The body, could have only been designed 00:00:39.57\00:00:43.41 by the "Master Designer", we read about in the Bible. 00:00:43.44\00:00:46.64 Hello, I'm Patti Barnes, 00:01:05.03\00:01:06.76 Director of the Midwifery Program at Hartland College. 00:01:06.80\00:01:09.70 In this segment, we are going to discuss the importance 00:01:09.73\00:01:13.37 of prenatal care. 00:01:13.40\00:01:14.74 Prenatal, also called Antepartum is a period of the time 00:01:14.77\00:01:19.61 the baby is developing in the womb prior to birth. 00:01:19.64\00:01:22.91 Several studies have revealed the importance of mothers 00:01:22.94\00:01:26.78 of teen prenatal checkups early on in pregnancy. 00:01:26.82\00:01:30.19 It has been found that proper and early prenatal care 00:01:30.22\00:01:33.96 can help reduce high risk situations, 00:01:33.99\00:01:37.03 and bad outcomes such as: 00:01:37.06\00:01:39.19 These are just a few, but you can see from these 00:01:50.11\00:01:52.71 that there is certainly value in good prenatal care. 00:01:52.74\00:01:55.44 According to the Maternal and Child Health Bureau, 00:01:55.48\00:01:58.48 mothers that fail to receive prenatal care, 00:01:58.51\00:02:01.55 are three times more likely to have low birth weight babies, 00:02:01.58\00:02:05.42 and the chances of their babies dying is increased 00:02:05.45\00:02:09.16 by five times. Child Health USA reports that babies born 00:02:09.19\00:02:14.76 with a birth weight under five and a half pounds, 00:02:14.80\00:02:17.63 and especially a very low birth weight of less than 00:02:17.67\00:02:21.50 three and a quarter pounds are more likely than infants 00:02:21.54\00:02:25.37 of normal weight to die in the first year of life, 00:02:25.41\00:02:28.61 and to experience long range physical and developmental 00:02:28.64\00:02:33.01 health problems. 00:02:33.05\00:02:34.38 These are terrible, yet often avoidable outcomes, 00:02:34.42\00:02:38.15 and it isn't just the physical health we should be 00:02:38.19\00:02:41.32 concerned about. 00:02:41.36\00:02:42.69 In the book Mind, Character, and Personality, 00:02:42.72\00:02:46.19 Volume 1 we read: 00:02:46.23\00:02:48.20 Ok, so when should prenatal care begin? 00:03:14.12\00:03:16.66 The answer is surprisingly enough before pregnancy 00:03:16.69\00:03:20.70 even begins. 00:03:20.73\00:03:22.06 In the book Education, page 276, we find this 00:03:22.10\00:03:25.47 very important council: 00:03:25.50\00:03:27.70 Young people, we cannot stress enough, that you take this 00:03:50.99\00:03:56.16 responsibility seriously, it can make a great difference 00:03:56.20\00:04:00.64 in your lives and the lives of your offspring. 00:04:00.67\00:04:03.91 How sad is it today, that few people understand 00:04:03.94\00:04:07.71 these principles. I like to think of prenatal care 00:04:07.74\00:04:11.28 as the assessment of a mother's: 00:04:11.31\00:04:13.45 needs, and ways of providing for 00:04:17.02\00:04:18.89 those needs. The first visit with your healthcare provider, 00:04:18.92\00:04:22.19 should take place as early as possible in the 00:04:22.22\00:04:25.63 first trimester of pregnancy. 00:04:25.66\00:04:27.40 The gestational period is basically divided into three 00:04:27.43\00:04:30.73 major time periods called: 00:04:30.77\00:04:34.00 Which roughly breaks down as follows: 00:04:34.04\00:04:36.60 At the first prenatal visit, your healthcare provider 00:04:49.92\00:04:52.49 should determine your estimated due date. 00:04:52.52\00:04:54.96 Then go over the health or medical history of you 00:04:54.99\00:04:57.39 and your family. 00:04:57.43\00:04:58.76 Review previous pregnancies if any. 00:04:58.79\00:05:01.46 Evaluate possible risk factors. 00:05:01.50\00:05:03.70 In addition, lab tests are done, including: 00:05:03.73\00:05:06.84 Which checks for the iron level, to rule out anemia. 00:05:06.87\00:05:10.57 Which is your blood type. 00:05:10.61\00:05:14.58 To certain conditions like: 00:05:14.61\00:05:17.75 Then a complete physical exam should be done. 00:05:23.95\00:05:26.35 After the first visit, regular prenatals should be done 00:05:26.39\00:05:30.03 every month until 28 weeks, then every two weeks 00:05:30.06\00:05:34.53 until 36 weeks, increasing to every week up until delivery, 00:05:34.56\00:05:40.47 now, more visits may be needed in high risk cases. 00:05:40.50\00:05:44.07 For subsequent visits, I like to use the acronym, "CHECKUP". 00:05:44.11\00:05:48.18 Edema, that's checking for swelling. 00:05:53.58\00:05:55.82 Checking the urine. 00:05:55.85\00:05:59.15 Let's start with checking the mother's weight. 00:06:06.80\00:06:08.56 It will be important to monitor the weight gain at each visit. 00:06:08.60\00:06:12.57 Why is this important? 00:06:12.60\00:06:14.00 Generally speaking, a healthy mother, will gain roughly 00:06:14.04\00:06:18.57 around 20 to 32 lbs. during pregnancy. 00:06:18.61\00:06:22.01 This can vary according to pre-pregnancy weight. 00:06:22.04\00:06:25.81 One who begins under weight, should gain more during 00:06:25.85\00:06:28.92 pregnancy than one who begins overweight. 00:06:28.95\00:06:31.09 On the average a mother might gain four pounds or less 00:06:31.12\00:06:35.32 the first trimester, and about one pound per week 00:06:35.36\00:06:38.49 through the remainder of the pregnancy. 00:06:38.53\00:06:40.56 If significant weight gain takes place, 00:06:40.60\00:06:44.07 there is a higher risk of gestational diabetes 00:06:44.10\00:06:47.20 or oversize baby, which could lead to birthing difficulties. 00:06:47.24\00:06:51.57 Weight loss could lead to an underweight baby, 00:06:51.61\00:06:55.64 and then higher risk of infant mortality. 00:06:55.68\00:06:58.48 By monitoring the weight gain, the mother's diet, and lifestyle 00:06:58.51\00:07:03.49 can be adjusted to meet the circumstances. 00:07:03.52\00:07:06.25 Let's see where the normal weight gain comes from. 00:07:06.29\00:07:09.39 So we get the sum of 20- 32 Lbs. 00:07:37.05\00:07:39.72 I once had a client that was very overweight for pregnancy, 00:07:39.75\00:07:43.99 along with my backup doctor, we worked with her diet, 00:07:44.03\00:07:46.93 eliminating junk foods, but imp- lementing good wholesome foods. 00:07:46.96\00:07:52.10 At delivery, she had a net gain of a mere 3 lbs. 00:07:52.13\00:07:56.00 The baby was born a healthy seven and a half pounds. 00:07:56.04\00:07:59.64 Subtract that extra weight for the: 00:07:59.67\00:08:02.54 and you can see, she actually lost some body weight. 00:08:04.85\00:08:07.78 Another client who began her pregnancy slightly underweight, 00:08:08.15\00:08:11.82 was determined to maintain a two meal a day diet 00:08:11.85\00:08:15.86 even though I counseled her to eat a third meal 00:08:15.89\00:08:19.63 during pregnancy. The baby was born in the hospital 00:08:19.66\00:08:23.40 weighing only five pounds. 00:08:23.43\00:08:25.53 Such slight weight gain may mean that quantity or quality 00:08:25.57\00:08:30.24 of the food is insufficient to supply both 00:08:30.27\00:08:33.78 the mother and the baby. 00:08:33.81\00:08:35.34 The next item on the check-up list is her: 00:08:35.38\00:08:38.01 and pulse rate. 00:08:38.05\00:08:39.98 The normal blood pressure is around 120/70, 00:08:40.02\00:08:45.02 high blood pressure is said to be anything above 140/90, 00:08:45.05\00:08:49.46 If high blood pressure continues too long, Preeclampsia, 00:08:49.49\00:08:54.63 also known as Toxemia, can develop. 00:08:54.66\00:08:57.37 a condition that can lead to organ damage in the mother. 00:08:57.40\00:09:00.90 If the Preeclampsia is unchecked, it could lead to 00:09:00.94\00:09:05.47 seizures, and a condition called Eclampsia, 00:09:05.51\00:09:08.68 this can be fatal. 00:09:08.71\00:09:10.98 Some signs of Preeclampsia are protein in the urine, 00:09:11.01\00:09:14.88 headaches, swelling in the face, hands, and feet, 00:09:14.92\00:09:18.92 also referred to as Edema, which is the next item 00:09:18.95\00:09:24.09 in our checkup acronym. 00:09:24.13\00:09:25.69 So how do we help prevent this condition? 00:09:25.73\00:09:29.10 The first thing to consider is the elimination of 00:09:29.13\00:09:32.60 certain things from the diet including pork, caffeine, 00:09:32.63\00:09:37.31 and other stimulants. Fried and fatty foods, 00:09:37.34\00:09:40.71 spicy foods, sugar and sugar substitutes. 00:09:40.74\00:09:45.58 But adding good things like fresh fruits and vegetables, 00:09:45.61\00:09:49.65 proper protein balance, fresh lemon in the place of vinegar, 00:09:49.68\00:09:53.92 garlic, parsley, onions, and cucumbers, proper hydration. 00:09:53.96\00:09:59.39 Now we come to Chemstrip Urinalysis, 00:09:59.43\00:10:02.36 at each visit the urine should be analyzed to detect 00:10:02.40\00:10:05.13 indicators of possible problems. 00:10:05.17\00:10:07.37 For example, the presence of Glucose in the urine can be 00:10:07.40\00:10:11.14 an indicator of Diabetes. 00:10:11.17\00:10:12.77 Protein presence can be an indicator of 00:10:12.81\00:10:16.11 Preeclampsic conditions. 00:10:16.14\00:10:17.91 Blood in the urine may be a sign of anemia. 00:10:17.95\00:10:21.42 Or Leukocytes, which is white blood cells, may be the result 00:10:21.45\00:10:25.52 of fighting off a Urinary Tract Infection. 00:10:25.55\00:10:27.89 So you can see the values in having these Chemstrip Tests 00:10:27.92\00:10:32.16 done. Next on the list is Kicking and Fetal Movement. 00:10:32.19\00:10:35.53 Around seventeen weeks is when you might start noticing 00:10:35.56\00:10:39.33 your baby move, and by twenty weeks, 00:10:39.37\00:10:41.87 it should be quite noticeable. 00:10:41.90\00:10:43.71 As the baby grows, you will begin to pick up his regular 00:10:43.74\00:10:48.18 pattern of movement. 00:10:48.21\00:10:49.64 There can be anywhere between ten to one hundred movements 00:10:49.68\00:10:53.35 within a three hour period. Watch for any marked deviation 00:10:53.38\00:10:58.79 from what your baby is used to doing. 00:10:58.82\00:11:01.66 Now we come to the uterus size. At each prenatal session, 00:11:01.69\00:11:06.93 the doctor or midwife will measure what they call the 00:11:06.96\00:11:10.43 Fundal Height to assure that the baby is growing. 00:11:10.47\00:11:13.57 This is the distance between the top of the pubic bone 00:11:13.60\00:11:17.14 which is called the "Symphysis Pubis" 00:11:17.17\00:11:19.07 to the top of the Uterus, or Fundus. 00:11:19.11\00:11:22.31 The Uterus normally grows 1CM/week, 00:11:22.34\00:11:26.38 meaning that at 28 weeks, the uterus should be around 00:11:26.41\00:11:30.75 28 centimeters, or at 32 weeks, it should measure around 00:11:30.79\00:11:35.39 32 centimeters and so forth. 00:11:35.42\00:11:37.49 If the measurement is significantly greater than 00:11:37.53\00:11:41.93 expected, maybe you have twins. 00:11:41.96\00:11:45.63 Finally we come to Palpation and Fetal Heart Tones. 00:11:45.67\00:11:48.87 Palpating, is simply feeling with your hands 00:11:48.90\00:11:52.37 how the baby is positioned, which comes with practice. 00:11:52.41\00:11:56.34 A common technique used by midwives is called 00:11:56.41\00:11:59.88 Leopold's Maneuver. 00:11:59.91\00:12:01.25 A systematic way of palpating the mother's abdomen. 00:12:01.28\00:12:05.09 The fetal heart tones are also checked confirming the position 00:12:05.12\00:12:08.72 of the baby. A Doppler is commonly used to pick up 00:12:08.76\00:12:12.19 the heart tones of the baby starting at around 00:12:12.23\00:12:14.93 eight to twelve weeks. A Fetal Doppler is simply a hand held 00:12:14.96\00:12:19.30 device that aids in detecting the heart tones and simulating 00:12:19.33\00:12:23.41 them as an audible level so they can be heard by everyone 00:12:23.44\00:12:27.58 in the room. The baby's heart rate should be 00:12:27.61\00:12:31.15 anywhere from 120 to 160 beats per minute. 00:12:31.18\00:12:34.75 In addition to the physical benefits of prenatal care, 00:12:34.78\00:12:38.05 there are also spiritual and emotional benefits. 00:12:38.09\00:12:41.19 A mother may be encouraged from the word of God 00:12:41.22\00:12:44.43 and prayer support. 00:12:44.46\00:12:46.06 I once had a client come to me explaining that she was scared 00:12:46.09\00:12:49.96 because the first birth had been a terrible experience. 00:12:50.00\00:12:52.73 As she went into transition, she found herself completely 00:12:52.77\00:12:57.54 unprepared, not knowing what was going on inside of her. 00:12:57.57\00:13:01.84 Panic set in, all she could think of was escaping the birth 00:13:01.88\00:13:08.38 and the hospital. She climbed out of bed and started 00:13:08.42\00:13:12.15 streaking down the hall, with the doctors and nurses 00:13:12.19\00:13:15.12 chasing her. After that experience, she was terrified 00:13:15.16\00:13:18.53 to have another baby, and mortified to think 00:13:18.56\00:13:21.40 she would have to face that hospital staff again. 00:13:21.43\00:13:23.97 I worked with her, giving her childbirth classes, 00:13:24.00\00:13:27.27 and also encouraging her spiritually, 00:13:27.30\00:13:30.77 I had them cultivate and atmosphere of praise 00:13:30.81\00:13:34.74 by singing and playing scripture songs, 00:13:34.78\00:13:37.68 hymns and peaceful music. 00:13:37.71\00:13:39.51 When delivery time arrived, she was an entirely different 00:13:39.55\00:13:43.28 woman, she had one of the most peaceful births that 00:13:43.32\00:13:46.32 I can remember.
