side effects or risk factors 00:05:05.38\00:05:09.67 that we listed earlier which is the breast cancer, 00:05:09.70\00:05:13.39 and the thrombosis and those things, 00:05:13.42\00:05:17.04 the traditional view in the medical establishment 00:05:17.07\00:05:21.02 is to accept those risks in order to prevent the 00:05:21.05\00:05:26.28 osteoporosis because so many women suffer from osteoporosis. 00:05:26.31\00:05:31.70 I want to go on and talk a little bit more about this 00:05:31.73\00:05:34.66 osteoporosis and about how we can maybe avoid it 00:05:34.69\00:05:38.64 with other things other than estrogen and progesterone, 00:05:38.67\00:05:41.13 like diet but I wanted to ask why is it that the increase 00:05:41.16\00:05:44.78 comes for cancer risk when you 00:05:44.81\00:05:47.01 take an estrogen or a progesterone? 00:05:47.04\00:05:49.60 Specifically for breast cancer, there are estrogen receptors 00:05:49.63\00:05:54.90 in the breast as well as progesterone receptors, 00:05:54.93\00:05:57.76 and there is no question that if we give estrogen 00:05:57.79\00:06:01.57 to a woman who has breast cancer, 00:06:01.60\00:06:03.65 especially if that cancer has the estrogen receptors 00:06:03.68\00:06:07.09 then the cancer will grow... 00:06:07.12\00:06:09.54 But we also know, through multiple studies, 00:06:09.57\00:06:14.92 that estrogen actually does cause an increased risk 00:06:14.95\00:06:18.53 in the breast cancer risk and that's probably because of 00:06:18.56\00:06:22.88 these receptors. 00:06:22.91\00:06:24.60 So birth control pills - that's why that happens as well? 00:06:24.63\00:06:28.62 Birth control pills do increase the risk of breast cancer 00:06:28.65\00:06:32.51 slightly in a certain age group and that age group 00:06:32.54\00:06:37.13 happens to be the women in their 30s who are taking it... 00:06:37.16\00:06:40.24 But women who take it in their teens or in their 20s, 00:06:40.27\00:06:45.94 when we look at the risk of breast cancer when they're 00:06:45.97\00:06:48.96 in their 40s and 50s, we don't see an increased risk 00:06:48.99\00:06:52.31 unless PERHAPS, they continue to take it into their 30s, 00:06:52.34\00:06:56.21 and into their 40s, then there may be an 00:06:56.24\00:07:00.70 increased risk that we need to talk about... 00:07:00.73\00:07:02.89 But it's a little different than hormone replacement therapy 00:07:02.92\00:07:07.61 because the birth control pill is made of estrogens that are 00:07:07.64\00:07:13.52 totally synthetic and are high doses that turn the ovary 00:07:13.55\00:07:17.11 and everything else off... 00:07:17.14\00:07:18.82 And so they may not combine with the receptors in the breast 00:07:18.85\00:07:22.86 like hormone replacement therapy. 00:07:22.89\00:07:24.83 Okay, so it's a little different issue but... 00:07:24.86\00:07:26.50 So it's a little different issue and hormone replacement therapy 00:07:26.53\00:07:30.71 is actually more associated with breast cancer than 00:07:30.74\00:07:33.55 the birth control pill. 00:07:33.58\00:07:35.10 Let's look then back at this idea of 00:07:35.13\00:07:37.63 estrogens and progesterone in terms of osteoporosis... 00:07:37.66\00:07:40.60 You say they really do help to avoid that, 00:07:40.63\00:07:42.55 but are there other things we can do to avoid osteoporosis? 00:07:42.58\00:07:46.14 Absolutely - I think the most basic thing that we can do is 00:07:46.17\00:07:50.64 have a proper diet. 00:07:50.67\00:07:52.13 And when I say that, most Americans, 00:07:52.16\00:07:55.07 at least most people in this country, 00:07:55.10\00:07:57.28 and I know this program goes around the world, 00:07:57.31\00:07:59.59 and I don't know exactly what they're saying in Bangladesh 00:07:59.62\00:08:04.02 or in China or other places, but we find that diet 00:08:04.05\00:08:09.42 in this country is definitely associated with osteoporosis, 00:08:09.45\00:08:14.70 so that a high protein diet will 00:08:14.73\00:08:17.56 cause a loss of calcium in the bones... 00:08:17.59\00:08:19.76 And an animal protein actually causes a greater loss 00:08:19.79\00:08:23.75 of calcium than does plant protein. Interesting! 00:08:23.78\00:08:26.17 And then the plant sources of calcium, 00:08:26.20\00:08:29.40 as the graphic shows here, are absorbed more efficiently 00:08:29.43\00:08:33.13 than are animal sources to the tune of about 00:08:33.16\00:08:36.30 2/3 of the calcium that is in a green, leafy vegetable, 00:08:36.33\00:08:39.49 for example, will be absorbed as opposed to milk 00:08:39.52\00:08:43.53 where only about 1/3 of that calcium in the milk is absorbed. 00:08:43.56\00:08:48.84 So these high protein things are 00:08:48.87\00:08:52.24 really what is causing the problem. 00:08:52.27\00:08:53.39 We look at our diet, we can really be addressing a lot of 00:08:53.42\00:08:56.25 the issues with osteoporosis other than estrogen therapy. 00:08:56.28\00:08:59.81 Yes and that would be an alternative to hormone therapy - 00:08:59.84\00:09:02.25 is to simply change our diet to one that is compatible 00:09:02.28\00:09:07.84 with good bone function and the bone growth that we want to have 00:09:07.87\00:09:15.92 We hear a lot about - if you are going to be taking or eating 00:09:15.95\00:09:20.88 foods that are high in calcium or taking calcium supplements, 00:09:20.91\00:09:23.62 or these kind of things, that we need to be having 00:09:23.65\00:09:26.02 weightbearing exercise as well. That's right 00:09:26.05\00:09:28.63 Why is that and how does that work? 00:09:28.66\00:09:30.76 Well the bones are constantly resorbing - 00:09:30.79\00:09:36.38 that is taking calcium, taking bone out and depositing bone. 00:09:36.41\00:09:42.24 Actually, there are 2 different types of cells... 00:09:42.27\00:09:44.91 Osteo - that's bone Clast - which takes bone out 00:09:44.94\00:09:49.20 Osteoblast - which actually puts bone down... 00:09:49.23\00:09:52.12 And so there's this constant mottling of bone 00:09:52.15\00:09:55.16 between these 2 different cells, 00:09:55.19\00:09:56.87 and exercise, weightbearing exercise actually 00:09:56.90\00:09:59.70 stimulates those osteoblasts to lay down more bone tissue... 00:09:59.73\00:10:04.63 And so we can prevent osteoporosis by exercising 00:10:04.66\00:10:09.83 at least 20 minutes every day and it does have to be 00:10:09.86\00:10:14.45 vigorous exercise, it can be continuous walking, 00:10:14.48\00:10:18.54 especially if we can get our heart rate up 00:10:18.57\00:10:22.11 even to say 60, just 60% of target heart rate. 00:10:22.14\00:10:28.89 That will definitely be benefiting our bones 00:10:28.92\00:10:32.25 because we are putting the pressure 00:10:32.28\00:10:33.79 on the bones that we need to be. 00:10:33.82\00:10:35.47 Now, I remember while I was on a trip to Romania, 00:10:35.52\00:10:37.60 a mission trip, and I was supposed to take 00:10:37.63\00:10:39.08 a bunch of calcium to help the people there that were 00:10:39.11\00:10:42.77 supposedly having problems with their bones... 00:10:42.80\00:10:44.59 When I got there, the doctor said, 00:10:44.62\00:10:46.08 "That's not going to help them at all because it didn't have 00:10:46.11\00:10:47.97 any phosphorous in it" Is there any truth to that? 00:10:48.00\00:10:51.01 Yes there is! We do need phosphorous 00:10:51.04\00:10:53.82 in order to absorb our calcium, 00:10:53.85\00:10:58.38 and, in fact, that's probably one of the reasons... 00:10:58.41\00:11:02.42 we need phosphorous, but there is something else 00:11:02.45\00:11:05.76 in our diet called "phosphates" that actually leach calcium out. 00:11:05.79\00:11:11.07 So we need to be careful... we need phosphorous, 00:11:11.10\00:11:13.17 but we don't need phosphates which are in cola drinks, 00:11:13.20\00:11:16.55 and which are in meat. 00:11:16.58\00:11:20.08 Animal protein has a lot of phosphate and that 00:11:20.11\00:11:23.44 typically acts as a diuretic to get the 00:11:23.47\00:11:26.75 calcium out of our bones. Gets rid of it. 00:11:26.78\00:11:29.03 I've heard of Fosamax, a medication that's supposed 00:11:29.07\00:11:33.01 to increase, I guess, bone density or something like that. 00:11:33.04\00:11:36.46 Is there anything to that? Is that helpful? 00:11:36.49\00:11:38.71 Yes, well getting back to the idea of alternatives 00:11:38.74\00:11:42.26 to hormone replacement, Fosamax would be one of those. 00:11:42.29\00:11:45.90 Actually, it's a bisphosphonate, I think we have a graphic here. 00:11:45.93\00:11:51.24 Bisphosphonates is a class of medications that Fosamax 00:11:51.27\00:11:54.73 is actually an example of and it's actually not a hormone. 00:11:54.76\00:12:00.22 And, it increases bone density. 00:12:00.25\00:12:05.86 There are very good studies that show that it actually 00:12:05.89\00:12:08.22 will increase bone density rather than losing bone 00:12:08.25\00:12:11.08 that we typically see in older folks. 00:12:11.11\00:12:15.13 It can be used to either prevent or it can also be used to 00:12:15.16\00:12:19.99 actually treat osteoporosis. 00:12:20.02\00:12:21.80 So there are some other options other than just taking 00:12:21.83\00:12:24.03 estrogens or the progesterones. 00:12:24.06\00:12:26.20 that can help with the osteoporosis. 00:12:26.23\00:12:28.89 What about so-called, "designer estrogens" 00:12:28.92\00:12:31.12 What are they? 00:12:31.15\00:12:32.55 In other words, they don't supposedly, I guess 00:12:32.58\00:12:34.62 maybe impact one area of the body but the other... 00:12:34.65\00:12:37.70 Tell us a little bit about that. 00:12:37.73\00:12:39.17 Designer estrogens, one example would be - Evista or raloxifene. 00:12:39.20\00:12:45.54 Raloxifene is actually the chemical name for it, 00:12:45.57\00:12:49.43 and what they do is actually increase bone density, 00:12:49.46\00:12:54.85 but they don't increase the risk for breast cancer... 00:12:54.88\00:12:57.75 and the reason that they don't increase the 00:12:57.78\00:13:00.13 risk for breast cancer, like estrogens do, 00:13:00.16\00:13:03.37 is because they act as an anti-estrogen on some receptors; 00:13:03.40\00:13:09.64 in other words, they will bind to an estrogen receptor 00:13:09.67\00:13:12.48 but they won't stimulate it... 00:13:12.51\00:13:15.04 So that's like an anti-estrogen and on other receptors, 00:13:15.07\00:13:19.26 they do act like an estrogen. 00:13:19.29\00:13:22.08 So, for example, if we take raloxifene or Evista, 00:13:22.11\00:13:26.80 and we give it to a patient, it will bind with the 00:13:26.83\00:13:30.81 estrogen receptors in the bone and, thus, increase bone density 00:13:30.84\00:13:35.38 without increasing the risk of breast cancer... 00:13:35.41\00:13:39.85 But I should hasten to say that there is no medication 00:13:39.88\00:13:45.73 that is on the market that has no side effects, 00:13:45.76\00:13:50.91 and though the drug companies would make us think or 00:13:50.94\00:13:54.30 like us to think that we can take these medications 00:13:54.33\00:13:58.03 and never worry about anything, 00:13:58.06\00:13:59.70 that they are the answer to all our problems; 00:13:59.73\00:14:02.16 we know that there is always some problem somewhere... 00:14:02.19\00:14:06.97 And one concern I would have with raloxifene or Evista 00:14:07.00\00:14:12.01 is that one of its cousins is Tamoxifen. 00:14:12.04\00:14:16.40 Tamoxifen is a treatment for breast cancer, 00:14:16.43\00:14:19.44 and so it does help to prevent breast cancer, 00:14:19.47\00:14:23.81 but we found that Tamoxifen, given for more than 5 years, 00:14:23.84\00:14:27.86 may not prevent breast cancer anymore, 00:14:27.89\00:14:30.52 and it may actually increase at that point. 00:14:30.55\00:14:33.38 So it works for a time but then it maybe potentiates it. 00:14:33.41\00:14:35.96 Yes, so do we want to be taking Evista for 10, 15 or 20 years? 00:14:35.99\00:14:41.53 I think the jury is out on that but it's certainly 00:14:41.56\00:14:44.16 an alternative to hormone therapy that may be promising. 00:14:44.19\00:14:48.28 So we really need to think closely about 00:14:48.31\00:14:50.25 hormone replacement therapy before we get involved, 00:14:50.28\00:14:53.37 and try and do the basic underlying things 00:14:53.40\00:14:55.58 like diet and exercise, weightbearing and all that stuff 00:14:55.61\00:14:59.06 What about the so-called natural progesterone? 00:14:59.09\00:15:01.68 Some people say it's natural so it must mean it's safe... is it? 00:15:01.71\00:15:05.74 Well, if it's natural, it's got to be, right? 00:15:05.77\00:15:09.94 Sound's good! It sounds good... 00:15:09.97\00:15:12.15 Well natural progesterone actually is not associated 00:15:12.18\00:15:16.64 with breast cancer - that's the good thing about it. 00:15:16.67\00:15:19.71 It's also not associated with heart disease. 00:15:19.74\00:15:22.18 And, what I mean by that is that if we look at 00:15:22.21\00:15:25.85 synthetic progesterones, 00:15:25.88\00:15:29.07 it increases both of them probably. 00:15:29.10\00:15:32.54 But this one doesn't. 00:15:32.57\00:15:34.59 There is no convincing data, however, to confirm 00:15:34.62\00:15:37.53 that it will increase bone density. 00:15:37.56\00:15:40.00 So I can't say that it will prevent osteoporosis, 00:15:40.03\00:15:44.21 though some claim it will, especially those that are more 00:15:44.24\00:15:49.06 familiar with natural remedies, 00:15:49.09\00:15:50.93 but there have been a lot of studies that have been done 00:15:50.96\00:15:55.03 that have not shown it to increase the bone density... 00:15:55.06\00:15:59.50 So in other words, why would you take it - if it doesn't help one 00:15:59.53\00:16:01.88 and it doesn't help the other? 00:16:01.91\00:16:03.15 Well what natural progesterone will do is, as an alternative 00:16:03.18\00:16:07.93 to hormone replacement therapy... 00:16:07.96\00:16:09.70 We've been talking mainly about osteoporosis, 00:16:09.73\00:16:12.11 and I don't think it's going to do much for that, 00:16:12.14\00:16:14.15 but what it can do is, it can help a lot of other 00:16:14.18\00:16:17.68 menopausal symptoms - some that we haven't 00:16:17.71\00:16:20.13 really talked about such as hot flashes. 00:16:20.16\00:16:23.51 And natural progesterone, I think if a woman is 00:16:23.54\00:16:28.65 to take progesterone, she really wants to take 00:16:28.68\00:16:31.11 natural progesterone because synthetic progesterone has been 00:16:31.14\00:16:35.48 shown to actually increase the risk of breast cancer. 00:16:35.51\00:16:39.33 You know, it wasn't too long ago that we thought estrogen 00:16:39.36\00:16:42.65 was the only culprit in increasing breast cancer. 00:16:42.68\00:16:45.38 And, a study came out not too long ago within the last year 00:16:45.41\00:16:50.05 that showed that if a woman took estrogen by itself, 00:16:50.08\00:16:54.94 her increased risk of breast cancer 00:16:54.97\00:16:58.02 was as much as 33% increased risk, 00:16:58.05\00:17:01.73 but if she took estrogen with a synthetic progesterone, 00:17:01.76\00:17:05.11 her risk was 66% increased for breast cancer... 00:17:05.14\00:17:09.27 And so we find that synthetic progesterone is something 00:17:09.30\00:17:14.02 we want to avoid but natural progesterone, 00:17:14.05\00:17:17.32 it doesn't appear that it has the same effects. 00:17:17.35\00:17:20.77 And it probably doesn't have an adverse effect on 00:17:20.80\00:17:25.00 heart disease - whereas synthetic progesterone does. 00:17:25.03\00:17:27.74 Again, synthetic progesterone, what it does is it lowers 00:17:27.77\00:17:33.98 the good cholesterol, that HDL - it lowers that, 00:17:34.01\00:17:37.14 and then the bad cholesterol, the LDL, it RAISES that. 00:17:37.17\00:17:43.27 Heart disease is a big issues too then. 00:17:43.30\00:17:45.21 Heart disease is a big issue if you take progesterone, 00:17:45.24\00:17:48.61 but it doesn't appear that natural 00:17:48.64\00:17:50.06 progesterone will do that. 00:17:50.09\00:17:51.24 Does any of this stuff give you a headache - I don't know if you would say great, 00:19:46.63\00:19:49.50 but some people would say "great" the increased 00:19:49.53\00:19:51.54 risk of breast cancer and different things like this 00:19:51.57\00:19:54.32 that we've talked about, but ALSO we talked that 00:19:54.35\00:19:56.97 even though there are those risks, 00:19:57.00\00:19:58.37 there are some real things that cause us to think about 00:19:58.40\00:20:01.11 maybe those are positive things to do like - 00:20:01.14\00:20:03.57 how the effect of estrogen on osteoporosis 00:20:03.60\00:20:08.52 so it doesn't effect this as much, 00:20:08.55\00:20:09.99 but you talked about a more excellent way, 00:20:10.02\00:20:12.23 in a sense than even those. 00:20:12.26\00:20:14.55 I'm not saying that they don't have a place, 00:20:14.58\00:20:17.15 but we've talked about diet and we've talked about exercise 00:20:17.18\00:20:19.71 and about osteoporosis, and we've talked about 00:20:19.74\00:20:21.48 hot flashes and how we can be involved in helping 00:20:21.51\00:20:27.37 those things in other ways 00:20:27.40\00:20:28.76 other than hormone replacement therapy. 00:20:28.79\00:20:31.56 What are some other things that can be used 00:20:31.59\00:20:36.02 to help with hot flashes other than hormone replacement therapy 00:20:36.05\00:20:41.89 Well, hot flashes are a big problem for many women 00:20:41.92\00:20:47.82 who go through the menopause. 00:20:47.85\00:20:49.59 But I should say that not all women have hot flashes, 00:20:49.62\00:20:52.46 and so women don't necessarily 00:20:52.49\00:20:56.36 need to dread going through the menopause. 00:20:56.39\00:20:59.25 But hot flashes are a significant issue, 00:20:59.28\00:21:02.72 and one of things that we can use as opposed to 00:21:02.75\00:21:07.38 traditional estrogen replacement therapy 00:21:07.41\00:21:10.40 would be natural progesterone, 00:21:10.43\00:21:12.59 and we've talked about that before, 00:21:12.62\00:21:14.15 but natural progesterone cream... 00:21:14.18\00:21:17.73 It's a transdermal - you put it on the skin, 00:21:17.76\00:21:24.54 it can significantly decrease hot flashes. 00:21:24.57\00:21:28.03 Studies have shown an 85% response rate. 00:21:28.06\00:21:31.92 Clonidine is another medication, actually it's a centrally active 00:21:31.95\00:21:38.41 ...to be technical, an alpha-2-adrenergic agonist, 00:21:38.44\00:21:42.47 and so what it does, it's actually given for 00:21:42.50\00:21:45.27 high blood pressure many times, 00:21:45.30\00:21:47.19 but it also has an 80% effectiveness rate 00:21:47.22\00:21:53.27 in reducing hot flashes. 00:21:53.30\00:21:55.13 Medications such as selective serotonin reuptake inhibitors - 00:21:55.16\00:22:00.00 These people know these like Zoloft and Prozac, 00:22:00.03\00:22:05.26 they are antidepressants. 00:22:05.29\00:22:07.40 And then a natural remedy would be black cohosh. 00:22:07.43\00:22:11.46 That is an herb and that has been shown to reduce 00:22:11.49\00:22:16.37 hot flashes - probably most studies would say 60-70% 00:22:16.40\00:22:21.84 Well how much do you take of black cohosh? 00:22:21.87\00:22:24.34 Well that's at a food store or at a natural food store, 00:22:27.27\00:22:32.56 you can find this - they come in tablets 00:22:32.59\00:22:35.35 and you can take a couple of those a day. 00:22:35.38\00:22:37.63 Take one in the morning and one in the evening, 00:22:37.66\00:22:40.21 would be a fine way to do it. 00:22:40.24\00:22:42.31 So, again, then the natural progesterone cream 00:22:42.34\00:22:46.12 you have said it's always better than the synthetic. 00:22:46.15\00:22:49.90 That's right, and I think that's probably a good rule 00:22:49.93\00:22:53.36 of thumb, that we want to stay with natural things 00:22:53.39\00:22:55.96 rather than the synthetic ones as much as possible. 00:22:55.99\00:22:59.38 Now another thing that sometimes ladies deal with is 00:22:59.41\00:23:03.51 severe vaginal dryness after menopause, 00:23:03.54\00:23:06.69 and sometimes hormone replacement therapy is 00:23:06.72\00:23:10.61 shown to help that. 00:23:10.64\00:23:12.80 I guess as you described before, maybe tell us how that works, 00:23:12.83\00:23:16.17 and then what can we do in place of hormone replacement therapy? 00:23:16.20\00:23:20.46 Well what we can do is... 00:23:20.49\00:23:24.54 the first thing we can do is moisturizers which should be 00:23:24.57\00:23:31.64 water-soluble - there are things called Astroglide or Replens, 00:23:31.67\00:23:35.18 but it needs to be water-soluble. 00:23:35.21\00:23:38.06 There are also very low dose estrogens. 00:23:38.09\00:23:42.04 If you take an extremely low dose estrogen intravaginally, 00:23:42.07\00:23:47.50 it does not appear to increase the risk for breast cancer, 00:23:47.53\00:23:52.29 and there are many low dose estrogens on the market... 00:23:52.32\00:23:57.15 And I should say - that there is a difference, in my opinion, 00:23:57.18\00:24:01.72 between natural products in hormones such as estrogen now, 00:24:01.75\00:24:09.41 as opposed to bioidentical. 00:24:09.44\00:24:13.25 I think bioidentical is what we want to be focusing on 00:24:13.28\00:24:16.29 for the female - the human female that is 00:24:16.32\00:24:19.80 because there are 3 types of estrogen that a 00:24:19.83\00:24:22.96 woman's body makes; estradiol, estriol, and estrone, 00:24:22.99\00:24:28.70 and those are the types of estrogens 00:24:28.73\00:24:33.14 that a woman should be taking. 00:24:33.17\00:24:35.55 So that we may call something "natural" 00:24:35.58\00:24:38.91 such as - say, Premarin, the most commonly prescribed 00:24:38.94\00:24:42.59 estrogen in this country. 00:24:42.62\00:24:44.54 It gets its name "Premarin" from its source, 00:24:44.57\00:24:49.58 and that is - it comes from pregnant mares' urine. 00:24:49.61\00:24:53.58 So Pre-mar-in is coming from pregnant mares' urine... 00:24:53.61\00:24:58.50 I kind of wish I didn't know that! 00:24:58.53\00:24:59.69 ...And it is natural because it's coming from a horse, 00:24:59.72\00:25:04.77 but is that what a woman needs? 00:25:04.80\00:25:07.34 Actually, there's about 21 different types of estrogens 00:25:07.37\00:25:10.68 in a horse and they are then in the urine, 00:25:10.71\00:25:14.35 and they concentrate those and put it in a pill. 00:25:14.38\00:25:17.09 And that could technically be called "natural" 00:25:17.12\00:25:20.47 but obviously that's not the natural 00:25:20.50\00:25:22.23 that we're talking about here. 00:25:22.26\00:25:23.50 So you want to get into what's really natural for the lady. 00:25:23.53\00:25:26.94 It's what I would call bioidentical. 00:25:26.97\00:25:29.20 It is what our body makes and 00:25:29.23\00:25:33.60 that would be either the estradiol, estriol or estrone, 00:25:33.63\00:25:37.06 and there are a lot of products out there, 00:25:37.09\00:25:38.68 even though they might be synthetically made, 00:25:38.71\00:25:40.70 they are bioidentical to the estrogens 00:25:40.73\00:25:44.61 that our own body makes. 00:25:44.64\00:25:45.84 I think that's probably true in almost everything you get 00:25:45.87\00:25:48.08 over-the-counter, even these vitamins and different things 00:25:48.11\00:25:51.78 that they have a lot of... 00:25:51.81\00:25:54.59 You never really know what's going to work 00:25:54.62\00:25:56.60 unless you get what is bioidentical... That's right 00:25:56.63\00:25:59.74 ...figuring that out. 00:25:59.77\00:26:00.84 Well, we've talked about this hormone replacement therapy 00:26:00.87\00:26:03.89 but again, it seems that God's way of dealing with things is 00:26:03.92\00:26:07.04 always better. 00:26:07.07\00:26:08.14 Why don't we end up by talking a little bit more about diet 00:26:08.17\00:26:10.61 and how it relates to all these issues. 00:26:10.64\00:26:13.45 Yes, actually if we had the right diet, 00:26:13.48\00:26:15.32 I'm not sure we'd even need to be talking about this 00:26:15.35\00:26:17.61 because if you look at Chinese women out in the rural parts 00:26:17.64\00:26:21.30 of the country, they don't have menopausal symptoms, 00:26:21.33\00:26:24.56 and they don't have osteoporosis and why is that? 00:26:24.59\00:26:26.91 Because they're living on a simple diet; 00:26:26.94\00:26:30.28 one that is primarily plant-based; 00:26:30.31\00:26:33.01 one that doesn't have any or just a very little bit of 00:26:33.04\00:26:38.05 animal protein and so they don't have the problems 00:26:38.08\00:26:41.47 with the osteoporosis... 00:26:41.50\00:26:43.01 And the fat that is in the Western diet is what 00:26:43.04\00:26:47.27 increases the amount of estrogen that a woman produces 00:26:47.30\00:26:51.04 and she gets used to this high level of estrogen 00:26:51.07\00:26:54.79 throughout her life and then she goes through the menopause 00:26:54.82\00:26:57.21 and the symptoms often are due to the degree of change 00:26:57.25\00:27:03.33 in the estrogen rather than the 00:27:03.36\00:27:04.39 actual level of estrogen itself. 00:27:04.42\00:27:06.68 So you were running on high octane and then you went to 00:27:06.71\00:27:08.96 nothing and it's like the car just STOPPED! That's right 00:27:08.99\00:27:14.44 Well again, it sounds like hormone replacement therapy 00:27:14.47\00:27:17.47 and all these things that God designed 00:27:17.50\00:27:19.08 us with hormones in our bodies, the best thing for our bodies 00:27:19.11\00:27:23.65 if we are women or for men, 00:27:23.68\00:27:25.19 is to eat what He told us to eat to begin with.
