And how old are they? 00:01:12.41\00:01:14.94 Genesis is 24, and Jason, he's 18. 00:01:14.97\00:01:22.45 All right, well great! 00:01:22.48\00:01:24.22 So you have a wealth of experience as a mother, 00:01:24.23\00:01:26.24 and as a midwife, and with the time as well. 00:01:26.25\00:01:29.76 You work with a team, I understand, 00:01:29.79\00:01:31.11 you have some physicians and people that 00:01:31.14\00:01:32.69 work with you as well where you are practicing. 00:01:32.72\00:01:35.13 Today we're going to be talking about postpartum care. 00:01:35.16\00:01:39.57 What does that big word mean? 00:01:39.58\00:01:41.34 Postpartum care is right after the birth thru 6 weeks, 00:01:41.37\00:01:46.05 and we do postpartum care on the mother right after delivery. 00:01:46.08\00:01:51.98 We go back the 3rd day to check her and the baby again. 00:01:52.01\00:01:56.11 Then we see her in 2 weeks, 00:01:56.12\00:01:57.94 and then we would like to see her again at 6 weeks. 00:01:57.97\00:02:02.31 So right afterwards, then 2 weeks... 00:02:02.34\00:02:04.23 3 days, 2 weeks and then 6 weeks... Okay, got it! 00:02:04.26\00:02:09.31 You know, one thing before we get into that, 00:02:09.34\00:02:11.40 you and I were talking and maybe we can just 00:02:11.43\00:02:15.84 discuss this a little bit. 00:02:15.87\00:02:17.00 What if someone has had several pregnancies, 00:02:17.03\00:02:19.72 and maybe the last one or maybe their first one, 00:02:19.75\00:02:22.31 or whatever it was, was a cesarean section or a C-section, 00:02:22.34\00:02:25.69 and they want to have a vaginal delivery or a regular birth, 00:02:25.72\00:02:30.20 can that ever happen? 00:02:30.23\00:02:32.06 Yes, they are actually encouraging VBACs. 00:02:32.09\00:02:37.50 They call it "vaginal birth after cesareans. " 00:02:37.51\00:02:39.35 This was proven in a Flamm Study, 1994. 00:02:52.06\00:02:56.00 Okay, so it is possible then to have it again. 00:02:56.03\00:02:59.76 Okay, so does it mean you just 00:03:07.99\00:03:09.73 naturally have to have another one? Right 00:03:09.76\00:03:12.24 So, if your physician is a younger OB-GYN 00:03:20.47\00:03:25.03 they're going to be more open to that. 00:03:25.06\00:03:27.71 Have you ever, as a midwife, delivered someone 00:03:27.74\00:03:30.70 that was a cesarean before? 00:03:30.73\00:03:32.27 Yes, but we encourage the VBACs to be delivered in the hospital 00:03:32.30\00:03:39.51 Just in case there is a rupture. Yes 00:03:39.52\00:03:41.39 And like we said, it's very rare, 00:03:41.40\00:03:43.60 but I know they are estimating about $200,000 00:03:43.63\00:03:50.55 could be saved a year by giving them a trial of labor, vaginally 00:03:50.58\00:03:55.38 $200,000 across the nation... okay 00:03:55.41\00:03:59.03 Let's go back to postpartum care... 00:03:59.06\00:04:02.64 You know, we talked about when it is, 00:04:02.65\00:04:04.50 3 days, 2 weeks, and then 6 weeks again, right? 00:04:04.53\00:04:08.46 Immediately after the birth too. Um hm 00:04:08.47\00:04:09.84 But what do you do then? 00:04:09.88\00:04:10.91 What do you do immediately after the birth, 00:04:10.95\00:04:12.73 and walk us through that; what does it entail? 00:04:12.76\00:04:15.67 Okay, well immediately after the birth, 00:04:15.70\00:04:17.71 we check the uterus to make sure that it's firm, 00:04:17.74\00:04:20.98 and the baby will be nursing right after the birth 00:04:21.01\00:04:24.95 to contract the uterus. 00:04:24.96\00:04:26.26 The uterus releases oxytocin, it's a natural oxytocin 00:04:26.29\00:04:31.73 to cause the uterus to contract... 00:04:31.74\00:04:35.05 So that's a hormone or something? 00:04:35.08\00:04:36.22 Yes, and it causes the uterus to go back to size 00:04:36.25\00:04:39.20 called uterine involution, back to its pre-pregnant state. 00:04:39.23\00:04:45.14 So we check the uterus to make sure it's firm, 00:04:45.17\00:04:49.18 and check for bleeding; make sure that 00:04:49.21\00:04:52.32 there is very minimal bleeding... 00:04:52.35\00:04:54.08 And we make sure that the mother is breastfeeding, 00:04:54.11\00:04:58.47 the baby has latched-on and they're bonding. 00:04:58.50\00:05:01.77 Let's go back to that... checking the... 00:05:01.80\00:05:03.62 what did you call it, the fundus... 00:05:03.63\00:05:05.71 The top of the uterus is the fundus. 00:05:05.74\00:05:08.64 And that's the top of the uterus, 00:05:08.67\00:05:09.98 and how do you make sure it's firm? 00:05:10.01\00:05:11.74 You just feel the top of the uterus to make sure 00:05:11.77\00:05:14.85 that it's firm... 00:05:14.88\00:05:16.29 So, like if I had a uterus, which I don't, 00:05:16.32\00:05:19.18 which I'm fairly certain, I'd be touching it 00:05:19.21\00:05:21.37 right up here above my belly button? 00:05:21.40\00:05:23.18 Well after the baby is born, it's usually about 00:05:23.21\00:05:25.73 2 fingers below the umbilicus. 00:05:25.76\00:05:27.77 Okay, so my belly button... 00:05:27.80\00:05:29.21 and I just take 2 fingers and below that 00:05:29.22\00:05:31.22 there should be what there? 00:05:31.25\00:05:32.92 The top of the uterus. What does it feel like? 00:05:32.95\00:05:35.40 It will feel just like a hard... 00:05:35.43\00:05:38.57 I want to say grapefruit or something. 00:05:38.60\00:05:41.93 Okay, and if it's not there, what do I do? 00:05:41.96\00:05:43.73 If it's real loose and boggy, it will feel loose, 00:05:43.76\00:05:49.81 but you probably have a bleed. 00:05:49.82\00:05:51.59 A pretty good bleed. Okay, what do you do? 00:05:51.62\00:05:53.65 Well, what we would do is what they call... 00:05:53.66\00:05:57.63 "bimanual compression" where you clamp it yourself, 00:05:57.66\00:06:00.18 and you hold it until it gets firm. 00:06:00.21\00:06:02.14 You can give... if you use herbs to control bleeding. 00:06:02.15\00:06:09.20 And of course, in the hospital, they'll give Pitocin 00:06:09.23\00:06:13.23 or a drug to clamp the uterus. 00:06:13.26\00:06:15.34 So that's like the oxytocin but it's a medication. 00:06:15.37\00:06:19.53 So you check and make sure and if it's NOT, 00:06:19.56\00:06:22.35 then you probably have a bleed or something... 00:06:22.38\00:06:24.68 and you have to be careful about that. 00:06:24.71\00:06:27.02 Anything else then? Any education or 00:06:27.05\00:06:29.64 different things that you tell moms afterwards... 00:06:29.67\00:06:31.21 Well, we make sure that they are well-educated, 00:06:31.24\00:06:33.51 what to look for; if they have fever, pain, excess bleeding. 00:06:33.54\00:06:38.86 Check the baby; make sure that the baby is nursing well... 00:06:38.87\00:06:45.10 How do you know if a baby is nursing well? 00:06:45.13\00:06:46.54 Well, we wouldn't leave until the baby has latched-on, 00:06:46.57\00:06:51.69 and nursing well, and we want to make sure that the 00:06:51.72\00:06:56.35 mother who had the baby can get up and void, have a shower, 00:06:56.38\00:07:01.44 and everything works normal before we leave. 00:07:01.47\00:07:04.63 We wouldn't leave her until everything is fine, she's fed. 00:07:04.64\00:07:08.44 Okay, back to that breastfeeding thing... 00:07:08.47\00:07:10.32 How do you know that it has attached well or whatever? 00:07:10.35\00:07:12.58 Latched-on... the baby is latched-on. 00:07:12.61\00:07:14.19 I pretty much can figure that out, 00:07:14.22\00:07:16.46 but what do you mean exactly? 00:07:16.49\00:07:17.52 How do you know if it has latched-on well? 00:07:17.55\00:07:19.27 The baby will be nursing... 00:07:19.30\00:07:23.24 Okay, and no air or different things in there. 00:07:23.27\00:07:26.80 Yes, you would know because it wouldn't be crying. 00:07:26.83\00:07:30.23 It would be nursing. 00:07:30.26\00:07:32.19 Plus, let me just say this... 00:07:32.22\00:07:35.74 It wouldn't hurt... when the baby is nursing right, 00:07:35.77\00:07:39.54 it wouldn't hurt her. 00:07:39.57\00:07:41.18 Oh, it doesn't hurt the mother. Yeah 00:07:41.19\00:07:43.55 Because if it doesn't have a good latch on, 00:07:43.56\00:07:46.24 then she could say, "Well, this just doesn't feel right. " 00:07:46.27\00:07:50.65 So, what should the mother then be watching for, 00:07:50.68\00:07:56.21 I mean, you're there right after the delivery... 00:07:56.24\00:07:58.02 Do you do the Apgar score then right after? 00:07:58.05\00:07:59.99 Is that part of postpartum care? Yes 00:08:00.02\00:08:02.18 What are they checking on an Apgar score? 00:08:02.21\00:08:04.32 Okay, we check at 1 minute, and then at 5 again. 00:08:04.33\00:08:07.16 And what are you checking? 00:08:07.20\00:08:08.88 We check the baby's color. 00:08:08.91\00:08:10.55 We make sure that the baby is breathing well. 00:08:10.56\00:08:12.66 The heart rate is 120 or above, 00:08:12.69\00:08:18.69 and the normal rate is anywhere from 120-160... 00:08:18.72\00:08:22.03 And we also make sure the respiration is good. 00:08:22.06\00:08:26.43 The baby should be well-flexed and not limp... 00:08:26.46\00:08:30.24 And of course, we make sure the baby is warm. 00:08:30.27\00:08:32.49 We put a warm towel on the baby and cap on its head. 00:08:32.52\00:08:37.93 So babies can get cold pretty easily; 00:08:37.96\00:08:39.46 it's very important to do all that. 00:08:39.49\00:08:40.76 Yes, it's hard to hold their heat in for the first 24 hours, 00:08:40.79\00:08:43.68 and that's why we encourage the baby to be extra warm. 00:08:43.71\00:08:46.95 What about when a baby gets yellow? Jaundiced? 00:08:46.98\00:08:49.86 Or jaundiced, whatever you call that. 00:08:49.87\00:08:52.02 Jaundice is fairly normal in newborn babies. 00:08:52.05\00:08:56.01 And why is that? What's going on? 00:08:56.04\00:08:57.48 Because we have an elevated Bilirubin, 00:08:57.51\00:09:00.54 the excess Bilirubin and the weakened or the immature 00:09:00.57\00:09:05.27 liver of the baby is having trouble... Can't handle it... 00:09:05.31\00:09:09.98 And around 3 days, the third day is the peak; 00:09:10.01\00:09:12.23 in 8-10 days then it should be fine. 00:09:12.26\00:09:17.11 But usually the jaundice isn't a problem. 00:09:17.14\00:09:22.93 I know in the hospital when I was in nurses' training, 00:09:22.94\00:09:26.04 they put me in this little nursery thing, 00:09:26.07\00:09:27.52 and there were like 30 babies in there, and they had a 00:09:27.55\00:09:29.13 bunch of them under these little lamps or different things. 00:09:29.17\00:09:30.70 Do you do that at home? 00:09:31.95\00:09:33.29 We use sunlight. 00:09:33.32\00:09:35.31 Sunlight... put them by the window? 00:09:35.34\00:09:36.89 Yes, and nursing is a benefit. 00:09:36.92\00:09:40.82 It helps flush excess Bilirubin, 00:09:40.85\00:09:43.92 and also activated charcoal. 00:09:43.95\00:09:47.76 What about circumcisions? 00:09:47.79\00:09:49.34 Well, if they want to have circumcisions, 00:09:49.37\00:09:53.70 we always recommend the 8th day, 00:09:53.73\00:09:56.40 like the Bible says, because the vitamin K factor 00:09:56.43\00:09:59.26 is the highest in the baby. 00:09:59.29\00:10:00.48 And they do that in the hospital? 00:10:00.51\00:10:03.08 No, they have their own pediatrician if they 00:10:03.11\00:10:05.09 choose to have circumcision, they would go to. 00:10:05.12\00:10:07.79 Many times new moms like to call the doctor 00:10:07.82\00:10:10.30 and check things out or maybe they call you very often. 00:10:10.33\00:10:13.50 When should they call their pediatrician? 00:10:13.53\00:10:15.26 Okay, well they should call their pediatrician when 00:10:15.29\00:10:20.78 they don't see stools or urination in 24-48 hours. 00:10:20.81\00:10:24.77 That's constipation then? 00:10:24.80\00:10:25.85 They could be bound up, that's why? 00:10:25.86\00:10:27.96 Well after delivery, we just want to make sure 00:10:27.99\00:10:32.65 everything works. 00:10:32.68\00:10:35.38 It might be but it might not be. 00:10:35.41\00:10:36.91 It might be that something is not developed properly. 00:10:36.94\00:10:40.89 Difficult breathing. Baby's color is blue. 00:10:40.92\00:10:45.09 Jaundice with lethargy... 00:10:45.12\00:10:46.97 Remember we talked about jaundice as... 00:10:47.00\00:10:48.72 Yellowing skin and all that. 00:10:48.75\00:10:49.98 But is fairly normal, but if you have jaundice 00:10:50.01\00:10:53.54 with lethargy and failure to nurse, then there's a problem. 00:10:53.57\00:10:57.34 So lethargy - that means just like what? 00:10:57.37\00:11:01.07 Just being out-of-it? Yes 00:11:01.10\00:11:03.46 It just lays around and you have to wake 00:11:03.49\00:11:06.67 ...sleepy baby, doesn't want to nurse. 00:11:06.70\00:11:08.93 Vomiting; fever over 99.6 under the arm, 00:11:08.96\00:11:13.62 or umbilical cord red or swollen with odor. 00:11:13.65\00:11:16.98 Okay, so there might be an infection. 00:11:17.01\00:11:18.16 Might be an infection. 00:11:18.19\00:11:20.18 Okay, so these are the reasons that you need to call 00:11:20.21\00:11:23.29 the doctor then right away. Yes 00:11:23.32\00:11:25.91 And, we show them how to watch for signs, 00:11:25.94\00:11:29.96 and how to take care of the umbilical cord; 00:11:29.99\00:11:33.44 make sure that it stays... Well how do they do that? 00:11:33.45\00:11:35.73 Well, we make sure it stays above their diaper. 00:11:35.76\00:11:39.03 Every time they change a diaper, 00:11:39.06\00:11:40.18 clean it real well with alcohol. 00:11:40.21\00:11:42.14 So none of the fecal material gets on it 00:11:42.17\00:11:44.23 causing an infection from that. 00:11:44.26\00:11:45.60 Keep it above their diaper so they don't urinate on it. 00:11:45.63\00:11:48.47 Look for any pus or any swelling, 00:11:48.50\00:11:52.32 or red irritation or odor. 00:11:52.35\00:11:55.62 So as long as you keep it clean... 00:11:55.65\00:11:57.59 it usually falls off about the 10th day. 00:11:57.62\00:12:00.75 So these stools that the baby has right off the bat, 00:12:00.78\00:12:03.20 they could be a little bit 00:12:03.23\00:12:05.85 something to deal with, can't they? Yes 00:12:05.88\00:12:09.97 So what advice do you give to them so they don't develop 00:12:10.00\00:12:13.08 a rash or different things... Or if they do what 00:12:13.11\00:12:15.14 I've seen some pretty bad cases of diaper rash. 00:12:15.17\00:12:17.56 Well, just to check them and to make sure that they 00:12:17.59\00:12:21.18 change their diaper often and not leave 00:12:21.21\00:12:23.48 the urination or the messy stools on their bottom. 00:12:23.51\00:12:26.95 But if they develop a diaper rash, 00:12:26.98\00:12:34.42 they could use... there are some natural remedies 00:12:34.45\00:12:36.73 like blended-up oatmeal... 00:12:36.76\00:12:39.08 Blended up oatmeal... Um hm 00:12:39.11\00:12:40.61 Just like into powder and that's real soothing 00:12:40.64\00:12:43.67 and healing for the diaper rash. 00:12:43.70\00:12:46.03 Open to air? Yes, that's fine. 00:12:46.06\00:12:48.86 What would you say, cloth diapers better than regular ones 00:12:48.89\00:12:53.53 Yes, 100% cotton. 00:12:53.56\00:12:54.96 100% cotton... Yes, she does. 00:14:52.14\00:14:55.26 We always recommend that hopefully the father can 00:14:55.29\00:14:59.78 stay home and help her, or she can have her mother 00:14:59.81\00:15:03.59 come to help her for at least 2 weeks. 00:15:03.62\00:15:06.56 Also, I find that a lot of church members 00:15:06.59\00:15:09.96 will bring in meals for the next 2 weeks postpartum. 00:15:09.99\00:15:12.53 So the biggest thing is to bring in activities 00:15:12.56\00:15:14.85 that they normally would have to do on their own; 00:15:14.88\00:15:16.85 cooking meals and different things, 00:15:16.86\00:15:18.07 take care of those things for them. All right 00:15:38.87\00:15:42.47 When can she return to a normal lifestyle... 00:15:42.50\00:15:44.58 And again, this is something 00:15:44.61\00:15:45.59 we talked about we needed to talk about. 00:15:45.62\00:15:47.86 Usually after the 6-week period, 00:15:47.89\00:15:51.58 because 6-weeks postpartum is actually when 00:15:51.61\00:15:56.27 the placental site is healed over... 00:15:56.30\00:16:00.27 Because when the placenta comes out, 00:16:00.30\00:16:02.30 after the placenta detaches, 00:16:02.33\00:16:05.12 there is a space where the placenta was 00:16:05.15\00:16:12.48 and the bleeding takes place. 00:16:12.51\00:16:15.31 So there's the potential for bleeding and different things 00:16:15.34\00:16:17.22 if you get too vigorous and exercise and different things 00:16:17.25\00:16:20.86 How soon can you start exercising afterwards? 00:16:20.89\00:16:22.98 Right after the 6-week period. 00:16:23.01\00:16:24.90 Right after the 6-week period, but up until that point 00:16:24.93\00:16:27.41 you kind of take it easy. 00:16:27.44\00:16:28.44 Can you walk? Yes 00:23:13.31\00:23:16.19 Okay, I'm not real sure how long it takes for it to dry up, 00:23:16.22\00:23:20.38 people vary, but you can do things, I guess naturally 00:23:20.41\00:23:28.56 to help, like they say cabbage leaf will help dry up milk. 00:23:28.59\00:23:34.54 So you wear that kind of like a poultice on that area.
