Thank you, Don 00:00:52.95\00:00:55.06 Now, you're a certified professional midwife, 00:00:55.09\00:00:57.56 and you've delivered now over what... 00:00:57.59\00:01:01.14 750 babies, you told me, 00:01:01.17\00:01:02.93 and you've been at this for how long? 00:01:02.96\00:01:04.89 About 15 years now. 00:01:04.92\00:01:06.59 You work as part of a team I understand. 00:01:06.62\00:01:08.63 We have wonderful doctor backup. 00:01:08.66\00:01:10.68 Okay, so you work with area hospital and physicians there... 00:01:10.71\00:01:13.91 And today, we're going to be talking about 00:01:13.94\00:01:16.39 having babies and different things. 00:01:16.42\00:01:19.10 What's one of the most important needs you see 00:01:19.14\00:01:21.16 when you work with people that are going to have a baby? 00:01:21.19\00:01:24.54 I see a very important need by needing prenatal care. 00:01:24.55\00:01:30.69 Okay, prenatal care and that's what we're going to 00:01:30.72\00:01:32.68 talk about today. Yes 00:01:32.71\00:01:34.00 Prenatal care... what is that? 00:01:34.01\00:01:36.08 What does that mean when you talk about prenatal care? 00:01:36.11\00:01:38.17 Prenatal care is caring for the mother and baby 00:01:38.20\00:01:41.96 in a way to help promote good physical and mental health 00:01:41.99\00:01:48.12 from conception to birth, 00:01:48.15\00:01:50.47 and that's around 38-40 weeks' gestation sometimes. 00:01:50.48\00:01:53.77 So, how often do you meet with them, 00:01:53.80\00:01:55.85 and what are you talking about with mothers 00:01:55.88\00:01:58.35 before they have their baby? 00:01:58.38\00:01:59.91 Well, we meet with them about once a month, 00:01:59.92\00:02:04.25 and then around 7 months we'll see them every 2 weeks, 00:02:04.28\00:02:08.84 and then the last month of pregnancy, 00:02:08.87\00:02:10.99 we'll see them once a week. 00:02:11.00\00:02:12.31 Do you want the dads to come in too? 00:02:12.34\00:02:13.81 Yes, we try to get the fathers involved. 00:02:13.84\00:02:15.74 And what do you say to them when they come in? 00:02:15.77\00:02:17.21 Well, they usually have a lot of questions, 00:02:17.24\00:02:21.24 and so I would think a big portion of it is 00:02:21.27\00:02:25.65 educating them as well. 00:02:25.68\00:02:27.58 You know, my wife is pregnant, and so we went in to see 00:02:27.61\00:02:33.78 the doctor and different things, and it was kind of unusual 00:02:33.81\00:02:38.46 for me to sit there in the waiting room with 00:02:38.49\00:02:39.97 40 or 50 pregnant ladies and they kind of looked at me 00:02:40.00\00:02:43.19 like I was a sore thumb that maybe I shouldn't be there. 00:02:43.22\00:02:46.44 Some of them looked ANGRY at me, actually... not angry, 00:02:46.47\00:02:49.51 you know... sort of like why did you get us into this 00:02:49.54\00:02:51.91 kind of problem or whatever... 00:02:51.94\00:02:55.49 So 38-40 weeks; so you said, what... 00:02:55.52\00:02:57.35 once every week after 7 months. 00:02:57.38\00:03:00.88 Well, the last month of pregnancy, 00:03:00.91\00:03:03.14 we encourage them to come once a week for prenatal visits... 00:03:03.17\00:03:07.39 Unless if we see a problem, we would see them more often. 00:03:07.42\00:03:12.17 Well what if someone says, "Well, this sounds interesting, 00:03:12.20\00:03:15.23 but it doesn't seem that important" 00:03:15.26\00:03:16.49 WHY is it so important? Or is it? 00:03:16.52\00:03:18.34 It's very important because during prenatal care, 00:03:18.37\00:03:22.11 you could rule out high risk situations that possibly 00:03:22.14\00:03:26.29 could be turned around, or refer high risk conditions 00:03:26.32\00:03:31.91 to obstetrical care... 00:03:31.94\00:03:33.57 And it's very important that during the prenatal period 00:03:33.58\00:03:39.78 that we could figure out what person would be in that 00:03:39.81\00:03:45.74 category of high risk, because it could 00:03:45.77\00:03:47.89 endanger the mother and the baby. 00:03:47.92\00:03:49.51 Do they run tests? Do they do different 00:03:49.54\00:03:51.07 things during that time? 00:03:51.10\00:03:53.07 Yes. For example, when a pregnant lady comes in to 00:03:53.10\00:03:59.81 see me and then we start off doing their blood work, 00:03:59.84\00:04:04.26 urinalysis, check their weight, and different kinds of things. 00:04:04.29\00:04:09.94 I think that you have a graphic about some of the things 00:04:09.97\00:04:12.13 that you do on prenatal care and we can look at that... 00:04:12.16\00:04:16.63 But what are the things that we're looking for then? 00:04:16.66\00:04:20.99 Okay, we check weight at each visit to make sure that 00:04:21.02\00:04:26.01 her weight gain is appropriate, and we check her blood pressure, 00:04:26.04\00:04:32.62 and her pulse because we need to know the baseline; 00:04:32.65\00:04:35.13 what her blood pressure normally is. 00:04:35.16\00:04:37.66 What it normally runs, how fast it is, 00:04:37.67\00:04:40.49 so that when they have the baby... If there's a problem 00:04:40.52\00:04:42.74 that we would know about it. 00:04:42.75\00:04:44.19 We check for edema, any swelling. 00:04:44.20\00:04:47.96 And, we do a Chemstrip urinalysis to check their urine 00:04:47.99\00:04:52.05 to make sure that there are no abnormalities, 00:04:52.08\00:04:54.94 or anything that would be a problem in the urine. 00:04:54.97\00:04:57.53 And, of course, we palpate the baby... 00:04:57.56\00:05:00.97 We check for kicking and fetal movement 00:05:00.98\00:05:05.14 because we want to determine how much the baby is moving... 00:05:05.17\00:05:07.72 Also, we check the uterus size and we check the uterus size by 00:05:07.75\00:05:14.14 measuring the fundal height. 00:05:14.17\00:05:15.47 The fundus is the top of the uterus. 00:05:15.50\00:05:19.15 So to see how tall it's getting, so to speak. 00:05:19.18\00:05:22.78 Right, how big the baby is growing. 00:05:22.81\00:05:24.97 For example, if she's 28 weeks in gestation, 00:05:25.00\00:05:28.91 then she should be measuring around 28 centimeters. 00:05:28.94\00:05:32.05 So "gestation" that means what? 00:05:32.08\00:05:34.34 That's 28 weeks pregnant. 00:05:34.37\00:05:35.68 ...28 weeks pregnant, then they measure, 00:05:35.71\00:05:37.67 and it should be about that... Around 27-28 centimeters. 00:05:37.70\00:05:42.39 Also, we palpate the baby; 00:05:42.42\00:05:44.88 the palpation determines the position. 00:05:44.91\00:05:50.41 "Palpation" - is this a fancy word for you feel where it is? 00:05:50.44\00:05:52.98 Yes, feeling the baby through the uterine wall. 00:05:53.01\00:05:55.52 Okay, and then you figure out what... 00:05:55.55\00:05:57.24 I notice that that is kind of an acronym... 00:05:57.27\00:06:00.44 "CHECKUP" is that right? Yes! 00:06:00.47\00:06:03.10 And that's how you remember it? Yes 00:06:03.13\00:06:05.42 So, if you're watching, you write down that word "CHECKUP" 00:06:05.43\00:06:08.68 By the way, I think you recently have written a book 00:06:08.71\00:06:11.22 about all this, haven't you? Um hm 00:06:11.26\00:06:12.37 Kind of putting those practical things down here... 00:06:12.40\00:06:14.13 I have a copy of it... 00:06:14.16\00:06:15.53 "The Ministry of Midwifery" 00:06:15.56\00:06:17.26 And you have a whole section on prenatal care in that book. Yes 00:06:17.29\00:06:21.72 It looks excellent, I saw that little "CHECKUP" as I was 00:06:21.75\00:06:24.09 looking through the material here. 00:06:24.12\00:06:25.79 Well, you mentioned one thing on that graphic 00:06:25.82\00:06:28.65 that we just looked at, check weight... 00:06:28.68\00:06:31.33 And I think you want to elaborate a little bit on that. 00:06:31.36\00:06:33.33 Why are we so worried about weight? 00:06:33.36\00:06:35.31 Well weight, I think is important for the mother to 00:06:35.34\00:06:42.31 watch her weight, but if she's on a good diet, 00:06:42.34\00:06:44.82 and has a healthy lifestyle, she doesn't need to be 00:06:44.83\00:06:48.86 concerned all that much or worried about it... 00:06:48.89\00:06:51.53 But, there is a normal weight gain, it's between 20-25 pounds. 00:06:51.56\00:06:58.11 Their weight gain is coming from the amniotic fluid or the water. 00:06:58.12\00:07:06.10 It is 1.5 to 2 pounds. 00:07:06.13\00:07:08.53 Then we have extra mass 4-9 pounds. 00:07:08.56\00:07:13.19 Increased blood and body fluid, about 4-8 pounds. 00:07:13.22\00:07:19.43 That's the blood of the mother 00:07:19.44\00:07:20.60 and the fetus together right? Um hm 00:07:20.63\00:07:22.80 And growth of the uterus around 2-3 pounds, 00:07:22.83\00:07:26.01 so this is just an average. 00:07:26.04\00:07:27.97 Her placenta 1.5-2 pounds, sometimes less. 00:07:28.00\00:07:31.86 And the baby 7-8 pounds, of course we could have a 00:07:31.89\00:07:34.91 6 pound baby or over 8 pounds. 00:07:34.94\00:07:37.84 But, the average weight gain is around 20-25 pounds. 00:07:37.87\00:07:41.44 Do some mothers get worried 00:07:41.47\00:07:42.62 that they're gaining too much weight? 00:07:42.63\00:07:44.17 Yes, most of the clients that come to me, 00:07:44.18\00:07:49.15 they know that I'm going to help them by encouraging 00:07:49.18\00:07:55.16 them on a good diet and a good lifestyle and exercise. 00:07:55.17\00:07:58.41 What about those mothers that are real nauseous 00:07:58.44\00:08:00.67 during the first part of their pregnancy, 00:08:00.70\00:08:02.57 and they're not able to eat too much? 00:08:02.60\00:08:04.52 What do you do with them? 00:08:04.55\00:08:05.58 What are some practical things you do with them? 00:08:05.61\00:08:07.24 Well, it's amazing how much 00:08:07.27\00:08:09.90 just by drinking a lot of water it helps. 00:08:09.93\00:08:12.38 I find that if they're well-hydrated, 00:08:12.41\00:08:16.16 they will have less nausea. 00:08:16.19\00:08:17.98 So drink more water if you're nauseous. 00:08:18.01\00:08:20.58 And get exercise, but there IS a natural remedy 00:08:20.61\00:08:27.02 that I find, it helps me when I travel. 00:08:27.03\00:08:29.88 You know, when I go on airplane flights... 00:08:29.91\00:08:32.64 It's ginger root. 00:08:32.67\00:08:33.89 Ginger root, that's not going to harm the baby? No 00:08:33.90\00:08:37.01 Oh no, it's medicinal. All right! 00:08:37.04\00:08:39.13 So that helps with nausea? Yes 00:08:39.14\00:08:42.62 All right, so ginger root and more water, 00:08:42.65\00:08:45.42 and drink water all day. 00:08:45.45\00:08:47.61 Well, we recommend about 6-8 glasses a day. 00:08:47.64\00:08:51.80 ...6-8 glasses which is, you know, that's what they say 00:08:51.83\00:08:54.68 for normal people too... to drink 6-8 glasses, right? 00:08:54.71\00:08:57.08 And just sipping on peppermint tea. 00:08:57.11\00:08:59.11 Hmm, peppermint tea? Um hm 00:08:59.14\00:09:01.21 That's an antiemetic, it relieves nausea. 00:09:01.24\00:09:06.43 Great, well, anything else for those prenatal moms that are 00:09:06.46\00:09:12.48 wondering what they should be worried with. 00:09:12.51\00:09:14.05 We've talked about weight. 00:09:14.08\00:09:17.39 Wait, back to this weight thing... 00:09:17.42\00:09:18.70 What's too LITTLE weight? 00:09:18.71\00:09:20.28 When do you start to get worried... 00:09:20.31\00:09:22.45 Do you ever get worried when someone doesn't gain? 00:09:22.46\00:09:24.92 Does anyone have a baby that doesn't gain, that much weight? 00:09:24.93\00:09:29.61 Well yes, it would probably be smokers... 00:09:29.64\00:09:35.47 And that's something to worry about... 00:09:35.51\00:09:41.31 Yes, I would classify them as a high risk situation, 00:09:41.32\00:09:48.88 but I wouldn't recommend fasting, 00:09:48.91\00:09:51.95 because I feel like the baby needs 3 meals a day, 00:09:51.98\00:09:55.79 and the mother. 00:09:55.82\00:09:58.26 But you can tell, you know, how well the baby is growing 00:09:58.29\00:10:01.43 by prenatal visits. 00:10:01.46\00:10:04.52 You know, like I said, measuring the uterus, the fundus, 00:10:04.55\00:10:10.14 and palpating the baby. 00:10:10.17\00:10:13.18 So, you said that a baby is there for what... 00:10:13.21\00:10:16.57 38-40 weeks for pregnancy, is that right? 00:10:16.60\00:10:20.64 Usually it's from conception to birth is 38-40 weeks, 00:10:20.67\00:10:25.19 sometimes 42 weeks. 00:10:25.22\00:10:26.49 What about the first 10 weeks, 00:10:26.52\00:10:29.61 what's happening with the baby, what's developing, 00:10:29.64\00:10:33.55 and what can the mom do or not do that's going to 00:10:33.58\00:10:36.06 help that be good or not good? 00:10:36.09\00:10:38.67 Well around 8 weeks, most of the pregnant moms 00:10:38.70\00:10:44.44 don't even realize they're pregnant, 00:10:44.47\00:10:46.62 and that is the most critical time. 00:10:46.65\00:10:48.24 Eight weeks is the most critical time. Um hm 00:10:48.27\00:10:49.90 It's when the cells are RAPIDLY dividing, 00:10:49.93\00:10:53.27 and they need to be very careful around teratogens 00:10:53.30\00:10:57.65 which would be x-rays, drugs, fumes... 00:10:57.68\00:11:04.77 All those different things... 00:11:04.80\00:11:05.88 Fumes, any kind of fumes; carbon monoxide... 00:11:05.91\00:11:11.93 Paint, no paint, but later on that's okay? 00:11:11.96\00:11:15.84 Well, I would say, if you're doing any painting, 00:11:15.87\00:11:20.44 you would want it to be well-ventilated. 00:11:20.45\00:11:22.77 Well-ventilated... so why is that so important, 00:11:22.80\00:11:25.87 what's happening around 8 or 10 weeks with the baby? 00:11:25.88\00:11:27.89 That's all dividing, but what's really... 00:11:27.92\00:11:29.97 The baby is developing very rapidly, 00:11:30.00\00:11:32.80 and you just need to concentrate on eating healthy, 00:11:32.83\00:11:38.02 and making sure that nothing goes into your body that 00:11:38.05\00:11:41.10 would be damaging. 00:11:41.11\00:11:42.75 So this idea of having cravings and different things 00:11:42.78\00:11:45.11 when you're pregnant... do you have to just rise above that? 00:11:45.14\00:11:47.71 And say I'm going to eat that even though I feel like it? 00:11:47.74\00:11:49.89 Yes, you cannot trust your cravings. 00:11:49.92\00:11:54.13 Pickles and ice cream! 00:11:54.16\00:11:55.41 Pickles and ice cream... do you hear that a lot? 00:11:55.44\00:11:58.42 Well, no not really, but most of the people 00:11:58.45\00:12:02.89 that come to me really they want me to encourage them 00:12:02.92\00:12:07.89 on a healthy diet. 00:12:07.92\00:12:09.62 So, instead of the water... excuse me, 00:12:09.65\00:12:12.59 instead of the ice cream, we should have water 00:12:12.62\00:12:14.12 or something like that? 00:12:14.15\00:12:15.20 What's so important about water? 00:12:15.23\00:12:16.41 The reason why they need to drink so much water, 00:12:16.44\00:12:20.95 and we encourage hydration is because the amniotic fluid 00:12:20.98\00:12:25.99 that's surrounding the baby is reproduced every 3 hours. 00:12:26.02\00:12:30.92 So they change the water in that little swimming pool 00:12:30.95\00:12:33.52 for the little guy or gal, every 3 hours? 00:12:33.55\00:12:35.79 Yes, at least 30% of it... 00:12:35.82\00:12:37.97 But I can tell you we're talking around 1,000 mL of fluid 00:12:38.00\00:12:43.91 that's surrounding the baby and that's the protection 00:12:43.94\00:12:46.53 for the baby, the amniotic fluid. 00:12:46.54\00:12:49.28 That's the bumper. Yes, it protects the cord 00:12:49.31\00:12:52.02 from being compressed, and the baby can move 00:12:52.05\00:12:56.43 all different positions and still be protected. 00:12:56.46\00:13:02.32 So, you don't want them swimming on ice cream and pickles! 00:13:02.35\00:13:05.04 No! You want water! Right, white blood cells. 00:15:51.51\00:15:53.10 The fighters. Um hm 00:15:53.13\00:15:55.18 And if you're showing leukocytes with nitrites, 00:15:55.19\00:16:00.95 then you could possibly be fighting a bladder infection, 00:16:00.98\00:16:04.58 or "UTI" urinary tract infection. 00:16:04.61\00:16:06.72 So can you give antibiotics during pregnancy? What do you do 00:16:06.75\00:16:09.24 Well, the doctors will usually prescribe... 00:16:09.94\00:16:14.05 Take care of that; do something. 00:16:14.08\00:16:15.88 Cranberry juice work for that? 00:16:15.91\00:16:17.18 Cranberry juice, and we encourage drinking 00:16:17.21\00:16:19.33 drinking a LOT of water. 00:16:19.36\00:16:20.82 A lot of water! Um hm 00:16:20.85\00:16:22.17 Can't go wrong there. 00:16:22.20\00:16:23.19 No, you can't go wrong there... 00:16:23.22\00:16:24.30 And also, we check for glucose because we want to rule out 00:16:24.33\00:16:28.23 gestational diabetes. 00:16:28.26\00:16:30.55 Gestational diabetes... that means diabetes 00:16:30.58\00:16:33.36 during pregnancy. 00:16:33.39\00:16:35.25 Now if someone has gestational diabetes, then what do you do 00:16:35.28\00:16:40.39 as a nurse... I know you have the physician 00:16:40.42\00:16:42.20 working with you and different things, 00:16:42.23\00:16:43.26 but what do you do as a midwife with that? 00:16:43.29\00:16:46.06 Well, if they are spilling sugar in their urine, 00:16:46.09\00:16:51.85 first we go over their diet and make sure that it's 00:16:51.88\00:16:55.11 just maybe they had too much sugar that day or something, 00:16:55.14\00:16:59.40 and we would try to correct it with their diet. 00:16:59.43\00:17:02.62 If not, then of course, they would be referred 00:17:02.65\00:17:05.12 to obstetrical care where they would do other things 00:17:05.15\00:17:10.55 to rule out any problems. 00:17:10.58\00:17:12.37 Before we went to the break, you were talking about 00:17:12.40\00:17:15.78 that list of things...
